The implementation of an electronic care records service (CRS) at our hospital in 2008 has led to widespread press coverage of ensuing chaos in clinical administration. We wanted to know how this new electronic system impacted on our working patterns in outpatient clinics and – more specifically – on the time actually spent with the patients. Material & methods: This study was performed 4 and 12 months after implementation of CRS to assess its impact on the time distribution in clinic. Senior doctors were monitored with a stop clock during consultations. Timings for pre- and post-consultation administration, and the actual consultation with the patient were recorded. A total of 170 consultations were evaluated in this way. Results: The key findings were that the total time needed to spend on a urological outpatient of 16 minutes remains unchanged from the pre-CRS era, but a majority (57%) of this time is spent in administration on the computer without the patient involved. Conclusion: No more than 15 patients should be seen in a 4 hour outpatient clinic per doctor. This recommendation drawn up by BAUS before CRS remains still valid. Patient administration related to the consultation that has previously been done by administrative aides is now to be done by the doctors on the computer in the same consultation session. Intended to streamline patient pathways, this does reduce the quality interaction-time between doctor and patients significantly.展开更多

The results showed that the males used 191 different resting sites, while females u...The use of resting sites of sables (Martes zibellina) was studied by radio-tracking techniques in Daxing’an Mountains in 1994–1996. The results showed that the males used 191 different resting sites, while females used 159 sites averagely in a year. The number of used resting sites varied among seasons, and the reuse index calculated for each individual in each season varied from 0.07 to 0.94. The reuse index was highest for males in autumn-winter. In spring the number of resting sites of females was significantly less than that of males. In summer, both sexes used more resting sites. In winter, the reuse index was negative related to snow depth. The average distance between consecutively used resting sites differed significantly between males (716 m, SD=479,n=1 081) and females (455 m, SD=298,t=8.59,P<0.001). For males the average distance was the shortest in February–March and the longest in August–September, whereas the shortest distance was recorded in April–May for females. In spring and autumn-winter, most individuals used resting sites that randomly distribute in their home ranges. The standardized Morisita coefficient of resting site dispersion in the home range varied from 0.06 to 0.50. In summer, the standardized Morisita coefficient of resting site dispersion in the home range varied from 0.38 to 0.51. Functional connectivity reflects the synchrony of function...The main symptom of patients with Alzheimer’s disease is cognitive dysfunction. Alzheimer’s disease is mainly diagnosed based on changes in brain structure. Functional connectivity reflects the synchrony of functional activities between non-adjacent brain regions, and changes in functional connectivity appear earlier than those in brain structure. In this study, we detected resting-state functional connectivity changes in patients with Alzheimer’s disease to provide reference evidence for disease prediction. Functional magnetic resonance imaging data from patients with Alzheimer’s disease were used to show whether particular white and gray matter areas had certain functional connectivity patterns and if these patterns changed with disease severity. In nine white and corresponding gray matter regions, correlations of normal cognition, early mild cognitive impairment, and late mild cognitive impairment with blood oxygen level-dependent signal time series were detected. Average correlation coefficient analysis indicated functional connectivity patterns between white and gray matter in the resting state of patients with Alzheimer’s disease. Functional connectivity pattern variation correlated with disease severity, with some regions having relatively strong or weak correlations. We found that the correlation coefficients of five regions were 0.3–0.5 in patients with normal cognition and 0–0.2 in those developing Alzheimer’s disease. Moreover, in the other four regions, the range increased to 0.45–0.7 with increasing cognitive impairment. In some white and gray matter areas, there were specific connectivity patterns. Changes in regional white and gray matter connectivity patterns may be used to predict Alzheimer’s disease;however, detailed information on specific connectivity patterns is needed. In general, most of the previous power ...Nowadays, power quality issues are becoming a significant research topic because of the increasing inclusion of very sensitive devices and considerable renewable energy sources. In general, most of the previous power quality classification techniques focused on single power quality events and did not include an optimal feature selection process. This paper presents a classification system that employs Wavelet Transform and the RMS profile to extract the main features of the measured waveforms containing either single or complex disturbances. A data mining process is designed to select the optimal set of features that better describes each disturbance present in the waveform. Support Vector Machine binary classifiers organized in a “One Vs Rest” architecture are individually optimized to classify single and complex disturbances. The parameters that rule the performance of each binary classifier are also individually adjusted using a grid search algorithm that helps them achieve optimal performance. This specialized process significantly improves the total classification accuracy. Several single and complex disturbances were simulated in order to train and test the algorithm. The results show that the classifier is capable of identifying >99% of single disturbances and >97% of complex disturbances.展开更多
The identification of contamination sources within the food chain with polychlorinated dibenzo-p-dioxins (PCDDs) and polychlorinated dibenzo-furans (PCDFs), (“dioxins”), and PCBs is difficult and complex. PCDD/PCDF ...The identification of contamination sources within the food chain with polychlorinated dibenzo-p-dioxins (PCDDs) and polychlorinated dibenzo-furans (PCDFs), (“dioxins”), and PCBs is difficult and complex. PCDD/PCDF can be formed as unintentional compounds in a number of chemical processes as well as in almost every combustion process. PCBs were intentionally produced chemicals that were manufactured for decades before the ban in marketing and use in many countries around 1985. The pattern of occurrence can change from the original source in particular via feedingstuffs to food of animal origin as result of bioaccumulation. A number of examples illustrate the challenging detective work and key scientific aspects for identification of the sources, for support of the risk management and for performance of monitoring programs. The contamination of milk and milk products in European countries with dioxins was caused by compound feeds containing citrus pulp pellets from Brazil which had high dioxin levels as a result of the use of heavily contaminated lime used for neutralization. The Belgian dioxin crisis was caused by a feed additive heavily contaminated with PCBs which were discharged into a recycled fat used in the production of animal feed. Guar gum from India was contaminated with sodium pentachlorophenate and dioxins. Clay was found to be possibly highly contaminated with dioxins possibly formed by geothermal processes over time;use of such clay as feed additive or for human intake led to elevated dioxin levels in food and humans. Bioanalytical screening in combination with comprehensive physicochemical investigations led to the detection of brominated flame retardants and brominated dioxins in a feed additive. Buffalo milk was contaminated in Italy presumably caused by illegal deposition of waste. High PCB levels in fuel oil for drying of breadcrumb used as a feed ingredient caused a major dioxin crisis with pork meat in Ireland. Fatty acids for technical purposes originating from a biodiesel company were used for production of feed fat which contaminated parts of the food chain in Germany. In addition to effects on human and animal health these incidents also have serious economic consequences which could be mitigated by more frequent control on food and feed. Addressing both these issues, the European Community has developed a strategy to reduce the presence of dioxins and PCBs in the environment and in feed and food comprising the establishment of maximum and action levels.展开更多
