Evaluation of Postoperative Psychological Distress and Its Driving Factors in Patients with Oral and Maxillofacial Malignant Tumors

Objective:To explore and analyze the evaluation and driving factors of postoperative psychological pain inpatients with oral and maxillofacial malignant tumors.Methods:Relevant data were collected from 80 patients with oral and maxillofacial malignant tumors who attended the outpatient clinic for follow-up consultations between May 2021 to May 2023.The patients used the psychological distress thermometer(DT)to circle words that best described their experiences in the past week,assigning a numerical value(0-10)to indicate their pain level on each day.The scoring results were employed to assess the psychological pain in these patients.A self-developed patient basic information questionnaire was utilized to record demographic details.Logistic regression analysis was employed to evaluate patients two weeks after surgery,focusing on the assessment of psychological distress and the identification and location of driving factors.Results:Following evaluation,the results revealed that the average postoperative DT score for the 80 patients with oral and maxillofacial malignant tumors was 4.53±1.98 points.Scores<4 points indicated no psychological pain(Group N)in 48 cases,while scores≥4 points indicated psychological pain(Group Y)in 32 cases.The differences in postoperative DT scores among patients with varying educational levels,fears and worries about disease progression,economic problems,sleep problems,level of hope,and oral pain were statistically significant(P<0.05).Multiple linear regression analysis results indicated that education level,fear and worry about disease progression,economic problems,sleep problems,level of hope,and oral pain are driving factors of postoperative psychological pain in patients with oral and maxillofacial malignant tumors(P<0.05).Conclusion:The postoperative psychological pain level in patients with oral and maxillofacial malignant tumors is at a moderate level.Educational level,fear and worry about disease progression,economic problems,sleep problems,level of hope,and oral pain were identified as driving factors for postoperative psychological pain in these patients. 展开更多

Personal factors could act as intervening variables between the causes and effects of psychological health conditions of employees. This study focused on six personal intervening factors that were associated with the risks of occupational psychological disorders namely: low self-esteem, negative personality trait, unproductive core beliefs, poor self-concept evaluation on performance, poor relationship with others and poor time management skills. An investigation on the level of severity of these personal intervening factors was conducted involving 150 construction professionals and 150 construction trade workers, who were purposively selected in Ghana. The personal factors that were found to be prevalent among the construction employees were low self-esteem and poor time management skills. Correlation analysis and regression analysis were used to determine the relationships between the personal intervening factors and demographic factors such as the age, educational levels and marital status of the research participants. The results of the study indicated that the age and marital status of the respondents had no significant relationship with any of the personal intervening factors. Personal factors such as personality trait and relationship with others also had no relation with any of the demographic factors analyzed. The educational level of the respondents, however, had a significant relationship with the factors of self-esteem, productive core beliefs, self-evaluation on performance and time management skills. An independent two-sample T-test was used to compare the means of the factors with significant relations. This study revealed that construction employees who were highly educated had better self-esteem and time management skills than those who were less educated. The findings from this study broaden the view of moderators on influential sources of psychological health conditions of employees. 展开更多

BACKGROUND: Assessing and evaluating mental health status can provide educational planners valuable information to predict the quality of physicians' performance at work. These data can help physicians to practice in the most desired way. The study aimed to evaluate factors affecting psychological morbidity in Iranian emergency medicine practitioners at educational hospitals of Tehran.METHODS: In this cross sectional study 204 participants(emergency medicine residents and specialists) from educational hospitals of Tehran were recruited and their psychological morbidity was assessed by using a 28-question Goldberg General Health Questionnaire(GHQ-28). Somatization, anxiety and sleep disorders, social dysfunction and depression were evaluated among practitioners and compared to demographic and job related variables.RESULTS: Two hundreds and four participants consisting of 146(71.6%) males and 58(28.4%) females were evaluated. Of all participants, 55(27%) were single and 149(73%) were married. Most of our participants(40.2%) were between 30–35 years old. By using GHQ-28, 129(63.2%) were recognized as normal and 75(36.8%) suffered some mental health disorders. There was a signifi cant gender difference between normal practitioners and practitioners with disorder(P=0.02) while marital status had no significant difference(P=0.2). Only 19(9.3%) declared having some major mental health issue in the previous month.CONCLUSION: Females encountered more mental health disorders than male(P=0.02) and the most common disorder observed was somatization(P=0.006). 展开更多

To investigate the psychological status of the main caregivers for patients who were transferred out of ICU after craniocerebral tumor surgery and analyze their factors. <strong>Methods:</strong> 112 patients with cerebral tumor surgery were reviewed from sun yat-sen university cancer hospital. The research object is caregivers. The general data questionnaire, hospital anxiety depression scale and migration of ICU patients’ family members stress questionnaire scale investigation are collected. Multiple linear regression analysis in cerebral tumor patients with postoperative ICU transition out main factors influencing the psychological condition of the caregivers. <strong>Results:</strong> The anxiety and depression scores were (15.60 ± 5.83) and (38.73 ± 3.23) respectively. The results of multiple regression analysis showed that the gender, education level, relationship with the patient and family monthly income of the primary caregivers of the patients were the influencing factors of anxiety, depression and migration stress psychological state (P < 0.05). <strong>Conclusion:</strong> The main caregivers of patients who were transferred out of ICU after craniocerebral tumor surgery had relatively severe anxiety, depression and migration stress. Objective:The objective of the study is to investigate the psychological status of nursing staff both working on nonfront line and on the front line during the epidemic period of COVID‑19 and to analyze the influencing factors related to mental health state,as well as to provide targeted psychological intervening measures for nursing staff in the face of this major public health event.Methods:We used data from January 28,2019,to February 08,2020,on the number of cases who worked in the 3A‑level hospital as nonfront‑line nurses(n=29,54%)and those who worked in Wuhan infectious hospital as the front‑line supporters(n=25,46%)came from the 3A‑level hospital.The participants completed the symptom checklist 90,and the researchers recorded data on demographics,psychological status,and the influencing factors.Results:In our baseline scenario,we estimated that the total symptom index was 1.676±0.654 for nurses who worked on the front line,1.260±0.304 for those who worked as the front‑line reserve nurses,and was 1.486±0.647 for those who worked as the nonfront‑line nurses.The results of correlation analysis show that the factors related to the psychological status of nurses include age,professional title,marital status,and family support;different factors have different effects on psychological status.Conclusion:In the face of major public health emergencies,both front‑line nursing staff and nonfront‑line staff are confronted with different degrees of psychological stress,so effective psychological support should be given to improve the working efficiency and enthusiasm of nurses. 展开更多

Objective:The aim of this study is to analyze the differences in galanin(GAL)and 5‑hydroxytryptamine(5‑HT)levels in patients with different subtypes of gastroesophageal reflux disease(GERD)and its correlation with anxiety and depression in patients.Methods:Using the reflux disease questionnaire,238 patients with GERD were selected as the study group.According to the endoscopic performance,they were divided into the nonerosive reflux disease(NERD)group(114 patients)and the RE group(124 patients).Sixty healthy people were selected as the control group.All research subjects were tested using the self‑rating anxiety scale(SAS)and the self‑rating depression scale(SDS)to assess the severity of anxiety and depression.The levels of 5‑HT and GAL were analyzed using enzyme‑linked immunosorbent assay.Results:Compared with the control group,5‑HT level in GERD patients without anxiety and depression was higher(t=1.97,P<0.05)and GAL level was lower(t=1.97,P<0.05).NERD patients demonstrated more server anxiety(F=55.92,P<0.05)and depression problems(F=36.51,P<0.05)compared to reflux esophagitis(RE)patients.The 5‑HT level was lower(F=54.53,P<0.05)and the GAL level was higher(F=8.00,P<0.05)in NERD patients compared with the control group.Furthermore,5‑HT level was negatively correlated with SAS(r=−0.789,P<0.05)and SDS(r=−0.787,P<0.05)scores;GAL level was positively correlated with SAS(r=0.688,P<0.05)and SDS(r=0.705,P<0.05)scores;5‑HT and GAL level were negatively correlated(r=−0.744,P<0.05).Conclusions:5‑HT level is higher and GAL level was lower in GERD patients without anxiety and depression than healthy people;the symptoms of anxiety and depression of NERD patients are more severe compared to those of RE patients;the severity of anxiety and depression was negatively correlated with the level of 5‑HT and positively with GAL level. 展开更多

Psychological Factors Are Closely Associated with the Bell's Palsy: A Case-control Study

To observe the differences in psychological status between Bell's palsy (BP) patients and healthy subjects, and to examine the relationship between psychological factors and the severity of BP, we conducted a case-control, multi-center clinical investigation. A total of 695 subjects were as-signed to the case group (n=355) and the control group (n=340). House-Brackmann grading system and Facial Disability Index (FDI) were adopted to assess the BP patients; Kessler Psychological Dis-tress Scale (K10) and 16 Personality Factor (16PF) scale were employed to evaluate the psychologi-cal distress and personality profiles of all subjects. Two independent samples t test was used to com-pare the differences between cases and controls, and to compare the differences among different BP patients. Pearson correlation analysis was used to examine the relationship between psychological factors and severity of facial paralysis. The results showed that psychological distress (K10) in case group (27.09±5.80) was significantly higher than that in control group (13.43±3.02) (t=-37.219, P=0.000). The scores of personality factor Warmth (A), Openness to Change (Q1), Self-Reliance (Q2) were lower in cases than in the controls (P<0.01, P<0.05, P<0.05, respectively), whereas the scores of Sensitivity (I), Vigilance (L), Apprehension (O), and Tension (Q4) were significantly higher in cases than in the controls (P<0.05, P<0.01, P<0.01, P<0.01, respectively). In addition, the psy-chological distress was significantly higher in female patients, severe (HB score Ⅳ-Ⅵ) patients, and subacute (onset time 72-168 h) patients compared with that in male patients, mild (HB score Ⅰ-Ⅲ)patients, and acute (onset time≤72 h) patients (P<0.05). The scores of personality factor in fe-male patients, severe patients, and subacute patients were also significantly different from male pa-tients, mild patients, and acute patients (P<0.05). The result of Pearson correlation analysis showed that psychological factors (K10, personality A, F, L, N, O, Q4) were closely related to HB scores. We are led to conclude that the psychological status between BP patients and healthy people are different; psychological distress and personality factors are closely associated with severity of facial paralysis. 展开更多

Background: The coronavirus disease 2019 (COVID-19) is a public health problem that has caused harm to the mental health of healthcare workers. In Africa, the COVID-19 pandemic has led healthcare workers to experience mental health disorders such as anxiety, depression, stress, insomnia and burnout. This study aimed to review published studies on the effect of COVID-19 on the mental health of healthcare workers, associated factors and coping strategies that have been employed in Africa. Methods: This was a systematic review that was conducted through searching databases including;PubMed/Medline and Google Scholar. The study included published literature from January 2020 to May 2022 that met the inclusion criteria. The selection of articles was conducted following the 2020 PRISMA guidelines. Results: A total of 39 articles were retrieved, of which only 18 met the inclusion criteria and were used in this study. Our review revealed that healthcare workers experienced mental health disorders such as anxiety, depression, insomnia, stress and burnout that were associated with the COVID-19 pandemic. Coping strategies such as religious practices, support from family members and colleagues and avoiding listening to social media about COVID-19 were used to minimize mental health problems. Conclusion: The COVID-19 pandemic has caused increased mental health disorders among healthcare workers in Africa. Identification of factors associated with mental health problems is cardinal in developing coping mechanisms against the psychological impact of COVID-19. Therefore, there is a need for governments to develop and implement strategies for protecting the mental health of healthcare workers during crises such as the COVID-19 pandemic. 展开更多

Previous studies suggest that serotonin(5-HT) might interact with brain-derived neurotrophic factor(BDNF) during the stress response.However,the relationship between 5-HT and BDNF expression under purely psychological stress is unclear.In this study,one hour before psychological stress exposure,the 5-HT1 A receptor agonist 8-OH-DPAT or antagonist MDL73005,or the 5-HT2 A receptor agonist DOI or antagonist ketanserin were administered to rats exposed to psychological stress.Immunohistochemistry and in situ hybridization revealed that after psychological stress,with the exception of the ventral tegmental area,BDNF protein and m RNA expression levels were higher in the 5-HT1 A and the 5-HT2 A receptor agonist groups compared with the solvent control no-stress or psychological stress group in the CA1 and CA3 of the hippocampus,prefrontal cortex,central amygdaloid nucleus,dorsomedial hypothalamic nucleus,dentate gyrus,shell of the nucleus accumbens and the midbrain periaqueductal gray.There was no significant difference between the two agonist groups.In contrast,after stress exposure,BDNF protein and m RNA expression levels were lower in the 5-HT1 A and 5-HT2 A receptor antagonist groups than in the solvent control non-stress group,with the exception of the ventral tegmental area.Our findings suggest that 5-HT regulates BDNF expression in a rat model of acute psychological stress. 展开更多

In order to explore the criminal psychological mechanism of burglars,this study adopts the qualitative research method of grounded theory to conduct in-depth interviews with 41 burglars in two prisons in Jiangxi Province,China.Nvivo 11.0 was used to code-construct and qualitatively analyze the interview content in order to refine the influencing factors and psychological evolution process of burglary behavior.Thefindings revealed that(1)burglary risk factors include burglary cognition,burglary motivation,burglary decision-making,delinquent peers,burglary opportunity,and incomplete reformation.(2)There are three stages in the psychological evolution pro-cess of burglars:cognitive formation,motivational dominance,and behavioral decision.(3)The interpretation of the criminal psychological mechanism of burglars is a comprehensive and dynamic outcome of the interaction of internal and external factors that shape the individual.Participants’inspection and non-participants’inspection were adapted to verify the research results’validity,which showed that the results were reliable. 展开更多

Objective The psychologic health level of college and secondaryschool students and the relevant factors were investigated to scientific basis and guidance for school mental health work.Methods Standard 1251 cases were drawn from 1‰ of students in colleges and middle schools of Shaanxi province.Taking 14 psychic health level indexes in SCL 90 as dependent variable and 109 indexes of psychic health back ground as in dependent variable, multi factor analyses have been made.Results 22.6% of students had relatively serious psychological problems.The score of SCL 90 in females was a little bit higher than that in males.The scores of students at both universities and senior middle schools were higher than that in junior middle schools students.The score of SCL 90 of students who came from the countryside was higher than that of city students.The score of the whole students was higher than that of the normal.The students with psychic problems showed obsession,interpersonal sensitivity,depression,anxiety,paranoia and hostility.Factor analysis showed that influencing factors included history of positive individual risking behavior,physical conditions,grade,address,family influences,menses and sexual prombles,bad relation with others,poor self assessment.Conclusion The psychologic health level of the students investigated is lower than that of the whole society.The factors,which hamper psychic health of students, are biological,psychological and social in nature. 展开更多

The majority of noma patients surviving with aesthetical effects are exposed to stigmatization and social rejection. Studies focusing on the socio-psychological impact of noma survivors have rarely been done. Our study aimed to identify the differences in social acceptance/rejection and the influencing factors associated with social acceptance in noma patients. Methods: A cross-sectional study was conducted at the NGO-Sentinelles (Niger) reception center on patients with noma from Zinder, Maradi, and Tahoua regions between 9<sup>th</sup> May 2017 and 2<sup>nd</sup> June 2017. The survey was conducted through a face-to-face interview on patients admitted to the center and those discharged from the centre after the treatment. The interview questionnaire comprised 45 questions (Cronbach’s alpha coefficient = 0.812) with pathological information, sociodemographic characteristics, and socio-psychological qualitative information. Findings: We recorded 50 noma patients (43 from Zinder and 7 from Maradi and Tahoua). The younger patients (1 - 5 years old), noma patients who stayed in school during follow-up treatment, patients who were referred by a health structure, patients enrolled into the centre in a short time (<30 days), and patients in the acute phase of noma had a significantly high social acceptance rate with 60.0%, 82.9%, 60.0%, 57.1% and 94.3% respectively;whereas single adults and cheek lesion site had the highest social rejection rate when compared to their corresponding factors with 60.0% and 86.7% respectively. There were significant differences in victims’ perception of noma [χ<sup>2</sup> = 45.536, (P < 0.001)] and acceptance of their new faces [P = 0.023], between the social acceptance and social rejection rate, therefore all patients who accepted their new faces felt social acceptance. Social acceptance was significantly highly correlated with pathological history (admission method, phase of noma, care, and treatment received at center) with r<sub>s</sub> ranging from 0.609 to 0.810, moderately correlated with patient’s sociodemographic characteristics (age, marital status, and region) with r<sub>s</sub> ranging from 0.381 to 0.474. Lowly correlated with clinical evolution after treatment (r<sub>s</sub> = 0.293). Logistic regression results showed that the likelihood of social acceptance increased when the patient’s age was young (≤15 years), their marital status was minor, they were enrolled at the school before noma appearance, they were referred to the centre after diagnosis, the admission time to the centre was short (≤30 days), acute phase of noma, and care received at the centre was non-surgery. The location of the lesion on the cheek was a risk factor for social acceptance, indicating cheek lesions from noma increased the likelihood of social rejection in our study. Conclusion: The sociodemographic characteristics, pathological history, and psychological aspects of noma patients were correlated and were found to be important factors influencing their social acceptance/rejection rate. 展开更多

Aiming at the limitations of psychopathology (PTH), the dual-factor model of mental health (DFM) was proposed as a new mental health concept and methodology under the background of positive psychology trend. In this paper we propose giving an overview of DFM, and doubt, criticize, and modify DFM from the perspective of Chinese psychological suzhi research. The available literature from 1983 to 2012 that is related to DFM and concerning psychological suzhi research in the past 20 years has been reviewed. In addition, we also absorbed the idea of positive psychology and Traditional Chinese Medicine (TCM) Constitution theory to develop theoretically the relationship model between psychological suzhi and mental health. The relationship model between psychological suzhi and mental health modifies and transcends PTH and DFM. It will be the new research area of mental health research. 展开更多
