Methods: Learning was measured by comparing residents’ communication skills in a simulated consultation before and after a communication skills training. Communication skills were transcribed and tagged with a computer-assisted program. Potential variables associated with learning (residents’ characteristics, contextual characteristics and pre-training communication skills) were measured before the training and entered in regression analysis. Results: Fifty-six residents followed the training between 2002 and 2006. Poor pre-training assessment and supportive skills predicted the respective learning of these skills. Better assessment skills’ learning was predicted by copings (i.e. lower level of emotional coping), lower levels of self-efficacy and depersonalization. Better supportive skills’ learning was predicted by a lower work experience and associated with a higher training attendance rate. Conclusions: Predictors and correlates of assessment and supportive skills learning were different. Trainers needed to detect certain residents’ characteristics (i.e. depersonalization) in order to optimize assessment skills learning. Trainers needed to be aware that supportive skills are difficult to learn and to teach and may need more training hours.展开更多

Background: Communication difficulties due to cognitive impairment among residents with dementia disease may complicate care situations. Training can improve nurses’ communication skills and increase care quality. The validation method aims to facilitate communication with residents with dementia disease through empathic and confirmatory approaches. Evaluations of the validation method have primarily focused on the residents’ perspective, and reports on nurses’ experiences are sparse. Improved communication and relationships with residents after validation method training have been described previously. Videotaped data could provide additional information about these earlier results. Design: A descriptive qualitative design. Methods: Eight nurses participated in a year of validation method training, including videotaped conversations with eleven residents. Videotapes with at least five months between the first and last recording were analysed and compared qualitatively. Results: The analysis revealed an overall pattern: nurses’ movements within and between various paths when improving their communication skills. This was based on three sub-patterns: from controlling communication towards developing attentiveness in communication, from ambiguous communication towards developing coherence in communication, and from being open and attentive towards having a refined attuned communication. Conclusions: All nurses developed their communication skills during the programme, albeit to different degrees. The findings are in congruence with the experiences described by nurses, and so it is reasonable to believe that the programme helped to improve the nurses’ skills in communicating with residents with dementia disease. Relevance to clinical practice: A validation method training programme could give nurses the possibility to develop their skills in communicating with residents with dementia disease.展开更多

During the field experiment, velocity responses were measured at different locations of a six-story masonry structure near the Beijing- Guangzhou Railway and along a small road adjacent to the building. The results show that the velocity response levels of the environmental ground and the building floors increase with train speed, and attenuate with the distance to the railway track. Heavier freight trains induce greater vibrations than lighter passenger trains. In the multi-story building, the lateral velocity levels increase monotonically with floor elevation, while the vertical ones increase with floor elevation in a fluctuating manner. The indoor floor vibrations are much lower than the outdoor ground vibrations. The lateral vibration of the building along the direction of weak structural stiffness is greater than along the direction with stronger stiffness. A larger room produces greater floor vibrations than the staircase at the same elevation, and the vibration at the center of a room is greater than at its corner. The vibrations of the building were compared with the Federal Transportation Railroad Administration (FTA) criteria for acceptable ground-borne vibrations expressed in terms ofrms velocity levels in decibels. The results show that the train-induced building vibrations are serious, and some exceed the allowance given in relevant criterion.展开更多

The irregularities of the near field, including the tunnel structure and parts of the soil, are modeled by the finite elements, and the wave propagation properties of the far field extending to infinity are modeled by the infinite elements. One particular feature of the 2.5D approach is that it enables the computation of the three-dimensional response of the half-space, taking into account the load-moving effect, using only a two-dimensional profile. Although the 2.5D finite/infinite element approach shows a great advantage in studying the wave propagation caused by moving trains, attention should be given to the calculation aspects, such as the rules for mesh establishment, in order to avoid producing inaccurate or erroneous results. In this paper, some essential points for consideration in analysis are highlighted, along with techniques to enhance the speed of the calculations. All these observations should prove useful in making the 2.5D finite/infinite element approach an effective one.展开更多

The train-bridge-ground interaction system is divided into two subsystems: the train-bridge interaction and the soil-structure interaction. First, the analytical program to simulate bridge vibration with consideration of train-bridge interaction is developed to obtain the vibration reaction forces at the pier bottoms. The high- speed train is described by a multi-DOFs vibration system and the rigid-frame viaduct is modeled with 3D beam elements. Second, applying these vibration reaction forces as input external excitations, the ground vibration is simulated by using a general-purpose program that includes soil-structure interaction effects. The validity of the analytical procedure is confirmed by comparing analytical and experimental results. The characteristics of high-speed train-induced vibrations, including the location of predominant vibration, are clarified. Based on this information a proposed vibration countermeasure using steel strut and new barrier is found effective in reducing train-induced vibrations and it satisfies environmental vibration requirements. The vibration screening efficiency is evaluated by reduction VAL based on 1/3 octave band spectral analysis.展开更多

A novel ground structure composed of five rectangular plates is proposed to enhance the impedance bandwidth and reduce the antenna size. The proposed antenna is comprised of two patches surrounded by the ground structure. Two metal patches of the antenna are located on two opposite sides of the dielectric substrate. The feed patch is used to excite the radiation patch. This unique design is realized by properly choosing the suitable feed patch shape, selecting similar slot shape on the radiation patch, and tuning their dimensions. The proposed antenna with an extremely small size of 6 mm × 9 mm has an operating impedance bandwidth ranging from 4.5 to 6.1 GHz for S11 < -10 dB, which also covers the two IEEE 802.11a wireless local area network bands (5.15 - 5.35 GHz and 5.725 - 5.825 GHz). In addition to be very small in size, the antenna exhibits omni-directional radiation patterns in the entire operating bandwidth and low cross polarization. The distortionless time domain performance of the antenna is confirmed by investigation of the phase response and group delay. The obtained results in both frequency and time domain show that the proposed antenna is suitable for use in wireless communication systems.展开更多

文摘BACKGROUND Patients with knee arthroplasty often have problems such as slow recovery of knee function, which may cause negative emotions and affect their postoperative rehabilitation. The application of sensory and motor training in the Acknowledge,Introduce,Duration,Evaluation,Thank You(AIDET)communication mode in patients after knee arthroplasty can provide reference for the selection of postoperative rehabilitation training measures.AIM To explore the sensory and motor training effects in AIDET communication mode on knee function recovery and resilience of patients after knee arthroplasty.METHODS One hundred patients who underwent knee arthroplasty at our hospital between January 2022 and January 2024 were randomly divided into two groups. The control group(n=50)received routine rehabilitation training. In the observation group(n=50),the AIDET communication mode was used to perform sensory and motor training,in addition to routine rehabilitation training. The rehabilitation training was administered for 8 weeks. After surgery, knee function, balance ability, walking ability, proprioception, and resilience were compared between the two groups.RESULTS The New York Hospital for Special Surgery knee scores of the observation group at the time of discharge and 8 weeks after intervention were(65.23±6.84,84.53±5.27),which was higher than those of the control group(61.03±7.15,74.92±6.52)(P<0.05).The balance ability of the observation group at the time of discharge and 8 weeks after the intervention was higher than that of the control group, the time of time up to go test was shorter than that of the control group, and proprioceptive function was higher than that of the control group(P<0.05).The resilience level in the observation group after the intervention was higher than that in the control group(P<0.05).CONCLUSION Sensory and motor training in AIDET communication mode promotes knee function recovery of patients after knee arthroplasty, improves their limb walking ability and balance function, and increases their resilience level.
