Analysis of quality of life in patients after transgastric natural orifice transluminal endoscopic gallbladder-preserving surgery

BACKGROUND At present,laparoscopic cholecystectomy(LC)is the main surgical treatment for gallstones.But,after gallbladder removal,there are many complications.Therefore,it is hoped to remove stones while preserving the function of the gallbladder,and with the development of endoscopic technology,natural orifice transluminal endoscopic surgery came into being.AIM To compare the quality of life,perioperative indicators,adverse events after LC and transgastric natural orifice transluminal endoscopic gallbladder-preserving surgery(EGPS)in patients with gallstones.METHODS Patients who were admitted to The First Affiliated Hospital of Xinjiang Medical University from 2020 to 2022 were retrospectively collected.We adopted propen-sity score matching(1:1)to compare EGPS and LC patients.RESULTS A total of 662 cases were collected,of which 589 cases underwent LC,and 73 cases underwent EGPS.Propensity score matching was performed,and 40 patients were included in each of the groups.In the EGPS group,except the gastr-ointestinal defecation(P=0.603),the total score,physical well-being,mental well-being,and gastrointestinal digestion were statistically significant compared with the preoperative score after surgery(P<0.05).In the LC group,except the mental well-being,the total score,physical well-being,gastrointestinal digestion,the gastrointestinal defecation was statistically significant compared with the preoperative score after surgery(P<0.05).When comparing between groups,gastrointestinal defecation had significantly difference(P=0.002)between the two groups,there was no statistically significant difference in the total postoperative score and the other three subscales.In the surgery duration,hospital stay and cost,LC group were lower than EGPS group.The recurrence factors of gallstones after EGPS were analyzed:and recurrence was not correlated with gender,age,body mass index,number of stones,and preoperative score.CONCLUSION Whether EGPS or LC,it can improve the patient’s symptoms,and the EGPS has less impact on the patient’s defecation.It needed to,prospective,multicenter,long-term follow-up,large-sample related studies to prove.

Nature of Employment and Its Effect on the Quality of Life of Employees in SMTEs in Mombasa County,Kenya

Tourism is an important economic sector globally contributing to economic development of many countries.Though the tourism sector has been credited with the creation of millions of jobs to populations’worldwide,there has been wide-ranging apprehension on its capacity to improve the populaces’quality of life.This study therefore sought to establish whether the nature of jobs created had an influence on quality of life of employees working in the STMES found in Mombasa County,Kenya.A mixed methods research design using the embedded approach was adopted.Primary data was collected using researcher administered semi-structured questionnaires and semi-structured interviews.The target population was the 1572 employees in the SMTEs.Probability Proportional to Size Sampling was used for the SMTEs employees where a total of 464 employees formed the sample size while purposive sampling was used for interview respondents.Linear regression was used to analyse quantitative data while tables were used for data presentation.The nature of employment had a statically significant effect on employees’quality of life(R=0.881;P=0.000;V=0.804).This finding has implications for developing a regulatory framework that would enable SMTEs create productive employment in that,it is a wake-up call to the government to re-look at the implementation of the economic blue print(Vision 2030)and identify functional implementation strategies it can use to ensure its’overall goal of providing a high quality of life for citizens is achieved.

Nuclear geyser model of the origin of life:Driving force to promote the synthesis of building blocks of life

We propose the nuclear geyser model to elucidate an optimal site to bear the first life.Our model overcomes the difficulties that previously proposed models have encountered.Nuclear geyser is a geyser driven by a natural nuclear reactor,which was likely common in the Hadean Earth,because of a much higher abundance of 235U as nuclear fuel.The nuclear geyser supplies the following：（1）high-density ionizing radiation to promote chemical chain reactions that even tar can be used for intermediate material to restart chemical reactions,（2）a system to maintain the circulation of material and energy,which includes cyclic environmental conditions（warm/cool,dry/wet,etc.）to enable to produce complex organic compounds,（3）a lower temperature than 100℃ as not to break down macromolecular organic compounds,（4）a locally reductive environment depending on rock types exposed along the geyser wall,and（5）a container to confine and accumulate volatile chemicals.These five factors are the necessary conditions that the birth place of life must satisfy.Only the nuclear geyser can meet all five,in contrast to the previously proposed birth sites,such as tidal flat,submarine hydrothermal vent,and outer space.The nuclear reactor and associated geyser,which maintain the circulations of material and energy with its surrounding environment,are regarded as the nuclear geyser system that enables numerous kinds of chemical reactions to synthesize complex organic compounds,and where the most primitive metabolism could be generated.

A Comparative Environmental Evaluation of the Coal and Natural Gas Life Cycle

Natural gas and coal are the main primary energy resources used in the Romanian energy sector, 73.7% in 2011, taking into account the fuel imports. The objective of the article consists in analyzing all the processes along the coal and the natural gas life cycle in order to assess their overall environmental impact. Two energy technologies were analyzed, for each resource: the pulverized coal combustion with supra-critical parameters and CO2 capture unit and the natural gas combined cycle. Considering the functional unit of electricity production for 1 year, it was found that the natural gas combined cycle remains the more interesting energy technology from an environmental point of view. However, the pulverized coal with supra-critical parameters equipped with a CO2 capture unit has the lowest environmental impact on the climate change. The weakest point of the coal technology is its low efficiency.

Petroleum substitution, greenhouse gas emissions reduction and environmental benefits from the development of natural gas vehicles in China

This study develops a bottom-up model to quantitatively assess the comprehensive effects of replacing traditional petroleum-powered vehicles with natural gas vehicles（NGVs） in China based on an investigation of the direct energy consumption and critical air pollutant（CAP） emission intensity, life-cycle energy use and greenhouse gas（GHG） emission intensity of NGV fleets. The results indicate that, on average, there are no net energy savings from replacing a traditional fuel vehicle with an NGV. Interestingly, an NGV results in significant reductions in direct CAP and life-cycle GHG emissions compared to those of a traditional fuel vehicle, ranging from 61% to 76% and 12% to 29%, respectively. Due to the increasing use of natural gas as a vehicle fuel in China（i.e. approximately 28.2 billion cubic metres of natural gas in2015）, the total petroleum substituted with natural gas was approximately 23.8 million tonnes（Mt）, which generated a GHG emission reduction of 16.9 Mt of CO2 equivalent and a CAP emission reduction of 1.8 Mt in 2015. Given the significant contribution of NGVs, growing the NGV population in 2020 will further increase the petroleum substitution benefits and CAP and GHG emission reduction benefits by approximately 42.5 Mt of petroleum-based fuel, 3.1 Mt of CAPs and 28.0 Mt of GHGs. By 2030, these benefits will reach 81.5 Mt of traditional petroleum fuel, 5.6 Mt of CAPs and 50.5 Mt of GHGs, respectively.

The author of this article proposes that the representation of Charles Darwin and Alfred Wallace’s theory on “variation and selection” in the living world is a reflection of the action of hierarchical thermodynamics. Hierarchical thermodynamics is based on the law of temporal hierarchies and on the principle of substance stability. This principle enables the transmission of thermodynamic information between lower and higher structural hierarchies, in both forward and reverse direction: from nucleic acids to higher structural hierarchies and back. The principle of substance stability, in fact, is the main dynamical and thermodynamic mechanism of natural selection. It is alleged that the natural selection of atoms, molecules, organisms, populations, and other hierarchical structures takes place under the action of a variety of internal factors within organisms and the external environmental factors that can be considered as tropisms. Forms (design) of living organisms are formed as a result of spontaneous and non-spontaneous processes that lead to the adaptation of living systems to the environment. The selection is carried out as a result of the impacts of different energy types and the principle of substance stability at all levels of hierarchical structures. Actions of tropisms are presented by various members of the generalized Gibbs equation.

Significance Analysis of Flexural Behaviour of Hybrid Sandwich Panels

This paper presents the significance analysis of a new type of hybrid composite sandwich wall panel which can be manufactured as modular panelised system. Two different types of natural fibers reinforced plastics (NFRP) laminate were incorporated into the new sandwich panel as an intermediate layer. The significance analysis in this research has been carried out using analysis of variance (ANOVA). As the aim of the analysis is to select the most appropriate natural fiber composites for the intermediate layer, the experiments were arranged as a single factor experiment in which 3 levels of a factor have been examined. The factor refers to the type of intermediate layer used in the sandwich panel. The result of this study shows that the incorporation of intermediate layer has significantly enhanced the load carrying capacity of the sandwich panels.

Impact of comorbid anxiety and depression on quality of life and cellular immunity changes in patients with digestive tract cancers

AIM: A study was performed to investigate the impact of comorbid anxiety and depression (CAD) on quality of life (QOL) and cellular immunity changes in patients with digestive tract cancers.METHODS: One hundred and fifty-six cases of both sexes with cancers of the digestive tract admitted between March 2001 and February 2004 in the Department of Medical Oncology, First Affiliated Hospital of Xi'an Jiaotong University were randomly enrolled in the study. Depressive and anxiety disorder diagnoses were assessed by using the Structured Clinical Interview for DSM-Ⅳ. All adult patients were evaluated with the Hamilton depressive scale (HAMD, the 24-item version), the Hamilton anxiety scale (HAMA, a modified 14-item version), quality of life questionnaire-core 30 (QLQ-C30), social support rating scale (SSRS), simple coping style questionnaire (SCSQ), and other questionnaires, respectively. In terms of HAMD ≥ 20 and HAMA ≥ 14, the patients were categorized, including CAD (n = 31) in group A, anxiety disorder (n = 23) in group B,depressive disorder (n = 37) in group C, and non-disorder (n = 65) in group D. Immunological parameters such as T-lymphocyte subsets and natural killer (NK) cell activities in peripheral blood were determined and compared among the four groups.RESULTS: The incidence of CAD was 21.15% in patients with digestive tract cancers. The average scores of social support was 43.67±7.05 for 156 cases, active coping 20.34±7.33, and passive coping 9.55±5.51. Compared with group D, subjective support was enhanced slightly in group A, but social support, objective support, and utilization of support reduced, especially utilization of support with significance (6.16 vs 7.80, P＜0.05); total scores of active coping decreased, while passive coping reversed; granulocytes proliferated, monocytes declined,and lymphocytes declined significantly (32.87 vs 34.00,P＜0.05); moreover, the percentage of CD3, CD4, CD8and CD56 in T lymphocyte subsets was in lower level,respectively, and CD56 showed a significant decline in group A (26.02 vs 32.20, P＜0.05), however, CD4/CD8ratio increased. Physical function, role function, fatigue,sleeplessness and constipation had significant changes among different groups by one-way ANOVA, and group A was in poor QOL. It revealed that global health-related quality of life (QL) were positively correlated with active coping and CD56; CAD was negatively correlated with QL, active coping and CD56. Furthermore, the step-wise regression analysis suggested that utilization of support,CD56, active coping, fatigue, sleeplessness and depression were significant factors contributing to QOL.CONCLUSION: CAD, which can impair QOL and cellular immunity, occurs with a higher incidence in patients with digestive tract cancers. Hence, it is essential to improve mental health for them with specifically tailored interventions.

Life Table of Natural Population of Gastrolina depressa

[Objective]The paper was to understand the population development regularity of Gastrolina depressa in natural environment in Guizhou Province.[Method]The population dynamics of G.depressa was studied by establishing life table of natural population of G.depressa via field observation and calculating dynamic trend index of population.[Result]The mortality rates in egg stage,pupal stage and adult stage accounted for 26.42%,10.96%and 8.22%of the total mortality rate,respectively.The mortality rate in larval stage was the highest,accounting for 54.40%of the total mortality rate,higher than the total mortality rate in the other three stages.The population dynamic trend index I=9.25,I>1 indicated that the population showed an upward trend.[Conclusion]Under the natural conditions,the population of G.depressa shows upward trend,and human intervention is needed to control the population.The larval stage is the weak stage of the insect,which is the best time to control the insect.
