Methods: 36 patients who visited our hospital from January 2020 to September 2023 were selected as research subjects and randomly divided into the study group (18 cases) and the control group (18 cases). The control group adopted routine examination and rehabilitation training, and the study group adopted sports medicine ultrasound and rehabilitation training. The ankle function scores and daily living ability of the two groups were compared before treatment and 9 weeks after treatment. Results: There was no statistically significant difference between the study group and the control group in terms of ankle function scores and ability to perform activities of daily living before treatment (P > 0.05), but the difference was statistically significant after treatment (P < 0.05). Conclusion: Ultrasound can be used as a supplement to magnetic resonance imaging (MRI) of the ankle joint for diagnosing anterior talofibular ligament injury;ultrasound can show the location, distance, and degree of tear of the anterior talofibular ligament rupture and accurately predict muscle atrophy around the ankle joint;ultrasound combined with computed tomography (CT) can better determine the type of intra-articular fracture of the talar subtalar joint, and guide the choice of surgical timing. In addition, ultrasound can be used to determine the severity of peroneal muscle atrophy, assess the progress of early postoperative functional exercise, and guide personalized rehabilitation programs.展开更多

English rugby,American football, and soccer.Basic science studies have demonstrated that a muscle strain injury occurs due to excessive strain in eccentric contraction instead of force, and that elongation speed and duration of activation before eccentric contraction affect the severity of the injury.Hamstring strain injury is likely to occur during the late swing phase and late stance phase of sprint running.Shortened optimum muscle length, lack of muscle flexibility, strength imbalance, insufficient warm-up,fatigue,lower back injury, poor lumbar posture, and increased muscle neural tension have been identified as modifiable risk factors while muscle compositions, age, race, and previous injuries are non-modifiable risk factors. The theoretical basis of some of these risk factors, however,is lacking, and the results of clinical studies on these risk factors are inconsistent.Future studies are needed to establish the cause-and-effect relationships between those proposed risk factors and the injury.展开更多

Brain metastasis(BM) is a common complication of NSCLC, with 25%–40% of patients developing BM during the course of the disease. A significant strategy of local disease control in the central nervous system is radiation therapy. With the development of precision medicine, the concept of treating lung cancer BM has gradually changed. In this case, we performed a surgical procedure to obtain enough tumor tissue for the detection of the target gene and other related experiments after the patient was informed. Finally, we found that the patient had both hepatocyte growth factor receptor(MET) gene amplification and kinesin light chain 1-anaplastic lymphoma kinase fusion(KLC1-ALK) through next-generation sequencing and showed sensitivity to the targeted therapy of crizotinib. The patient exhibited good response. Our case was successful and underwent targeted therapy with the guidance of precise diagnosis.展开更多

Information was obtained from the hospital's trauma registry and medical records were retrospectively reviewed for data pertaining to the injuries.RESULTS: Fifty-four patients were identified. Ninetysix percent of patients were male with a mean age of 47.9 years(range 15-69). The mean Injury Severity Score was 12.53 ± 1.17(range 2-34). The average height of fall was 18.2 feet(range 4-40 feet). All patients fell to the ground with the exception of one who landed on rocks, and many hit the tree or branches on the way down. A reason for the fall was documented in only 13 patients, and included tree stand construction(3), loss of balance(3), falling asleep(3), structural failure(2), safety harness breakage(3) or lightheadedness(1). The most common injuries were spinal fractures(54%), most commonly in the cervical spine(69%), followed by the thoracic(38%) and lumbar(21%) spine. Eight patients required operative repair. Head injuries occurred in 22%. Other systemic injuries include rib/clavicular fractures(47%), pelvic fractures(11%), solid organ injury(23%), and pneumothorax or hemothorax(19%). No patient deaths were reported. The average hospital length of stay was 6.56 ± 1.07 d. Most patients were discharged home without(72%) or with(11%) services and 17% required rehabilitation. CONCLUSION: Falls from hunting tree stands are still common, with a high rate of neurological injury. Compared to a decade ago we have made no progress in preventing these neurological injuries, despite an increase in safety advances. Neurosurgeons must continue to advocate for increased safety awareness and participate in leadership roles to improve outcomes for hunters.展开更多

Descriptive statistics and a logistic regression were completed to analyze data. Results: Factors such as education level(p = 0.05) and number of concussions treated(p = 0.05) predicted use of the stepwise progression, whereas sex(p = 0.17), employment setting(p = 0.17), state law(p = 0.86), and years practicing(p = 0.17) did not predict whether ATs were following the stepwise progression. Conclusion: The majority of the ATs from this study are employing the stepwise progression to safely return athletes to play after sustaining a concussion. This demonstrates that ATs are providing a standard of care for concussed athletes across various athletic training settings; however, having a graduate degree and treating more concussions per year are predictors of whether an AT follows all steps of the stepwise progression.展开更多

Thirty six male (age 33 ± 9) and twenty nine female (age 31 ±8) elite athletes, members of the German national team, were examined in their final training camp preparing for world championship. At two time points blood panels (pre-training and one hour post-training) were collected and concentrations of high sensitive troponin T, N-terminal pro brain natriuretic peptide (NT-pro BNP), ereatine phosphokinase (CPK), MB-creatine kinase (CKMB) and myoglobin were assessed. After exercise, serum levels ofNT-pro BNP, CPK, myoglobin and CKMB increased significantly (P 〈 0.01 for each) with only few values exceeding the upper reference limits. High sensitive troponin T remained below the limit of detection both before and after exercise in all athletes. Significant gender-related differences were found with a higher increase ofNT-pro BNP levels in female athletes (P 〈 0.01) compared to males. In contrast, male athletes displayed a significant higher increase of CPK (P 〈 0.01) and myoglobin (P 〈 0.01) compared to female athletes. In conclusion, three high-intensity training units did not lead to elevated high sensitive troponin T concentrations in elite athletes of dragon boating but to significant increases ofNT-pro BNP, CPK and myoglobin levels. This suggests that high-intensity training units do not lead to a cardiac injury in these athletes.展开更多

Athletic Pubalgia is often multifactorial with several pathologies overlapping to cause groin pain. Soft tissue disruptions at the insertions of adductor longus and rectus abdominis to the pubic symphysis are often encountered. Prolotherapy is the method of injecting an irritant solution into a joint space, ligament or tendon insertion site. The solution is thought to induce a regenerative response in the affected area through several mechanisms, including the induction of inflammation and vascular reconfiguration. Research objectives: The use of prolotherapy to treat Athletic Pubalgia has not been reported. The aim of this study was to assess ultrasound-guided prolotherapy as a therapeutic option in the treatment of Athletic Pubalgia. Methods: A case report, with informed consent (human material or human data were performed in accordance with the Declaration of Helsinki), involving a 24-year-old female presenting with Athletic Pubalgia. The patient presented with severe chronic right groin pain sustained while playing camogie. Conservative management options, including rest and rehabilitation with physiotherapy had failed to control this pain. Through dynamic ultrasound scanning, focal lesions were identified in the insertions of rectus abdominis and adductor longus to the pubic symphysis. Prolotherapy with 5% dextrose to the affected areas was performed. Assessment of analgesic and rehabilitation response was planned. Results: A sustained analgesic effect was observed at clinical follow-up at 6 weeks. Patient returned to full sporting activity. Conclusions: Ultrasound-guided prolotherapy has not yet been described as a treatment option in Athletic Pubalgia. In this case report, it was an effective pain-relieving therapy with improved rehabilitation. Larger studies are required to evaluate its efficacy.展开更多

With the development of medical imaging, information on different modality imaging can be integrated and comprehensively analyzed by imaging fusion system. This review aimed to update the application of multimodality imaging fusion technology in the precise diagnosis and treatment of malignant tumors under the precision medicine plan. We introduced several multimodality imaging fusion technologies and their application to the diagnosis and treatment of malignant tumors in clinical practice. Date Sources: The data cited in this review were obtained mainly from the PubMed database from 1996 to 2016, using the keywords of "precision medicine", "fusion imaging", "multimodality", and "tumor diagnosis and treatment". Study Selection: Original articles, clinical practice, reviews, and other relevant literatures published in English were reviewed. Papers focusing on precision medicine, fusion imaging, multimodality, and tumor diagnosis and treatment were selected. Duplicated papers were excluded. Results: Multimodality imaging fusion technology plays an important role in tumor diagnosis and treatment under the precision medicine plan, such as accurate location, qualitative diagnosis, tumor staging, treatment plan design, and real-time intraoperative monitoring. Multimodality imaging fusion systems could provide more imaging information of tumors from different dimensions and angles, thereby offing strong technical support for the implementation of precision oncology. Conclusion: Under the precision medicine plan, personalized treatment of tumors is a distinct possibility. We believe that multimodality imaging fusion technology will find an increasingly wide application in clinical practice.展开更多

文摘Function-specific homeostasis(FSH)has been defined as a negative-feedback response of a biosystem to maintain its interior function-specific conditions so that the function is perfectly performed. There is no photobiomodulation of intranasal low intensity laser therapy(ILILT)on a function in its FSH,but ILILT could modulate a function far from its FSH.This rehabilitation has been found to be mediated by the ratio of intracellular nicotinamide adenine dinucleotide(NAD+)and its reduced form NADH,NAD+/NADH,and then sirtuin 1(SIRT1).There might be FSH-specific NAD+/NADH(FSN)and SIRT1 activity(FSA).ILILT might enhance NAD+/NADH and SIRT1 activity until they arrive at FSN and FSA,respectively. The NAD+/NADH and SIRT1 activity of related cells of many athletic diseases such as upper respiratory tract infection,asthma,osteoarthritis,exercise-induced muscle damage, wound, traumatic brain injury, and osteoporosis are lower than FSN and FSA,respectively. Therefore, there may be therapeutic effects of ILILT on those athletic diseases.Furthermore, many phenomena and the ILILT mechanism have been integrated to support the prophylaxis effects of ILILT on the swine-origin influenza A(H1N1).

文摘Non-small cell lung cancer(NSCLC) ranks as the leading cause of cancer-related death in the world. Our case was successful and underwent targeted therapy with the guidance of precise diagnosis.
文摘BACKGROUND This case study describes an atypical presentation of avascular necrosis(AVN)of the first metatarsal head, which is largely unfounded in the literature.CASE SUMMARY A healthy 24-year-old female initially presented with pain at the first metatarsophalangeal joint(MTPJ)and was diagnosed with AVN by physical examination and magnetic resonance imaging. The patient demonstrated atypically poor progress in recovery, despite being in otherwise good health and being of young age, with no history of corticosteroid or alcohol use.The patient also did not have any history or clinical features of autoimmune disease or vasculitis, such as systemic lupus erythematosus. The patient was managed with conservative treatment for 18 mo,which allowed for gradual return of full range of motion of the first MTPJ and subsiding pain, permitting the patient to return to highintensity sports training and full weight-bearing.Throughout her recovery, many differential diagnoses were ruled out through specific investigations leading to further reinforcement of the diagnosis of AVN of the 1st metatarsal head.CONCLUSION Atypical AVN may occur with no predisposing risk factors.Treatment is mainly conservative, with unclear guidelines in literature on management.
文摘Platelet-rich plasma (PRP) therapy in orthopedics and sports medicine has seen an expansion in use since 2003. Theobjective of this study is to not only review evidence of clinical effectiveness but also to evaluate its security and detect any disparitybetween its use and its popularity. A prospective, descriptive study, which included 104 athletes were performed. Minor adverse effectswere recorded and these were self-limited. Although there is a lack of more detailed studies which compare other factors, PRP therapyhas been shown to have positive results in relation to certain lesions.
文摘AIM: To characterize and compare our current series of patients to prior reports in order to identify any changes in the incidence of neurological injury related to hunting accidents in Rochester, New York.METHODS: All tree stand-related injuries referred to our regional trauma center from September 2003 through November 2011 were reviewed. Neurosurgeons must continue to advocate for increased safety awareness and participate in leadership roles to improve outcomes for hunters.
文摘Purpose: The purpose of this study was to determine whether practicing athletic trainers(ATs) were using the stepwise progression to make return-to-play(RTP) decisions after concussion and to determine what factors influenced their decision to use the stepwise progression. Methods: A total of 166 ATs(response rate = 16.6%) completed a 21-item questionnaire that evaluated participant demographics, methods of concussion management, and RTP decision-making using the stepwise progression. This demonstrates that ATs are providing a standard of care for concussed athletes across various athletic training settings; however, having a graduate degree and treating more concussions per year are predictors of whether an AT follows all steps of the stepwise progression.

文摘Abstract: This study investigated the effects of three sport-specific high-intensity training units on cardiac biomarker alteration in elite athletes of dragon boating. This suggests that high-intensity training units do not lead to a cardiac injury in these athletes.

文摘Research background: Athletic Pubalgia is a common, debilitating sports condition encountered by athletes who participate in competitive sports requiring pivoting movements. Larger studies are required to evaluate its efficacy.
文摘Objective: The arrival of precision medicine plan brings new opportunities and challenges for patients undergoing precision diagnosis and treatment of malignant tumors. We believe that multimodality imaging fusion technology will find an increasingly wide application in clinical practice.
