Clinical Observation of the Therapeutic Effects of Wenzhong Hewei Formula in Treating Spleen and Stomach Qi Deficiency Syndrome in Internal Medicine Diseases

Objective:To observe the clinical efficacy and safety of Wenzhong Hewei Formula in treating spleen and stomach qi deficiency syndrome in internal medicine diseases.Methods:Sixty patients with spleen and stomach qi deficiency syndrome admitted to the hospital from April 2022 to June 2023 were randomly divided into observation and control groups,with 30 patients in each group.The control group received conventional internal medicine treatment,while the observation group was additionally treated with Wenzhong Hewei Formula on the basis of conventional treatment.Both groups were treated for 4 weeks.Results:The total effective rate of treatment in the observation group was higher than that of the control group(P<0.05).After treatment,the traditional Chinese medicine syndrome scores of both groups were significantly lower than before treatment,with the observation group showing a more pronounced reduction(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Wenzhong Hewei Formula can effectively improve clinical symptoms in patients with spleen and stomach qi deficiency syndrome,enhance clinical efficacy,and have a high level of safety,making it worthy of clinical promotion.

Analysis of the Spleen and Stomach Thoughts in Treating Eyelids Diseases in Essential Subtleties on the Silver Sea(《银海精微》)

Essential Subtleties on the Silver Sea(《银海精微》)is a well-known ophthalmology work of traditional Chinese medicine(TCM).There are many discourses on eyelids diseases.This paper analyzed the classification of related diseases and their treatment methods and intends to sort out the pathogenesis of eyelids diseases related to the spleen and stomach function in TCM,thus inspiring new treatment ideas and guiding clinical treatment.Through the sorting out,it is found that the eyelids diseases are closely related to the function of the spleen and stomach such as the Qi(气)and blood generation and transformation,the blood circulation,and the Yin(阴)-Yang(阳)harmony.The corresponding treatments are various,including expelling the pathogen,moving Qi(气)and invigorating blood,warming and activating the spleen Yang(阳).

Dyspepsia is a clinical syndrome caused by gastric motility disorder,including gastroparesis and esophageal reflux disease.Dyspepsia of spleen stomach weakness type is the most common TCM syndrome type in clinic.TCM has unique advantages in the prevention and treatment of spleen stomach weakness dyspepsia.This paper summarizes the research progress of TCM in the prevention and treatment of spleen stomach weakness dyspepsia in recent years,discusses its mechanism and advantages,and analyzes the existing problems,so as to provide research ideas and directions for TCM to further prevent and treat the disease and improve human quality of life.

Potential Mechanism and Application of Strengthening the Spleen, Clearing Heat and Dampness in Treatment of Gastroesophageal Reflux Disease

Gastroesophageal reflux disease(GERD)is one of refractory digestive disease in clinic characterized by high prevalence,complex and recurrent symptoms.Traditional Chinese medicine(TCM)has a significant effect on the treatment of GERD.Spleen deficiency and damp-heat syndrome becomes one of the main syndrome patterns of GERD adapting with the changes of modern lifestyle.Although the therapy of strengthening the spleen,clearing heat and dampness is widely used in treatment of GERD,further studies still need to be conducted.The article comprehensively analyses the theoretical basis,mechanism and clinical efficacy of the therapy in order to provide more ideas for clinical treatment.

Research on the Progress of Treating Gastroesophageal Reflux Disease with Modified Classic Traditional Chinese Medicine Prescription

Gastroesophageal reflux disease (GERD) is a high-incidence digestive system disease. Western medicine mainly uses drugs such as proton pump inhibitors to inhibit gastric acid secretion, but some patients are accompanied by symptoms such as non-acid reflux and gas reflux, which cannot effectively treat the disease. It is necessary to actively explore other treatment schemes. Traditional Chinese medicine (TCM) has a long history of research on gastroesophageal reflux disease, which emphasizes the treatment based on syndrome differentiation as a whole. Through the treatment of various and multi-component TCM prescriptions, the patient’s body condition can be adjusted, and the treatment effect on gastroesophageal reflux disease is reliable, which has obvious therapeutic advantages. To further clarify the treatment of gastroesophageal reflux disease, this study reviewed and analyzed the research progress of the treatment of liver disease with modified prescriptions, and the report is as follows.

辛智科“脾胃中心”理论及其在杂病中的应用

Study on the Drug Selection Law for Treatment of Chronic Gastritis with Spleen Deficiency and Stomach Dryness by Complex System Entropy Cluster

Objective:To study on Prof. GAO Zhong-ying’s drug selection law for treatment of chronic gastritis with data mining technique. Methods: The 407 medical records of chronic gastritis treated by Prof. GAO Zhong-ying were collected and the study on these drugs in the recipes was carried out with data mining method. Among them, the recipe composed of one drug was studied with frequency statistical method, correlativity between drug pairs with improved mutual information, correlativity among multi-drugs with complex system entropy cluster technique. Results: In treatment of chronic gastritis by Prof. GAO Zhong-ying there were 30 drugs with a higher use frequency of over 38 times, 94 commonly-used drug pairs with correlation coefficient of over 0.05, 11 commonly-used drug core combinations. Conclusion: The results attained with data mining technique for studying experience of famous and old TCM physicians conform to the clinical practice and the method is of an important significance for summarization of famous and old TCM physicians’ experiences.

[目的]介绍国医大师葛琳仪教授(以下简称葛教授)分层次施用风药辨治脾胃病的临床特色及其临证经验。[方法]通过理论溯源,跟师随诊,整理并分析葛教授基于气化理论运用风药治疗脾胃系疾病的临床医案,总结葛教授巧施风药辨治脾胃病的思路、心得和用药特色,并附医案一则加以佐证。[结果]气化是生命的基本活动,气化异常是疾病产生的重要原因。葛教授指出,脾胃病的基本病机是“气化异常”,常从“气化”切入,巧施风药以调畅气机,恢复脾升胃降功能,维稳五脏气机,具体分为四个层次:以风药疏肝和脾、壮气运脾、燥湿健脾以及搜风通络养脾。常用的风药有柴胡、防风、黄芪、升麻、佛手、紫苏梗、苍术、娑罗子、桑叶、藿香、草果、厚朴、佩兰、徐长卿、炒僵蚕等及其组成的药对、药串。[结论]葛教授临证精于辨证,从“气”切入,按不同层次巧施风药治疗脾胃病,特色鲜明,疗效显著,其学术思想值得临床借鉴和推广。

丁书文从脾胃论治心系疾病的临证应用

丁书文教授推崇李东垣的脾胃内伤学说,强调脾胃在人体脏腑功能中的重要性,主张心系疾病从脾胃论治。丁书文教授基于李东垣的学术思想,创立了心系疾病“热毒”学说,指出心系热毒是脾胃功能失调、湿痰瘀热胶结导致。治疗既治心系疾病之标,亦调脾胃后天之本。

基于医学院校解剖学微课资源库联合内镜病例图库在脾胃病临床实践教学中受训学生反馈情况的研究

目的探讨医学院校解剖学微课资源库联合内镜病例图库在脾胃病临床实践教学的受训学生反馈情况的应用效果。方法于2021—2022年选取2019级广西中医药大学中医专业本科生为研究对象,整群抽样法抽取90名本科生,分为A组(采用传统实践教学模式)、B组(采用传统结合内镜图库教学模式)、C组(传统联合微课及内镜图库教学模式)各30名,受训结束后,进行对比研究。带教教师经过系统培训并完成一致性检验后,采用微信小程序(问卷调查方式),了解学生对校解剖学微课资源库联合内镜病例图库在脾胃病临床实践教学中的认识及临床实践教学体会。结果三组在教学效果的互动性、生动性、学习效率方面的评价,C组的互动性、生动性、学习效率均优于A、B组,差异有统计学意义(P<0.05)。三组学生在学习环境轻松、可自主决定学习步调、提高临床思维能力、临床技能、专业知识掌握程度和提高学习兴趣等方面,C组优于A、B组,差异有统计学意义(P<0.05)。结论在脾胃病科临床实践教学中应用医学院校解剖学微课资源库联合内镜病例图库,受训学生反馈情况良好,有助于提高临床实践教学效果。

聂莉芳运用调理脾胃八法治疗慢性肾脏病经验

聂莉芳教授认为慢性肾脏病的发生与脾胃功能失调密切相关,故在临床中常根据不同主症或针对不同病机灵活运用和胃降逆止呕、健脾祛湿利水、健脾益气升清、清化中焦湿热、清胃泻火通腑、脾肾气阴双补、健脾补肾温阳、肝脾同调和血八种方法治疗慢性肾脏病。聂老强调运用调理脾胃八法时要注意治脾、治胃有所侧重,主张健脾益气佐以行气消导之品以达补而不滞之效,同时兼顾脏腑之间的生克制化关系,尤其强调煎服法,认为各个诊疗细节兼顾,方能取得较好疗效。

从“有诸内者必形诸外”及体质论治小儿慢性荨麻疹

慢性荨麻疹因其反复发作、瘙痒难耐的特点严重影响患儿的身心健康,属于儿科临床难治病证。慢性荨麻疹虽属于外科范畴,侯江红教授认为“内之症或不及其外,外之症则必根于其内也”,治疗时不可拘泥于外治之法,必当思外而揣内,究其内在本因。考虑小儿好发该病,多责之于乳食所伤,侯教授指出小儿慢性荨麻疹的发生多由脾胃不和导致,因此临床辨证可从脾胃湿热型、气虚热盛型、营卫失和型入手,治疗采用培土生金法健运脾胃、调补肺脾、调和肺脾,以扶正固表、调和营卫、解表透疹。体质形成由脏腑盛衰偏颇所决定,因此亦认为体质因素是荨麻疹治疗与预防的重要病机,其中高敏偏颇体质患儿易出现荨麻疹、易咳嗽、哮喘等非健康倾向,故将调理高敏体质作为防治的重点,有效减少了患儿荨麻疹的复发率。此外还注重治养兼顾,讲求医患配合,从饮食、运动、作息、家长行为心理等方面改变养护观念,避免家长过度就医对患儿造成过度用药,通过非药物干预的方法来有效调动患儿自身免疫功能,亦增进了医患之间的信任。文章附案例一则,详细介绍侯江红教授治疗小儿慢性荨麻疹临证经验,以期为临床相关治疗提供参考。
