This procedure was successful in 80% of cases, and the peripheral anterior syenchiae did not exist until one year later. Nonetheless, there is little evidence of its efficacy in our context. Our study aims to investigate the efficacy of goniosynechialysis during phacoemulsification in patients with primary angle closure disease and concomitant cataract. Methods: This was an observational, prospective study. The intra-ocular pressure, need for anti-glaucoma drugs, visual acuity, the extent of synechiae, anterior chamber depth, surgical success rate, and other indicators were monitored for at least three months following surgery. Results: This study included 114 patients (118 eyes), 61 with chronic angle closure glaucoma (51.69%), 33 with primary angle closure (27.97%), and 24 with acute attack angle closure (20.34%), who were surgically treated with phacoemulsification and goniosynechialysis (Phaco-GSL). The mean intra-ocular pressure had significantly decreased three months after surgery (pre- vs post-op: 22.04 ± 10.86 vs 15.41 ± 6.06 mmHg, p-value p-value p-value p-value p-value Conclusion: Regardless of the type of glaucoma, combined phacoemulsification-goniosynechialysis is effective in lowering pressure, restoring vision, reducing the need for anti-glaucoma drugs, and preventing the synechial recurrence. Objective This study aimed to evaluate the ability of a digital fundus camera to observe the development of the anterior chamber angle(ACA)in premature infants.Methods Forty-eight eyes of preterm infants(n=48)were exa...Objective This study aimed to evaluate the ability of a digital fundus camera to observe the development of the anterior chamber angle(ACA)in premature infants.Methods Forty-eight eyes of preterm infants(n=48)were examined by a digital fundus camera to observe the development of the ACA.ACA grading was performed based on the visualization of the anterior chamber structures according to the Scheie Angle Depth Evaluating System.Results ACA images from all 48 infants were successfully acquired using RetCam3.The corrected gestational age ranged from 30 weeks to 49 weeks,which covered the period from 2 months preterm to>2 months post-term.As the corrected gestational age increased,the corrected gestational age grading was significantly decreased.The mean corrected gestational ages of the infants corresponding to the ACA classification from grade IV to grade 0 were 32.75±1.89,37.20±1.30,39.75±2.38,40.56±2.24,and 44.23±2.14 weeks,respectively,which were all significantly different(P<0.05).The regression analysis showed a linear correlation between the grading of the ACA and the corrected gestational age(R2=0.724,P=0.0001).Conclusion The ACA of a full-term newborn can be fully detected and evaluated by a digital fundus camera.For premature infants,part of the ACA is not visible physiologically;however,it should not be misdiagnosed as angle closure or a narrow angle.展开更多

Lens extraction has been proposed as a method of deepening anterior chamber and managing intraocular pressure. Purpose: To assess changes in anterior chamber depth after phacoemulsification with intraocular lens implantation in narrow angle eyes. Design: Prospective controlled trial (before-after) study. Method: The study was conducted from March 2015 to August 2017 among the patients of department of Ophthalmology of BSMMU who were diagnosed as cataract with narrow angles. Anterior chamber angle grading of 2 or less (Shaffer grading) in 3 or more quadrants was considered narrow angle (NA). The purposive type sampling technique was applied to collect sample from the study population, as per inclusion and exclusion criteria. Complete clinical evaluation including history, physical examination, relevant ocular examinations and systemic examinations were performed. In this prospective study, subjects underwent phacoemulsification with foldable lens implantation. A scan ultrasonography was performed preoperatively and 10<sup>th</sup> and 30<sup>th</sup> postoperative days of surgery. Results: Thirty eyes of 29 patients included in the study, male: female ratio was 1:1, with an overall mean age of 62.03 ± 8.95 years. The mean preoperative central ACD was 2.95 ± 0.35 mm. At 10<sup>th</sup> and 30<sup>th</sup> POD mean central ACD were 3.94 ± 0.32, and 3.92 ± 0.28 mm respectively. Mean of increase in central ACD at final follow-up was 0.96 mm (p Conclusion: Phacoemulsification with posterior chamber in bag lens implantation can deepen the anterior chamber depth in patients with narrow angles. BACKGROUND Glaucoma is an irreversible optic neuropathy with the loss of visual field and decrease of vision.The variable clinical manifestations may result in differential diagnostic difficulties.The early screening ...BACKGROUND Glaucoma is an irreversible optic neuropathy with the loss of visual field and decrease of vision.The variable clinical manifestations may result in differential diagnostic difficulties.The early screening and diagnosis of glaucoma are currently experiencing a demand for anterior segment analysis tools that can gather more information with one short measurement.Therefore,we analyzed the agreement,difference,and correlation of chamber angle parameters such as angel opening distance at 500μm(AOD500)and trabeculo-iris space area at 500μm^2(TISA500)measured by anterior segment optical coherence tomography(ASOCT)and ultrasound biomicroscopy(UBM).AIM To compare the differences,correlation,and agreement in measuring AOD500 and TISA500 by AS-OCT and UBM.METHODS Both AS-OCT and UBM were performed to measure AOD500 and TISA500 in 45 subjects(72 eyes).All subjects without glaucoma were collected from October 2015 to August 2016 at the Ophthalmology Department of the Fourth Affiliated Hospital of China Medical University.Data of the two groups(AOD500 and TISA500)were compared by nonparametric tests.Pearson correlative analysis and Bland-Altman analysis were used to compare the correlation and agreement.RESULTS There were no significant differences between AS-OCT and UBM in measuring AOD500(P1=0.110,P2=0.633,P3=0.078,and P4=0.474)and TISA500(P1=0.584,P2=0.889,P3=0.297,and P4=0.550)of the four quadrants of the anterior chamber angle.There was a high correlation in measuring AOD500(r1=0.562,r2=0.671,r3=0.635,and r4=0.720;P<0.001)and TISA500(r1=0.584,r2=0.889,r3=0.297,and r4=0.550;P<0.001).There was a good agreement in measuring AOD500 and TISA500 by the two modalities.CONCLUSION There is a high correlation and agreement between AOD500 and TISA500 measurements by AS-OCT and UBM.They are interchangeable under some circumstances.AS-OCT proves to be a better early screening tool for glaucoma.展开更多

Totally 14 eyes of primary chronic angle-closure glaucoma(PCACG), 12 eyes of lens-induced secondary chronic angleclosure glaucoma(LSACG) and 14 healthy eyes were recruited. After miosis, for PCACG group, intraocular pressure(IOP) and anterior chamber depth(ACD) changed not significantly, while anterior chamber angle widened significantly. LSACG group showed a significant increase in IOP, decrease in ACD, and narrowing in anterior chamber angle. Healthy eyes showed significant decreases in IOP and anterior chamber parameters. Thus, miosis could widen the anterior chamber angle of patients with PCACG, while increase the narrowing of anterior chamber angle and IOP of patients with LSACG. We should pay attention to the distinction between PCACG and LSACG patients and the proper administration of pilocarpine in the treatment of patients with chronic ACG.展开更多

Twenty eyes in group A received the conventional approach by using toothed forceps through corneal limbus incision, and 22 eyes in group B underwent the novel method through the opposite corneal limbus incision. RESULTS:The success rate of ACFB once removal was 75%(15/20) in group A, and 100%(22/22) in group B. The average operation time of group A was significantly longer compared with group B(34.9±9.88 min vs 22.13±8.85min; P〈0.05). The average size of corneal limbus incision in group A was significantly larger than that of group B(4.85±1.89 mm vs 3.95±1.17 mm; P〈0.05). The corneal limbus incision suturing were conducted in all eyes in group A, and only 5 eyes in group B. CONCLUSION:Removal of ACFB using a prism contact lens and 23-gauge foreign body forceps is a safer, more effective, and convenient technique compared with the conventional approach.展开更多
Purpose:To evaluate the relationship between the position of the focal adjustment knob of a fundus camera and refractive error and biometric data as measured in the same eye.Methods:Normal eyes of patients presenting ...Purpose:To evaluate the relationship between the position of the focal adjustment knob of a fundus camera and refractive error and biometric data as measured in the same eye.Methods:Normal eyes of patients presenting to clinics at the Beijing Tongren Hospital were examined with a non-mydriatic fundus camera.The position on the focal scale of a knob adjusting the distance between the camera lens and film plane,used to adjust focus the image of the patients fundus relative to the refractive power of the eye,was recorded in degrees.Ocular biometry and refractometry were performed on the same eyes.Results:The study included 136 subjects with a mean age of 36.5 ±19.6 years and a mean refractive error of-1.31 ±2.77 diopters.In univariate analysis,the position of the adjustment knob was significantly associated with refractive error.(P < 0.001;correlation coefficient r=-0.77),axial length.(P<0.001;r=0.65) and anterior chamber depth (P<0.001;r=0.48).After adjustment for age,anterior chamber depth decreased by 0.01 mm(95% confidence interval:0.003,0.017) for change per degree in the position of the adjustment knob.Conclusion:A fundus camera can be used to estimate anterior chamber depth,axial length and refractive error.In a screening setting,a fundus camera operated by a technician may be helpful to detect a shallow anterior chamber and evaluate a potential risk factor for primary angle closure.展开更多

Aim: To investigate variation in ACA measurement between gonioscopy, van Herick technique, anterior segment optical coherence tomography (AS-OCT) and Sirius Scheimpflug-Camera both nasally and temporally. Method: The ACA of 50 eyes of 25 healthy subjects was measured with gonioscopy, van Herick technique, AS-OCT and Sirius Scheimpflug-Camera. The angle was measured both nasally and temporally. Results: No statistically significant difference could be found between gonioscopy, van Herick technique and AS-OCT either nasally or temporally. The Sirius Scheimpflug-Camera on the other hand showed statistically significant difference to gonioscopy (p < 0.0001) both nasally (p = 0.03, p = 0.001, p < 0.0001) and temporally (p = 0.0002, p = 0.001, p Conclusion: This study showed good agreement between three of the four techniques. ACA measurements obtained by the Sirius Scheimpflug-Camera should therefore not be considered interchangeable with those obtained by the remaining three methods.展开更多
AIM: To compare the safety and effectiveness of phacotrabeculectomy versus sequential surgery in chronic angle-closure glaucoma(CACG) with coexisting cataract.·METHODS: One hundred and sixty-two CACG patients...AIM: To compare the safety and effectiveness of phacotrabeculectomy versus sequential surgery in chronic angle-closure glaucoma(CACG) with coexisting cataract.·METHODS: One hundred and sixty-two CACG patients(162 eyes) were retrospectively analyzed. Of them, 87patients(87 eyes) in group A had underwent phacotrabeculectomy with intraocular lens(IOL)implantation, and 75 patients(75 eyes) in group B had underwent sequential surgery with IOL implanted. Best-corrected visual acuity(BCVA), intraocular pressure(IOP), complications and anterior chamber angle(ACA)were measured.· RESULTS: Demographic characteristics of the two groups were similar. A mean follow-up period was 15±6mo(range 13 to 24mo), a mean IOP of 12.14 ±5.32 mm Hg in group A and 11.38 ±4.06 mm Hg in group B(P =0.84) at the last follow up. The Kaplan-Meier analysis revealed that the cumulative probability of success in both groups was similar(P =0.61). Anterior uveitis and hypotony were the most common complications in group A, whereas group B experienced shallow anterior chamber with trabeculectomy. With the exception of anterior uveitis, no complications occurred to 11 trabeculectomized eyes. All postoperative measurements of anterior chamber showed statistically significant differences in each group according to the preoperative data(P〈 0.05). However,fewer changes occurred in group B than in group A.· CONCLUSION: Phacotrabeculectomy and sequential surgery exhibit similar IOP reduction, visual recovery,and complications when treating CACG patients with cataract. However, for a wider ACA, phacotrabeculectomy has demonstrated higher effectiveness than sequential surgery.展开更多

METHODS: Th...AIM: To investigate whether the gene variants in MYOC and ABCA1 are associated with primary angle-closure glaucoma(PACG) and anterior chamber depth(ACD) and axial length(AL) in samples from northern China. METHODS: The present case-control association study consisted of 500 PACG patients and 720 unrelated controls. Each participant was genotyped for eleven single nucleotide polymorphisms(SNPs) in MYOC and ABCA1 genes(rs12076134, rs183532, rs235875 and rs235913 in MYOC, rs2422493, rs2487042, rs2472496, rs2472493, rs2487032, rs2472459 and rs2472519 near ABCA1) using an improved multiplex ligation detection reaction(iMLDR) technique. The genetic association analyses were performed by PLINK using a logistic regression model. The association between genotypes and ocular biometric parameters was performed by SPSS using generalized estimation equation. Bonferroni corrections wereimplemented and the statistical power was calculated by the Power and Sample Size Calculation. RESULTS: Two SNPs rs183532 and rs235875 as well as a haplotype TTC in MYOC were nominally associated with PACG despite the significance was lost after Bonferroni correction. No association was observed between ABCA1 and PACG, neither did the association between these variants and ACD as well as AL. CONCLUSION: The present study suggests MYOC and ABCA1 do not play a part in the pathogenesis of PACG as well as the regulation of ocular biometric parameters in a northern Chinese population. Further investigations with large sample size are needed to verify this consequence.展开更多
AIM:To compare changes in anterior segment topography and axial length(AL)evaluated with Pentacam and IOL Master after pars plana vitrectomy(PPV)performed with and without vitreous base shaving.METHODS:This prospectiv...AIM:To compare changes in anterior segment topography and axial length(AL)evaluated with Pentacam and IOL Master after pars plana vitrectomy(PPV)performed with and without vitreous base shaving.METHODS:This prospective study included patients who underwent PPV or phacoemulsification+PPV(Phaco+PPV)for various indications.Patients who underwent total posterior hyaloid detachment and excessive vitreous base shaving with scleral indentation were referred to as complete PPV(c-PPV).The patients whom posterior hyaloid was separated as far as the posterior arcades and vitreous base shaving with scleral depression was not performed were classified as the partial PPV(p-PPV)group.All patients underwent detailed ophthalmologic examinations preoperatively and lwk,I,and 3mo postoperatively.Changes in the anterior chamber depth(ACD),anterior chamber volume(ACV),iridocorneal angle(ICA)f central corneal thickness(Cd),and keratometric measurements(K1 and K2)were evaluated with Pentacam HR.Changes in the AL measurements were analyzed with IOL Master.RESULTS:A significant increase in ACD was observed in c-PPV cases(P=0.02),but this increase was not significant in the p-PPV group(P=0.053).In contrast,ICA increased significantly in the c-PPV group(P=0.02)but decreased in the p-PPV group(P=0.09).BCVA was significantly improved in the c-PPV group from week 1(P<0.001)while the increase in the p-PPV group reached significance at 3mo(P=0.035).Cd increased in the first week and later returned to baseline in both groups.No significant differences in the other parameters were observed between the groups,and there were no significant changes in intraocular pressure,ACV,AL,K1 or K2 values(P>0.05 for all).CONCLUSION:Incomplete posterior hyaloid excision and not removing the vitreous base in PPV surgeries may create a more stable anterior chamber,thus preventing the downward movement of the lens-iris diaphragm,and may cause ciliary body retraction,thereby reducing ICA.Awareness of these effects can provide some amount of guidance to physicians in selecting the appropriate PPV procedure and preempting surgical complications.展开更多

Methods:.The anterior chamber depth (ACD), and lens thickness (LT), of 150 normal subjects (150 eyes) was measured by anterior segment optical coherence tomography (AS- OCT)..Gender differences were assessed by independent t-test,.and correlation analysis was used to examine the effect of age. Results:.The mean values of ACD and LT were 2.69±0.32 mm and 4.85±0.43 mm, respectively. Women had a significantly lower Mean ACD as compared to men (2.56±0.33 mm vs 2.85±0.29 mm; P﹤0.05). No statistically significant difference was found in LT between male and female subjects (P>0.05)..Correlation analysis findings suggest that LT increases with age (r=0.83,P﹤0.05), and that ACD decreases with age (r=-0.57,P﹤0.05). After controlling for LT, no significant correlation was observed between age and ACD (P>0.05). Data was collected from charts of...AIM: To assess the refractive status, anterior chamber depth (ACD) and axial length (AL) of patients with primary angle-closure disease (PACD). METHODS: Retrospective cohort. Data was collected from charts of all PACD patients treated from April 2013 to December 2015. Analysis was done on 137 patient charts with complete biometric data. Patient demographics, PACD type, refractive status (spherical equivalent), ACD and AL were studied. RESULTS: The median age of 137 subjects [53 with primary angle-closure suspects (PACS), 27 with primary angle- closure (PAC) and 57 with primary angle-closure glaucoma (PACG)] was 68y (range 21-88y). The majority was Chinese (n=68; 49.6%) and most of them were women (n=75; 54.7%). The distribution of myopia (n=51; 37.2%) and hyperopia (n=49; 35.8%) was similar. The ACD was shallower in myopes compared to hyperopes (P=-0.02) and emmetropia (P=-0.049) but the AL was not significantly different between groups. There were no patients blind from PACG. CONCLUSION: Both myopia and hyperopia can occur in PACD. Despite a shallower ACD in angle closure myopes, the AL was not different between groups.展开更多

文摘Objective This study aimed to evaluate the ability of a digital fundus camera to observe the development of the anterior chamber angle(ACA)in premature infants.Methods Forty-eight eyes of preterm infants(n=48)were examined by a digital fundus camera to observe the development of the ACA.ACA grading was performed based on the visualization of the anterior chamber structures according to the Scheie Angle Depth Evaluating System.Results ACA images from all 48 infants were successfully acquired using RetCam3.The corrected gestational age ranged from 30 weeks to 49 weeks,which covered the period from 2 months preterm to>2 months post-term.As the corrected gestational age increased,the corrected gestational age grading was significantly decreased.The mean corrected gestational ages of the infants corresponding to the ACA classification from grade IV to grade 0 were 32.75±1.89,37.20±1.30,39.75±2.38,40.56±2.24,and 44.23±2.14 weeks,respectively,which were all significantly different(P<0.05).The regression analysis showed a linear correlation between the grading of the ACA and the corrected gestational age(R2=0.724,P=0.0001).Conclusion The ACA of a full-term newborn can be fully detected and evaluated by a digital fundus camera.For premature infants,part of the ACA is not visible physiologically;however,it should not be misdiagnosed as angle closure or a narrow angle.
文摘Background: The burden of cataract and glaucoma has been increasing. Primary angle closure occurs as a result of crowded anterior segment anatomy causing appositional contact between peripheral iris and trabecular meshwork. Lens extraction has been proposed as a method of deepening anterior chamber and managing intraocular pressure. Purpose: To assess changes in anterior chamber depth after phacoemulsification with intraocular lens implantation in narrow angle eyes. Design: Prospective controlled trial (before-after) study. Method: The study was conducted from March 2015 to August 2017 among the patients of department of Ophthalmology of BSMMU who were diagnosed as cataract with narrow angles. Anterior chamber angle grading of 2 or less (Shaffer grading) in 3 or more quadrants was considered narrow angle (NA). The purposive type sampling technique was applied to collect sample from the study population, as per inclusion and exclusion criteria. Complete clinical evaluation including history, physical examination, relevant ocular examinations and systemic examinations were performed. In this prospective study, subjects underwent phacoemulsification with foldable lens implantation. A scan ultrasonography was performed preoperatively and 10<sup>th</sup> and 30<sup>th</sup> postoperative days of surgery. Results: Thirty eyes of 29 patients included in the study, male: female ratio was 1:1, with an overall mean age of 62.03 ± 8.95 years. The mean preoperative central ACD was 2.95 ± 0.35 mm. At 10<sup>th</sup> and 30<sup>th</sup> POD mean central ACD were 3.94 ± 0.32, and 3.92 ± 0.28 mm respectively. Mean of increase in central ACD at final follow-up was 0.96 mm (p Conclusion: Phacoemulsification with posterior chamber in bag lens implantation can deepen the anterior chamber depth in patients with narrow angles. Based on these findings, it is concluded that phacoemulsification with foldable intraocular lens implantation is an effective tool in deepening the anterior chamber.
文摘BACKGROUND Glaucoma is an irreversible optic neuropathy with the loss of visual field and decrease of vision.The variable clinical manifestations may result in differential diagnostic difficulties.The early screening and diagnosis of glaucoma are currently experiencing a demand for anterior segment analysis tools that can gather more information with one short measurement.Therefore,we analyzed the agreement,difference,and correlation of chamber angle parameters such as angel opening distance at 500μm(AOD500)and trabeculo-iris space area at 500μm^2(TISA500)measured by anterior segment optical coherence tomography(ASOCT)and ultrasound biomicroscopy(UBM).AIM To compare the differences,correlation,and agreement in measuring AOD500 and TISA500 by AS-OCT and UBM.METHODS Both AS-OCT and UBM were performed to measure AOD500 and TISA500 in 45 subjects(72 eyes).All subjects without glaucoma were collected from October 2015 to August 2016 at the Ophthalmology Department of the Fourth Affiliated Hospital of China Medical University.Data of the two groups(AOD500 and TISA500)were compared by nonparametric tests.Pearson correlative analysis and Bland-Altman analysis were used to compare the correlation and agreement.RESULTS There were no significant differences between AS-OCT and UBM in measuring AOD500(P1=0.110,P2=0.633,P3=0.078,and P4=0.474)and TISA500(P1=0.584,P2=0.889,P3=0.297,and P4=0.550)of the four quadrants of the anterior chamber angle.There was a high correlation in measuring AOD500(r1=0.562,r2=0.671,r3=0.635,and r4=0.720;P<0.001)and TISA500(r1=0.584,r2=0.889,r3=0.297,and r4=0.550;P<0.001).There was a good agreement in measuring AOD500 and TISA500 by the two modalities.CONCLUSION There is a high correlation and agreement between AOD500 and TISA500 measurements by AS-OCT and UBM.They are interchangeable under some circumstances.AS-OCT proves to be a better early screening tool for glaucoma.

文摘To evaluate post-miosis changes in the anterior chamber structures in various angle-closure glaucomas(ACG). Totally 14 eyes of primary chronic angle-closure glaucoma(PCACG), 12 eyes of lens-induced secondary chronic angleclosure glaucoma(LSACG) and 14 healthy eyes were recruited. After miosis, for PCACG group, intraocular pressure(IOP) and anterior chamber depth(ACD) changed not significantly, while anterior chamber angle widened significantly. LSACG group showed a significant increase in IOP, decrease in ACD, and narrowing in anterior chamber angle. Healthy eyes showed significant decreases in IOP and anterior chamber parameters. Thus, miosis could widen the anterior chamber angle of patients with PCACG, while increase the narrowing of anterior chamber angle and IOP of patients with LSACG. We should pay attention to the distinction between PCACG and LSACG patients and the proper administration of pilocarpine in the treatment of patients with chronic ACG.
文摘AIM:To introduce a novel approach in removal of anterior chamber angle foreign body(ACFB) using a prism contact lens and 23-gauge foreign body forceps.METHODS:Data of 42 eyes of 42 patients who had undergone removal of ACFB using a prism contact lens and 23-gauge foreign body forceps from January 2008 to October 2013 were collected and analyzed. Twenty eyes in group A received the conventional approach by using toothed forceps through corneal limbus incision, and 22 eyes in group B underwent the novel method through the opposite corneal limbus incision. RESULTS:The success rate of ACFB once removal was 75%(15/20) in group A, and 100%(22/22) in group B. The average operation time of group A was significantly longer compared with group B(34.9±9.88 min vs 22.13±8.85min; P〈0.05). The average size of corneal limbus incision in group A was significantly larger than that of group B(4.85±1.89 mm vs 3.95±1.17 mm; P〈0.05). The corneal limbus incision suturing were conducted in all eyes in group A, and only 5 eyes in group B. CONCLUSION:Removal of ACFB using a prism contact lens and 23-gauge foreign body forceps is a safer, more effective, and convenient technique compared with the conventional approach.
文摘Purpose:To evaluate the relationship between the position of the focal adjustment knob of a fundus camera and refractive error and biometric data as measured in the same eye.Methods:Normal eyes of patients presenting to clinics at the Beijing Tongren Hospital were examined with a non-mydriatic fundus camera.The position on the focal scale of a knob adjusting the distance between the camera lens and film plane,used to adjust focus the image of the patients fundus relative to the refractive power of the eye,was recorded in degrees.Ocular biometry and refractometry were performed on the same eyes.Results:The study included 136 subjects with a mean age of 36.5 ±19.6 years and a mean refractive error of-1.31 ±2.77 diopters.In univariate analysis,the position of the adjustment knob was significantly associated with refractive error.(P < 0.001;correlation coefficient r=-0.77),axial length.(P<0.001;r=0.65) and anterior chamber depth (P<0.001;r=0.48).After adjustment for age,anterior chamber depth decreased by 0.01 mm(95% confidence interval:0.003,0.017) for change per degree in the position of the adjustment knob.Conclusion:A fundus camera can be used to estimate anterior chamber depth,axial length and refractive error.In a screening setting,a fundus camera operated by a technician may be helpful to detect a shallow anterior chamber and evaluate a potential risk factor for primary angle closure.
文摘Background: Anterior chamber angle (ACA) can be measured by many different techniques. In order for a technique to be a part of the routine eye examination, it has to be quick and easy in good agreement with gonioscopy both nasally and temporally. Aim: To investigate variation in ACA measurement between gonioscopy, van Herick technique, anterior segment optical coherence tomography (AS-OCT) and Sirius Scheimpflug-Camera both nasally and temporally. Method: The ACA of 50 eyes of 25 healthy subjects was measured with gonioscopy, van Herick technique, AS-OCT and Sirius Scheimpflug-Camera. The angle was measured both nasally and temporally. Results: No statistically significant difference could be found between gonioscopy, van Herick technique and AS-OCT either nasally or temporally. The Sirius Scheimpflug-Camera on the other hand showed statistically significant difference to gonioscopy (p < 0.0001) both nasally (p = 0.03, p = 0.001, p < 0.0001) and temporally (p = 0.0002, p = 0.001, p Conclusion: This study showed good agreement between three of the four techniques. ACA measurements obtained by the Sirius Scheimpflug-Camera should therefore not be considered interchangeable with those obtained by the remaining three methods.

文摘AIM: To compare the safety and effectiveness of phacotrabeculectomy versus sequential surgery in chronic angle-closure glaucoma(CACG) with coexisting cataract.·METHODS: One hundred and sixty-two CACG patients(162 eyes) were retrospectively analyzed. Of them, 87patients(87 eyes) in group A had underwent phacotrabeculectomy with intraocular lens(IOL)implantation, and 75 patients(75 eyes) in group B had underwent sequential surgery with IOL implanted. Best-corrected visual acuity(BCVA), intraocular pressure(IOP), complications and anterior chamber angle(ACA)were measured.· RESULTS: Demographic characteristics of the two groups were similar. A mean follow-up period was 15±6mo(range 13 to 24mo), a mean IOP of 12.14 ±5.32 mm Hg in group A and 11.38 ±4.06 mm Hg in group B(P =0.84) at the last follow up. The Kaplan-Meier analysis revealed that the cumulative probability of success in both groups was similar(P =0.61). Anterior uveitis and hypotony were the most common complications in group A, whereas group B experienced shallow anterior chamber with trabeculectomy. With the exception of anterior uveitis, no complications occurred to 11 trabeculectomized eyes. All postoperative measurements of anterior chamber showed statistically significant differences in each group according to the preoperative data(P〈 0.05). However,fewer changes occurred in group B than in group A.· CONCLUSION: Phacotrabeculectomy and sequential surgery exhibit similar IOP reduction, visual recovery,and complications when treating CACG patients with cataract. However, for a wider ACA, phacotrabeculectomy has demonstrated higher effectiveness than sequential surgery.
文摘Ultrasonic biometry was done in 232 normal eyes and 138 eyes with primary angle closure glaucoma (ACG), using Ultrascan Digital B System IV (10 MHz). 文摘AIM: To investigate whether the gene variants in MYOC and ABCA1 are associated with primary angle-closure glaucoma(PACG) and anterior chamber depth(ACD) and axial length(AL) in samples from northern China. METHODS: The present case-control association study consisted of 500 PACG patients and 720 unrelated controls. Each participant was genotyped for eleven single nucleotide polymorphisms(SNPs) in MYOC and ABCA1 genes(rs12076134, rs183532, rs235875 and rs235913 in MYOC, rs2422493, rs2487042, rs2472496, rs2472493, rs2487032, rs2472459 and rs2472519 near ABCA1) using an improved multiplex ligation detection reaction(iMLDR) technique. The genetic association analyses were performed by PLINK using a logistic regression model. The association between genotypes and ocular biometric parameters was performed by SPSS using generalized estimation equation. Bonferroni corrections wereimplemented and the statistical power was calculated by the Power and Sample Size Calculation. RESULTS: Two SNPs rs183532 and rs235875 as well as a haplotype TTC in MYOC were nominally associated with PACG despite the significance was lost after Bonferroni correction. No association was observed between ABCA1 and PACG, neither did the association between these variants and ACD as well as AL. CONCLUSION: The present study suggests MYOC and ABCA1 do not play a part in the pathogenesis of PACG as well as the regulation of ocular biometric parameters in a northern Chinese population. Further investigations with large sample size are needed to verify this consequence.
文摘AIM:To compare changes in anterior segment topography and axial length(AL)evaluated with Pentacam and IOL Master after pars plana vitrectomy(PPV)performed with and without vitreous base shaving.METHODS:This prospective study included patients who underwent PPV or phacoemulsification+PPV(Phaco+PPV)for various indications.Patients who underwent total posterior hyaloid detachment and excessive vitreous base shaving with scleral indentation were referred to as complete PPV(c-PPV).The patients whom posterior hyaloid was separated as far as the posterior arcades and vitreous base shaving with scleral depression was not performed were classified as the partial PPV(p-PPV)group.All patients underwent detailed ophthalmologic examinations preoperatively and lwk,I,and 3mo postoperatively.Changes in the anterior chamber depth(ACD),anterior chamber volume(ACV),iridocorneal angle(ICA)f central corneal thickness(Cd),and keratometric measurements(K1 and K2)were evaluated with Pentacam HR.Changes in the AL measurements were analyzed with IOL Master.RESULTS:A significant increase in ACD was observed in c-PPV cases(P=0.02),but this increase was not significant in the p-PPV group(P=0.053).In contrast,ICA increased significantly in the c-PPV group(P=0.02)but decreased in the p-PPV group(P=0.09).BCVA was significantly improved in the c-PPV group from week 1(P<0.001)while the increase in the p-PPV group reached significance at 3mo(P=0.035).Cd increased in the first week and later returned to baseline in both groups.No significant differences in the other parameters were observed between the groups,and there were no significant changes in intraocular pressure,ACV,AL,K1 or K2 values(P>0.05 for all).CONCLUSION:Incomplete posterior hyaloid excision and not removing the vitreous base in PPV surgeries may create a more stable anterior chamber,thus preventing the downward movement of the lens-iris diaphragm,and may cause ciliary body retraction,thereby reducing ICA.Awareness of these effects can provide some amount of guidance to physicians in selecting the appropriate PPV procedure and preempting surgical complications.

文摘Purpose:.To investigate gender differences in lens thickness (LT) and central anterior chamber depth (ACD) in normal subjects,.and to assess age associated changes in these measures. Methods:.The anterior chamber depth (ACD), and lens thickness (LT), of 150 normal subjects (150 eyes) was measured by anterior segment optical coherence tomography (AS- OCT)..Gender differences were assessed by independent t-test,.and correlation analysis was used to examine the effect of age. Results:.The mean values of ACD and LT were 2.69±0.32 mm and 4.85±0.43 mm, respectively. Women had a significantly lower Mean ACD as compared to men (2.56±0.33 mm vs 2.85±0.29 mm; P﹤0.05). No statistically significant difference was found in LT between male and female subjects (P>0.05)..Correlation analysis findings suggest that LT increases with age (r=0.83,P﹤0.05), and that ACD decreases with age (r=-0.57,P﹤0.05). After controlling for LT, no significant correlation was observed between age and ACD (P>0.05). Conclusion:.The ACD of female subjects was,.on average, shallower than that of their male counterparts..Aging was associated with increasing LT,.and the observed narrowing of ACD with age,.might be partially mediated by the increasing LT.
文摘AIM: To assess the refractive status, anterior chamber depth (ACD) and axial length (AL) of patients with primary angle-closure disease (PACD). METHODS: Retrospective cohort. Data was collected from charts of all PACD patients treated from April 2013 to December 2015. Analysis was done on 137 patient charts with complete biometric data. Patient demographics, PACD type, refractive status (spherical equivalent), ACD and AL were studied. RESULTS: The median age of 137 subjects [53 with primary angle-closure suspects (PACS), 27 with primary angle- closure (PAC) and 57 with primary angle-closure glaucoma (PACG)] was 68y (range 21-88y). The majority was Chinese (n=68; 49.6%) and most of them were women (n=75; 54.7%). The distribution of myopia (n=51; 37.2%) and hyperopia (n=49; 35.8%) was similar. The ACD was shallower in myopes compared to hyperopes (P=-0.02) and emmetropia (P=-0.049) but the AL was not significantly different between groups. There were no patients blind from PACG. CONCLUSION: Both myopia and hyperopia can occur in PACD. Despite a shallower ACD in angle closure myopes, the AL was not different between groups.
文摘Purpose:To determine the prevalence of primary angle-closure glaucoma(PACG)in Doumen Countty,Guangdong province.Methods:Four villages from eight districts in doumen were selected by stratified,cluster and random sampling.Of a total of 1055people aged 45years or over,932(88.62)were examined.The examination and diagnostic criterion were similar to those of shunyi survey.Results:The prevalence of PACGin subjects over 45year of eye is0.64%,in those of shunyi survey.Results:The prevalence of PACG in subjects over 45year of eye is 0.64%,in those over 50years of age is 0.85%in Doumen,which is significantly lower than the corresponding figure of 1.99%inshunyi(P<0.05).Conclusion This study identifies the prevalence of PACG in Doumen is lower than in Shunyi.
文摘目的:探讨在Van Herick法基础上更方便准确评估前房角宽度的方法。方法:纳入2021-01/12于我院就诊的年龄相关性白内障患者58例69眼,参考Van Herick法分为房角宽度≥1/2颞侧角膜厚度(CT)组(37例44眼)和<1/2CT组(21例25眼),应用超声生物显微镜测量中央前房深度和周边房角度数。结果:房角宽度≥1/2CT组和<1/2CT组患者中央前房深度有明显差异(2.64±0.27 mm vs 2.23±0.29 mm,P<0.01),且两组间上方、颞侧、下方和鼻侧象限房角度数均有明显差异(P<0.01)。房角宽度≥1/2CT组患者上方与下方象限房角度数无显著差异(P>0.05),其余各象限房角度数均有差异(P<0.05);房角宽度<1/2CT组患者上方与鼻、颞侧象限,下方与颞侧象限房角度数均有差异(P<0.05)。结论:裂隙灯下采用Van Herick法评估颞侧房角宽度,同时评估下方象限房角宽度,可以更简单、快速、准确地评估前房角的整体情况。
