Over the last 30 years, the globalprevalence of diabetes increased from 4% to 6.4%. If no better treatment or cure isfound, this amount might climb to 430 million in the coming years. The major fact-ors of the disease’s deterioration include age, obesity, and a sedentary lifestyle.Finding new therapies to manage diabetes safely and effectively without jeopardizingpatient compliance has always been essential. Among the medicationsavailable to manage DM on this journey are glucagon-like peptide-1 agonists,thiazolidinediones, sulphonyl urease, glinides, biguanides, and insulin-targetingreceptors discovered more than 10 years ago. Despite the extensive preliminarystudies, a few clinical observations suggest this process is still in its early stages.The present review focuses on targets that contribute to insulin regulation andmay be employed as targets in treating diabetes since they may be more efficientand secure than current and traditional treatments.展开更多
BACKGROUND:Targeted temperature management(TTM),as a therapeutic temperature control strategy for cardiac arrest(CA),is recommended by guidelines.However,the relationship between postrewarming fever(PRF)and the progno...BACKGROUND:Targeted temperature management(TTM),as a therapeutic temperature control strategy for cardiac arrest(CA),is recommended by guidelines.However,the relationship between postrewarming fever(PRF)and the prognosis of CA patients is unclear.Therefore,we aim to summarize the studies regarding the infl uence of PRF on patients with CA.METHODS:EMBASE,PubMed,and Cochrane Central databases were searched from inception to March 13,2022.Randomized clinical trials(RCTs)and cohort studies on PRF in CA patients were included.According to the heterogeneity,the meta-analysis was performed using a random eff ects model or fi xed eff ects model to calculate the pooled odds ratios(ORs)and corresponding 95%confi dence intervals(CIs).The outcome data were unfavorable neurological outcome and mortality.RESULTS:The meta-analysis included 11 observational studies involving 3,246 patients.The results of the meta-analysis show that PRF(body temperature>38.0℃)has no eff ect on the neurological outcome of CA patients(OR 0.71,95%CI 0.43–1.17,I282%)and has a signifi cant relationship with lower mortality(OR 0.63;95%CI 0.49–0.80,I239%).However,PRF with a stricter defi nition(body temperature>38.5℃)was associated with worse neurological outcome(OR 1.44,95%CI 1.08–1.92,I245%)and higher mortality(OR 1.71,95%CI 1.25–2.35,I247%).CONCLUSION:This study suggests that PRF>38.0℃ may not affect the neurological outcome and have a lower mortality in CA patients who completed TTM.However,PRF>38.5℃ is a potential prognostic factor for worse outcomes in CA patients.展开更多

However, contents of target entities for assessment are various, and it is very difficult to define the whole scope of TQM. On the other hand, it is very important to define the whole target entities of quality management about the TQM because lack of important target entities may cause significant risk of loss in future. Furthermore, target entities of TQM should correspond to needs and priority of an objective requirement and that should be based on the consideration of basic principle of quality management. In the previous study, we have proposed the “framework of new TQM” of assessment for a total quality management of organizations based on the original concept of “TQM matrix”. On the other hand, we have proposed the view point of “Three-Dimensional Unification Value Models” for an evaluation of system product. Therefore, in this paper, we propose the target entities of whole assessment of quality management of organization totally based on the consideration of “New TQM framework” and the view point of “Three-Dimensional Unification Value Models”. In this study, we performed a case study regarding river pollution control plan based on water quality target management in the North Canal River catchment of Beijing section, in order to obtain effective water quality improvement programs. The ammonia nitrogen (NH3-N) and chemical oxygen demand (COD) were taken as the main controlling pollutants. Water quality targets and basic water quality improvement scenarios were set up considering different intensities of population regulation scenarios and gradually strengthening emission control measures. The MIKE11 model was adopted to simulate the effects of a range of water quality improvement scenarios. Results indicated that the basic scenarios could dramatically improve the surface water environment. However, additional intensive and combined measure programs should be implemented to ensure that the water quality would basically meet the targets of corresponding water function zones. The results highlight the need to implement water conservation in water shortage urban river basin and show the importance of enhancing drainage communication and conducting ecological water replenishment in such kind basins. It is expected to provide a reference for the water environment management practice of other metropolis in the world facing both crisis of water resource shortage and water environment pollution.展开更多

They receive treatment in outpatient setting and have a need to self-manage at home. The purpose of this study was to clarify how elderly patients with lung cancer who are undergoing treatment with molecularly targeted drugs in outpatient setting establish self-management. Methods: The study used Kinoshita’s Modified Grounded Theory. Semi-structured interviews were conducted with 17 patients (eight males and nine females). Results: This study identified six categories: Accepting life with cancer, Maintaining activities of daily living without feeling shackled by cancer, Reconsidering how to continue being themselves in daily life until life comes to an end, Using trial and error to integrate treatment and daily living, Formulating their criteria for continuing treatment, and Obtaining help from caregivers. Conclusion: The self-management process in elderly patients consisted of maintaining treatment and integrating treatment with daily living. And they are exploring the effects of treatment and side effects in order to live their own life. Implications for Nursing: In many elderly patients with lung cancer the purpose of treatment is curative extension of survival and improved quality of life. Our findings suggest that is important we help patients to identify how patients want to live, identify ways to improve their quality of life, and share the goals of treatment between the patient and the caregiver.展开更多

Objective: The purpose of the study was to explore the application effects of the cloud platform-based comprehensive online management for breast cancer patients using dual-targeted therapy with macromolecular monoclo...Objective: The purpose of the study was to explore the application effects of the cloud platform-based comprehensive online management for breast cancer patients using dual-targeted therapy with macromolecular monoclonal antibodies. Methods: 120 breast cancer patients treated by dual-targeted therapy with macromolecular monoclonal antibodies were managed by a cloud platform from March to November 2019. Comprehensive online management included consultation about drugs and side effects and frequently asked questions in the dual-targeted therapy with macromolecular monoclonal antibodies. Results: In the consultation about drugs and side effects, there were five patients with fever, neutrophil, cough, and fatigue;24 with diarrhea;25 with nausea;11 with oral mucosal inflammation;10 with rashes and dry skin;8 with insomnia;and 1 with palpitation. Moreover, 110 patients with anxiety about the missed or delayed treatment were properly handled. Conclusion: The comprehensive online management of dual-targeted therapy with macromolecular monoclonal antibodies based on the cloud platform is helpful to satisfy the at-home breast cancer patients’ needs, ensure the continuity of dual-targeted therapy with macromolecular monoclonal antibodies for breast cancer patients, prevent misinformation, alleviate patients’ negative psychological emotions, and reduce patients’ economic losses. The online cloud platform integrated management model is crucial for managing patients with breast cancer treated by dual-targeted therapy.展开更多

A covariance control approach, which selects sensor combinations or subset based on the difference between the desired covariance matrix and the actual covariance of each target, is used for sensor management, including some matrix metrics to measure the differentia between two covariance matrices. Besides, to meet the requirements of the space based warning system, the original covariance control approach is improved. Simulation results demonstrate that the covariance control approach is able to provide a better tracking performance by providing a well-designed desired covariance and balance tracking performance goals with system demands.展开更多
Background:Post–cardiac arrest syndrome involves systemic inflammation,which causes subsequent neurological impairments.We investigated the influence of targeted temperature management(TTM)therapy in patients with ou...Background:Post–cardiac arrest syndrome involves systemic inflammation,which causes subsequent neurological impairments.We investigated the influence of targeted temperature management(TTM)therapy in patients with out-of-hospital cardiac arrest(OHCA)after return of spontaneous circulation(ROSC)by observing the changes in circulating CD14^(+)monocytes and the expression of human leukocyte antigen D–related(HLA-DR)and programmed cell death ligand 1(PD-L1)in CD14^(+)monocytes.Methods:Adult patients admitted to the emergency department of Beijing Chao-Yang Hospital after OHCA between January 2017 and March 2018 were included in this study.Thirty control subjects,10 patients with OHCA,and 37 patients with OHCA who received 72 hours of TTM therapy were enrolled.Peripheral blood samples of patients in the OHCA and TTM groups were collected on Days 1 and 3(D1 and D3)after ROSC and evaluated for HLA-DR and PD-L1 expression on CD14^(+)monocytes using flow cytometry.Results:Compared with control subjects,the percentage of circulating CD14^(+)monocytes,HLA-DR+/CD14^(+)monocyte ratios,and mean fluorescence intensity were significantly decreased in patients with OHCA.After ROSC,HLA-DR expression in CD14^(+)monocytes in the TTM group was lower than that in patients with OHCA.However,there were no significant differences in the percentage of PD-L1+/CD14^(+)monocytes or the mean fluorescence intensity between patients with OHCA and healthy volunteers.Conclusion:After ROSC,circulating CD14^(+)monocytes and HLA-DR+/CD14^(+)monocyte ratios decreased significantly in patients with OHCA.Human leukocyte antigen D–related expression in CD14^(+)monocytes was lower in patients treated with TTM.展开更多
As the leading cause of disease-related deaths,cancer is a major public health threat worldwide.Surgical resection is still the first-line therapy for patients with early-stage cancers.However,postoperative relapse an...As the leading cause of disease-related deaths,cancer is a major public health threat worldwide.Surgical resection is still the first-line therapy for patients with early-stage cancers.However,postoperative relapse and metastasis remain the cause of 90%of deaths of patients with solid organ malignancies,including hepatocellular carcinoma(HCC).With the rapid development of molecular biology techniques in recent years,molecularly targeted therapies using monoclonal antibodies,small molecules,and vaccines have become a milestone in cancer therapeutic by significantly improv-ing the survival of cancer patients,and have opened a window of hope for patients with advanced cancer.Hypervascularization is a major characteristic of HCC.It has been reported that anti-angiogenic treatments,which inhibit blood vessel formation,are highly effective for treating HCC.However,the efficacy and safety of anti-angiogenesis therapies remain controversial.Sorafenib is an oral multikinase inhibitor with antiproliferative and anti-angiogenic effects and is the first molecular target drug approved for the treatment of advanced HCC.While sorafenib has shown promising therapeutic effects,substantial evidence of primary and acquired resistance to sorafenib has been reported.Numerous clinical trials have been conducted to evaluate a large number of molecularly targeted drugs for treating HCC,but most drugs exhibited less efficacy and/or higher toxicity compared to sorafenib.Therefore,understanding the mechanism(s)underlying sorafenib resistance of cancer cells is highlighted for efficiently treating HCC.This concise review aims to provide an overview of anti-angiogenesis therapy in the management of HCC and to discuss the common mechanisms of resistance to anti-angiogenesis therapies.展开更多

The distribution of the main forest species and their ecosystems in the Geographic regions of Greece is presented in a tabular form. Their multiple functions and uses, their management type/regime, their risks and threats, protection and conservation measures are also synoptically described. Furthermore, adaptive forest management and silviculture as well as management of genetic diversity in order to face climate change are also analysed. Research priority areas on biodiversity and challenges for biodiversity research and related policy in Greece have been summarized. The new Rural Development Program (2014-2020), which is currently in the final phase of implementation, will take into consideration the forest itself and forest-related European and National legal obligations. This study was carried out to evaluate the management, control, complication profile and treatment strategies in patients with diabetes and to determine what extent management of diabetes in a tertiary-care diabetic hospital in Bangladesh adhered to current guidelines. Total 140 diabetic pa- tients (type-1 = 3, type-2 = 137) were randomly selected from outpatient department of a tertiary care diabetic hospital in the Rajshahi city, Bangladesh, during the month of August to September. A standard questionnaire was constructed in local language and interview was administrated. The result was expressed as mean ± SD and the age was 53.2 ± 10.5 yr, duration of diabetes was 6.3 ± 5.6 yr and age at the onset of diabetes was 46.9 ± 9.9 yr. The study group comprised of about 43% male and 57% female with varying risk factors including family history (49%), smoking (11%) and both smoking and family history (4%). Results showed deteriorating glycemic control with mean FBG (fasting blood glucose) and PPG (postprandial blood glucose) level was 8.9 ± 3.6 mmol/L and 11.2 ± 4.7 mmol/L respectively. About 25% patients had FBG level < 6.1 mmol/L, 24% had FBG 6.1 - 7.8 mmol/L and rest 51% had FBG > 7.8 mmol/L. Of the 51% patients with hypertension, 94% were taking anti-hypertensive medicine and 21% patients with dyslipidemia, 59% were treated with lipid lowering agents. Micro-vascular and Macro-vascular complications were reported in 49% and 11% patients respectively. The rates of diabetic complications were cataract 19%, diabetic retinopathy 14%, neuropathy symptoms 35%, nephropathy 6%, MI 6%, cerebral stroke 4% and history of angina pectoris was 7%. Proportion of patients on diet control alone, oral hypoglycemic agent (OHA), insulin and combination of insulin & OHA was 10, 44, 25 and 21 percent respectively. Quality of life evaluation showed that about half of patients have poor quality of life as well as poor adherence to diet, exercise and self testing of blood glucose. In conclusion, majority of the patients were still not satisfactorily controlled. There is an urgent need for effective remedial measures to increase adherence to practice guidelines and to educate both patients and healthcare personnel on importance of achieving clinical targets for metabolic control.展开更多

Furthermore, target entities of TQM should correspond to needs and priority of an objective requirement and that should be based on the consideration of basic principle of quality management. In the previous study, we have proposed the “framework of new TQM” of assessment for a total quality management of organizations based on the original concept of “TQM matrix”. On the other hand, we have proposed the view point of “Three-Dimensional Unification Value Models” for an evaluation of system product. Therefore, in this paper, we propose the target entities of whole assessment of quality management of organization totally based on the consideration of “New TQM framework” and the view point of “Three-Dimensional Unification Value Models”. Also, this paper proposes the result of verification based on the result of comparison between proposed target entities of assessment and American Malcolm Baldrige Prize for Performance Excellence.
文摘Water quality target management in watershed is the fundamental choice of city rivers suffering both serious pollution and severe water shortage. In this study, we performed a case study regarding river pollution control plan based on water quality target management in the North Canal River catchment of Beijing section, in order to obtain effective water quality improvement programs. The ammonia nitrogen (NH3-N) and chemical oxygen demand (COD) were taken as the main controlling pollutants. Water quality targets and basic water quality improvement scenarios were set up considering different intensities of population regulation scenarios and gradually strengthening emission control measures. The MIKE11 model was adopted to simulate the effects of a range of water quality improvement scenarios. Results indicated that the basic scenarios could dramatically improve the surface water environment. However, additional intensive and combined measure programs should be implemented to ensure that the water quality would basically meet the targets of corresponding water function zones. The results highlight the need to implement water conservation in water shortage urban river basin and show the importance of enhancing drainage communication and conducting ecological water replenishment in such kind basins. It is expected to provide a reference for the water environment management practice of other metropolis in the world facing both crisis of water resource shortage and water environment pollution.
文摘Purpose: In Japan, many elderly cancer patients are receiving chemotherapy using oral molecularly targeted drugs. They receive treatment in outpatient setting and have a need to self-manage at home. The purpose of this study was to clarify how elderly patients with lung cancer who are undergoing treatment with molecularly targeted drugs in outpatient setting establish self-management. Methods: The study used Kinoshita’s Modified Grounded Theory. Semi-structured interviews were conducted with 17 patients (eight males and nine females). Results: This study identified six categories: Accepting life with cancer, Maintaining activities of daily living without feeling shackled by cancer, Reconsidering how to continue being themselves in daily life until life comes to an end, Using trial and error to integrate treatment and daily living, Formulating their criteria for continuing treatment, and Obtaining help from caregivers. Conclusion: The self-management process in elderly patients consisted of maintaining treatment and integrating treatment with daily living. And they are exploring the effects of treatment and side effects in order to live their own life. Implications for Nursing: In many elderly patients with lung cancer the purpose of treatment is curative extension of survival and improved quality of life. Our findings suggest that is important we help patients to identify how patients want to live, identify ways to improve their quality of life, and share the goals of treatment between the patient and the caregiver.
文摘Standard costing is used as a control for product costing. But with life cycle becoming shorter, costing should be done at the design and development stage of a product. This is achieved through target costing.The implementation of target costing and target pricing is done with the ultimate purpose of cost reduction, cost understanding, continuous improvement, competitiveness, early purchasing and supplier involvement, and improved design and accountability by manufacturers. The study explores the participation of the purchasing and supply chain management's role in target costing and target pricing process. Supply management plays an active role in monitoring the ongoing cost and performance of suppliers during the early stages of product development. Implementation of target costing and target pricing in various organizations are also explored. Leading Japanese manufacturers have used target costing and target pricing systems to their advantage and the paper also examines the adaptation of the Western companies to these proactive cost management techniques to improve their product development processes.

文摘Core body temperature(CBT)is increasingly attracting attention as crucial data during target temperature management(TTM).Accurate and continuous measurement of human CBT can effectively identify and monitor central high fever,and provide a basis for the effective implementation during TTM,which is therefore of great significance for human health care and disease monitoring.The reliable core measurement sites are nasopharynx,esophagus,bladder,rectum,pulmonary artery,etc.,but the measurement methods in these sites are all invasive.At present,the medical field is more inclined to noninvasive data collection methods through monitoring an appropriate site(such as forehead,mouth,or axilla)depending on clinical circumstances,so as to ensure the comfort and security of patients to the greatest extent.This review will provide reference choosing more safe and accurate temperature measurement methods for patients during TTM by reviewing the sites and accuracy of invasive and noninvasive CBT measurements.

文摘Objective: The purpose of the study was to explore the application effects of the cloud platform-based comprehensive online management for breast cancer patients using dual-targeted therapy with macromolecular monoclonal antibodies. Methods: 120 breast cancer patients treated by dual-targeted therapy with macromolecular monoclonal antibodies were managed by a cloud platform from March to November 2019. Comprehensive online management included consultation about drugs and side effects and frequently asked questions in the dual-targeted therapy with macromolecular monoclonal antibodies. Results: In the consultation about drugs and side effects, there were five patients with fever, neutrophil, cough, and fatigue;24 with diarrhea;25 with nausea;11 with oral mucosal inflammation;10 with rashes and dry skin;8 with insomnia;and 1 with palpitation. Moreover, 110 patients with anxiety about the missed or delayed treatment were properly handled. Conclusion: The comprehensive online management of dual-targeted therapy with macromolecular monoclonal antibodies based on the cloud platform is helpful to satisfy the at-home breast cancer patients’ needs, ensure the continuity of dual-targeted therapy with macromolecular monoclonal antibodies for breast cancer patients, prevent misinformation, alleviate patients’ negative psychological emotions, and reduce patients’ economic losses. The online cloud platform integrated management model is crucial for managing patients with breast cancer treated by dual-targeted therapy.

文摘This paper studies the multi-sensor management problem for low earth orbit(LEO) infrared warning constellation used to track a midcourse missile. A covariance control approach, which selects sensor combinations or subset based on the difference between the desired covariance matrix and the actual covariance of each target, is used for sensor management, including some matrix metrics to measure the differentia between two covariance matrices. Besides, to meet the requirements of the space based warning system, the original covariance control approach is improved. Simulation results demonstrate that the covariance control approach is able to provide a better tracking performance by providing a well-designed desired covariance and balance tracking performance goals with system demands.

文摘Background:Post–cardiac arrest syndrome involves systemic inflammation,which causes subsequent neurological impairments.We investigated the influence of targeted temperature management(TTM)therapy in patients with out-of-hospital cardiac arrest(OHCA)after return of spontaneous circulation(ROSC)by observing the changes in circulating CD14^(+)monocytes and the expression of human leukocyte antigen D–related(HLA-DR)and programmed cell death ligand 1(PD-L1)in CD14^(+)monocytes.Methods:Adult patients admitted to the emergency department of Beijing Chao-Yang Hospital after OHCA between January 2017 and March 2018 were included in this study.Thirty control subjects,10 patients with OHCA,and 37 patients with OHCA who received 72 hours of TTM therapy were enrolled.Peripheral blood samples of patients in the OHCA and TTM groups were collected on Days 1 and 3(D1 and D3)after ROSC and evaluated for HLA-DR and PD-L1 expression on CD14^(+)monocytes using flow cytometry.Results:Compared with control subjects,the percentage of circulating CD14^(+)monocytes,HLA-DR+/CD14^(+)monocyte ratios,and mean fluorescence intensity were significantly decreased in patients with OHCA.After ROSC,HLA-DR expression in CD14^(+)monocytes in the TTM group was lower than that in patients with OHCA.However,there were no significant differences in the percentage of PD-L1+/CD14^(+)monocytes or the mean fluorescence intensity between patients with OHCA and healthy volunteers.Conclusion:After ROSC,circulating CD14^(+)monocytes and HLA-DR+/CD14^(+)monocyte ratios decreased significantly in patients with OHCA.Human leukocyte antigen D–related expression in CD14^(+)monocytes was lower in patients treated with TTM.

文摘As the leading cause of disease-related deaths,cancer is a major public health threat worldwide.Surgical resection is still the first-line therapy for patients with early-stage cancers.However,postoperative relapse and metastasis remain the cause of 90%of deaths of patients with solid organ malignancies,including hepatocellular carcinoma(HCC).With the rapid development of molecular biology techniques in recent years,molecularly targeted therapies using monoclonal antibodies,small molecules,and vaccines have become a milestone in cancer therapeutic by significantly improv-ing the survival of cancer patients,and have opened a window of hope for patients with advanced cancer.Hypervascularization is a major characteristic of HCC.It has been reported that anti-angiogenic treatments,which inhibit blood vessel formation,are highly effective for treating HCC.However,the efficacy and safety of anti-angiogenesis therapies remain controversial.Sorafenib is an oral multikinase inhibitor with antiproliferative and anti-angiogenic effects and is the first molecular target drug approved for the treatment of advanced HCC.While sorafenib has shown promising therapeutic effects,substantial evidence of primary and acquired resistance to sorafenib has been reported.Numerous clinical trials have been conducted to evaluate a large number of molecularly targeted drugs for treating HCC,but most drugs exhibited less efficacy and/or higher toxicity compared to sorafenib.Therefore,understanding the mechanism(s)underlying sorafenib resistance of cancer cells is highlighted for efficiently treating HCC.This concise review aims to provide an overview of anti-angiogenesis therapy in the management of HCC and to discuss the common mechanisms of resistance to anti-angiogenesis therapies.
文摘This is a review paper based on national and international literature, national inventories and empirical approach. A general overview of the forests, forest management and forest biodiversity of Greece are briefly summarized. The distribution of the main forest species and their ecosystems in the Geographic regions of Greece is presented in a tabular form. Their multiple functions and uses, their management type/regime, their risks and threats, protection and conservation measures are also synoptically described. Furthermore, adaptive forest management and silviculture as well as management of genetic diversity in order to face climate change are also analysed. Research priority areas on biodiversity and challenges for biodiversity research and related policy in Greece have been summarized. The new Rural Development Program (2014-2020), which is currently in the final phase of implementation, will take into consideration the forest itself and forest-related European and National legal obligations. Finally, major conclusions and research priority areas on future management and conservation of forests are highlighted.
文摘The performance model proposed by this study represents an innovative approach to deal with performance assessment in ATM (air traffic management). It is based on Bayesian networks methodology, which presents several advantages but also some drawbacks as highlighted along the paper. We illustrate the main steps required for building the model and present a number of interesting results. The contribution of the paper is two-fold: (1) It presents a new methodological approach to deal with a problem which is of strategic importance for ANSPs (air navigation service providers); (2) It provides insights on the interdependencies between factors influencing performance. Both results are considered particularly important nowadays, due to the SES (Single European Sky) performance scheme and its related target setting process.

文摘Diabetes Mellitus is a leading cause of death in present world. This study was carried out to evaluate the management, control, complication profile and treatment strategies in patients with diabetes and to determine what extent management of diabetes in a tertiary-care diabetic hospital in Bangladesh adhered to current guidelines. Total 140 diabetic pa- tients (type-1 = 3, type-2 = 137) were randomly selected from outpatient department of a tertiary care diabetic hospital in the Rajshahi city, Bangladesh, during the month of August to September. A standard questionnaire was constructed in local language and interview was administrated. The result was expressed as mean ± SD and the age was 53.2 ± 10.5 yr, duration of diabetes was 6.3 ± 5.6 yr and age at the onset of diabetes was 46.9 ± 9.9 yr. The study group comprised of about 43% male and 57% female with varying risk factors including family history (49%), smoking (11%) and both smoking and family history (4%). Results showed deteriorating glycemic control with mean FBG (fasting blood glucose) and PPG (postprandial blood glucose) level was 8.9 ± 3.6 mmol/L and 11.2 ± 4.7 mmol/L respectively. About 25% patients had FBG level < 6.1 mmol/L, 24% had FBG 6.1 - 7.8 mmol/L and rest 51% had FBG > 7.8 mmol/L. Of the 51% patients with hypertension, 94% were taking anti-hypertensive medicine and 21% patients with dyslipidemia, 59% were treated with lipid lowering agents. Micro-vascular and Macro-vascular complications were reported in 49% and 11% patients respectively. The rates of diabetic complications were cataract 19%, diabetic retinopathy 14%, neuropathy symptoms 35%, nephropathy 6%, MI 6%, cerebral stroke 4% and history of angina pectoris was 7%. Proportion of patients on diet control alone, oral hypoglycemic agent (OHA), insulin and combination of insulin & OHA was 10, 44, 25 and 21 percent respectively. Quality of life evaluation showed that about half of patients have poor quality of life as well as poor adherence to diet, exercise and self testing of blood glucose. In conclusion, majority of the patients were still not satisfactorily controlled. There is an urgent need for effective remedial measures to increase adherence to practice guidelines and to educate both patients and healthcare personnel on importance of achieving clinical targets for metabolic control.
