BACKGROUND Gastroparesis is a common digestive disorder characterized by delayed gastric emptying,which can lead to symptoms such as nausea,vomiting,abdominal pain,and poor appetite.Traditional Chinese medicine(TCM)has been used for centuries to treat gastrointestinal disorders,including gastroparesis.TCM theory suggests that spleen and stomach qi deficiency syndrome is one of the main pathogenic factors in gastroparesis.Nursing care plays an important role in the treatment of gastroparesis,and TCM nursing interventions have shown promising results in improving patient outcomes.However,there is limited research on the clinical effectiveness of TCM nursing interventions for gastroparesis with spleen stomach deficiency syndrome.This study aimed to evaluate the clinical effect of TCM nursing intervention in the treatment of gastroparesis with spleen stomach deficiency syndrome and to compare it with routine nursing interventions.AIM To analyze the clinical effect of traditional Chinese medicine nursing intervention in the treatment of gastric paraplegia with spleen stomach deficiency syndrome.METHODS From January 2020 to July 2021,80 patients with gastroparesis of spleen stomach qi deficiency type diagnosed in our hospital were selected for the study.The 80 patients were randomly divided into a control group and an experimental group,with 40 cases in each group.During the treatment period,the control group received routine nursing interventions,while the experimental group received traditional Chinese medicine nursing procedures.Compare the nursing effects of the two groups and observe the changes in traditional Chinese medicine symptom scores,pain levels,and sleep quality before and after treatment.RESULTS After treatment,comparing the treatment effects of the two groups,the total effective rate of the experimental group was significantly higher than that of the control group,with statistical significance(P<0.05).There was no statistically significant difference in the TCM symptom score,visual analogue scale(VAS)score,and Pittsburgh sleep quality index(PSQI)score between the two groups before treatment(P>0.05).However,after treatment,the TCM syndrome scores,VAS scores,and PSQI scores of the experimental group were significantly lower than those of the control group,with a statistically significant difference(P<0.05).CONCLUSION In the clinical nursing intervention of patients with mild gastroparesis due to spleen and stomach qi deficiency,the traditional Chinese medicine nursing plan has good clinical application value and nursing effect,and has a good effect on improving patients’pain and sleep quality. 展开更多

BIO-CHARACTERISTICS OF INTESTINAL METAPLASIA IN THE STOMACH: HYPERPROLIFERATIVE AND USUAL TYPE

Background: In Padova and Vienna International Classification, the usual intestinal metaplasia (UIM) of the stomach, including complete and incomplete type, is defined as negative for dysplasia, and hyperproliferative intestinal metaplasia (HIM) as indefinite for dysplasia, but the biological characteristics of these two types of intestinal metaplasia (IM)remain to be studied. Objective: To investigate the biological differences between UIM, HIM and intestinal type gastric cancer (IGC), a panel of biomarkers were detected. Methods: A total of 38 cases of IGC, 41 HIM and 56 UIM adjacent to gastric cancer were studied. Immunohistochemistry was used to detect the expressions of pS2, MUC2, MUC5AC, MUC6, Ki-67, EGFR, p53 and sulfo-Lewisa in UIM, HIM and IGC. Microsatellite instability (MSI) in UIM, HIM and IGC was detected by using Denaturing High Performance Liquid Chromatography (DHPLC). Results: The pS2 antigen expression in UIM (78.6%) was significantly higher than in HIM and IGC (9.8%, 10.5%), p〈0.01. The MUC6, sulfo-Lewisa and EGFR protein expressions were significant increased in HIM (24.4%, 82.9%, 48.7%) and IGC (34.2%, 75.0%, 42.1%) than in UIM (3.6%, 25.5%, 17.9%), p〈0.01. A reversed pattern of expressions of MUC2 and MUC5AC was observed in UIM (96.4%, 50.0%) and HIM (82.9%, 36.6%) compared with IGC (52.6%, 13.2%), p〈0.05; and the p53 gene expression was increased from UIM (1.8%) to HIM (19.5%) to IGC (57.9%), p〈0.01. The Ki-67 labeling index was significantiy different among three lesions (UIM: 16%±6%, HIM: 45%±9%, IGC: 63%±10%, p〈0.01). Conclusion: These findings suggest that there are different bio-characteristics among UIM, HIM and IGC, and HIM may have higher potential to progress to more advanced lesions in comparison with UIM. 展开更多

Various biological indices were used to observe the effects of Tween-80 in combination with hyperthermia at different temperature (39-43℃) for different period of time (20-100 minutes) on human stomach cell line BGC-823. The results showed that Tween-80obviously reduced the activation energy of BGC-823 cells. Synergistic effect was observed if applied with heat at 39℃ with the increase in temperature and time,the inhibitory effect on the cancer cells was gradually intensified. The lethal rate of BGC-823 cells treated by heat at 41℃ in combination with Tween-80 was around 5.2 times as that treated by hyperthermia alone. The synergistic effect of heat at 41℃ for 100 min. in combination with Tween-80 was equivalent to the effect of 43℃ for 100 min.In other words, the critical temperature for BGC-823 cells was hereby reduced about 2℃. The measurement of membrane mobility, SDH activity etc.also showed that at 41℃ the synergistic effect of hyperthermia in combination with Tween-80 was the best, it exceeding the single effect of these factors in combination and showing effect of multiplication. The synergistic effect of heat at 41℃ in combination with Tween-80 was higher than that of heat at 41℃ in combination with MMC. It also demonstrated that the specific and sustained action on the inhibition of cancer cells did exist. These studies suggested that the synergistic mechanism of Tween-80 and hyperthermia probably was that both of them acted on the cell membrane system. BACKGROUND Computed tomography(CT)technology has been gradually used in the differen-tiation of small mesenchymal tumors of the stomach and intestines from smooth muscle tumours.AIM To explore the value of enhanced CT in the differentiation of small mesenchymal tumors of the stomach and intestines from smooth muscle tumours.METHODS Clinical data of patients with gastric mesenchymal or gastric smooth muscle tu-mours who were treated in our hospital from May 2018 to April 2023 were retrospectively analysed.Patients were divided into the gastric mesenchymal tumor group and the gastric smooth muscle tumor group respectively(n=50 cases per group).Clinical data of 50 healthy volunteers who received physical examinations in our hospital during the same period were selected and included in the control group.Serum levels of carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),carbohydrate antigen 19-9(CA19-9),CA-125 and cytokeratin 19 fragment antigen 21-1 were compared among the three groups.The value of CEA and CA19-9 in the identification of gastric mesenchymal tumours was analysed using the receiver operating characteristic(ROC)curve.The Kappa statistic was used to analyse the consistency of the combined CEA and CA19-9 test in identi-fying gastric mesenchymal tumours.RESULTS CEA levels varied among the three groups in the following order:The gastric mesenchymal tumour group>the control group>the gastric smooth muscle tumour group.CA19-9 levels varied among the three groups in the following order:The gastric mesenchymal group>the gastric smooth muscle group>the control group,the difference was statistically significant(P<0.05).ROC analysis showed that the area under the curve of CEA and CA19-9 was 0.879 and 0.782,respectively.CONCLUSION Enhanced CT has shown value in differentiating small mesenchymal tumors of the stomach and intestines from smooth muscle tumors. 展开更多

BACKGROUND The results of previous meta-analyses evaluating the association between the alcohol intake and gastric cancer risk have reported that a statistical significance only for men.AIM To investigate the different association between alcohol intake and gastric cancer risk between men and women.METHODS The selection criteria included a prospective cohort study for evaluating alcohol intake and gastric cancer risk,with relative risks adjusted for potential confounders.Adjusted relative risk(RR)for the potential confounders and its 95%confidence interval(CI)in the highest vs lowest level were extracted from each study and a random-effects meta-analysis was conducted.Subgroup analyses by region,level of adjustment for smoking status,adjusting for body mass index,and year of publication were conducted.RESULTS A meta-analysis of all 27 cohorts showed that alcohol intake increased the risk of gastric cancer(summary RR=1.13,95%CI:1.04-1.23,I2=58.2%).Further,13 men’s cohorts had higher summary RR while maintaining statistical significance,and only seven women’s cohorts had no statistical significance.CONCLUSION The present review suggests that alcohol consumption increases the risk of gastric cancer in men.These findings showed that the sex variable in the association between alcohol intake and gastric cancer risk seemed to be an effect modifier with an interaction term.It is necessary to re-estimate follow-up outcomes after stratifying for sex. 展开更多

The establishment of drug-free feeding systems has been required for secure and healthy livestock production. Although functional feed materials containing microorganisms as alternatives to enhance intestinal immunity are expected to be beneficial for reducing diarrhoea caused by pathogens in weaned piglets, the effects of such materials on porcine intestinal cells have not been investigated in detail. Therefore, this work evaluated the immunoregulatory functions of microbial feed materials in porcine intestinal immune and epithelial cells. Porcine immune cells isolated from Peyer’s patches and mesenteric lymph nodes were stimulated with six different feed materials containing microorganisms, and evaluated for lymphocyte mitogenicity and cytokine inductions. In addition, porcine intestinal epithelial cells were stimulated with the materials before treatment with heat-killed enterotoxigenic Escherichia coli (ETEC), and analyzed for the proinflammatory cytokine expressions. The material containing Bifidobacterium thermophilum significantly augmented lymphocytes’ mitogenicity and also induced a high expression of IL-2, IL-6 and IFN-γ in immune cells, and inhibited ETEC-induced overexpression of IL-6 and IL-8 via regulation of Toll-like receptor signaling. These results suggest that this feed material stimulates intestinal epithelial and immune cells to exert immunoregulation, suggesting that this feed is expected to contribute to promoting the health of piglets without using antimicrobial feed materials. 展开更多

To evaluate the anti-obesity effect of Xiaopang-mei,an experimental study by single method in threegroups of rats was done.The first group were fedwith Xiaopangmei-01,the second group with aplacebo,and the third group were controls.Ascompared with the control and the placebo groups,the rata that had been fed Xiaopangmei for six weeksshowed body weight,food intake,weight of epididy-mis fat pad and the intestinal absorption of glucoseall significantly decreased.The swimming tolerancetest showed they swam longer than the other twogroups.No significant difference was found betweenthe three groups in the growth and developmentindices such as body length,length of femur and tibia,weight of heart,lungs,spleen,kidneys,stomach,smallintestines,gastrocnemius muscle,and adrenal glands.No side effects were found on the liver and kidney.The secretion of insulin was inhibited after the swim-ming tolerance test and the gestrocnemius muscletest was similar for all three groups of rats.Weconcluded that Xiaopangmei is an effective anti-obesity drug. 展开更多

Blood was collected to examine serum NO and thyroid hormone concentration. Intestinal mucosa iNOS activity was assayed using immunochemical stain. Results Mortality rate in the prevention group was significantly lower than the septic group (Log rank = 3. 85, P 【 0.05). Serum NO concentration was significantly lower in the prevention group (F=19.6,F【0.01). The degree of inflammatory injury of intestinal mucosa was much milder in the prevention group than in the septic group (x2 = 5.303,P【0. 05). AIM: To describe the histology of the digestive tract and to investigate the occurrence of endocrine cells in Oligosarcus hepsetus (O. hepsetus ). METHODS: The digestive tract (DT) of O. hepsetus was divided into esophagus, two stomach regions (glandular and non-glandular) and two intestinal regions (anterior and posterior). These specimens were processed by routine histological techniques and stained with hematoxylin-eosin, Gomori's trichrome, periodic acid Schiff (PAS) and Alcian blue (AB). An immunohistochemical method using avidin-biotin-peroxidase was employed.RESULTS: The esophagus is lined with a non-keratinized stratified squamous epithelium that is reactive to PAS and AB. The stomach has a mucosa lined with a simple columnar epithelium with mucus-secreting cells that are reactive only to PAS. The intestine has a simple columnar epithelium with a brush border and goblet cells that are reactive to PAS and AB. Somatostatin, serotonin and cholecystokinin immunoreactive cells were identified throughout the DT.CONCLUSION: This study revealed adaptations for the species' diet and showed that the distribution and relative frequency of immunoreactive cells are similar to those of other fish. 展开更多

The integrity of the gastrointestinal mucosa plays a crucial role in gut homeostasis,which depends upon the balance between mucosal injury by destructive factors and healing via protective factors.The persistence of noxious agents such as acid,pepsin,nonsteroidal anti-inflammatory drugs,or Helicobacter pylori breaks down the mucosal barrier and injury occurs.Depending upon the size and site of the wound,it is healed by complex and overlapping processes involving membrane resealing,cell spreading,purse-string contraction,restitution,differentiation,angiogenesis,and vasculogenesis,each modulated by extracellular regulators.Unfortunately,the gut does not always heal,leading to such pathology as peptic ulcers or inflammatory bowel disease.Currently available therapeutics such as proton pump inhibitors,histamine-2 receptor antagonists,sucralfate,5-aminosalicylate,antibiotics,corticosteroids,and immunosuppressants all attempt to minimize or reduce injury to the gastrointestinal tract.More recent studies have focused on improving mucosal defense or directly promoting mucosal repair.Many investigations have sought to enhance mucosal defense by stimulating mucus secretion,mucosal blood flow,or tight junction function.Conversely,new attempts to directly promote mucosal repair target proteins that modulate cytoskeleton dynamics such as tubulin,talin,Ehm^(2),filamin-a,gelsolin,and flightless I or that proteins regulate focal adhesions dynamics such as focal adhesion kinase.This article summarizes the pathobiology of gastrointestinal mucosal healing and reviews potential new therapeutic targets. 展开更多

However, the mecha... BACKGROUND The exact mechanism of proton pump inhibitors(PPIs)-induced hypomagnesemia(PPIH) is largely unknown. Previous studies proposed that PPIH is a consequence of intestinal Mg2+ malabsorption. However, the mechanism of PPIs-suppressed intestinal Mg2+ absorption is under debate.AIM To investigate the effect of 12-wk and 24-wk omeprazole injection on the total,transcellular, and paracellular Mg2+ absorption in the duodenum, jejunum, ileum,and colon of male Sprague-Dawley rats.METHODS The rats received 20 mg/kg·d subcutaneous omeprazole injection for 12 or 24 wk.Plasma and urinary Mg2+, Ca2+, and PO43-levels were measured. The plasma concentrations of 1α,25-dihydroxyvitamin D3(1α,25(OH)2D3), parathyroid hormone(PTH), fibroblast growth factor 23(FGF-23), epidermal growth factor(EGF), and insulin were also observed. The duodenum, jejunum, ileum, and colon of each rat were mounted onto individual modified Using chamber setups to study the rates of total, transcellular, and paracellular Mg2+ absorption simultaneously. The expression of transient receptor potential melastatin 6(TRPM6) and cyclin M4(CNNM4) in the entire intestinal tract was also measured.RESULTS Single-dose omeprazole injection significantly increased the intraluminal p H of the stomach, duodenum, and jejunum. Omeprazole injection for 12 and 24 wk induced hypomagnesemia with reduced urinary Mg2+ excretion. The plasma Ca2+ was normal but the urinary Ca2+ excretion was reduced in rats with PPIH. The plasma and urinary PO43-levels increased in PPIH rats. The levels of1α,25(OH)2D3 and FGF-23 increased, whereas that of plasma EGF decreased in the omeprazole-treated rats. The rates of the total, transcellular, and paracellular Mg2+ absorption was significantly lower in the duodenum, jejunum, ileum, and colon of the rats with PPIH than in those of the control rats. The percent suppression of Mg2+ absorption in the duodenum, jejunum, ileum, and colon of the rats with PPIH compared with the control rats was 81.86%, 70.59%, 69.45%,and 39.25%, respectively. Compared with the control rats, the rats with PPIH had significantly higher TRPM6 and CNNM4 expression levels throughout the intestinal tract.CONCLUSION Intestinal Mg2+ malabsorption was observed throughout the intestinal tract of rats with PPIH. PPIs mainly suppressed small intestinal Mg2+ absorption. Omeprazole exerted no effect on the intraluminal acidic pH in the colon. Thus, the lowest percent suppression of total Mg2+ absorption was found in the colon. The expression levels of TRPM6 and CNNM4 increased, indicating the presence of a compensatory response to Mg2+ malabsorption in rats with PPIH. Therefore, the small intestine is an appropriate segment that should be modulated to counteract PPIH. 展开更多

BACKGROUND Intestinal inflammation is a common digestive tract disease, which is usually treated with hormone medicines. Hormone medicines are effective to some extent, but long-term use of them may bring about many complications.AIM To explore the protective effects of panax notoginseng saponin(PNS) against dextran sulfate sodium(DSS)-induced intestinal inflammatory injury through phosphoinositide-3-kinase protein kinase B(PI3K/AKT) signaling pathway inhibition in rats.METHODS Colitis rat models were generated via DSS induction, and rats were divided into control(no modeling), DSS, DSS + PNS 50 mg/k, and DSS + PNS 100 mg/kg groups. Then, the intestinal injury, oxidative stress parameters, inflammatory indices, tight junction proteins, apoptosis, macrophage polarization, and TLR4/AKT signaling pathway in colon tissues from rats in each of the groups were detected. The PI3 K/AKT signaling pathway in the colon tissue of rats was blocked using the PI3K/AKT signaling pathway inhibitor, LY294002.RESULTS Compared with rats in the control group, rats in the DSS group showed significantly shortened colon lengths, and significantly increased disease activity indices, oxidative stress reactions and inflammatory indices, as well as significantly decreased expression of tight junction-associated proteins. In addition, the DSS group showed significantly increased apoptotic cell numbers,and showed significantly increased M1 macrophages in spleen and colon tissues.They also showed significantly decreased M2 macrophages in colon tissues, as well as activation of the PI3K/AKT signaling pathway(all P < 0.05). Compared with rats in the DSS group, rats in the DSS + PNS group showed significantly lengthened colon lengths, decreased disease activity indices, and significantly alleviated oxidative stress reactions and inflammatory responses. In addition, this group showed significantly increased expression of tight junction-associated proteins, significantly decreased apoptotic cell numbers, and significantly decreased M1 macrophages in spleen and colon tissues. This group further showed significantly increased M2 macrophages in colon tissues, and significantly suppressed activation of the PI3K/AKT signaling pathway, as well as a dose dependency(all P < 0.05). When the PI3K/AKT signaling pathway was inhibited, the apoptosis rate of colon tissue cells in the DSS + LY294002 group was significantly lower than that of the DSS group(P < 0.05).CONCLUSION PNS can protect rats against DSS-induced intestinal inflammatory injury by inhibiting the PI3K/AKT signaling pathway, and therefore may be potentially used in the future as a drug for colitis. 展开更多
