The compromised regeneration of the spinal cord is primarily attributed to damaged neurons, inhibitory molecules, dysfunctional immune response, and glial scarring. Unfortunately, currently, there are no effective treatments available that can fully repair the spinal cord and improve functional outcomes. Nevertheless, numerous pre-clinical approaches have been studied for spinal cord injury recovery, including using biomaterials, cells, drugs, or technological-based strategies. Combinatorial treatments, which target various aspects of spinal cord injury pathophysiology, have been extensively tested in the last decade. These approaches aim to synergistically enhance repair processes by addressing various obstacles faced during spinal cord regeneration. Thus, this review intends to provide scientists and clinicians with an overview of pre-clinical combinatorial approaches that have been developed toward the solution of spinal cord regeneration as well as update the current knowledge about spinal cord injury pathophysiology with an emphasis on the current clinical management.展开更多

In this study, we analyzed the monthly Snow Cover Extent(SCE) from November to April in the Central Taurus Mountains(Bolkar, Aladaglar, Tahtali and Binboga Mountains) from 1981 to 2021. Linear trends of snow cover season(November to April) over the last 41 years showed decreases in SCE primarily at lower elevations. The downward trend in SCE was found to be more pronounced and statistically significant for only November and March. SCE in the Central Taurus Mountains has declined about-6.3% per decade for 2500-3000 m in November and about-6.0% per decade for 1000-1500 m and 3000+ m in March over the last 41 years. The loss of SCE has become evident since the 2000s, and the lowest negative anomalies in SCE have been observed in 2014, 2001, and 2007 in the last 41 years, which are consistent with an increase in air temperature and decreased precipitation. SCE was correlated with both mean temperature and precipitation, with temperature having a greater relative importance at all elevated gradients. Results showed that there is a strong linear relationship between SCE and the mean air temperature(r =-0.80) and precipitation(r = 0.44) for all elevated gradients during the snow season. The Arctic Oscillation(AO), the North Atlantic Oscillation(NAO), and the Mediterranean Oscillation(MO) winter indices were used to explain the year-to-year variability in SCE over the Central Taurus Mountains. The results showed that the inter-annual variability observed in the winter SCE on the Central Taurus Mountains was positively correlated with the phases of the winter AO, NAO and MO, especially below 2000 m elevation.展开更多

Even when therapy is initiated within the first 24 hours, it is associated with a mortality rate of up to 25%. Due to the varied initial presentation, treatment may be delayed, leading to poorer outcomes. Here, we present a unique case of a 27-year-old schizophrenic patient who initially presented with acute respiratory failure and septic shock and was ultimately diagnosed with cavitary pneumonia secondary to esophageal perforation.展开更多

The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. Only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and RUS placement concerning at least one of the defined outcome measures were included. Lastly, MEDLINE database and PubMed platform were screened again using the same terms, from June 2019 until November 2022.Results: Of 556 initial articles, seven were included in this review. Most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. One article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent among articles. Conclusion: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with RUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.展开更多

文摘The snow cover over the Taurus Mountains affects water supply, agriculture, and hydropower generation in the region. The results showed that the inter-annual variability observed in the winter SCE on the Central Taurus Mountains was positively correlated with the phases of the winter AO, NAO and MO, especially below 2000 m elevation.
文摘Esophageal perforation, a rare condition, can arise from iatrogenic, traumatic, or spontaneous origins. Even when therapy is initiated within the first 24 hours, it is associated with a mortality rate of up to 25%. Due to the varied initial presentation, treatment may be delayed, leading to poorer outcomes. Here, we present a unique case of a 27-year-old schizophrenic patient who initially presented with acute respiratory failure and septic shock and was ultimately diagnosed with cavitary pneumonia secondary to esophageal perforation.
文摘Dear Editor,We present a case of posterior cortical atrophy(PCA),which is a syndromic entity caused by different neurodegenerative diseases, mainly Alzheimer’s disease, but it has been described in several other entities. Its frequency is reported to be as high as 5%of all cases;however,it remains widely under-recognized among ophthalmologists and optometrists due to scarce reports in visual health-related journals[1].
文摘Objective: To assess if there is a preferable intervention between retrograde ureteral stent (RUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the other. Methods: We searched MEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

文摘Melt extrusion-based additive manufacturing(ME-AM)is a promising technique to fabricate porous scaffolds for tissue engi-neering applications. However, most synthetic semicrystalline polymers do not possess the intrinsic biological activity required to control cell fate.Grafting of biomolecules on polymeric surfaces of AM scaffolds enhances the bioactivity of a construct;however, there are limited strategies available to control the surface density. Here,we report a strategy to tune the surface density of bioactive groups by blending a low molecular weight poly(ε-caprolactone)5k(PCL5k)containing orthogonally reactive azide groups with an unfunctionalized high molecular weight PCL75k at different ratios.Stable porous three-dimensional(3D)scaf-folds were then fabricated using a high weight percentage(75 wt.%)of the low molecular weight PCL 5k.As a proof-of-concept test,we prepared films of three different mass ratios of low and high molecular weight polymers with a thermopress and reacted with an alkynated fluorescent model compound on the surface, yielding a density of 201-561 pmol/cm^(2).Subsequently,a bone morphogenetic protein 2(BMP-2)-derived peptide was grafted onto the films comprising different blend compositions, and the effect of peptide surface density on the osteogenic differentiation of human mesenchymal stromal cells(hMSCs)was assessed. After two weeks of culturing in a basic medium, cells expressed higher levels of BMP receptor II(BMPRII)on films with the conjugated peptide. In addition,we found that alkaline phosphatase activity was only significantly enhanced on films contain-ing the highest peptide density(i.e.,561 pmol/cm^(2)),indicating the importance of the surface density.Taken together, these results emphasize that the density of surface peptides on cell differentiation must be considered at the cell-material interface. Moreover,we have presented a viable strategy for ME-AM community that desires to tune the bulk and surface functionality via blending of(modified)polymers.Furthermore, the use of alkyne-azide“click”chemistry enables spatial control over bioconjugation of many tissue-specific moieties, making this approach a versatile strategy for tissue engineering applications.
文摘Background:Alcohol and illicit drugs(AID)continue to be a major global health concern.Although preventable,AID is linked to millions of deaths annually worldwide. The situation is particularly grave for young people, with AID being a major direct risk factor for disability-adjusted youth life-years lost and death. It further contributes to assaults, road crashes, accidental poisoning, and suicide, leading to long-term issues and public health concerns. Objective:This study aimed at disclosing current AID prevalence data for Argentinian,Bulgarian,Chilean and Romanian youth. It shed light on the predictors of AID in young people from those countries.Method:The study used an online survey to gather data from people aged 18 to 25(n=1,297).The survey was underpinned by the theory of planned behaviour(TPB).Predictors were investigated separately for drinking alcohol and using illicit drugs. Results:Our data revealed that across the four target countries,49%to 90%of the participants drank alcohol, and 8%to 35%used illicit drugs in the past three months.Between 20%and 91%of them intended to drink, and between 8%and 31%intended to use illicit drugs in the following three months. Our TPB model predicted statistically significant(P<0.001)amounts of variance in drinking alcohol(between 61%and 72%)and using illicit drugs(between 20.3%and 74.4%).Intention was consistent in significantly predicting both behaviours.Evidence around the predictive validity of self-efficacy,age and gender was mixed across the investigated countries. Conclusion:This research provided an update on the scarce AID epidemiological data.It also supplied evidence about what theoretically-informed measures might be useful targets of interventions in the case of Argentina,Bulgaria,Chile and Romania. This new knowledge of understanding substance abuse determinants and prevalence may help researchers and practitioners better meet young people's health prevention needs.
