Objective:We evaluated the efficacy and safety of transdermal preparations of Sinomenium acutum(SA)for rheumatoid arthritis(RA).Methods:Randomized controlled trials(RCTs)of SA transdermal preparations for RA were extr...Objective:We evaluated the efficacy and safety of transdermal preparations of Sinomenium acutum(SA)for rheumatoid arthritis(RA).Methods:Randomized controlled trials(RCTs)of SA transdermal preparations for RA were extracted from relevant databases and screened in accordance with the inclusion criteria.The Cochrane System Evaluation Manual(version 5.1.0)was used to assess the quality of the included trials.We used the Cochrane Review Manager(version 5.4)to conduct the meta-analysis.Results:Six trials comprising 436 patients(220 patients in the treatment group and 216 patients in the control group)were analyzed.The meta-analysis indicated that SA transdermal preparations in combination with disease-modifying antirheumatic drugs(DMARDs)enhanced the overall effect(odds ratio[OR]3.97,95%confidence interval[CI][2.25,7.00],P<0.00001),decreased visual analogue scale(VAS)results(mean difference[MD]-0.64,95%CI[-1.20,-0.09],P=0.02),decreased laboratory indexes including the erythrocyte sedimentation rate(ESR)(MD-4.36,95%CI[-5.63,-3.08],P<0.00001)and C-reactive protein(CRP)(MD-3.6,95%CI[-3.99,-3.21,P<0.00001]),and decreased the Disease Activity Score-28(DAS28)(MD-0.41,95%CI[-0.78,-0.03],P=0.03).The results suggest that combination therapy did not shorten the duration of morning stiffness(DMS;standardized MD[SMD]-6.13,95%CI[-17.33,5.06],P=0.28)or reduce rheumatoid factor(RF)laboratory indexes(SMD-0.85;95%CI[-2.19,0.49],P=0.21).Only one study reported adverse reactions,and thus,it was difficult to determine whether adverse drug reactions in the combination therapy group were significantly different from those in the control group.Conclusion:We found that SA transdermal preparations combined with DMARDs may have greater clinical efficacy than DMARDs for RA.More well-designed and high-quality RCTs are required to verify the findings and determine whether transder-mal preparations cause fewer adverse events.展开更多
Objective:The population density and diversity of Sinomenium acutum(Menispermaceae)have been greatly reduced recently by overharvesting for medicinal purposes in China.Therefore,it is urgent that the remaining populat...Objective:The population density and diversity of Sinomenium acutum(Menispermaceae)have been greatly reduced recently by overharvesting for medicinal purposes in China.Therefore,it is urgent that the remaining populations are investigated,and that strategies for the utilization and conservation of this species are developed.This study aimed to find the possible glacial refugia and define the genetic diversity of S.acutum for its proper utilization and conservation.Methods:A total of 77 S.acutum samples were collected from four locations,Qinling Mountains,Daba Mountains,Dalou Mountains,and Xuefeng Mountains,in subtropical China.Genetic diversity among and between these populations were phylogenetically analyzed using four chloroplast DNA molecular markers(atpI-atpH,trnQ-50 rps16,trnH-psbA and trnL-trnF).Results:A total of 14 haplotypes(C1 to C14)were found in collected samples.Haplotypes C1 and C3 were shared among all populations,with C3 as the ancestral haplotype.Haplotypes C11 and C12 diverged the most from C3 and other haplotypes.No obvious phylogeographic structure was found in four locations using the GST/NST test.There is no evidence of rapid demographic expansion in S.acutum based on the mismatch distribution,and the results of Tajima’s D test,and Fu’s FS test.Our analyses of molecular variance revealed a high level of genetic variation within populations.In contrast,the genetic differentiation among S.acutum populations was low,indicating frequent gene flow.Conclusion:Xuefeng,Dalou,and Daba Mountains were possible glacial refugia for the populations of S.acutum.C1,C3,C11 and C12 haplotypes of S.acutum should be carefully preserved and managed for their genetic value.展开更多

The structures of the isolated compounds were elucidated by analysis of the MS and NMR spectroscopic data and comparison with the literature. Compounds 1 and 2 are two new natural products, and their 1H and 13C NMR spectral data were fully assigned for the first time, with the help of2D NMR. All of the isolates were obtained from genus Sinomenium for the first time and the phenanthrene and anthraquinone are the structure type first reported from this genus plants.展开更多

Methods: SA ingredients were searched according to Chinese Pharmacopoeia, Chinese Medicine Dictionary and Traditional Chinese Medicines Database(TCMD). Active compounds and target proteins of SA were acquired through the Pubchem platform. Pathway, network and function analyses of SA were performed with ingenuity pathway analysis(IPA), a bioinformatics analysis platform. Disease, biofunction-target networks were established with Cytoscape. Results: Eighteen ingredients from SA were obtained. Seven active ingredients with 31 active target proteins were acquired according to PubChem Bioassay test. By IPA analysis, 277 canonical pathways belonging to 17 function categories were collected, 23 kinds of diseases, 21 categories bio-functions were obtained. Based on P value, calculated by IPA, the top 5 significant pathway of SA targets include phosphatidylinositol 3 kinase/Akt(PI3 K/Akt) signaling, prostate cancer signaling, macrophage migration inhibitory factor(MIF) regulation of innate immunity, Guanosine-binding protein coupled receptor(GPCR) signaling, and ataxia telangiectasia mutated protein(ATM) signaling. Disease and bio-function network analysis indicated that mitogen activated protein kinase 1(MAPK1), MAPK3, p65 nuclear factor κB(RELA), nuclear factor of κB inhibitor alpha(NFκBIA), interleukin 1β(IL-1β), prostaglandin G/H synthase 2(PTGS2) and tumor protein 53(TP53) were the critical targets in various diseases treated by SA. Conclusions: In the different view of target, pathway, disease and bio-function, inflammation was found to be a central theme in many chronic conditions. SA could be used not only as an anti-inflammatory agent, but also for the treatment of cancers, neurological diseases, psychological disorders and metabolic diseases.展开更多

The key factors for separation and determination were studied and the best analysis conditions were obtained. In the buffer of 2.0 mmol/L HAc-3.0 mmol/L NaAc-10.0% CH 3OH at a separation voltage of 22 kV, Sinomenine in the sample could be separated and detected within 4 min. The linear mass concentration of Sinomenine is in the range of 1.0—36.0 μg/mL, the limit of detection reached 0.20 μg/mL. The method was used for the analysis of Sinomenine in Sinomenium acutum and tablets satisfactorily with a recovery of 96%—102%.展开更多
