Simple and effective method for treating severe adult skeletal class II malocclusion: A case report

BACKGROUND Severe skeletal class II malocclusion is the indication for combined orthodontic and orthognathic treatment.CASE SUMMARY A woman with a chief complaint of a protruding chin and an inability to close her lips requested orthodontic camouflage.The treatment plan consisted of extracting the right upper third molar,right lower third molar,left lower second molar,and left upper third molar and moving the maxillary dentition distally using a convenient method involving microimplant nail anchors,push springs,long arm traction hooks,and elastic traction chains.After 52 months of treatment,her overbite and overjet were normal,and her facial profile was favorable.CONCLUSION This method can be used for distal movement of the maxillary dentition and to correct severe skeletal class II malocclusion in adults. 展开更多

BACKGROUND Patient satisfaction with facial appearance at the end of orthodontic camouflage treatment is very important, especially for skeletal malocclusion. This case report highlights the importance of the treatment plan for a patient initially treated with four-premolar-extraction camouflage, despite indications for orthognathic surgery.CASE SUMMARY A 23-year-old male sought treatment complaining about his unsatisfactory facial appearance. His maxillary first premolars and mandibular second premolars had been extracted, and a fixed appliance had been used to retract his anterior teeth for two years without improvement. He had a convex profile, a gummy smile, lip incompetence, inadequate maxillary incisor inclination, and almost a class I molar relationship. Cephalometric analysis showed severe skeletal class Ⅱ malocclusion(A point-nasion-B point = 11.5°) with a retrognathic mandible(sella-nasion-B point = 75.9°), a protruded maxilla(sella-nasion-A point = 87.4°), and vertical maxillary excess(upper incisor to palatal plane = 33.2 mm). The excessive lingual inclination of the maxillary incisors(upper incisor to nasion-A point line =-5.5°)was due to previous treatment attempts to compensate for the skeletal class Ⅱ malocclusion. The patient was successfully retreated with decompensating orthodontic treatment combined with orthognathic surgery. The maxillary incisors were repositioned and proclined in the alveolar bone, the overjet was increased, and a space was created for orthognathic surgery, including maxillary impaction, anterior maxillary back-setting, and bilateral sagittal split ramus osteotomy to correct his skeletal anteroposterior discrepancy. Gingival display was reduced, and lip competence was restored. In addition, the results remained stable after 2 years. The patient was satisfied with his new profile as well as with the functional malocclusion at the end of treatment.CONCLUSION This case report provides orthodontists a good example of how to treat an adult with severe skeletal class Ⅱ malocclusion with vertical maxillary excess after an unsatisfactory orthodontic camouflage treatment. Orthodontic and orthognathic treatment can significantly correct a patient’s facial appearance. 展开更多

This clinical report describes the management of a patient who was diagnosed with amniotic band sequence (ABS) complicated with severe skeletal malocclusion, associated with severe muscle weakness. The patient was a 23-year-old male who underwent treatment in our hospital to improve chewing, phonation, and swallowing difficulties caused by severe abnormality of the maxillofacial skeleton. We evaluated chewing, swallowing and language functions preoperatively, and started muscle function and oral rehabilitation therapy. We performed 2-jaw osteotomy after preoperative orthodontic treatment. The patient continued with postoperative orthodontic treatment and oral rehabilitation. Postoperatively, the patient showed improved maxillofacial morphology, but was unable to attain sufficient improvement of masticatory and language functions due to atrophy and poor development of the masticatory muscles. These results suggest that obtaining sufficient therapeutic effect on oral functions is not possible if the masticatory muscles are not sufficiently strong due to atrophy or hypoplasia. 展开更多

We investigated the compensatory trends of mesiodistal angulation of first molars in malocclusion cases. We compared differences in the angulation of first molars in different developmental stages, malocclusion classifications and skeletal patterns. The medical records and lateral cephalogrammes of 1 403 malocclusion cases taken before treatment were measured to evaluate compensation of molar angulation in relation to the skeletal jaw. The cases were stratified by age, Angle classification and skeletal patterns. Differences in the mesiodistal angulation of the first molars were compared among the stratifications. We observed three main phenomena. First,angulation of the upper first molar varied significantly with age and tipped most distally in cases aged,12 years and least distally in cases aged.16 years. The lower first molar did not show such differences. Second, in Angle Class II or skeletal Class II cases, the upper first molar was the most distally tipped, the lower first molar was the most mesially tipped, and opposite angulation compensation was observed in Class III cases. Third, in high-angle cases, the upper and lower first molars were the most distally tipped, and opposite angulation compensation was observed in low-angle cases. These data suggest that the angulation of the molars compensated for various growth patterns and malocclusion types. Hence, awareness of molar angulation compensation would help to adjust occlusal relationships, control anchorage and increase the chances of long-term stability. 展开更多

Objective: To discuss possible relationships between class In malocclusion and perioral forces by measuring the pressure from the lips and the tongue of children with class nI malocclusion. Methods: Thirty-one children with class In malocclusion were investigated and their perioral forces were measured at rest and during swallowing under natural head position by a custom-made miniperioral force computer measuring system. Results: The resting pressures exerted on the labial side and palatine side of the upper lett incisor, as well as the labial side and lingual side of the lower lett incisor, were 0 g/cm^2, 0 g/cm^2, 0.57 g/cm^2 and 0.23 g/cm^2, respectively. Correspondingly, the swallowing forces were 2.87 g/cm^2, 5.97 g/cm^2, 4.09 g/cm^2 and 7.89 g/cm^2, respectively. No statistical difference between muscular pressure and gender existed. During swallowing, the lingual forces were significantly higher than the labial forces (P〈0.01), however, at rest there was no significantly different force between these two sides. Compared to the normal occlusion patients, children with class Ⅲ malocclusion had lower pedoral forces. The upper labial resting forces (P〈0.01), the lower labial resting forces (P〈0.05) and all the swallowing pressures from the lips and the tongue (P〈0.01) showed statistical differences between the two different occlusion conditions. Meanwhile, no significant difference was found for the resting pressure from the tongue between class Ⅲ malocclusion and normal occlusion. Conclusion: Patients with class Ⅲ malocclusion have lower perioral forces and this muscle hypofunction may be secondary to the spatial relations of the jaws. BACKGROUND In growing patients with skeletal discrepancies,early assessment of functional factors can be vital for the restoration of normal craniofacial growth.AIM To compare airway volumes in patients with mandibular retrognathism with the normal anteroposterior skeletal relationship,thereby assessing the association between cephalometric variables and airway morphology.METHODS Cone-beam computed tomography volume scans,and lateral cephalograms,3-dimensional airway volume and cross-sectional areas of 120 healthy children(54 boys and 66 girls mean age 15.19±1.28)which were done for orthodontic assessment were evaluated.The subjects were divided into 2 groups based on the angle formed between point A,Nasion and point B(ANB)values and cephalometric variables(such as anterior and posterior facial height,gonial angle etc.)airway volumes,and cross-sectional measurements were compared using independent t tests.Pearson’s correlation coefficient test was used to detect any relationship of different parts of the airway and between airway volume and 2-dimensional cephalometric variables.RESULTS Means and standard deviations for cephalometric,cross-sectional,and volumetric variables were compared.ANB,mandibular body length and facial convexity were statistically highly significant(P<0.01)whereas condylion to point A,nasal airway and total airway volume(P<0.05)were statistically significant.The nasal airway volume and the superior pharyngeal airway volume had a positive correlation(P<0.01),nasal airway was correlated to middle(P<0.05)and total airway superior had a relation with middle(P<0.05),inferior and total airway(P<0.05),middle was related to all other airways;inferior was also related to all the airways except nasal.Lateral cephalometric values were positively correlated with the airway volume with Frankfurt Mandibular Plane Angle and facial convexity showed significant correlations with total airway volume(P<0.05).Additionally,ANB angle was significantly correlated with total airway volume and superior airway(P<0.05).CONCLUSION The mean total airway volume in patients with retrognathic mandible was significantly smaller than that of patients with a normal mandible. 展开更多

目的通过锥形束计算机断层扫描(cone beam computer tomography,CBCT)探究骨性Ⅲ类错(牙合)畸形患者双颌手术后上气道形态变化,以及颌骨移动与气道变化的关系。方法本研究纳入44例Ⅲ类双颌手术患者(男性21例,女性23例)。采集患者术前及... 目的通过锥形束计算机断层扫描(cone beam computer tomography,CBCT)探究骨性Ⅲ类错(牙合)畸形患者双颌手术后上气道形态变化,以及颌骨移动与气道变化的关系。方法本研究纳入44例Ⅲ类双颌手术患者(男性21例,女性23例)。采集患者术前及术后3~6个月内CBCT数据,使用Dophin3D 11.95软件行测量分析。采用配对t检验、非参数Wilcoxon秩和检验分析上气道手术前后变化。采用Pearson相关系数分析气道变化与颌骨移动量的相关性。结果术后患者的上气道容积、正中矢状面纵截面积、最小横截面积明显减少(P<0.01)。口咽区容积变化量与B点的变化量相关(P<0.05)。随B点后退量的增加,上气道容积降低的显著性增加,当B点后退量>7 mm时,上气道各区域容积均显著降低(P<0.01),同时患者气道最小横截面积的危险程度显著升高(P<0.01)。结论骨性Ⅲ类双颌手术可降低患者的上气道容积。术后上气道容积减少量与B点后退量相关。当行大量下颌后退(>7 mm)时,可造成上气道容积的显著减低,增加OSAHS患病风险。对上气道有狭窄倾向的患者,应适当调整方案以减少气道风险。 展开更多

BACKGROUND Correcting severe skeletal class III malocclusion with facial asymmetry in adults through orthodontic treatment alone is difficult.CASE SUMMARY In this case report,we describe orthodontic treatment and lower incisor extraction without orthognathic surgery for a 27-year-old man with a transverse discrepancy.The extraction sites were closed using an elastic chain.The use of intermaxillary elastics,improved super-elastic Ti-Ni alloy wire,and unilateral multibend edgewise arch wire was crucial for correcting facial asymmetry and the midline deviation.CONCLUSION After treatment,the patient had a more symmetrical facial appearance,acceptable overjet and overbite,and midline coincidence.The treatment results remained stable 3 years after treatment.This case report demonstrates that a minimally invasive treatment can successfully correct severe skeletal class III malocclusion with facial asymmetry. 展开更多

目的在上颌前方牵引的基础上给予骨性Ⅲ类错[牙合](替牙期)患儿螺旋扩弓器进行扩缩处理治疗,观察疗效以及对患儿牙颌面软硬组织的影响。方法按随机数表法将2021年1月至2022年12月信阳市中心医院诊治的60例替牙期骨性Ⅲ类错[牙合]患儿分... 目的在上颌前方牵引的基础上给予骨性Ⅲ类错[牙合](替牙期)患儿螺旋扩弓器进行扩缩处理治疗,观察疗效以及对患儿牙颌面软硬组织的影响。方法按随机数表法将2021年1月至2022年12月信阳市中心医院诊治的60例替牙期骨性Ⅲ类错[牙合]患儿分两组。对照组30例采用上颌前方牵引治疗,联合组30例在上颌前方牵引的基础上给予螺旋扩弓器治疗,评估两组临床疗效,对比两组治疗前后颌面结构、上气道间隙及三维指标。结果联合组临床有效率(86.67%)较对照组(63.33%)更高,差异有统计学意义(P<0.05)。联合组治疗后颌面结构相关指标上下颌突差(ANB)、下颌平面角(FH-MP)、下面高(ANS-Me)、腭平面角(SN-PP)较对照组更高,上中切牙的突度(L1-MP)较对照组更低,差异有统计学意义(P<0.05)。联合组治疗后上气道间隙指标鼻咽直径(PNS-R)较对照组更高,差异有统计学意义(P<0.05)。联合组治疗后上气道三维指标鼻咽段最小截面积(NP area)、鼻咽段最小截面积处冠状径(NP cor.)、鼻咽段最小截面积位置矢状径(NP sag.)、鼻咽段容积(NP volume)较对照组更高,差异有统计学意义(P<0.05)。结论上颌前方牵引联合螺旋扩弓器疗效确切,可有效促进替牙期骨性Ⅲ类错[牙合]患儿颌面结构及上气道状态恢复,改善颌面软硬组织状态。 展开更多

目的研究成人骨性Ⅲ类偏颌畸形的牙齿三维位置特征。方法收集骨性Ⅲ类错[牙合]畸形患者锥形束CT(CBCT)资料、导入Invivo 5.2图像处理软件,以头影测量ANB角(上-下牙槽座角,由上齿槽座点A.subspinale、鼻根点N.nasion和下齿槽座点B.supram... 目的研究成人骨性Ⅲ类偏颌畸形的牙齿三维位置特征。方法收集骨性Ⅲ类错[牙合]畸形患者锥形束CT(CBCT)资料、导入Invivo 5.2图像处理软件,以头影测量ANB角(上-下牙槽座角,由上齿槽座点A.subspinale、鼻根点N.nasion和下齿槽座点B.supramental构成的角)小于0°、上下第一磨牙至少一侧呈近中关系以及颏下点至正中矢状面的距离﹥2 mm作为骨性Ⅲ类偏颌畸形标准筛选出偏颌患者38例,偏颌牙弓的左右两侧以颏下点偏向的一侧为偏斜侧(作为偏斜侧组,S组),另一侧为非偏斜侧(作为非偏斜侧组,NS组),筛选非偏颌患者34例作为对照组(N组)。测量各组被检者的牙齿颊舌向、近远中向倾斜角以及牙冠垂直向高度。结果S组上颌第一前磨牙、第一磨牙较N组颊倾(P<0.05),下颌尖牙、第一前磨牙、第一磨牙较N组舌倾(P<0.05),上颌的尖牙、第一前磨牙、第一磨牙较NS组颊倾(P<0.05),下颌较NS组舌倾(P<0.01);S组上颌第一磨牙近远中向倾斜角小于N组(P<0.05)、大于NS组(P<0.05);S组上下颌第一前磨牙、上颌第一磨牙牙冠垂直高度小于N组(P<0.05),上颌的尖牙、第一前磨牙、第一磨牙牙冠垂直高度小于NS组(P<0.05)。结论骨性Ⅲ类偏颌畸形患者偏斜侧牙齿在三维方向上都发生了代偿,偏斜侧上颌牙相对颊向倾斜、下颌牙相对舌向倾斜、上颌磨牙相对直立、上颌牙冠垂直高度相对偏低。 展开更多

目的研究异常垂直骨面型的成人骨性Ⅱ类与Ⅲ类错的颏联合形态是否存在差异。方法选择109例中国成年女性骨性错畸形患者为研究对象,根据垂直骨面型和矢状骨面型分为4组:高角骨性Ⅱ类组30例,高角骨性Ⅲ类组25例,低角骨性Ⅱ类组29例,... 目的研究异常垂直骨面型的成人骨性Ⅱ类与Ⅲ类错的颏联合形态是否存在差异。方法选择109例中国成年女性骨性错畸形患者为研究对象,根据垂直骨面型和矢状骨面型分为4组:高角骨性Ⅱ类组30例,高角骨性Ⅲ类组25例,低角骨性Ⅱ类组29例,低角骨性Ⅲ类组25例。拍摄X线头颅侧位片,测量颏联合形态相关指标,观察并比较4组的颏联合形态。结果成人骨性Ⅱ类与骨性Ⅲ类错的颏联合存在形态差异,但整体厚度和高度差别不大。高角组中,骨性Ⅱ类比骨性Ⅲ类有较大的下牙槽高度、较小的颏突度和基骨高度(P<0.01);低角组中,骨性Ⅱ类比骨性Ⅲ类有较大的下牙槽高度(P<0.05)和下牙槽顶部厚度(P<0.001)。矢状骨面型相同时,高角组比低角组有较小的颏厚度、颏突度和较大的颏联合总高度(骨性Ⅱ类组P<0.001,骨性Ⅲ类组P<0.05),并且更易出现下牙槽基部薄于顶部的葫芦形颏部(P<0.01)。结论异常垂直骨面型的成人骨性Ⅱ类与骨性Ⅲ类错畸形者,颏联合形态存在差异,垂直骨面型对颏联合形态的影响大于矢状骨面型,高角骨性错畸形者移动下切牙的正畸风险较大。 展开更多

目的研究采用骨种植钉前牵引对骨性Ⅲ类错患者软、硬组织侧貌的改变。方法选取18例需行前牵引矫治的生长发育期骨性Ⅲ类错患者,采用双侧上颌颧牙槽嵴植入骨种植钉配合面框式前牵引,平均治疗时间为9个月,力值为(3.5±0.1)N。治... 目的研究采用骨种植钉前牵引对骨性Ⅲ类错患者软、硬组织侧貌的改变。方法选取18例需行前牵引矫治的生长发育期骨性Ⅲ类错患者,采用双侧上颌颧牙槽嵴植入骨种植钉配合面框式前牵引,平均治疗时间为9个月,力值为(3.5±0.1)N。治疗前后头影测量采用Legan&Burstone软组织分析法以及相关硬组织测量,分析比较患者治疗前后软、硬组织变化情况。结果所有患者Ⅲ类错均得到改善,鼻底趋于丰满,颏部顺时针旋转,侧貌由凹面型变为直面型,下唇突度减小,软组织上下面高比、鼻唇角、上唇突度及颏唇沟深度无明显变化。上前牙无明显变化,下前牙舌倾,上颌骨向前生长,A点前移,SNA、ANB增大;SNB减小,下颌骨顺时针旋转。结论骨种植钉前牵引可促进上颌生长,使面中份丰满,且对上颌前牙轴倾度无影响,从而纠正骨性Ⅲ类错。 展开更多

目的研究快速扩弓前方牵引与单纯前方牵引矫治早期骨性Ⅲ类错临床疗效的异同。方法选择31例替牙期骨性Ⅲ类错患者分为A、B组,A组15例采用快速扩弓前方牵引矫治,B组16例采用单纯前方牵引矫治。2组患者矫治前后分别拍摄头颅定位侧位片... 目的研究快速扩弓前方牵引与单纯前方牵引矫治早期骨性Ⅲ类错临床疗效的异同。方法选择31例替牙期骨性Ⅲ类错患者分为A、B组,A组15例采用快速扩弓前方牵引矫治,B组16例采用单纯前方牵引矫治。2组患者矫治前后分别拍摄头颅定位侧位片,选择23个测量项目,比较矫治前后的变化,并用Pancherz分析法进行分析。结果A组疗程平均10.14个月,B组平均9.77个月(P>0.05)。Pancherz分析结果表明,A、B组上颌基骨分别前移2.99mm和3.33mm;A组下颌基骨后移0.07mm,B组下颌基骨前移0.80mm;A、B组前牙覆盖分别增加4.51mm和6.37mm,磨牙关系改善分别为4.97mm和4.73mm。以上项目除前牙覆盖的变化2组间有统计学差异外(P<0.05),其余项目无统计学差异(P>0.05),均达到满意的临床效果。A组下颌磨牙在下颌基骨中前移了1.18mm,B组下颌磨牙在下颌基骨中后移了1.20mm(P<0.05)。从2组矫治前后治疗变化量的比较来看,矫治后2组上颌前牙的唇倾度均有增加,但B组较A组增加更多(P<0.05)。结论对早期骨性Ⅲ类错采用前方牵引矫治,无论快速扩弓与否,均可获得满意的临床疗效。 展开更多

目的:比较成人骨性Ⅲ类错不同垂直骨面型下颌骨及颏部形态,探讨下颌平面角与下颌骨及颏部形态的相互关系。方法:选择年龄17~29岁、平均(21±3.020)岁的骨性Ⅲ类错[ANB<0°,平均为(-3.927±2.7637)°]患者60例(... 目的:比较成人骨性Ⅲ类错不同垂直骨面型下颌骨及颏部形态,探讨下颌平面角与下颌骨及颏部形态的相互关系。方法:选择年龄17~29岁、平均(21±3.020)岁的骨性Ⅲ类错[ANB<0°,平均为(-3.927±2.7637)°]患者60例(男、女各30例)为研究对象,根据下颌平面角(FH-MP)的大小将研究对象分为低角、均角及高角组3组,每组各20例(男、女各10例),分别测量其下颌骨及颏部骨性结构,使用SPSS16.0软件包进行统计学分析。结果:高角组的下颌角、颏高度和颏曲度大于平均角组和低角组(P<0.05);高角组的颏最小厚度、颏最小厚度/颏厚度比、颏厚度/颏高度比和颏角小于平均角组和低角组(P<0.05);进一步经两两比较,下颌角、颏最小厚度、颏最小厚度/颏厚度比、颏高度、颏厚度/颏高度比和颏角在低角组与高角组,平均角组与高角组间均有显著差异(P<0.05);而颏曲度在低角组与高角组有统计学差异(P<0.05)。下颌平面角的大小与下颌角、颏高度和颏曲度呈正相关(P<0.05),而与颏最小厚度、颏最小厚度/颏厚度比、颏厚度/颏高度比和颏角呈负相关(P<0.05)。结论:成人骨性Ⅲ类错不同垂直骨面型下颌骨及颏部形态不同,下颌平面角与下颌骨及颏部形态存在相关关系。 展开更多

目的探讨非手术矫治对成人低角骨性Ⅲ类错患者颞下颌关节的影响。方法选取13例严重成人低角骨性Ⅲ类错病例,男5例,女8例,年龄18~23岁,平均年龄21.3岁。所有患者均采用OPA-K滑动直丝固定矫治技术矫治,矫治前后均做Helkimo指数分析,... 目的探讨非手术矫治对成人低角骨性Ⅲ类错患者颞下颌关节的影响。方法选取13例严重成人低角骨性Ⅲ类错病例,男5例,女8例,年龄18~23岁,平均年龄21.3岁。所有患者均采用OPA-K滑动直丝固定矫治技术矫治,矫治前后均做Helkimo指数分析,并拍摄双侧闭口矫正薛氏位片和X线头颅侧位片,行颞下颌关节间隙测量和头影测量分析。采用SPSS12.0统计软件进行统计分析。结果非手术矫治后,Helkimo主诉症状指数重新分布,Ai0级比例明显升高,AiⅡ级比例减少为零。间隙分析表明:矫治前,髁突处于关节窝过前位;矫治后,关节前间隙线距和面积增大(P<0.01),后间隙线距和面积减小(P<0.05),线性百分比由20.81%减小为8.04%,面积比P′/A′由1.62减小为1.01,关节前后间隙基本相等。髁突及下颌骨发生后移。结论成人低角骨性Ⅲ类错非手术矫治后,髁突由下前位向后移至中位范围,关节结构趋于协调和稳定,颞下颌关节紊乱症状得到了有效缓解。 展开更多

目的探讨经正畸-正颌联合治疗的骨性Ⅲ类错畸形患者在矫治前和手术后不同时期颌面部硬组织角度的变化,评估治疗效果的长期稳定性。方法选取24例骨性Ⅲ类错畸形患者,在矫治前(T0),正颌手术后3个月(T1)、术后1年(T2)和术后3年(T3)拍摄... 目的探讨经正畸-正颌联合治疗的骨性Ⅲ类错畸形患者在矫治前和手术后不同时期颌面部硬组织角度的变化,评估治疗效果的长期稳定性。方法选取24例骨性Ⅲ类错畸形患者,在矫治前(T0),正颌手术后3个月(T1)、术后1年(T2)和术后3年(T3)拍摄X线头颅定位侧位片,测量反映颌面部硬组织变化的16项角度指标,比较矫治前和正颌手术后不同时期这些指标的变化情况。结果1)T1与T0的比较:除N-S-Ar、N-S-Ba、N-S-Go、NBa-PtGn以及Y轴角的变化很小,其差异无统计学意义外(P>0.05),其余11项指标的变化均有统计学意义(P<0.05)。2)T2与T1的比较:SNPo、U1-SN的变化有统计学意义(P<0.05),其余指标的变化均无统计学意义(P>0.05)。3)T3与T1的比较:仅U1-SN变化明显(P<0.05)。4)T3与T2的比较:所有测量项目的变化均很轻微,其差异均无统计学意义(P>0.05)。结论经正畸-正颌联合治疗后,骨性Ⅲ类错畸形患者的硬组织测量角度在术后3个月到术后1年期间即术后正畸治疗阶段存在一定程度的变化,但并未对手术效果产生根本影响。 展开更多
