Economic Analysis of Port and PICC in Long-term Intravenous Administration for Malignant Tumor Patients in Chinese Oncology Hospital Setting

OBJECTIVE: To calculate and compare the cost of Port and PICC's application in long-term intravenous administration, and to support the decision making of hospital manager. METHODS: Literature review and patient survey in 2 oncology centers in China were carried out to investigate the cost and impact of Port and PICC for patients. The cost at different time of intravenous administration was calculated and compared. One-way sensitivity analysis was performed and tornado graph was drawn. RESULTS: Direct cost of Port at 0.5, 1, 1.5, and 2 years were7442, 8005, 8553, and 9131 CNY, and 4700, 9399, 14032, 18799 CNY for PICC respectively. Direct & indirect cost at 0.5, 1, 1.5, and 2 years were 9291, 11704, 14101, 16529 CNY for Port and 9697, 19393, 29023, 38787 CNY for PICC. Sensitivity analysis showed that productivity loss and device maintenance cost were the most in?uential factors to the result. CONCLUSION: Port had higher cost in short term and less in long term compared with PICC. Patients expected to get intravenous administration more than 0.5 year should use Port if both direct and indirect costs were included.

Study on Problems and Countermeasures in the Management of Spot Check of Chinese Herbal Pieces

Objective To provide suggestions for optimizing the sampling management of Chinese herbal pieces and improving the quality Chinese herbal pieces.Methods Logical reasoning method was used to analyze the problems in the drug spot check of Chinese herbal pieces based on the data of drug quality published by the National Medical Products Administration from 2013 to 2021,combined with the current provisions and implementation of drug spot check management in China.Results and Conclusion At present,there are some problems in the drug spot check of Chinese herbal pieces,such as fewer varieties of Chinese herbal pieces in the national drug spot check,the unreasonable sampling links,the insufficient territorial management in some areas,and error-prone sample extraction.It is suggested that the actual quantity and overall quality of Chinese herbal pieces should be fully considered in the formulation of sampling plan.Besides,the variety coverage of drug spot check of traditional Chinese medicine should be strengthened.In the planning,the sampling links shall be made as a whole,and the credit rating and distribution of drug production,operation and use units within the jurisdiction shall be fully combined.While strengthening the sampling management at the production end in areas with concentrated industries,regional special drug spot check should be carried out or the administrative forces at the provincial,municipal and county levels should be fully mobilized to avoid local protection.In the assigned tasks,the name of Chinese herbal pieces should be accurately expressed to avoid the sampling errors of Chinese herbal pieces with the same original medicinal materials and different processing methods.

Pharmacoeconomic Evaluation of Rituximab (Hanlikang) for Patients with Rheumatoid Arthritis

Objective To provide reference for clinical medication and drug policy formulation for patients with rheumatoid arthritis.Methods A Markov model was established for patients with moderate and severe rheumatoid arthritis.The model period was 6 months and the simulation time was the average life expectancy of Chinese residents(76 years).The cost-utility analysis method was used to analyze the effect of Hanlikang combined with methotrexate and Yisaipu combined with methotrexate from the perspective of the whole society.The economy of Hanlikang was evaluated and the robustness of the results was verified by sensitivity analysis.Results and Conclusion For the patients with moderate and severe rheumatoid arthritis,compared with Yisaipu,Hanlikang could save 69228 yuan and gain 0.837 quality adjusted of life years(QALYs)in the whole life cycle with great economic advantages.The results of sensitivity analysis were consistent with those of basic analysis,and the results of probability sensitivity analysis showed that when the willingness to pay was the per capita GDP(64644 yuan),the economic probability of Hanlikang group reached 79.3%.
