The symptoms of the inclusive cases were: itching at local parts, rash-like after scratch, the lichen-like skin appeared days after days. Besides that, skin itching caused by other various reasons were included. The affected parts of all selected itching cases were confined to the back and bilateral sides of neck, chelidon, popliteal fossa, the inner side of thighbone, the end of rumpbone, wrist, and ankle.展开更多
文摘目的探讨带状疱疹后神经痛(postherpetic neuralgia,PHN)的危险因素,并构建列线图预测模型,为带状疱疹患者的临床管理提供理论指导。方法收集2020年3月至2022年3月于武汉科技大学附属孝感医院住院的272例带状疱疹患者的临床资料,根据患者出院1个月后电话随访结果,将患者分为PHN组和非PHN组,比较两组患者的临床资料,探讨发生PHN的危险因素。构建预测PHN的列线图模型并评估列线图模型对PHN患者的预测价值。结果共258例患者完成随访,其中83例(32.2%)发生PHN。单因素分析结果显示,两组患者的年龄、初治时间、皮损面积、他汀类药物的使用、基础疾病、血清神经元特异性烯醇化酶(neuron specific enolase,NSE)、三酰甘油(triglyceride,TG)及疼痛视觉模拟评分法(visual analoguescale,VAS)评分比较,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,年龄≥70岁、初治时间>7d、皮损面积越大、既往服用他汀类药物、既往有基础疾病、血清NSE和TG水平升高、急性期VAS评分高均是发生PHN的独立危险因素(P<0.05)。基于发生PHN的独立危险因素建立列线图模型,其预测PHN发生的曲线下面积为0.897(95%CI:0.854~0.940)。校准曲线与理想曲线贴合良好。结论年龄、初治时间、皮损面积、他汀类药物用药史、基础疾病、血清NSE、TG、VAS评分均是发生PHN的独立危险因素,据此建立的列线图可较好地预测PHN的发生。
