Prediction of SMILE surgical cutting formula based on back propagation neural network

AIM:To predict cutting formula of small incision lenticule extraction(SMILE)surgery and assist clinicians in identifying candidates by deep learning of back propagation(BP)neural network.METHODS:A prediction program was developed by a BP neural network.There were 13188 pieces of data selected as training validation.Another 840 eye samples from 425 patients were recruited for reverse verification of training results.Precision of prediction by BP neural network and lenticule thickness error between machine learning and the actual lenticule thickness in the patient data were measured.RESULTS:After training 2313 epochs,the predictive SMILE cutting formula BP neural network models performed best.The values of mean squared error and gradient are 0.248 and 4.23,respectively.The scatterplot with linear regression analysis showed that the regression coefficient in all samples is 0.99994.The final error accuracy of the BP neural network is-0.003791±0.4221102μm.CONCLUSION:With the help of the BP neural network,the program can calculate the lenticule thickness and residual stromal thickness of SMILE surgery accurately.Combined with corneal parameters and refraction of patients,the program can intelligently and conveniently integrate medical information to identify candidates for SMILE surgery. 展开更多

In the study of the mechanism of wound healing after anal fistula surgery,how to scientifically and efficiently promote wound healing is of great significance.At present,modern medical treatment of wounds after anal fistula surgery mostly focuses on physical therapy intervention,new wound dressing and packing,and external application of growth factors.However,these therapies have many problems,and there is still no consensus on their clinical use.Traditional Chinese Medicine(TCM)has several methods to promote wound healing,such as oral administration,rubbing,and fumigation,which have a long history and obvious efficacy,but research in this area is relatively scattered and lacks classification and summarizing.Therefore,this paper analyzes and summarizes the existing research on TCM for promotion of wound healing after anal fistula surgery,carries out targeted analyses according to different clinical syndromes and treatment methods,and analyzes the defects in current research and anticipates future research trends in order to provide theoretical support for the advantages of TCM in promoting wound healing after anal fistula surgery. 展开更多

The coronavirus disease 2019(COVID-19)pandemic imposed arestructuring of global health systems by rethinking spaces used for the care of these patients and the additions of intensive care,infectious diseases and pneumology departments.This paper provides evidence on the presence of severe acute respiratory syndrome coronavirus 2 in hepatocytes and its direct cytopathic activity,as well as the degree of liver damage due to drug toxicity,inflammation and hypoxia in COVID-19.A review of clinical trials has quantified liver damage through both pathology and biochemistry studies.Additionally,we briefly present the results of a study conducted in our clinic on 849 patients admitted for COVID-19 treatment,of which 31 patients had pre-existing chronic liver disease and 388 patients had values above the normal limit for alanine aminotransferase,aspartate aminotransferase,and total bilirubin.It was observed that patients with abnormal liver tests were significantly statistically older,had more comorbidities and had a higher percentage of unfavourable evolution(death or transfer to intensive care).The conclusion of this paper is that the main causes of liver damage are direct viral aggression,coagulation dysfunction and endothelial damage,and patients with impaired liver function develop more severe forms of COVID-19 which requires special care by a multidisciplinary team that includes a hepatologist. 展开更多

Better Glycaemic Outcome, Low Levels of Acute Severe Complications, and High Patient Satisfaction in Routine Practice in Type 1 Diabetes Treated with an Insulin Pump

Treatment of patients with Type 1 diabetes with continuous subcutaneous insulin infusion (CSII) was re-introduced in Denmark in this millennium and initiated in the diabetes clinic, Fredericia Hospital, in 2005. The aim of the present study was to present yearly data of quality from 2005 to 2013 from a clinical database of patients treated with CSII in routine practice. Methods: A database was established in 2009-2010. Data from 2005 to 2009 are retrospective, data from 2009 to 2013 prospective. From 2009, patient satisfaction was measured by validated questionnaires. Results: By 31 December 2013, the database contained data from 143 active patients. HbA1C (median and range) decreased from 64 (40 - 126) mmol/mol (8.0 (5.8 - 13.7)%) (n = 104) to 60 (36 - 98) mmol/mol (7.6 (5.4 - 11.1)%) (n = 134) (p < 0.001) before and latest year on CSII. The improved glycaemic control was maintained each year until ≥5 years after initiation of CSII (p < 0.01). There was no change in weight. The number of attacks of severe hypoglycaemia was reduced from 60 attacks in 21 patients (n = 104) the year before CSII to only 5 attacks in 5 patients in 2013 (n = 134) (p < 0.01), corresponding to an incidence of 3.7 episodes per 100 patient years. Each year after initiation of CSII until ≥5 years, the number of episodes of severe hypoglycaemia was reduced (p < 0.01). Since 2005, nine episodes of ketoacidosis have been registered in eight patients corresponding to an incidence of 1.4 episodes per 100 patient years. Patient satisfaction was high one year after initiation of CSII (p < 0.01). Conclusion: Change of treatment to CSII is accompanied by an improvement in glycaemic control, very pronounced reduction in severe hypoglycaemia, low levels of diabetic ketoacidosis, and pronounced patient satisfaction. 展开更多

Objective: To evaluate the synergic effects of a novel oral supplement formulation, containing prebiotics,yeast b-glucans, minerals and silymarin(Silybum marianum), on lipid and glycidic metabolism, inflammatory and mitochondrial proteins of the liver, in control and high-fat diet-induced obese mice.Methods: After an acclimation period, 32 male C57 BL/6 mice were divided into the following groups:nonfat diet(NFD) vehicle, NFD supplemented, high-fat diet(HFD) vehicle and HFD supplemented. The vehicle and experimental formulation were administered orally by gavage once a day during the last four weeks of the diet(28 consecutive days). We then evaluated energy homeostasis, inflammation, and mitochondrial protein expression in these groups of mice.Results: After four weeks of supplementation, study groups experienced reduced glycemia, dyslipidemia,fat, and hepatic fibrosis levels. Additionally, proliferator-activated receptor-α, AMP-activated protein kinase-1α, peroxisome proliferator-activated receptor γ co-activator-1α, and mitochondrial transcription factor A expression levels were augmented;however, levels of inhibitor of nuclear factor-κB kinase subunit a and p65 nuclear factor-κB expression, and oxidative markers were reduced. Notably, the cortisol/C-reactive protein ratio, a well-characterized marker of the hypothalamic–pituitary–adrenal axis immune interface status, was found to be modulated by the supplement.Conclusion: We discovered that the novel supplement was able to modify different antioxidant, metabolic and inflammatory pathways, improving the energy homeostasis and inflammatory status, and consequently alleviated hepatic steatosis. 展开更多
