Assessing Empathy in Anesthesia Residents with the Chinese Version of the Consultation and Relational Empathy: A Pilot Standardized Patient Program 被引量:1

Objectives To validate the reliability of the Chinese version of the Consultation and Relational Empathy(CARE)in physician-standardized patient(SP)encounter.We also tried to examine the agreement between video-based ratings and in-room ratings,as well as the agreement between the faculty ratings and SP ratings.Methods The CARE was translated into Chinese.Forty-eight anesthesia residents were recruited to make preoperative interview in SP-counter.Performance of each resident was graded by in-room raters,video raters and SP raters.Consistency between different raters was examined.Results The Chinese-CARE measure demonstrated high scale reliability with a Cronbach's alpha value of 0.95 and high consistency in the in-room ratings in intraclass correlation(coefficient=0.888,P<0.001).Despite a good consistency in intraclass correlation,video ratings were significantly higher than in-room ratings(39.6±7.1 vs.24.0±10.0,P<0.001),and Wilcoxon signed-rank test indicated that the pass/fail rate was significantly higher based on video ratings than based on in-room ratings(45/48 vs.22/48,P<0.001).SP ratings had a moderate consistency with in-room faculty ratings(coefficient=0..568,P<0.001),and there was no significant difference between the pass/fail rates based on the in-room ratings and SP ratings(22/48vs.28/48,P=0.12).Conclusions The Chinese-CARE measure is reliable in the assessment of empathy during preoperative anesthesia interview.In-room and video ratings are not equivalent,while SP may provide a feasible alternative for the faculty rater in the assessment of communication skills with an appropriate measure. 展开更多

Purpose: Aim of this single center study was to determine whether elderly patients benefit from individualized treatment not excluding full standard therapy. Additionally predictive and prognostic factors influencing outcome in this patient’s population were evaluated. Material and Methods: Between 1997 and 2010, 119 patients equal or older than 60 years were enrolled in this retrospective review. All patients had neuropathology confirmed diagnosis of glioblastoma. Treatment outcome concerning progression free survival was measured by MRI. For evaluation of O6-Methylguanin-DNA-methyltransferase (MGMT) Methylation-specific PCR was used. The log rank test and the Cox proportional hazards model were used to analyze the data. Factors considered in univariate and multivariate analyses included age, gender, Karnofsky performance scale (KPS), extent of resection, treatment with radioand chemotherapy and MGMT status. Survival probabilities were estimated by means of the Kaplan Meier method. Results: Multivariate analysis demonstrated age, KPS and treatment more than surgery as prognostic factors for survival and sex, KPS, type of surgery and standard treatment as independent factors for PFS. MGMT status revealed no difference in survival between the methylated and unmethylated tumours in the whole study population (12.7 and 12.0, respectively). Surgery had an impact on survival with a significant advantage for complete resection. Conclusion: Extent of resection is essential even in elderly patients. Full standard treatment should be offered to elderly GBM patients with good clinical performance, there is no reason to withhold radioor chemotherapy from these patients. MGMT promotor methylation of the tumour is not relevant for treatment decision. 展开更多

The large global outbreak of coronavirus disease 2019(COVID-19)has seriously endangered the health care system in China and globally.The sudden surge of patients with severe acute respiratory syndrome coronavirus 2 infection has revealed the shortage of critical care medicine resources and intensivists.Currently,the management of non-critically ill patients with COVID-19 is performed mostly by non-intensive care unit(ICU)physicians,who lack the required professional knowledge,training,and practice in critical care medicine,especially in terms of continuous monitoring of the respiratory function,intervention,and feedback on treatment effects.This clinical problem needs an urgent solution.Therefore,here,we propose a series of clinical strategies for non-ICU physicians aimed at the standardization of the management of non-critically ill patients with COVID-19 from the perspective of critical care medicine.Isolation management is performed to facilitate the implementation of hierarchical monitoring and intervention to ensure the reasonable distribution of scarce critical care medical resources and intensivists,highlight the key patients,timely detection of disease progression,and early and appropriate intervention and organ function support,and thus improve the prognosis.Different management objectives are also set based on the high-risk factors and the severity of patients with COVID-19.The approaches suggested herein will facilitate the timely detection of disease progression,and thus ensure the provision of early and appropriate intervention and organ function support,which will eventually improve the prognosis. 展开更多

Background:In recent years,with the gradual expansion of the scale of medical education,the shortage of medical teaching resources and the reluctance of patients to cooperate with teaching have become increasingly prominent.Objective:To explore the application effect of student standardized patient(SSP)in the clinical teaching of acute abdomen.Methods:Fifty-four fifth-year general medical students from class 1826 of the general department of The First Affiliated Hospital of Xi’an Medical University were selected as the research subjects and randomly divided into two groups,with 27 students in the experimental teaching group,and the remaining 27 students in the conventional teaching group.The experimental teaching group adopted the SSP teaching approach.The SSPs were generated from the training students of the hospital through the recruitment and training process of SSP.In this study,seven qualified SSPs were selected for the clinical teaching of acute abdomen.At the end of the course,a periodic assessment was held.The rank sum test was used to compare the excellent and good rates between the two groups,while t-test was used to compare the difference between the two groups.Results:The results showed that the excellent and good rate of the experimental teaching group was significantly higher than that of the conventional group,in which the difference between the two groups was statistically significant(p<0.05).In terms of the assessment results,the theoretical scores and skills scores of the experimental teaching group were better than those of the conventional teaching group,in which the differences between the two groups were statistically significant(p<0.05).Conclusion:Through the training of SSP for acute abdomen and its application in surgical teaching and examination,the superiority of SSP is emphasized.Although there are still some shortcomings in the application of SSP in clinical teaching,it is a relatively new and effective teaching method,and it will play an increasingly critical role in clinical skills training pertaining to the medical specialty. 展开更多

目的探索情景式案例教学法(CBL)联合学生标准化病人(SSP)在中医住院医师医患沟通技能培训中的运用效果。方法选取2022年2月—2022年3月进行住院医师规范化培训的2021级住院医师120人作为研究对象,随机分为对照组和观察组,各60人,进行医... 目的探索情景式案例教学法(CBL)联合学生标准化病人(SSP)在中医住院医师医患沟通技能培训中的运用效果。方法选取2022年2月—2022年3月进行住院医师规范化培训的2021级住院医师120人作为研究对象,随机分为对照组和观察组,各60人,进行医患沟通技能培训。对照组采用传统授课方式,观察组采用情景式CBL联合SSP教学方式授课,教学时长4周,每周1次,每次2学时。培训结束后,应用医患沟通技能评价量表(SEGUE)对两组住院医师的医患沟通能力进行评分,同时发放调查问卷对教学方法进行评价。结果医患沟通培训后,对照组和观察组SEGUE量表的总评分分别为(20.85±3.43)分、(23.93±2.11)分,观察组总评分及各维度评分均高于对照组,差异有统计学意义(P<0.05)。在对教学方法的评价方面,观察组均优于对照组,差异具有统计学意义(P<0.05)。结论情景式CBL联合SSP教学法应用于住院医师医患沟通技能培训效果较好,值得在今后的实践教学中推广应用。 展开更多

目的探讨基于问题的学习(Problem-based Learning,PBL)教学法联合标准化病人(Standardized Patient,SP)在临床医学学士(Bachelor of Medicine and Bachelor of Surgry,MBBS)学生血液病实践教学中的应用效果。方法选取2023年2—6月由江... 目的探讨基于问题的学习(Problem-based Learning,PBL)教学法联合标准化病人(Standardized Patient,SP)在临床医学学士(Bachelor of Medicine and Bachelor of Surgry,MBBS)学生血液病实践教学中的应用效果。方法选取2023年2—6月由江苏大学附属人民医院血液科负责血液病临床教学的39名2019级MBBS学生为对照组,选取2024年2—6月32名2021级MBBS学生为观察组。观察组采用PBL联合SP模式实施临床实践教学,对照组采用传统讲授法实施临床实践教学。对比两组理论考试、临床思维能力考核成绩及对教学方案的认可度。结果观察组理论考试、临床思维能力考核成绩均高于对照组,差异有统计学意义(P均<0.05)。观察组对教学方案的认可度为90.63%(29/32),高于对照组的69.23%(27/39),差异有统计学意义(χ^(2)=8.178,P<0.05)。结论采取PBL联合SP模式可有效提高MBBS学生的见习质量、学习兴趣和实践能力,加深学生对于血液系统疾病的认识,全面提升学生的自学能力、团队协作能力以及临床思维能力。 展开更多

目的探讨案例教学法(case-based learning,CBL)、以问题为基础的教学法(problem-based learning,PBL)联合标准化病人(standardized patients,SP)及循证医学(evidence-based medicine,EBM)教学法在消化内科实践教学中的应用效果。方法选... 目的探讨案例教学法(case-based learning,CBL)、以问题为基础的教学法(problem-based learning,PBL)联合标准化病人(standardized patients,SP)及循证医学(evidence-based medicine,EBM)教学法在消化内科实践教学中的应用效果。方法选取2022年9月—2023年6月42名杭州师范大学附属医院消化内科见习生为研究对象,按照随机数字表法分为试验组和对照组,每组21名。对照组采用CBL教学法,试验组采用CBL、PBL联合SP及EBM教学法。比较2组学生的批判性思维能力评分及对教学效果的评价。结果试验组学生开放思想、批判思维的自信心及总分分别为(40.95±3.60)分、(42.35±2.66)分、(287.35±15.12)分,高于对照组的(38.59±3.46)分、(38.56±4.36)分、(274.28±13.97)分(P<0.05)。问卷调查的教学效果评价方面,试验组在对教学方法的满意度、主动学习积极性及兴趣水平、师生交流和课堂气氛活跃程度、希望今后使用该教学方法的人数占比上均优于对照组(P<0.05)。结论与CBL相比,CBL、PBL联合SP及EBM教学法能更好地提升医学生的批判性思维能力,更大程度激发学生自主学习的兴趣和更好地培养其发现和解决临床问题的能力,值得在医学教育教学中不断探索和应用。 展开更多

目的评价以问题为导向的学习(problem-based learning,PBL)与标准化病人(standardized patients,SP)联合教学法在心胸外科住院医师规范化培训教学中的应用效果。方法将2021年6月-2023年6月80名在南京医科大学第一附属医院心胸外科参加... 目的评价以问题为导向的学习(problem-based learning,PBL)与标准化病人(standardized patients,SP)联合教学法在心胸外科住院医师规范化培训教学中的应用效果。方法将2021年6月-2023年6月80名在南京医科大学第一附属医院心胸外科参加住院医师规范化培训的住院医师随机分成2组,试验组采用PBL教学法联合SP,对照组采用传统教学法,通过理论、技能考试及问卷调查评估教学效果。结果试验组出科技能考试为(80.23±0.92)分,总成绩为(80.48±0.54)分均高于对照组[出科技能考试(75.33±0.54)分及总成绩(78.23±0.43)分],差异有统计学意义(P<0.001)。对2组住培医师进行个人综合能力提高自我评定问卷调查,在提高自主学习能力、提高医患沟通能力、提高临床思维及提高自我学习兴趣选项中,试验组选择是的人数分别为34(85.00%)、31(77.50%)、31(77.50%)、34(85.00%),对照组选择是的人数分别为26(65.00%)、16(40.00%)、18(45.00%)、19(47.50%),试验组高于对照组,差异有统计学意义(P<0.05)。结论PBL教学法联合SP在心胸外科住院医师规范化培训教学中更有利于提高教学效果并赢得了学生的认可。 展开更多

BACKGROUND Global education in psychiatry is heavily influenced by knowledge from Western,high-income countries,which obscures local voices and expertise.AIM To adapt a human simulation model to psychiatric education in a context that is specific to local languages and cultures.METHODS We conducted an observational study consisting of six human simulation sessions with standardized patients from two host countries,speaking their native languages,and following an adaptation of the co-constructive patient simulation(CCPS)model.As local faculty became increasingly familiar with the CCPS approach,they took on the role of facilitators—in their country’s native language.RESULTS Fifty-three learners participated:19 child and adolescent psychiatry trainees and 3 faculty members in Türkiye(as a group that met online during 3 consecutive months);and 24 trainees and 7 faculty in Israel(divided into 3 groups,in parallel in-person sessions during a single training day).Each of the six cases reflected local realities and clinical challenges,and was associated with specific learning goals identified by each case-writing trainee.CONCLUSION Human simulation has not been fully incorporated into psychiatric education:The creation of immersive clinical experiences and the strengthening of reflective practice are two areas ripe for development.Our adaptations of CCPS can also strengthen local and regional networks and psychiatric communities of practice.Finally,the model can help question and press against hegemonies in psychiatric training that overshadow local expertise. 展开更多

目的评估基于小规模限制性网络在线课程(small private online course,SPOC)平台联合原位模拟(in situ simulation,ISS)和标准化病人(standardized patients,SP)混合教学模式在老年医学教学中的应用效果。方法选取2021年7月—2022年12... 目的评估基于小规模限制性网络在线课程(small private online course,SPOC)平台联合原位模拟(in situ simulation,ISS)和标准化病人(standardized patients,SP)混合教学模式在老年医学教学中的应用效果。方法选取2021年7月—2022年12月在中国科学技术大学附属第一医院老年科接受轮转的99名住培医师,随机分为试验组(n=50)和对照组(n=49)。试验组采用基于SPOC平台联合ISS和SP混合教学模式,对照组采用传统教学模式,比较2组住培医师的理论知识、临床技能、教学质量、教学评分、考试成绩和推荐评分。结果试验组的理论知识、临床技能、教学质量和教学评价评分均高于对照组,差异有统计学意义(P<0.001)。试验组的考试成绩为(80.50±7.53)分,高于对照组[(75.88±6.11)分],差异有统计学意义(P<0.001)。试验组的认可推荐评分为(8.66±0.75)分,高于对照组[(6.02±0.72)分],差异有统计学意义(P<0.001)。结论基于SPOC平台联合ISS和SP混合教学模式可提高老年医学的教学效果,提升住培医师的理论知识和临床技能水平。 展开更多

目的探讨情景模拟教学联合标准化病人(standardized patient,SP)教学在军医大学毕业后教育中对住培学员的传染病学岗位胜任力的影响。方法选取2021年2月—2022年11月在海军军医大学第一附属医院感染科住培的65名军队规范化培训学员为研... 目的探讨情景模拟教学联合标准化病人(standardized patient,SP)教学在军医大学毕业后教育中对住培学员的传染病学岗位胜任力的影响。方法选取2021年2月—2022年11月在海军军医大学第一附属医院感染科住培的65名军队规范化培训学员为研究对象,按照1∶1随机分为传统教学组(理论小讲课联合常规查房)和教改组(情景模拟教学联合SP教学)。通过理论考核、技能考核、日常评价、问卷调查等综合评估两组学员的传染病学岗位胜任力。结果教改组和传统教学组在理论考核总分、技能考核总分及总成绩差异均有统计学意义(P<0.05)。其中理论考核的主观题部分,技能考核的病史采集、体格检查、病例分析部分和医患沟通能力方面差异均有统计学意义(P<0.05)。问卷调查显示,教改组学员课程学习兴趣、认真参与程度、鼓舞临床热情、锻炼沟通表达和协同合作能力方面较传统教学组显著提高,起到很好的正向作用。教改组对带教形式和带教效果更满意,差异有统计学意义(P<0.05)。结论情景模拟教学联合SP教学可显著提升军医大学规范化培训学员的岗位胜任力,并将岗位胜任力转化为具体的临床行为,得到住培医师的高度评价。 展开更多

目的将案例教学法(Case-based Learning,CBL)+问题教学法(Problem-based Learning,PBL)+标准化流程教学法(Standardized Patients,SP)多元化教学模式应用于妇产科教学中,并对该模式的应用效果进行分析。方法选取河南大学淮河医院妇产科... 目的将案例教学法(Case-based Learning,CBL)+问题教学法(Problem-based Learning,PBL)+标准化流程教学法(Standardized Patients,SP)多元化教学模式应用于妇产科教学中,并对该模式的应用效果进行分析。方法选取河南大学淮河医院妇产科于2021年7月—2022年5月接受临床实习的60名实习生作为研究对象,根据随机数表法将其分为研究组和对照组,各30名。对照组给予传统的带教模式,研究组给予CBL+PBL+SP多元化教学模式,对比两组实习生的教学效果。结果研究组实习生的理论、操作考核成绩均高于对照组,差异有统计学意义(P均<0.05);研究组实习生自评结果中的教学模式得分、理论与实践相结合、职业认同、团队合作、人际交流、临床分析与解决问题、自主学习、文献检索与运用及总分均高于对照组,差异有统计学意义(P均<0.05);研究组的总满意率(96.67%)高于对照组(80.00%),差异有统计学意义(P<0.05)。结论在妇产科教学中实施CBL+PBL+SP多元化教学模式能够显著提高实习生的理论成绩及技能操作,且教学效果显著,实习生满意度较高。 展开更多
