BACKGROUND Maintenance rates of exclusive breastfeeding(EBF)worldwide are low,thus,one of the objectives of the summary of policies on breastfeeding(BF)in world nutrition goals for 2025 are that at least 50%of infants...BACKGROUND Maintenance rates of exclusive breastfeeding(EBF)worldwide are low,thus,one of the objectives of the summary of policies on breastfeeding(BF)in world nutrition goals for 2025 are that at least 50%of infants under six months of age receive EBF that year.The Objective of this study is to document the rates of EBF in children born in San Ignacio University Hospital(HUSI)and identify factors associated with maintenance.AIM To document the percentages of EBF in those that were born at HUSI and identify factors associated to their maintenance.METHODS This is a study of cases and controls in an analytic,retrospective cohort that took children born alive between January 2016 and January 2019 at HUSI located in the city of Bogotá,Colombia.RESULTS Receiving information about BF at HUSI was able to maintain EBF up until 4 mo(OR=1.65;95%CI:1.02-2.66).The presence of gynecologic and obstetric comorbidities(OR=0.32;95%CI:0.12-0.83),having mastitis(OR=0.56;95%CI:0.33-0.94),and receiving information from mass media(OR=0.52;95%CI:0.31-0.84)are factors associated with not maintaining EBF.CONCLUSION Receiving education at a Women-and Child-Friendly Institution was the only significant factor to achieve EBF until 4 mo,with a frequency greater than the one reported in the country,which matches multiple studies where counseling and individualized support on BF achieve this purpose.Knowledge about BF and early detection of obstetric/gynecologic complications must be strengthened among the healthcare staff in charge of mothers during post-partum.Additionally,strategies must be promoted to continue BF such as creating milk banks with the objective of increasing BF rates even when mothers return to work.展开更多
BACKGROUND The global prevalence of obesity has increased over the past 40 years,and bariatric surgery has proven to be the most effective therapy for long-term weight loss.Its principles are based on modifying the br...BACKGROUND The global prevalence of obesity has increased over the past 40 years,and bariatric surgery has proven to be the most effective therapy for long-term weight loss.Its principles are based on modifying the brain-gut axis by altering the gastrointestinal anatomy and affecting the function of gastrointestinal hormones,thereby modifying satiety signals.Single anastomosis duodeno-ileal bypass with sleeve gastrectomy(SADI-S)combines both techniques and has become an alternative to gastric bypass and biliopancreatic diversion procedures for treating severe obesity and associated metabolic diseases in selected patients.AIM To describe the outcomes and complications of SADI-S.METHODS We retrospectively analyzed the data of patients who underwent SADI-S laparoscopically at the Clínica Reina Sofía in Bogotá,Colombia.This study assessed the therapeutic effectiveness of SADI-S in terms of short-term preoperative clinical characteristics,postoperative complications,comorbidities,nutritional deficiencies,and intraoperative complications during a 2-year follow-up.RESULTS Sixty-one patients with a mean body mass index(BMI)of 50±7.1 kg/m2 underwent laparoscopic SADI-S.The mean operative time and hospital stays were 143.8±42 min and 2.3±0.8 d,respectively.The mean follow-up period was 18 mo,and the mean BMI decreased to 28.5±12.2 kg/m2.The excess BMI loss was 41.8%±13.5%,and the weight loss percentage was 81.1%±17.0%.Resolution of obesity-related comorbidities,including type 2 diabetes mellitus,hypertension,dyslipidemia,and obstructive sleep apnea,was achieved and defined as complete or partial remission.No intraoperative complications were observed.Short-term complications were observed in four(6.8%)patients.However,larger studies with longer follow-up periods are required to draw definitive conclusions.CONCLUSION SADI-S has a low intraoperative and postoperative complication rate and is effective for weight loss and improving obesity-related comorbidities,including hypertension,type 2 diabetes mellitus,dyslipidemia,and sleep apnea syndrome.展开更多

This case study describes a 62-year-old male patient with acquired brain damage of the anterior cingulate cortex as a result of an ischemic event in the territory of the left anterior cerebral artery. Cognitive function was assessed using the neuropsychological executive function and frontal lobe test battery (BANFE-2) as well as other neuropsychological tests. The results show a profile of higher mental functions characterized by the presence of dysexecutive syndrome with marked behavioral alteration and diencephalic amnesia. .展开更多

Predicting depression intensity from microblogs and social media posts has numerous benefits and applications,including predicting early psychological disorders and stress in individuals or the general public.A major ...Predicting depression intensity from microblogs and social media posts has numerous benefits and applications,including predicting early psychological disorders and stress in individuals or the general public.A major challenge in predicting depression using social media posts is that the existing studies do not focus on predicting the intensity of depression in social media texts but rather only perform the binary classification of depression and moreover noisy data makes it difficult to predict the true depression in the social media text.This study intends to begin by collecting relevant Tweets and generating a corpus of 210000 public tweets using Twitter public application programming interfaces(APIs).A strategy is devised to filter out only depression-related tweets by creating a list of relevant hashtags to reduce noise in the corpus.Furthermore,an algorithm is developed to annotate the data into three depression classes:‘Mild,’‘Moderate,’and‘Severe,’based on International Classification of Diseases-10(ICD-10)depression diagnostic criteria.Different baseline classifiers are applied to the annotated dataset to get a preliminary idea of classification performance on the corpus.Further FastText-based model is applied and fine-tuned with different preprocessing techniques and hyperparameter tuning to produce the tuned model,which significantly increases the depression classification performance to an 84%F1 score and 90%accuracy compared to baselines.Finally,a FastText-based weighted soft voting ensemble(WSVE)is proposed to boost the model’s performance by combining several other classifiers and assigning weights to individual models according to their individual performances.The proposed WSVE outperformed all baselines as well as FastText alone,with an F1 of 89%,5%higher than FastText alone,and an accuracy of 93%,3%higher than FastText alone.The proposed model better captures the contextual features of the relatively small sample class and aids in the detection of early depression intensity prediction from tweets with impactful performances.展开更多

The use of technologies like virtual reality presents an opportunity to enhance their skills through simulations and training. The main objective of this study is to qualitatively assess the stress levels experienced by medical students and residents by integrating virtual reality into their current learning methods, aiming to improve their ability to manage stressors in their practice. Material and Methods: A questionnaire was conducted with 12 medical students and 12 Traumatology and orthopedics residents. The purpose of the questionnaire was to evaluate the levels of academic stress using the SISCO inventory. The stress levels were calculated by transforming average values into percentages, and the following criteria were assigned: 0 to 33% for Mild Stress, 34 to 66% for Moderate Stress, and 67 to 100% for Deep Stress. Then, a virtual reality class focused on spine surgery was provided. Both medical students and residents were trained using the Non Nocere SharpSurgeon software platform and Oculus Quest 2 virtual reality glasses. At the end of the session, a second questionnaire related to the practice with virtual reality was conducted with the same evaluation criteria and a comparative analysis was carried out. Results: 12 undergraduate students from Hospital Angeles Mexico, CDMX and 12 traumatology and orthopedics residents at Hospital Santa Fe, Bogota were evaluated. The students in CDMX reported an average qualitative stress of 28.50% during habitual practices, which decreased to an average of 14.67% after virtual reality practice. Residents in Bogotá experienced an average qualitative stress of 30.50% with their current learning methods but this reduced to an average of 13.92% after using virtual reality. These findings indicate that the use of virtual reality has a positive impact on reducing stress levels qualitatively. Conclusions: The use of virtual reality as a learning method for medical students and residents qualitatively improves stress levels. Further studies are required to define the potential uses of Virtual Reality to improve learning methods and emotional state in medical students and residents and for a quantitative assessment to validate the training as certified learning methods.展开更多

Although studies are rarely reported on these soils, they have been identified in Mexico City, the Sea of Japan, the northeast coast of Australia,the equatorial Pacific, and the lacustrine deposit of Bogota(Colombia), among other locations. Features of this kind of soil include high friction angle, high initial void ratio, high compressibility index, high liquid limit, and low density. Some of these features are counterintuitive from a classical soil mechanics viewpoint. To understand the geotechnical properties of the diatomaceous soil, a comprehensive experimental plan consisting of more than 2400 tests was performed, including physical tests such as grain size distribution, Atterberg limits, density of solid particles, and organic matter content; and mechanical tests such as oedometric compression tests, unconfined compression tests, and triaxial tests.Laboratory tests were complemented with scanning electron microscope(SEM) observations to evaluate the microstructure of the soil. The test results show that there is an increase in liquid limit with increasing diatomaceous content, and the friction angle also increases with increasing diatomaceous content. In addition, several practical correlations were proposed for this soil type for shear strength mobilization and intrinsic compression line. Finally, useful correlations were presented, such as the relationship between the state consistency and the undrained shear strength, the friction angle and the liquid limit, the void ratio at 100 kPa and the liquid limit, the plasticity index and the diatomaceous content, among others.展开更多

It is a retrospective and concurrent evaluation of thirty patients f...The objective of this study is to determine the auditory gain, quality of life, audiological benefits, in bone-anchored hearing device users (BAHA). It is a retrospective and concurrent evaluation of thirty patients fitted unilaterally and seven fitted bilaterally for at least six months. Patients were assessed with audiometric testing and application of Glasgow Benefit Inventory (GBI) and Abbreviated Profile of Hearing Aid Benefit (APHAB). Regarding sound-field pure audiometry results, we found a statistically significant gain in all frequencies using the bone-anchored device. APHAB scores showed statistically significant subjective audiological gains in all subscales except for the aversiveness subscale. GBI mean scores for all items in both groups were all above 3, suggesting quality of life improvement in conductive and mixed hearing loss patients. BP100 users showed a greater clinical gain in the APHAB global score and subscales compared with Divino users. In conclusion the BAHA provides significant auditory gain, subjective audiological benefits and improves quality of life in all BAHA users. This study shows a significant clinical and statistical benefit of BAHA measured by audiometric testing and by the APHAB and GBI questionnaires.展开更多
Scope:High-fat diet(HFD)induces imbalance in the small intestine environment,where fat digestion and absorption mainly take place.This study aimed to elucidate the mechanisms by which Lonicera caerulea polyphenols(LCP...Scope:High-fat diet(HFD)induces imbalance in the small intestine environment,where fat digestion and absorption mainly take place.This study aimed to elucidate the mechanisms by which Lonicera caerulea polyphenols(LCP)might inhibit fat absorption,from the perspective of small intestine microbiota and epithelial barrier integrity.Methods and results:Male Sprague-Dawley rats were given HFD with or without co-administration of LCP for 8 weeks.The results showed that LCP supplementation significantly decreased the levels of serum triglycerides(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C),and increased the contents of fecal sterols,in HFD rats.LCP also inhibited the dysfunction of the small intestine epithelial barrier,via alleviating the oxidative stress activated by Nrf2-ARE pathway,and by modulating the expressions of pro-inflammatory factors such as tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),cyclooxygenase-2(COX-2),nuclear factor kappa-B p65(NF-κB p65)and inducible nitric oxide synthase(iNOS)in the small intestine.Additionally,LCP administration restored the balance in small intestine microbiota and increased the abundance of the specific bacteria,such as Lactobacillus,involved in fat absorption.Conclusion:Our results demonstrated that LCP may be beneficial to inhibit fat absorption.The mechanism seems to be associated with the protection of the epithelial barrier integrity and the modulation of specific bacteria in the small intestine.展开更多

The modified UNIFAC(UNIversal quasi-chemical Functional-group Activity Coefficients)approach for ionic liquids is used to develop a thermodynamic model that describes the reactive system methylimidazole(MIM), chloroethane(C_(2)H_(5)Cl) and [EMIM][Cl]. The model allows to study the phase equilibria coexistence(vapor–liquid equilibria and solid–liquid equilibria) and yields the theoretically optimal conditions to synthesize the ionic liquid. The model predictions are validated with the available experimental and reported data. By implementing the developed model, a simple way to synthesize ionic liquid [EMIM][Cl] was found allowing to study its influence on the structure and morphology of pretreated rice husk. The lignocellulosic materials involved in this study are characterized by their composition, enzymatic digestibility, scanning electron microscopy, and crystallinity. Compared to untreated material, [EMIM][Cl]-pretreated rice husk produces cellulose that can be efficiently enzymatic hydrolyzed with high sugar yields. This work offers a suitable methodology to include the synthesis and thermodynamics of the solvent media within the design of low-cost ionic liquids for lignocellulosic biomass pretreatment.展开更多

In the small intestine, NET are the most frequent tumors, even more frequent than adenocarcinomas. Due to atypical presentations and late symptoms, NET in the small intestine frequently represent a diagnostic challenge. It is important to take these tumors into consideration in differential diagnosis of gastrointestinal neoplasms. Surgeons, oncologists, endocrinologists, and gastroenterologists should understand the disease characteristics and management alternatives. This document aims to review the key points of NET and main diagnostic tools. We present the case of a 50-year-old male who presented lower gastrointestinal bleeding. Imaging and endoscopic studies showed no conclusive findings. A capsule endoscopy showed multiple ulcered lesions with neoplastic aspect in the distal jejune. Due to the multifocal nature of the lesions, clinicians suspected NET-associated digestive bleeding. The patient underwent exploratory laparoscopy with ileectomy and radical abdominal lymphadenectomy. Histopathologic examination confirmed the suspected diagnosis of NET. This case reflects the complexity of diagnostic approach and differential diagnoses for these tumors.展开更多
The material associated with tree pruning in a city in the Colombian neotropics was characterized in order to determine its energy potential.The species studied for their relevance in the territory were Prosopis julif...The material associated with tree pruning in a city in the Colombian neotropics was characterized in order to determine its energy potential.The species studied for their relevance in the territory were Prosopis juliflora,Licania tomentosa,Terminalia catappa,Azadirachta indica,Pithecellobium dulce,Ficus benjamina and Leucaena leucocephala.Moisture content,bulk density,hygroscopic response,elemental chemical analysis,proximal analysis,calorific value,ease of ignition and combustion,thermogravimetric and heat flow analysis,steam generation capacity,and qualitative analysis of gases(mass spectrometry)were determined.The results that were obtained show high initial moisture contents that vary between 37%and 67%and a variable density(when dry)between 0.21 and 0.41 g/cm3.Chemically,it was shown that all residues have lower carbon and sulfur content compared to a reference mineral coal.However,the residues of some species have higher nitrogen contents compared to the same coal.All biomasses are characterized by their high content of volatile fractions and by having a lower content of inorganic matter compared to carbon.The lower calorific values of these residues are between 14170 and 16928 kJ/kg,which are not negligible compared to other biomasses.This characteristic would be related to the high presence of hemicellulose in the residues.Flue gas monitoring revealed that there are different airflow needs in order to ensure complete combustion.Steam generation tests showed that the calorific value should not be the most relevant criterion to establish the potential use of the waste,since leucaena leucocephala,despite having the highest calorific value,was the material with the lowest performance in steam generation.展开更多
BACKGROUND Despite various therapies to treat sepsis,it is one of the leading causes of mortality in the intensive care unit patients globally.Knowledge about the pathophysiology of sepsis has sparked interest in extr...BACKGROUND Despite various therapies to treat sepsis,it is one of the leading causes of mortality in the intensive care unit patients globally.Knowledge about the pathophysiology of sepsis has sparked interest in extracorporeal therapies(ECT)which are intended to balance the dysregulation of the immune system by removing excessive levels of inflammatory mediators.AIM To review recent data on the use of ECT in sepsis and to assess their effects on various inflammatory and clinical outcomes.METHODS In this review,an extensive English literature search was conducted from the last two decades to identify the use of ECT in sepsis.A total of 68 articles from peer-reviewed and indexed journals were selected excluding publications with only abstracts.RESULTS Results showed that ECT techniques such as high-volume hemofiltration,coupled plasma adsorption/filtration,resin or polymer adsorbers,and CytoSorb®are emerging as adjunct therapies to improve hemodynamic stability in sepsis.CytoSorb®has the most published data in regard to the use in the field of septic shock with reports on improved survival rates and lowered sequential organ failure assessment scores,lactate levels,total leucocyte count,platelet count,interleukin-IL-6,IL-10,and TNF levels.CONCLUSION Clinical acceptance of ECT in sepsis and septic shock is currently still limited due to a lack of large random clinical trials.In addition to patient-tailored therapies,future research developments with therapies targeting the cellular level of the immune response are expected.展开更多

Oral sodium bicarbonate is currently the treatment of choice. Objective: The objective is to determine if venous bicarbonate is equal to arterial bicarbonate in the follow-up of a patient with chronic kidney disease. Materials Methods: Single-center Cross-sectional studies in a cohort of adult patients with stage 4 - 5 CKD. Samples were taken between January 2022 and January 2023, in a Clinic in the city of Ibague/ Colombia obtained from the radial artery. The inclusion criteria were: not being treated with alkaline at the time of inclusion. Results: A total of 71 patients were included, 73.2% male (52) and 26.8% female (19), with different stages: stage 3 with 5.6% (4), stage 4 with 60.6% (43), stage 5 with 33.8% (23). 66.2% were diabetic, 88.7% had arterial hypertension, and 15.5% of the patients presented hematoma as a complication and pain associated with arterial puncture. The result of mean venous bicarbonate was 18.8 with a standard deviation of 2.3, arterial bicarbonate a mean of 19.4 with a standard deviation of 2.1 with a value of P 0.46, venous pH with a mean of 7.37 with a standard deviation of 0.48 and a mean arterial pH of 7.38 with a standard deviation of 0.48 with a P value of 0.01. Values of venous bicarbonate compared to arterial bicarbonate showed no statistically significant difference in patients with chronic kidney disease, but there were more complications such as hematoma and pain in patients in the arterial puncture cohort, because of this result venous bicarbonate corresponds to arterial bicarbonate, but has less risk of complications associated with the procedure. Conclusion: Metabolic acidosis is a frequent alteration in advanced chronic kidney disease, these results showed that the values of arterial and venous bicarbonate have no statistically significant differences, but there is a greater risk of complications with arterial blood gases, due to this, venous bicarbonate could be a useful tool for patients with chronic kidney disease.展开更多

The program integrates Vertical Electrical Sounding (VE...An innovative inversion code, named “Curupira v1.0”, has been developed using Matlab to determine the vertical distribution of resistivity beneath the subsoil. The program integrates Vertical Electrical Sounding (VES), successful in shallow subsurface exploration and Time Domain Electromagnetic (TEM) techniques, better suited for deeper exploration, both of which are widely employed in geophysical exploration. These methodologies involve calculating subsurface resistivity through appropriate inversion processes. To address the ill-posed nature of inverse problems in geophysics, a joint inversion scheme combining VES and TEM data has been incorporated into Curupira v1.0. The software has been tested on both synthetic and real-world data, the latter of which was acquired from the Parana sedimentary basin which we summarise here. The results indicate that the joint inversion of VES and TEM techniques offers improved recovery of simulated models and demonstrates significant potential for hydrogeological studies.展开更多
AIM:To assess the significance of chromosome translocation t(11;18)(q21;q21),B-cell lymphoma 10 (BCL-10) protein and Helicobacter pylori (H.pylori) infection in gastric mucosa-associated lymphoid tissue (MALT) lymphom...AIM:To assess the significance of chromosome translocation t(11;18)(q21;q21),B-cell lymphoma 10 (BCL-10) protein and Helicobacter pylori (H.pylori) infection in gastric mucosa-associated lymphoid tissue (MALT) lymphoma in Colombia.METHODS:Fifty cases of gastric MALT lymphoma and their respective post-treatment follow-up biopsies were examined to assess the presence of the translocation t(11;18)(q21;q21) as identified by fluorescence in situ hybridization;to detect protein expression patterns of BCL10 using immunohistochemistry;and for evaluation of tumor histology to determine the correlation of these factors and resistance to H.pylori eradication.RESULTS:Infection with H.pylori was confirmed in all cases of gastric MALT lymphoma in association with chronic gastritis.Bacterial eradication led to tumor regression in 66% of cases.The translocation t(11;18)(q21;q21) was not present in any of these cases,nor was there evidence of tumor transformation to diffuse large B-cell lymphoma.Thirty-four percent of the patients showed resistance to tumor regression,and within this group,7 cases,representing 14% of all those analyzed,were considered to be t(11;18)(q21;q21)-positive gastric MALT lymphomas.Protein expression of BCL10 in the nucleus was associated with the presence of translocation and treatment resistance.Cases that were considered unresponsive to therapy were histologically characterized by the presence of homogeneous tumor cells and a lack of plasmacytic differentiation.Responder cases exhibited higher cellular heterogeneity and a greater frequency of plasma cells.CONCLUSION:Both t(11;18)(q21;q21)-positive MALT lymphoma cases and those with nuclear BCL10 expression are considered resistant to H.pylori eradication.It is suggested that chronic antigenic stimulation is not a dominant event in resistant cases.展开更多
Eosinophilic esophagitis is an immune-allergic pathology of multifactorial etiology(genetic and environmental)that affects both pediatric and adult patients.Its symptoms,which include heartburn,regurgitation,and esoph...Eosinophilic esophagitis is an immune-allergic pathology of multifactorial etiology(genetic and environmental)that affects both pediatric and adult patients.Its symptoms,which include heartburn,regurgitation,and esophageal stenosis(with dysphagia being more frequent in eosinophilic esophagitis in young adults and children),are similar to those of gastroesophageal reflux disease,causing delays in diagnosis and treatment.Although endoscopic findings such as furrows,esophageal mucosa trachealization,and whitish exudates may suggest its presence,this diagnosis should be confirmed histologically based on the presence of more than 15 eosinophils per high-power field and the exclusion of other causes of eosinophilia(parasitic infections,hypereosinophilic syndrome,inflammatory bowel disease,among others)for which treatment could be initiated.Currently,the 3“D”s(“Drugs,Diet,and Dilation”)are considered the fundamental components of treatment.The first 2 components,which involve the use of proton pump inhibitors,corticosteroids,immunosuppressants and empirical diets or guided food elimination based on allergy tests,are more useful in the initial phases,whereas endoscopic dilation is reserved for esophageal strictures.Herein,the most important aspects of eosinophilic esophagitis pathophysiology will be reviewed,in addition to evidence for the various treatments.展开更多

文摘BACKGROUND Maintenance rates of exclusive breastfeeding(EBF)worldwide are low,thus,one of the objectives of the summary of policies on breastfeeding(BF)in world nutrition goals for 2025 are that at least 50%of infants under six months of age receive EBF that year.The Objective of this study is to document the rates of EBF in children born in San Ignacio University Hospital(HUSI)and identify factors associated with maintenance.AIM To document the percentages of EBF in those that were born at HUSI and identify factors associated to their maintenance.METHODS This is a study of cases and controls in an analytic,retrospective cohort that took children born alive between January 2016 and January 2019 at HUSI located in the city of Bogotá,Colombia.RESULTS Receiving information about BF at HUSI was able to maintain EBF up until 4 mo(OR=1.65;95%CI:1.02-2.66).The presence of gynecologic and obstetric comorbidities(OR=0.32;95%CI:0.12-0.83),having mastitis(OR=0.56;95%CI:0.33-0.94),and receiving information from mass media(OR=0.52;95%CI:0.31-0.84)are factors associated with not maintaining EBF.CONCLUSION Receiving education at a Women-and Child-Friendly Institution was the only significant factor to achieve EBF until 4 mo,with a frequency greater than the one reported in the country,which matches multiple studies where counseling and individualized support on BF achieve this purpose.Knowledge about BF and early detection of obstetric/gynecologic complications must be strengthened among the healthcare staff in charge of mothers during post-partum.Additionally,strategies must be promoted to continue BF such as creating milk banks with the objective of increasing BF rates even when mothers return to work.
文摘BACKGROUND The global prevalence of obesity has increased over the past 40 years,and bariatric surgery has proven to be the most effective therapy for long-term weight loss.Its principles are based on modifying the brain-gut axis by altering the gastrointestinal anatomy and affecting the function of gastrointestinal hormones,thereby modifying satiety signals.Single anastomosis duodeno-ileal bypass with sleeve gastrectomy(SADI-S)combines both techniques and has become an alternative to gastric bypass and biliopancreatic diversion procedures for treating severe obesity and associated metabolic diseases in selected patients.AIM To describe the outcomes and complications of SADI-S.METHODS We retrospectively analyzed the data of patients who underwent SADI-S laparoscopically at the Clínica Reina Sofía in Bogotá,Colombia.This study assessed the therapeutic effectiveness of SADI-S in terms of short-term preoperative clinical characteristics,postoperative complications,comorbidities,nutritional deficiencies,and intraoperative complications during a 2-year follow-up.RESULTS Sixty-one patients with a mean body mass index(BMI)of 50±7.1 kg/m2 underwent laparoscopic SADI-S.The mean operative time and hospital stays were 143.8±42 min and 2.3±0.8 d,respectively.The mean follow-up period was 18 mo,and the mean BMI decreased to 28.5±12.2 kg/m2.The excess BMI loss was 41.8%±13.5%,and the weight loss percentage was 81.1%±17.0%.Resolution of obesity-related comorbidities,including type 2 diabetes mellitus,hypertension,dyslipidemia,and obstructive sleep apnea,was achieved and defined as complete or partial remission.No intraoperative complications were observed.Short-term complications were observed in four(6.8%)patients.However,larger studies with longer follow-up periods are required to draw definitive conclusions.CONCLUSION SADI-S has a low intraoperative and postoperative complication rate and is effective for weight loss and improving obesity-related comorbidities,including hypertension,type 2 diabetes mellitus,dyslipidemia,and sleep apnea syndrome.
文摘Stroke is a physiological alteration associated with changes in blood flow that can result in sudden-onset cognitive impairment. It has a heterogenous clinical presentation with varying degrees of severity correlated with specific central nervous system zones or areas, and its prognosis is uncertain. This case study describes a 62-year-old male patient with acquired brain damage of the anterior cingulate cortex as a result of an ischemic event in the territory of the left anterior cerebral artery. Cognitive function was assessed using the neuropsychological executive function and frontal lobe test battery (BANFE-2) as well as other neuropsychological tests. The results show a profile of higher mental functions characterized by the presence of dysexecutive syndrome with marked behavioral alteration and diencephalic amnesia. .
文摘BACKGROUND A progressive decrease in exclusive breastfeeding(BF)is observed in Latin America and the Caribbean compared with global results.The possibility of being breastfed and continuing BF for>6 months is lower in low birth weight than in healthy-weight infants.AIM To identify factors associated with BF maintenance and promotion,with particular attention to low-and middle-income countries,by studying geographic,socioeconomic,and individual or neonatal health factors.METHODS A scoping review was conducted in 2018 using the conceptual model of social determinants of health published by the Commission on Equity and Health Inequalities in the United States.The extracted data with common characteristics were synthesized and categorized into two main themes:(1)Sociodemographic factors and proximal determinants involved in the initiation and maintenance of BF in low-birth-weight term infants in Latin America;and(2)individual characteristics related to the self-efficacy capacity for BF maintenance and adherence in low-birth-weight term infants.RESULTS This study identified maternal age,educational level,maternal economic capacity,social stratum,exposure to BF substitutes,access to BF information,and quality of health services as mediators for maintaining BF.CONCLUSION Individual self-efficacy factors that enable BF adherence in at-risk populations should be analyzed for better health outcomes.
文摘Predicting depression intensity from microblogs and social media posts has numerous benefits and applications,including predicting early psychological disorders and stress in individuals or the general public.A major challenge in predicting depression using social media posts is that the existing studies do not focus on predicting the intensity of depression in social media texts but rather only perform the binary classification of depression and moreover noisy data makes it difficult to predict the true depression in the social media text.This study intends to begin by collecting relevant Tweets and generating a corpus of 210000 public tweets using Twitter public application programming interfaces(APIs).A strategy is devised to filter out only depression-related tweets by creating a list of relevant hashtags to reduce noise in the corpus.Furthermore,an algorithm is developed to annotate the data into three depression classes:‘Mild,’‘Moderate,’and‘Severe,’based on International Classification of Diseases-10(ICD-10)depression diagnostic criteria.Different baseline classifiers are applied to the annotated dataset to get a preliminary idea of classification performance on the corpus.Further FastText-based model is applied and fine-tuned with different preprocessing techniques and hyperparameter tuning to produce the tuned model,which significantly increases the depression classification performance to an 84%F1 score and 90%accuracy compared to baselines.Finally,a FastText-based weighted soft voting ensemble(WSVE)is proposed to boost the model’s performance by combining several other classifiers and assigning weights to individual models according to their individual performances.The proposed WSVE outperformed all baselines as well as FastText alone,with an F1 of 89%,5%higher than FastText alone,and an accuracy of 93%,3%higher than FastText alone.The proposed model better captures the contextual features of the relatively small sample class and aids in the detection of early depression intensity prediction from tweets with impactful performances.
文摘Introduction: The evaluation of academic stress in medical students and residents is a topic of significant interest, given the considerable challenges they face during their learning process with traditional teaching methods. The use of technologies like virtual reality presents an opportunity to enhance their skills through simulations and training. The main objective of this study is to qualitatively assess the stress levels experienced by medical students and residents by integrating virtual reality into their current learning methods, aiming to improve their ability to manage stressors in their practice. Material and Methods: A questionnaire was conducted with 12 medical students and 12 Traumatology and orthopedics residents. The purpose of the questionnaire was to evaluate the levels of academic stress using the SISCO inventory. The stress levels were calculated by transforming average values into percentages, and the following criteria were assigned: 0 to 33% for Mild Stress, 34 to 66% for Moderate Stress, and 67 to 100% for Deep Stress. Then, a virtual reality class focused on spine surgery was provided. Both medical students and residents were trained using the Non Nocere SharpSurgeon software platform and Oculus Quest 2 virtual reality glasses. At the end of the session, a second questionnaire related to the practice with virtual reality was conducted with the same evaluation criteria and a comparative analysis was carried out. Results: 12 undergraduate students from Hospital Angeles Mexico, CDMX and 12 traumatology and orthopedics residents at Hospital Santa Fe, Bogota were evaluated. The students in CDMX reported an average qualitative stress of 28.50% during habitual practices, which decreased to an average of 14.67% after virtual reality practice. Residents in Bogotá experienced an average qualitative stress of 30.50% with their current learning methods but this reduced to an average of 13.92% after using virtual reality. These findings indicate that the use of virtual reality has a positive impact on reducing stress levels qualitatively. Conclusions: The use of virtual reality as a learning method for medical students and residents qualitatively improves stress levels. Further studies are required to define the potential uses of Virtual Reality to improve learning methods and emotional state in medical students and residents and for a quantitative assessment to validate the training as certified learning methods.
文摘This work presents a study on the behaviors of diatomaceous soils. Although studies are rarely reported on these soils, they have been identified in Mexico City, the Sea of Japan, the northeast coast of Australia,the equatorial Pacific, and the lacustrine deposit of Bogota(Colombia), among other locations. Features of this kind of soil include high friction angle, high initial void ratio, high compressibility index, high liquid limit, and low density. Some of these features are counterintuitive from a classical soil mechanics viewpoint. To understand the geotechnical properties of the diatomaceous soil, a comprehensive experimental plan consisting of more than 2400 tests was performed, including physical tests such as grain size distribution, Atterberg limits, density of solid particles, and organic matter content; and mechanical tests such as oedometric compression tests, unconfined compression tests, and triaxial tests.Laboratory tests were complemented with scanning electron microscope(SEM) observations to evaluate the microstructure of the soil. The test results show that there is an increase in liquid limit with increasing diatomaceous content, and the friction angle also increases with increasing diatomaceous content. In addition, several practical correlations were proposed for this soil type for shear strength mobilization and intrinsic compression line. Finally, useful correlations were presented, such as the relationship between the state consistency and the undrained shear strength, the friction angle and the liquid limit, the void ratio at 100 kPa and the liquid limit, the plasticity index and the diatomaceous content, among others.
文摘The objective of this study is to determine the auditory gain, quality of life, audiological benefits, in bone-anchored hearing device users (BAHA). It is a retrospective and concurrent evaluation of thirty patients fitted unilaterally and seven fitted bilaterally for at least six months. Patients were assessed with audiometric testing and application of Glasgow Benefit Inventory (GBI) and Abbreviated Profile of Hearing Aid Benefit (APHAB). Regarding sound-field pure audiometry results, we found a statistically significant gain in all frequencies using the bone-anchored device. APHAB scores showed statistically significant subjective audiological gains in all subscales except for the aversiveness subscale. GBI mean scores for all items in both groups were all above 3, suggesting quality of life improvement in conductive and mixed hearing loss patients. BP100 users showed a greater clinical gain in the APHAB global score and subscales compared with Divino users. In conclusion the BAHA provides significant auditory gain, subjective audiological benefits and improves quality of life in all BAHA users. This study shows a significant clinical and statistical benefit of BAHA measured by audiometric testing and by the APHAB and GBI questionnaires.

文摘Scope:High-fat diet(HFD)induces imbalance in the small intestine environment,where fat digestion and absorption mainly take place.This study aimed to elucidate the mechanisms by which Lonicera caerulea polyphenols(LCP)might inhibit fat absorption,from the perspective of small intestine microbiota and epithelial barrier integrity.Methods and results:Male Sprague-Dawley rats were given HFD with or without co-administration of LCP for 8 weeks.The results showed that LCP supplementation significantly decreased the levels of serum triglycerides(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C),and increased the contents of fecal sterols,in HFD rats.LCP also inhibited the dysfunction of the small intestine epithelial barrier,via alleviating the oxidative stress activated by Nrf2-ARE pathway,and by modulating the expressions of pro-inflammatory factors such as tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),cyclooxygenase-2(COX-2),nuclear factor kappa-B p65(NF-κB p65)and inducible nitric oxide synthase(iNOS)in the small intestine.Additionally,LCP administration restored the balance in small intestine microbiota and increased the abundance of the specific bacteria,such as Lactobacillus,involved in fat absorption.Conclusion:Our results demonstrated that LCP may be beneficial to inhibit fat absorption.The mechanism seems to be associated with the protection of the epithelial barrier integrity and the modulation of specific bacteria in the small intestine.

文摘This work is focused on the determination of the optimal reaction conditions to synthesize the ionic liquid 1-ethyl-3-methylimidazolium chloride([EMIM][Cl]) and assess its suitability for the pretreatment of rice husk. The modified UNIFAC(UNIversal quasi-chemical Functional-group Activity Coefficients)approach for ionic liquids is used to develop a thermodynamic model that describes the reactive system methylimidazole(MIM), chloroethane(C_(2)H_(5)Cl) and [EMIM][Cl]. The model allows to study the phase equilibria coexistence(vapor–liquid equilibria and solid–liquid equilibria) and yields the theoretically optimal conditions to synthesize the ionic liquid. The model predictions are validated with the available experimental and reported data. By implementing the developed model, a simple way to synthesize ionic liquid [EMIM][Cl] was found allowing to study its influence on the structure and morphology of pretreated rice husk. The lignocellulosic materials involved in this study are characterized by their composition, enzymatic digestibility, scanning electron microscopy, and crystallinity. Compared to untreated material, [EMIM][Cl]-pretreated rice husk produces cellulose that can be efficiently enzymatic hydrolyzed with high sugar yields. This work offers a suitable methodology to include the synthesis and thermodynamics of the solvent media within the design of low-cost ionic liquids for lignocellulosic biomass pretreatment.
文摘Incidence of neuroendocrine tumors (NET) has significantly increased in the past three decades. In the small intestine, NET are the most frequent tumors, even more frequent than adenocarcinomas. Due to atypical presentations and late symptoms, NET in the small intestine frequently represent a diagnostic challenge. It is important to take these tumors into consideration in differential diagnosis of gastrointestinal neoplasms. Surgeons, oncologists, endocrinologists, and gastroenterologists should understand the disease characteristics and management alternatives. This document aims to review the key points of NET and main diagnostic tools. We present the case of a 50-year-old male who presented lower gastrointestinal bleeding. Imaging and endoscopic studies showed no conclusive findings. A capsule endoscopy showed multiple ulcered lesions with neoplastic aspect in the distal jejune. Due to the multifocal nature of the lesions, clinicians suspected NET-associated digestive bleeding. The patient underwent exploratory laparoscopy with ileectomy and radical abdominal lymphadenectomy. Histopathologic examination confirmed the suspected diagnosis of NET. This case reflects the complexity of diagnostic approach and differential diagnoses for these tumors.

文摘The material associated with tree pruning in a city in the Colombian neotropics was characterized in order to determine its energy potential.The species studied for their relevance in the territory were Prosopis juliflora,Licania tomentosa,Terminalia catappa,Azadirachta indica,Pithecellobium dulce,Ficus benjamina and Leucaena leucocephala.Moisture content,bulk density,hygroscopic response,elemental chemical analysis,proximal analysis,calorific value,ease of ignition and combustion,thermogravimetric and heat flow analysis,steam generation capacity,and qualitative analysis of gases(mass spectrometry)were determined.The results that were obtained show high initial moisture contents that vary between 37%and 67%and a variable density(when dry)between 0.21 and 0.41 g/cm3.Chemically,it was shown that all residues have lower carbon and sulfur content compared to a reference mineral coal.However,the residues of some species have higher nitrogen contents compared to the same coal.All biomasses are characterized by their high content of volatile fractions and by having a lower content of inorganic matter compared to carbon.The lower calorific values of these residues are between 14170 and 16928 kJ/kg,which are not negligible compared to other biomasses.This characteristic would be related to the high presence of hemicellulose in the residues.Flue gas monitoring revealed that there are different airflow needs in order to ensure complete combustion.Steam generation tests showed that the calorific value should not be the most relevant criterion to establish the potential use of the waste,since leucaena leucocephala,despite having the highest calorific value,was the material with the lowest performance in steam generation.
文摘BACKGROUND Despite various therapies to treat sepsis,it is one of the leading causes of mortality in the intensive care unit patients globally.Knowledge about the pathophysiology of sepsis has sparked interest in extracorporeal therapies(ECT)which are intended to balance the dysregulation of the immune system by removing excessive levels of inflammatory mediators.AIM To review recent data on the use of ECT in sepsis and to assess their effects on various inflammatory and clinical outcomes.METHODS In this review,an extensive English literature search was conducted from the last two decades to identify the use of ECT in sepsis.A total of 68 articles from peer-reviewed and indexed journals were selected excluding publications with only abstracts.RESULTS Results showed that ECT techniques such as high-volume hemofiltration,coupled plasma adsorption/filtration,resin or polymer adsorbers,and CytoSorb®are emerging as adjunct therapies to improve hemodynamic stability in sepsis.CytoSorb®has the most published data in regard to the use in the field of septic shock with reports on improved survival rates and lowered sequential organ failure assessment scores,lactate levels,total leucocyte count,platelet count,interleukin-IL-6,IL-10,and TNF levels.CONCLUSION Clinical acceptance of ECT in sepsis and septic shock is currently still limited due to a lack of large random clinical trials.In addition to patient-tailored therapies,future research developments with therapies targeting the cellular level of the immune response are expected.
文摘Introduction: Metabolic acidosis (MA) is a frequent alteration in chronic kidney disease (CKD) that is associated with numerous complications, which is why its correction is recommended. Oral sodium bicarbonate is currently the treatment of choice. Objective: The objective is to determine if venous bicarbonate is equal to arterial bicarbonate in the follow-up of a patient with chronic kidney disease. Materials Methods: Single-center Cross-sectional studies in a cohort of adult patients with stage 4 - 5 CKD. Samples were taken between January 2022 and January 2023, in a Clinic in the city of Ibague/ Colombia obtained from the radial artery. The inclusion criteria were: not being treated with alkaline at the time of inclusion. Results: A total of 71 patients were included, 73.2% male (52) and 26.8% female (19), with different stages: stage 3 with 5.6% (4), stage 4 with 60.6% (43), stage 5 with 33.8% (23). 66.2% were diabetic, 88.7% had arterial hypertension, and 15.5% of the patients presented hematoma as a complication and pain associated with arterial puncture. The result of mean venous bicarbonate was 18.8 with a standard deviation of 2.3, arterial bicarbonate a mean of 19.4 with a standard deviation of 2.1 with a value of P 0.46, venous pH with a mean of 7.37 with a standard deviation of 0.48 and a mean arterial pH of 7.38 with a standard deviation of 0.48 with a P value of 0.01. Values of venous bicarbonate compared to arterial bicarbonate showed no statistically significant difference in patients with chronic kidney disease, but there were more complications such as hematoma and pain in patients in the arterial puncture cohort, because of this result venous bicarbonate corresponds to arterial bicarbonate, but has less risk of complications associated with the procedure. Conclusion: Metabolic acidosis is a frequent alteration in advanced chronic kidney disease, these results showed that the values of arterial and venous bicarbonate have no statistically significant differences, but there is a greater risk of complications with arterial blood gases, due to this, venous bicarbonate could be a useful tool for patients with chronic kidney disease.
文摘The twenty-first century is one of the most complex in the history of humanity,mainly due to the ecological crisis it is going through.The construction sector generates about 40%of CO2 emissions into the environment;the foregoing should motivate this sector to seek new alternatives to develop new building practices.Taking these current needs into account,this document classifies and presents a multidisciplinary solution that integrates biology,engineering and architecture to develop a new and innovative lightweight timber structure;it divides with a main structure made of timber and an innovative joint system made of bio-polymers connecting all the panels.Through the study of diatoms,it was able to analyze the bio-morphology of the structure,joints and in particular the geometry since they were the inspiration for the design of this structure that presents an innovative and novel design of structural optimization.Through parametric design and digital fabrication,it was able to create a complex geometry that obtains excellent structural behavior.This research discusses and explores how materials,geometry led to the optimization of a structure and how new structures can arise,thanks to biology new solutions can be obtained that are completely sustainable,being a clear example of how to combat the effects of the climate change and in a precise way it highlights the advantages of the bio-design in the architectural design.
文摘An innovative inversion code, named “Curupira v1.0”, has been developed using Matlab to determine the vertical distribution of resistivity beneath the subsoil. The program integrates Vertical Electrical Sounding (VES), successful in shallow subsurface exploration and Time Domain Electromagnetic (TEM) techniques, better suited for deeper exploration, both of which are widely employed in geophysical exploration. These methodologies involve calculating subsurface resistivity through appropriate inversion processes. To address the ill-posed nature of inverse problems in geophysics, a joint inversion scheme combining VES and TEM data has been incorporated into Curupira v1.0. The software has been tested on both synthetic and real-world data, the latter of which was acquired from the Parana sedimentary basin which we summarise here. The results indicate that the joint inversion of VES and TEM techniques offers improved recovery of simulated models and demonstrates significant potential for hydrogeological studies.

文摘AIM:To assess the significance of chromosome translocation t(11;18)(q21;q21),B-cell lymphoma 10 (BCL-10) protein and Helicobacter pylori (H.pylori) infection in gastric mucosa-associated lymphoid tissue (MALT) lymphoma in Colombia.METHODS:Fifty cases of gastric MALT lymphoma and their respective post-treatment follow-up biopsies were examined to assess the presence of the translocation t(11;18)(q21;q21) as identified by fluorescence in situ hybridization;to detect protein expression patterns of BCL10 using immunohistochemistry;and for evaluation of tumor histology to determine the correlation of these factors and resistance to H.pylori eradication.RESULTS:Infection with H.pylori was confirmed in all cases of gastric MALT lymphoma in association with chronic gastritis.Bacterial eradication led to tumor regression in 66% of cases.The translocation t(11;18)(q21;q21) was not present in any of these cases,nor was there evidence of tumor transformation to diffuse large B-cell lymphoma.Thirty-four percent of the patients showed resistance to tumor regression,and within this group,7 cases,representing 14% of all those analyzed,were considered to be t(11;18)(q21;q21)-positive gastric MALT lymphomas.Protein expression of BCL10 in the nucleus was associated with the presence of translocation and treatment resistance.Cases that were considered unresponsive to therapy were histologically characterized by the presence of homogeneous tumor cells and a lack of plasmacytic differentiation.Responder cases exhibited higher cellular heterogeneity and a greater frequency of plasma cells.CONCLUSION:Both t(11;18)(q21;q21)-positive MALT lymphoma cases and those with nuclear BCL10 expression are considered resistant to H.pylori eradication.It is suggested that chronic antigenic stimulation is not a dominant event in resistant cases.
文摘Eosinophilic esophagitis is an immune-allergic pathology of multifactorial etiology(genetic and environmental)that affects both pediatric and adult patients.Its symptoms,which include heartburn,regurgitation,and esophageal stenosis(with dysphagia being more frequent in eosinophilic esophagitis in young adults and children),are similar to those of gastroesophageal reflux disease,causing delays in diagnosis and treatment.Although endoscopic findings such as furrows,esophageal mucosa trachealization,and whitish exudates may suggest its presence,this diagnosis should be confirmed histologically based on the presence of more than 15 eosinophils per high-power field and the exclusion of other causes of eosinophilia(parasitic infections,hypereosinophilic syndrome,inflammatory bowel disease,among others)for which treatment could be initiated.Currently,the 3“D”s(“Drugs,Diet,and Dilation”)are considered the fundamental components of treatment.The first 2 components,which involve the use of proton pump inhibitors,corticosteroids,immunosuppressants and empirical diets or guided food elimination based on allergy tests,are more useful in the initial phases,whereas endoscopic dilation is reserved for esophageal strictures.Herein,the most important aspects of eosinophilic esophagitis pathophysiology will be reviewed,in addition to evidence for the various treatments.
