This paper applies the methods of questionnaires,field testing and statistical analysis to compare and analyze two different square cases of Qingyunpu in Liaoning Province,and conclude the main factors which affect the physical environmental comfort. The improvement strategies for physical environment of rural square are put forward from the aspects of site selection,road position,orientation,landscape design,and ground pavement material selection,aiming to provide the design basis for the physical environment of square in severe cold regions' rural areas. 展开更多

Continuous regional arterial infusion for the treatment of severe acute pancreatitis: a meta-analysis 被引量:17

BACKGROUND: Continuous regional arterial infusion(CRAI) is a drug delivery system, which dramatically increases the drug concentration in the pancreas. Previous clinical and basic studies have demonstrated the possible therapeutic efficacy of CRAI for severe acute pancreatitis(SAP). This meta-analysis of all published randomized controlled trials(RCTs) was conducted to assess the efficacy and safety of CRAI for the treatment of SAP. DATA SOURCES: Up to August 10, 2014, RCTs comparing CRAI with intravenous infusion for SAP in PubM ed, Embase, EBSCO, MEDLINE, Science Citation Index Expanded, Cochrane Library, China Academic Journals Full-Text Database, Chinese Biomedical Literature Database, and Chinese Scientific Journals Database were selected by two independent reviewers. The relative risk(RR) and their 95% confidence intervals(CI) for duration of elevated serum amylase and urine amylase, duration of abdominal pain, infection rate, incidence of complication, overall mortality, curative rate, hospital stay and details of subgroup analysis were extracted. Meta-analyses were made using the software Review Manager(RevM an version 5.10).RESULTS: Six RCTs with 390 patients meeting the inclusion criteria were included in the final analysis. Compared with intravenous infusion route, CRAI significantly shortened the duration of elevated urine amylase(MD=-2.40, 95% CI=-3.20,-1.60; P〈0.00001) and the duration of abdominal pain(MD=-1.46, 95% CI=-1.94,-0.98; P〈0.00001), decreased the incidence of complication(RR=0.35, 95% CI=0.15, 0.81; P=0.01) and overall mortality(RR=0.25, 95% CI=0.08, 0.78;P=0.02), shortened the duration of hospital stay(MD=-10.36, 95% CI=-17.05,-3.68; P=0.002), and increased the curative rate(RR=1.66, 95% CI=1.13, 2.46; P=0.01). No mortality and catheter-related infections due to CRAI administration was reported in these studies. Subgroup analysis showed that the combination of drug administration via CRAI did not significantly improve the outcomes.CONCLUSION: CRAI is effective for the treatment of SAP, and the combination of drug administration via CRAI did not have a significant effect on the improvement of the outcomes. 展开更多

Characteristics of Severely Anemic Pregnant Women and Perinatal Outcomes in Banfora Regional Hospital, Burkina Faso: An Epidemiological Study

Severe anemia during pregnancy remains an acute maternal health problem in low income countries and its management is a challenge. This study aimed to analyze the characteristics of hospitalized pregnant women with severe anemia and issue of the management in a regional hospital of a low income country. Material and Methods: This was a retrospective descriptive study from January 1, 2007 to December 31, 2008, at a regional hospital maternity. Data of the 283 pregnancy severe anemia cases have been analyzed using usual Pearson’s Chi square test or Fisher’s exact test. Results: Of the 283 women with pregnancy severe anemia, 98% were illiterate, 98% were household women;majority were in the third trimester (64%);41% had no previous delivery;49% did not had antenatal care. There were no statistics association between hemoglobin values stage and gestational age, gravidity and parity. During the hospitalization, from the 283 women with pregnancy severe anemia, deliveries occurred on 22.6%;abortion on 1%. From the 64 deliveries, 45.3% were alive preterm birth, 20.3% preterm stillbirth, 25.0% at term birth and 9.4% at term stillbirth. Between the 45 alive babies, 91% were low birth weight. In global, from the 283 women, mortality rate was 2.8%. Conclusion:?Severe anemia during pregnancy results in maternal mortality, preterm, low birth and stillbirth even between hospitalized women. As most of the pregnancy anemia risk factors are chronic poverty related factors, intervention must be focused on prevention including health pregnant women iron supplementation and adequate nutritional diet promoting at both health facilities and community level. 展开更多

Historically,frequent and heavy snow disaster(SD)has caused serious livestock death and casualties,resulting in a devastating impact on animal husbandry development in the Three Rivers Source Region(TRSR).From winter in 2018 to spring in 2019,the largest SD occurred in this area over the past 10 years,especially in core zones of the Lancang River Source Region.Field research results show that the main causes of the major SD include weak infrastructure(i.e.,roads,communications,warm sheds,and insufficient forage reserve),low rate of domestic animals for sale before the SD,and low loss settlement rate.SD occurrence could furtherly reduce the ability of disaster prevention,mitigation and relief of disaster loss.In the future,heavily affected SD areas should improve the forecasting ability of snowfall incidents,strengthen infrastructure construction,implement grass and livestock balance strategies,optimize livestock structure,improve loss settlement rate,and develop a modern compound model of animal husbandry development model that combines breeding,slaughtering and deep processing of animal product. 展开更多

Through this model, resource status of the regional environment in Mianzhu is analyzed, as well as the ecological capacity and the ecological security. 针对高海拔严寒地区隧道冻害问题,结合隧道冻害的相关研究,从冻害类型及原因分析、冻害机制、温度场、处置措施等4个方面的现状进行阐述和分析,对当前研究存在的不足和未来的发展方向进行探讨。主要成果为:1)通过对高海拔严寒地区隧道... 针对高海拔严寒地区隧道冻害问题,结合隧道冻害的相关研究,从冻害类型及原因分析、冻害机制、温度场、处置措施等4个方面的现状进行阐述和分析,对当前研究存在的不足和未来的发展方向进行探讨。主要成果为:1)通过对高海拔严寒地区隧道工程实例进行分析,总结4种冻害类型及4种冻害原因;2)基于含水风化层冻胀理论、整体性围岩冻胀理论、衬砌背后积水冻胀理论、裂隙水冻胀理论等4种冻胀理论,建立多年冻土区以整体性围岩冻胀理论为主、季冻区以裂隙水冻胀理论为主的多理论联合分析的方法;3)温度场研究方法分为现场监测、理论解析、模型试验、数值模拟等4种,其中现场监测是对洞内外温度进行监测,而理论分析、模型试验、数值模拟则是对监测结果进行验证分析;4)冻害处置措施有防排水措施、保温防寒措施、抗冻措施等3种,防冻的关键是将三者相结合进行综合治理;5)指出冻害机制适用性、温度场研究方法现状、处置措施适用性等方面的不足,未来应在高海拔严寒地区建立多种冻胀理论、多种研究方法综合应用的研究体系,并在隧道防排水、保温防寒、抗冻等方面给出处置措施。 展开更多

我国严寒地区气候寒冷且供暖季漫长,农村住宅采暖能耗问题突出,多数住宅采用自建形式,围护结构的热工性能普遍不理想。本研究从严寒地区农村住宅围护结构实际情况出发,对本溪农村地区200余户现有住宅进行调研。结果显示,本溪现有农宅基... 我国严寒地区气候寒冷且供暖季漫长,农村住宅采暖能耗问题突出,多数住宅采用自建形式,围护结构的热工性能普遍不理想。本研究从严寒地区农村住宅围护结构实际情况出发,对本溪农村地区200余户现有住宅进行调研。结果显示,本溪现有农宅基本未设置外墙保温,虽部分住宅屋顶设置保温,但保温材料均已超过使用年限,保温效果较差。有86.95%的农宅室内温度基本维持在18℃以下,农户满意度仅24.2%。实地测量3户典型农宅的室内环境温度。采用EnergyPlus模拟得到农宅围护结构的传热系数相比居住建筑节能设计标准规定限值超出300%~400%。根据实测数据和模拟数据对外墙、屋顶、外窗进行热工性能改造。对比单一围护结构改造前后的供暖季热负荷,得出外墙的改造效果最好。 展开更多

碳中和导向下寒地近零能耗建筑形态多目标优化研究 被引量:2

双碳目标的大背景下,严寒地区因特殊的气候条件,碳排放问题较为突出,且寒地近零能耗建筑热工性能优化空间有限。为研究严寒地区近零能耗建筑进一步降低碳排放的可能,以及建筑参数与碳排放等性能之间的关系,以中德节能中心为例,依托Rhino... 双碳目标的大背景下,严寒地区因特殊的气候条件,碳排放问题较为突出,且寒地近零能耗建筑热工性能优化空间有限。为研究严寒地区近零能耗建筑进一步降低碳排放的可能,以及建筑参数与碳排放等性能之间的关系,以中德节能中心为例,依托Rhino+Grasshopper平台,整体考虑建筑的产能端和用能端,按建筑参数层级不同,基于RBFOpt算法分形态、立面两个阶段进行多目标优化,并利用Simlab对建筑参数进行全局敏感性分析。结果表明:在近零能耗建筑围护结构热工性能已达限值的前提下,经过多目标优化,最优方案单位面积碳排放下降了4.75%,产能消费比提高了8.2%。证明多阶段多目标优化方法对于提升严寒地区近零能耗建筑性能、降低碳排放是有效的。另外,对于建筑碳排放敏感程度较高的两个参数是面宽进深比和天窗面积比。产能消费端协同的多阶段多目标优化方法可以为严寒地区近零能耗建筑设计与优化提供一定的借鉴参考。 展开更多

严寒地区装配式钢结构ALC板围护体系试验房热工性能研究

针对一栋位于严寒地区的主体结构采用装配式钢管混凝土框架结构、墙面采用蒸压加气混凝土板(ALC)自承重墙结合外保温围护体系的单层居住功能试验房,在冬季采暖期采用原位测量非破坏性方法进行周期为119 d的连续测试,完成围护结构热工缺... 针对一栋位于严寒地区的主体结构采用装配式钢管混凝土框架结构、墙面采用蒸压加气混凝土板(ALC)自承重墙结合外保温围护体系的单层居住功能试验房,在冬季采暖期采用原位测量非破坏性方法进行周期为119 d的连续测试,完成围护结构热工缺陷检测、室内湿热环境测试、围护结构热工性能测试。结果表明:ALC墙板竖向拼接板缝、钢管混凝土柱墙角、墙脚、窗体均存在热桥。试验房构造做法的墙体传热系数平均值为0.239 W/(m^(2)·K)。试验房蓄热性能良好,停暖后室内气温缓慢降低,历时7 d达到最低温度,基本保持恒定。恢复供暖后,室内气温快速回升。测试期内试验房平均气温为22.4℃,平均相对湿度为53.8%,舒适性良好。为严寒地区推广应用装配式钢结构和选择保温体系提供了实测数据支持。 展开更多

为提升预制保温模板对坝体的防护能力,增强严寒区坝面保温材料耐久性,开展了模板混凝土配合比设计与性能研究。从提高模板混凝土抗裂性和耐久性角度考虑,提出了原材料质量控制技术指标;通过优化设计,提出了满足C50W12F400设计要求的模... 为提升预制保温模板对坝体的防护能力,增强严寒区坝面保温材料耐久性,开展了模板混凝土配合比设计与性能研究。从提高模板混凝土抗裂性和耐久性角度考虑,提出了原材料质量控制技术指标;通过优化设计,提出了满足C50W12F400设计要求的模板混凝土配合比;研究了模板混凝土的抗压强度、抗拉强度、极限拉伸值、弹性模量、抗冻、抗渗等性能发展规律。结果表明试验制备的模板混凝土不仅工作性良好,抗压强度、耐久性优于设计指标,且具有高极限拉伸值、低弹性模量的优点;研究成果为严寒区预制保温模板的生产和应用提供了有益的参考和借鉴。 展开更多

随着有砟轨道高速铁路的相继开通运营,出现了一定程度的冻害问题,给高速铁路的养护维修工作带来了更大的挑战。本文结合敦白(敦化—白河)高速铁路所处区域气候环境、设计标准,针对其开通以来发生的路基冻胀问题,通过对动态、静态检测数... 随着有砟轨道高速铁路的相继开通运营,出现了一定程度的冻害问题,给高速铁路的养护维修工作带来了更大的挑战。本文结合敦白(敦化—白河)高速铁路所处区域气候环境、设计标准,针对其开通以来发生的路基冻胀问题,通过对动态、静态检测数据统计分析,总结路基冻胀规律,初步研究冻胀成因,提出防治对策。结果表明:敦白高速铁路全线路基冬季存在不同程度的冻胀问题,冻融循环期长达5个月,初步分析路基发生冻害的主要原因为线路所处地理位置气温低、入冬前降雨多导致基床含水率高、基床表层级配碎石水泥掺量低,导致表层冻害数量较多。建议采取强化疏排水、不良填料改性、填料强化置换等措施在夏季、秋季进行综合整治,以减少或消灭冬季冻害,降低冬季现场作业频次。 展开更多
