全髋置换术后远程居家康复和住院康复疗效比较

目的 比较远程居家康复和传统住院康复对老年全髋关节置换术患者的疗效。方法 本试验于2021年1月—2022年12月于上海市第一康复医院进行,受试者均于上海交通大学医学院附属第六人民医院和同济大学附属第十人民医院完成全髋关节置换手术... 目的 比较远程居家康复和传统住院康复对老年全髋关节置换术患者的疗效。方法 本试验于2021年1月—2022年12月于上海市第一康复医院进行,受试者均于上海交通大学医学院附属第六人民医院和同济大学附属第十人民医院完成全髋关节置换手术(手术时间为2021年1月—2022年6月),康复干预于上海市第一康复医院完成。共64例患者被纳入该项单盲、平行、非劣效性随机对照试验,受试者被随机分为远程居家康复组(远程组)和住院康复组(住院组)。试验组与对照组各32人,于术后1周入组。试验组(远程组)的康复干预包括为期4周(每周5次×45 min)的远程视频电话指导与移动APP康复训练跟练,在干预后接受每2周1次的电话随访直至12周随访结束,并使用APP继续居家训练直至12周随访结束;对照组(住院组)接受4周(每周5次×45 min)的常规线下物理治疗干预,此外不接受其他康复相关治疗(如针灸、理疗和作业治疗等),出院前物理治疗师布置训练作业并以纸质方式交付患者及家属,出院后接受电话随访,训练频率与远程组一致。本试验的评估数据采集分别在入组时、干预结束(入组4周)及随访结束(入组12周)进行,主要观察指标为随访结束时的髋关节功能障碍和骨性关节炎结局评分(HOOS)的生活质量亚量表(QOL)评分,次要结局指标为入组4周和12周时HOOS的其他亚量表评分、起立行走计时测试(TUG)、Berg平衡测试(BBS)和12项生活质量调查简表(SF-12)评分以及入组4周的HOOS评分。结果 远程组与住院组在人口统计学与临床特征上无显著差异。在随访12周结束时的评估中,两组的HOOS各亚量表评分(经初始基线值调整后)差异无统计学意义(P>0.05);两组的TUG和BBS结果均有显著提升,但组间差异无显著性(P>0.05);在各个随访时间点,两组在躯体和精神方面均没有差异(P>0.05)。结论 远程居家康复有利于促进老年全髋关节置换术后的髋关节功能恢复,且疗效与住院康复无显著差异,这为全髋关节置换术后患者的康复提供了新的选择。 展开更多

目的·评价基于智能可穿戴设备BPMpathway的居家康复方案在全膝关节置换(total knee arthroplasty,TKA)术后患者中的应用效果。方法·采用方便抽样法,选取苏州大学附属第四医院(苏州市独墅湖医院)和苏州大学附属第一医院骨科202... 目的·评价基于智能可穿戴设备BPMpathway的居家康复方案在全膝关节置换(total knee arthroplasty,TKA)术后患者中的应用效果。方法·采用方便抽样法,选取苏州大学附属第四医院(苏州市独墅湖医院)和苏州大学附属第一医院骨科2021年4月—2022年12月收治的膝关节骨关节炎并接受TKA术的患者作为研究对象。按照患者入院顺序,将住院号尾数为奇数的患者纳入对照组,住院号尾数为偶数的患者纳入干预组。对照组患者给予常规居家康复训练方案,干预组在对照组的基础上实施基于BPMpathway的远程居家康复训练方案。分别于基线及术后1、3、6个月采用骨科患者功能锻炼依从性量表、美国特种外科医院膝关节评分表(Hospital for Special Surgery Scale,HSS)和36项简明健康状况调查表(36-item Short Form Health Survey,SF-36)对2组患者进行测评,同时测量患者的膝关节活动度。结果·最终纳入110例患者,其中对照组56例,干预组54例。2组患者的一般资料差异无统计学意义。2组患者基线时的膝关节活动度、HSS评分、SF-36评分差异均无统计学意义。重复测量方差分析结果显示:2组患者膝关节活动度、功能锻炼依从性评分、HSS评分及SF-36评分的组间效应、时间效应及交互效应均具有统计学意义(均P<0.05)。术后1、3、6个月干预组的膝关节活动度、功能锻炼依从性评分、HSS评分及SF-36评分均高于对照组(均P<0.05)。结论·医护人员基于智能可穿戴设备BPMpathway开展的远程康复训练方案,有助于提高TKA术后患者居家康复期间功能锻炼的依从性,改善膝关节活动度,促使膝关节功能恢复,提高其生存质量,对患者预后具有积极的作用。 展开更多

目的探讨基于家庭远程康复(HTR)平台的康复训练运动方案对脑卒中患者肢体功能恢复和认知功能的作用。方法前瞻性选取2020年6月至2022年7月于陕西省康复医院接受治疗且符合脑卒中临床诊断标准的100例患者作为研究对象,所有患者均自愿出院... 目的探讨基于家庭远程康复(HTR)平台的康复训练运动方案对脑卒中患者肢体功能恢复和认知功能的作用。方法前瞻性选取2020年6月至2022年7月于陕西省康复医院接受治疗且符合脑卒中临床诊断标准的100例患者作为研究对象,所有患者均自愿出院,选择居家康复治疗,按照随机数表法分为观察组和对照组各50例。对照组患者采用常规护理,观察组患者在对照组的基础上采用基于HTR平台的康复训练运动方案。分别于入组时和出院1、3个月时,采用Fugl-Meyer运动功能量表(FMA)、Berg平衡能力量表(BBS)比较两组患者肢体功能恢复情况;采用美国国立卫生院卒中量表(NIHSS)比较两组患者认知功能恢复状况;采用改良的Barthel指数(MBI)及脑卒中专用生活质量量表(SS-QOL)比较两组患者日常生活质量情况;出院康复训练3个月回院复查时,采用自制调查问卷比较两组患者的护理满意度。结果入组时,两组患者的FMA、BBS评分比较差异均无统计学意义(P>0.05);干预1、3个月后,两组各评分均升高,且观察组患者的FMA、BBS评分分别为(62.88±4.93)分、(85.76±10.54)分,(32.88±4.23)分、(44.43±5.43)分,明显高于对照组的(50.32±4.53)分、(74.38±9.73)分、(25.32±4.56)分、(38.72±2.47)分,差异均有统计学意义(P<0.05);入组时,两组患者的NIHSS、MBI评分比较差异均无统计学意义(P>0.05);干预后1、3个月后,两组NIHSS评分均降低MBI评分均升高,组内比较差异均有统计学意义(P<0.05),且观察组患者的NIHSS评分分别为(15.88±4.34)分、(6.13±0.23)分,明显低于对照组的(19.32±4.53)分、(9.82±1.63)分,MBI评分分别为(49.88±4.56)分、(67.43±7.13)分,明显高于对照组的(48.32±4.73)分、(61.72±2.63)分,差异均具有统计学意义(P<0.05);入组时,两组患者的SS-QOL评分比较差异无统计学意义(P>0.05),干预后1、3个月后,两组各评分均升高,组内比较差异有统计学意义(P<0.05),且观察组患者的SS-QOL评分分别为(149.88±4.88)分、(136.32±4.53)分,明显高于对照组的(190.63±3.23)分、(176.10±6.63)分,差异均有统计学意义(P<0.05);干预3个月后,观察组患者的满意度为(96.62±4.22)分,明显高于对照组的(89.47±4.21)分,差异有统计学意义(P<0.05)。结论基于HTR平台的康复训练运动方案可显著改善脑卒中患者肢体功能恢复、认知功能,促进患者康复,具有较高的临床应用价值。 展开更多

多媒体远程会诊在医联体儿科疾病诊治中的应用与探讨

目的探讨远程会诊在基层医院儿科的应用价值。方法收集我院2018年7月至2022年3月开展的167例远程会诊病例,对病例资料进行回顾性调查并统计分析。结果会诊患儿中危重病例87例(52.1%),极危重病例24(14.4%),疑难病例48例(28.7%),一般病例8... 目的探讨远程会诊在基层医院儿科的应用价值。方法收集我院2018年7月至2022年3月开展的167例远程会诊病例,对病例资料进行回顾性调查并统计分析。结果会诊患儿中危重病例87例(52.1%),极危重病例24(14.4%),疑难病例48例(28.7%),一般病例8例(4.8%)。采用远程视频会诊的病例有144例(86.2%),采用远程机器人床边会诊的病例有23例(13.8%)。会诊后患者治愈或好转出院的154例(92.2%),转上级医院进一步诊治的9例(5.4%),因疾病死亡或者家属放弃治疗的4例(2.4%)。结论远程会诊方式多样化有利于会诊质量的提高,实现危重疑难患儿属地治疗,推动儿科医联体建设。 展开更多

Telemedicine is an emerging field in recent medical achievements with rapid development. The "smartphone" availability has increased in both developed and developing counffies even among people in rural and remotes areas. Telebased services can be used for screening ophthalmic diseases and also monitoring patients with known diseases. Electronic ophthalmologic records of the patients including captured images by smartphones from anterior and posterior segments of the eye will be evaluated by ophthalmologists, and if patients require further evaluations, they will be referred to experts in the relevant field. Eye diseases such as cataract, glaucoma, age-related macular degeneration, diabetic retinopathy, and retinopathy of prematurity are the most common causes of blindness in many countries and beneficial use of teleophthalmology with smartphones will be a good way to achieve the aim of VISION 2020 all over the world. Numerous studies have shown that teleophthalmology is similar to the conventional eye care system in clinical outcomes and even provides more patient satisfaction as it saves time and cost, This review explains how teleophthalmology helps to improve patient outcomes through smartphones. 展开更多

A new hybrid event based control architecture for tele-robotic systems controlled through the Internet is pro-posed in this paper. Different from the traditional event based control method, the new framework does not require every part of the system to be strictly event synchronized. Instead, it allows time referenced control components to be integrated into this framework, which makes it more convenient to develop Internet based control systems. Since there are two reference variables, time and event, in this architecture, how to coordinate these components with different references to keep the stability of the whole system is discussed in detail in this paper. To verify this new idea, an experiment was conducted to control the end effector of a PUMA robot tracking a continuous state trajectory given on-line by the remote operator. Optical fiber has been used as typical physical transmission medium in Colombian high power substations for effective communication between substations and control centers. However, suita- ble transmissions of tele-protection services continues to be a challenging area in the electrical- supply sector since link stability as well as various performance tests suggest that link recurrence requirements still exhibit limitations when using Metroethernet networks in this context. At pre- sent, improvements have been achieved via proprietary protocols such as Mirrored Bits, which matches the performance of a tele-protection link transmission under normal operation conditions. However, this imposes commercial limitations on the Colombian electrical-supply sector since proprietary protocols hinder fair competition among manufacturers. This is the rationale behind conducting trials with precision time protocol as it is defined in IEEE 1588 standard. The main purpose is to validate whether, under real operation conditions, it is possible to substitute such tele-protection links via Metroethernet networks, yet considering a standard protocol that allows incorporating various pieces of equipment from different manufacturers into the system as well as providing common benefits derived from using this type of communication networks. The present work provides technical results obtained after conducting various simulated tele-trigger tests, including stress tests, where results-stability validation occurs through network-saturation simulations. These saturation conditions are achieved by incorporating additional traffic, which involves several switching events over link paths and permits validating tele-protection-response timing when specifically using IEEE 1588 precision time protocol. Based on the aforementioned reasons as well as on the resulting benefits, a series of laboratory tests have been defined in order to evaluate delay and availability on tele-protection services when transported over IP networks. 展开更多

This study used TELES to simulate the earthquake scenario parameters, such as PGA distribution, casualty case, liquefaction potential index and the total number of occurring fires based on the Meinong fault seismic source. Finally, according to the most damaging areas of the simulation results, we propose to strengthen the important buildings and facilities such as the reservoir area, and the emergency responsive hospitals for a better seismic risk estimation and the earthquake disaster prevention and response. According to the simulation results analysis, we found the areas with the strongest magnitude were in Meinong Dist. in Kaohsiung city. Nevertheless, the areas suffering the serious disasters were in Tainan city. Due to the serious disasters occurring in Tainan city, it is crucial to discuss how to perform anti-earthquake practices in Tainan area and how to coordinate the support from different cities and counties. 展开更多

This paper describes the development of a new ECG tele-monitoring method and system based on the embedded web server. The system consists of ECG recorders with network interface and the embedded web server, internet networks and computers, with the system operating on browser/server(B/S) mode. The ECG recorder was designed by ARM9 (S3C2410X) and embedded operating system (Linux). Once the ECG recorder has been connected to the internet network, medical experts can use the internet to access the server of the ECG recorder, monitor ECG signals, and diagnose patients by browsing the dynamic web pages in the embedded web server. The experimental results reveal that the designed system is stable, reliable, and suitable for the use in real-time ECG tele-monitoring for both family and community health care. 展开更多

Building an Internet based tele-robotic system requires the developers beingproficient in computer theory, network technology, control theory, etc. A flexible, open structuredand module based framework for tele-control system is proposed on the basis of abstraction andanalysis of existing Internet-based control systems. This framework is designed following the peerto peer (P2P) distributed computing model. As a key to the system, the XML based ontology ofresources/modules/peers is discussed, so do the model for dynamically allocating of resources, basedon which the coordination among modules or peers can then be implemented.
