Rehabilitation is a medical discipline which is needed wherever people have lost full functionality, and it is needed increasingly in noncommunicable diseases, such as neurological problems, but also, and perhaps more...Rehabilitation is a medical discipline which is needed wherever people have lost full functionality, and it is needed increasingly in noncommunicable diseases, such as neurological problems, but also, and perhaps more importantly, in middle- and high-income countries, where populations are growing more aged. The burden is borne by the health care systems, especially where family cannot take the full load of care. Rehabilitation is also a discipline which is carried out most effectively in multi- or interdisciplinary teams, which carry their own weight of learning and group tensions. They are also bound to consider best practice guidelines. Furthermore, there is also a wide gap between learning in college or university and practice in the ward which leads to a loss of confidence in newly trained medical and paramedical personnel such as doctors, nurses, physiatrists, occupational therapists and physiotherapists. There is a genuine need for expert rehabilitation specialists who can mediate within teams as well as between groups and patients and their families. This paper lays out the range of considerations such a rehabilitation therapist must have under their belt. A model is presented. There are also two sets of parameters when using the model in practice. The first is the levels at which it works: hospital, departmental and individual. Then there are the specific challenges. Curriculum practice guidelines work at the hospital and departmental and professional levels;teamwork with its intrinsic challenges is of paramount importance;finally, the individuals will learn how to represent their insights in writing, most specifically represented in full-blown case reports. METHODS: A survey of 3462 employees was conducted to generate responses to Sexton's Safety At...AIM: To understand how work climate and related factors influence teamwork and collaboration in a large medical center. METHODS: A survey of 3462 employees was conducted to generate responses to Sexton's Safety Attitudes Questionnaire(SAQ) to assess perceptions of work environment via a series of five-point, Likert-scaled questions. Path analysis was performed, using teamwork(TW) and collaboration(CO) as endogenous variables. The exogenous variables are effective communication(EC), safety culture(SC), job satisfaction(JS), work pressure(PR), and work climate(WC). The measure-ment instruments for the variables or summated subscales are presented. Reliability of each sub-scale are calculated. Alpha Cronbach coefficients are relatively strong: TW(0.81), CO(0.76), EC(0.70), SC(0.83), JS(0.91), WP(0.85), and WC(0.78). Confirmatory factor analysis was performed for each of these constructs. RESULTS: Path analysis enables to identify statistically significant predictors of two endogenous variables, teamwork and intra-organizational collaboration. Significant amounts of variance in perceived teamwork(R2 = 0.59) and in collaboration(R2 = 0.75) are accounted for by the predictor variables. In the initial model, safety culture is the most important predictor of perceived teamwork, with a β weight of 0.51, and work climate is the most significant predictor of collaboration, with a β weight of 0.84. After eliminating statistically insignificant causal paths and allowing correlated predictors1, the revised model shows that work climate is the only predictor positively influencing both teamwork(β = 0.26) and collaboration(β = 0.88). A relatively weak positive(β = 0.14) but statistically significant relationship exists between teamwork and collaboration when the effects of other predictors are simultaneously controlled.CONCLUSION: Hospital executives who are interested in improving collaboration should assess the work climate to ensure that employees are operating in a setting conducive to intra-organizational collaboration.展开更多
AIM To describe the development and implementation of a person-centered endoscopy safety checklist and to evaluate the effects of a "checklist intervention".METHODS The checklist,based on previously publishe...AIM To describe the development and implementation of a person-centered endoscopy safety checklist and to evaluate the effects of a "checklist intervention".METHODS The checklist,based on previously published safety checklists,was developed and locally adapted,taking patient safety aspects into consideration and using a person-centered approach. This novel checklist was introduced to the staff of an endoscopy unit at a Stockholm University Hospital during half-day seminars and team training sessions. Structured observations of the endoscopy team's performance were conducted before and after the introduction of the checklist. In addition,questionnaires focusing on patient participation,collaboration climate,and patient safety issues were collected from patients and staff. RESULTS A person-centered safety checklist was developed and introduced by a multi-professional group in the endoscopy unit. A statistically significant increase in accurate patient identity verification by the physicians was noted(from 0% at baseline to 87% after 10 mo,P < 0.001),and remained high among nurses(93% at baseline vs 96% after 10 mo,P = nonsignificant). Observations indicated that the professional staff made frequent attempts to use the checklist,but compliance was suboptimal: All items in the observed nurse-led "summaries" were included in 56% of these interactions,and physicians participated by directly facing the patient in 50% of the interactions. On the questionnaires administered to the staff,items regarding collaboration and the importance of patient participation were rated more highly after the introduction of the checklist,but this did not result in statistical significance(P = 0.07/P = 0.08). The patients rated almost all items as very high both before and after the introduction of the checklist;hence,no statistical difference was noted.CONCLUSION The intervention led to increased patient identity verification by physicians-a patient safety improvement. Clear evidence of enhanced person-centeredness or team work was not found.展开更多

Several studies have reported the negative effect of physician burnout and the adverse effect it has on the individual and health institutions. This study therefore examined the moderating effect of perceived organizational support on the relationship between teamwork and physician burnout in the Chinese context. Using a survey data consisting of a sample of physicians from hospitals within the Jiangsu Province of China, we estimated three models in a hierarchical framework and identified that in the presence of respondent’s demographic characteristics as control variables, perceived organizational support, play a very vital role in improving teamwork while reducing burnout among physicians.展开更多

As referenced in a previo...Disruptive behaviors in health care can have a significant adverse effect on staff interactions that can negatively impact staff satisfaction, staff performance, and patient outcomes of care. As referenced in a previously published article, the Obstetrics and Gynecology specialty is one of the service areas where these behaviors occur more frequently. Despite growing evidence of the ill effects of these types ofbehaviors many organizations are still having a difficult time in addressing these issues in an effective manner. Gaining a better understanding of the nature, causes, and impact of these behaviors is crucial to finding the right remedies for solution. Nobody intentionally starts the day planning to be disruptive, it's just that things get in the way. A combination of deep seated factors related to age and gender preferences, culture and ethnicity, life experiences, and other events that help shape values, attitudes and personalities, and more external factors related to training, environmental pressures, stress and burnout, and other personal issues all contribute to the mix. Given the complexities of today's health care environment, each person needs to recognize the importance of being held accountable for appropriate actions and behaviors that affect work relationships and care coordination that impact patient care. Early recognition, early intervention, and taking a pro-active supportive approach to improve individual behaviors will result in better relationships, less disruption, more satisfaction, and better outcomes of care.展开更多
