BACKGROUND Sepsis,as a non-limiting host infection disease,can be accompanied by serious complications such as organ failure,which seriously threatens patient quality of life.AIM To investigate the effect of early ste...BACKGROUND Sepsis,as a non-limiting host infection disease,can be accompanied by serious complications such as organ failure,which seriously threatens patient quality of life.AIM To investigate the effect of early stepwise cardiopulmonary rehabilitation on cardiopulmonary function and quality of life in patients evacuated from mechanical ventilation with sepsis.METHODS A total of 80 patients with sepsis who were hospitalized in our hospital from January 2021 to January 2022 were selected and divided into the observation group(n=40)and the control group(n=40)according to the random number table method.The observation group was treated with early stepwise cardiopulmonary rehabilitation,and the control group was treated with a conventional treatment regimen.Cardiac function indexes(central venous pressure,cardiac troponin I,B-type brain natriuretic peptide),lung function indicators(diaphragmatic mobility,changes in central venous oxygen saturation,oxygenation index),and quality of life(Quality of Life Evaluation Scale)were compared between the two groups after treatment.RESULTS After treatment,the central venous pressure,diaphragm mobility,central venous oxygen saturation,oxygenation index,and Quality of Life Evaluation Scale scores in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The observation group was less than that of the control group for other parameters,and the differences were statistically significant(P<0.05).CONCLUSION Early stepwise cardiopulmonary rehabilitation can effectively enhance cardiac and pulmonary function and improve the quality of life in patients evacuated from mechanical ventilation with sepsis.展开更多
目的探讨在机械通气患者的治疗中采用体外膈肌起搏器联合早期康复训练的应用效果。方法选取2021年1月—2023年4月福建医科大学附属漳州市医院内科监护室收治的119例膈肌功能障碍机械通气患者,根据不同的康复训练方式分为对照组(n=60,早...目的探讨在机械通气患者的治疗中采用体外膈肌起搏器联合早期康复训练的应用效果。方法选取2021年1月—2023年4月福建医科大学附属漳州市医院内科监护室收治的119例膈肌功能障碍机械通气患者,根据不同的康复训练方式分为对照组(n=60,早期康复训练)与研究组(n=59,体外膈肌起搏联合早期康复训练)。对比2组的血气指标、膈肌厚度及膈肌移动度、机械通气时间及住院时间、急性生理与慢性健康评分(acute physiology and chronic health score,APACHEⅡ)、肺部感染评分、并发症发生率。结果研究组动脉血二氧化碳分压(partial pressure of carbon dioxide in arterial blood,PaCO_(2))水平较低,血氧饱和度(oxygen saturation of blood,SaO_(2))、氧合指数(partial pressure of oxygen/fraction of inspiration oxygen,PaO_(2)/FiO_(2))水平高于对照组(P<0.05)。研究组吸气末与呼气末膈肌厚度、膈肌移动度高于对照组(P<0.05)。研究组APACHEⅡ、临床肺部感染评分(clinical pulmonary infection score,CPIS)低于对照组(P<0.05)。研究组并发症发生率为3.38%,低于对照组的16.67%(P<0.05);研究组机械通气时间(10.36±1.15)d及住院时间(19.16±1.53)d短于对照组[(13.58±1.43)d,(21.25±2.02)d](P<0.05)。结论在机械通气患者的治疗中采取体外膈肌起搏联合早期康复训练可改善其血气指标与膈肌功能,减少并发症的发生,促进患者病情好转,提高其健康水平。展开更多

Users can interact...Virtual reality is a new technology that simulates a three-dimensional virtual world on a com- puter and enables the generation of visual, audio, and haptic feedback for the full immersion of users. Users can interact with and observe objects in three-dimensional visual space without limitation. At present, virtual reality training has been widely used in rehabilitation therapy for balance dysfunction. This paper summarizes related articles and other articles suggesting that virtual reality training can improve balance dysfunction in patients after neurological diseases. When patients perform virtual reality training, the prefrontal, parietal cortical areas and other motor cortical networks are activated. These activations may be involved in the reconstruction of neurons in the cerebral cortex. Growing evidence from clinical studies reveals that virtual reality training improves the neurological function of patients with spinal cord injury, cerebral palsy and other neurological impairments. These findings suggest that virtual reality training can acti- vate the cerebral cortex and improve the spatial orientation capacity of patients, thus facilitating the cortex to control balance and increase motion function.展开更多
目的了解脑卒中后语言和非语言认知功能相关研究的趋势。方法检索建库至2022年8月Web of Science核心合集脑卒中后语言与非语言认知主题相关的文献,对年度、作者、期刊、机构/国家发文量分布情况进行文献计量分析。采用VOSviewer软件对...目的了解脑卒中后语言和非语言认知功能相关研究的趋势。方法检索建库至2022年8月Web of Science核心合集脑卒中后语言与非语言认知主题相关的文献,对年度、作者、期刊、机构/国家发文量分布情况进行文献计量分析。采用VOSviewer软件对作者合作关系、作者共被引、期刊共被引、国家/机构合作关系及动态演变和关键词热点、趋势进行可视化分析。结果共纳入1630篇。2004年检索到第1篇,之后发文量总体呈逐年增加的趋势,2019年之后有所下降。高产、高被引、高中心度的作者为Matthew A.Lambon Ralph和Elizabeth Jefferies。高产、高被引的期刊是Neuropsychologia。高产、高中心度的国家/机构分别为英国曼彻斯特大学、美国约翰斯霍普金斯大学,中国跻身高产前5名,但中心度不足。关键词动态演变显示研究最开始从基于神经影像的损伤机制研究,到关注记忆障碍、注意力、抑郁等功能障碍在语言损伤和康复中的作用,随后广泛开展如非侵入性脑刺激、电子远程康复设备等疗效及机制探索,机制的研究从关注额、颞叶转为关注脑网络。结论近年来脑卒中后语言与非语言认知相关研究取得了一定成果,尤其是基于神经影像的共性损伤机制研究,以及协同治疗的干预疗效研究等。未来不同区域之间、不同作者团队之间的学术合作有待加强,亟待开展从非语言认知角度完善认知心理模型,探索新的治疗技术,以提高脑卒中后失语临床疗效。展开更多
