Middle segmental pancreatectomy: A safe and organ-preserving option for benign and low-grade malignant lesions 被引量:18

AIM: To study the feasibility and safety of middle segmental pancreatectomy (MSP) compared with pancreaticoduodenectomy (PD) and extended distal pancreatectomy (EDP). METHODS: We studied retrospectively 36 cases that underwent MSP, 44 patients who underwent PD, and 26 who underwent EDP with benign or low-grade malignant lesions in the mid-portion of the pancreas, between April 2003 and December 2009 in Ruijin Hospital. The perioperative outcomes and long-term outcomes of MSP were compared with those of EDP and PD. Periop-erative outcomes included operative time, intraoperative hemorrhage, transfusion, pancreatic fistula, intraabdominal abscess/infection, postoperative bleeding, reoperation, mortality, and postoperative hospital time. Long-term outcomes, including tumor recurrence, newonset diabetes mellitus (DM), and pancreatic exocrine insufficiency, were evaluated. RESULTS: Intraoperative hemorrhage was 316.1 ± 309.6, 852.2 ± 877.8 and 526.9 ± 414.5 mL for the MSP, PD and EDP groups, respectively (P < 0.05). The mean postoperative daily fasting blood glucose level was significantly lower in the MSP group than in the EDP group (6.3 ± 1.5 mmol/L vs 7.3 ± 1.5 mmol/L, P < 0.05). The rate of pancreatic fistula was higher in the MSP group than in the PD group (42% vs 20.5%, P = 0.039), all of the fistulas after MSP corresponded to grade A (9/15) or B (6/15) and were sealed following conservative treatment. There was no significant difference in the mean postoperative hospital stay between the MSP group and the other two groups. After a mean follow-up of 44 mo, no tumor recurrences were found, only one patient (2.8%) in the MSP group vs five (21.7%) in the EDP group developed new-onset insulin-dependent DM postoperatively (P = 0.029). Moreover, significantly fewer patients in the MSP group than in the PD (0% vs 33.3%, P < 0.001) and EDP (0% vs 21.7%, P = 0.007) required enzyme substitution. CONCLUSION: MSP is a safe and organ-preserving option for benign or low-grade malignant lesions in the neck and proximal body of the pancreas. 展开更多

Judgment of Aroma Types of Middle and Upper Flue-cured Tobacco Leaves Based on Stepwise Discriminant Analysis

[Objective] The aim of this study was to establish mathematical models for judging the aroma types of middle and upper flue-cured tobacco leaves according to the contents and proportions of aroma compositions. [Method] The aroma types of tobacco leaves were judged based on stepwise discriminant analysis, using 63 C3F and 65 B2F tobacco leaf samples from 13 tobacco producing regions in 11 provinces of China (Huili in Sichuan, Baokang in Hubei, Wulong in Chongqing, Lu- oyang in Henan, Zhucheng in Shandong, Wuyi Mountain in Fujian, Malong in Yun- nan, Chuxiong in Yunnan, Bijie in Guizhou, Liuyang in Hunan, Suiyang in Guizhou, Kaiyuan in Liaoning, Nanxiong in Guangdong) as calibration samples, and 67 aroma components as indices. And the Fisher discriminant functions were verified using 21 C3F and 19 B2F tobacco leaf samples. [Result] Variation coefficients of the propor- tions were lower than that of contents of most aroma components in middle and upper leaves of the samples, indicating that the proportions were more stable than contents of aroma components. The proportions of benzyl alcohol, solanone, β-dam- ascone, neophytadiene, farnesylacetone A, palmitic acid, thunbergol, methyl linole- nate and cembratriene-diol were all over 1% in both middle and upper leaves, al- though the dominant aroma components of the same aroma type varied between middle and upper leaves. Moreover, 11, 18, 7 and 11 aroma components were re- spectively introduced into the Fisher discriminant functions established based on the contents and proportions of middle and upper flue-cured tobacco leaves, which ex- hibited accuracy rates of 91.7%, 100%, 91.7% and 91.7% in the judgments of other tobacco leaf samples. The results revealed that the components those determined aroma types in middle leaves were obviously more than in upper leaves. In middle leaves, the accuracy rates of aroma type judgment could be improved by using the proportions rather than the contents of aroma components as indices. However, the functions based on the proportions and the contents of aroma components in upper leaves gave close accuracy rates. [Conclusion] The results of the study will provide references for identifying aroma types of flue-cured tobacco leaves in future work. 展开更多

Farm Production Growth in the Upper and Middle Parts of the Yellow River Basin,China,During 1980-1999 被引量:2

The impact of inputs on farm production growth was evaluated by analyzing the economic data of the upper and middle parts of the Yellow River basin, China for the period of 1980-1999. Descriptive statistics were employed to characterize the temporal trends and spatial patterns in farm production and five pertinent inputs of cultivated cropland, irrigation ratio, agricultural labor, machinery power and chemical fertilizer. Stochastic frontier production function was applied to quantify the dependence of the farm production on these inputs. The growth of farm production was decomposed to reflect the contributions by input growths and change in total factor productivity.. The change in total factor productivity was further decomposed into the changes in technology and in technical efficiency. The gross value of farm production in the region of study increased by 1.6 fold during 1980-1999. Among the five selected farm inputs, machinery power and chemical fertilizer increased by 1.8 and 2.8 fold, respectively. The increases in cultivated cropland, irrigated cropland, and agricultural labor were all less than 0.16 fold. The growth in the farm production was primarily contributed by the increase in the total factor productivity during 1980-1985, and by input growths after 1985. More than 80% of the contributions by input growths were attributed to the increased application of fertilizer and machinery. In the change of total factor productivity, the technology change dominated over the technical efficiency change in the study period except in the period of 1985-1990, implying that institution and investment played important roles in farm production growth. There was a decreasing trend in the technical efficiency in the region of study, indicating a potential to increase farm production by improving the technical efficiency in farm activities. Given the limited natural resources in the basin, the results of this study suggested that, for a sustainable growth of farm production in the area, efforts should be directed to technology progress and improvement in technical efficiency in the use of available resources. 展开更多

Twelve paleocommunities dominated by benthic brachiopod are recognized in the Givetian-Frasnian stages of the Devonian in the Longmenshan area, southwestern China, in which two kinds of brachiopod community replacement are classified. One is the abrupt replacement, represented by abrupt alternation between the Leiorhynchus community and Zhonghuacoelia-Striatopugnax community in the Frasnian Tuqiaozi Formation. The other is the gradual one, developed in the Givetian Guanwushan Formation, which had been completed by the shift of the Independatrypa lemma-Uncinulus heterocostellis-Emanuella takwanensis community via the Sinospongophyllum irregulare-Pseudomicroplasma fongi community to the Clathrocoilona spissa-Hexagonaria composite reef community. According to analyses of the paleocommunities, either the abrupt or gradual paleocommunity replacement of the Middle-Upper Devonian in the Longmenshan area is suggested as a response to the 5th-order sea level fluctuation due to the replacements of the paleocommunities in a tracts-system of depositional sequence. It is supposed that changes of paleocommunity diversity, one of the results of paleocommunity replacement, are depended on the range and magnitude of sea level fluctuation, but there is not a linear relationship between them. Furthermore, a suggestion is proposed that the concept of paleocommunity succession seem to be abandoned in the paleocommunity analysis because it almost never be practiced to recover the information of community succession in the geological record at present. 展开更多

[Objective] This study aimed to establish mathematical models for judging the aroma types of flue-cured tobacco leaves from the upper and middle parts of plants. [Method] A total of 128 samples (63 C3F and 65 B2F) from 11 main tobac- co production provinces of China were selected as materials. Stepwise discriminant analysis was applied to samples with different aroma types and discriminant function was expressed with the proportions of 67 aroma components in total aroma con- stituents as the index. [Result] The ratio of most aroma components in clear and full aroma tobacco leaves was higher than that in middle aroma leaves. The ratios of 51, 43 and 40 aroma components of clear, middle and full aroma tobaccos were higher in upper leaves than that in middle leaves. Aroma components dominated certain aroma types differed between middle and upper leaves. The proportions of 18 and 11 aroma components in upper and middle leaves were led in the stepwise discriminant function respectively. Self-validation and cross-validation methods were applied to evaluate the original samples, and the accuracy rates reached 100% and 98.6% on middle leaves, 96.37% and 94.4% on upper leaves. The accuracy rates on some other samples reached 100% on middle leaves and 91.7% on upper leaves predicted with the model. [Conclusion] The ratio of aroma components as discriminant index could improve discriminant accuracy significantly in the middle and upper leaves. It could be used to analyze aroma types objectively, accurately and quickly. 展开更多

BACKGROUND Laparoscopic hepatectomy is a proven safe and technically feasible approach for liver tumor resection,but laparoscopic anatomical SVIII resection(LASVIIIR)remains rarely reported due to poor accessibility,difficult exposure,and the deep-lying Glissonean pedicle.This study examined the safety,feasibility,and perio-perative outcomes of LASVIIIR via a middle hepatic fissure approach at our in-stitution.AIM To investigate the safety,feasibility,and perioperative outcomes of LASVIIIR via a middle hepatic fissure approach at our institution.METHODS From November 2017 to December 2022,all patients with a liver tumor who underwent LASVIIIR were enrolled.The perioperative outcomes and postope-rative complications were evaluated.RESULTS Thirty-four patients underwent LASVIIIR via a middle hepatic fissure approach from the side or cranio side and were included.The mean operation time was 164±54 minutes,and the intra-operative blood loss was 100 mL(range:20-1000 mL).The mean operative times were,respectively,152±50 minutes and 222±29 minutes(P=0.001)for the caudal side and cranial side approaches.In addition,the median blood loss volumes were 100 mL(range:20-300 mL)and 250 mL(range:20-1000 mL),respectively,for the caudal and cranial sides(P=0.064).Three patients treated using the cranial side approach experienced bile leakage,while 1 patient treated using the caudal side approach had subphrenic collection and underwent percutaneous drainage to successfully recover.There were no differences regarding postoperative hospital stays for the caudal and cranial side approaches[9(7-26)days vs 8(8-19)days](P=0.226).CONCLUSION LASVIIIR resection remains a challenging operation,but the middle hepatic fissure approach is a reasonable and easy-to-implement technique. 展开更多

Using geographic information system (GIS) techniques and the newest seasonal and annual average precipitation data of 679 meteorological stations from 1971 to 2000, the multiple regressions equations of the precipitation and topographical variables are established to extract the effect of topography on the annual and seasonal precipitation in the upper-middle reaches of the Yangtze River. Then, this paper uses a successive interpolation approach (SIA), which combines GIS techniques with the multiple regressions, to improve the accuracy of the spatial interpolation of annual and seasonal rainfall. The results are very satisfactory in the case of seasonal rainfall, with the relative error of 6.86%, the absolute error of 13.07 mm, the average coefficient of variation of 0.070, and the correlation coefficient of 0.9675; in the case of annual precipitation, with the relative error of 7.34%, the absolute error of 72.1 mm, the average coefficient of variation of 0.092, and the correlation coefficient of 0.9605. The analyses of annual mean precipitation show that the SIA calculation of 3-5 steps considerably improves the interpolation accuracy, decreasing the absolute error from 211.0 mm to 62.4 mm, the relative error from 20.74% to 5.97%, the coefficient of variation from 0.2312 to 0.0761, and increasing the correlation coefficient from 0.5467 to 0.9619. The SIA iterative results after 50 steps identically converge to the observed precipitation. 展开更多

The Upper Ordovician Wufeng-Lower Silurian Longmaxi and the Lower Cambrian Qiongzhusi shales are the major targets for shale gas exploration and development in China.Although the two organic-rich shales share similar distribution ranges and thicknesses,they exhibit substantially different exploration and development results.This work analyzed the nanopore structures of the shale reservoirs in this region.Pore development of 51 shale samples collected from various formations and locations was compared using the petromineralogical,geochemical,structural geological and reservoir geological methods.The results indicate that the reservoir space in these shales is dominated by organic pores and the total pore volume of micropores,mesopores,macropores in different tectonic areas and formations show different trends with the increase of TOC.It is suggested that organic pores of shale can be well preserved in areas with simple structure and suitable preservation conditions,and the shale with smaller maximum ancient burial depth and later hydrocarbongeneration-end-time is also more conducive to pore preservation.Organic pore evolution models are established,and they are as follows:①Organic matter pore development stage,②Early stage of organic matter pore destruction,and③late stage of organic matter pore destruction.The areas conducive to pore development are favorable for shale gas development.Research results can effectively guide the optimization and evaluation of favorable areas of shale gas. 展开更多

Stable carbon isotope ratio (δ13Ccarb) analysis has been widely applied to the study of the inter-conti- nental or global marine carbonate correlation. Large-scale Cambrian-Ordovician carbonate platforms were developed in the Tarim Basin. But research on fluctuation character- istics and global correlation of δ13Ccarb is still weak. Based on conodont biostratigraphy and whole-rock δ13Ccarb data in the Tahe oil-gas field of the northern Tarim Basin, the global correlation and genesis of positive carbon isotope excursions in the Darriwilian--Early Katian was exam- ined. Three positive excursions were identified in the Tahe oil-gas field including the middle Darriwilian carbon iso- tope excursion (MDICE), the Guttenberg carbon isotope excursion (GICE), and a positive excursion within the Pygodus anserinus conodont zone which is named the Early Sandbian carbon isotope excursion (ESICE) in this paper. Furthermore, these positive excursions had no direct relation with sea level fluctuations. MDICE and GICE could be globally correlated. The Middle-Upper Ordovi- cian Saergan Formation source rocks of the Kalpin outcrops were in accordance with the geological time of MDICE and ESICE. Correlation of Middle and Late Cambrian cyclic carbonates from the widely separated Zhangxia section, Qingxing section, Xishan section and Zhaogezhuang section has defined five major third-order depositional sequences. Correlation of biostratigraphy is used to establish the equivalency of stratigraphic sequences between individual sections. Even though the individ- ual sections are composed of different lithofacies and meter-scale cycles that reflect the varied environmental settings in the localities, the overall upward shallowing character of individ- ual sequences is evident. The sequences, named on their relation to the regional stages in range, are in ascending order: Zhangxia Sequence, Gushan Sequence, Guchang Sequence, Changshan Sequence and Fengshan Sequence, of which the upper four sequences were correlated with those defined by Osleger and Read in the United States, with the aid of biostratigraphic correlation. The Middle/Upper Cambrian boundary is tentatively suggested to be between the Guchang Sequence and the Changshan Sequence in North China. The optimization of the existing litho-and regional chronostratigraphic boundaries is discussed based on the sequence boundaries. 展开更多

[Objective] Species composition and flora of vascular plants in the middle and upper reaches of the Yarlung Zangbo River were analyzed,to provide scientific basis for the estimation and protection of plant diversity,construction of ecological security shelter for the Tibetan Plateau.[Methods] In the middle and upper reaches of the Yarlung Zangbo River,quadrat (line transect) method was applied to investigate the phytocoenoses in sample plots,and relative literature was used to analyze the floristic composition and floristic element of vascular plants in the middle and upper reaches of the Yarlung Zangbo River.[Results]① There are 96 species (including subspecies and variations),69 genera of 35 vascular plant families,accounting for 16.83%,5.48%,1.66% and 11.63%,2.02%,0.31% of total vascular plant families,genera and species in Tibet and China.② There are 24 monotypic-genus families,accounting for 68.57% of the total family number in the local area;22 monotypic-species families,accounting for 62.86% of the total family number in the local area.Local flora shows the characteristics of ancient relict,such as Euphorbiaceae,Rubiaceae,Liliaceae and Ranunculaceae,they are all ancient primitive groups of the third flora and the fourth flora.③ Local vascular plant genera fall into 10 areal types and 8 variations.④ Temperate elements are much more than tropical elements,R/T is 0.19,indicating that vascular plants have typical temperate nature and residual tropical nature.There are only a few endemic genera,but more endemic species to the local area,and these species are closely related,of which there are some transitional species,indicating that local flora is too young to experience more differentiation and specialization.⑤ There are 24 Chinese endemic species,accounting for 25% of total number of species in the local area;among which there are 12 endemic species to Tibet,accounting for 50% of endemic species to China,12.5% of total number of species in the local area.Four key wild plant species are included in the second batch of national key protection.[Conclusions]① There are scarce plant groups in the local area.② Among the obvious advantages,single-genus (species) families possess the absolute advantage,and the flora has ancient origins.③ The local flora has diversified distribution types,and the complicated geographical elements.④ Temperate species possess obvious advantages,and the flora is young.⑤ Rich unique and rare species have high species diversity protection and research value. 展开更多

This paper explores and identifies the rules of the unsteady evolution of COD in the upper and middle reaches of Minjiang River through multi-scale analysis on the weekly COD data for the last six years provided by the Water Quality Monitoring Station,Minjiang River Bridge,Leshan City,Sichuan Province.The results of the wavelet analysis indicate that the COD index oscillates alternatively and differently at each time-scale in an oneyear cycle,and the density of COD,subjected to the runoff volume,the effects of Wenchuan Earthquake and agricultural seeding and industrial construction,etc.,is high in winter and spring,mild in summer and autumn. 展开更多
