In sub-Sahara region home deliveries still high. Giving birth at health facilities in most of sub-Saharan African countries Zambia inclusive is still a challenge whereby more than 51% of first-time mothers give birth at home and this gives a risk of high maternal and perinatal deaths. Therefore Reducing number of home deliveries is important to improve maternal and perinatal health issues. In this study, the aim was to investigate the determinants of home deliveries by pregnant mothers in the Luumbo zone of Gwembe district, Zambia. Purpose: Access to skilled care and facilities with capacity to provide emergency and newborn care is critical to reduce maternal death. In Zambia 42% of women still deliveries from home, suggesting a persistent challenge for women to seek, reach, and receive quality maternity care. This study aimed investigate the determinants of home deliveries by pregnant mothers in Luumbo zone of Gwembe district, Zambia. Methods: The study was conducted among postnatal mothers who came for postnatal care at 6 weeks in Luumbo Chabbobboma clinic in Gwembe district southern province of Zambia. This was a descriptive cross-sectional study where a Simple random sampling technique was used to select 105 women of childbearing age who attended postnatal and had a recent delivery. Data were collected using a researcher-administered structured questionnaire to identify determinants of home deliveries in Luumbo Chabbobboma zone. Data analysis was done using SPSS computer software version 27.0. Both descriptive and inferential (chi-square test) analyses were performed and statistical significance was taken at α ≤ 0.05. Results: The results show that 46 (43.8%) respondents were in the age bracket 20 - 29 years. Of the 105 respondents included in the study, 24 (22.9%) of them delivered from home. The results show that high maternal age (p = 0.03), occupation (p = 0.024), distance to the facility (p = 0.014), means of transportation (p = 0.023), multiparity (p = 0.01), timing and number of ANC visits (p Conclusion: From this population. The major reason why women still deliver at home was long distance to the nearest facility. To reduce maternal and perinatal mortality access to health facilities by pregnant women needs to be improved. There should also be active engagement of the traditional and religious institutions in the area.展开更多

Likewise, it increases the risk of overweight/obesity and related comorbidities among the recipients. In countries like Botswana where ±84% of PLHIV is on ART, the paucity of data comparing overweight/obesity between HIV-positive on ART and HIV-negative patients may impede clinical and policy decision-making. This study sought to estimate and compare: i) the prevalence of overweight/obesity between HIV-positive on ART and HIV-negative patients;ii) the prevalence of hypertension (HTN), diabetes mellitus (DM)/coronary heart disease (CHD) between HIV-positive patients on ART and HIV-negative patients attending same outpatient departments of general clinics in Gaborone, Botswana. Patients and Methods: Five hundred eighty-one (581) outpatients were recruited in four major clinics of Gaborone, Botswana, between June and July 2019;294 or 51% of them were HIV-negative and 287 or 49% were HIV-positive on ART. The prevalence of overweight/obesity and of HTN and DM/CHD were calculated and examined using stratified analysis. Subgroups were compared using Chi-square analysis with Yates correction or Fisher exact test and t-student test for continuous data. Results: Major findings after stratification of the study population by HIV status were: i) the prevalence of all categories of (BMI), including overweight/obesity, were comparable between HIV-negative-patients and HIV-positive. In fact, there were 24 (8.0%) cases of underweight among HIV-negative-Patients and 15(5%) cases among HIV-positive patients, p = 0.2;145 (49%) HIV-negative-patients and 128 (45%) HIV-positive, p = 0.07 cases of normal weight;72 (25%) HIV-negative-patients and 87 (30%) HIV-positive, p = 0.08, were overweight;53 (18%) HIV-negative-patients and 57 (20%) HIV-positive, p = 0.12, were obese;125 (43%) HIV-negative patients and 144 (50%) HIV-positive, p=0.06 were overweight/obese;ii) the prevalence of HTN and DM/CHD among HIV-positive-patients were significantly lower (p < 0.05) compared to HIV-negative patients: There were 32 (10.9%) cases of HTN among HIV-negative patients compared to 18 (6.3%) cases of HTN among HIV-positive patients, p = 0.001;32 (11%) cases of DM/CHD HIV-negative patients compared to 4 (1.4%) cases of DM/CHD among HIV-positive patients, p = 0.001. Conclusion: the prevalence of overweight/obesity observed between HIV-negative and HIV-positive patients may suggest that the two groups shared the same exposure factors. That HTN and DM/CHD prevalence was lower among HIV-positive compared to HIV-negative patients, is possibly due to interplay factors of ART, HIV or the host population. Further studies are, however, recommended for clarifications.展开更多

Globally, an estimation of 180,000 children under 15 years acquire the Human immunodeficiency virus every day, and more than 90% of those infections are due to Mother to Child Transmission. The study sought to explore the experiences of mothers on the Elimination of Mother to Child Transmission-HIV services at Mtendere Clinic, Lusaka. Materials and Methods: Qualitative interpretive phenomenology study design was employed using in-depth interviews to collect data from a sample that was selected using purposive sampling technique. Thirteen participants were recruited, and these were HIV-positive mothers at least between the ages of 15 and 49 years and enrolled in the Elimination of Mother To Child Transmission-HIV program. The in-depth interviews were audiotape recorded and transcribed verbatim. Data was analyzed using thematic method. Findings: Three main themes that emerged are;living with HIV, support system and barriers to utilization of Elimination of Mother To Child Transmission-HIV services. Most of the participants expressed having acquired knowledge from the program, and received counselling and support from spouses, family as well as health personnel at Mtendere health facility which culminated into a positive experience and enhanced their uptake of the Elimination of Mother to Child Transmission-HIV services. However, barriers to service utilization were identified and these included fear of stigma, negative attitudes from health workers, long waiting times, lack of support and lack of transport to the health facility. Conclusion: Interventions such as community awareness campaigns on Elimination of Mother To Child Transmission-HIV, male involvement and implementing mother-to-mother peer support strategies in Elimination of Mother To Child Transmission-HIV service utilization should be prioritized so as to alleviate stigma and enhancing a positive experience for these mothers thus reducing on the Mother to Child HIV Transmission burden and mortality rates.展开更多

But most hospitals in the developing countries face challenges of constant supply of blood to carry out different life saving procedures that require blood, due to paucity of blood donors. Many studies have been done on the perception of blood donation but only a few have been on Medical and Pharmaceutical Students. Aim: To determine the perception of blood donation among Medical and Pharmaceutical Students of Nnamdi Azikiwe University, Nnewi and Agulu Campuses respectively. Material and Methods: Stratified sampling technique was used to divide the students into strata (200, 300, 400, 500, and 600), then simple random sampling was used to select different respondents from each class and self-administered questionnaires were given to the respondents. Data was collected and results were analyzed. Results: 294 respondents, 147 medical students and 147 pharmaceutical students participated in the study 141 (95.9%) of medical students and 137 (93.2%) of pharmaceutical students had heard of blood donation. 139 (94.6%) of medical students and 140 (95.2%) of pharmaceutical students knew their blood groups. 59.5% of the respondents had donated blood. The commonest motivating factor towards blood donation is “to save a friend or family member”. Conclusion: The knowledge of the students regarding blood donation was high but the practice of blood donation was low.展开更多

It has remained uncertain whether or not it has a lot of positive benefits for the students’ experiences during placement as there have been divided opinions among researchers. Objective and review question: The objective of this qualitative systematic review was to explore the perceptions and experiences of nursing and midwifery students regarding mentorship during clinical placement. The question of the review was: what are the perceptions and experiences of nursing and midwifery students regarding mentorship during clinical placement. Methods and materials: Electronic data bases, journals and grey literature were searched for relevant studies and seven qualitative studies were systematically included in the review. The Joanna Briggs Institute-Qualitative Assessment and Review instrument (JBI-QARI) was used to critically appraise the studies. Data extraction was done using the JBI-QARI Data Extraction Tool. A thematic analysis was used to synthesise the findings. Findings: The findings suggest that there are still gaps within the mentorship programme that could have an impact on the students’ learning experiences during clinical placement. From the themes identified which include: need for feedback, hands on practice, continuity of mentorship and mentor-mentee relationship, the reviewers concluded that such areas of mentorship require attention in order to benefit the students. Conclusions: It can therefore be concluded that there are many lessons to be learnt in order to continue improving mentorship. It is also clear that mentorship will be beneficial to students if a good relationship is established between the mentor and mentee, as this is the beginning of the experience. All staff members should be willing to support students in learning for them to develop the knowledge, skill and attitudes that are necessary to their learning.展开更多

Clinical learning environments may influence nursing students’ career choices. Few studies have explored learning environments in nursing homes, although students increasingly have placements there. Objectives: The aim was to produce information for developing nursing students’ learning opportunities in nursing homes. Design: A cross-sectional survey design was used. Settings: The study was conducted at a university college in southeast Norway. Participants: Students in two cohorts of a bachelor degree program in nursing participated, N = 499. Methods: Data were collected on nursing students’ perceptions of clinical learning environments during placements in five nursing homes. A 42-item validated questionnaire with the subscales personalization, individualization, involvement, task orientation, innovation, and satisfaction was used. Data analysis used descriptive statistics, t-tests, and linear regression analysis. Results: Total scores showed overall satisfaction with clinical learning environments in nursing homes. However, innovation subscale scores were very low. First year students had significantly higher scores than third year students on the total scale, and most subscales. Age was significantly associated with total scale scores and subscale satisfaction scores. Higher education and work experience before entering nursing education were significantly associated with involvement subscale scores. Conclusions: Students are more positive than negative about their clinical learning environments. Low valuation of innovation seems to be a consistent finding in studies in both nursing homes and hospitals internationally. For innovative learning strategies to function they must be anchored at the organizational level. Future research should develop and test more innovative learning strategies for nursing students.展开更多

The sleep status of 21 participants aged ≥65 years was monitored while they slept with a sensor placed under their bedding, after ruling out insomnia and dementia. The incidence of delirium was 28.6%;delirium onset occurred within an average of 2.7 (SD = 12) days after the start of home care among those whose care environment was changed due to hospital discharge or moving. Increased interrupted sleep and activity during sleep indicated that sleep fragmentation occurred before delirium onset. In conclusion, individuals aged ≥65 years and those whose care environment has changed should be screened for delirium because the time to delirium onset is short. Further, interventions to monitor the sleep status and prevent delirium onset should be implemented from the day home care begins.展开更多

We surveyed 31 nurses (25 women and 6 men) who performed MVT with accelerometers attached to the backs of their hands, and the data obtained were analyzed. The mean vibration frequency was 8.3 Hz (standard deviation [SD]: 1.9 Hz) bilaterally, with a left-right difference of 0.8 Hz (SD: 1.1 Hz, right > left). Furthermore, vibration frequency was correlated with duration of MVT use (rs = 0.5, P < 0.01). The vibration frequency was higher in men (9.2 Hz, SD: 2.4 Hz) than in women (8.1 Hz, SD: 1.8 Hz), but this difference was not significant (P = 0.34). The vibrations of MVT are of a lower frequency than those of other vibration therapies.展开更多

The goal was to identify future challenges in nursing support in clinical practice in Japan. Method: Literature searchable as of May 2023 was retrieved using PubMed, resulting in 11 relevant articles. Result: The results revealed the following: 1) For support provided to children, 13 codes were condensed into 5 subcategories and 4 categories. 2) For support provided to families, 36 codes were condensed into 11 subcategories and 4 categories. Conclusion: Open communication was found to be essential for supporting children and their families who have experienced parental bereavement. Moreover, involvement of multiple professions facilitated the provision of specialized support to address diverse needs of children and families, playing a crucial role in overcoming grief. Additionally, the effectiveness of support systems for bereaved families highlighted the need for nursing professionals in Japan to gain knowledge through learning opportunities and to establish a multi-disciplinary approach to support, thus indicating future challenges in nursing support.展开更多

文摘Objective:The study assessed the quality of life(QoL)and coping strategies of patients on hemodialysis in selected hospitals in Ilorin,Kwara State. Methods:The study was a descriptive cross-sectional design using a quantitative strategy.A multistage sampling technique was adopted to select 80 participants. The data were collected using an adapted questionnaire including four sections:sociodemographic characteristics, hemodialysis stressor scale consisting of four stressors,Jaloweic Coping Scale consisting of eight coping strategies, and QoL consisting of four items. The data were analyzed using SPSS version 23.Results:Findings from the study revealed that 50%of the participants were between the ages of 18 and 39 years,40%of the participants have been on hemodialysis for 1-2 years, whereas 52.5%had hypertension as a comorbidity.A total of 12.5%,63.8%,and 23.8%of the respondents were categorized as experiencing low, moderate, and high stress due to hemodialysis, respectively.QoL of the respondents revealed that 49.4%had no problem in self-care dimension.Average monthly income(χ^(2)=16.36,P=0.04)was observed to be significantly associated with coping strategies, whereas religion(χ^(2)=11.14,P=0.03)and frequency of hemodialysis(χ^(2)=10.26,P=0.04)were significantly associated with coping helpfulness. Conclusion:All participants experienced stress at varying degrees.Hemodialysis results in marked changes in the quality of patients’life, since it includes a number of modifications and restrictions, which affects patients’health functioning. This study revealed a wide range of information on the psychological and physiological stressors of people under hemodialysis.
文摘Background: Home deliveries is still high globally at 42% WHO 2022, due to high home deliveries, maternal death is also high at 43% globally. There should also be active engagement of the traditional and religious institutions in the area.
文摘Context: Antiretroviral therapy (ART) helps restore the health status of people living with HIV (PLHIV). Further studies are, however, recommended for clarifications.
文摘Introduction: Acquired immunodeficiency syndrome is one of the leading causes of mortality among women of reproductive age and Mother to Child Transmission of Human immunodeficiency virus is still a challenge affecting many countries. Conclusion: Interventions such as community awareness campaigns on Elimination of Mother To Child Transmission-HIV, male involvement and implementing mother-to-mother peer support strategies in Elimination of Mother To Child Transmission-HIV service utilization should be prioritized so as to alleviate stigma and enhancing a positive experience for these mothers thus reducing on the Mother to Child HIV Transmission burden and mortality rates.
文摘Background: Certain ovarian cancers that were previously common in postmenopausal women are now increasingly observed in women of reproductive age. The research on using β-HCG as a diagnostic biomarker for ovarian cancer in women of reproductive age is ongoing. Aim: This study assessed the level of serum β-HCG in non-pregnant women of reproductive age and determined its potential association with suspicious ovarian ultrasonography results. Methods: The study was conducted in Port Harcourt, Nigeria. This study adopted a cross-sectional design on a quota sample of 224 case notes of women aged 18 - 40 years obtained from eight diagnostic centres. A data extraction form was used for data collection. Data analysis employed descriptive statistics, Chi-square, Fisher’s exact test, and Odds Ratio at 95% confidence and 5% significance levels. 文摘Objective:To identify the knowledge and awareness level of human mpox viral infection among healthcare workers in southwestern Nigeria. Methods:A cross-sectional study was conducted in Ekiti State, southwest Nigeria among 316 healthcare workers that were selected through a systematic random sampling.Data were collected with the aid of a semi-structured,self-administered questionnaire. The Chi-square test and binary logistic regression were used to find the association between the independent and dependent variables. The significance level was set at P-value<0.05.Results:Two hundred and twenty-two(70.3%)of the respondents were aged≤40 years, mean age(36±9)years,189(59.8%)were female,306(96.8%)were Christians, and 203(64.2%)were married.Three hundred and fourteen(99.4%)of the respondents were aware of mpox infection.Main sources of information about mpox were medical education(44.0%),radio/television(32.0%)and newspaper(21.0%).However,among those aware of the disease,209(67.0%)demonstrated poor knowledge levels.Longer than 5 years’experience of medical practice was the only significant predictor of higher knowledge level of the disease(OR 1.76,95%CI 1.01-3.06;P=0.046).Conclusions:Despite the high awareness level of mpox infection among healthcare workers, there still exists a huge knowledge gap.It is recommended that targeted intervention could be directed towards continuous medical education and simulation exercises on re-emerging infectious diseases like mpox to improve the knowledge of the healthcare workers.
文摘Background: Safe blood is a vital component in improving healthcare globally and millions of lives are saved each year through blood donation. Conclusion: The knowledge of the students regarding blood donation was high but the practice of blood donation was low.
文摘Hypertension(HTN)is one of the most common chronic diseases affecting over 30%of the adult population globally, with a growing incidence rate. This article aims to identify the commonly used herbs for HTN treatment and examine their claims, criticisms, and challenges. It further aims to provide useful recommendations regarding the use of herbs for HTN treatment.HTN complications, such as coronary heart disease, stroke, peripheral vascular disease, vision impairment, and renal failure can result in morbidity and mortality. The high cost of conventional medications, which sometimes may not even be available or easily accessible with their unfavorable side effects as well as taking more than one pill per day,has led hypertensive patients, particularly those in rural areas,to explore alternative treatments such as herbal therapies. It is crucial to determine the different modes of action, doses, safety, and efficacy of herbal remedies used in combination with conventional medications to improve treatment adherence and enhance patient outcomes.
文摘Background: Mentorship has been the model adopted by many countries to support learning and assessment of students in the clinical placement. All staff members should be willing to support students in learning for them to develop the knowledge, skill and attitudes that are necessary to their learning.

文摘Background: Nursing students hesitate to choose aged care as a career, and the aged care sectors are on an edge regarding nursing positions. Future research should develop and test more innovative learning strategies for nursing students.

文摘Objective:The objective of the study is to validate the short version of evidence-based practice(EBP)instruments among nurses in clinical practice. Methods:An institutional-based cross-sectional research design was used and a stratified sampling technique to select 285 nurse clinicians. The study utilized a structured questionnaire comprising of demographic data from the participants and three validated scales:the shortened versions of the EBP Beliefs Scale, the EBP Implementation Scale, and the Organizational Culture and Readiness for System-Wide Integration of Evidence-Based Practice(OCRSIEP)survey. With the use of descriptive statistics, the data were analyzed and presented in frequencies and percentages, while inter-item correlation coefficient(ICC)and the Kaiser-Meyer-Olkin measure of sampling adequacy were used to confirm the validity of using factor analysis. Results:Findings revealed the mean scores of the EBP Beliefs Scale ranged from 1.50 to 1.61,EBP Implementation Scale ranged from 1.84 to 1.94,and the OCRSIEP Scale ranged from 1.93 to 2.19.All the three shortened scales accordingly had good internal reliability,29.30±9.93 out of 80 for the EBP Beliefs Scale,19.56±7.37 out of 72 for the EBP Implementation Scale, and 66.32±20.35 out of 125 for the OCRSIEP Scale. Conclusion:This study has generated a valid Short Version of EBP reliable instrument that is psychometrically robust that can be used by nurses and clinicians to evaluate EBP in clinical settings since the results presented as a whole confirmed the high reliability and factorial validity.
文摘We aimed to clarify the sleep status before delirium onset among older adults receiving home care. Further, interventions to monitor the sleep status and prevent delirium onset should be implemented from the day home care begins.
文摘Objective:More health workers die from infectious diseases such as tuberculosis(TB)than from any other infectious agent,so the current study is designed to assess TB treatment and preventive measures as they relate to nursing care practice.Materials and Methods:A cross-sectional study was conducted among 82 nurses from December 2022 to January 2023.The data were collected through a self-developed questionnaire and analyzed by frequency table and analysis of variance(ANOVA)statistics. Results:The researchers found that nurses have sufficient knowledge on Infection Prevention and Control(IPC)policy, but not all nurses follow this management protocol, and the reason for their action is addressed in this study. The management and preventive measures used by the participants were identified as:proper cough etiquette/hygiene,working in a well-ventilated area, use of mask, appropriate disposal of used items and so on was reported. Moreover, this study also discovered that there is a positive correlation between nurses who had received training in IPC policy and their use of airborne preventive measures for TB patients(F=1.87,P=0.002)as well as a positive correlation between the availability of resources and their adherence to the proper use of airborne preventive measures when caring for TB patients(F=1.96,P˂0.001).Conclusion:We proposed infection and control training for nurses and adequate equipment supplies to the TB ward which are required in quick TB diagnosis, and must be carried out on a regular basis by health-care personnel, for efficient nursing practice.Ensuring proper safety equipment and isolation units should be available and assessable for nurses or other health workers showing sign or symptoms of TB.Similarly,it is necessary for government to put in place the control and regulations that will safeguard nurses and mandate them to adopt TB prevention protocols, which will aid in lowering the prevalence of TB among nurses.
文摘Cervical cancer remains a major public health problem accounting for continuous female death in developing countries.Cervical cancer is the second most common cancer in women globally with an estimated number of 500,000 new cases of cervical cancer and 273,000 mortalities annually.Cervical screening is the best cancer screening test in the history of medicine and most cost-effective of all medical screening tests. The study review aimed to highlight methods of cervical cancer prevention, identify the uptake of cervical screening among women, and explain the role of nurses in uptake of cervical screening.Cervical cancer is preventable and easily treatable if it is detected at early stages, but poor information on prevention and access to screening and treatment contributes to 90%of deaths. In the developing countries such as Nigeria,a large proportion of cervical cancers are diagnosed in advanced stages, with poor rates of survival. The three main methods for preventing cervical cancer are through primary prevention(human papillomavirus vaccination and health education),secondary prevention(cervical screening, early detection of precancerous lesions, and treatment),and tertiary prevention(measures to slow the progression or recurrence, surgical removal, radiotherapy, and chemotherapy).Nurses can help improve the acceptability of this neglected promise by focusing on health education on cervical screening and vaccination in prenatal clinics and infant welfare clinics where there are more women.Inclusion of cyberspace could also serve as a successful and popular platform for this health teaching. All nurses/midwives must preach and fervently work toward the WHO 90–70–90 plan for eradicating cervical cancer.
文摘This study investigated the vibration frequency in micro-vibration therapy (MVT) performed as a part of nursing care in Japan. The vibrations of MVT are of a lower frequency than those of other vibration therapies.
文摘Purpose: This study aimed to understand the actual nursing support in a wide perspective by reviewing overseas literature on support for children who have experienced parental bereavement and their families. Additionally, the effectiveness of support systems for bereaved families highlighted the need for nursing professionals in Japan to gain knowledge through learning opportunities and to establish a multi-disciplinary approach to support, thus indicating future challenges in nursing support.
