Effect Analysis of WeChat Platform Combined with PBL Teaching Method for Standardized Training of Residents in Anesthesiology Department 被引量:1

Objective: To explore and analyze the effect of the WeChat platform combined with the PBL teaching method in the standardized training of anesthesia residents. Methods: 120 anesthesiology residents from January 2018 to the end of December 2019 were selected, and divided into a control group and an observation group of 60 each according to the time sequence of admission. The control group adopted the conventional teaching mode, and the observation group adopted the WeChat platform combined PBL teaching method. The scores of theoretical knowledge and anesthesia skills operation after training, the scores of the teaching interest survey, and the satisfaction with the teaching mode between the two groups were compared. Results: The scores of theoretical knowledge and anesthesia skills operation in the observation group were significantly higher than those in the control group, and the indicators of teaching interest in the observation group were also higher than those in the control group. The differences were statistically significant (P < 0.05). The satisfaction degree of the observation group was significantly higher than that of the control group, and the difference was also statistically significant (P < 0.05). Conclusion: The WeChat platform combined with the PBL teaching method is beneficial to improve the training effect of anesthesiology residents, stimulating autonomous learning ability, ensuring the effective practice of theoretical knowledge, and promoting them to move towards a higher standard of anesthesia skills. 展开更多

Objective:To explore the effect of fragmented case teaching in the standardized training of residents in anesthesiology department.Methods:80 doctors who participated in the standardized training of residents in anesthesiology department from January 2021 to January 2022 were selected as the research objects,and the 80 doctors were divided into experimental groups according to the clinical teaching mode(n=40,implemented case fragmented teaching method)and the control group(n=40,traditional teaching method).The training lasted for 2 months,and the medical history collection,medical record analysis,practical operation ability,theoretical assessment results,and the degree of satisfaction towards the training of the two groups of doctors were compared.Results:After 2 months of training,the theoretical knowledge and operational ability of the doctors in the two groups have improved to a certain extent,but the medical history collection,medical record analysis,practical operation ability,theoretical assessment results and satisfaction of the doctors in the experimental group were significantly better than those in the control group(P<0.05).Conclusion:The effect of the fragmented case teaching method in the standardized training of anesthesiology residents is significantly better than the traditional teaching mode.The abilities of doctors have significantly improved after the training.Hence,the fragmented case teaching method is worthy of promotion in clinical practice. 展开更多

Physician-Rated Utility of Procedure Videos for Teaching Procedures in the Emergency Department, Overall and during Emergency Department Crowding

Background: Real-time use of procedure videos as educational tools has not been studied. We sought to determine whether viewing a video of a medical procedure prior to procedure performance in the emergency department improves the quality of teaching of procedures, and whether videos are particularly beneficial during periods of emergency department crowding. Methods: In this single-centre, prospective, before and after study standardized data collection forms were completed by both trainees and supervising emergency physicians (EPs) at the end of each emergency department shift in the before (August 2008-March 2009) and after (August 2009-March 2010) phase. Online procedure videos were introduced on emergency department computers in the after phase. The primary outcome measure was EP rating of the quality of teaching provided (5-point Likert scale). The interaction between crowding and videos was also assessed, to determine whether videos provide a specific additional benefit during periods of emergency department crowding. Results: There were 1159 procedures performed by 192 trainees. Median procedures performed per shift was 1.0 (IQR 0 - 2.0). Mean EP rating of teaching provided was significantly higher in the group that viewed videos, at 4.2 versus 3.7 (p 0.001). In the adjusted analysis, EP ratings increased by 0.5 with a video (p 0.001), while the odds of a score of 5.0 were 2.2 times greater if a video was viewed (p = 0.03). The interaction of crowding and procedure videos was not significant (the use of videos increased the average score by 0.24 in times of crowding compared to times of non-crowding, p = 0.19). Conclusions: Use of procedural videos was associated with EP perception of improved quality of teaching provided around procedures. While EPs rated the quality of their teaching as improved overall, the effect of videos on teaching quality was the same in crowded settings as it was in non-crowded setting. 展开更多

Background: The world of cancer care is an emotional place, given the severity of the disease being treated, the heavy workload, the suffering of patients, and the high number of deaths. The nursing staff who work there may be exposed to constant stress from these factors. This study aimed to explore the psychological experience of the nursing staff of the Medical Oncology Department of the Yaoundé General Hospital. Methods: We carried out a cross-sectional study, in June 2017, involving the nursing staff of the Medical Oncology Department of the Yaoundé General Hospital. Results: Sixteen out of 18 nurses were interviewed. All had worked in at least one other department prior to Oncology. The number of years of experience in Oncology ranged from less than one year to 13 years. The choice of the Oncology department was involuntary for 100% of the staff;of these, 62% had intentions of changing departments. Twenty-five percent had applied for a change of department, which was unsuccessful. Stress was expressed by 94% of the staff;93% thought that there was a solution to the stress experienced on a daily basis in the Oncology department. The main solutions proposed to manage this stress were: support for the staff, provision of work materials, and staff training. Conclusion: The nursing staff of the Medical Oncology Department of the Yaoundé General Hospital experiences a high level of stress and are in need of solutions. 展开更多

BACKGROUND: Cancer patients frequently visit the emergency department (ED) with various symptoms of cancer. The purpose of this study was to determine the clinical characteristics and 1-year survival rate of cancer patients in the ED of a university hospital. METHODS: We conducted a retrospective review of 408 cancer patients who visited the ED between January 2011 and December 2011. Patient information on demographics, chief complaints, findings, and survival was gathered from the hospital registry and corresponding health administration. RESULTS: The study included 240 (58.8%) males and 168 (41.2%) females with a median age of 57.9 years (range 19-87). Regarding cancer staging, 266 patients (65.3%) had metastatic disease and 142 (34.7%) had local and Ioco-regional disease. The hospitalization rate was 59.6%. The most common symptoms were shortness of breath (23.2%), pain (17.8%), fever (14.2%), and nausea/vomiting (14.4%). The most common cancer sites were the lung (32.5%), gastrointestinal system (25.4%), and breast (9.3%). The initial evaluation determined progressive disease (42.4%), chemotherapy effects (20.7%), infections (17.2%), radiotherapy effects (4.7%), extravasation (1.8%), anemia (1.4%), and unknown (11.3%). During follow up, 191 (46.8%) patients died after admission to the ED. The 1-year overall survival of all patients was 7.3 months. CONCLUSIONS: Symptom management in cancer patients is a complex multifaceted concern for the emergency physician. Because of the increasing prevalence of cancer patients, emergency physicians should develop consensus algorithms in collaboration with the relevant disciplines to manage the commonly encountered problems. 展开更多

Objective:To explore the effects of different infusion schemes on colloid osmotic pressure during major abdominal surgery and perioperative albumin in elderly patients.Methods:140 elderly patients of 65-80 years old undergoing major abdominal surgery were divided into 4 groups according to the method of random number table,and different proportions of crystals and colloids were given to different groups(group A:full crystal,group B:crystal-colloid ratio 1:1,group C:crystal-colloid ratio 2:1,Group D:crystalcolloid ratio 1:2).The plasma colloid osmotic pressure and level of albumin during perioperative period were monitored.Simultaneously observe the arterial blood pH,blood glucose,extubating time of endotracheal tube,postoperative feeding time,et al.Results:The total amount of liquid infusion in the group of whole crystal was 3056ml(3056±253),which was significantly more than other groups(P<0.01).At the same time,the colloid osmotic pressure decreased by 11.9 mmHg(11.9±2.8),which was more obviously decreased than that of the other groups(B group3.9±1.3,C group 1.5±0.3,D group 4.7±2.1).The difference was a statistically significant(P<0.01).On the next day after surgery,the level of albumin decreased by an average of 4.3 g/L(4.5±1.9)compared with that in group B before surgery,and group C decreased by 2.9 g/L(2.9±1.2)in average,which was significantly different(P<0.05)from group A 10.2 g/L(10.2±1.8).There was no statistically significant difference between group A and group D(P>0.05).And the other indexes were not significantly different between the two groups.Conclusion:This study found that different infusion solutions with different crystal-colloid ratios had an effect on perioperative colloid osmotic pressure and level of albumin. 展开更多

目的 探讨MiniQuest教学模式在感染科实习医生教学中的应用。方法 选择2020年9月—2021年6月本校60名感染科实习医生作为研究对象,按不同年级分为对照组(2020级)和干预组(2021级)两组,每组各30名。对照组采用传统教学方法,干预组采用Min... 目的 探讨MiniQuest教学模式在感染科实习医生教学中的应用。方法 选择2020年9月—2021年6月本校60名感染科实习医生作为研究对象,按不同年级分为对照组(2020级)和干预组(2021级)两组,每组各30名。对照组采用传统教学方法,干预组采用MiniQuest教学方法。比较两组中文版评判性思维能力测量量表、自我效能量表、团队凝聚力测量问卷、教学效果评价问卷、考试成绩。结果 干预后,干预组实习医生的寻找真相、开放思想、分析能力、系统化能评判性思想的自信心、求知欲和认知成熟度及总分得分高于对照组,差异有统计学意义(P<0.05);干预后,干预组实习医生的自我效能评分明显高于对照组,差异有统计学意义(P<0.05);干预后,干预组实习医生的共乐群性、自律性、归属性、价值性及总分评分明显低于对照组,差异有统计学意义(P<0.05);干预组实习医生喜欢此种教学方式占90.00%,能掌握重点内容、能提高问题分析能力及能提高团结合作能力均占86.67%,能提高评判性思维能力和能提高自主学习能力均占83.33%,能提高操作技能占80.00%,能提高语言表达能力、能提高人际沟通能力及能提高学习兴趣均占76.67%;干预组实习医生的理论成绩和操作成绩均高于对照组,差异有统计学意义(P<0.05)。结论 MiniQuest教学模式能提高感染科实习医生评判性思维能力、自主学习能力、实习医生教育水平及核心竞争能力。 展开更多

目的采用迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)联合临床技能操作评分量表(direct observation of procedural skills,DOPS)的形成性评价,探讨其在推拿科住院医师规范化培训教学中的应用价值。方法选取2022年6... 目的采用迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)联合临床技能操作评分量表(direct observation of procedural skills,DOPS)的形成性评价,探讨其在推拿科住院医师规范化培训教学中的应用价值。方法选取2022年6月—2023年6月在安徽中医药大学第一附属医院推拿科进行住院医师规范化培训的60名住院医师规范化培训学员,由获得安徽中医药大学住院医师规范化培训师资证书的教师进行临床规范化教学。在每期住培学员入科后1周及出科前利用Mini-CEX和DOPS评分量表各测评1次,通过对考核成绩、教师和学员互评满意度进行数据分析,评价形成性评价的临床教学效果。结果Mini-CEX量表的出科前考核成绩与入科后考核成绩[(54.95±4.17)分vs.(53.93±3.65)分]比较,差异均有统计学意义(P<0.05)。DOPS评分量表的出科前考核成绩与入科后考核成绩[(87.00±8.32)分vs.(84.97±6.75)分]比较,差异均有统计学意义(P<0.05)。出科前考核教师对评估结果的满意度分值高于入科后,出科前考核住培学员对评估结果的满意度分值高于入科后,差异均有统计学意义(P<0.05)。结论Mini-CEX结合DOPS评分量表作为推拿科住院医师规范化培训的形成性评价工具效果良好。 展开更多

目的探究Seminar-以案例为导向的教学法(case-based learning,CBL)结合模拟教学法在肿瘤康复教学中的效果。方法采取随机数字表法对2018年6月—2020年6月在滨州医学院附属医院肿瘤科学习的120名医学生,每组各60名,对照组给予常规教学法... 目的探究Seminar-以案例为导向的教学法(case-based learning,CBL)结合模拟教学法在肿瘤康复教学中的效果。方法采取随机数字表法对2018年6月—2020年6月在滨州医学院附属医院肿瘤科学习的120名医学生,每组各60名,对照组给予常规教学法,观察组给予Seminar-CBL结合模拟教学法,对比2组学生实施不同教学方法后的操作成绩及理论成绩、学生自我评价结果、实施教学方法前后的批判性思维能力评分以及沟通能力评分。结果与对照组相比,观察组学生操作成绩及理论成绩均较高,差异有统计学意义(P<0.05)。自主学习能力及自主思考能力显著提升、对肿瘤康复知识与技术有更深的掌握、合作性学习能力提升、利用学习资源的能力提升、批判性思维能力基本形成、制定科学合理康复方案的能力提升、发现及解决问题的能力提升、对临床病例及临床实习知识理解更加深入的学生所占比例较高,差异有统计学意义(P<0.05)。2组教学后与教学前相比,批判性思维能力评分以及沟通能力评分均较高,与对照组教学后相比,观察组批判性思维能力评分以及沟通能力评分均较高,差异有统计学意义(P<0.05)。结论Seminar-CBL结合模拟教学在肿瘤康复教学中可明显提高教学成绩,学生自我评价结果更好,同时也提高了其综合能力。 展开更多

目的评价Seminar教学法用于风湿免疫科的教学效果。方法选取2020年3月—2021年3月在内蒙古科技大学包头医学院第一附属医院风湿免疫科实习的62名学生记作参照组,予以常规教学法;选取2021年4月—2022年4月在本院风湿免疫科实习的63名学... 目的评价Seminar教学法用于风湿免疫科的教学效果。方法选取2020年3月—2021年3月在内蒙古科技大学包头医学院第一附属医院风湿免疫科实习的62名学生记作参照组,予以常规教学法;选取2021年4月—2022年4月在本院风湿免疫科实习的63名学生记作研究组,予以Seminar教学法。对比两组教学效果。结果研究组的专科知识掌握评分、自我能力提升评分、自主学习能力评分、临床思维能力评分高于参照组,差异有统计学意义(P均<0.05)。研究组教学满意度(96.83%)高于参照组(85.48%),差异有统计学意义(χ^(2)=5.008,P<0.05)。结论Seminar教学法可提升风湿免疫科实习生的专科知识掌握度,提升其自我能力,培养其自主学习力与临床思维,具有较高的教学满意度。 展开更多
