The MPL environment was conceived as a physical and conceptual space where students could learn and practice the principles and techniques of working in organizations in its three levels operational, tactical, and strategic. The foundations of the project come from social constructivist perspective on learning, from experiential learning literature and from researches that call for a new epistemological ground in management learning. In this paper, the authors will stress some challenges and frustrations with the project since these could be helpful to those interested in similar initiatives. Due to limited space, only two challenges will be stressed: (1) the construction of legitimacy for the project; and (2) the persistent dissonance between theory and practice. Acute pancreatitis(AP)is a serious condition presenting catastrophic consequences.In severe AP,the mortality rate is high,and some patients initially diagnosed with mild-to-moderate AP can progress to a life-threateni...Acute pancreatitis(AP)is a serious condition presenting catastrophic consequences.In severe AP,the mortality rate is high,and some patients initially diagnosed with mild-to-moderate AP can progress to a life-threatening severe state.Treatment of AP has evolved over the years.Drainage was the first surgical procedure performed for AP;however,later,surgical approaches were replaced by more conservative approaches due to the availability of advanced medical care and improved understanding of the course of AP.Currently,surgery is used to manage several complications of AP,such as pseudocysts,pancreatic fistulas,and biliary tract obstruction.Patients who are unresponsive to conservative treatment or have complications are typically considered for surgical intervention.This review focuses on the surgical approaches(endoscopic,percutaneous,and open)that have been established in recent studies to treat this acute condition and summarizes the common management guidelines for AP,discussing the relevant indications,significance,and complications.It is evident that despite their reduced involvement,surgeons lead the multidisciplinary care of patients with AP;however,given the gaps in existing knowledge,more research is required to standardize surgical protocols for AP.展开更多

The natural composition of forests has undergone significant changes over recent centuries.A closer-to-natural tree species composition has long been perceived as key to a high biodiversity.We investigated the impact ...The natural composition of forests has undergone significant changes over recent centuries.A closer-to-natural tree species composition has long been perceived as key to a high biodiversity.We investigated the impact on communities of click beetles(Elateridae)caused by changes in the tree species composition of spruce monocultures compared to reference sites of recently unmanaged natural beech forests.To collect data,passive interception traps were distributed within managed spruce stands of different age classes and natural beech forests of various developmental stages.The beetle species richness was slightly but not significantly higher in the beech forests.The saproxylic species group was significantly more common in the spruce stands,whereas the group of nonsaproxylic species was significantly more abundant in the beech stands.In the commercial stands,the significantly highest species richness was in the clearings(0–10-year-old stands),and at this forest age class,the vast majority of the beetle species occurred in the spruce stands.In the developmental stages of the natural forest,a slightly higher beetle richness was found at the disintegration stage.The study results suggested that different tree species compositions and stand structures affect the communities of click beetles and substantially change their species composition and thus their response to external influences.Therefore,management of stands using diverse silvicultural systems is recommended for creating diverse ecological niches in forests.展开更多

The present paper introduces a combined discrete event simulation (DES)/business process management (BPM) approach as innovative means to study the workflow of the activities within the Department of Laboratory Medicine of the “San Paolo” Hospital in Naples (Italy). After a first “As-Is” analysis to identify the current workflows of the system and to gather information regarding its behaviour, a following DES-based “What-If” analysis is implemented to figure out alternative work hypotheses in order to highlight possible modifications to the system’s response under varying operating conditions and improve its overall performances. The structure of the simulation program is explained and the results of the scenario analysis are discussed. Thi...Background: Request for preoperative laboratory investigations is usually done by surgeons. On some occasions, the patient may come with laboratory investigations that have been requested by the primary physician. This occurs in situations where the primary physician saw the patient first and referred to the surgeon. There is usually no indications based on history or physical examination before these laboratory request is done but rather on speculations that the “anaesthetist may require them”. This is done in order to avoid cancellation or delay of cases. The aim of this study was to find out how tests ordered in Komfo Anokye Teaching Hospital (K.A.T.H.) by the surgeons affected the decisions of the anaesthetists in the perioperative management of the patients. Methodology: This was a prospective, cross sectional study of patients undergoing elective surgery at K.A.T.H from 1st to 31st March 2014. A quantitative technique was used to effectively quantify laboratory results that were contained in a patient’s folder before an elective surgical procedure. A close and open-ended questionnaire was developed and answered by reviewing patient’s folders during the pre-anesthesia assessment. Data were analyzed using Statistical Package of Social Sciences (SSPS) version 22. Results: The average age of patients studied was 50 years. Abnormal test results did not influence the anaesthetic management in 70.5% of cases but led to either delay or cancelation of cases or requirement for transfusion of blood or blood products in 29.5% of cases. Conclusion: Preoperative laboratory tests ordered by Surgeons in KomfoAnokye Teaching Hospital do not significantly influence the anaesthetic managements of patients.展开更多

However, there is neither a national nor ...In Democratic Republic of the Congo (DRC), the laboratory TAT is significantly very long and do not comply with either international standards or the suggestions of customers. However, there is neither a national nor a local strategy to improve the laboratory TAT. The aim of the present study is to develop practical management strategies to shorten clinical laboratory tests’ TAT. This was a qualitative study conducted in Kinshasa. Focus groups and Lean tools were used respectively to generate a wide range of views from a variety of laboratory staff and to eliminate several form of waste in the laboratory flow process. Based on the identified root causes of delay, focus groups participants reported that there is a lot of scope for the improvement of TAT in DRC. Consistent attendance and punctuality are essential. The hospital management should implement the Laboratory Information Systems (LIS) and install Middleware. Total laboratory automation, inventory system for all reagents and supplies used in the laboratory, expansion of the sampling area, sufficient number of high-power machine and a clear job description are indispensable. LIS, 3.5 mL BD vacutainer Barricor<sup>TM</sup> tube and point-of-care testing (POCT) are necessary for workflow improvement. A reduction of 312 minutes was achieved by eliminating or decreasing non-value-added activities. Applying the suggested key strategies, and particularly the new workflow process, is a basis for improving the laboratory tests’ TAT. The algorithm presented can be easily implemented in other laboratories that face this type of problem.展开更多

Methods: This article was retrospective and descriptive, covering a period of six months, running from January 1st to December 31st, 2013 and conducted?to evaluate therapeutic and prognosis of Uretero-pelvic junction abnormalities. Included criteria were all patients whose diagnosis was abnormalities of the uretero-pelvic junction and had been confirmed after para clinical explorations. The stricture of the junction after a first kidney surgery did not include. The Creatinine level has considered high from 15 mg/l. Results: The median age was 35 years old. Age groups (30 - 40) years old have predominated in 36.2%. A male predominance had been noted in 62.3% versus 37.7% women. The most affected occupation was the officials in 37.7% of cases. The main reason for consultation was lumbar pain in 97.1%. The high creatinine level has observed in 19 patients (27.5%). Sixty-one patients (88.41%) have been operated. An abstention has been observed in 7 patients (10.14%) and one patient died before the surgical issue in renal failure. The early post-operative course has been uneventful in 78.7% (48 cases) and complicated in 21.3% (21 cases). Conclusions: Open surgery tends to disappear at the expense of the endopyelotomy and laparoscopy which gives best results. However, it is still relevant.展开更多

Disaster exposure is a collection of the element at risk to potential loss. Gampaha divisional secretariat (DS) is a study area laid on the lower part of the Attanagalu Oya river basin. As the geospatial tools, OpenStreetMap (OSM), Java OpenStreetMap (JOSM), QGIS, GPS Essentials, and Open Map Kit (OMK) are used. The elements of disaster exposure, including the number of people or types of assets, are surveyed and inventoried using the OSM platforms. Local, national, and international agencies produce and evaluate the data. The study developed spatial data for building footprints of 165,000 households, street lengths of 2300 km, hospital units of 16, and utility units of 2300. This could overcome the main challenges of exposure mapping in the area. The procedure developed in the exposure mapping can be used in a data-sparse environment. Exposure mapping is generally used to estimate the impact of hazards or disasters, which are essential in effective disaster management. How are there still remaining challenges in disaster exposure mapping such as less awareness about the mapping procedure, lack of government support, internet access, hardware, and inability to understand the value of exposure mapping?展开更多

文摘The natural composition of forests has undergone significant changes over recent centuries.A closer-to-natural tree species composition has long been perceived as key to a high biodiversity.We investigated the impact on communities of click beetles(Elateridae)caused by changes in the tree species composition of spruce monocultures compared to reference sites of recently unmanaged natural beech forests.To collect data,passive interception traps were distributed within managed spruce stands of different age classes and natural beech forests of various developmental stages.The beetle species richness was slightly but not significantly higher in the beech forests.The saproxylic species group was significantly more common in the spruce stands,whereas the group of nonsaproxylic species was significantly more abundant in the beech stands.In the commercial stands,the significantly highest species richness was in the clearings(0–10-year-old stands),and at this forest age class,the vast majority of the beetle species occurred in the spruce stands.In the developmental stages of the natural forest,a slightly higher beetle richness was found at the disintegration stage.The study results suggested that different tree species compositions and stand structures affect the communities of click beetles and substantially change their species composition and thus their response to external influences.Therefore,management of stands using diverse silvicultural systems is recommended for creating diverse ecological niches in forests.

文摘The authors designed an ecological storm water system in a residential area to replace the conventional underground channels for the collection of storm water so as to reduce the nutrients and sediments discharged. This system contains natural sub-creeks as drainage channels discharging overflow to nearby creeks, an open green trench, a storage pond, and natural sub-creeks. The sub -creeks were designed to be integrated into community landscape, which not only increases the efficiency of water usage, but also improves the aesthetic qualities of the community residence area as required by Agenda 21. This research proved the feasibility of an open storm water collection and utilization system for the design of a community water system.
文摘The management systems currently used in the Italian healthcare sector provide fragmented and incomplete information on this system and are generally unlikely to give accurate information on the performances of the healthcare processes. The present paper introduces a combined discrete event simulation (DES)/business process management (BPM) approach as innovative means to study the workflow of the activities within the Department of Laboratory Medicine of the “San Paolo” Hospital in Naples (Italy). After a first “As-Is” analysis to identify the current workflows of the system and to gather information regarding its behaviour, a following DES-based “What-If” analysis is implemented to figure out alternative work hypotheses in order to highlight possible modifications to the system’s response under varying operating conditions and improve its overall performances. The structure of the simulation program is explained and the results of the scenario analysis are discussed. The paper starts with a brief exploration of the use of DES in healthcare and ends with general observations on the subject.
文摘Background: Request for preoperative laboratory investigations is usually done by surgeons. On some occasions, the patient may come with laboratory investigations that have been requested by the primary physician. This occurs in situations where the primary physician saw the patient first and referred to the surgeon. There is usually no indications based on history or physical examination before these laboratory request is done but rather on speculations that the “anaesthetist may require them”. This is done in order to avoid cancellation or delay of cases. The aim of this study was to find out how tests ordered in Komfo Anokye Teaching Hospital (K.A.T.H.) by the surgeons affected the decisions of the anaesthetists in the perioperative management of the patients. Methodology: This was a prospective, cross sectional study of patients undergoing elective surgery at K.A.T.H from 1st to 31st March 2014. A quantitative technique was used to effectively quantify laboratory results that were contained in a patient’s folder before an elective surgical procedure. A close and open-ended questionnaire was developed and answered by reviewing patient’s folders during the pre-anesthesia assessment. Data were analyzed using Statistical Package of Social Sciences (SSPS) version 22. Results: The average age of patients studied was 50 years. Abnormal test results did not influence the anaesthetic management in 70.5% of cases but led to either delay or cancelation of cases or requirement for transfusion of blood or blood products in 29.5% of cases. Conclusion: Preoperative laboratory tests ordered by Surgeons in KomfoAnokye Teaching Hospital do not significantly influence the anaesthetic managements of patients.
文摘In Democratic Republic of the Congo (DRC), the laboratory TAT is significantly very long and do not comply with either international standards or the suggestions of customers. However, there is neither a national nor a local strategy to improve the laboratory TAT. The aim of the present study is to develop practical management strategies to shorten clinical laboratory tests’ TAT. This was a qualitative study conducted in Kinshasa. Focus groups and Lean tools were used respectively to generate a wide range of views from a variety of laboratory staff and to eliminate several form of waste in the laboratory flow process. Based on the identified root causes of delay, focus groups participants reported that there is a lot of scope for the improvement of TAT in DRC. Consistent attendance and punctuality are essential. The hospital management should implement the Laboratory Information Systems (LIS) and install Middleware. Total laboratory automation, inventory system for all reagents and supplies used in the laboratory, expansion of the sampling area, sufficient number of high-power machine and a clear job description are indispensable. LIS, 3.5 mL BD vacutainer Barricor<sup>TM</sup> tube and point-of-care testing (POCT) are necessary for workflow improvement. A reduction of 312 minutes was achieved by eliminating or decreasing non-value-added activities. Applying the suggested key strategies, and particularly the new workflow process, is a basis for improving the laboratory tests’ TAT. The algorithm presented can be easily implemented in other laboratories that face this type of problem.
文摘Objectives: To evaluate therapeutic and prognosis of Uretero-pelvic junction abnomalities. Methods: This article was retrospective and descriptive, covering a period of six months, running from January 1st to December 31st, 2013 and conducted?to evaluate therapeutic and prognosis of Uretero-pelvic junction abnormalities. Included criteria were all patients whose diagnosis was abnormalities of the uretero-pelvic junction and had been confirmed after para clinical explorations. The stricture of the junction after a first kidney surgery did not include. The Creatinine level has considered high from 15 mg/l. Results: The median age was 35 years old. Age groups (30 - 40) years old have predominated in 36.2%. A male predominance had been noted in 62.3% versus 37.7% women. The most affected occupation was the officials in 37.7% of cases. The main reason for consultation was lumbar pain in 97.1%. The high creatinine level has observed in 19 patients (27.5%). Sixty-one patients (88.41%) have been operated. An abstention has been observed in 7 patients (10.14%) and one patient died before the surgical issue in renal failure. The early post-operative course has been uneventful in 78.7% (48 cases) and complicated in 21.3% (21 cases). Conclusions: Open surgery tends to disappear at the expense of the endopyelotomy and laparoscopy which gives best results. However, it is still relevant.
文摘The use of open-source data and tools in disaster exposure mapping is presented in this paper. Disaster exposure is a collection of the element at risk to potential loss. Gampaha divisional secretariat (DS) is a study area laid on the lower part of the Attanagalu Oya river basin. As the geospatial tools, OpenStreetMap (OSM), Java OpenStreetMap (JOSM), QGIS, GPS Essentials, and Open Map Kit (OMK) are used. The elements of disaster exposure, including the number of people or types of assets, are surveyed and inventoried using the OSM platforms. Local, national, and international agencies produce and evaluate the data. The study developed spatial data for building footprints of 165,000 households, street lengths of 2300 km, hospital units of 16, and utility units of 2300. This could overcome the main challenges of exposure mapping in the area. The procedure developed in the exposure mapping can be used in a data-sparse environment. Exposure mapping is generally used to estimate the impact of hazards or disasters, which are essential in effective disaster management. How are there still remaining challenges in disaster exposure mapping such as less awareness about the mapping procedure, lack of government support, internet access, hardware, and inability to understand the value of exposure mapping?

文摘Emerging viruses are viruses whose occurrence has risen within the past twenty years,or whose presence is likely to increase in the near future.Diseases caused by emerging viruses are a major threat to global public health.In spite of greater awareness of safety and containment procedures,the handling of pathogenic viruses remains a likely source of infection,and mortality,among laboratory workers.There is a steady increase in both the number of laboratories and scientist handling emerging viruses for diagnostics and research.The potential for harm associated to work with these infectious agents can be minimized through the application of sound biosafety concepts and practices.The main factors to the prevention of laboratory-acquired infection are well-trained personnel who are knowledgable and biohazard aware,who are perceptive of the various ways of transmission,and who are professional in safe laboratory practice management.In addition,we should emphasize that appropriate facilities,practices and procedures are to be used by the laboratory workers for the handling of emerging viruses in a safe and secure manner.This review is aimed at providing researchers and laboratory personnel with basic biosafety principles to protect themselves from exposure to emerging viruses while working in the laboratory.This paper focuses on what emerging viruses are,why emerging viruses can cause laboratory-acquired infection,how to assess the risk of working with emerging viruses,and how laboratory-acquired infection can be prevented.Control measures used in the laboratory designed as such that they protect workers from emerging viruses and safeguard the public through the safe disposal of infectious wastes are also addressed.
