This approach prioritizes the safety and health of local citizens, placing communal productivity above corporate profitability. This research aims to assess air quality surrounding 28 chemical industry sites in Baton Rouge, Louisiana, to understand the environmental and health impacts of industrial pollutants, with a focus on environmental justice. Air quality pollutants, including PM2.5, PM10, O3, NO2, CO, and SO2, were monitored for 75 days during the Summer, using the BreezoMeter app. Python, Mapize, and QGIS software technologies were utilized for data analysis and visualization. Findings indicate a reduction in NO2 and CO levels, compared to existing literature. However, the persistent challenge of particulate matter suggests areas for further environmental management efforts. Additionally, the research suggests a significant disparity in air pollution exposure, probably affecting marginalized communities. Although the nature of the study might not fully capture annual pollution trends, the findings highlight the urgent need for the chemical industry to adopt efficient production methods and for policymakers to enhance air quality standards and enforcement, particularly in pollution-sensitive areas. The disproportionate impact of air pollution on vulnerable communities calls for a more inclusive approach to environmental justice, ensuring equitable distribution of clean air benefits and community involvement in pollution management decisions.展开更多

文摘BACKGROUND There are very few cases of cardiac occluder detachment, and it is rare to completely remove the occluder using interventional methods without undergoing thoracotomy surgery after detachment. This case innovatively used ultrasound guidance combined with digital subtraction angiography(DSA)to completely remove the occluder, accumulating some experience.CASE SUMMARY The patient underwent left atrial appendage occlusion surgery in our hospital due to atrial fibrillation. After the surgery, the occluder fell off and became free in the left ventricle, which is very dangerous. We innovatively used ultrasound guidance, combined with DSA,and interventional surgery to successfully capture the free occluder using a catcher, completely remove it,and then re implant a new left atrial appendage occluder. After the surgery, the patient recovered very well.CONCLUSION The size selection of the occluder is slightly conservative, and the shape of the left atrial appendage opening is irregular.

文摘BACKGROUND Fulminant type 1 diabetes mellitus(FT1DM)that occurs during pregnancy or the perinatal period is known as pregnancy-related FT1DM(PF),always without history of abnormal glucose metabolism. Here,we present four patients who developed FT1DM during treatment but were first diagnosed with gestational diabetes mellitus(GDM).CASE SUMMARY The clinical data of four patients with GDM combined with FT1DM admitted to our hospital between July 2018 and April 2021 were collected, and the patients and their infants were followed up.All patients were diagnosed with GDM during the second trimester and were treated. The blood glucose level elevated suddenly during the third trimester and then were diagnosed with FT1DM.Two patients had an insulin allergy, and two had symptoms of upper respiratory tract infection before onset. One patient developed ketoacidosis, and three developed ketosis. Two patients had cesarean section deliveries, and two had vaginal deliveries. The growth and development of the infants were normal.C-peptide levels were lower than those at onset, suggesting progressive impairment of islet function. The frequencies of the DRB109:01,DQB103:03,DQA103:02,DPA101:03,DPA102:02,DPB105:01,DRB401:03,G 01:01,and G 01:04 human leukocyte antigen(HLA)-G alleles were high in the present study.CONCLUSION In comparison with pregnancy-associated FT1DM(PF),patients with GDM combined with FT1DM had an older age of onset, higher body mass index, slower onset, fewer prodromal symptoms, and less acidosis. The pathogenesis may be due to various factors affecting the already fragileβ-cells of GDM patients with genetically susceptible class II HLA genotypes. We speculate that GDM combined with FT1DM during pregnancy, referred to as“double diabetes,”is a subtype of PF with its own unique characteristics that should be investigated further.
文摘In a local context, sustainable development entails utilizing the current resources—material and immaterial, measurable and immeasurable, popular and unpopular—of the community in a manner that avoids overexploitation and ensures intergenerational equity. The disproportionate impact of air pollution on vulnerable communities calls for a more inclusive approach to environmental justice, ensuring equitable distribution of clean air benefits and community involvement in pollution management decisions.

文摘Functional diarrhea(FDr)accounts for a relatively high proportion of digestive diseases. It is an infection that is not accompanied by abdominal pain and shows negative results in laboratory tests for bacteria and viruses. The main symptoms are persistent or recurrent discharge of watery and soft stools. The course of the disease is prolonged and recurring, and the treatment cost is higher and greatly affects the patient’s daily life. The incidence rate has a gradual increase in the trend. Its pathogenesis is complex where Western medicine is mostly used in symptomatic drug treatment. The treatment can be fast-acting and effective in relieving diarrhea. However, the long-term use of Western medicine poses a high risk in terms of side effects and a high chance of recurring upon stopping medication. At the same time, some diarrhea patients show the existence of drug-resistant strains of bacteria, and the overall efficacy of the drug is limited.Chinese medicine is mild and able to provide excellent treatment of diarrhea. With its lower price and cost, most families can afford it.Fengliang Tian, director of traditional Chinese medicine, implemented the“needle and medicine”method, which combines medicine and acupuncture, involving the usage of Jianpi Yishen Sechang Decoction and acupuncture in the treatment of functional diarrhea. The method has a low price, fewer side effects,is easy to accept, and can significantly reduce the recurrence rate with high efficacy. The study would like to share the clinical thinking and cases as follows to provide ideas and methods for the treatment of functional diarrhea by traditional Chinese medicine.
文摘BACKGROUND Synchronous endometrial and ovarian cancer(SEOC)is a rare genital tract tumor.Precise diagnosis is crucial for the disease management since prognosis and overall survival differ substantially between metastatic endometrial cancer(EC)or OC.In this review we present 2 cases of women who were diagnosed with SEOC,and discuss the clinical characteristic of SEOC,diagnostic and molecular profiling issues.Next generation sequencing of 10 gene panel was performed on cancerous tissue and uterine lavage samples.CASE SUMMARY In our report patients with SEOC had endometroid type histology with early stage and low-grade histology for both EC and OC.They underwent surgical treatment and staging.Next-generation sequencing of 10 gene-panel identified CTNNB1,PIK3CA,and PTEN gene mutations in ovarian tissue in one case, while none of these genes were mutated in other case.Literature review in support to our data suggest a good prognosis for SEOC diagnosed at early stage.CONCLUSION Accurate diagnosis of SEOC is essential for disease management and gene mutation analysis can be helpful as a complementary diagnostic and prognostic tool.
文摘BACKGROUND Clear cell sarcoma(CCS)is a rare soft-tissue sarcoma. The most common metastatic sites for CCS are the lungs, bones and brain.CCS is highly invasive and mainly metastasizes to the lung, followed by the bone and brain;however, pancreatic metastasis is relatively rare.CASE SUMMARY We report on a rare case of CCS with pancreatic metastasis in a 47-year-old man.The patient had a relevant medical history 3 years ago, with abdominal pain as the main clinical manifestation. No abnormalities were observed on physical examination and the tumor was found on abdominal computed tomography.Based on the medical history and postoperative pathology, the patient was diagnosed with CCS with pancreatic metastasis. The patient was successfully treated with surgical interventions, including distal pancreatectomy and sple-nectomy.CONCLUSION This report summarizes the available treatment modalities for CCS and the importance of regular postoperative follow-up for patients with CCS.

文摘The dynamic coupling between the fluttering motions and hydrodynamic characteristics of codend is essential in understanding the trawl selectivity through fish response and the drag force acting on the whole trawl. This study investigated the effect of towing speed, warp length, warp tension, and catch size on the fluttering motions of Antarctic krill trawl codend during net shooting, towing, and hauling by using sea trial data. The time-periodicity of codend oscillation was analyzed by the Morlet wavelet transform method. Results indicated that the period of codend oscillation was between 50 s and 90 s and showed an increasing trend with the warp tension but a decreased value at the towing stage. The coefficient amplitude of codend oscillation was between 0 and 4 at the net shooting and hauling stages, and between 0.2 and 0.6 at the towing stage. The amplitude of codend oscillation increased with the warp tension, towing speed, and catch size, but decreased with the increase of the warp length. In addition, the period of codend oscillation increased with the towing speed at the net shooting and hauling stages, but decreased at the towing stage. These results from codend fluttering motions can improve the understanding of fish behavior and gear shape that modify the hydrodynamic force on the codend instantaneously.

文摘When narrative medicine(NM)was introduced into China, traditional Chinese medicine scholars found that the core concepts advocated by NM are manifested in Chinese yi’an.But why NM echoes with ancient Chinese yi’an?How can we better integrate NM into Chinese medical practices? To answer those questions, this article first investigates how NM establishes itself as a remedy to biomedicine by taking traditional healing models including TCM as its ideal Other. Then, the narrative traditions of both case histories and yi’an are examined respectively. This article argues that NM is searching for a lost tradition of narrative case histories, but yi’an functions as a living tradition of TCM.The Parallel Chart in NM,designed as a complement to the dehumanized hospital chart,is still based on a dichotomy of science and art and a conflictual doctor-patient model. But yi’an exemplifies the holistic and humane healthcare that NM hopes to achieve.A comparison of both genres also inspired us to rethink the genre of yi’an in NM.Thus,it is concluded that yi’an should be viewed as an epistemic genre integrating individualization and generalization,a bridge linking medicine and literature. And narrative yi’an can well serve as a tool for NM in China. It is also proposed that a thick description of yi’an be encouraged to further promote a pluralistic NM in China.
文摘Persistent methicillin-sensitive staphylococcus aureus(MSSA)bacteremia is usually defined as persistent bacteremia despite 7 days of appropriate antibiotic therapy. While nafcillin, cefazolin,or oxacillin are excellent antimicrobial options, persistent MSSA is a commonly encountered phenomenon in clinical practice for which further guidelines on appropriate management are necessitated. Here we highlight two cases of MSSA bacteremia requiring salvage therapy with ertapenem.

文摘Dear Editor,Two cases of primary angle-closure glaucoma(PACG)with persistent shallow anterior chamber after phacoemulsification, intraocular lens(IOL)implantation and goniosynechialysis(GSL)were presented.PACG,which mainly presents with mechanical obstruction of the trabecular meshwork,is clinically characterized by elevated intraocular pressure(IOP)secondary to the apposition of peripheral iris or a synechial closure of the angle[1].A previous study reported that angle-closure glaucoma eyes experienced widening and deepening of anterior chamber angles following cataract extraction and IOL implantation, which could probably normalize the IOP[2].A randomized controlled trial demonstrated that in comparison with laser peripheral iridotomy(LPI),clear-lens extraction showed greater advantages in efficacy and cost-effectiveness and could be proposed as the first-line initial treatment for PACG[3].
文摘Public health strategies have resulted in declines in some respiratory infectious viruses during the COVID-19 pandemic[1],while the way to assess the extent of public health measures and their impacts on respiratory infectious diseases varies[2-4].During the pandemic,Thailand has implemented a strong and rapid governmental response according to its incidence rate[3],and we attempted to investigate the association between the extent of public health measures and other respiratory infectious diseases cases amidst the COVID-19 pandemic in Thailand. In this way,the level of public health measures and their impacts on other kinds of non-SARS-CoV-2 respiratory infections will help the government formulate more efficient policies.

文摘AIM:To assess glaucoma patient satisfaction and follow-up adherence in case management and identify associated predictors to improve healthcare quality and patient outcomes.METHODS:In this cross-sectional study,a total of 119 patients completed a Patient Satisfaction Questionnaire-18 and a sociodemographic questionnaire.Clinical data was obtained from the case management system.Follow-up adherence was defined as completing each follow-up within±30d of the scheduled time set by ophthalmologists during the study period.RESULTS:Average satisfaction scored 78.65±7,with an average of 4.39±0.58 across the seven dimensions.Age negatively correlated with satisfaction(P=0.008),whilst patients with follow-up duration of 2 or more years reported higher satisfaction(P=0.045).Multivariate logistics regression analysis revealed that longer follow-up durations were associated with lower follow-up adherence(OR=0.97,95%CI,0.95-1.00,P=0.044).Additionally,patients with suspected glaucoma(OR=2.72,95%CI,1.03-7.20,P=0.044)and those with an annual income over 100000 Chinese yuan demonstrated higher adherence(OR=5.57,95%CI,1.00-30.89,P=0.049).CONCLUSION:The case management model proves effective for glaucoma patients, with positive adherence rates. The implementation of this model can be optimized in the future based on the identified factors and extended to glaucoma patients in more hospitals.
文摘BACKGROUND Mediastinal emphysema is a condition in which air enters the mediastinum between the connective tissue spaces within the pleura for a variety of reasons. It can be spontaneous or secondary to chest trauma, esophageal perforation,medi-cally induced factors, etc. Its common symptoms are chest pain, tightness in the chest, and respiratory distress. Most mediastinal emphysema patients have mild symptoms, but severe mediastinal emphysema can cause respiratory and circulatory failure, resulting in serious consequences.CASE SUMMARY A 75-year-old man, living alone, presented with sudden onset of severe epigastric pain with chest tightness after drinking alcohol.Due to the remoteness of his residence and lack of neighbors, the patient was found by his nephew and brought to the hospital the next morning after the disease onset.Computed tomography(CT)showed free gas in the abdominal cavity, mediastinal emph-ysema,and subcutaneous pneumothorax.Upper gastrointestinal angiography showed that the esophageal mucosa was intact and the gastric antrum was perforated. Therefore,we chose to perform open gastric perforation repair on the patient under thoracic epidural anesthesia combined with intravenous anesthesia. An operative incision of the muscle layer of the patient's abdominal wall was made, and a large amount of subperitoneal gas was revealed. And a continued incision of the peritoneum revealed the presence of a perforation of approx-imately 0.5 cm in the gastric antrum, which we repaired after pathological examination.Postoperatively, the patient received high-flow oxygen and cough exercises.Chest CT was performed on the first and sixth postoperative days, and the mediastinal and subcutaneous gas was gradually reduced.CONCLUSION After gastric perforation,a large amount of free gas in the abdominal cavity can reach the mediastinum through the loose connective tissue at the esophageal hiatus of the diaphragm, and upper gastrointestinal angiography can clarify the site of perforation. In patients with mediastinal emphysema, open surgery avoids the elevation of the diaphragm caused by pneumoperitoneum compared to laparoscopic surgery and avoids increasing the mediastinal pressure. In addition, thoracic epidural anesthesia combined with intravenous anesthesia also avoids pressure on the mediastinum from mechanical ventilation.
