Objective Allocation of human resources to address inequalities in the public health system has increasingly attracted societal and political attention.Using the Centers for Disease Control and Prevention(CDCs)system ...Objective Allocation of human resources to address inequalities in the public health system has increasingly attracted societal and political attention.Using the Centers for Disease Control and Prevention(CDCs)system of China as an example,we evaluated inequality in the public health workforce distribution across different regions in China between 2008 and 2017,with the aim of providing information for policymakers to support resource allocation and address growing health inequities.Methods We used three standard public health workforce inequality indices-Gini coefficient,Theil L,and Theil T-and spatial autocorrelation analysis to explore spatial clusters of the workforce in different provinces,visualized with geographical tools.Results The aggregate workforce-to-population ratio decreased from 1.47 to 1.42 per 10,000 population from 2008 to 2017,and was consistently lower than the National Health Commission’s(NHC)recommended critical shortage threshold of 1.75.The workforce distribution inequality indices varied by regional socioeconomic and health system development.Geographic clustering of CDCs workforce distribution was evident,with H–H and L–L clusters in western China and the Guangdong-Fujian region,respectively.Conclusions Our study addressed key issues for government and policymakers in allocation of public health human resources.There is an urgent need for careful identification of analytic questions that will help carry out public health functions in the new era,alongside policy implications for an equitable distribution of the public health workforce focusing on the western region and low–low cluster areas.展开更多
This study aims to evaluate the biosafety awareness of laboratory staff working on pathogens detection in seven Centers for Disease Control and Prevention and support these staff's biosafety management and trainin...This study aims to evaluate the biosafety awareness of laboratory staff working on pathogens detection in seven Centers for Disease Control and Prevention and support these staff's biosafety management and training.A total of 208 laboratory staff from seven provincial CDCs were chosen by stratified random sampling to undergo a self-administered questionnaire survey.The collected data were analyzed using SPSS 22.0.The overall average biosafety awareness of the CDC laboratory staff involved in pathogen detection was 82.5 points.The average biosafety awareness score was the highest in health monitoring(92.63 points)and the lowest in risk assessment and control(41.6 points).Among the seven provincial CDCs,the Guizhou Provincial Center for Disease Control and Prevention laboratory staff showed the lowest average biosafety awareness score(74.3 points).The team who worked for 5–14 years were more likely to score above the passing score(≥88 points;corrected OR 0.400,95%CI:0.168–0.951)than the staff with less than five years of work experience.In addition,the mid-level and junior-level staff,as well as the lower position ones were more likely to score below the passing score(<88 points;junior level and lower:corrected OR 3.473,95%CI:1.028–11.737;mid-level:corrected OR 2.797,95%CI:1.027–7.618)compared with the senior-level staff.Among the surveyed team,66.3%identified a lack of specific funds related to work,61.5%identified a lack of designated personnel for the laboratory biosafety management.The biosafety awareness in CDC laboratory staff involved in pathogen detection is low,especially regarding risk assessment and control.The biosafety awareness in Guizhou and Guangxi Provincial CDC laboratory staff is also low.Laboratory funding,job title,and years of experience in a laboratory influence biosafety awareness in CDC laboratory staff.The biosafety knowledge,education,and training of CDC laboratory staff involved in pathogen detection need to improve by paying attention to the content and coverage of biosafety training,exploring new training modalities,and increasing funding for activities related to biosafety in CDC.展开更多
Objective Keshan disease(KD)is a myocardial mitochondrial disease closely related to insufficient selenium(Se)and protein intake.PTEN induced putative kinase 1(PINK1)/Parkin mediated mitochondrial autophagy regulates ...Objective Keshan disease(KD)is a myocardial mitochondrial disease closely related to insufficient selenium(Se)and protein intake.PTEN induced putative kinase 1(PINK1)/Parkin mediated mitochondrial autophagy regulates various physiological and pathological processes in the body.This study aimed to elucidate the relationship between PINK1/Parkin-regulated mitochondrial autophagy and KD-related myocardial injury.Methods A low Se and low protein animal model was established.One hundred Wistar rats were randomly divided into 5 groups(control group,low Se group,low protein group,low Se+low protein group,and corn from KD area group).The JC-1 method was used to detect the mitochondrial membrane potential(MMP).ELISA was used to detect serum creatine kinase MB(CK-MB),cardiac troponin I(cTnI),and mitochondrial-glutamicoxalacetic transaminase(M-GOT)levels.RT-PCR and Western blot analysis were used to detect the expression of PINK1,Parkin,sequestome 1(P62),and microtubule-associated proteins1A/1B light chain 3B(MAP1LC3B).Results The MMP was significantly decreased and the activity of CK-MB,cTnI,and M-GOT significantly increased in each experimental group(low Se group,low protein group,low Se+low protein group and corn from KD area group)compared with the control group(P<0.05 for all).The mRNA and protein expression levels of PINK1,Parkin and MAP1LC3B were profoundly increased,and those of P62 markedly decreased in the experimental groups compared with the control group(P<0.05 for all).Conclusion Low Se and low protein levels exacerbate myocardial damage in KD by affecting the PINK1/Parkin-mediated mitochondrial autophagy pathway.展开更多

We searched Pub Med, Scopus...AIM: To determine the efficacy of calcium supplementation in reducing the recurrence of colorectal adenomas.METHODS: We conducted a systematic review and meta-analysis of published studies. We searched Pub Med, Scopus, the Cochrane Library, the WHO International Clinical Trials Registry Platform, and the Clinical Trials.gov website, through December 2015. Randomized, placebo-controlled trials assessing supplemental calcium intake for the prevention of recurrence of adenomas were eligible for inclusion. Two reviewers independently selected studies based on predefined criteria, extracted data and outcomes(recurrence of colorectal adenomas, and advanced or "high-risk" adenomas), and rated each trial's riskof-bias. Between-study heterogeneity was assessed, and pooled risk ratio(RR) estimates with their 95% confidence intervals(95%CI) were calculated using fixed- and random-effects models. To express the treatment effect in clinical terms, we calculated the number needed to treat(NNT) to prevent one adenoma recurrence. We also assessed the quality of evidence using GRADE.RESULTS: Four randomized, placebo-controlled trials met the eligibility criteria and were included. Daily doses of elemental calcium ranged from 1200 to 2000 mg, while the duration of treatment and follow-up of participants ranged from 36 to 60 mo. Synthesis of intention-to-treat data, for participants who had undergone follow-up colonoscopies, indicated a modest protective effect of calcium in prevention of adenomas(fixed-effects, RR = 0.89, 95%CI: 0.82-0.96; randomeffects, RR = 0.87, 95%CI: 0.77-0.98; high quality of evidence). The NNT was 20(95%CI: 12-61) to prevent one colorectal adenoma recurrence within a period of 3 to 5 years. On the other hand, the association between calcium treatment and advanced adenomas did not reach statistical significance(fixed-effects, RR = 0.92, 95%CI: 0.75-1.13; random-effects, RR = 0.92, 95%CI: 0.71-1.18; moderate quality of evidence). CONCLUSION: Our results suggest a modest chemopreventive effect of calcium supplements against recurrent colorectal adenomas over a period of 36 to 60 mo. Further research is warranted.展开更多
Objective This study aimed to evaluate the effects of a mindfulness-based psychosomatic intervention on depression,anxiety,fear of childbirth(FOC),and life satisfaction of pregnant women in China.Methods Women experie...Objective This study aimed to evaluate the effects of a mindfulness-based psychosomatic intervention on depression,anxiety,fear of childbirth(FOC),and life satisfaction of pregnant women in China.Methods Women experiencing first-time pregnancy(n=104)were randomly allocated to the intervention group or a parallel active control group.We collected data at baseline(T0),postintervention(T1),3 days after delivery(T2),and 42 days after delivery(T3).The participants completed questionnaires for the assessment of the levels of depression,anxiety,FOC,life satisfaction,and mindfulness.Differences between the two groups and changes within the same group were analyzed at four time points using repeated-measures analysis of variance.Results Compared with the active control group,the intervention group reported lower depression levels at T2(P=0.038)and T3(P=0.013);reduced anxiety at T1(P=0.001)and T2(P=0.003);reduced FOC at T1(P<0.001)and T2(P=0.04);increased life satisfaction at T1(P<0.001)and T3(P=0.015);and increased mindfulness at T1(P=0.01)and T2(P=0.006).Conclusion The mindfulness-based psychosomatic intervention effectively increased life satisfaction and reduced perinatal depression,anxiety,and FOC.展开更多
Objective To investigate the spatial patterns of the prevalence,awareness,treatment,and control rates of dyslipidemia at the provincial level in China.Methods A national and provincial representative cross-sectional s...Objective To investigate the spatial patterns of the prevalence,awareness,treatment,and control rates of dyslipidemia at the provincial level in China.Methods A national and provincial representative cross-sectional survey was conducted among 178,558 Chinese adults in 31 provinces in China's Mainland in 2018–2019,using a multi-stage,stratified,cluster-randomized sampling design.Subjects,as households,were selected,followed by a home visit to collect information.Both descriptive and linear regression procedures were applied in the analyses.Results The overall prevalence of dyslipidemia was 35.6%,and wide geographic variations of prevalence,treatment,and control rates of dyslipidemia were identified among 178,558 eligible participants with a mean age of 55.1±13.8 years.The highest-lowest difference regarding the provincial level prevalence rates were 19.7%vs.2.1%for high low-density lipoprotein cholesterol,16.7%vs.2.5%for high total cholesterol,35.9%vs.5.4%for high triglycerides,and 31.4%vs.10.5%for low high-density lipoprotein cholesterol.The treatment rate of dyslipidemia was correlated with the socio-demographic index(P<0.001),urbanization rate(P=0.01),and affordable basic technologies and essential medicines(P<0.001).Conclusion Prevailing dyslipidemia among the Chinese population and its wide geographic variations in prevalence,treatment,and control suggest that China needs both integrated and localized public health strategies across provinces to improve lipid management.展开更多
The aim of this study was to explore the associations of moderate-to-vigorous-intensity physical activity(MVPA)time and sedentary(SED)time with a history of cardiovascular disease(CVD)and multifactorial(i.e.,blood pre...The aim of this study was to explore the associations of moderate-to-vigorous-intensity physical activity(MVPA)time and sedentary(SED)time with a history of cardiovascular disease(CVD)and multifactorial(i.e.,blood pressure(BP),body mass index(BMI),low-density lipoprotein cholesterol(LDL-C),and glycated hemoglobin A1c(HbA1c))control status among type 2 diabetes mellitus(T2DM)patients in China.A cross-sectional analysis of 9152 people with type 2 diabetes from the Multifactorial Intervention on Type 2 Diabetes(MIDiab)study was performed.Patients were grouped according to their self-reported MVPA time(low,<150 min·week−1;moderate,150 to<450 min·week−1;high,≥450 min·week−1)and SED time(low,<4 h·d–1;moderate,4 to<8 h·d–1;high,≥8 h·d–1).Participants who self-reported a history of CVD were identified as having a CVD risk.Odds ratios(ORs)and 95%confidence intervals(CIs)of CVD risk and multifactorial control status associated with MVPA time and SED time were estimated using mixed-effect logistic regression models,adjusting for China’s geographical region characteristics.The participants had a mean±standard deviation(SD)age of(60.87±8.44)years,44.5%were women,and 25.1%had CVD.After adjustment for potential confounding factors,an inverse association between high MVPA time and CVD risk that was independent of SED time was found,whereas this association was not observed in the moderate-MVPA group.A higher MVPA time was more likely to have a positive effect on the control of BMI.Compared with the reference group(i.e.,those with MVPA time≥450 min·week−1 and SED time<4 h·d–1),CVD risk was higher in the low-MVPA group:The OR associated with an SED time<4 h·d–1 was 1.270(95%CI,1.040–1.553)and that associated with an SED time≥8 h·d–1 was 1.499(95%CI,1.149–1.955).We found that a high MVPA time(i.e.,≥450 min·week−1)was associated with lower odds of CVD risk regardless of SED time among patients with T2DM.展开更多

We have investigated infection and co-infection of the two diseases in the population of forest areas of eight provinces in China by measuring seroprevalence of antibodies against B. burgdorferi and A. phagocytophilum. Methods Forest areas in 8 provinces were chosen for investigation using whole sampling and questionnaire survey methods. 3 669 serum samples from people in the forest areas were tested for the presence of antibodies by indirect immunofluorescent assay (IFA). Results Seroprevalence against B. burgdorferi was 3% to 15% and against A. phagocytophilum was 2% to 18% in the study sites in the 8 provinces in China. We also found co-infection of B. burgdorferi and A. phagocytophilum in 7 of the 8 provinces (the exception being the Miyun area in Beijing). The seroprevalence for both B. burgdorferi and A. phagocytophilum was significantly higher among people exposed to ticks than among people who were not exposed to ticks. Conclusion We conclude that both pathogens are endemic in the forest areas in the eight provinces, but the prevalence of B. burgdorferi and A. phagocytophilum differs between the provinces.展开更多

Methods A systematic review was performed to identify potential studies by searching the electronic databases of Pub Med, Cochrane Library, WHO Library, High Wire, CNKI, and other sources. The selection criteria included randomized controlled trials that compared the efficacy of Na Fe EDTA-fortified soy sauce with that of non-fortified soy sauce. Anemia rates and hemoglobin levels were the outcomes of interest. Inclusion decisions, quality assessment, and data extraction were performed by two reviewers independently. A total of 16 studies met the inclusion criteria for anemia rate analysis, of which 12 studies met the inclusion criteria for hemoglobin analysis. All included studies assessed the effect of Na Fe EDTA-fortified soy sauce on anemia rates and hemoglobin concentrations. Results After the intervention, the hemoglobin concentration increased and anemia rates decreased significantly as compared with the non-fortified soy sauce groups. For anemia rates, data from 16 studies could be pooled, and the pooled estimate odds ratio was 0.25(95% CI 0.19-0.35). For hemoglobin concentrations, data from 12 studies could be pooled, and the pooled weighted mean difference was 8.81 g/L(95% CI 5.96-11.67). Conclusion Na Fe EDTA-fortified soy sauce has a positive effect on anemia control and prevention in the at-risk population.展开更多

Objective Despite the remarkable progress in efforts to control disease spread,the nationwide elimination of hepatitis B in China is still hindered by the persistently high rate of hepatitis B virus(HBV)infection in W...Objective Despite the remarkable progress in efforts to control disease spread,the nationwide elimination of hepatitis B in China is still hindered by the persistently high rate of hepatitis B virus(HBV)infection in Western China.This study aimed to evaluate the strategy of hepatitis B prevention and control in Western China and identify potential areas and strategies for improvement.Methods Susceptible population vaccination,health education,professional training of doctors,and other prevention and control measures have been implemented in Wuwei city since 2010.Data were obtained from three representative cross-sectional serosurveys conducted in 2010,2013,and 2015.The serum samples were subjected to enzyme-linked immunosorbent assays to detect the following seromarkers:HBV surface antigen(HBsAg),antibody against hepatitis B surface antigen(anti-HBs),and antibody against hepatitis B core antigen(anti-HBc).Estimates of variance were determined using Taylor series linearization methods.Results The three serosurveys revealed decreases in the prevalence of HBsAg(7.19% in 2010 vs.6.51% in 2013 vs.5.87% in 2015)and anti-HBc positivity(43.89%vs.32.87%vs.28.46%)and an increase in the prevalence of anti-HBs positivity(49.07%vs.53.66%vs.53.72%)over time.From 2010 to 2015,the legally reported incidence of hepatitis B in Wuwei city decreased from 686.53/100,000 to 53.72/100,000.Notably,persistently high HBsAg-positive rates(above 5.40%)were observed among subjects aged 20–69 years old in the three serosurveys;the prevalence of HBsAg was above 1% among children younger than 10 years old.Furthermore,rural subjects had higher rates of HBsAg and anti-HBc positivity than their urban counterparts(6.04%vs.4.83% and 30.26%vs.20.35%,respectively)in 2015 but had a lower rate of anti-HBs positivity(49.68 vs.55.18%).Multivariate regression analysis showed that age,urban and rural areas,and education level were the main factors affecting HBV infection.Conclusion Although vaccine-based prevention and control measures reduced the rate of HBV infection in Wuwei City over time,the hepatitis B infection rate in children younger than 10 years was still higher than the national average level.Therefore,the prevention and control of mother-to-child transmission and the management of the infected should be the focus of future prevention and control work.展开更多

The CIDARS plays an important role in and has been integrated into the routine outbreak monitoring efforts of the Center for Disease Control(CDC) at all levels in China. In the CIDARS, thresholds are determined using the ?Mean+2SD? in the early stage which have limitations. This study compared the performance of optimized thresholds defined using the ?Mean +2SD? method to the performance of 5 novel algorithms to select optimal ?Outbreak Gold Standard(OGS)? and corresponding thresholds for outbreak detection. Data for infectious disease were organized by calendar week and year. The ?Mean+2 SD?, C1, C2, moving average(MA), seasonal model(SM), and cumulative sum(CUSUM) algorithms were applied. Outbreak signals for the predicted value(Px) were calculated using a percentile-based moving window. When the outbreak signals generated by an algorithm were in line with a Px generated outbreak signal for each week, this Px was then defined as the optimized threshold for that algorithm. In this study, six infectious diseases were selected and classified into TYPE A(chickenpox and mumps), TYPE B(influenza and rubella) and TYPE C [hand foot and mouth disease(HFMD) and scarlet fever]. Optimized thresholds for chickenpox(P_(55)), mumps(P_(50)), influenza(P_(40), P_(55), and P_(75)), rubella(P_(45) and P_(75)), HFMD(P_(65) and P_(70)), and scarlet fever(P_(75) and P_(80)) were identified. The C1, C2, CUSUM, SM, and MA algorithms were appropriate for TYPE A. All 6 algorithms were appropriate for TYPE B. C1 and CUSUM algorithms were appropriate for TYPE C. It is critical to incorporate more flexible algorithms as OGS into the CIDRAS and to identify the proper OGS and corresponding recommended optimized threshold by different infectious disease types.展开更多
Objective To study the effectiveness and feasibility of cryogenic disinfectants in different cold scenarios and analyze the key points of on-site cryogenic disinfection.Methods Qingdao and Suifenhe were selected as ap...Objective To study the effectiveness and feasibility of cryogenic disinfectants in different cold scenarios and analyze the key points of on-site cryogenic disinfection.Methods Qingdao and Suifenhe were selected as application sites for the manual or mechanical spraying of cryogenic disinfectants.The same amount of disinfectant(3,000 mg/L)was applied on cold chain food packaging,cold chain containers,transport vehicles,alpine environments,and article surfaces.The killing log value of the cryogenic disinfectant against the indicator microorganisms(Staphylococcus aureus and Escherichia coli)was used to evaluate the on-site disinfection effect.Results When using 3,000 mg/L with an action time of 10 min on the ground in alpine regions,the surface of frozen items,cold-chain containers,and cold chain food packaging in supermarkets,all external surfaces were successfully disinfected,with a pass rate of 100%.The disinfection pass rates for cold chain food packaging and cold chain transport vehicles of centralized supervised warehouses and food processing enterprises were 12.5%(15/120),81.67%(49/60),and 93.33%(14/15),respectively;yet,the surfaces were not fully sprayed.Conclusion Cryogenic disinfectants are effective in disinfecting alpine environments and the outer packaging of frozen items.The application of cryogenic disinfectants should be regulated to ensure that they cover all surfaces of the disinfected object,thus ensuring effective cryogenic disinfection.展开更多
Low birth weight(LBW) and preterm birth(PB) are associated with newborn mortality and diseases in adulthood.We explored factors related to LBW and PB by conducting a population-based case-control study from January 20...Low birth weight(LBW) and preterm birth(PB) are associated with newborn mortality and diseases in adulthood.We explored factors related to LBW and PB by conducting a population-based case-control study from January 2011 to December 2013 in Wuhan,China.A total of 337 LBW newborn babies,472 PB babies,and 708 babies with normal birth weights and born from term pregnancies were included in this study.Information of newborns and their parents was collected by trained investigators using questionnaires and referring to medical records.Univariate and logistic regression analyses with the stepwise selection method were used to determine the associations of related factors with LBW and PB.Results showed that maternal hypertension(OR=6.78,95% CI:2.27–20.29,P=0.001),maternal high-risk pregnancy(OR=1.53,95% CI:1.06–2.21,P=0.022),and maternal fruit intake ≥300 g per day during the first trimester(OR=1.70,95% CI:1.17–2.45,P=0.005) were associated with LBW.BMI ≥24 kg/m^2 of mother prior to delivery(OR=0.48,95% CI:0.32–0.74,P=0.001) and gestation ≥37 weeks(OR=0.01,95% CI:0.00–0.02,P<0.034) were protective factors for LBW.Maternal hypertension(OR=3.36,95% CI:1.26–8.98,P=0.016),maternal high-risk pregnancy(OR=4.38,95% CI:3.26–5.88,P<0.001),maternal meal intake of only twice per day(OR=1.88,95% CI:1.10–3.20,P=0.021),and mother liking food with lots of aginomoto and salt(OR=1.60,95% CI:1.02–2.51,P=0.040) were risk factors for PB.BMI ≥24 kg/m^2 of mother prior to delivery(OR=0.66,95% CI:0.47–0.93,P=0.018),distance of house from road ≥36 meters(OR=0.72,95% CI:0.53– 0.97,P=0.028),and living in rural area(OR= 0.60,95% CI:0.37–0.99,P=0.047) were protective factors for PB.Our study demonstrated some risk factors and protective factors for LBW and PB,and provided valuable information for the prevention of the conditions among newborns.展开更多

文摘Objective Allocation of human resources to address inequalities in the public health system has increasingly attracted societal and political attention.Using the Centers for Disease Control and Prevention(CDCs)system of China as an example,we evaluated inequality in the public health workforce distribution across different regions in China between 2008 and 2017,with the aim of providing information for policymakers to support resource allocation and address growing health inequities.Methods We used three standard public health workforce inequality indices-Gini coefficient,Theil L,and Theil T-and spatial autocorrelation analysis to explore spatial clusters of the workforce in different provinces,visualized with geographical tools.Results The aggregate workforce-to-population ratio decreased from 1.47 to 1.42 per 10,000 population from 2008 to 2017,and was consistently lower than the National Health Commission’s(NHC)recommended critical shortage threshold of 1.75.The workforce distribution inequality indices varied by regional socioeconomic and health system development.Geographic clustering of CDCs workforce distribution was evident,with H–H and L–L clusters in western China and the Guangdong-Fujian region,respectively.Conclusions Our study addressed key issues for government and policymakers in allocation of public health human resources.There is an urgent need for careful identification of analytic questions that will help carry out public health functions in the new era,alongside policy implications for an equitable distribution of the public health workforce focusing on the western region and low–low cluster areas.

文摘This study aims to evaluate the biosafety awareness of laboratory staff working on pathogens detection in seven Centers for Disease Control and Prevention and support these staff's biosafety management and training.A total of 208 laboratory staff from seven provincial CDCs were chosen by stratified random sampling to undergo a self-administered questionnaire survey.The collected data were analyzed using SPSS 22.0.The overall average biosafety awareness of the CDC laboratory staff involved in pathogen detection was 82.5 points.The average biosafety awareness score was the highest in health monitoring(92.63 points)and the lowest in risk assessment and control(41.6 points).Among the seven provincial CDCs,the Guizhou Provincial Center for Disease Control and Prevention laboratory staff showed the lowest average biosafety awareness score(74.3 points).The team who worked for 5–14 years were more likely to score above the passing score(≥88 points;corrected OR 0.400,95%CI:0.168–0.951)than the staff with less than five years of work experience.In addition,the mid-level and junior-level staff,as well as the lower position ones were more likely to score below the passing score(<88 points;junior level and lower:corrected OR 3.473,95%CI:1.028–11.737;mid-level:corrected OR 2.797,95%CI:1.027–7.618)compared with the senior-level staff.Among the surveyed team,66.3%identified a lack of specific funds related to work,61.5%identified a lack of designated personnel for the laboratory biosafety management.The biosafety awareness in CDC laboratory staff involved in pathogen detection is low,especially regarding risk assessment and control.The biosafety awareness in Guizhou and Guangxi Provincial CDC laboratory staff is also low.Laboratory funding,job title,and years of experience in a laboratory influence biosafety awareness in CDC laboratory staff.The biosafety knowledge,education,and training of CDC laboratory staff involved in pathogen detection need to improve by paying attention to the content and coverage of biosafety training,exploring new training modalities,and increasing funding for activities related to biosafety in CDC.

文摘Objective Keshan disease(KD)is a myocardial mitochondrial disease closely related to insufficient selenium(Se)and protein intake.PTEN induced putative kinase 1(PINK1)/Parkin mediated mitochondrial autophagy regulates various physiological and pathological processes in the body.This study aimed to elucidate the relationship between PINK1/Parkin-regulated mitochondrial autophagy and KD-related myocardial injury.Methods A low Se and low protein animal model was established.One hundred Wistar rats were randomly divided into 5 groups(control group,low Se group,low protein group,low Se+low protein group,and corn from KD area group).The JC-1 method was used to detect the mitochondrial membrane potential(MMP).ELISA was used to detect serum creatine kinase MB(CK-MB),cardiac troponin I(cTnI),and mitochondrial-glutamicoxalacetic transaminase(M-GOT)levels.RT-PCR and Western blot analysis were used to detect the expression of PINK1,Parkin,sequestome 1(P62),and microtubule-associated proteins1A/1B light chain 3B(MAP1LC3B).Results The MMP was significantly decreased and the activity of CK-MB,cTnI,and M-GOT significantly increased in each experimental group(low Se group,low protein group,low Se+low protein group and corn from KD area group)compared with the control group(P<0.05 for all).The mRNA and protein expression levels of PINK1,Parkin and MAP1LC3B were profoundly increased,and those of P62 markedly decreased in the experimental groups compared with the control group(P<0.05 for all).Conclusion Low Se and low protein levels exacerbate myocardial damage in KD by affecting the PINK1/Parkin-mediated mitochondrial autophagy pathway.

文摘Forsythia suspensa,belonging to the deciduous shrubs of the Luteaceae family,a traditional Chinese medicine,has effects of alleviating swelling,clearing heat,detoxification and promoting blood circulation.The leaves of F.suspensa contain multiple chemical components and have a long history of use in folk medicines and health foods.The purpose of this study was to explore the effects of forsythin extract from F.suspensa leaves on intestinal microbiota and short-chain fatty acid(SCFA)content in rats with obesity induced by a high-fat diet.Forsythin extract in F.suspensa leaves increased the abundance of the intestinal microbiota,ameliorated intestinal microbiota disorders and inhibited the increase in total SCFA content in the intestinal tract in rats with obesity induced by a high-fat diet.These results suggested that forsythin extract in F.suspensa leaves may slow the development of obesity induced by a high-fat diet;thus,its active components and efficacy are worthy of further study.

文摘AIM: To determine the efficacy of calcium supplementation in reducing the recurrence of colorectal adenomas.METHODS: We conducted a systematic review and meta-analysis of published studies. We searched Pub Med, Scopus, the Cochrane Library, the WHO International Clinical Trials Registry Platform, and the Clinical Trials.gov website, through December 2015. Randomized, placebo-controlled trials assessing supplemental calcium intake for the prevention of recurrence of adenomas were eligible for inclusion. Two reviewers independently selected studies based on predefined criteria, extracted data and outcomes(recurrence of colorectal adenomas, and advanced or "high-risk" adenomas), and rated each trial's riskof-bias. Between-study heterogeneity was assessed, and pooled risk ratio(RR) estimates with their 95% confidence intervals(95%CI) were calculated using fixed- and random-effects models. To express the treatment effect in clinical terms, we calculated the number needed to treat(NNT) to prevent one adenoma recurrence. We also assessed the quality of evidence using GRADE.RESULTS: Four randomized, placebo-controlled trials met the eligibility criteria and were included. Daily doses of elemental calcium ranged from 1200 to 2000 mg, while the duration of treatment and follow-up of participants ranged from 36 to 60 mo. Synthesis of intention-to-treat data, for participants who had undergone follow-up colonoscopies, indicated a modest protective effect of calcium in prevention of adenomas(fixed-effects, RR = 0.89, 95%CI: 0.82-0.96; randomeffects, RR = 0.87, 95%CI: 0.77-0.98; high quality of evidence). The NNT was 20(95%CI: 12-61) to prevent one colorectal adenoma recurrence within a period of 3 to 5 years. On the other hand, the association between calcium treatment and advanced adenomas did not reach statistical significance(fixed-effects, RR = 0.92, 95%CI: 0.75-1.13; random-effects, RR = 0.92, 95%CI: 0.71-1.18; moderate quality of evidence). CONCLUSION: Our results suggest a modest chemopreventive effect of calcium supplements against recurrent colorectal adenomas over a period of 36 to 60 mo. Further research is warranted.

文摘Objective This study aimed to evaluate the effects of a mindfulness-based psychosomatic intervention on depression,anxiety,fear of childbirth(FOC),and life satisfaction of pregnant women in China.Methods Women experiencing first-time pregnancy(n=104)were randomly allocated to the intervention group or a parallel active control group.We collected data at baseline(T0),postintervention(T1),3 days after delivery(T2),and 42 days after delivery(T3).The participants completed questionnaires for the assessment of the levels of depression,anxiety,FOC,life satisfaction,and mindfulness.Differences between the two groups and changes within the same group were analyzed at four time points using repeated-measures analysis of variance.Results Compared with the active control group,the intervention group reported lower depression levels at T2(P=0.038)and T3(P=0.013);reduced anxiety at T1(P=0.001)and T2(P=0.003);reduced FOC at T1(P<0.001)and T2(P=0.04);increased life satisfaction at T1(P<0.001)and T3(P=0.015);and increased mindfulness at T1(P=0.01)and T2(P=0.006).Conclusion The mindfulness-based psychosomatic intervention effectively increased life satisfaction and reduced perinatal depression,anxiety,and FOC.

文摘Objective To investigate the spatial patterns of the prevalence,awareness,treatment,and control rates of dyslipidemia at the provincial level in China.Methods A national and provincial representative cross-sectional survey was conducted among 178,558 Chinese adults in 31 provinces in China's Mainland in 2018–2019,using a multi-stage,stratified,cluster-randomized sampling design.Subjects,as households,were selected,followed by a home visit to collect information.Both descriptive and linear regression procedures were applied in the analyses.Results The overall prevalence of dyslipidemia was 35.6%,and wide geographic variations of prevalence,treatment,and control rates of dyslipidemia were identified among 178,558 eligible participants with a mean age of 55.1±13.8 years.The highest-lowest difference regarding the provincial level prevalence rates were 19.7%vs.2.1%for high low-density lipoprotein cholesterol,16.7%vs.2.5%for high total cholesterol,35.9%vs.5.4%for high triglycerides,and 31.4%vs.10.5%for low high-density lipoprotein cholesterol.The treatment rate of dyslipidemia was correlated with the socio-demographic index(P<0.001),urbanization rate(P=0.01),and affordable basic technologies and essential medicines(P<0.001).Conclusion Prevailing dyslipidemia among the Chinese population and its wide geographic variations in prevalence,treatment,and control suggest that China needs both integrated and localized public health strategies across provinces to improve lipid management.

文摘The aim of this study was to explore the associations of moderate-to-vigorous-intensity physical activity(MVPA)time and sedentary(SED)time with a history of cardiovascular disease(CVD)and multifactorial(i.e.,blood pressure(BP),body mass index(BMI),low-density lipoprotein cholesterol(LDL-C),and glycated hemoglobin A1c(HbA1c))control status among type 2 diabetes mellitus(T2DM)patients in China.A cross-sectional analysis of 9152 people with type 2 diabetes from the Multifactorial Intervention on Type 2 Diabetes(MIDiab)study was performed.Patients were grouped according to their self-reported MVPA time(low,<150 min·week−1;moderate,150 to<450 min·week−1;high,≥450 min·week−1)and SED time(low,<4 h·d–1;moderate,4 to<8 h·d–1;high,≥8 h·d–1).Participants who self-reported a history of CVD were identified as having a CVD risk.Odds ratios(ORs)and 95%confidence intervals(CIs)of CVD risk and multifactorial control status associated with MVPA time and SED time were estimated using mixed-effect logistic regression models,adjusting for China’s geographical region characteristics.The participants had a mean±standard deviation(SD)age of(60.87±8.44)years,44.5%were women,and 25.1%had CVD.After adjustment for potential confounding factors,an inverse association between high MVPA time and CVD risk that was independent of SED time was found,whereas this association was not observed in the moderate-MVPA group.A higher MVPA time was more likely to have a positive effect on the control of BMI.Compared with the reference group(i.e.,those with MVPA time≥450 min·week−1 and SED time<4 h·d–1),CVD risk was higher in the low-MVPA group:The OR associated with an SED time<4 h·d–1 was 1.270(95%CI,1.040–1.553)and that associated with an SED time≥8 h·d–1 was 1.499(95%CI,1.149–1.955).We found that a high MVPA time(i.e.,≥450 min·week−1)was associated with lower odds of CVD risk regardless of SED time among patients with T2DM.

文摘Objective Lyme disease and Human granulocytic anaplasmosis are tick‐borne diseases caused by Borrelia burgdorferi and Anaplasma phagocytophilum respectively. We have investigated infection and co-infection of the two diseases in the population of forest areas of eight provinces in China by measuring seroprevalence of antibodies against B. burgdorferi and A. phagocytophilum. Methods Forest areas in 8 provinces were chosen for investigation using whole sampling and questionnaire survey methods. 3 669 serum samples from people in the forest areas were tested for the presence of antibodies by indirect immunofluorescent assay (IFA). Results Seroprevalence against B. burgdorferi was 3% to 15% and against A. phagocytophilum was 2% to 18% in the study sites in the 8 provinces in China. We also found co-infection of B. burgdorferi and A. phagocytophilum in 7 of the 8 provinces (the exception being the Miyun area in Beijing). The seroprevalence for both B. burgdorferi and A. phagocytophilum was significantly higher among people exposed to ticks than among people who were not exposed to ticks. Conclusion We conclude that both pathogens are endemic in the forest areas in the eight provinces, but the prevalence of B. burgdorferi and A. phagocytophilum differs between the provinces.

文摘This paper first applies the sequential cluster method to set up the classification standard of infectious disease incidence state based on the fact that there are many uncertainty characteristics in the incidence course.Then the paper presents a weighted Markov chain,a method which is used to predict the future incidence state.This method assumes the standardized self-coefficients as weights based on the special characteristics of infectious disease incidence being a dependent stochastic variable.It also analyzes the characteristics of infectious diseases incidence via the Markov chain Monte Carlo method to make the long-term benefit of decision optimal.Our method is successfully validated using existing incidents data of infectious diseases in Jiangsu Province.In summation,this paper proposes ways to improve the accuracy of the weighted Markov chain,specifically in the field of infection epidemiology.

文摘Objective To assess the effect of sodium iron ethylenediaminetetraacetate(Na Fe EDTA)-fortified soy sauce on anemia prevalence in the Chinese population. Methods A systematic review was performed to identify potential studies by searching the electronic databases of Pub Med, Cochrane Library, WHO Library, High Wire, CNKI, and other sources. The selection criteria included randomized controlled trials that compared the efficacy of Na Fe EDTA-fortified soy sauce with that of non-fortified soy sauce. Anemia rates and hemoglobin levels were the outcomes of interest. Inclusion decisions, quality assessment, and data extraction were performed by two reviewers independently. A total of 16 studies met the inclusion criteria for anemia rate analysis, of which 12 studies met the inclusion criteria for hemoglobin analysis. All included studies assessed the effect of Na Fe EDTA-fortified soy sauce on anemia rates and hemoglobin concentrations. Results After the intervention, the hemoglobin concentration increased and anemia rates decreased significantly as compared with the non-fortified soy sauce groups. For anemia rates, data from 16 studies could be pooled, and the pooled estimate odds ratio was 0.25(95% CI 0.19-0.35). For hemoglobin concentrations, data from 12 studies could be pooled, and the pooled weighted mean difference was 8.81 g/L(95% CI 5.96-11.67). Conclusion Na Fe EDTA-fortified soy sauce has a positive effect on anemia control and prevention in the at-risk population.

文摘Objective Despite the remarkable progress in efforts to control disease spread,the nationwide elimination of hepatitis B in China is still hindered by the persistently high rate of hepatitis B virus(HBV)infection in Western China.This study aimed to evaluate the strategy of hepatitis B prevention and control in Western China and identify potential areas and strategies for improvement.Methods Susceptible population vaccination,health education,professional training of doctors,and other prevention and control measures have been implemented in Wuwei city since 2010.Data were obtained from three representative cross-sectional serosurveys conducted in 2010,2013,and 2015.The serum samples were subjected to enzyme-linked immunosorbent assays to detect the following seromarkers:HBV surface antigen(HBsAg),antibody against hepatitis B surface antigen(anti-HBs),and antibody against hepatitis B core antigen(anti-HBc).Estimates of variance were determined using Taylor series linearization methods.Results The three serosurveys revealed decreases in the prevalence of HBsAg(7.19% in 2010 vs.6.51% in 2013 vs.5.87% in 2015)and anti-HBc positivity(43.89%vs.32.87%vs.28.46%)and an increase in the prevalence of anti-HBs positivity(49.07%vs.53.66%vs.53.72%)over time.From 2010 to 2015,the legally reported incidence of hepatitis B in Wuwei city decreased from 686.53/100,000 to 53.72/100,000.Notably,persistently high HBsAg-positive rates(above 5.40%)were observed among subjects aged 20–69 years old in the three serosurveys;the prevalence of HBsAg was above 1% among children younger than 10 years old.Furthermore,rural subjects had higher rates of HBsAg and anti-HBc positivity than their urban counterparts(6.04%vs.4.83% and 30.26%vs.20.35%,respectively)in 2015 but had a lower rate of anti-HBs positivity(49.68 vs.55.18%).Multivariate regression analysis showed that age,urban and rural areas,and education level were the main factors affecting HBV infection.Conclusion Although vaccine-based prevention and control measures reduced the rate of HBV infection in Wuwei City over time,the hepatitis B infection rate in children younger than 10 years was still higher than the national average level.Therefore,the prevention and control of mother-to-child transmission and the management of the infected should be the focus of future prevention and control work.

文摘The China Infectious Disease Automated-alert and Response System(CIDARS) was successfully implemented and became operational nationwide in 2008. The CIDARS plays an important role in and has been integrated into the routine outbreak monitoring efforts of the Center for Disease Control(CDC) at all levels in China. In the CIDARS, thresholds are determined using the ?Mean+2SD? in the early stage which have limitations. This study compared the performance of optimized thresholds defined using the ?Mean +2SD? method to the performance of 5 novel algorithms to select optimal ?Outbreak Gold Standard(OGS)? and corresponding thresholds for outbreak detection. Data for infectious disease were organized by calendar week and year. The ?Mean+2 SD?, C1, C2, moving average(MA), seasonal model(SM), and cumulative sum(CUSUM) algorithms were applied. Outbreak signals for the predicted value(Px) were calculated using a percentile-based moving window. When the outbreak signals generated by an algorithm were in line with a Px generated outbreak signal for each week, this Px was then defined as the optimized threshold for that algorithm. In this study, six infectious diseases were selected and classified into TYPE A(chickenpox and mumps), TYPE B(influenza and rubella) and TYPE C [hand foot and mouth disease(HFMD) and scarlet fever]. Optimized thresholds for chickenpox(P_(55)), mumps(P_(50)), influenza(P_(40), P_(55), and P_(75)), rubella(P_(45) and P_(75)), HFMD(P_(65) and P_(70)), and scarlet fever(P_(75) and P_(80)) were identified. The C1, C2, CUSUM, SM, and MA algorithms were appropriate for TYPE A. All 6 algorithms were appropriate for TYPE B. C1 and CUSUM algorithms were appropriate for TYPE C. It is critical to incorporate more flexible algorithms as OGS into the CIDRAS and to identify the proper OGS and corresponding recommended optimized threshold by different infectious disease types.

文摘Objective To study the effectiveness and feasibility of cryogenic disinfectants in different cold scenarios and analyze the key points of on-site cryogenic disinfection.Methods Qingdao and Suifenhe were selected as application sites for the manual or mechanical spraying of cryogenic disinfectants.The same amount of disinfectant(3,000 mg/L)was applied on cold chain food packaging,cold chain containers,transport vehicles,alpine environments,and article surfaces.The killing log value of the cryogenic disinfectant against the indicator microorganisms(Staphylococcus aureus and Escherichia coli)was used to evaluate the on-site disinfection effect.Results When using 3,000 mg/L with an action time of 10 min on the ground in alpine regions,the surface of frozen items,cold-chain containers,and cold chain food packaging in supermarkets,all external surfaces were successfully disinfected,with a pass rate of 100%.The disinfection pass rates for cold chain food packaging and cold chain transport vehicles of centralized supervised warehouses and food processing enterprises were 12.5%(15/120),81.67%(49/60),and 93.33%(14/15),respectively;yet,the surfaces were not fully sprayed.Conclusion Cryogenic disinfectants are effective in disinfecting alpine environments and the outer packaging of frozen items.The application of cryogenic disinfectants should be regulated to ensure that they cover all surfaces of the disinfected object,thus ensuring effective cryogenic disinfection.

文摘Low birth weight(LBW) and preterm birth(PB) are associated with newborn mortality and diseases in adulthood.We explored factors related to LBW and PB by conducting a population-based case-control study from January 2011 to December 2013 in Wuhan,China.A total of 337 LBW newborn babies,472 PB babies,and 708 babies with normal birth weights and born from term pregnancies were included in this study.Information of newborns and their parents was collected by trained investigators using questionnaires and referring to medical records.Univariate and logistic regression analyses with the stepwise selection method were used to determine the associations of related factors with LBW and PB.Results showed that maternal hypertension(OR=6.78,95% CI:2.27–20.29,P=0.001),maternal high-risk pregnancy(OR=1.53,95% CI:1.06–2.21,P=0.022),and maternal fruit intake ≥300 g per day during the first trimester(OR=1.70,95% CI:1.17–2.45,P=0.005) were associated with LBW.BMI ≥24 kg/m^2 of mother prior to delivery(OR=0.48,95% CI:0.32–0.74,P=0.001) and gestation ≥37 weeks(OR=0.01,95% CI:0.00–0.02,P<0.034) were protective factors for LBW.Maternal hypertension(OR=3.36,95% CI:1.26–8.98,P=0.016),maternal high-risk pregnancy(OR=4.38,95% CI:3.26–5.88,P<0.001),maternal meal intake of only twice per day(OR=1.88,95% CI:1.10–3.20,P=0.021),and mother liking food with lots of aginomoto and salt(OR=1.60,95% CI:1.02–2.51,P=0.040) were risk factors for PB.BMI ≥24 kg/m^2 of mother prior to delivery(OR=0.66,95% CI:0.47–0.93,P=0.018),distance of house from road ≥36 meters(OR=0.72,95% CI:0.53– 0.97,P=0.028),and living in rural area(OR= 0.60,95% CI:0.37–0.99,P=0.047) were protective factors for PB.Our study demonstrated some risk factors and protective factors for LBW and PB,and provided valuable information for the prevention of the conditions among newborns.
