目的:研究利多卡因气管内表麻能否抑制甲状腺手术的牵拉反应,提高神经探测仪灵敏度。方法:选择拟全麻下行甲状腺大部切除术患者60例,随机分为两组。Ⅰ组:对照组:通过气管导管向气道内注入0.9%生理盐水2.5 m L;Ⅱ组:治疗组:气道内注入2%...目的:研究利多卡因气管内表麻能否抑制甲状腺手术的牵拉反应,提高神经探测仪灵敏度。方法:选择拟全麻下行甲状腺大部切除术患者60例,随机分为两组。Ⅰ组:对照组:通过气管导管向气道内注入0.9%生理盐水2.5 m L;Ⅱ组:治疗组:气道内注入2%利多卡因溶液2.5 m L。观察指标:探测仪信号、气道压力、气道峰压(Ppeak)、血压(BP)、心率(HR)。结果:(1)探测仪信号变化:Ⅰ组深部手术刺激,患者出现气道高压以及循环系统波动,追加肌松药后探测仪信号消失;Ⅱ组利多卡因表麻对探测仪信号无影响;(2)Ⅱ组Ppeak明显<Ⅰ组,且两组气道压力的差值(ΔP)有统计学差异(P<0.01);(3)Ⅱ组循环指标更稳定,组间有统计学差异(P<0.05);(4)Ⅱ组耐管时间相对延长,组间有统计学差异(P<0.05)。结论 :2%利多卡因气管内表麻可改善牵拉肿物、刺激气道造成的气道高压,延长耐管时间、减少肌松药用量,不影响喉返神经探测仪功能,维持循环系统稳定。展开更多

There are four main superiority of intracarotid infusion drugs : ①the rapid effect of infusing drugs;②the strong therapy purpose ;③ the good therapy effects ;and ④ the minor general side effect ,et al. More important, animal and clinical experiments have indicated that it is a safety way to infuse drugs though intracarotid artery without brain vascular nerve injury since 1945. In order to prove the possibility of clinical anesthetize using of intracarotid infusion anesthetics, therefore, the purpose of this paper is to review the research status que of intracarotid infusion drugs in recent years.展开更多
