There is global concern on the methodological limitations, transferability and effectiveness of clinical practice guidelines (CPGs) for chronic non-communicable diseases, particularly for hypertension. Bolivia, Ecuado...There is global concern on the methodological limitations, transferability and effectiveness of clinical practice guidelines (CPGs) for chronic non-communicable diseases, particularly for hypertension. Bolivia, Ecuador and Peru have regularly produced CPGs;however no formal assessment has been done on their contents, transferability and effectiveness. The past decade saw significant migration from Andean countries to Europe. Knowing how European CPGs compare with those produced in Andean countries is necessary to recommend future changes targeted to the migrant population. A systematic search of CPGs was done on indexed databases and non-indexed publications. Recognized and approved CPGs were identified by technical officers in the Ministries of Health of the respective countries. The guidelines of the European Society of hypertension and four selected CPGs from the Andean countries were assessed by two independent evaluators using the “Agree II instrument for assessing clinical practice guidelines, AGREE II Consortium, May 2009”. Comparison of the CPGs is based on the six domain scores provided by the Agree II instrument. The overall score of CPGs ranged from 1.85 to 2.94 of 6 maximum possible. The European CPG scored highest in 3 of 6 domains compared, most notably in rigor of development. Average domain scores for clarity of presentation (0.84) and scope and purpose (0.64) were highest scores for applicability (0.30). Stakeholder involvement (0.28) and rigor of development (0.17) were the lowest. The CPGs assessed appear to fail meeting the standards of quality and pertinence. They show a progressive worsening from domains declaring good intentions of being clear, to those which measure their hard aspects and implications.展开更多

However, the pharmacist in developing countries has remained obscure in direct health- care of the patient. The main aim of this article is to reinforce the concept that there is a need to strengthen the basic pharmaceutical system in the Arab and African world before implementing any advanced clinical pharmacy or pharmaceutical care services to our communities. This article gives a good insight of where pharmacy practice in Arab, African and other developing countries globally stand in comparison to developed nations. A trend is apparent towards increased emphasis on clinical pharmacy practice. Country-specific pharmacy education and practice are discussed, analyzing the progress and opportunities available to pharmacy graduates. In addition, the future of pharmacy practice and clinical roles of hospital and community pharmacists were reviewed. It was realized that pharmacy practice and progress in many of the Arab and African countries were a mirror image of the status of the country itself. A new vision for the future of pharmacy practice needs to be considered and reorientation of pharmacy services in the existing system is needed. There should be a professional approach to speed up the rise towards practicing clinical pharmacy and pharmaceutical profession. This is achieved by interviewing patients, documenting their health details and educating them on methods of improving their health, including proper medication usage and lifestyle modification. Furthermore, colleges of pharmacy need to revise and update their curricula to accommodate the progressively increasing development in the pharmaceutical education and the evolving new roles of practicing pharmacists in their community.展开更多

Some societies in developing countries have become sources of specimen for clinical trials of biomedicine which is unaffordable to their citizens. This paper explores the neglect of traditional African medicinal innovations and research in favour of imported Western medicine perpetuated by the developed countries. The paper argues that global medicine and health care have neither utilized nor recognized the African Traditional Medicine (ATM) fully, despite the fact that cultures in developed world used and continue to utilize the indigenous medical knowledge. The paper further argues that instead of neglecting African Traditional Medicine, ATM and biomedicine can be more beneficial by blending them into a single system, through what we would call in this paper High-Performance Medical Research (HPMR). This would allow participation of communities to achieve both socio-eeonomic and medical knowledge growth rather than being a monopoly and preserve of developed organizations in the North. This paper proposes that HPMR should be a systematic and scientific approach for enhancing local people's participation in the development of medical ventures. This paper takes new consumption phenomena in the digital media-dominated polymedia environment as the object of study to review the theoretical history of “mediatization,” and explores new consumption’s emergence, essence, and related issues systematically through deductive reasoning. More specifically, this paper is to first interpret the mediatization theory and then analyze two dimensions(i.e., the formation of mediatized consumption and the resultant reinvention of practice rules) from the perspective of media affordances. On this basis, we conclude mediatized consumption’s essence, forms, and practice rules. This study indicates that new consumption is essentially mediatized consumption, which concerns “media-shaped consumption” and “consumption-shaped media,” and that new consumption has two representations;consumption-oriented media and banal consumption. Media affordances make new consumption fields human-oriented, connecting “people” and “things,” and are becoming a major contributor to the formation of mediatized consumption. Thus, the practice rules concerning “people, goods, and fields” in the new consumption sector are reinvented;the social interactions in the “middle region” are highlighted;the imaginative, narrative, and social characteristics of goods are emphasized;and the cross-field integration of consumption is realized.展开更多

A road safety management (RSM) system can be defined as “a complex institutional structure that involves cooperating and interacting bodies which support the tasks and processes necessary for the prevention and reduc...A road safety management (RSM) system can be defined as “a complex institutional structure that involves cooperating and interacting bodies which support the tasks and processes necessary for the prevention and reduction of road traffic injuries”. RSM should promote the road safety progress of the country. However, the details of this relationship are generally lacking. This study explored the RSM systems in European countries based on the information collected through interviews with experts and officials, in each country, and using a “good practice” criteria questionnaire. The dataset included 14 countries with fifty items related to five RSM areas: institutional organization;policy formulation and adoption;policy implementation and funding;monitoring and evaluation;scientific support, information and capacity building. Cluster analyses and correlations were used to identify country groups with similar RSM components, to recognize typical RSM structures if available and to examine the relationship between RSM and road safety performance of the countries. The findings showed that all the countries are different when RSM systems are considered as a whole, making it impossible to identify typical RSM structures or a single best working model at a national level. However, it is possible to compare countries when the RSM areas are considered separately, where the clusters of countries recognized by the study present the patterns common for those European countries. Across the analyses, a number of countries with a consistently higher and lower availability of the RSM components were identified, enabling a final countries’ ranking into a number of groups. The latter actually reflects the level of RSM in the country, in terms of its correspondence to the “good practice” criteria. A further analysis indicated a positive correlation between the higher level of the RSM system and better safety performance of the countries.展开更多

More specifically, this paper is to first interpret the mediatization theory and then analyze two dimensions(i.e., the formation of mediatized consumption and the resultant reinvention of practice rules) from the perspective of media affordances. On this basis, we conclude mediatized consumption’s essence, forms, and practice rules. This study indicates that new consumption is essentially mediatized consumption, which concerns “media-shaped consumption” and “consumption-shaped media,” and that new consumption has two representations;consumption-oriented media and banal consumption. Media affordances make new consumption fields human-oriented, connecting “people” and “things,” and are becoming a major contributor to the formation of mediatized consumption. Thus, the practice rules concerning “people, goods, and fields” in the new consumption sector are reinvented;the social interactions in the “middle region” are highlighted;the imaginative, narrative, and social characteristics of goods are emphasized;and the cross-field integration of consumption is realized.

文摘A public health system generally refers to public utilities oriented toward protecting and improving health. It is intended to protect the health of the general public. Since the founding of the People’s Republic of China, China has constructed the largest healthcare system in the world. The 20th National Congress of the Communist Party of China(CPC), stressed that China will improve its public health system. Looking back at the history of China’s public health system from its inception to the reform and development and then improvement, it can be found that its development has always revolved around the theoretical logic of unifying rights and obligations and the practical logic of balancing the protection of the right to health and the regulation of state power. The fundamental goal is to protect the right to health of the people. The increasing health risks in modern society have changed require that efforts be made to construct a crisscrossing legal framework for public health, improve early-warning and emergency response mechanisms for public health emergencies,and integrate the health concept into all policies to further improve the public health system.

文摘Children and adults are often exposed to lead in homes as a result of deteriorating lead-based paint surfaces and any disturbance to those surfaces. There are a number of laws, regulations, standards and guidance documents in place aimed at minimization of lead dust contamination following renovation and repair activities. These laws and regulations are oftentimes confusing and conflicting for the maintenance contractor who has limited time and resources for researching the right and appropriate course of action. This paper provides a comprehensive review and discussion of the lead laws, regulations, standards and guidelines for contractors.
文摘A road safety management (RSM) system can be defined as “a complex institutional structure that involves cooperating and interacting bodies which support the tasks and processes necessary for the prevention and reduction of road traffic injuries”. RSM should promote the road safety progress of the country. However, the details of this relationship are generally lacking. This study explored the RSM systems in European countries based on the information collected through interviews with experts and officials, in each country, and using a “good practice” criteria questionnaire. The dataset included 14 countries with fifty items related to five RSM areas: institutional organization;policy formulation and adoption;policy implementation and funding;monitoring and evaluation;scientific support, information and capacity building. Cluster analyses and correlations were used to identify country groups with similar RSM components, to recognize typical RSM structures if available and to examine the relationship between RSM and road safety performance of the countries. The findings showed that all the countries are different when RSM systems are considered as a whole, making it impossible to identify typical RSM structures or a single best working model at a national level. However, it is possible to compare countries when the RSM areas are considered separately, where the clusters of countries recognized by the study present the patterns common for those European countries. Across the analyses, a number of countries with a consistently higher and lower availability of the RSM components were identified, enabling a final countries’ ranking into a number of groups. The latter actually reflects the level of RSM in the country, in terms of its correspondence to the “good practice” criteria. A further analysis indicated a positive correlation between the higher level of the RSM system and better safety performance of the countries.

文摘The digital development rights in developing countries are based on establishing a new international economic order and ensuring equal participation in the digital globalization process to achieve people's well-rounded development in the digital society.The relationship between cross-border data flows and the realization of digital development rights in developing countries is quite complex.Currently,developing countries seek to safeguard their existing digital interests through unilateral regulation to protect data sovereignty and multilateral regulation for cross-border data cooperation.However,developing countries still have to face internal conflicts between national digital development rights and individual and corporate digital development rights during the process of realizing digital development rights.They also encounter external contradictions such as developed countries interfering with developing countries'data sovereignty,developed countries squeezing the policy space of developing countries through dominant rules,and developing countries having conflicts between domestic and international rules.This article argues that balancing openness and security on digital trade platforms is the optimal solution for developing countries to realize their digital development rights.The establishment of WTO digital trade rules should inherently reflect the fundamental demands of developing countries in cross-border data flows.At the same time,given China's dual role as a digital powerhouse and a developing country,it should actively promote the realization of digital development rights in developing countries.

文摘Like domestic law, international law can be used in domestic governance, where it can become an important instrument assisting China's Party and government to rule the country and manage state affairs. Incorporating part of international law into domestic law and comprehensively advancing the rule of law in accord with the principle of national sovereignty demonstrate the openness and international vision of China's rule of law construction. Translating these propositions into concrete institutional practice will be of vital significance for improving the socialist rule of law with Chinese characteristics, advancing the international rule of law and promoting world peace.

文摘The call to advance the rule of law in foreign-related affairs is an important component of China's strategic move to a great modern country.In modern times,Western countries have applied rule of law practices in their modernization,demonstrating the values of a sound rule of law system for national strength.In particular,the rule of law,extended beyond national borders,provides a crucial guarantee for a great modern country's sustained,stable,and healthy path,and is also a clear symbol of a country's soft power.As China is progressing to become a great modern country,there is a need to pay a particularly close attention to the rule of law,actively promoting the legalization of foreign-related work.China's stance on the rule of law in foreign-related affairs should include both a principle of defending its core interests and a principle of advocating shared values and of promoting mutually beneficial cooperation.In the context of this era,China's international law professional community should delve into appropriate models that can facilitate smooth progress on the path to modernization,further enhancing the rule of law in China and the successful emergence of a great modern country.
