Tuberculosis(TB)remains a leading cause of death among infectious diseases,particularly in poor countries.Viral infections,multidrug-resistant and ex-tensively drug-resistant TB strains,as well as the coexistence of c...Tuberculosis(TB)remains a leading cause of death among infectious diseases,particularly in poor countries.Viral infections,multidrug-resistant and ex-tensively drug-resistant TB strains,as well as the coexistence of chronic illnesses such as diabetes mellitus(DM)greatly aggravate TB morbidity and mortality.DM[particularly type 2 DM(T2DM)]and TB have converged making their control even more challenging.Two contemporary global epidemics,TB-DM behaves like a syndemic,a synergistic confluence of two highly prevalent diseases.T2DM is a risk factor for developing more severe forms of multi-drug resistant-TB and TB recurrence after preventive treatment.Since a bidirectional relationship exists between TB and DM,it is necessary to concurrently treat both,and promote recommendations for the joint management of both diseases.There are also some drug-drug interactions resulting in adverse treatment outcomes in TB-DM patients including treatment failure,and reinfection.In addition,autophagy may play a role in these comorbidities.Therefore,the TB-DM comorbidities present several health challenges,requiring a focus on multidisciplinary collaboration and integrated strategies,to effectively deal with this double burden.To effectively manage the comorbidity,further screening in affected countries,more suitable drugs,and better treatment strategies are required.展开更多
Background:Physical activity(PA)is important for cancer survivors.Trials of remotely delivered interventions are needed to assist in reaching under-served non-metropolitan cancer survivors.The objective of this study ...Background:Physical activity(PA)is important for cancer survivors.Trials of remotely delivered interventions are needed to assist in reaching under-served non-metropolitan cancer survivors.The objective of this study was to ascertain whether wearable technology,coupled with health coaching was effective in increasing PA in breast and colorectal cancer survivors living in regional and remote areas in Australia.Methods:Cancer survivors from 5 states were randomized to intervention and control arms.Intervention participants were given a Fitbit Charge 2TMand received up to 6 telephone health coaching sessions.Control participants received PA print materials.Accelerometer assessments at baseline and 12 weeks measured moderate-to-vigorous PA(MVPA),light PA,and sedentary behavior.Results:Eighty-seven participants were recruited(age=63±11 years;74(85%)female).There was a significant net improvement in MVPA of 49.8 min/week,favoring the intervention group(95%confidence interval(95%CI):13.6-86.1,p=0.007).There was also a net increase in MVPA bouts of 39.5 min/week(95%CI:11.9-67.1,p=0.005),favoring the intervention group.Both groups improved light PA and sedentary behavior,but there were no between-group differences.Conclusion:This’s the first study to demonstrate that,when compared to standard practice(i.e.,PA education),a wearable technology intervention coupled with distance-based health coaching,improves MVPA in non-metropolitan cancer survivors.The results display promise for the use of scalable interventions using smart wearable technology in conjunction with phone-based health coaching to foster increased PA in geographically disadvantaged cancer survivors.展开更多

Current grafts, when long, can create initial donor site morbidity. In this article, we evaluate the progress made in finding a tissue engineered substitute for the human urethra. Researchers have investigated cell-free and cell-seeded grafts. We discuss different approaches to developing these grafts and review their reported successes in human studies. With further work, tissue engineered grafts may facilitate the management of lengthy urethral strictures requiring oral mucosa substitution urethroplasty.展开更多
Although ammonium dinitramide(ADN)has been targeted as a potential green monopropellant in future space vehicles,its application potential in Micro-electrical–Mechanical System(MEMS)thrusters or microthrusters has be...Although ammonium dinitramide(ADN)has been targeted as a potential green monopropellant in future space vehicles,its application potential in Micro-electrical–Mechanical System(MEMS)thrusters or microthrusters has been seldom reported in open literature.In this paper,electrolytic decomposition of Ammonium Dinitramide(ADN)-based liquid monopropellant FLP-103 was carried out in an open chamber and MEMS thrusters were fabricated from poly-dimethylsiloxane(PDMS)to characterize the power consumption.Two thrust measurement methods were employed to investigate the electrolytic decomposition of FLP-103 in MEMS microthrusters.The results show that the monopropellant can be successfully ignited at room temperature through 80 V,0.1 A(8 W)using copper wire as electrodes.In the current thruster design,low thrust was obtained at FLP-103 flowrate of 40μl·min^(-1)but it generated the highest specific impulse,Isp,among all the flowrates tested.The experiments successfully demonstrated the potential application of electrolytic decomposition of FLP-103 in MEMS thrusters.展开更多

For high altitude ...Fuel cell is a device that converts the chemical energy in the reactants into the electrical energy after steps of sequential electrochemical processes with no significant impact on the environment. For high altitude long endurance (HALE) of unmanned aircraft system (UAS) where fuel cell operates as a prime source of power, the operation and performance of a PEM fuel cell at different level of altitudes is vitally important. In this paper, the impact of direct using extracted air from high altitudes atmosphere in order to feed the stack is investigated, and the governing equations of the supplied air and oxygen to the PEM fuel cell stack are developed. The impact of high altitudes upon the operation and the consumption of air are determined in order to maintain certain level of delivered power to the load. Also the implications associated with operating the PEM fuel cell stack at high altitudes and different technical solutions are proposed. Various modes of Integral, Proportional-Integral, and Proportional-Integral-Derivative controller are introduced and examined for different time setting responses in order to determine the most adequate trade-off choice between fast response and reactants consumption which provides the necessary optimization of the air consumption for the developed model of PEM fuel cell used for UAS operation.展开更多

The two schemes are analyzed and compared under the chips of L6561 and UC 3854 which are commonly used for CRM and CCM respectively. Both schemes are based on multiplier;however, the CCM is more complex and needs more periphery components which increase the cost. The Boost PFC under CRM is easier to be implemented. Nevertheless, the variable switch frequency makes the system (including the power-stage inductor and capacitor) hard to design. It seems that the CRM PFC is more attractive in low power applications which only need to meet IEC61000-3-2 D standard. Some experiment results are also presented for the comparison.展开更多

The model incorporates major electric and thermodynamic variab...This paper presents an experimental testing and validation results for a zero-dimensional self-humidifying PEM (Proton Exchange Membrane) fuel cell stack. The model incorporates major electric and thermodynamic variables and parameters involved in the operation of the PEM fuel cell under different operational conditions. The mathematical equations are modelled by using Matlab-Simulink tools in order to simulate the operation of the developed model with a commercially available 1 kW Horizon (H-1000) PEM fuel cell stack, which is used for the purposes of model validation and tuning of the developed model. The model is mathematically modelled and presented in the recent published work of authors. The observations from model simulations provide sufficient evidence and support to the results and observations obtained from testing 1 kW Horizon (H-1000) PEM fuel cell stack used in this research. The developed model can be used as a generic model and simulation platform for a self-humidifying PEM fuel cell with an output power varying from 50 W to 1 kW, with extrapolation to higher powers is also possible.展开更多

Current treatments include drugs that target serotonin and noradrenaline levels within the central nervous system, e.g., tricyclic antidepressants, serotonin noradrenaline reuptake inhibitors, and voltage-gated calcium channel subunit ligands, e.g., gabapentin and pregabalin. Investigation of a range of novel targets, such as melatoninergic, cannabinoid, dopamine, NMDA, angiotensin, orexin and opioid receptors, and ion channels, in addition revisiting bioamine modulation and subunits has provided efficacy outcomes that improve the health status of patients with FM. Nevertheless, modest and limited efficacy is often observed reflecting the heterogeneity of FM with existence of subpopulations of patients, the contribution of peripheral and central components to the pathophysiology, and the extensive range of accompanying co-morbidities. The complexity and multidimensional nature of FM is emphasized by the diversity of pharmacological targets gaining interest. Clues to underlying mechanisms which offer themselves as novel and potential targets for new medications are being provided by advances in the understanding of the pathophysiology of FM.展开更多

The hybrid composite was reinforced by hybrid fibers from carbon and Kevlar fibers on woven roving form (45o -0o), by using a surface layer of 4mm thick of Zinc Borate flame retardant. Afterward, the structure was exposed directly to gas flame of 2000oC due to 10 mm and 20mm exposure interval. The retardant layer thermal resistance and protection capability were determined. The study was continued to improve the performance of Zinc Borate layer mixed by 10%, 20% and 30% of Antimony Trioxide. To determine the heat transfer of the composite material the opposite surface temperature method was used. Zinc Borate with (30%) Antimony Trioxide gives the optimized result of the experiment.展开更多

Background:One-hour postprandial hyperglycemia is associated with increased risk of type 2 diabetes and cardiovascular disease.Physical activity(PA)has short-term beneficial effects on post-meal glucose response.This ...Background:One-hour postprandial hyperglycemia is associated with increased risk of type 2 diabetes and cardiovascular disease.Physical activity(PA)has short-term beneficial effects on post-meal glucose response.This study compared the oral glucose tolerance test results of 3 groups of people with habitually different levels of PA.Methods:Thirty-one adults without diabetes(age 25.9±6.6 years;body mass index 23.8±3.8 kg/m^2;mean±SD)were recruited and divided into 3 groups based on self-reported PA volume and intensity:low activity<30 min/day of moderate-intensity activity(n=11),moderately active≥30 min/day of moderate-intensity PA(n=10),and very active≥60 min/day of PA at high intensity(n=10).Participants completed an oral glucose tolerance test(50 g glucose)with capillary blood samples obtained at baseline,15 min,30 min,45 min,60 min,90 min,and 120 min post-ingestion.Results:There were no significant differences between groups for age or body fat percentage or glycated hemoglobin(p>0.05).The groups were significantly different in terms of baseline glucose level(p=0.003)and,marginally,for gender(p=0.053)and BMI(p=0.050).There was a statistically significant effect of PA on the 1-h postprandial glucose results(p=0.029),with differences between very active and low activity groups(p=0.008)but not between the moderately active and low activity groups(p=0.360),even when baseline glucose level and gender differences were accounted for.For incremental area under the curve there was no significant effect of activity group once gender and body fat percentage had been accounted for(p=0.401).Those in the low activity group took 15 min longer to reach peak glucose level than those in the very active group(p=0.012).Conclusion:The results suggest that high levels of PA have a beneficial effect on postprandial blood glucose profiles when compared to low and moderate levels of activity.展开更多
Objective:Research on methods of improving the affective experience of exercise remains limited,especially for low-active overweight adults.We investigated the effectiveness of a virtual-reality headset and headphones...Objective:Research on methods of improving the affective experience of exercise remains limited,especially for low-active overweight adults.We investigated the effectiveness of a virtual-reality headset and headphones in improving affective responses over conventionally delivered audiovisual stimulation.Methods:Low-active,overweight adults(16 women,5 men;age:34.67±9.62 years;body mass index:28.56±4.95 kg/m^2;peak oxygen uptake for men:29.14±6.56 mL/kg/min,for women:22.67±4.52 mL/kg/min,mean±SD)completed 15-min sessions of recumbent cycling at the ventilatory threshold:(a)high immersion(HI,virtual reality headset and headphones),(b)low immersion(LI,television screen and speakers),and(c)Control.During-exercise pleasure and post-exercise enj oyment were self-reported.Oxygenation of the right dorsolateral pre frontal cortex(dIPFC)was assessed with near infrared spectroscopy.Results:Higher pleasure was reported during HI than during LI and Control(Condition×Time interaction;p<0.001,ηp^2=0.43).Participants who reported a preference for low exercise intensity showed higher dIPFC oxygenation during Control,but this difference diminished during LI and HI(Condition×Time×Preference interaction;p=0.036,ηp^2=0.10).Conclusion:Compared with conventionally delivered audiovisual stimulation,using a virtual-reality headset strengthens the dissociative effect,further improving affective responses to exercise at the ventilatory threshold among overweight,low-active adults.Presumably by competing with interoceptive afferents at the level of sensory input,audiovisual stimulation may lessen reliance on cognitive efforts to attenuate declining affect,as indicated by lower right dIPFC activity,particularly among participants disinclined toward high exercise intensity.展开更多

For high altitude long endurance (HALE) of unmanned aircraft system (UAS) where fuel cell operates as a prime source of power, the operation and performance of a PEM fuel cell at different level of altitudes is vitally important. In this paper, the impact of direct using extracted air from high altitudes atmosphere in order to feed the stack is investigated, and the governing equations of the supplied air and oxygen to the PEM fuel cell stack are developed. The impact of high altitudes upon the operation and the consumption of air are determined in order to maintain certain level of delivered power to the load. Also the implications associated with operating the PEM fuel cell stack at high altitudes and different technical solutions are proposed. Various modes of Integral, Proportional-Integral, and Proportional-Integral-Derivative controller are introduced and examined for different time setting responses in order to determine the most adequate trade-off choice between fast response and reactants consumption which provides the necessary optimization of the air consumption for the developed model of PEM fuel cell used for UAS operation.
文摘Accounting scandals are becoming perpetual in nature.They range from the ancient Mesopotamia,to the South Sea Bubble of 1720,to the famous Enron of 2001,down to Parmalat,Tesco,and Toshiba of today.The series of accounting scandals that have occurred in the last two decades calls for a greater concern by the accounting profession.The accounting scandals that have occurred in this 21st century alone have shown that there is a need to look beyond corporate governance in the fight against financial deception.In this paper,we argue that even in the face of the Sarbanes-Oxley Act(SOA)of 2002 and other regulations around the world that are targeted towards effective corporate governance,accounting scandals have never ceased to occur.Most of the legislations that have been passed in recent times were targeted at corporate governance,forgetting the crucial role that audit plays within the agency relationship.And whenever there is any revelation of fraudulent financial reporting,investors do not ask who are the directors,but the first question they ask is who are the auditors?Hence,there is a need to improve audit quality by approaching it from a forensic accounting perspective in order to reduce the incidence of financial statement frauds in this era of information revolution,thus restoring investors’confidence back in the financial reporting process and corporate governance.In this paper,we propose a forensic accounting paradigm as a viable option for reducing accounting scandals,since this will compliment corporate governance systems.
文摘Palmer&Harvey(P&H)is a recent example of a UK corporate failure which raises questions about current corporate governance practice,the quality and integrity of audit reporting,and the“sugar coating”of Annual Reports.P&H is but one example of UK firms currently struggling to survive,or failing.The paper presents some details about the P&H case,and then considers questions about corporate governance practice,and whether it is designed to truly safeguard the interests of stakeholders;it raises questions about conventional audit reporting,and whether it is too limited in its analysis and reporting.The paper recommends a strengthening of corporate governance guidelines and practice within the terms of the current Financial Reporting Council(FRC)review,and a wider adoption of forensic accounting practice and reporting,in part taking account of the impact of behavioural factors in management practice.A wider study is proposed to take this analysis and discussion further.
文摘The paper presents detailed comparisons between CRM (critical conduction mode) and CCM (continuous conduction mode) control schemes used for Boost PFC converter. The two schemes are analyzed and compared under the chips of L6561 and UC 3854 which are commonly used for CRM and CCM respectively. Both schemes are based on multiplier;however, the CCM is more complex and needs more periphery components which increase the cost. The Boost PFC under CRM is easier to be implemented. Nevertheless, the variable switch frequency makes the system (including the power-stage inductor and capacitor) hard to design. It seems that the CRM PFC is more attractive in low power applications which only need to meet IEC61000-3-2 D standard. Some experiment results are also presented for the comparison.
文摘This paper presents an experimental testing and validation results for a zero-dimensional self-humidifying PEM (Proton Exchange Membrane) fuel cell stack. The model incorporates major electric and thermodynamic variables and parameters involved in the operation of the PEM fuel cell under different operational conditions. The mathematical equations are modelled by using Matlab-Simulink tools in order to simulate the operation of the developed model with a commercially available 1 kW Horizon (H-1000) PEM fuel cell stack, which is used for the purposes of model validation and tuning of the developed model. The model is mathematically modelled and presented in the recent published work of authors. The observations from model simulations provide sufficient evidence and support to the results and observations obtained from testing 1 kW Horizon (H-1000) PEM fuel cell stack used in this research. The developed model can be used as a generic model and simulation platform for a self-humidifying PEM fuel cell with an output power varying from 50 W to 1 kW, with extrapolation to higher powers is also possible.
文摘Fibromyalgia(FM) has been described as a chronic clinical condition related to multisensory hypersensitivitypresenting with a complex of symptoms dominated by chronic widespread pain associated with the existence of a range of co-morbidities, such as fatigue, sleep disturbance, cognitive impairment, anxiety and depression. Current treatments include drugs that target serotonin and noradrenaline levels within the central nervous system, e.g., tricyclic antidepressants, serotonin noradrenaline reuptake inhibitors, and voltage-gated calcium channel subunit ligands, e.g., gabapentin and pregabalin. Investigation of a range of novel targets, such as melatoninergic, cannabinoid, dopamine, NMDA, angiotensin, orexin and opioid receptors, and ion channels, in addition revisiting bioamine modulation and subunits has provided efficacy outcomes that improve the health status of patients with FM. Nevertheless, modest and limited efficacy is often observed reflecting the heterogeneity of FM with existence of subpopulations of patients, the contribution of peripheral and central components to the pathophysiology, and the extensive range of accompanying co-morbidities. The complexity and multidimensional nature of FM is emphasized by the diversity of pharmacological targets gaining interest. Clues to underlying mechanisms which offer themselves as novel and potential targets for new medications are being provided by advances in the understanding of the pathophysiology of FM.
文摘This study aims to investigate the possibility of improving the flame Retardancy for the hybrid composite material consisting araldite resin (CY223). The hybrid composite was reinforced by hybrid fibers from carbon and Kevlar fibers on woven roving form (45o -0o), by using a surface layer of 4mm thick of Zinc Borate flame retardant. Afterward, the structure was exposed directly to gas flame of 2000oC due to 10 mm and 20mm exposure interval. The retardant layer thermal resistance and protection capability were determined. The study was continued to improve the performance of Zinc Borate layer mixed by 10%, 20% and 30% of Antimony Trioxide. To determine the heat transfer of the composite material the opposite surface temperature method was used. Zinc Borate with (30%) Antimony Trioxide gives the optimized result of the experiment.

文摘Genotypes and allele distributions for the interleukin (IL)- 1β - 511 C/T and the IL- 1 receptor antagonist gene intron 2 tandem repeat polymorphisms were compared in 206 women with recurrent miscarriage and a control population. No significant differences were observed between the distributions of IL- 1β or IL- 1 receptor antagonist gene alleles in either the recurrent miscarriage group as a whole or when divided according to the cause of recurrent miscarriage compared with controls, which suggests that variation in the IL- 1 receptor antagonist gene and IL- 1β genes individually does not play a role in susceptibility to recurrent miscarriage.
文摘Background:One-hour postprandial hyperglycemia is associated with increased risk of type 2 diabetes and cardiovascular disease.Physical activity(PA)has short-term beneficial effects on post-meal glucose response.This study compared the oral glucose tolerance test results of 3 groups of people with habitually different levels of PA.Methods:Thirty-one adults without diabetes(age 25.9±6.6 years;body mass index 23.8±3.8 kg/m^2;mean±SD)were recruited and divided into 3 groups based on self-reported PA volume and intensity:low activity<30 min/day of moderate-intensity activity(n=11),moderately active≥30 min/day of moderate-intensity PA(n=10),and very active≥60 min/day of PA at high intensity(n=10).Participants completed an oral glucose tolerance test(50 g glucose)with capillary blood samples obtained at baseline,15 min,30 min,45 min,60 min,90 min,and 120 min post-ingestion.Results:There were no significant differences between groups for age or body fat percentage or glycated hemoglobin(p>0.05).The groups were significantly different in terms of baseline glucose level(p=0.003)and,marginally,for gender(p=0.053)and BMI(p=0.050).There was a statistically significant effect of PA on the 1-h postprandial glucose results(p=0.029),with differences between very active and low activity groups(p=0.008)but not between the moderately active and low activity groups(p=0.360),even when baseline glucose level and gender differences were accounted for.For incremental area under the curve there was no significant effect of activity group once gender and body fat percentage had been accounted for(p=0.401).Those in the low activity group took 15 min longer to reach peak glucose level than those in the very active group(p=0.012).Conclusion:The results suggest that high levels of PA have a beneficial effect on postprandial blood glucose profiles when compared to low and moderate levels of activity.

文摘Objective:Research on methods of improving the affective experience of exercise remains limited,especially for low-active overweight adults.We investigated the effectiveness of a virtual-reality headset and headphones in improving affective responses over conventionally delivered audiovisual stimulation.Methods:Low-active,overweight adults(16 women,5 men;age:34.67±9.62 years;body mass index:28.56±4.95 kg/m^2;peak oxygen uptake for men:29.14±6.56 mL/kg/min,for women:22.67±4.52 mL/kg/min,mean±SD)completed 15-min sessions of recumbent cycling at the ventilatory threshold:(a)high immersion(HI,virtual reality headset and headphones),(b)low immersion(LI,television screen and speakers),and(c)Control.During-exercise pleasure and post-exercise enj oyment were self-reported.Oxygenation of the right dorsolateral pre frontal cortex(dIPFC)was assessed with near infrared spectroscopy.Results:Higher pleasure was reported during HI than during LI and Control(Condition×Time interaction;p<0.001,ηp^2=0.43).Participants who reported a preference for low exercise intensity showed higher dIPFC oxygenation during Control,but this difference diminished during LI and HI(Condition×Time×Preference interaction;p=0.036,ηp^2=0.10).Conclusion:Compared with conventionally delivered audiovisual stimulation,using a virtual-reality headset strengthens the dissociative effect,further improving affective responses to exercise at the ventilatory threshold among overweight,low-active adults.Presumably by competing with interoceptive afferents at the level of sensory input,audiovisual stimulation may lessen reliance on cognitive efforts to attenuate declining affect,as indicated by lower right dIPFC activity,particularly among participants disinclined toward high exercise intensity.
