非增殖期糖尿病视网膜病变两种分期方法一致性的研究

目的观察非增殖期糖尿病视网膜病变两种分期方法的一致性。方法回顾性研究2014年1月~2016年6月于我院行FFA检查的非增殖期糖尿病视网膜病变患者共84例162眼。对每例患者每眼的眼底彩照以及FFA图像分别依据我国六期分期法以及国际分期法进行分期。统计两种方法各期的一致性。结果 162眼中,按照六期分类法,以免散瞳下眼底彩照为标准,诊断为Ⅰ期DR患者为59眼,诊断为Ⅱ期DR患眼为37眼,诊断为Ⅲ期DR患眼为66眼;以50°眼底彩照为标准,Ⅰ期DR患者为52眼,Ⅱ期DR患者为38眼,Ⅲ期DR患者为72眼。按照国际分期法,轻度NPDR为34眼,中度NPDR患者为30眼,重度NPDR者为92眼,PDR为6眼。小瞳下眼底彩照以及散瞳下2张50°眼底彩照判断DR临床分期的一致性较好(Kappa=0.91)。六期分期法以及国际分期法两种分期方法的一致性一般(Kappa=0.634)。以轻度NPDR为筛查阈值,Ⅰ期病变的敏感度及特异度分别是100.0%、85.9%;以中度NPDR为筛查阈值,Ⅱ期病变的敏感度及特异度分别66.6%、86.3%;以重度NPDR为筛查阈值,Ⅲ期病变的敏感度及特异度分别71.0%、91.4%。结论 FFA指导下的国际分期法更能够真实、客观地反映糖尿病性视网膜病变的微血管病变情况以及严重程度。对存在弥漫出血点以及微血管瘤的Ⅰ、Ⅱ、Ⅲ期糖尿病视网膜病变患者应及早进行FFA检查。

NUTRITIONAL LIPID LIVER DISEASE OF GRASS CARP CTENOPHARYNGODON IDULLUS(C.et V.)

The inadequate nutrient content of pellet feeds widely used in recent years in China for grass carp forming led to lipid liver degeneration in the fish. The present studies show that the pathological features of lipid liver disease are anaemia and hepatic ceroidosis. Other clinical features are: the ratio of liver to body weight exceeds 3% and lipid content exceeds 5%. Extreme infiltration of hepaiocytes by lipid results in the following deteriorative effects: swelling of the liver cells, increase of lipid droplets in the cytoplasm and dislocation of the nucleus, loss of cytoplasm staining affinity, and increased activities of GOT and GPT in serum. Lipid liver degeneration of grass carp can be divided into three stages: 1) deposition of liver lipid; 2) lipid infiltration of hepatic parenchyma; 3) atrophy of liver nucleus. The causes of lipid liver degeneration are complicated, but the main cause is assumed to be an imbalance of nutrients in daily feed and the lack of some lipotropic substances.

Preparation of Gd Co alloy in dimethylsulfoxide by electrodeposition method

Electrodeposition of Gd Co alloy film from dimethylsulfoxide (DMSO) electrolyte solutions has been investigated using cyclic voltammogram and potentiostatic technique. Potentiostatic depositions between -1.20 V and -2.70 V were applied to uncomplexed and complexed solution. The deposits obtained at potentials within -2.30～-2.70 V from complexed solutions are silver gray smooth films.

UNCONVENTIONAL GURTIN-TYPE VARIATIONAL PRINCIPLES FOR FINITE DEFORMATION ELASTODYNAMICS

A theory of tailing wavetrain generation for the precursor soliton generation in two-layer flow is presented by using averaged KdV equations (AKdV), which are derived by the authors in terms of Whitham's method of averaging[1,2]. From the AKdV equations, group velocities of the tailing wavetrain generation are obtained by means of generating conditions of the tailing wavetrains, furthermore an analytical solution of the tailing wavetrain generation is found theoretically. A comparison between the theoretical and numerical results is carried out in the present paper, which shows that the theoretical results are in good agreement with the numerical ones, obtained from the fKdV equation in two-layer flow with the depth of unity in the rest.

Morphodynamic Processes of Tidal Inlet and Ebb-Tidal Delta of Shuidong Bay

Based on the observed data in 1982 approximately 1985 at Shuidong Bay area, west Guangdong Province, the morphodynamic processes of tidal inlet and ebb-tidal delta in the barrier-lagoon system are presented, including the dynamic features of tides and in the tidal inlet, the regional dynamics and longshore sediment transport in the ebb-tidal delta, the genesis of the entrance bar, the recent erosion and deposition in the ebb-tidal delta and so on. The paper attempts to answer two questions, i.e., the stability of the tidal inlet and the feasibility of dredging on the entrance bar for the course of Shuidong Harbour. The results show that the stability of the tidal inlet is ideal and that dredging action on the entrance bar may be successful if dredging is deep enough and the course position reasonable.

Preparation,physicochemical characterization and cytotoxicity in vitro of gemcitabine-loaded PEG-PDLLA nanovesicles

AIM:To investigate the preparation,physicochemical characterization and cytotoxicity in vitro of Gemcitabine-loaded poly(ethylene glycol)-block-poly(D,L-lactide) (PEG-PDLLA) nanovesicles. METHODS:The nanovesicle carriers were prepared from the amphiphilic block copolymer of PEG-PDLLA by a double emulsion technique,and gemcitabine was used as the model drug. The morphology of the nanovesicles was determined by scanning and transmission electron microscopy,and the drug content,drug entrapment and drug-release curve in vitro were detected by UV-Vis-NIR spectrophotometry. Cytotoxicity in the human pancreatic cancer cell line SW1990 was tested by 3-(4,5-dimethyl) ethiazole (MTT) assay.RESULTS:The gemcitabine-loaded nanovesicles were hollow nanospheres with a mean size of 200.6 nm,drugloading of 4.14% and drug embedding ratio of 20.54%. The nanovesicles showed excellent controlled release that was characterized by a fast initial release during the first 72 h,followed by a slower and continuous release. The MTT assay demonstrated that gemcitabine-loaded nanovesicles exhibited dose-dependent and time-delayed cytotoxicity in the human pancreatic cancer cell line SW1990.CONCLUSION:Gemcitabine-loaded PEG-PDLLA nanovesicles prepared by a double emulsion technique exhibited good performance for controlled drug release,and had similar cytotoxic activity to free gem-citabine.

Study on Electrochemical Formation of Neodymium-Iron Alloy

The cyclic voltammetry was used to investigate the electrode processes of Nd(III) reduced on iron electrode and Nd(III),Fe(II) reduced on molybdenum electrode in molten chlorides. The Nd-Fe and Nd-rich RE-Fe alloys contained rare earth up to 90wt% were prepared by consumable cathode and electrolytic codeposition. The mechanism of electrochemical formation of Nd-Fe alloy had been discussed.

Horizontal Slow Oscillation of a Moored Tanker and Numerical Method of Hopf-Bifurcation

In this paper, a six-order nonlinear dynamic model with three degrees of freedom is presented for the study of the 'fishtail' motion of a Single Point Mooring System. The effect of parameter variations on the equilibrium state of the system is analyzed. In order to study the stability of the equilibrium state, the mooring-line length l is chosen as a bifurcation parameter, so that all eigenvalues of the Jacobian matrix under different parameters can be worked out, and then the Hopf-bifurcation point can be found. Finally, the Hopf-bifurcation periodic solution of the system is computed.

Study of Snap-acting Two Way Shape Memory Effect in NiTi Alloy

Snap-acting transformation and snap-acting two way shape memory (TWSM) effect have been obtained in NiTi alloy using the constrained effect of stress on the driving force of TWSM element. The TWSM strain of the snap-acting element made of Ni51Ti49 alloy finishes abruptly at certain temperature without any additional mechanism, within an interval less than 1 ms. A thermodynamic model is presented to account for these observations.

Nonlinear Coupled Motions for a Given Two-Point Tension Mooring System

The nonlinear behaviors of plane coupled motions for a given two-point tension mooring system, are discussed in the present paper. For a cylinder moored by two taut lines under the action of gravity, buoyance and forces due to wave-current and mooring lines, a mathematical model of motions with three degrees of freedom is established. The steady solution and stability are analyzed. By integrating the equations of motions, history, phase map and Poincare map are obtained. The Liapunov exponents are also computed. The numerical results show that: the horizontal movement will increase, and stability will also increase as the steady force increases. The amplitude of responses will decrease as time-dependent forces decrease. Because of the geometric nonlinearity, there exist many windows bifurcating to pseudo-periodic or multi-periodic solution. The bifurcating patterns may be different. The behaviors are very complex. Under wave excitation alone, the motions are nonsymmetrical but still symmetrical statistically.

Hybrid Element Method for Calculation of Harbor Resonance

This paper presents a hybrid element method for calculating harbor resonance in a coastal or offshore harbor under the effects of friction and boundary absorption. The friction term is assumed to be proportional to the flow velocity with a phase difference. The boundary absorption adopts a condition similar to the impedance condition in acoustics. In the near region a variational principle is established. In the far region the solution satisfies the Helmholtz equation. Computation of results of harbor resonance by the model show good agreement between experimental data and theoretical results.

Electromagnetic Field Due to a Loop Antenna Buried in a Medium with Complex Boundaries

The radiation of a loop antenna embedded in a dissipative medium with complex boundaries isanalyzed by a perturbation method and an efficient fast multiple-integration technique. But theperturbation method can not be used directly because there is a finite-length metal cylinder in the vicinityof the loop antenna. The prolate ellipsoid equivalence of the metal cylinder is made, then the cylinder maybe removed and the perturbation method is valid. Numerical results indicate that the approach is accurateat low frequencies and stable.

A New Cytotoxic Dihydroxy Sterol from Laurencia Majuscula

24s-Stigmasta 4,25-diene,3 beta,6 beta-diol 1,a new sterol has been isolated from the algae Laurencia majuscula collected from the south China sea. Its structure was elucidated by spectroscopic methods. Compound 1 was cytotoxic against the P-388 cell line with an IC50 value of 1.2 mu g/ml.

Prevention, diagnosis and treatment of biliary complications after liver transplantation

Objective To study the prevention, diagnosis and treatment of biliary complications after liver transplantation. Methods One hundred and twenty-three recipients who had received liver transplantation between April 1993 and November 2001 were retrospectively reviewed. Results Biliary complication was diagnosed cholangiographyically in 11 patients. Nine patients were cured, 1 patient was unproved and 1 patient died. The incidence for biliary complications was 8.9% (11/123),and the mortality was 0.8% (1/123). The incidence of biliary complicatins due to T tube was 4.2% (5/ 119). The incidence of biliary complications due to hepatic artery was 1. 6% (2/123). In recipients with WIT】3 min and CIT】8 h, the incidence of biliary complications elevated significantly. Conclusion The most important reason for biliary complications was preservative and ischemic injury. While repairing donor liver, damage to the blood supply system of donor liver bile ducts should be avoided. Meticulous T tube placement may reduce
