目的探讨左西孟旦治疗肺动脉高压老年患者的临床疗效及安全性。方法选取2019年6月~2020年5月于滨州市人民医院住院的肺功脉高压老年患者126例作为研究对象,根据治疗方案不同将其分为对照组(69例)和试验组(57例)。对照组患者给予吸氧,常...目的探讨左西孟旦治疗肺动脉高压老年患者的临床疗效及安全性。方法选取2019年6月~2020年5月于滨州市人民医院住院的肺功脉高压老年患者126例作为研究对象,根据治疗方案不同将其分为对照组(69例)和试验组(57例)。对照组患者给予吸氧,常规治疗肺部原发疾病;试验组患者在对照组治疗基础上,加用左西孟旦治疗。比较两组患者的近期疗效、生活质量、远期疗效和不良反应发生情况。结果治疗1周后,两组患者的下肢周径、肺动脉压、脑钠肽(brain natriuretic peptide,BNP)均显著低于本组治疗前(均P<0.05),24h尿量、动脉血氧分压(arterial partial pressure of oxygen,PaO2)均显著高于本组治疗前(均P<0.05);试验组患者的下肢周径、肺动脉压、BNP均显著低于对照组(均P<0.05),24h尿量、PaO2均显著高于对照组(均P<0.05)。出院后1个月,两组患者的6min步行距离均显著长于本组治疗前(均P<0.05),明尼苏达心力衰竭生活质量问卷(Minnesota living with heart failure questionnaire,MLHFQ)评分均显著低于本组治疗前(均P<0.05);试验组患者的6min步行距离显著长于对照组(P<0.05),MLHFQ评分显著低于对照组(P<0.05)。试验组患者半年内再次入院率显著低于对照组(χ^(2)=6.100,P=0.014)。试验组患者主要不良反应为低血压(5.3%,3/57)和心律失常(3.5%,2/57),两组患者不良事件发生率比较差异无统计学意义(χ^(2)=3.704,P=0.054)。结论左西孟旦治疗由肺部疾病和低氧血症导致的肺动脉高压老年患者,可有效改善患者的临床症状,降低肺动脉压,提高生活质量,且不良反应发生率低,安全性高。展开更多

Twenty-six resected specimens were studied by hematoxylin an...A combined histopathological, mucin histochemi-cal and immunohistochemical study of the transitional mucosa (TM) adjacent to colorectal cancer is presented. Twenty-six resected specimens were studied by hematoxylin and eosin (HE) and high iron diamine-alcian blue (HID-AB). Carcinoem-bryonic antigen (CEA) was demonstrated by peroxi-dase antiperoxidass (PAP) technique. The appearance of the TM is usually thicker, longer and dilated crypts with increased immature and intermediate cells. Variable amount of sialomucins and decrease sulphomucins content as well as increased CEA content are found in the TM. These changes are not seen in non-transitional zone and normal colorectal mucosa. It is suggested that the mucin changes and expression of CEA in the TM may indicate an early primary premalignant changes and may be one of the reasons for the TM affecting the prognosis of patients with large bowel cancer after radical resection.展开更多

A MEMS electrostatic kinetic energy harvester(e-KEH)of about 1 cm2,working at ultralow frequency(1–20 Hz),without any supported additional mass on its mobile electrode,and working even without a vacuum environment is...A MEMS electrostatic kinetic energy harvester(e-KEH)of about 1 cm2,working at ultralow frequency(1–20 Hz),without any supported additional mass on its mobile electrode,and working even without a vacuum environment is reported.The prototype is especially suitable for environments with abundant low frequency motions such as wearable electronics.The proposed e-KEH consists of a capacitor with a finger-teeth interdigited comb structure.This greatly reduces the air damping effect,and thus the capacitance variation remains important regardless of the presence of air.With the new design,the energy transduced per cycle of excitation is no less than 33 times higher than the classic design within 10–40 Hz/2 g_(peak),while is 85 times higher at 15 Hz/2 g_(peak).An enclosed miniature ball combined with non-linear stoppers enables the oscillation of the movable electrode through impact-based frequency up-conversion mechanism,which is also improved by the low air damping.Thanks to this new design,a higher efficiency than the classic gap-closing comb structure is obtained,as a larger range of working frequency(1–180 Hz)in air.A maximum energy conversion of 450 nJ/cycle is obtained with a bias voltage of 45 V and an acceleration of 11 Hz,3 g_(peak).Working with a diode AC-DC rectifier,the proposed KEH is able to support up to 3 RFID communications within 16 s while operated at 11 Hz,3 g_(peak).展开更多
Haptic feedback is typically missing during telemanipulation of surgical robots in minimally invasive surgeries,i.e.,surgeons cannot feel the interaction forces between the instruments and tissues.Instead,surgeons hav...Haptic feedback is typically missing during telemanipulation of surgical robots in minimally invasive surgeries,i.e.,surgeons cannot feel the interaction forces between the instruments and tissues.Instead,surgeons have to solely rely on visual feedback,which increases complexity of guiding the instruments and poses the safety threat of unperceivable contacts outside the field of view.We propose a novel series elastic actuation design for articulated robotic endoscopes to overcome these limitations and evaluate an according device with one joint.Similar to the actuation of human fingers,the joint is driven by antagonistic tendons.Springs are integrated in the transmission between the tendons and the motors outside of the endoscope shaft.We estimated the joint angle and thereby the endoscope shape,measured spring deflection,estimated tendon forces from that deflection,and implemented force control for the endoscope joint.Zero torque control and impedance control were evaluated under application of both a continuous force and an impact force to the endoscope tip.The springs reduced impact forces at the tip of the endoscope through their inherent compliance.At the same time,feeding back the estimated force resulted in a stable tendon force control and a tunable endoscope joint control:Zero torque control effectively reduced the external forces,while the endoscope joint showed the expected stiffness in impedance control.These results show that antagonistic series elastic actuation is a promising concept for endoscope joint actuation and that it can lead towards safer robot–tissue interactions in surgical robotics.展开更多
