Increase in attention, relaxation and parasympathetic system activity is believed to enhance the shooting performance of archers during competition. This pilot study aims to investigate the effects of single-dose of Tai Chi practice on archer’s performance and the feasibility of an investigation protocol under a competition scenario. Methods: This was a randomized controlled trial (RCT) with a crossover study design. Pre- and post-intervention measurements were conducted to evaluate the effects of Tai Chi practice on the archer’s autonomic nervous system activity, attention and relaxation through electroencephalography investigation, and shooting performance when compared with stretching as control. Results: The protocol of the study was found to be feasible during a competition setting. No significant difference was detected in the outcome measures under the two intervention conditions. Participants after practicing Tai Chi were found to improve significantly in their shooting performance (p = 0.021). Conclusion: This pilot study showed that a real-time measurement of autonomic nervous system and brain wave activities in the competition field was feasible and it did not hinder the archers’ performance. There was a significant improvement observed in the shooting performance of archers after Tai Chi practice when compared with baseline. The findings of this pilot study provide the basis for the conduction of a larger RCT study to examine the potential beneficial effects of Tai Chi practice on archers.展开更多

The objective of this study was to examine the relationship between participation in mind-body-spir...Length of stay in treatment is thought to be the best predictors for long-term recovery from substance use disorders. The objective of this study was to examine the relationship between participation in mind-body-spirit (MBS) therapeutic programs and length of stay in a residential treatment facility. A retrospective analysis of data from 1719 subjects who were admitted to a 30-day residential program for substance use disorders (SUD) was conducted. Subjects participated in MBS programs that included yoga, acupuncture and meditation. We examined the effects of subject participation in MBS programs for subjects who left against staff advice (ASA) and those who successfully completed the residential program. A higher percentage of subjects with severe heroin use disorder left ASA compared with subjects with severe alcohol use disorder (16% vs. 9%, respectively). Most subjects from both substance groups who failed to complete the 30-day treatment program, left within the first two weeks of stay (59% of alcohol group and 75% of heroin group);however, again, the percentage of heroin users leaving during the first two weeks was significantly greater compared with that of alcohol subjects. We found a highly significant, positive correlation (r = 0.56, p < 0.01) and a statistically significant increase in LOS for all subjects, regardless of the substance type, and the number of MBS program sessions attended showing an association between MBS participation and increased LOS. These data support the inclusion of MBS programs as part of a comprehensive treatment strategy for SUD in combination with traditional counseling to help develop a sustainable long-term recovery.展开更多

Objectives:Prevalence rates of overweight and obesity are dramatically ever-increasing across the world.Therefore,this study was to evaluate the effect of mindfulness-based cognitive therapy(MBCT)on weight loss,hypert...Objectives:Prevalence rates of overweight and obesity are dramatically ever-increasing across the world.Therefore,this study was to evaluate the effect of mindfulness-based cognitive therapy(MBCT)on weight loss,hypertension,and attentional bias towards food cues in a group of women affected with this condition.Methods:A total of 45 participants were selected out of women referring to the Nutrition and Diet Therapy Clinic affiliated to Shahid Beheshti University of Medical Sciences,Iran,and then randomized into three groups of 15.The first experimental group was subjected to an energy-restricted diet therapy together with MBCT during 8 sessions,the second group took the diet therapy alone,and the third group received no intervention.Body mass index(BMI),hypertension,and attentional bias towards food cues were correspondingly evaluated before,at the end,and four weeks after the completion of the interventions.Results:The results of this study revealed that MBCT,along with diet therapy,had been significantly more effective in weight loss,decrease in BMI,lower systolic blood pressure(SBP),and attentional bias towards food cues compared with the diet therapy alone(P≤0.01).MBCT had no significant impact on the decline in diastolic blood pressure(DBP)in participants in the follow-up phase.Conclusion:This study demonstrated that MBCT along with the conventional diet therapy was more effective in weight loss,decrease in BMI,hypertension control,as well as attentional bias towards food cues than the diet therapy alone.展开更多

The primary treatment involves the removal of loose bodies;however, their complete eradication is often challenging and may not address underlying diseases, leading to persistent symptoms and the risk of new loose body formation. Aim: This case report aims to present the findings and surgical management of a 52-year-old male with an unusually large osseous loose body in the knee joint and associated pathologies. Case Presentation: The patient, a 52-year-old male, experienced recurrent episodes of severe, sudden, and painful locking of the knee joint, leading to difficulties moving. A plain MRI study was conducted to evaluate the condition of the knee joint, which revealed various degenerative changes and the presence of a loose body. Subsequently, an arthroscopic examination was performed under general anesthesia, uncovering the presence of an abnormally large loose body, as well as other pathologies including chondropathy, meniscal degeneration, and Baker’s cyst. Conclusion: Loose bodies (LBs) in the knee joint pose significant challenges and may lead to debilitating symptoms. Timely diagnosis and appropriate surgical intervention are crucial for symptom relief and the prevention of further joint damage as arthroscopic excision. Comprehensive imaging has a vital role in guiding treatment decisions and optimizing patient outcomes. In this case, the removal of the loose body improved patient outcomes and helped prevent potential joint complications.展开更多

METHODS: We evaluated 249 pat...AIM: To further evaluate the relationship between BSA and the effects of lamivudine in a greater number of cases and over a longer period of observation than in our previous evaluation. METHODS: We evaluated 249 patients with chronic hepatitis B. The effects of treatment for one year (n = 249), two years (n = 147), and three years (n = 72) were evaluated from the levels of serum ALT and HBVoDNA, as biological and virological effects (undetectable levels by PCR), respectively. Moreover, several variables that could influence the response to treatment, including ALT, albumin, bilirubin, platelet counts, BSA, HBVoDNA, and HBeAg were analyzed. RESULTS: For 1-year treatment, multivariate analysis revealed that BSA (P = 0.0002) was the only factor for the biological effect, and that ALT (P = 0.0017), HBV- DNA (P = 0.0004), and HBeAg (P = 0.0021) were independent factors for the virological effect. For 2-year treatment, multivariate analysis again showed that BSA (P = 0.0147) was the only factor for the biological effect, and that ALT (P = 0.0192) and HBeAg (P = 0.0428) were independent factors for the virological effect. For 3-year treatment, multivariate analysis, however, could not reveal BSA (P = 0.0730) as a factor for the normalization of ALT levels. CONCLUSION: BSA is a significant predictor for the normalizing the effect of lamivudine therapy on ALT for an initial 2-year period, suggesting that lamivudine dosage should be based on the individual BSA.展开更多

These women, al...Multiparity amongst women with a body mass index (BMI) ≥ 30 Kg/m2 is a common occurrence despite there being a known clear association with a decline in fecundity in women who are overweight or obese. These women, also pose further concerns, as they are at increased risk of antenatal complications such as preeclampsia and gestational diabetes. Over the years, a number of different modalities of fertility treatments have been tried and tested in this cohort of women to find the optimal treatment to improve their reproductive capacity. There has been an exponential increase in knowledge and understanding towards managing patients with a raised BMI, particularly through assisted reproductive treatments. Although the efficacies of various forms of fertility treatments have been shown to be affected by a rise in BMI, there is yet to be a definitive understanding as to the optimal management of these patients. The literature supports weight loss alone as an effective intervention in improving the reproductive capacity of women with a raised BMI with unexplained infertility. Furthermore, if live birth rate is taken as the desired outcome measure, then ovarian drilling and in vitro fertilisation (IVF) treatment have been shown to yield the best results in overweight and obese patients when comparisons are drawn to other interventions such as natural conception and treatment with clomiphene citrate.展开更多

Although most public...Perioperative hypothermia, core temperature below 36.0 ℃, transpires due to disruption of thermoregulationby anesthesia coupled with cold exposure to procedural surroundings and cleansing agents. Although most publications have focused on thermoregulation disruption with general anesthesia, neuraxial anesthesia may also cause significant hypothermia. The clinical consequences of perioperative hypothermia are multiple and include patient discomfort, shivering, platelet dysfunction, coagulopathy, and increased vasoconstriction associated with a higher risk of wound infection. Furthermore, postoperative cardiac events occur at a higher rate; although it is unclear whether this is due to increased oxygen consumption or norepinephrine levels. Hypothermia may also affect pharmacokinetics and prolong postoperative recovery times and hospital length of stay. In order to combat perioperative hypothermia, many prevention strategies have been examined. Active and passive cutaneous warming are likely the most common and aim to both warm and prevent heat loss; many consider active warming a standard of care for surgeries over one hour. Intravenous nutrients have also been examined to boost metabolic heat production. Additionally, pharmacologic agents that induce vasoconstriction have been studied with the goal of minimizing heat loss. Despite these multiple strategies for prevention and treatment, hypothermia continues to be a problem and a common consequence of the perioperative period. This literature review presents the most recent evidence on the disruption of temperature regulation by anesthesia and perioperative environment, the consequences of hypothermia, and the methods for hypothermia prevention and treatment.展开更多

And further we believe that quantum language is the only scientifically successful theory in dualistic idealism. If this turn is regarded as progress in the history of western philosophy (i.e., if “philosophical progress” is defined by “approaching to quantum language”), we should study the linguistic mind-body problem more than the epistemological mind-body problem. In this paper, we show that to solve the mind-body problem and to propose “measurement axiom” in quantum language are equivalent. Since our approach is always within dualistic idealism, we believe that our linguistic answer is the only true solution to the mind-body problem.展开更多

RESULTS: For 1-year treatment, multivariate analysis revealed that BSA (P = 0.0002) was the only factor for the biological effect, and that ALT (P = 0.0017), HBV- DNA (P = 0.0004), and HBeAg (P = 0.0021) were independent factors for the virological effect. For 2-year treatment, multivariate analysis again showed that BSA (P = 0.0147) was the only factor for the biological effect, and that ALT (P = 0.0192) and HBeAg (P = 0.0428) were independent factors for the virological effect. For 3-year treatment, multivariate analysis, however, could not reveal BSA (P = 0.0730) as a factor for the normalization of ALT levels. CONCLUSION: BSA is a significant predictor for the normalizing the effect of lamivudine therapy on ALT for an initial 2-year period, suggesting that lamivudine dosage should be based on the individual BSA.

文摘Multiparity amongst women with a body mass index (BMI) ≥ 30 Kg/m2 is a common occurrence despite there being a known clear association with a decline in fecundity in women who are overweight or obese. These women, also pose further concerns, as they are at increased risk of antenatal complications such as preeclampsia and gestational diabetes. Over the years, a number of different modalities of fertility treatments have been tried and tested in this cohort of women to find the optimal treatment to improve their reproductive capacity. There has been an exponential increase in knowledge and understanding towards managing patients with a raised BMI, particularly through assisted reproductive treatments. Although the efficacies of various forms of fertility treatments have been shown to be affected by a rise in BMI, there is yet to be a definitive understanding as to the optimal management of these patients. The literature supports weight loss alone as an effective intervention in improving the reproductive capacity of women with a raised BMI with unexplained infertility. Furthermore, if live birth rate is taken as the desired outcome measure, then ovarian drilling and in vitro fertilisation (IVF) treatment have been shown to yield the best results in overweight and obese patients when comparisons are drawn to other interventions such as natural conception and treatment with clomiphene citrate.
文摘Perioperative hypothermia, core temperature below 36.0 ℃, transpires due to disruption of thermoregulationby anesthesia coupled with cold exposure to procedural surroundings and cleansing agents. Although most publications have focused on thermoregulation disruption with general anesthesia, neuraxial anesthesia may also cause significant hypothermia. The clinical consequences of perioperative hypothermia are multiple and include patient discomfort, shivering, platelet dysfunction, coagulopathy, and increased vasoconstriction associated with a higher risk of wound infection. Furthermore, postoperative cardiac events occur at a higher rate; although it is unclear whether this is due to increased oxygen consumption or norepinephrine levels. Hypothermia may also affect pharmacokinetics and prolong postoperative recovery times and hospital length of stay. In order to combat perioperative hypothermia, many prevention strategies have been examined. Active and passive cutaneous warming are likely the most common and aim to both warm and prevent heat loss; many consider active warming a standard of care for surgeries over one hour. Intravenous nutrients have also been examined to boost metabolic heat production. Additionally, pharmacologic agents that induce vasoconstriction have been studied with the goal of minimizing heat loss. Despite these multiple strategies for prevention and treatment, hypothermia continues to be a problem and a common consequence of the perioperative period. This literature review presents the most recent evidence on the disruption of temperature regulation by anesthesia and perioperative environment, the consequences of hypothermia, and the methods for hypothermia prevention and treatment.
文摘Recently we proposed “quantum language”, which was not only characterized as the metaphysical and linguistic turn of quantum mechanics but also the linguistic turn of Descartes = Kant epistemology. And further we believe that quantum language is the only scientifically successful theory in dualistic idealism. If this turn is regarded as progress in the history of western philosophy (i.e., if “philosophical progress” is defined by “approaching to quantum language”), we should study the linguistic mind-body problem more than the epistemological mind-body problem. In this paper, we show that to solve the mind-body problem and to propose “measurement axiom” in quantum language are equivalent. Since our approach is always within dualistic idealism, we believe that our linguistic answer is the only true solution to the mind-body problem.
文摘Liver metastases occur commonly in many solid malignancies.With advances in systemic therapies and increased life expectancy,the role of using local therapies in oligo-metastases is rapidly increasing.Stereotactic body ra-diotherapy(SBRT)is an emerging precision therapy that is being used more frequently in the treatment for un-resectable liver metastases.This review focuses on the role of SBRT for liver metastases,principles of treatment,clinical outcomes,toxicity,and optimal patient selection.

文摘Hepatocellular carcinoma(HCC)is a common malignancy with high mortality rates.While surgery can be curative in early-stage disease,80% of patients cannot undergo surgical resection.Stereotactic body radiotherapy(SBRT),an emerging,non-invasive,precision treatment,has shown promising results across various stages of HCC and has thus been adopted in practice to varying degrees around the world.This article aims to review current guideline recommendations on SBRT,clinical evidence,and outcome comparisons with other local treatment modalities.Attempts are also made to compare the differences in clinical trials between Asian and Western countries.
