A pressure controlled mechanical ventilator with an automatic secretion clearance function can improve secretion clearance safely and efficiently.Studies on secretion clearance by pressure controlled systems show that these are suited for clinical applications.However,these studies are based on a single lung electric model and neglect the coupling between the two lungs.The research methods applied are too complex for the analysis of a multi-parameter system.In order to understand the functioning of the human respiratory system,this paper develops a dimensionless mathematical model of doublelung mechanical ventilation system with a secretion clearance function.An experiment is designed to verify the mathematical model through comparison of dimensionless experimental data and dimensionless simulation data.Finally,the coupling between the two lungs is studied,and an orthogonal experiment designed to identify the impact of each parameter on the system. 展开更多

In this paper,a hybrid electronic noses’system(HENS)based on MOS-SAW detection units intended for lung cancer diagnosis is proposed.The MOS gas sensors are used to detect the VOC molecules with low molecular weight(LMW),and the SAW sensors are adopted for the detection of VOC with high molecular weight(HMW).Thus,the novel combination of these two kinds of gas sensors provides higher sensitivities to more of VOC species in breath than that of using only a single kind of sensor.The signals from MOS-SAW detection units are then recognized by a multi-model diagnosis method.Applying four algorithms,six models were established for diagnosis and tested by leave-one-out cross-validation method.The model by artificial neural network(ANN)was selected as the best model to analyze breath samples.89 clinical samples were tested with MOS-SAW ANN diagnostic model,which takes the features derived from both the MOS and SAW sensors.It shows the highest sensitivity of 93.62%,and the highest selectivity of 83.37%.The study shows that,promisingly,our HENS is effective during screening of lung cancer patients,especially among the people of high risk. 展开更多

In recent years,increasing attention has been given to public health.Cancer and obesity are hot topics,in which glucose and lipid metabolism play an important role.Glycogen synthase GYS2 has been found to regulate glycogen metabolism in hepatocellular carcinoma(HcC)and is a potential prognostic factor and therapeutic target.i Excessive bile acids induce intestinal injury and inflammatory bowel disease,and targeting bile acid receptors(FXRs)can regulate inflammation and repair damaged intestinal epithelial barriers.2 However,the research model of metabolic pathways focuses on the key enzymes,while ignoring the overall relevance.Here,we targeted the glycogen anabolism pathway and screened the upstream factor,bile acid transporter SLC10A1.SLC10A1 participates in the enterohepatic circulation of bile acids and regulates the transcriptional activity of the bile acid receptor FXR.Besides,we found that diazinon could bind NTCP and inhibit glycogen synthesis by mimicking the disordered bile acid enterohepatic circulation.Collectively,enterohepatic circulation of bile acids coordinates lipid metabolism and glucose metabolism and draws the metabolic crosstalk network,which provides a new perspective for the clinical treatment of HCC. 展开更多

Nkx2-1 (Nkx homeobox-1 gene), also known as TTF-1 (thyroid transcription factor-1), is a tissue-specific transcription factor of the thyroid, lung, and ventral forebrain. While it has been shown to play a critical role in lung development and lung cancer differentiation and morphogenesis, molecular mechanisms mediating Nkx2-1 cell- and tissue-specific expression in normal and cancerous lungs have yet to be fully elucidated. The recent identification of prognostic biomarkers in lung cancer, particularly in lung adenocarcinoma (ADC), and the different reactivity of patients to chemotherapeutic drugs have opened new avenues for evaluating patient survival and the development of novel effective therapeutic strategies. The function of Nkx2-1 as a proto-oncogene was recently characterized and the gene is implicated as a contributory factor in lung cancer development. In this review, we summarize the role of this transcription factor in the development, diagnosis, and prognosis of lung cancer in the hope of providing insights into the utility of Nkx2-1 as a novel biomarker of lung cancer. 展开更多

Coccidioidomycosis is an endemic disease of the Western hemisphere.The cases in non-endemic areas are mostly imported.Determining a history of exposure is critical for performing the diagnosis of coccidioidomycosis,especially for cases occurring in a non-endemic area.In this study,a 71-year-old Chinese male presented to our hospital with chronic cough and malaise,and was found to have a mass in the middle lobe of right lung.He had been visiting Arizona,USA for four months before admission.Pulmonary coccidioidomycosis was confirmed by the results of histopathological examination after lobectomy.Typical microscopic features of Coccidioides are definitely diagnostic,but need be differentiated from other budding yeast infection or carcinoma histopathologically. 展开更多

Lymph node involvement in pancreatic neuroendocrine tumors: significance as a predictor of survival

Whether regional lymph node involvement exerts significant effect on the prognosis still remains obscure for pancreatic neuroendocrine tumors.To clarify this association and identify predictors for lymph node involvement,we studied the data of patients aged>18 years with regional lymph node involvement histologically confirmed pancreatic neuroendocrine tumors from 2004 to 2014 in the Surveillance,Epidemiology,and End Results database(http://seer.cancer.gov/about).We evaluated Lymph node involvement as a prognostic factor by Cox regression.We reduced 9 variables of demographic and tumor characteristics to 5 potential predictors using least absolute shrinkage and selection operator(LASSO)regression model.We further constructed a lymph node involvement model by logistic regression.The model was verified by the verification set,and the visual expression of the model was realized by a nomogram.A total of 1545 cases of pancreatic neuroendocrine tumors were included in our study.Lymph node positivity was significantly associated with disease-specific survival(P<0.001).Younger patients(P<0.05),patients with tumors in the pancreatic head(P<0.05),patients at high American Joint Committee on Cancer T stage(P<0.001),and patients of an undifferentiated status(P<0.05)showed a significantly higher possibility of developing lymph node involvement.The reliability of this model was verified by cross-validation between the training and testing set,and we obtained good discrimination and calibration power.This model also showed great performance in C-index and area under receiver operating characteristic curve.Lymph node positivity was an important negative prognostic predictor for pancreatic neuroendocrine tumor.We developed a lymph node involvement model based on the predictors including age,marital status,primary site,T status,and tumor grade. 展开更多

Looking retrospectively at the development of humanity, vaccination is an unprecedented medical landmark that saves lives by harnessing the human immune system. During the ongoing coronavirus disease 2019(COVID-19) pandemic, vaccination is still the most effective defense modality. The successful clinical application of the lipid nanoparticle-based Pfizer/BioNTech and Moderna mRNA COVID-19 vaccines highlights promising future of nanotechnology in vaccine development. Compared with conventional vaccines, nanovaccines are supposed to have advantages in lymph node accumulation,antigen assembly, and antigen presentation;they also have, unique pathogen biomimicry properties because of well-organized combination of multiple immune factors. Beyond infectious diseases, vaccine nanotechnology also exhibits considerable potential for cancer treatment. The ultimate goal of cancer vaccines is to fully mobilize the potency of the immune system as a living therapeutic to recognize tumor antigens and eliminate tumor cells, and nanotechnologies have the requisite properties to realize this goal.In this review, we summarize the recent advances in vaccine nanotechnology from infectious disease prevention to cancer immunotherapy and highlight the different types of materials, mechanisms, administration methods, as well as future perspectives. 展开更多

The emerging two-dimensional monoelemental materials(2D Xenes)have been commonly supposed as promising drug delivery carriers,photothermal and photodynamic therapeutic agents,biosensors,theranostics,and some other candidates for biomedical applications.Here,high-performance and bioactive ultrathin 2D Tellurium nanosheets(Te NSs)are prepared by a simple but efficient liquid-phase exfoliation approach.The as-obtained Te NSs possess a mean size of~90 nm and a mean thickness of~5.43 nm.The pegylation Te NSs(Te-PEG NSs)possess excellent biocompatibility and stability.The Te-PEG NSs could generate local hyperthermia with a remarkable photothermal conversion efficiency of about 55%under 808 nm laser irradiation.Additionally,Te-PEG NSs exhibit an extremely high loading capacity of chemo drug(~162%)owing to their ultra-high surface area and tumor microenvironment-triggered drug release superiority.The results of in vivo experiments show that the Te-PEG NSs have higher tumor elimination efficiency via the combination of photothermal and chemotherapy,comparing to any other single therapeutic modalities.Therefore,our work not only highlights the promising potentials of tellurene as an ideal anti-cancer platform but also expands the application of 2D Te for cancer nanomedicine. 展开更多

An estimate of the incidence of pulmonary embolism in myeloma patients based on postmortem examinations is about 3.2%.
