CNS lymphomas are typically high grade, large cell lymphomas;low grade CNS lymphomas are rare. We encountered a case of 48-year old patient with recurrent episodes of seizures, weakness of bilateral extremities and memory loss. On MRI and CT scan, a 2.0 cm area of ill-defined enhancement with associated edema was present in the left frontal subcortex. The biopsy revealed an infiltrate of small lymphocytes expressing pan B-cell markers and an immunoprofile consistent with an extranodal marginal zone B cell lymphomas of mucosa associated lymphoid tissue (MALT). Clonal rearrangement was demonstrated by PCR analysis. Subsequently, the patient was treated with radiation therapy and responded well, with decrease in the size of the lesion. He is alive and well for 1 year and 3 months up to the date of this report.展开更多

The technique can be safely used in humans intraoperatively, in the intensive care unit, and in ambulatory settings. Microdialysis probes, micropumps, and analytical equipment are commercially available and have been used extensively for neurochemical monitoring in traumatic brain injury, stroke, and subarachnoid hemorrhage. There has been very limited use of microdialysis in neuro-oncology, but this technique has great promise in the study of the basic neurochemistry of brain tumors, alterations in neurochemistry in response to therapy, and the pharmacokinetics of chemotherapeutic agents. Microdialysis probes may also be used to deliver drugs while simultaneously permitting monitoring of neurochemical changes induced by this therapy.展开更多
Endometriosis outside the pelvic cavity is rare, but its most common extra-pelvic location is the abdominal wall, occurring most frequently after cesarean delivery. Fistula tracks through these lesions in the abdomina...Endometriosis outside the pelvic cavity is rare, but its most common extra-pelvic location is the abdominal wall, occurring most frequently after cesarean delivery. Fistula tracks through these lesions in the abdominal wall to the uterus are rare. However, a fistula between abdominal wall endometriosis to an endometrioma has not been reported. A 46-year-old multigravid woman presented with complaint of cyclical pain and bleeding from her cesarean delivery scar and pelvic pressure. Computed tomography revealed a complex, 16-cm left adnexal mass. Intraoperatively, a fistulous tract connecting a large endometrioma in the abdominal-pelvic cavity to the skin was noted and wide local excision of this track was performed in conjunction with aggressive surgical management of endometriosis throughout the pelvis. Pathologic analysis of the resected specimen confirmed the diagnosis. Although rare, endometrioma-cutaneous fistula should be included in the differential diagnosis of suspected abdominal wall endometriosis with concurrent adnexal mass for appropriate surgical planning.展开更多
