Non-invasive Doppler ultrasonographic study of cerebral arteries [transcranial Doppler(TCD)] has been extensively applied on both outpatient and inpatient settings. It is performed placing a low-frequency(≤ 2 MHz) transducer on the scalp of the patient over specific acoustic windows, in order to visualize the intracranial arterial vessels and to evaluate the cerebral blood flow velocity and its alteration in many different conditions. Nowadays the most widespread indication for TCD in outpatient setting is the research of right to left shunting, responsable of so called "paradoxical embolism", most often due to patency of foramen ovale which is responsable of the majority of cryptogenic strokes occuring in patients younger than 55 years old. TCD also allows to classify the grade of severity of such shunts using the so called "microembolic signal grading score". In addition TCD has found many useful applications in neurocritical care practice. It is useful on both adults and children for day-to-day bedside assessment of critical conditions including vasospasm in subarachnoidal haemorrhage(caused by aneurysm rupture or traumatic injury), traumatic brain injury, brain stem death. It is used also to evaluate cerebral hemodynamic changes after stroke. It also allows to investigate cerebral pressure autoregulation and for the clinical evaluation of cerebral autoregulatory reserve. 展开更多

Contrast agents are increasingly being used to characterize the vasculature in an organ of interest,to better delineate benign from malignant pathology and to aid in staging and directing therapeutic procedures.We review the mechanisms of action of first,second and third generation contrast agents and their use in various endoscopic procedures in the gastrointestinal tract.Various applications of contrast-enhanced endoscopic ultrasonography include differentiating benign from malignant mediastinal lymphadenopathy,assessment of depth of invasion of esophageal,gastric and gall bladder cancers and visualization of the portal venous system and esophageal varices.In addition,contrast agents can be used to differentiate pancreatic lesions.The use of color Doppler further increases the ability to diagnose and differentiate various pancreatic malignancies.The sensitivity of power Doppler sonography to depict tumor neovascularization can be increased by contrast agents.Contrast-enhanced harmonic imaging is a useful aid in identifying the tumor vasculature and studying pancreatic microperfusion.In the future,these techniques could potentially be used to quantify tumor perfusion,to assess and monitor the efficacy of antiangiogenic agents,to assist targeted drug delivery and allow molecular imaging. 展开更多

Objective To evaluate the safety and efficacy of antegrade selective cerebral perfusion (ASCP) during aortic arch surgery as a means of extending the safe period of systemic circulatory arrest using multimodality neuromonitoring to objectively quantify the physiologic responses Methods In twenty two patients (all less than age 60) scheduled for repair of an aortic arch aneurysm, preoperative verification of effective collateral perfusion through both the carotid and vertebrobasilar arterial systems was documented with transcranial Doppler ultrasonography (TCD) During cardiopulmonary bypass, the sole arterial inflow from the pump was via the right subclavian artery The magnitude of ASCP was quantified by TCD using peak middle cerebral artery velocity, while flow adequacy was measured by continuous regional cerebrovenous oxygen saturation (rSO 2) using dual wavelength spatially resolved near infrared spectroscopy Results All patients experienced an uneventful recovery Flow in the middle cerebral artery became undetectable at ASCP < 5?ml·kg 1 ·min 1 , so adjustments from a 15-20?ml·kg 1 ·min 1 baseline were used to maintain rSO 2 above 50% Furthermore, ASCP flow was highly correlated ( P <0 01) with both peak middle cerebral artery velocity and rSO 2 ( r =0 86 and 0 96, respectively) Conclusion Neuromonitoring guided ASCP may be expected to extend the safe period and is at least partly responsible for the absence of neurologic complications in this patient cohort 展开更多

BACKGROUND: Latest researches at home and abroad indicate that glycerol trinitrate plays its function because it can metabolize into nitrogen monoxide (NO) in vivo. OBJECTIVE: To study the therapeutic effects of NO vector of ultrasonic atomizing inhalation on vertebro-basilar artery insufficiency (VBI) through transcranial Doppler (TCD) detection and serum NO content and indirect effect of TCD on cerebral blood flow changes. DESIGN: Randomized grouping and controlled clinical study. SETTING: Department of Neurology, the Fourth People's Hospital of Jinan. PARTICIPANTS: A total of 130 patients who were diagnosed as VBI were selected from Department of Neurology, the Fourth People's Hospital of Jinan from December 2001 to December 2005. The involved inpatients were checked by CT and MRI, and met the VBI diagnostic standard enacted by the Fourth National Academic Meeting of Cerebrovascular Disease in 1995. All patients and their relatives provided the confumed consent. They were randomly divided into low-dose treatment group (n =60), high-lose treatment group (n =30) and control group (n =40). METHODS: Patients in the low-dose and high-dose treatment groups were given ultrasonic atomizing inhalation of 3 mg and 5 mg glycerol trinitrate, respectively, for 20 minutes, once a day. In addition, ligustrazine and energy mixture were used once a day for three days in a course. Cases in the control group were only given ligustrazine and energy mixture. All selected cases accepted TCD, blood NO content was checked at the time of beginning, after the first time and after a period of treatment. According to the TCD test, VBI patients were divided into two groups (high-low flow velocity). The vertebral artery (VA) and basal artery (BA) of left or right sides were detected by 2 Hz detector via occipital window. MAIN OUTCOME MEASURES: ①Blood flow velocity of systolic phase, blood flow velocity of diastole phase and vascular resistance in left and right VA and BA detected by using TCD before treatment, after treatment for one course; ②content of serum NO indirectly measured by using nitric acid disoxidation technique. RESULTS: All 130 VBI patients were involved in the final analysis. ①Changes of hemodynamic indexes: Systolic phase of VA and diastole phase of BA were higher in low-dose treatment group than that in the control group after first treatment, and there was significant difference (P 〈 0.05); meanwhile, systolic phase and diastole phase of VA and systolic phase of BA were also higher in treatment group than that in the control group after one course (P 〈 0.05). However, both systolic phase and diastole phase of VA and BA were lower in high-dose treatment group than that in the control group after first treatment and one course, and there was significant difference (P 〈 0.05). ②Content of serum NO: After first treatment, there was no significant difference between low-dose treatment group and high-dose treatment group (P 〉 0.05); but both groups were higher than control group, and there was significant difference (P 〈 0.05, 0.01). CONCLUSION: NO vector of ultrasonic atomizing inhalation can improve VBI so as to improve cerebral blood-supply state. 展开更多

Living donor liver transplantation(LDLT) has beenwidely used to treat end-stage liver disease with improvement in surgical technology and the application of new immunosuppressants. Vascular complications after liver transplantation remain a major threat to the survival of recipients. LDLT recipients are more likely to develop vascular complications because of their complex vascular reconstruction and the slender vessels. Early diagnosis and treatment are critical for the survival of graft and recipients. As a non-invasive, cost-effective and non-radioactive method with bedside availability, conventional gray-scale and Doppler ultrasonography play important roles in identifying vascular complications in the early postoperative period and during the follow-up. Recently, with the detailed vascular tracing and perfusion visualization, contrastenhanced ultrasound(CEUS) has significantly improved the diagnosis of postoperative vascular complications. This review focuses on the role of conventional grayscale ultrasound, Doppler ultrasound and CEUS for early diagnosis of vascular complications after adult LDLT. 展开更多

目的调查我国脑血管超声诊断现状、诊断难点以及制定脑血管超声检测若干问题中国专家共识的必要性。方法采用网络问卷调查方式,以各级医疗机构经颅多普勒超声(TCD)和经颅彩色多普勒超声(TCCD)从业人员为调查对象,对脑血管超声检测若干... 目的调查我国脑血管超声诊断现状、诊断难点以及制定脑血管超声检测若干问题中国专家共识的必要性。方法采用网络问卷调查方式,以各级医疗机构经颅多普勒超声(TCD)和经颅彩色多普勒超声(TCCD)从业人员为调查对象,对脑血管超声检测若干问题中国专家共识的需求情况进行问卷调查。问题条目涉及应答人群的基本情况,包括医疗机构所在区域(Q1)、医疗机构级别(Q2),TCD或TCCD从业人员执业范围(Q3)、职称(Q4);脑血管及颈部血管评估模式(Q5);专家共识总体需求情况(Q6)以及TCD诊断相关问题(Q7~Q17)。TCD诊断相关问题有3个维度(诊断标准、多维度综合评估和血运重建围手术期动态监测),共11个条目(Q7~Q17:颅内动脉狭窄或闭塞、血管发育变异、侧支循环判断、血运重建术后的超声动态监测、脑血管痉挛、对比增强TCD检查等问题)。同时设立了开放性问题:脑血管超声最需要完善的内容(Q18)。问卷的信度分析采用可靠性分析,以Cronbach′sα系数≥0.7为信度良好,效度评价采用因子分析。不同条件下对专家共识各条目需求的比较采用χ^(2)检验,等级资料组间比较采用秩和检验(Kruskal-Wallis H)。结果共收回完整、有效的调查问卷1395份,涉及31个省、自治区、直辖市的TCD或TCCD从业人员(西藏自治区无应答),调查具有一定的代表性。信度分析结果显示,Cronbach′sα系数=0.917(F=132.702,P<0.01)。效度分析结果显示,KMO值=0.930(χ^(2)=8478.844,P<0.01),提取3个因子,其累计贡献率为70.600%。应答人群中以三级甲等医院占比最高[64.2%(896/1395)];职称分布以主治医师占比最高[48.5%(676/1395)],其次是高级职称[37.2%(519/1395)];应答人群中,亟需专家共识者占66.6%(929/1395),需要完善现有标准者占32.7%(456/1395)。医疗机构所在区域、从业人员不同职称及不同评估模式的应答人群对专家共识总体需求的差异均有统计学意义(均P<0.05)。结论调查问卷具有较高的信度和效度。应答人群对脑血管超声检测若干问题中国专家共识的需求具有迫切性,专家共识的出台可能有助于建立规范化、精准化评估体系,从而促进脑血管超声检测的质量控制管理及同质化水平。 展开更多

目的探讨对比增强经颅多普勒对房间隔膨出瘤患者心脏水平右向左分流的评估及临床意义。资料与方法回顾性收集2020年6月—2021年7月焦作市人民医院40例房间隔膨出瘤患者和同期242例非膨出瘤患者,均行对比增强经颅多普勒(发泡试验)检查,... 目的探讨对比增强经颅多普勒对房间隔膨出瘤患者心脏水平右向左分流的评估及临床意义。资料与方法回顾性收集2020年6月—2021年7月焦作市人民医院40例房间隔膨出瘤患者和同期242例非膨出瘤患者,均行对比增强经颅多普勒(发泡试验)检查,比较在静息状态下和Valsalva动作后两组右向左分流的总体阳性率;并按监测到的微气泡数量将两组右向左分流进一步分为小量分流和大量分流,比较两组小量分流和大量分流的发生比例,以及Valsalva动作后两组分流量的变化。结果静息状态对比增强多普勒:膨出瘤组右向左分流阳性率高于非膨出瘤组[47.5%(19/40)比29.8%(72/242);χ^(2)=4.95,P=0.03];膨出瘤组小量分流14例(73.7%),大量分流5例(26.3%);非膨出瘤组小量分流61例(84.7%),大量分流11例(15.3%)。静息状态下,两组均以小量分流居多,两组小量和大量的比例差异无统计学意义(χ^(2)=1.26,P=0.264)。Valsalva动作对比增强多普勒:Valsalva动作后两组总体阳性率较静息状态下均显著提高(膨出瘤组77.5%,χ^(2)=8.64,P=0.002;非膨出瘤组48.8%,χ^(2)=80.49,P<0.001);其中大量分流病例数显著增加,膨出瘤组大量分流发生率为64.5%(20/31),非膨出瘤组大量分流发生率为47.5%(56/118),膨出瘤组显著高于非膨出瘤组(χ^(2)=12.58,P<0.001)。结论房间隔膨出瘤患者常伴右向左分流,尤其是Valsalva动作后,对于此类患者,临床上应给予足够的重视,进行规范诊断和治疗。 展开更多

右心声学造影联合经颅多普勒发泡实验在卵圆孔未闭诊断中的应用价值

目的:探讨经胸右心声学造影(cTTE)联合经颅多普勒发泡实验(cTCD)在诊断卵圆孔未闭(PFO)中的应用价值。方法:选取隐源性脑卒中、短暂性脑缺血发作或顽固性偏头痛病人110例,均行cTTE联合cTCD检查,以经食管超声作为诊断PFO的“金标准”,均... 目的:探讨经胸右心声学造影(cTTE)联合经颅多普勒发泡实验(cTCD)在诊断卵圆孔未闭(PFO)中的应用价值。方法:选取隐源性脑卒中、短暂性脑缺血发作或顽固性偏头痛病人110例,均行cTTE联合cTCD检查,以经食管超声作为诊断PFO的“金标准”,均排除PFO以外的其他结构性心脏病。分析单一种方法和联合诊断PFO的诊断率及差异性。结果:110例病人中有88例存在PFO,发病率为80%。静息状态下,cTTE诊断PFO 57例,诊断率70.00%,灵敏度63.64%,特异度95.45%;cTCD诊断PFO 62例,诊断率72.73%,灵敏度68.18%,特异度90.91%,2种方法比较差异无统计学意义(P>0.05);Valsalva动作下,cTTE诊断PFO 61例,诊断率71.82%,灵敏度67.05%,特异度90.91%;cTCD诊断PFO73例,诊断率82.73%,灵敏度80.68%,特异度90.91%。cTTE诊断PFO低于cTCD,两者差异有统计学意义(P<0.05);两者联合诊断PFO 83例,诊断率91.82%,灵敏度92.05%,特异度90.91%,两者联合诊断PFO明显高于单一种方法诊断,差异有统计学意义(P<0.05)。结论:cTTE联合cTCD诊断诊断PFO具有更高的检出率,操作简便,病人无痛苦,易于接受,值得临床推广。 展开更多

目的研究经颅脑血管多普勒(transcraniocerebral vessel Doppler,TVD)超声技术在I型Chiari畸形患儿诊断以及术后疗效评估中的应用价值。方法回顾性分析2018年3月至2021年12月上海交通大学医学院附属儿童医院神经外科收治的27例Ⅰ型Chiar... 目的研究经颅脑血管多普勒(transcraniocerebral vessel Doppler,TVD)超声技术在I型Chiari畸形患儿诊断以及术后疗效评估中的应用价值。方法回顾性分析2018年3月至2021年12月上海交通大学医学院附属儿童医院神经外科收治的27例Ⅰ型Chiari畸形患儿临床资料。按年龄分为两组,学龄前组(1~6岁)15例,学龄后组(7~16岁)12例。两组患儿均于术前、术后24 h及术后1个月行TVD超声探测,以后颅窝内双侧大脑后动脉(posterior cerebral artery,PCA)、椎动脉(vertebral artery,VA)及基底动脉(basilar artery,BA)为靶血管,收集相应靶血管的收缩期峰流速(peak systolic velocity,PSV)、舒张末期流速(end-diastolic velocity,EVD)及搏动指数(pulsative index,PI)等监测指标。分别比较两组患儿手术前后后颅窝动脉血流动力学差异;比较27例患儿术前TVD与MRI诊断结果的一致性,以及术后TVD检查结果与Tator疗效评估结果的一致性。结果学龄前组患儿手术后双侧PCA的PSV较术前增加[左侧:(44.25±13.06)cm/s比(66.76±14.45)cm/s,t=5.148,P=0.023;右侧:(45.12±13.41)cm/s比(65.33±10.12)cm/s,t=5.389,P=0.021]、PI较术前降低[左侧:(1.18±0.42)比(0.91±0.18),t=4.545,P=0.033;右侧:(1.24±0.48)比(0.92±0.13),t=4.776,P=0.028];双侧VA的PSV[左侧:(43.50±11.99)cm/s比(70.94±7.56)cm/s,t=7.042,P=0.008;右侧:(44.56±8.45)cm/s比(68.82±9.02)cm/s,t=6.833,P=0.009]、EVD[左侧:(19.01±9.22)cm/s比(27.18±8.53)cm/s,t=4.587,P=0.032;右侧:(18.28±5.77)cm/s比(28.32±7.26)cm/s,t=4.683,P=0.030]较术前增加,双侧PI较术前降低[左侧:(1.12±0.45)比(0.86±0.19),t=4.712,P=0.029;右侧:(1.31±0.46)比(0.84±0.31)cm/s,t=5.277,P=0.022];BA的PSV[(48.75±16.57)cm/s比(69.17±11.86)cm/s,t=5.413,P=0.019]、EVD[(27.73±7.34)cm/s比(27.18±8.53)cm/s,t=4.738,P=0.027]较术前增加、PI较术前降低[(1.13±0.55)比(0.90±0.28),t=4.721,P=0.030]。学龄后组患儿手术后双侧VA的PSV较术前增加[左侧:(48.16±18.47)cm/s比(53.77±24.73)cm/s,t=4.187,P=0.045;右侧:(45.72±18.53)cm/s比(56.31±19.82)cm/s,t=3.872,P=0.036],BA的PSV[(48.50±11.44)cm/s比(58.17±18.86)cm/s,t=5.108,P=0.024]、EVD[(18.63±9.91)cm/s比(23.19±10.63)cm/s,t=4.763,P=0.029]较术前增加、PI较术前降低[(1.06±0.42)比(0.92±0.25),t=4.572,P=0.032]。27例术前TVD超声检出率低于MRI(χ^(2)=5.511,P=0.019);术后1个月Tator疗效评估症状改善19例(19/27,70.4%)、无改善8例(8/27,29.6%);TVD超声监测指标改善22例(22/27,81.5%)、无改善5例(5/27,18.5%),TVD超声检查与Tator疗效评估结果具有较高的一致性(χ^(2)=0.911,P=0.340)。结论小儿Ⅰ型Chiari畸形的诊断应以MRI检查结果为金标准,但本研究中TVD超声具有一定的准确性和辅助性,可有效显示Ⅰ型Chiari畸形患儿后颅窝动脉的血流动力学变化,间接、无创地评估颅内压,不仅可以帮助医师及时诊断、给予治疗,同时TVD超声对于Ⅰ型Chiari畸形术后疗效评估也有良好的应用价值。 展开更多

目的 探讨经颅多普勒超声发泡试验(c-TCD)与经胸右心声学造影(c-TTE)在卵圆孔未闭(PFO)诊断中的价值。方法 选择2022年1月至2023年9月华东师范大学附属芜湖医院收治的疑似PFO患者119例,常规行经胸超声心动图(TTE)检查,同步完成c-TCD、c-... 目的 探讨经颅多普勒超声发泡试验(c-TCD)与经胸右心声学造影(c-TTE)在卵圆孔未闭(PFO)诊断中的价值。方法 选择2022年1月至2023年9月华东师范大学附属芜湖医院收治的疑似PFO患者119例,常规行经胸超声心动图(TTE)检查,同步完成c-TCD、c-TTE检查。以经食管超声心动图右心声学造影(c-TEE)为PFO诊断金标准,比较c-TCD与c-TTE在PFO诊断中的价值。结果 c-TCD、c-TTE在静息状态下和在Valsalva动作下对心脏右向左分流(RLS)检出率比较差异均无统计学意义(P>0.05);与静息状态相比,在Valsalva动作下c-TCD、c-TTE对RLS的检出率均显著提高(P<0.05)。在静息状态下,c-TTE与c-TCD量化RLS分流等级比较差异有统计学意义(P<0.05),其中c-TCD对大量分流更有诊断优势;在Valsalva动作下,c-TTE与c-TCD量化RLS分流等级差异无统计学意义(P>0.05)。c-TTE与c-TCD检出RLS半定量分级比较差异无统计学意义(P>0.05)。共有33例患者完成c-TEE,以c-TEE为诊断PFO金标准,c-TTE诊断PFO的灵敏度为88.9%、特异度为83.3%、约登指数0.72,c-TCD诊断PFO的灵敏度96.3%、特异度66.7%、约登指数0.63。c-TCD与c-TTE联合诊断PFO的灵敏度为100.0%。结论 在Valsalva动作下c-TTE及c-TCD对PFO的检出率均提高,在静息状态下c-TCD对RLS的分流等级量化更有优势。c-TCD诊断PFO的灵敏度高于c-TTE,特异度低于c-TTE,二者联合诊断PFO比其中任一种方法更有优势。 展开更多

目的应用CDFI、超微血管成像(SMI)、超声造影(CEUS)鉴别诊断肾实性肿块良恶性,探讨多模态超声的临床应用价值。方法选取我院病理证实的肾实性肿块患者59例,均为单发病灶,其中良性13例,恶性46例,均行CDFI、SMI及CEUS检查,比较各方法及多... 目的应用CDFI、超微血管成像(SMI)、超声造影(CEUS)鉴别诊断肾实性肿块良恶性,探讨多模态超声的临床应用价值。方法选取我院病理证实的肾实性肿块患者59例,均为单发病灶,其中良性13例,恶性46例,均行CDFI、SMI及CEUS检查,比较各方法及多模态超声鉴别诊断肾实性肿块良恶性的灵敏度、特异度、准确率。绘制受试者工作特征(ROC)曲线分析各方法及多模态超声对肾实性肿块良恶性的鉴别诊断效能。结果59例患者中,CDFI诊断良性25例,恶性34例,灵敏度67.39%,特异度76.92%,准确率69.49%;SMI诊断良性20例,恶性39例,灵敏度80.43%,特异度84.62%,准确率81.36%;CEUS诊断良性28例,恶性31例,灵敏度60.87%,特异度76.92%,准确率64.41%;多模态超声诊断良性13例,恶性46例,灵敏度93.48%,特异度76.92%,准确率89.83%。ROC曲线分析结果显示,CDFI、SMI、CEUS鉴别诊断肾实性肿块良恶性的曲线下面积(AUC)分别为0.722、0.825、0.689,多模态超声鉴别诊断的AUC为0.852。结论多模态超声对肾实性肿块良恶性的鉴别诊断具有一定的临床应用价值。 展开更多

目的研究经颅多普勒超声(TCD)发泡试验联合声学造影对卵圆孔未闭患者介入封堵术的评估效果。方法回顾分析,采集2020年3月至2021年12月于郑州大学附属郑州中心医院超声科就诊时确定为高度疑似卵圆孔未闭的124例患者的资料。所有患者均完... 目的研究经颅多普勒超声(TCD)发泡试验联合声学造影对卵圆孔未闭患者介入封堵术的评估效果。方法回顾分析,采集2020年3月至2021年12月于郑州大学附属郑州中心医院超声科就诊时确定为高度疑似卵圆孔未闭的124例患者的资料。所有患者均完成经食管超声心电图(TEE)、TCD发泡试验、声学造影检查,因目前无明确的卵圆孔未闭诊断金标准,结合核心文献及既往研究,以TEE作为诊断卵圆孔未闭的可行对照。对确诊为卵圆孔未闭且符合介入封堵术标准的患者实施介入封堵术,并通过TCD发泡试验联合声学造影检查评估介入封堵术的疗效,记录患者术后复发情况。结果124例患者中,115例经TEE明确诊断为卵圆孔未闭,检出率为92.74%。以TEE为对照,TCD发泡试验联合声学造影诊断卵圆孔未闭的灵敏度为98.26%(113/115),特异度为77.78%(7/9),准确率为96.77%(120/124);联合检查诊断与TEE的一致性K值为0.760,具有较好的一致性(P<0.05)。115例患者中有113例符合介入封堵术适应证,均顺利完成治疗,经联合检查评估有109例患者手术有效,有效率为96.46%;113例患者均完成术后随访,随访期间无病死,有2例出现复发,复发率为1.77%。结论TCD发泡试验联合声学造影检查诊断卵圆孔未闭具有较高的准确性,通过准确评估介入封堵术的手术效果,可减少因错误判断手术效果导致的术后复发,保证了介入封堵术治疗的有效性和准确性。 展开更多
