Exceptionally, it can occur in young children making the diagnosi...Trichobezoars are an infrequent form of bezoars formed from ingested hair. They are more common in adolescent females with history of trichotillomania. Exceptionally, it can occur in young children making the diagnosis difficult. We report the case of a 6-year-old girl with a history of abdominal pain, distension, weight loss, and attacks of vomiting. Upper gastrointestinal endoscopy revealed a trichobezoar occupying almost the whole gastric cavity. The gastric bezoar was removed by surgery. Gastric trichobezoar is exceptional in young children and can lead to stunting and gastric outlet obstruction. After definitive surgical or endoscopic treatment, pediatric psychiatric consultation should be opted for any mental disorder and for prevention of further recurrence.展开更多

We report the case of a 35-year-old female with uncontrolled diabetes mellit...Necrotizing fasciitis is an uncommon infection, but potentially lethal, especially when associated with systemic disorders such as diabetes. We report the case of a 35-year-old female with uncontrolled diabetes mellitus, presenting with edema of the neck, facial and left mammary gland, secondary to untreated dental infection, progressing to a full-blown necrotizing fasciitis in a short period of time with sepsis. The patient was managed with aggressive multidisciplinary medical and surgical treatment. Despite the technologic advances in diagnosis and treatment, complications still result with astounding high mortality. Clearly, the morbidity associated to this infection, even in diabetic patients, can be minimized if an early diagnosis and effective debridement are done.展开更多

Th...Introduction: Gastro intestinal stromal tumors are rare which can arise at any age, mostly between 50 and 60 years old. They are characterized by the expression on the surface of their cells, a specific gene c-kit. They can be located in any segment of the digestive tract, with a preference for the stomach (60%). The jejunum location is found in 200% to 30% of the cases. The originality of our observation is the revelation of a stromal jejunum tumor by a digestive bleeding. Case Report: A 71-year-old man was admitted in emergencies for a low digestive plentiful bleeding with state of hypovolemia shock. Result: The operation was performed after stabilization of his dynamic state and blood transfusion and we discovered a jejunal stromal tumor in operation. The follow-up was simple with a good evolution under treatment by Imatinib. Conclusion: Stromal digestive tumors are rare which can interest all the segments of the digestive tract and their diagnosis must be often evoked in front of any digestive bleeding.展开更多

In a country with very limited resources, the prognosis may be favorable in the absence of renal replacement. We report the case of a 63 years old woman, hypertensive and diabetic, admitted to the ICU for unconsciousness. Clinically, neurological examination notes a Glasgow of 8/15 (Y2, V2, M4) and a left hemiplegia. Temperature was 39°C. Diuresis was 100 ml during the first 24 hours. Blood pressure was 90/60 mmHg, tachycardia at 133/min. Cardiac auscultation is normal. Vascular axes were weakly perceptible. Oxygen saturation was 95%. The skin examination notes a dry skin and mucous membranes, a flattening of the superficial veins, sunken eyes and a persistent skin fold. There is no hepatomegaly or splenomegaly, or jaundice. Biological point of view note natremia: 176 mmol/L;osmolarity: 390 mosmol/kg;creatinin: 300 μmol/L;glycemia > 6 g/L;transaminases 217 UI. Diagnostics: malignant hypernatremia with a high plasma osmolarity associated with an acute anuric renal failure, hydro electrolytic disorders, an abnormal liver function, a fever of central origin and a stroke. The treatment consisted of a correction of the electrolyte disorders by infusion of isotonic and hypotonic fluids with insulin. In a country with very limited resources, the severe hypernatremia prognosis associated with anuric acute renal failure may be favorable in the absence of renal replacement.展开更多

Liver and splenic tuberculosis is a classic disease which represents 1.5% of all gastrointestinal tuberculosis. Patients and Methods: We reported the case of a fifty-six years old patient admitted in the department of surgery for acute cholecystitis, and the biological balance showed a hyperleukocytosis at 16,000 elements/mm3 and a CRP at 25 with a light cholestasis. The abdominal scanner showed a micronodular splenomegaly with coelio mesenteric ganglions. We discovered a tuberculosis location in the liver and the spleen and we realized a cholecystectomy with a hepatic biopsy and a splenectomy. Results: A gallbladder ablation associated with an enlarged spleen and liver biopsy was diagnosed with liver and splenic tuberculosis. The patient was put under anti tubercular treatment with the good clinical and radiological evolution. Conclusion: The diagnosis of tuberculosis should always be considered especially in endemic countries.展开更多

Hemodialysis patients are at high risk because of the co-morbidities. We report the clinical and biological characteristics of two patients who developed COVID-19 infection in our dialysis center in Donka National Hospital. None of the patients had contact with an infected person. The age was 38 and 54 years. The symptoms common to both patients were: fatigue, diarrhea, fever. Lymphopenia was present in both patients. None of them had a chest X-ray or chest scan because they were positive for the test before admission.展开更多
