Core competencies and perceived training needs of nurses in the background of Internet plus nursing service:a qualitative descriptive study

Background:The Internet plus nursing service program is being piloted in China,which has become a new home nursing service model led by nurses.To enable safe and effective homecare service delivery,nurses working in the program need a high level of competency.However,the content of these core competencies and the training needs of nurses for Internet plus nursing services are still unknown.Aim:To describe nurses’perceptions of core competencies and training needs to work in the Internet plus nursing service program,thereby providing a reference for the development of training programs.Methods:A qualitative descriptive study using semi-structured interviews was conducted on 15 nurses with experience of working in the Internet plus nursing service program.Interviews were audio-recorded and transcribed verbatim,and the data were analyzed using thematic analysis.Results:Core competencies involved comprehensive assessment competence,direct care practice competence,health education and consulting competence,risk estimation and response competence,and communication competence.Training needs involved complex operational items,knowledge of chronic disease management,professional communication,risk identification and response,nursing standards,norms,and procedures,and utilization of information technology.Conclusions:The training system may be developed based on nurses’core competencies and their training needs to promote professional development of the Internet plus nursing service.

Reliability Modelling and Analysis of Redundant Systems Connected to Supporting External Device for Operation Attended by a Repairman and Repairable Service Station

In this paper, probabilistic models for three redundant configurations have been developed to analyze and compare some reliability characteristics. Each system is connected to a repairable supporting external device for operation. Repairable service station is provided for immediate repair of failed unit. Explicit expressions for mean time to system failure and steady-state availability for the three configurations are developed. Furthermore, we compare the three configurations based on their reliability characteristics and found that configuration II is more reliable and efficient than the remaining configurations.

Factors Affecting Patient Satisfaction with Community Health Service under the Gatekeeper System:A Cross-sectional Study in Nanjing,China

The gatekeeper policy has been implemented for approximately ten years on a pilot population in China. It is necessary to assess the satisfaction of patients utilizing community health service（CHS） under the gatekeeper system. Our study showed that the cognition of gatekeeper policy was associated with four dimensions including doctor‐patient relationships, information and support, organization of care, and accessibility（P 〈 0.001）. One or more factors such as gender and self‐perceived health scores also affected their satisfaction. General practitioners must be prepared to focus on these aspects of information and support, organization of care, and accessibility as indicators of potential opportunities for improvement. Additionally, policymakers can improve patients＇ satisfaction with CHS by strengthening their awareness of the gatekeeper policy.

Improved Ant Colony Algorithm for Vehicle Scheduling Problem in Airport Ground Service Support

Support vehicles are part of the main body of airport ground operations,and their scheduling efficiency directly impacts flight delays.A mathematical model is constructed and the responsiveness of support vehicles for current operational demands is proposed to study optimization algorithms for vehicle scheduling.The model is based on the constraint relationship of the initial operation time,time window,and gate position distribution,which gives an improvement to the ant colony algorithm(ACO).The impacts of the improved ACO as used for support vehicle optimization are compared and analyzed.The results show that the scheduling scheme of refueling trucks based on the improved ACO can reduce flight delays caused by refueling operations by 56.87%,indicating the improved ACO can improve support vehicle scheduling.Besides,the improved ACO can jump out of local optima,which can balance the working time of refueling trucks.This research optimizes the scheduling scheme of support vehicles under the existing conditions of airports,which has practical significance to fully utilize ground service resources,improve the efficiency of airport ground operations,and effectively reduce flight delays caused by ground service support.

Light-Weighted Decision Support Framework for Selecting Cloud Service Providers

Multi-criteria decision making(MCDM)is a technique used to achieve better outcomes for some complex business-related problems,whereby the selection of the best alternative can be made in as many cases as possible.This paper proposes a model,the multi-criteria decision support method,that allows both service providers and consumers to maximize their profits while preserving the best matching process for resource allocation and task scheduling.The increasing number of service providers with different service provision capabilities creates an issue for consumers seeking to select the best service provider.Each consumer seeks a service provider based on various preferences,such as price,service quality,and time to complete the tasks.In the literature,the problem is viewed from different perspectives,such as investigating how to enhance task scheduling and the resource allocation process,improve consumers’trust,and deal with network problems.This paper offers a novel model that considers the preferences of both service providers and consumers to find the best available service provider for each consumer.First,the model adopts the best-worst method(BWM)to gather and prioritize tasks based on consumers’and service providers’preferences.Then,the model calculates and matches similarities between the sets of tasks from the consumer’s side with the sets of tasks from the provider’s side to select the best service provider for each consumer using the two proposed algorithms.The complexity of the two algorithms is found to be O(n3).

Location Service Information Supporting System Based on Ontology

In order to solve the problem of how to collect and manage large amount of location information and improve the accuracy of location service, interactive information supporting system based on ontology and location service is presented in this paper. It can transmit special information about users＇ location to front-end instantaneously, so it is very convenient for their daily life. It makes full use of the available wireless communication, which is easy to arrange and convenient for users.

Donors’ Quality of Life after Live Liver Transplantation and Social Support: A Correlation Study

Objective: Donors’ quality of life (QOL) and social support were investigated in this study. Method: The SF-36 scale and social support scale were used to survey the quality of life and social support of donors of live liver transplantation. Result: The PCS (Physical Correlated Score) is 84.78 ± 13.21, the MCS (Mental Correlated Score) is 80.71 ± 14.65, and the total score is 165.49 ± 22.63. The results show the significant difference at GH (General Health) & VT (Vitality) domain regarding the age, P < 0.05;while there’s a significant difference at BP (Body Pain) domain regarding the sex, P < 0.05. The social support score of male & female donors are 37.75 ± 7.50 and 44.00 ± 4.30, and the difference is significant, P < 0.05. The MCS and the social support score are correlated, the correlation factor is 0.483, P < 0.05. Conclusion: There’s no significant difference between the donors and the normal population. The age is the risk factor of QOL, and it should be paid more attention when we select the donor. Excellent social support relates to high quality of life.

Reduction of the incidence and mortality of rectal cancer bypolypectomy:a prospective cohort study in Haining County

AIM: To reduce the incidence and morlality of rectal cancerand address the hypothesis that colorectal cancer often arisefrom precursor lesion (s), either adenomas or non-alenomatous polyps, by conducting a population-basedmass screening for colorectal cancer in Haining County,Zhejiang, PRCMETHODS: From 1977 to 1980, physicians screened thepopulation of Haining County using 15 cm rigid endoscopy.Of over 240 000 participants, 4076 of them were diagnoselwith Precursor Iesions, eitler alenomas or non-adenomatous polyps, which were then removedl surgically.All individuals with precursor lesionswere followed up andreexamined by endoscopy every two to five years upto 1998.RESULTS: After the initial screening, 953 metactronousadenomas and 417 non-adenomatous polyps ware detected andremoved from the members of this cohort. Further, 27 cases ofcolorectal cancer were detected and treated. Log-rank tuestsshowed that the survival time among those cancer patients whounderwent mass screening increased significantly comparedtothat of other colorectal cancer patients (P ＜ 0.0001). Accordingto the population-based cancer registry in Haining County,age-adjusted incidence and mortality of rectal cancer decreasedby 41% and 29 % from 1977-1981 to 1992-1996, respectively.Observed cumulative 20-year rectal cancer incidence was 31%lower than the expected in the screened group; the mortalitydue to rectal cancer was 18 % lower thah the expected in thescreened group.CONCLUSION: Mass screening for rectal cancer andprecursor lesions with protocoscopy in the generalpopulation and periodical following-up with routineendoscopy for high-risk patients may decrease both theincidence and mortality of rectal cancer.

In the paper, a solution of one dimensional fore region pressure build up is put forward. The performance of spring supported thrust bearing is carried out with 3 dimensional thermo elasto hydrodynamic (TEHD) lubrication theory inclusive of inlet pressure build up, thermal elastic distortion of pad and thermal effect. The effects of fore region pressure build up and the variation of some operating conditions on the performance of the pad are studied.

The Role of Mutual Support Groups for the Control of Diabetes in a Mexican City: Achievements and Limitations from the Patients’ Perspective

Background: Mutual Support Groups (MSG) provides to the patient, the ability to effective self-management behaviors, such as taking prescribed medications, following diet and exercise regimens, self-monitoring, and coping emotionally with the rigors of living with diabetes. Physicians, nurses and health promoters from public primary Health Care Centers (HCC) are responsible for providing follow-up care through MSGs. However, although the MSG program has been carried out, in the last decade Mexico presents the most alarming statistics in the prevalence and complications of Diabetes Mellitus type II (DM-II), suggesting a low impact of MSG in the strategy to support the effective control of the disease. Objetive: The aim of this work was to assess whether knowledge or benefits of effective management to control of DM-II, also to identify strengths and limitations of MSGs, in six different Health Care Centers (HCC), in San Luis Potosi, Mexico. This research provides an overview of patients’ perception, and significant issues that demand to adjust MGSs strategies, with the goal of exceed the expectations of current health statistics. Methods: A qualitative evaluation was carried out, with an ethnographic approach design. The study included 28 diabetic persons, (21 women and 7 men from ruraland urban carecenters). Semi-structured interviews, non-participant observation, and structured content analysis were used. Results: Support groups give patients a way to address emotional issues, and learn about their disease and self-care, although some patients consider participation an inconvenient obligation. Support group users mention barriers such as lack of continuity in support group activities, inconvenient meeting times, and the difficulty of commuting to attend group sessions. They also mention that overworked health care workers find it difficult to provide leadership to keep the group going. These issues have multiple implications for the success of strategies to control the disease. Conclusions: This study shows the need to apply a participatory model to disease support group strategies to reorganize their actions in such a way as to meet the needs and requirements of patients and to ensure their participation and help them control their disease.

Attitude of Health Care Providers towards Adolescent Sexual and Reproductive Health Services in Developing Countries: A Systematic Review

The aim of the review was to synthesise the best available evidence regarding attitude of health care providers towards adolescent sexual and reproductive health services in developing countries. The review was conducted following qualitative research method. An online search of Cumulative Index to Nursing and Allied Health Literature (CINAHL), British Nursing Index (BNI), EMBASE and MEDLINE databases was conducted to identify relevant studies for the review. There were nine studies that met the inclusion criteria and these were critically assessed by two independent reviewers using the standardised Joanna Briggs Institute (JBI) critical appraisal forms. Data were extracted using the standardised JBI data extraction forms. A narrative synthesis was done on the findings. Key findings from the review indicate that unprofessional attitude of health care professionals and lack of youth friendly reproductive health services, inhibit adolescents from gaining access to sexual and reproductive health services in developing countries. It is recommended that youth friendly reproductive health services be provided to increase uptake of re-productive health services by adolescents.
