The Chinese traditional medicine,agarwood,is a commonly used medicine for regulating qi,which has many clinical applications because of its unique curative effect.In this study,CiteSpace software was used to visually ...The Chinese traditional medicine,agarwood,is a commonly used medicine for regulating qi,which has many clinical applications because of its unique curative effect.In this study,CiteSpace software was used to visually analyze the application and development trend of agarwood in the literature from CNKI website in the period of 2007-2022.The analysis results showed that the research on agarwood has basically formed core groups of authors,and universities and their affiliated hospitals are main publishing institutions.Moreover,in this study,several research directions of agarwood were also summarized,including clinical research,chemical composition,structural identification and quality standards,showing that agarwood has rich and flexible application prospects in many aspects.On this basis,several suggestions were put forward:strengthening the cooperation between universities and research institutes and building a scientific research cooperation community,and promoting the combination of clinical research and laboratory research.展开更多
Objective:To evaluate the current state of research and areas of interest for traditional Chinese medicine(TCM)in the field of colorectal cancer treatment.Methods:Related papers published between January 1,2012,and No...Objective:To evaluate the current state of research and areas of interest for traditional Chinese medicine(TCM)in the field of colorectal cancer treatment.Methods:Related papers published between January 1,2012,and November 27,2021,were found using the Web of Science Core Collection Science Citation Index Expanded.Using CiteSpace's network map generation capability,we then determined the top writers,organizations,countries,keywords,co-cited writers,journals,references,and research trends.Results:This investigation yielded a total of 336 relevant papers.China is the most productive country.Shanghai University of Traditional Chinese Medicine is the leading institution.The journal with the most popularity and publishing volume is Evidence-based Complementary and Alternative Medicine.The author with the most citations and centrality is Lin JM.The terms"epithelial-mesenchymal transition,""cell cycle arrest,""apoptosis,"and"autophagy"are highly frequent and have a high betweenness centrality.Conclusion:According to the results,research on natural products,traditional Chinese medicine(TCM)extracts,and the molecular mechanisms of TCM chemical constituents constitutes the primary focus within TCM cancer treatment investigations.In recent years,there has been a surge of interest in exploring the role of gut microbiota in TCM chemical constituents research,particularly in its ability to induce apoptosis and autophagy in tumor cells,thereby suppressing tumor cell proliferation,metastasis,and invasion.However,due to the intricate composition of TCM and existing technical limitations,the underlying principles guiding TCM's efficacy in treating colorectal cancer remain unclear and warrant further investigation.展开更多
BACKGROUND The cognitive impairment in type 2 diabetes mellitus(T2DM)is a multifaceted and advancing state that requires further exploration to fully comprehend.Neu-roinflammation is considered to be one of the main m...BACKGROUND The cognitive impairment in type 2 diabetes mellitus(T2DM)is a multifaceted and advancing state that requires further exploration to fully comprehend.Neu-roinflammation is considered to be one of the main mechanisms and the immune system has played a vital role in the progression of the disease.AIM To identify and validate the immune-related genes in the hippocampus associated with T2DM-related cognitive impairment.METHODS To identify differentially expressed genes(DEGs)between T2DM and controls,we used data from the Gene Expression Omnibus database GSE125387.To identify T2DM module genes,we used Weighted Gene Co-Expression Network Analysis.All the genes were subject to Gene Set Enrichment Analysis.Protein-protein interaction network construction and machine learning were utilized to identify three hub genes.Immune cell infiltration analysis was performed.The three hub genes were validated in GSE152539 via receiver operating characteristic curve analysis.Validation experiments including reverse transcription quantitative real-time PCR,Western blotting and immunohistochemistry were conducted both in vivo and in vitro.To identify potential drugs associated with hub genes,we used the Comparative Toxicogenomics Database(CTD).RESULTS A total of 576 DEGs were identified using GSE125387.By taking the intersection of DEGs,T2DM module genes,and immune-related genes,a total of 59 genes associated with the immune system were identified.Afterward,machine learning was utilized to identify three hub genes(H2-T24,Rac3,and Tfrc).The hub genes were associated with a variety of immune cells.The three hub genes were validated in GSE152539.Validation experiments were conducted at the mRNA and protein levels both in vivo and in vitro,consistent with the bioinformatics analysis.Additionally,11 potential drugs associated with RAC3 and TFRC were identified based on the CTD.CONCLUSION Immune-related genes that differ in expression in the hippocampus are closely linked to microglia.We validated the expression of three hub genes both in vivo and in vitro,consistent with our bioinformatics results.We discovered 11 compounds associated with RAC3 and TFRC.These findings suggest that they are co-regulatory molecules of immunometabolism in diabetic cognitive impairment.展开更多

·AIM: To review existing data for the prevalence of corrected, uncorrected, and inadequately corrected refractive errors and spectacle wear in Hungary.·METHODS: Data from two nationwide cross-sectional studi...·AIM: To review existing data for the prevalence of corrected, uncorrected, and inadequately corrected refractive errors and spectacle wear in Hungary.·METHODS: Data from two nationwide cross-sectional studies were analysed. The Rapid Assessment of Avoidable Blindness study collected population-based representative national data on the prevalence of visual impairment due to uncorrected refractive errors and spectacle coverage in 3523 people aged ≥50y(Group I). The Comprehensive Health Test Program of Hungary provided data on the use of spectacles in 80 290 people aged ≥18y(Group Ⅱ).·RESULTS: In Group I, almost half of the survey population showed refractive errors for distant vision, about 10% of which were uncorrected(3.2% of all male participants and 5.0% of females). The distance spectacle coverage was 90.7%(91.9% in males;90.2% in females). The proportion of inadequate distance spectacles was found to be 33.1%. Uncorrected presbyopia was found in 15.7% of participants. In all age groups(Group Ⅱ), 65.4% of females and 56.0% of males used distance spectacles,and approximately 28.9% of these spectacles were found to be inappropriate for dioptric power(with 0.5 dioptres or more). The prevalence of inaccurate distance spectacles was significantly higher in older age groups(71y and above) in both sexes.·CONCLUSION: According to this population-based data, uncorrected refractive errors are not rare in Hungary. Despite recent national initiatives, fur ther steps are required to reduce uncorrected refractive errors and associated negative effects on vision, such as avoidable visual impairment.展开更多

The purpose of this study is to conduct a bibliometric analysis of the field of digital healthcare and to understand the research background and development trend in this field. Methods: A visual analysis of authors, institutions, journals and keywords was conducted using CiteSpace 5.8R3 software. Results: A total of 1646 digital healthcare-related retrieved from WoS and PubMed studies. There was an overall upward trend in the number of digital healthcare publications, with the highest number of publications in 2021 (290). The author AZIZ SHEIKH is ranked first in the number of published articles (13), while King Saud University (23) is the research institution with the most articles. Keyword clustering showed that the first cluster was data security;the common high frequency keywords that appeared were systems (85), artificial intelligence (82), mobile health (70), internet (61), and technology (57). Digital healthcare, artificial intelligence, healthcare services, machine learning and deep learning are the hotspot of current research. Conclusion: This paper summarises the state of the art in digital healthcare research. Using statistical analysis and network visualisation, it highlights the background, trends and hot topics in digital healthcare research. The paper finds that there is significant potential for artificial intelligence to help bridge the digital divide and reduce health inequalities. To understand the current state, hot trends and future directions of digital healthcare research, this paper can serve as a reference. .展开更多

Methods: Relevant literature was searched, screened and downloaded from the Web of Science (WOS) database, and bibliometric and visualization analyses were performed using CiteSpace VI software. Results: Through the search and screening of related literature, 2443 English articles were finally included. Among them, most of the Chinese publishers were mainly universities and had less connection with each other, while the foreign publishers were mainly universities and medical institutions in the United States and Europe, and had close connection with each other. The research content of Chinese scholars mainly focuses on the assessment of patients’ life and psychological status, as well as the assessment of cardiac function. Foreign research focuses on physical training assessment, disease perception, etc. Conclusion: Through visualizing relevant research with CiteSpace VI software in the form of a knowledge map, the research frontiers and trends in the field of cardiac rehabilitation for coronary heart disease patients in China and abroad can be discovered more intuitively. Compared with foreign research, the development of cardiac rehabilitation for coronary heart disease patients in China is relatively slow and insufficient, and institutions lack cooperation. Cataract is the leading cause of visual impairment globally.The scarcity and uneven distribution of ophthalmologists seriously hinder early visual impairment grading for cataract patients in the clin-ic.In this study,...Cataract is the leading cause of visual impairment globally.The scarcity and uneven distribution of ophthalmologists seriously hinder early visual impairment grading for cataract patients in the clin-ic.In this study,a deep learning-based automated grading system of visual impairment in cataract patients is proposed using a multi-scale efficient channel attention convolutional neural network(MECA_CNN).First,the efficient channel attention mechanism is applied in the MECA_CNN to extract multi-scale features of fundus images,which can effectively focus on lesion-related regions.Then,the asymmetric convolutional modules are embedded in the residual unit to reduce the infor-mation loss of fine-grained features in fundus images.In addition,the asymmetric loss function is applied to address the problem of a higher false-negative rate and weak generalization ability caused by the imbalanced dataset.A total of 7299 fundus images derived from two clinical centers are em-ployed to develop and evaluate the MECA_CNN for identifying mild visual impairment caused by cataract(MVICC),moderate to severe visual impairment caused by cataract(MSVICC),and nor-mal sample.The experimental results demonstrate that the MECA_CNN provides clinically meaning-ful performance for visual impairment grading in the internal test dataset:MVICC(accuracy,sensi-tivity,and specificity;91.3%,89.9%,and 92%),MSVICC(93.2%,78.5%,and 96.7%),and normal sample(98.1%,98.0%,and 98.1%).The comparable performance in the external test dataset is achieved,further verifying the effectiveness and generalizability of the MECA_CNN model.This study provides a deep learning-based practical system for the automated grading of visu-al impairment in cataract patients,facilitating the formulation of treatment strategies in a timely man-ner and improving patients’vision prognosis.展开更多

Objectives In order to provide theoretical support for clinical standardization and promotion of the use of Hongfen(hydrargyrum oxydatum crudum),a mercury-containing Chinese medicine,as well as to make it easier to ap...Objectives In order to provide theoretical support for clinical standardization and promotion of the use of Hongfen(hydrargyrum oxydatum crudum),a mercury-containing Chinese medicine,as well as to make it easier to apply traditional Chinese topical preparations in a more contemporary manner.Methods We screened the topical research literature on Hongfen.Firstly,we analyzed the current research status of Hongfen statistically in terms of disciplinary distribution,research institutions,funding,publication volume,research types,and research focus of the literature;secondly,we analyzed the research status of Hongfen by MeSH term analysis,abstract and combined with the herbal literature,the indications,usage,dosage,and adverse effects of Hongfen were statistically analyzed,and keyword co-occurrence analysis and data visualization were performed using python+pyecharts.Results Early studies on Hongfen were mainly on the evaluation of the formulation process,while the current studies are mainly focused on clinical effectiveness and safety,with a considerable part of the studies also focused on drug toxicology,metabolism of mercury components target organ toxicity studies,while the basic studies on its mechanism of action are less.The clinical application is mainly focused on chronic ulcerated sores,refractory sinus fistula,and other surgical sore diseases,the use of which is mostly in the form of compounding and adding excipients,the use of which requires consideration of the accumulation of drug toxicity and attention to drug dosage and duration of use in order to reduce the occurrence of clinical side effects.Conclusion Hongfen is a type of mineral medicine that is extremely toxic but has a known curative effect.Its clinical use is low-risk,and its advantages greatly outweigh its drawbacks.Molecular biology technology should be actively carried out to analyze the pharmacological mechanism of Hongfen,which will play a positive role in the transmission and promotion of traditional Chinese medicine topical preparations.展开更多

Results: A total of 1,576 documents were included, and the annual number of documents showed a fluctuating upward trend, forming a relatively stable research team represented by authors such as LYU Xiaodong, PANG Lijian and LIU Chuang;According to the atlas of document issuing institutions, Shandong University of Traditional Chinese Medicine and its affiliated hospitals ranked first in the number of documents issued, and the cooperation between institutions is dominated by the University of traditional Chinese medicine and its affiliated hospitals;Keyword cluster analysis shows that a large number of studies have been carried out in the field of etiology and pathogenesis, TCM compound, clinic and experiment. Objective: To assess and compare the clinical efficacy and safety of cognitive enhancers(donepezil, galantamine, rivastigmine, and memantine) on cognition, behavior, function, and global status in patients with vascul...Objective: To assess and compare the clinical efficacy and safety of cognitive enhancers(donepezil, galantamine, rivastigmine, and memantine) on cognition, behavior, function, and global status in patients with vascular cognitive impairment.Data sources: The initial literature search was performed with PubMed, EMBASE, the Cochrane Methodology Register, the Cochrane Central Register of Controlled Trials, and Cumulative Index to Nursing & Allied Health(CINAHL) from inception to January 2018 for studies regarding donepezil, galantamine, rivastigmine, and memantine for treatment of vascular cognitive impairment.Data selection: Randomized controlled trials on donepezil, galantamine, rivastigmine, and memantine as monotherapy in the treatment of vascular cognitive impairment were included. A Bayesian network meta-analysis was conducted. Outcome measures: Efficacy was assessed by changes in scores of the Alzheimer's Disease Assessment Scale, cognitive subscale, Mini-Mental State Examination, Neuropsychiatric Inventory scores and Clinician's Interview-Based Impression of Change Scale Plus Caregiver's Input, Activities of Daily Living, the Clinical Dementia Rating scale. Safety was evaluated by mortality, total adverse events(TAEs), serious adverse events(SAEs), nausea, vomiting. diarrhea, or cerebrovascular accidents(CVAs). Results: After screening 1717 citations, 12 randomized controlled trials were included. Donepezil and rivastigmine(mean difference(e) = –0.77, 95% confidence interval(CI): 0.25–1.32; MD = 1.05, 95% CI: 0.18–1.79) were significantly more effective than placebo in reducing Mini-Mental State Examination scores. Donepezil, galantamine, and memantine(MD = –1.30, 95% CI: –2.27 to –0.42; MD = –1.67, 95% CI: –3.36 to –0.06; MD = –2.27, 95% CI: –3.91 to –0.53) showed superior benefits on the Alzheimer's Disease Assessment Scale–cognitive scores compared with placebo. Memantine(MD = 2.71, 95% CI: 1.05–7.29) improved global status(Clinician's Interview-Based Impression of Change Scale Plus Caregiver's Input) more than the placebo. Safety results revealed that donepezil 10 mg(odds ratio(OR) = 3.04, 95% CI: 1.86–5.41) contributed to higer risk of adverse events than placebo. Galantamine(OR = 5.64, 95% CI: 1.31–26.71) increased the risk of nausea. Rivastigmine(OR = 16.80, 95% CI: 1.78–319.26) increased the risk of vomiting. No agents displayed a significant risk of serious adverse events, mortality, cerebrovascular accidents, or diarrhea.Conclusion: We found significant efficacy of donepezil, galantamine, and memantine on cognition. Memantine can provide significant efficacy in global status. They are all safe and well tolerated.展开更多
Mild cognitive impairment(MCI)is a prodrome of Alzheimer’s disease pathology.Cognitive impairment patients often have a delayed diagnosis because there are no early symptoms or conventional diagnostic methods.Exosome...Mild cognitive impairment(MCI)is a prodrome of Alzheimer’s disease pathology.Cognitive impairment patients often have a delayed diagnosis because there are no early symptoms or conventional diagnostic methods.Exosomes play a vital role in cell-to-cell communications and can act as promising biomarkers in diagnosing diseases.This study was designed to identify serum exosomal candidate proteins that may play roles in diagnosing MCI.Mass spectrometry coupled with tandem mass tag approach-based non-targeted proteomics was used to show the differentially expressed proteins in exosomes between MCI patients and healthy controls,and these differential proteins were validated using immunoblot and enzyme-linked immunosorbent assays.Correlation of cognitive performance with the serum exosomal protein level was determined.Nanoparticle tracking analysis suggested that there was a higher serum exosome concentration and smaller exosome diameter in individuals with MCI compared with healthy controls.We identified 69 exosomal proteins that were differentially expressed between MCI patients and healthy controls using mass spectrometry analysis.Thirty-nine exosomal proteins were upregulated in MCI patients compared with those in control patients.Exosomal fibulin-1,with an area under the curve value of 0.81,may be a biomarker for an MCI diagnosis.The exosomal protein signature from MCI patients reflected the cell adhesion molecule category.In particular,higher exosomal fibulin-1 levels correlated with lower cognitive performance.Thus,this study revealed that exosomal fibulin-1 is a promising biomarker for diagnosing MCI.展开更多

文摘The Chinese traditional medicine,agarwood,is a commonly used medicine for regulating qi,which has many clinical applications because of its unique curative effect.In this study,CiteSpace software was used to visually analyze the application and development trend of agarwood in the literature from CNKI website in the period of 2007-2022.The analysis results showed that the research on agarwood has basically formed core groups of authors,and universities and their affiliated hospitals are main publishing institutions.Moreover,in this study,several research directions of agarwood were also summarized,including clinical research,chemical composition,structural identification and quality standards,showing that agarwood has rich and flexible application prospects in many aspects.On this basis,several suggestions were put forward:strengthening the cooperation between universities and research institutes and building a scientific research cooperation community,and promoting the combination of clinical research and laboratory research.

文摘Objective:To evaluate the current state of research and areas of interest for traditional Chinese medicine(TCM)in the field of colorectal cancer treatment.Methods:Related papers published between January 1,2012,and November 27,2021,were found using the Web of Science Core Collection Science Citation Index Expanded.Using CiteSpace's network map generation capability,we then determined the top writers,organizations,countries,keywords,co-cited writers,journals,references,and research trends.Results:This investigation yielded a total of 336 relevant papers.China is the most productive country.Shanghai University of Traditional Chinese Medicine is the leading institution.The journal with the most popularity and publishing volume is Evidence-based Complementary and Alternative Medicine.The author with the most citations and centrality is Lin JM.The terms"epithelial-mesenchymal transition,""cell cycle arrest,""apoptosis,"and"autophagy"are highly frequent and have a high betweenness centrality.Conclusion:According to the results,research on natural products,traditional Chinese medicine(TCM)extracts,and the molecular mechanisms of TCM chemical constituents constitutes the primary focus within TCM cancer treatment investigations.In recent years,there has been a surge of interest in exploring the role of gut microbiota in TCM chemical constituents research,particularly in its ability to induce apoptosis and autophagy in tumor cells,thereby suppressing tumor cell proliferation,metastasis,and invasion.However,due to the intricate composition of TCM and existing technical limitations,the underlying principles guiding TCM's efficacy in treating colorectal cancer remain unclear and warrant further investigation.

文摘BACKGROUND The cognitive impairment in type 2 diabetes mellitus(T2DM)is a multifaceted and advancing state that requires further exploration to fully comprehend.Neu-roinflammation is considered to be one of the main mechanisms and the immune system has played a vital role in the progression of the disease.AIM To identify and validate the immune-related genes in the hippocampus associated with T2DM-related cognitive impairment.METHODS To identify differentially expressed genes(DEGs)between T2DM and controls,we used data from the Gene Expression Omnibus database GSE125387.To identify T2DM module genes,we used Weighted Gene Co-Expression Network Analysis.All the genes were subject to Gene Set Enrichment Analysis.Protein-protein interaction network construction and machine learning were utilized to identify three hub genes.Immune cell infiltration analysis was performed.The three hub genes were validated in GSE152539 via receiver operating characteristic curve analysis.Validation experiments including reverse transcription quantitative real-time PCR,Western blotting and immunohistochemistry were conducted both in vivo and in vitro.To identify potential drugs associated with hub genes,we used the Comparative Toxicogenomics Database(CTD).RESULTS A total of 576 DEGs were identified using GSE125387.By taking the intersection of DEGs,T2DM module genes,and immune-related genes,a total of 59 genes associated with the immune system were identified.Afterward,machine learning was utilized to identify three hub genes(H2-T24,Rac3,and Tfrc).The hub genes were associated with a variety of immune cells.The three hub genes were validated in GSE152539.Validation experiments were conducted at the mRNA and protein levels both in vivo and in vitro,consistent with the bioinformatics analysis.Additionally,11 potential drugs associated with RAC3 and TFRC were identified based on the CTD.CONCLUSION Immune-related genes that differ in expression in the hippocampus are closely linked to microglia.We validated the expression of three hub genes both in vivo and in vitro,consistent with our bioinformatics results.We discovered 11 compounds associated with RAC3 and TFRC.These findings suggest that they are co-regulatory molecules of immunometabolism in diabetic cognitive impairment.

文摘The landscape environment of urban blocks plays a significant role in improving the comfort of urban thermal environment and promoting green and high-quality development.The 342 papers related to the research on the impact of urban block landscape environment on thermal comfort in China,collected by CNKI(China National Knowledge Infrastructure)from 2002 to 2022,are used as the research object.Through bibliometric statistical analysis,LLR algorithm,and cluster analysis,the current research status of the impact of urban block landscape environment on thermal comfort in China is analyzed and processed,and its external characteristics are identified.Using the information visualization software CiteSpace,the research topics in the field of the impact of urban block landscape environment on thermal comfort are presented in the form of knowledge graphs.Through co-occurrence analysis of keywords and trend of word frequency changes,the development trends of research hotspots and cutting-edge fields of the impact of urban block landscape environment on thermal comfort are determined,hoping to provide reference for future research in this field.

文摘·AIM: To review existing data for the prevalence of corrected, uncorrected, and inadequately corrected refractive errors and spectacle wear in Hungary.·METHODS: Data from two nationwide cross-sectional studies were analysed. The Rapid Assessment of Avoidable Blindness study collected population-based representative national data on the prevalence of visual impairment due to uncorrected refractive errors and spectacle coverage in 3523 people aged ≥50y(Group I). The Comprehensive Health Test Program of Hungary provided data on the use of spectacles in 80 290 people aged ≥18y(Group Ⅱ).·RESULTS: In Group I, almost half of the survey population showed refractive errors for distant vision, about 10% of which were uncorrected(3.2% of all male participants and 5.0% of females). The distance spectacle coverage was 90.7%(91.9% in males;90.2% in females). The proportion of inadequate distance spectacles was found to be 33.1%. Uncorrected presbyopia was found in 15.7% of participants. In all age groups(Group Ⅱ), 65.4% of females and 56.0% of males used distance spectacles,and approximately 28.9% of these spectacles were found to be inappropriate for dioptric power(with 0.5 dioptres or more). The prevalence of inaccurate distance spectacles was significantly higher in older age groups(71y and above) in both sexes.·CONCLUSION: According to this population-based data, uncorrected refractive errors are not rare in Hungary. Despite recent national initiatives, fur ther steps are required to reduce uncorrected refractive errors and associated negative effects on vision, such as avoidable visual impairment.
文摘Objective: Digital healthcare is rapidly becoming a new model for medical development in the information society with its convenience, and personalization, and a research boom in digital healthcare has formed at home and abroad in recent years. The purpose of this study is to conduct a bibliometric analysis of the field of digital healthcare and to understand the research background and development trend in this field. Methods: A visual analysis of authors, institutions, journals and keywords was conducted using CiteSpace 5.8R3 software. Results: A total of 1646 digital healthcare-related retrieved from WoS and PubMed studies. There was an overall upward trend in the number of digital healthcare publications, with the highest number of publications in 2021 (290). The author AZIZ SHEIKH is ranked first in the number of published articles (13), while King Saud University (23) is the research institution with the most articles. Keyword clustering showed that the first cluster was data security;the common high frequency keywords that appeared were systems (85), artificial intelligence (82), mobile health (70), internet (61), and technology (57). Digital healthcare, artificial intelligence, healthcare services, machine learning and deep learning are the hotspot of current research. Conclusion: This paper summarises the state of the art in digital healthcare research. Using statistical analysis and network visualisation, it highlights the background, trends and hot topics in digital healthcare research. The paper finds that there is significant potential for artificial intelligence to help bridge the digital divide and reduce health inequalities. To understand the current state, hot trends and future directions of digital healthcare research, this paper can serve as a reference. .
文摘Objective: This study visualizes and analyzes the current status, hotspots, and frontiers of cardiac rehabilitation for patients with Coronary Heart Disease (CHD) over the past decade (2012.01-2022.12), and explores the future development trend and research direction of the CR for CHD patients in China. Methods: Relevant literature was searched, screened and downloaded from the Web of Science (WOS) database, and bibliometric and visualization analyses were performed using CiteSpace VI software. Results: Through the search and screening of related literature, 2443 English articles were finally included. Among them, most of the Chinese publishers were mainly universities and had less connection with each other, while the foreign publishers were mainly universities and medical institutions in the United States and Europe, and had close connection with each other. The research content of Chinese scholars mainly focuses on the assessment of patients’ life and psychological status, as well as the assessment of cardiac function. Foreign research focuses on physical training assessment, disease perception, etc. Conclusion: Through visualizing relevant research with CiteSpace VI software in the form of a knowledge map, the research frontiers and trends in the field of cardiac rehabilitation for coronary heart disease patients in China and abroad can be discovered more intuitively. Compared with foreign research, the development of cardiac rehabilitation for coronary heart disease patients in China is relatively slow and insufficient, and institutions lack cooperation. In the future, China should accelerate the connection between regions in the field of cardiac rehabilitation and develop a cardiac rehabilitation model suitable for coronary heart disease patients in China with its own characteristics.

文摘Cataract is the leading cause of visual impairment globally.The scarcity and uneven distribution of ophthalmologists seriously hinder early visual impairment grading for cataract patients in the clin-ic.In this study,a deep learning-based automated grading system of visual impairment in cataract patients is proposed using a multi-scale efficient channel attention convolutional neural network(MECA_CNN).First,the efficient channel attention mechanism is applied in the MECA_CNN to extract multi-scale features of fundus images,which can effectively focus on lesion-related regions.Then,the asymmetric convolutional modules are embedded in the residual unit to reduce the infor-mation loss of fine-grained features in fundus images.In addition,the asymmetric loss function is applied to address the problem of a higher false-negative rate and weak generalization ability caused by the imbalanced dataset.A total of 7299 fundus images derived from two clinical centers are em-ployed to develop and evaluate the MECA_CNN for identifying mild visual impairment caused by cataract(MVICC),moderate to severe visual impairment caused by cataract(MSVICC),and nor-mal sample.The experimental results demonstrate that the MECA_CNN provides clinically meaning-ful performance for visual impairment grading in the internal test dataset:MVICC(accuracy,sensi-tivity,and specificity;91.3%,89.9%,and 92%),MSVICC(93.2%,78.5%,and 96.7%),and normal sample(98.1%,98.0%,and 98.1%).The comparable performance in the external test dataset is achieved,further verifying the effectiveness and generalizability of the MECA_CNN model.This study provides a deep learning-based practical system for the automated grading of visu-al impairment in cataract patients,facilitating the formulation of treatment strategies in a timely man-ner and improving patients’vision prognosis.

文摘Background With the development of information technology,there is a significant increase in the number of network traffic logs mixed with various types of cyberattacks.Traditional intrusion detection systems(IDSs)are limited in detecting new inconstant patterns and identifying malicious traffic traces in real time.Therefore,there is an urgent need to implement more effective intrusion detection technologies to protect computer security.Methods In this study,we designed a hybrid IDS by combining our incremental learning model(KANSOINN)and active learning to learn new log patterns and detect various network anomalies in real time.Conclusions Experimental results on the NSLKDD dataset showed that KAN-SOINN can be continuously improved and effectively detect malicious logs.Meanwhile,comparative experiments proved that using a hybrid query strategy in active learning can improve the model learning efficiency.

文摘[Objectives]Based on the related literature of benefit finding in China National Knowledge Infrastructure(CNKI),this paper analyzes the research status,hot spots and trends in this field.[Methods]To search for the related literature on benefit finding in China National Knowledge Infrastructure(CNKI),and use Citespace software to visually analyze the number of papers,authors,institutions and keywords.[Results]A total of 204 related papers were obtained,and the annual number of published papers showed a gradual increasing trend.The authors of published papers were represented by Zhang Fenglan,Liu Zhunzhun and Zhang Ting,and the Nursing College of Anhui Medical University published the most papers.The research focus was on social support,coping method and other influencing factors,and qualitative research was the main research type.The key population was breast cancer,stroke patients and their caregivers,and the development trend focused on reliability and validity test and correlation analysis.[Conclusions]The field of benefit finding in China is in the initial exploration stage,and there is little cooperation and exchange between authors and institutions.In the future,academic exchanges and multidisciplinary assistance should be strengthened to promote the rapid development of this field.

文摘The existing methods of landscape visual quality evaluation is mostly based on people's subjective feelings,and the objectivity and scientificity of evaluation results are insufficient.As an important means of experimental psychology,eye movement technology can show great advantages and potential in landscape visual quality evaluation.On the basis of combing and summarizing the relevant literature,based on the explanation of the technical principle of eye movement analysis,the application field,research content,technical methods and other aspects of eye movement analysis were reviewed.The application prospect of eye movement technology in landscape visual quality evaluation was explored to provide theoretical reference for the in-depth evaluation and research of landscape visual quality evaluation theory.

文摘Objectives In order to provide theoretical support for clinical standardization and promotion of the use of Hongfen(hydrargyrum oxydatum crudum),a mercury-containing Chinese medicine,as well as to make it easier to apply traditional Chinese topical preparations in a more contemporary manner.Methods We screened the topical research literature on Hongfen.Firstly,we analyzed the current research status of Hongfen statistically in terms of disciplinary distribution,research institutions,funding,publication volume,research types,and research focus of the literature;secondly,we analyzed the research status of Hongfen by MeSH term analysis,abstract and combined with the herbal literature,the indications,usage,dosage,and adverse effects of Hongfen were statistically analyzed,and keyword co-occurrence analysis and data visualization were performed using python+pyecharts.Results Early studies on Hongfen were mainly on the evaluation of the formulation process,while the current studies are mainly focused on clinical effectiveness and safety,with a considerable part of the studies also focused on drug toxicology,metabolism of mercury components target organ toxicity studies,while the basic studies on its mechanism of action are less.The clinical application is mainly focused on chronic ulcerated sores,refractory sinus fistula,and other surgical sore diseases,the use of which is mostly in the form of compounding and adding excipients,the use of which requires consideration of the accumulation of drug toxicity and attention to drug dosage and duration of use in order to reduce the occurrence of clinical side effects.Conclusion Hongfen is a type of mineral medicine that is extremely toxic but has a known curative effect.Its clinical use is low-risk,and its advantages greatly outweigh its drawbacks.Molecular biology technology should be actively carried out to analyze the pharmacological mechanism of Hongfen,which will play a positive role in the transmission and promotion of traditional Chinese medicine topical preparations.

文摘Objective: To analyze the relevant research literature on the prevention and treatment of pulmonary interstitial fibrosis with traditional Chinese medicine (TCM), understand the current research status, hot spots and future development trend in this field, and provide basis and feasible suggestions for further research in this field. Methods: The journal literatures related to the prevention and treatment of pulmonary interstitial fibrosis with TCM in recent 20 years in CNKI database were searched and passed through CiteSpace 5.8.R3 generates the knowledge map of relevant literature authors, document issuing institutions and keywords, and makes visual analysis. Results: A total of 1,576 documents were included, and the annual number of documents showed a fluctuating upward trend, forming a relatively stable research team represented by authors such as LYU Xiaodong, PANG Lijian and LIU Chuang;According to the atlas of document issuing institutions, Shandong University of Traditional Chinese Medicine and its affiliated hospitals ranked first in the number of documents issued, and the cooperation between institutions is dominated by the University of traditional Chinese medicine and its affiliated hospitals;Keyword cluster analysis shows that a large number of studies have been carried out in the field of etiology and pathogenesis, TCM compound, clinic and experiment. Conclusion: The research on the prevention and treatment of pulmonary interstitial fibrosis with TCM has a high degree of attention, but the cooperation network between the research authors and institutions needs to be strengthened. The research on the pathogenesis and improving the quality of life of patients is the trend of development in the future.

文摘Objective: To assess and compare the clinical efficacy and safety of cognitive enhancers(donepezil, galantamine, rivastigmine, and memantine) on cognition, behavior, function, and global status in patients with vascular cognitive impairment.Data sources: The initial literature search was performed with PubMed, EMBASE, the Cochrane Methodology Register, the Cochrane Central Register of Controlled Trials, and Cumulative Index to Nursing & Allied Health(CINAHL) from inception to January 2018 for studies regarding donepezil, galantamine, rivastigmine, and memantine for treatment of vascular cognitive impairment.Data selection: Randomized controlled trials on donepezil, galantamine, rivastigmine, and memantine as monotherapy in the treatment of vascular cognitive impairment were included. A Bayesian network meta-analysis was conducted. Outcome measures: Efficacy was assessed by changes in scores of the Alzheimer's Disease Assessment Scale, cognitive subscale, Mini-Mental State Examination, Neuropsychiatric Inventory scores and Clinician's Interview-Based Impression of Change Scale Plus Caregiver's Input, Activities of Daily Living, the Clinical Dementia Rating scale. Safety was evaluated by mortality, total adverse events(TAEs), serious adverse events(SAEs), nausea, vomiting. diarrhea, or cerebrovascular accidents(CVAs). Results: After screening 1717 citations, 12 randomized controlled trials were included. Donepezil and rivastigmine(mean difference(e) = –0.77, 95% confidence interval(CI): 0.25–1.32; MD = 1.05, 95% CI: 0.18–1.79) were significantly more effective than placebo in reducing Mini-Mental State Examination scores. Donepezil, galantamine, and memantine(MD = –1.30, 95% CI: –2.27 to –0.42; MD = –1.67, 95% CI: –3.36 to –0.06; MD = –2.27, 95% CI: –3.91 to –0.53) showed superior benefits on the Alzheimer's Disease Assessment Scale–cognitive scores compared with placebo. Memantine(MD = 2.71, 95% CI: 1.05–7.29) improved global status(Clinician's Interview-Based Impression of Change Scale Plus Caregiver's Input) more than the placebo. Safety results revealed that donepezil 10 mg(odds ratio(OR) = 3.04, 95% CI: 1.86–5.41) contributed to higer risk of adverse events than placebo. Galantamine(OR = 5.64, 95% CI: 1.31–26.71) increased the risk of nausea. Rivastigmine(OR = 16.80, 95% CI: 1.78–319.26) increased the risk of vomiting. No agents displayed a significant risk of serious adverse events, mortality, cerebrovascular accidents, or diarrhea.Conclusion: We found significant efficacy of donepezil, galantamine, and memantine on cognition. Memantine can provide significant efficacy in global status. They are all safe and well tolerated.

文摘Mild cognitive impairment(MCI)is a prodrome of Alzheimer’s disease pathology.Cognitive impairment patients often have a delayed diagnosis because there are no early symptoms or conventional diagnostic methods.Exosomes play a vital role in cell-to-cell communications and can act as promising biomarkers in diagnosing diseases.This study was designed to identify serum exosomal candidate proteins that may play roles in diagnosing MCI.Mass spectrometry coupled with tandem mass tag approach-based non-targeted proteomics was used to show the differentially expressed proteins in exosomes between MCI patients and healthy controls,and these differential proteins were validated using immunoblot and enzyme-linked immunosorbent assays.Correlation of cognitive performance with the serum exosomal protein level was determined.Nanoparticle tracking analysis suggested that there was a higher serum exosome concentration and smaller exosome diameter in individuals with MCI compared with healthy controls.We identified 69 exosomal proteins that were differentially expressed between MCI patients and healthy controls using mass spectrometry analysis.Thirty-nine exosomal proteins were upregulated in MCI patients compared with those in control patients.Exosomal fibulin-1,with an area under the curve value of 0.81,may be a biomarker for an MCI diagnosis.The exosomal protein signature from MCI patients reflected the cell adhesion molecule category.In particular,higher exosomal fibulin-1 levels correlated with lower cognitive performance.Thus,this study revealed that exosomal fibulin-1 is a promising biomarker for diagnosing MCI.
文摘Childhood visual impairment(VI) have a significant impact on the educational achievement, career choices and social life of affected individual, and in children, is mainly due to either preventable or treatable causes. Reliable data on the prevalence and causes of VI in children will guide the development of a systematic vision screening program for its early detection and successful treatment of possible causes. The purpose of this literature review is to summarize the available data on prevalence and causes of VI in school-age children from various regions globally. A discussion on the major findings highlighting the definition criteria, classifications and limitations for further studies is also presented.
