It demands adequate, at least 16 lymph nodes (LNs) to be dissected; therefore different staging systems were invented. Methods: Between March 2005 and March 2010, 164 patients were evaluated at the Department of General Surgery in the Ken^zy Gyula Hospital and at the Department of General, Thoracic and Vascular Surgery in the Kaposi M6r Hospital. The 6th, 7th and 8th UICC N-staging systems, the number of examined LNs, the number of harvested negative LNs, the metastatic lymph node ratio (MLR) and the log odds of positive LNs (LODDS) were determined to measure their 5-year survival rates and to compare them to each other. Results: The overall 5year survival rate for all patients was 55.5% with a median overall survival time of 102 months. The tumor stage, gender, UICC N-stages, MLR and the LODDS were significant prognostic factors for the 5-year survival with univariate analysis. The 6th UICC N-stage did not follow the adequate risk in comparing N2 vs. NO and N3 vs. NO with multivariate investigation. Comparison of performances of the residual N classifications proved that the LODDS system was first in the prediction of prognosis during the evaluation of all patients and in cases with less than 16 harvested LNs. The MLR gave the best prognostic prediction when adequate (more than or equal to 16) lymphadenectomy was performed. Conclusions: We suggest the application of LODDS system routinely in western patients and the usage of MLR classification in cases with extended lymphadenectomy.展开更多

METHODS: We retrospectively studied 347 consecutive su...AIM: To investigate the clinical relevance and prognosis regarding survival according to the changes of the tumor-node-metastasis(TNM) in gastric cancer patients. METHODS: We retrospectively studied 347 consecutive subjects who underwent surgery for gastric adenocarcinoma at the Division of General Surgery, Hospital of Busto Arsizio, Busto Arsizio, Italy between June 1998 and December 2009. Patients who underwent surgery without curative intent, patients with tumors of the gastric stump and patients with tumors involving the esophagus were excluded for survival analysis. Patients were staged according to the 6thand 7thedition TNM criteria; 5-year overall survival rates were investigated, and the event was defined as death from any cause. RESULTS: After exclusion, our study population included 241 resected patients with curative intent for gastric adenocarcinoma. The 5-year overall survival(5-year OS) rate of all the patients was 52.8%. Thediagnosed stage differed in 32% of 241 patients based on the TNM edition used for the diagnosis. The patients in stage Ⅱ according to the 6thedition who were reclassified as stage Ⅲ had significantly worse prognosis than patients classified as stage Ⅱ(5-year OS, 39% vs 71%). According to the 6thedition, 135 patients were classifed as T2, and 75% of these patients migrated to T3 and exhibited a significantly worse prognosis than those who remained T2, regardless of lymph node involvement(37% vs 71%). The new N1 patients exhibited a better prognosis than the previous N1 patients(67% vs 43%). CONCLUSION: 7thTNM allows new T2 and N1 patients to be selected with better prognosis, which leads to different staging. New stratification is important in multimodal therapy.展开更多

METHODS: A prospectively maintained database was interrogated. All patients undergoing curative colonic resection were included. Neutrophil lymphocyte ratio(NLR) and albumin were used as markers of SIR. In keeping with previously studies, NLR ≥ 4, albumin < 35 was used as cut off points for SIR. Statistical analysis was performed using 2 sample t-test and χ~2 tests where appropriate.RESULTS: Three hundred and two patients were included for analysis. One hundred and ninety-five patients had NLR < 4 and 107 had NLR ≥ 4. There was no difference in age or sex between groups. Patients with NLR of ≥ 4 had lower mean lymph node yields than patients with NLR < 4 [17.6 ± 7.1 vs 19.2 ± 7.9(P = 0.036)]. More patients with an elevated NLR had node positive disease and an increased lymph node ratio(≥ 0.25, P = 0.044). CONCLUSION: Prognosis in colon cancer is intimately linked to the patient’s immune response. Assuming standardised surgical technique and sub specialty pathology, lymph node count is reduced when systemic inflammatory response is activated.展开更多

Results of node degrees for the Northern Hemisphere (NH) mid-high latitude (30° N-90°N) circulation system (NHS) networks with and without the Arctic Oscillations (AO), the North Atlantic Oscillations (NAO) and the Pacific-North American pattern (PNA) demonstrate that the teleconnections greatly shorten the mean shortest path length of the networks, thus being advantageous to the rapid transfer of local fluctuation information over the network and to the stability of the NHS. The impact of the AO on the NHS connection structure is most important and the impact of the NAO is the next important. The PNA is a relatively independent teleconnection, and its role in the NHS is mainly manifested in the connection between the NHS and the tropical circulation system (TRS). As to the Southern Hemisphere mid-high latitude (30°S-90°S) circulation system (SHS), the impact of the Antarctic Arctic Oscillations (AAO) on the structural stability of the system is most important. In addition, there might be a stable correlation dipole (AACD) in the SHS, which also has important influence on the structure of the SHS networks.展开更多

The aim of this review...Seventh tumor-node-metastasis(TNM) classification for gastric cancer,published in 2010,introduced changes in all of its three parameters with the aim to increase its accuracy in prognostication. The aim of this review is to analyze the efficacy of these changes and their implication in clinical practice. We reviewed relevant Literature concerning staging systems in gastric cancer from 2010 up to March 2016. Adenocarcinoma of the esophago-gastric junction still remains a debated entity,due to its peculiar anatomical and histological situation: further improvement in its staging are required. Concerning distant metastases,positive peritoneal cytology has been adopted as a criterion to define metastatic disease: however,its search in clinical practice is still far from being routinely performed,as staging laparoscopy has not yet reached wide diffusion. Regarding definition of T and N: in the era of multimodal treatment these parameters should more influence both staging and surgery. The changes about T-staging suggested some modifications in clinical practice. Differently,many controversies on lymph node staging are still ongoing,with the proposal of alternative classification systems in order to minimize the extent of lymphadenectomy. The next TNM classification should take into account all of these aspects to improve its accuracy and the comparability of prognosis in patients from both Eastern and Western world.展开更多

Our design effort complies with the stringent limitations existing in these architectures in terms of hardware complexity, available storage and energy consumption. Rather that precluding deliberately harmful programs from producing their effects, our solution is aimed at limiting the spread of programming errors outside the memory scope of the running program. The discussion evaluates the resulting protection environment from a number of salient viewpoints that include the implementation of common protection paradigms, efficiency in the distribution and revocation of access privileges, and the lack of a privileged (kernel) mode.展开更多

Previous studies have demonstrated that proteins undergo carbonylated modification at the CN in rice, indicating oxidative damage. However, the levels of reactive carbonyl species(RCS) and the associated scavenging system at the CN are largely unknown. In this study, we optimized methods for the extraction and analysis of RCS from dry rice embryos. In order to acquire seeds at the CN, rice seeds were subjected to natural conditions for 7, 9, 11 and 13 months, and the seed germination rates were reduced to 90%, 82%, 71% and 57%, respectively. We chose the stage with seed germination rate of 82% as the CN according to the rice seed vigor loss curve. The levels of crotonaldehyde and 4-hydroxy-(E)-2-nonenal(HNE) were significantly increased at the CN. In addition, genes encoding carbonyl-scavenging enzyme, including Os ALDHs and Os AKRs, were significantly down-regulated at the CN, and reductions in the expression of Os ALDH2-2, Os ALDH2-5, Os ALDH3-4, Os ALDH7, Os AKR1 and Os AKR2 in particular could be responsible for RCS accumulation. Thus, the accumulations of crotonaldehyde and HNE and down-regulation of genes encoding carbonyl-scavenging enzyme might be related to an accelerating loss of seed viability at the CN.展开更多

Any surplus production is fed into the grid and contributes to improving the voltage. Several techniques are developed to model their connection. However, studies on methods of injecting energy production into the Low Voltage (LV) network are nowadays a problem. This paper proposes a mathematical model to determine the current to be injected and calculate each node’s voltage. The current equation is a recurrence relation with an initial condition. This initial condition is for the case of a single PV system connected to the LV grid. The equation can also be written in matrix form. Similarly, the voltage solution is a recurrence relation. It also has an initial condition for the first node. Both mathematical formulae with the proposed initial conditions are consistent and can be used for the determination of the current and voltage of the different nodes in the grid.展开更多

In this model, expressions are derived to calculate two performance metrics, namely, the queue node and system utilization factors. In order to demonstrate the flexibility and effectiveness of the mQN model in analyzing the performance of an mQN network router, two scenarios are performed. These scenarios investigated the variation of queue nodes and system utilization factors against queue nodes dropping probability for various system sizes and packets arrival routing probabilities. The performed scenarios demonstrated that the mQN analytical model is more flexible and effective when compared with experimental tests and computer simulations in assessing the performance of an mQN network router.展开更多
