BACKGROUND Heart failure(HF),a common cardiovascular condition,is characterized by significant morbidity and mortality. While traditional Chinese medicine(TCM)is often used as a complementary approach in HF management, systematic evalua-tions of its impact on clinical outcomes,TCM syndrome scores, and B-type natriuretic peptide(BNP)levels are lacking. This study fills this gap through a comprehensive analysis of randomized controlled trials(RCTs)focusing on TCM for HF treatment. It encompasses an assessment of methodological quality,a meta-analysis,and an evaluation of evidence quality based on established standards. The results offer crucial insights into the potential advantages and constraints of TCM in HF management.RCTs on TCM for HF treatment published since the establishment of the database were searched in four Chinese and English databases, including China National Knowledge Infrastructure,Wanfang,VIP Information Chinese Science and Technology Journal, and PubMed.Methodological quality was assessed for the included studies with the Cochrane risk-of-bias assessment tool, and the meta-analysis and publication bias assessment was performed with the RevMan5.3 software. Finally, the quality of evidence was rated according to the GRADE criteria.RESULTS A total of 1098 RCTs were initially retrieved. After screening,16 RCTs were finally included in our study, which were published between 2020 and 2023.These RCTs involved 1660 HF patients, including 832 in the TCM group[TCM combined with conventional Western medicine(CMW)treatment]and 828 in the CWM group(CWM treatment). The course of treatments varied from 1 wk to 3 months.TCM syndrome differentiation was analyzed in 11 of the included RCTs.In all included RCTs,outcome indicators included comprehensive clinical outcomes,TCM syndrome scores, and BNP levels. The meta-analysis results showed significant differences between the TCM and CWM groups in terms of comprehensive clinical outcomes[risk ratio=-0.54;95%confidence interval(CI)=-0.61,-0.47;P<0.00001],TCM syndrome scores[weighted mean difference(WMD)=-142.07;95%CI=-147.56,-136.57;P<0.00001],and BNP levels(WMD=-142.07;95%CI=-147.56,-136.57;P<0.00001).According to the GRADE criteria,RCTs where"TCM improves clinical comprehensive outcomes"were rated as low-quality evidence, and RCTs where"TCM reduces TCM syndrome scores"or"TCM decreases BNP levels"were rated as medium-quality evidence.CONCLUSION TCM combined with CWM treatment effectively improves comprehensive clinical outcomes and diminishes TCM syndrome scores and BNP levels in HF patients.Given the low and medium quality of the included RCTs,the application of these results should be cautious. 展开更多

The wastewater effluent from Radix aconiti processing, an important step in the production processes of traditional Chinese medicine(TCM), is a type of toxic wastewater and difficult to treat. Plasma oxidation methods have emerged as feasible techniques for effective decomposition of toxic organic pollutants. This study examined the performance of a plasma reactor operated in a dielectric barrier discharge(DBD) to degrade the effluent from R. aconiti processing. The effects of treatment time, discharge voltage, initial pH value and the feeding gas for the reactor on the degradation of this TCM wastewater were investigated. A bacterium bioluminescence assay was adopted in this study to test the toxicity of the TCM wastewater after non-thermal plasma treatment. The degradation ratio of the main toxic component was 87.77% after 60 min treatment with oxygen used as feed gas and it was 99.59% when the initial p H value was 8.0. High discharge voltage and alkaline solution environment were beneficial for improving the degradation ratio. The treatment process was found to be capable of reducing the toxicity of the wastewater to a low level or even render it non-toxic. These experimental results suggested that the DBD plasma method may be a competitive technology for primary decomposition of biologically undegradable toxic organic pollutants in TCM wastewater. 展开更多

Background:Traditional Chinese medicines are usually processed before they are used for clinical treatment. This is done in a way associated with the Maillard reaction. Methods:Based on the Maillard reaction, this paper analyzed the degree of processing of rehmannia root(Rehmanniae radix)relative to the dynamic variation rules of Maillard reaction index parameters, including pH,A420,amino acids, and 5-hydroxymethylfurfural.Furthermore,this study introduced thermal analysis techniques and pyrolysis kinetics to assess the influence of the correlation between processing raw rehmannia root and the Maillard reaction during carbonization. It then went through the whole process of transforming the raw material to end-product in order to explain the scientific connotation of processing it.Results:The results showed that each time rehmannia root was processed, its pH value and amino acid content decreased, while the A420 value and 5-HMF increased.Processing with wine shows a significant difference in these experimental indexes. The position and intensity of the maximum thermal weight loss rate peak of processed rehmannia root at different degrees of processing are different.Comprehensive quantitative 221±0.2°C for processed rehmannia root carbonization was the processing temperature limit. Moreover, the kinetic solution verified that the activation energy corresponding to the carbonization temperature was close to the maximum value of the activation energy of the whole carbonization process, and the optimal mechanism function was g(α)=((1−α)−1/3−1)2.Conclusion:The Maillard reaction occurred during the processing of rehmannia root mixed with carbonization. With each increase of the number of steaming and drying cycles involved in the processing, the level of Maillard reaction increased significantly. The wine-steaming method had a significant effect on the quality of the processed product. 展开更多

Pulse diagnosis equipment used in Traditional Chinese Medicine(TCM)has long been developed for collecting pulse information and in TCM research. However,it is still difficult to implement pulse taking automatically or efficiently in clinical practice. Here,we present a digital protocol for TCM pulse information collection based on bionic pulse diagnosis equipment, which ensures high efficiency, reliability and data integrity of pulse diagnosis information.A four-degree-of-freedom pulse taking platform together with a wrist bracket can satisfy the spatial positioning and angle requirements for individually adaptive pulse acquisition.Three-dimensional reconstruction of a wrist surface and an image localization model are combined to provide coordinates of the acquisition position and detection direction automatically.Three series elastic joints can not only simulate the TCM pulse taking method that“Three fingers in a straight line, the middle finger determining the‘Guan’location and finger pulp pressing on the radial artery,”but also simultaneously carry out the force-controlled multi-gradient pressing process. In terms of pulse information integrity, this proposed protocol can generate rich pulse information, including basic individual information, pulse localization distribution,multi-gradient dynamic pulse force time series, and objective pulse parameters, which can help establish the fundamental data sets that are required as the pulse phenotype for subsequent comprehensive analysis of pulse diagnosis. The implementation of this scheme is beneficial to promote the standardization of the digitalized collection of pulse information, the effectiveness of detecting abnormal health status, and the promotion of the fundamental and clinical research of TCM,such as TCM pulse phenomics. 展开更多

Objective:With using natural language processing (NLP) technology to analyze and process the text of "Treatise on Febrile Diseases (TFDs)"for the sake of finding important information, this paper attempts to apply NLP in the field of text mining of traditional Chinese medicine (TCM)literature. Materials and Methods:Based on the Python language, the experiment invoked the NLP toolkit such as Jieba, nltk, gensim, and sklearn library, and combined with Excel and Word software. The text of "TFDs" was sequentially cleaned, segmented, and moved the stopped words, and then implementing word frequency statistics and analysis, keyword extraction, named entity recognition (NER) and other operations, finally calculating text similarity. Results:Jieba can accurately identify the herbal name in "TFDs." Word frequency statistics based on the word segmentation found that "warm therapy" is an important treatment of "TFDs." Guizhi decoction is the main prescription, and five core decoctions are identified. Keyword extraction based on the term "frequency-inverse document frequency" algorithm is ideal. The accuracy of NER in "TFDs" is about 86%;latent semantic indexing model calculating the similarity,"Understanding of Synopsis of Golden Chamber (SGC)" is much more similar with "SGC" than with "TFDs." The results meet expectation. Conclusions:It lays a research foundation for applying NLP to the field of text mining of unstructured TCM literature. With the combination of deep learning technology,NLP as an important branch of artificial intelligence will have broader application prospective in the field of text mining in TCM literature and construction of TCM knowledge graph as well as TCM knowledge services. 展开更多
