Within the scope of this study,it is aimed to contribute to the efforts for forming a database including the natural radiation levels throughout the country and developing a foundation for radiation safety.Karasu port...Within the scope of this study,it is aimed to contribute to the efforts for forming a database including the natural radiation levels throughout the country and developing a foundation for radiation safety.Karasu portion of the Euphrates valley within the provincial border of Erzincan was chosen for the investigation and natural radiation levels were detected by analyzing the samples taken in terms of radioactivity,natural environment’s radiation streams.Throughout the region of Karasu,29 different samples from the water and 17 from the soil were taken and their total alpha,beta activities were detected.To take samples from the water and then they were measured by low background counter(Berthold brand LB770 10-channel counter).Total alpha and beta analyses were made according to the standard methods.Average values of alpha and beta concentrations of samples were measured as 0.0421 Bq/L and 1.806 Bq/L,respectively.For the soil samples HPGe gamma spectrometer system was used and radioisotope activity concentrations were found within the intervals of 2.7-27.8 Bq/kg for 226Ra;3.2-39.4 Bq/kg for 232Th;98-714 Bq/kg for 40K;<0.4-57.8 Bq/kg for 137Cs,respectively.Different radioisotopes were detected in some of the samples taken from streaming beds joining river water.展开更多
BACKGROUND This article examines primary hepatic angiosarcoma(PHA)and fat-poor angiomyolipoma(AML),two uncommon vascular cancers.Clinical decisions in these situations are frequently aided by pathology reports and ima...BACKGROUND This article examines primary hepatic angiosarcoma(PHA)and fat-poor angiomyolipoma(AML),two uncommon vascular cancers.Clinical decisions in these situations are frequently aided by pathology reports and imaging techniques.Uncommon malignant tumors of the vascular endothelium include PHA.Another diagnosis that should not be overlooked when employing contrastenhanced MR and contrast-enhanced computed tomography(CT)imaging techniques is fat-poor AML,one of the uncommon vascular tumors of the liver.In both conditions,biopsy is the primary means of diagnosis.CASE SUMMARY In our article,besides the diagnosis of PHA,fat-poor AML,one of the other rare vascular tumors of the liver,is mentioned.In the case,a 50-year-old female patient with VHL Syndrome was admitted to our hospital with nonspecific lesions such as right upper quadrant pain,weight loss,and nausea.Abdominal ultrasonography(US)revealed a hypoechoic heterogeneous lesion with occasional faint contours.In computed tomography,it was observed as a hyperdense nodular lesion in segment 4.Magnetic resonance imaging(MRI)revealed that the lesion did not contain fat.In connection with the known history of VHL Syndrome,we first evaluated the possibility of AML.Thereupon,a histopathological sample was taken and the diagnosis was made as fat-poor AML with 5%fat content.CONCLUSION In conclusion,PHA in our case report and fat-poor AML in our clinic are two uncommon liver vascular malignancies with comparable incidences.Important imaging techniques like contrast-enhanced US(CEUS),CECT,and CEMRI give us substantial advantages in both cases.However,a biopsy is used to provide the final diagnosis.展开更多

Because of increasing pa...The purpose of this review article is to discuss the clinical spectrum of recurrent traumatic anterior shoulder instability with the current concepts and controversies at the scientific level. Because of increasing participation of people from any age group of the population in sports activities, health care professionals dealing with the care of trauma patients must have a thorough understanding of the anatomy, patho-physiology, risk factors, and management of anterior shoulder instability. The risk factors for recurrent shoulder dislocation are young age, participation in high demand contact sports activities, presence of Hill-Sachs or osseous Bankart lesion, previous history of ipsilateral traumatic dislocation, ipsilateral rotator cuff or deltoid muscle insufficiency, and underlying ligamentous laxity. Achieving the best result for any particular patient depends onthe procedure that allows observation of the joint surfaces, provides the anatomical repair, maintains range of motion, and also can be applied with low rates of complications and recurrence. Although various surgical techniques have been described, a consensus does not exist and thus, orthopedic surgeons should follow and try to improve the current evidence-based treatment modalities for the patients.展开更多

The most common causes of SCI in the world are traffic accidents, gunshot injuries, knife injuries, falls and sports injuries. There is a strong relationship between functional status and whether the injury is complete or not complete, as well as the level of the injury. The results of SCI bring not only damage to independence and physical function, but also include many complications from the injury. Neurogenic bladder and bowel, urinary tract infections, pressure ulcers, orthostatic hypotension, fractures, deep vein thrombosis, spasticity, autonomic dysreflexia, pulmonary and cardiovascular problems, and depressive disorders are frequent complications after SCI. SCI leads to serious disability in the patient resulting in the loss of work, which brings psychosocial and economic problems. The treatment and rehabilitation period is long, expensive and exhausting in SCI. Whether complete or incomplete, SCI rehabilitation is a long process that requires patience and motivation of the patient and relatives. Early rehabilitation is important to prevent joint contractures and the loss of muscle strength, conservation of bone density, and to ensure normal functioning of the respiratory and digestive system. An interdisciplinary approach is essential in rehabilitation in SCI, as in the other types of rehabilitation. The team is led by a physiatrist and consists of the patients' family, physiotherapist, occupational therapist, dietician, psychologist, speech therapist, social worker and other consultant specialists as necessary.展开更多

AIM:To determine the effects of Rutin on methanol induced optic neuropathy and compare the results with the effects of ethanol.METHODS:Totally 30 rats were divided into 5 groups,with 6 rats in each group as follows...AIM:To determine the effects of Rutin on methanol induced optic neuropathy and compare the results with the effects of ethanol.METHODS:Totally 30 rats were divided into 5 groups,with 6 rats in each group as follows:healthy controls(C),methotrexate(MTX),methotrexate+methanol(MTM),methotrexate+methanol+ethanol(MTME) and methotrexate+ methanol+Rutin(MTMR).In all rabbits except those of the control group,MTX,diluted in sterile serum physiologic,0.3 mg/kg per oral was applied for 7 d by the aid of a tube.After this procedure to the rats of MTM,MTME and MTMR groups,20% methanol with a dose of 3 g/kg per oral was given by the aid of a tube.In MTME group,4 h after the application of methanol,20% ethanol was applied by the same way with a dose of 0.5 g/kg.On the other hand,in MTMR group 4 h after the application of methanol,Rutin,which was dissolved in distilled water,was applied by the same way with a dose of 50 mg/kg.RESULTS:There were statistically significant differences in tissue 8-hydroxy-2 deoxyguanine(8-OHdG),interleukin-1β(IL-1β),tumor necrosis factor-alpha(TNF-α),malondialdehyde(MDA),myeloperoxidase(MPO).glutathione peroxidase(t GSH) and superoxide dismutase(SOD) levels between groups(P〈0.001).In MTMR group tissue 8-OHdG,IL-1β,MDA,and MPO levels were similar with the healthy controls but significantly different than the other groups.In histopathological evaluations,in MTX group there was moderate focal destruction,hemorrhage and decrease innumber of astrocytes and oligodendrocytes;in MTM group there was severe destruction and edema with decrease in number of astrocytes and oligodendrocytes;in MTME group there was mild hemorrhage,mild edema,mildly dilated blood vessels with congestion while in MTMR group,optic nerve tissue was resembling the healthy controls.CONCLUSION:Rutin may prevent methanol-induced optic neuropathy via anti-inflammatory effects and decreasing the oxidative stress.New treatment options are warranted in this disease to avoid loss of vision in patients.展开更多

In this study,Tungsten Oxide(WO_3)thin films were prepared by Chemical Spray Pyrolysis(CSP)and Spin Coating(SC)techniques and it was investigated effects of technique and parameter on the films.WO_3 thin films were de...In this study,Tungsten Oxide(WO_3)thin films were prepared by Chemical Spray Pyrolysis(CSP)and Spin Coating(SC)techniques and it was investigated effects of technique and parameter on the films.WO_3 thin films were deposited on ITO(Indium Tin Oxide)coated glasses.The structural,optical and electrochromic properties of the WO_3 thin films were characterized by XRD,SEM,UV,and CV measurements.The sharpest(200)peak was observed in the XRD spectra and optical band gaps were calculated around 2.6~3.1 eVvia UV-Vis spectra for all of the samples.Micro fibrous reticulated surface(filamentous like)morphology for the films deposited by CSP technique and smooth surface morphology with high optical transmittance for the film deposited by SC Technique were obtained from SEM images.In addition to these results,it was revealed that all the samples exhibit good electrochromic performance.展开更多

The ground state hydrogen conformations and vibrational analysis of 3-deazauracil(3DAU)and 6-azauracil(6AU)tautomers(4-enol and 2,4-diol forms)have been calculated using ab initio Hartree-Fock(HF)and density functiona...The ground state hydrogen conformations and vibrational analysis of 3-deazauracil(3DAU)and 6-azauracil(6AU)tautomers(4-enol and 2,4-diol forms)have been calculated using ab initio Hartree-Fock(HF)and density functional theory(B3LYP)methods with 6-311++G(d,p)basis set level.The calculations have shown that the most probably preferential tautomer of 3DAU and 6AU are the 4-enol form,which gives best fit to the corresponding experimental data.The ground state conformer of the 2,4-diol form has two O—H bonds which are oriented externally and internally(to the N—H bond).The vibrational analyses of the ground state conformer of each tautomeric form of 3DAU and 6AU were done and their optimized geometry parameters(bond lengths and bond angles)were given.Furthermore,from the correlations values it was concluded that the B3LYP method is superior to the HF method for both the vibrational frequencies and the geometric parameters.展开更多

The present study aims at determining the effect of activities of daily living of patients with type 2 diabetes mellitus on their self-care agency. Methods: The population of this descriptive study consisted of patients with type 2 diabetes mellitus who were being treated at the internal medicine clinic of a provincial state hospital between July 2014 and November 2015 and its sample consisted of 150 diabetic patients who volunteered to take part in the study and who were open to communication. A personal information form, the Activities of Daily Living (ADL) Scale, the Instrumental Activities of Daily Living (IADL) Scale and the Self-Care Ability Scale (CAS) were used as data collecting tools. The data were analyzed using descriptive statistics (numbers, percentages, mean, standard deviation, mean rank and frequencies), the Shapiro-Wilk, Kruskal-Wallis Variance Analysis, Mann-Whitney U test and Cronbach’s alpha formula and correlation analysis. Results: Approximately 97.3% of the patients with type 2 diabetes stated that they were independent in ADL and 75.3% of them in IADL. The patients’ mean self-care ability score was found to be 83.85 ± 17.87 ADL and IADL were found to be affected by age, marital status, education, duration of disease, willingness to receive further education, and presence of another disease besides diabetes. There was a significant correlation between the self-care agency score and marital status, education, duration of disease, willingness to receive further education, presence of another disease besides diabetes, regular checking of blood sugar and compliance with diet. A significant positive correlation was found between the patients’ activities of daily living and their self-care agency. Conclusion: The patients’ activities of daily living were found to affect their self-care agency. The personal and disease-related characteristics of patients should be identified so that their self-care behaviors can be increased.展开更多
AIM To investigate the role of minimally invasive surgery for gastric cancer and determine surgical, clinical, and oncological outcomes.METHODS This is a propensity score-matched case-control study, comparing three tr...AIM To investigate the role of minimally invasive surgery for gastric cancer and determine surgical, clinical, and oncological outcomes.METHODS This is a propensity score-matched case-control study, comparing three treatment arms: robotic gastrectomy(RG), laparoscopic gastrectomy(LG), open gastrectomy(OG). Data collection started after sharing a specific study protocol. Data were recorded through a tailored and protected web-based system. Primary outcomes: harvested lymph nodes, estimated blood loss, hospital stay, complications rate. Among the secondary outcomes, there are: operative time, R0 resections, POD of mobilization, POD of starting liquid diet and soft solid diet. The analysis includes the evaluation of type and grade of postoperative complications. Detailed information of anastomotic leakages is also provided.RESULTS The present analysis was carried out of 1026 gastrectomies. To guarantee homogenous distribution of cases, patients in the RG, LG and OG groups were 1:1:2 matched using a propensity score analysis with a caliper = 0.2. The successful matching resulted in a total sample of 604 patients(RG = 151; LG = 151; OG = 302). The three groups showed no differences in all baseline patients characteristics, type of surgery(P = 0.42) and stage of the disease(P = 0.16). Intraoperative blood loss was significantly lower in the LG(95.93 ± 119.22) and RG(117.91 ± 68.11) groups compared to the OG(127.26 ± 79.50, P = 0.002). The mean number of retrieved lymph nodes was similar between the RG(27.78 ± 11.45), LG(24.58 ± 13.56) and OG(25.82 ± 12.07) approach. A benefit in favor of the minimally invasive approaches was found in the length of hospital stay(P < 0.0001). A similar complications rate was found(P = 0.13). The leakage rate was not different(P = 0.78) between groups.CONCLUSION Laparoscopic and robotic surgery can be safely performed and proposed as possible alternative to open surgery. The main highlighted benefit is a faster postoperative functional recovery.展开更多
BACKGROUND Total exposure to ionizing radiation has nearly doubled in the last two decades.This increase is primarily due to increased computed tomography(CT)exposure.Concerns have been raised about the risks associat...BACKGROUND Total exposure to ionizing radiation has nearly doubled in the last two decades.This increase is primarily due to increased computed tomography(CT)exposure.Concerns have been raised about the risks associated with patients'exposure to medical imaging radiation,which can increase a person's lifetime risk of developing cancer.Preventing unnecessary examinations becomes critical at this point.To avoid unnecessary examinations,it is necessary to understand the demanding process.AIM To ascertain clinicians'awareness of and reasons for requesting a CT examination.METHODS We developed an online questionnaire that included 20 questions about clinicians'awareness of radiation safety and their reasons for requesting a CT examination,as well as demographic information such as age,gender,and year of medical practice experience.Additionally,we asked participants the number of CT scans requested in a month,the patients'questions and approaches about the imaging method,the effect of the patient's previous imaging history on the current imaging request,whether they believed that they had sufficient information about radiation doses,and whether they requested CT without an indication.We administered the questionnaire to clinicians from a variety of different professions in four different cities.RESULTS A total of 195 clinicians participated.Internal medicine specialists were the most crowded group(38/195,19.5%).Mean age of the population was 33.66±5.92 years.Mean year of experience was 9.01±5.96.Mean number of requested CT scans in a month was 36.88±5.86.Forty-five(23.1%)participants stated that they requested CT scans without clinical indication.The most common reasons for CT scan requests were work load,fear of malpractice,and patient demand/insistence.CONCLUSION CT scan requests are influenced by a variety of factors,both internal and external to the doctors and patients.Raising awareness of radiation safety and reducing fear of malpractice by limiting the number of patients per physician may result in a reduction in unnecessary CT examinations and ionizing radiation exposure.展开更多

Objective:To investigate Blastocystis’etiologic role and association with gastrointestinal symptomatology in acute and chronic urticaria patients and to identify Blastocystis subtypes responsible for urticaria.Method...Objective:To investigate Blastocystis’etiologic role and association with gastrointestinal symptomatology in acute and chronic urticaria patients and to identify Blastocystis subtypes responsible for urticaria.Methods:The study included urticaria patients and healthy individuals that presented to our polyclinic between June 2015 and May 2017.The participants were assigned into Group栺(137 patients),subdivided into acute(72)and chronic urticaria patients(65),and Group栻(129 control individuals).Blastocystis presence was investigated by native-Lugol examination,trichrome staining,PCR using sequence tagged site primers,and DNA sequencing analysis.The phylogenetic tree was constructed.Results:The native-Lugol and trichrome staining methods revealed that 16 patients(16/133,12.0%)had Blastocystis-positive stool samples,of which seven samples(7/133,5.3%)belonged acute and nine(9/133,6.8%)to chronic urticaria patients.Concerning Blastocystis subtypes,of the acute urticaria patients,three had subtype 1(ST1),one had ST2,and three had ST3.Of the chronic urticaria patients,one had ST1 and eight had ST3.Blastocystis positivity was detected in two control individuals(2/123,1.6%),both being ST3.All subtypes identified by PCR were confirmed by the sequencing analysis.The acute and chronic urticaria groups showed no statistically significant differences for Blastocystis positivity(P=0.60)and subtype distribution(P=0.15).A statistically significant difference was found between the urticaria patients and the controls for Blastocystis positivity(P<0.01),but not for subtype distribution(P=0.67)or for Blastocystis presence and gastrointestinal complaints.Conclusions:This study on Blastocystis subtype distribution among Turkish urticaria patients showed results consistent with the literature.It was concluded that Blastocystis should be kept in mind in patients with urticaria.展开更多

文摘BACKGROUND This article examines primary hepatic angiosarcoma(PHA)and fat-poor angiomyolipoma(AML),two uncommon vascular cancers.Clinical decisions in these situations are frequently aided by pathology reports and imaging techniques.Uncommon malignant tumors of the vascular endothelium include PHA.Another diagnosis that should not be overlooked when employing contrastenhanced MR and contrast-enhanced computed tomography(CT)imaging techniques is fat-poor AML,one of the uncommon vascular tumors of the liver.In both conditions,biopsy is the primary means of diagnosis.CASE SUMMARY In our article,besides the diagnosis of PHA,fat-poor AML,one of the other rare vascular tumors of the liver,is mentioned.In the case,a 50-year-old female patient with VHL Syndrome was admitted to our hospital with nonspecific lesions such as right upper quadrant pain,weight loss,and nausea.Abdominal ultrasonography(US)revealed a hypoechoic heterogeneous lesion with occasional faint contours.In computed tomography,it was observed as a hyperdense nodular lesion in segment 4.Magnetic resonance imaging(MRI)revealed that the lesion did not contain fat.In connection with the known history of VHL Syndrome,we first evaluated the possibility of AML.Thereupon,a histopathological sample was taken and the diagnosis was made as fat-poor AML with 5%fat content.CONCLUSION In conclusion,PHA in our case report and fat-poor AML in our clinic are two uncommon liver vascular malignancies with comparable incidences.Important imaging techniques like contrast-enhanced US(CEUS),CECT,and CEMRI give us substantial advantages in both cases.However,a biopsy is used to provide the final diagnosis.

文摘The purpose of this review article is to discuss the clinical spectrum of recurrent traumatic anterior shoulder instability with the current concepts and controversies at the scientific level. Because of increasing participation of people from any age group of the population in sports activities, health care professionals dealing with the care of trauma patients must have a thorough understanding of the anatomy, patho-physiology, risk factors, and management of anterior shoulder instability. The risk factors for recurrent shoulder dislocation are young age, participation in high demand contact sports activities, presence of Hill-Sachs or osseous Bankart lesion, previous history of ipsilateral traumatic dislocation, ipsilateral rotator cuff or deltoid muscle insufficiency, and underlying ligamentous laxity. Achieving the best result for any particular patient depends onthe procedure that allows observation of the joint surfaces, provides the anatomical repair, maintains range of motion, and also can be applied with low rates of complications and recurrence. Although various surgical techniques have been described, a consensus does not exist and thus, orthopedic surgeons should follow and try to improve the current evidence-based treatment modalities for the patients.
文摘Spinal cord injury(SCI) is the injury of the spinal cord from the foramen magnum to the cauda equina whichoccurs as a result of compulsion, incision or contusion. The most common causes of SCI in the world are traffic accidents, gunshot injuries, knife injuries, falls and sports injuries. There is a strong relationship between functional status and whether the injury is complete or not complete, as well as the level of the injury. The results of SCI bring not only damage to independence and physical function, but also include many complications from the injury. Neurogenic bladder and bowel, urinary tract infections, pressure ulcers, orthostatic hypotension, fractures, deep vein thrombosis, spasticity, autonomic dysreflexia, pulmonary and cardiovascular problems, and depressive disorders are frequent complications after SCI. SCI leads to serious disability in the patient resulting in the loss of work, which brings psychosocial and economic problems. The treatment and rehabilitation period is long, expensive and exhausting in SCI. Whether complete or incomplete, SCI rehabilitation is a long process that requires patience and motivation of the patient and relatives. Early rehabilitation is important to prevent joint contractures and the loss of muscle strength, conservation of bone density, and to ensure normal functioning of the respiratory and digestive system. An interdisciplinary approach is essential in rehabilitation in SCI, as in the other types of rehabilitation. The team is led by a physiatrist and consists of the patients' family, physiotherapist, occupational therapist, dietician, psychologist, speech therapist, social worker and other consultant specialists as necessary.
文摘Diabetes mellitus(DM) is a systemic and complex disease with micro and macrovascular complications that result from impaired metabolic pathways and genetic susceptibilities. DM has been accepted as an epidemic worldwide during the last two decades. A substantial gap in our knowledge exists regarding the pathophysiology of this metabolic disorder despite the improved diagnostic tools and therapeutic approaches. Sirtuins are a group of NAD+ dependent enzymes that are involved in cellular homeostasis due to their deacetylating activity. In the present review, we aimed to discuss the role of associated sirtuins in the pathogenesis and treatment of diabetes mellitus.
文摘AIM:To determine the effects of Rutin on methanol induced optic neuropathy and compare the results with the effects of ethanol.METHODS:Totally 30 rats were divided into 5 groups,with 6 rats in each group as follows:healthy controls(C),methotrexate(MTX),methotrexate+methanol(MTM),methotrexate+methanol+ethanol(MTME) and methotrexate+ methanol+Rutin(MTMR).In all rabbits except those of the control group,MTX,diluted in sterile serum physiologic,0.3 mg/kg per oral was applied for 7 d by the aid of a tube.After this procedure to the rats of MTM,MTME and MTMR groups,20% methanol with a dose of 3 g/kg per oral was given by the aid of a tube.In MTME group,4 h after the application of methanol,20% ethanol was applied by the same way with a dose of 0.5 g/kg.On the other hand,in MTMR group 4 h after the application of methanol,Rutin,which was dissolved in distilled water,was applied by the same way with a dose of 50 mg/kg.RESULTS:There were statistically significant differences in tissue 8-hydroxy-2 deoxyguanine(8-OHdG),interleukin-1β(IL-1β),tumor necrosis factor-alpha(TNF-α),malondialdehyde(MDA),myeloperoxidase(MPO).glutathione peroxidase(t GSH) and superoxide dismutase(SOD) levels between groups(P〈0.001).In MTMR group tissue 8-OHdG,IL-1β,MDA,and MPO levels were similar with the healthy controls but significantly different than the other groups.In histopathological evaluations,in MTX group there was moderate focal destruction,hemorrhage and decrease innumber of astrocytes and oligodendrocytes;in MTM group there was severe destruction and edema with decrease in number of astrocytes and oligodendrocytes;in MTME group there was mild hemorrhage,mild edema,mildly dilated blood vessels with congestion while in MTMR group,optic nerve tissue was resembling the healthy controls.CONCLUSION:Rutin may prevent methanol-induced optic neuropathy via anti-inflammatory effects and decreasing the oxidative stress.New treatment options are warranted in this disease to avoid loss of vision in patients.
文摘Objective:This descriptive study reveals how nurses working in public hospitals rated their practice environments with respect to nurse-friendly hospital criteria.Methods:This study was conducted on 460 nurses volunteering to participate this study among 735 nurses working in inpatient wards of three public hospitals.Data were collected using a personal information form and the Adapted Nursing Work IndexeRevised.Numbers,percentages,means and standard deviations,t-test and one-way variance analysis were used to evaluate the data.Results:Among the nurse-friendly hospital criteria,‘control of nursing practice’,‘middle management accountability’and‘quality initiatives’had the highest mean scores,and‘competitive wages’had the lowest mean score.The assessments of the nurses presented statistically significant differences with respect to personal and occupational variables.Conclusion:The nurses found most of the nurse-friendly hospital criteria adequate,but they believed that several areas needed improvement.

文摘In this study,Tungsten Oxide(WO_3)thin films were prepared by Chemical Spray Pyrolysis(CSP)and Spin Coating(SC)techniques and it was investigated effects of technique and parameter on the films.WO_3 thin films were deposited on ITO(Indium Tin Oxide)coated glasses.The structural,optical and electrochromic properties of the WO_3 thin films were characterized by XRD,SEM,UV,and CV measurements.The sharpest(200)peak was observed in the XRD spectra and optical band gaps were calculated around 2.6~3.1 eVvia UV-Vis spectra for all of the samples.Micro fibrous reticulated surface(filamentous like)morphology for the films deposited by CSP technique and smooth surface morphology with high optical transmittance for the film deposited by SC Technique were obtained from SEM images.In addition to these results,it was revealed that all the samples exhibit good electrochromic performance.

文摘The ground state hydrogen conformations and vibrational analysis of 3-deazauracil(3DAU)and 6-azauracil(6AU)tautomers(4-enol and 2,4-diol forms)have been calculated using ab initio Hartree-Fock(HF)and density functional theory(B3LYP)methods with 6-311++G(d,p)basis set level.The calculations have shown that the most probably preferential tautomer of 3DAU and 6AU are the 4-enol form,which gives best fit to the corresponding experimental data.The ground state conformer of the 2,4-diol form has two O—H bonds which are oriented externally and internally(to the N—H bond).The vibrational analyses of the ground state conformer of each tautomeric form of 3DAU and 6AU were done and their optimized geometry parameters(bond lengths and bond angles)were given.Furthermore,from the correlations values it was concluded that the B3LYP method is superior to the HF method for both the vibrational frequencies and the geometric parameters.
文摘Diabetes mellitus continues to be a global health problem with increasing importance across the world by affecting the activities of daily living and self-care ability of patients due to its incidence and troubles caused by it. The present study aims at determining the effect of activities of daily living of patients with type 2 diabetes mellitus on their self-care agency. Methods: The population of this descriptive study consisted of patients with type 2 diabetes mellitus who were being treated at the internal medicine clinic of a provincial state hospital between July 2014 and November 2015 and its sample consisted of 150 diabetic patients who volunteered to take part in the study and who were open to communication. A personal information form, the Activities of Daily Living (ADL) Scale, the Instrumental Activities of Daily Living (IADL) Scale and the Self-Care Ability Scale (CAS) were used as data collecting tools. The data were analyzed using descriptive statistics (numbers, percentages, mean, standard deviation, mean rank and frequencies), the Shapiro-Wilk, Kruskal-Wallis Variance Analysis, Mann-Whitney U test and Cronbach’s alpha formula and correlation analysis. Results: Approximately 97.3% of the patients with type 2 diabetes stated that they were independent in ADL and 75.3% of them in IADL. The patients’ mean self-care ability score was found to be 83.85 ± 17.87 ADL and IADL were found to be affected by age, marital status, education, duration of disease, willingness to receive further education, and presence of another disease besides diabetes. There was a significant correlation between the self-care agency score and marital status, education, duration of disease, willingness to receive further education, presence of another disease besides diabetes, regular checking of blood sugar and compliance with diet. A significant positive correlation was found between the patients’ activities of daily living and their self-care agency. Conclusion: The patients’ activities of daily living were found to affect their self-care agency. The personal and disease-related characteristics of patients should be identified so that their self-care behaviors can be increased.

文摘AIM To investigate the role of minimally invasive surgery for gastric cancer and determine surgical, clinical, and oncological outcomes.METHODS This is a propensity score-matched case-control study, comparing three treatment arms: robotic gastrectomy(RG), laparoscopic gastrectomy(LG), open gastrectomy(OG). Data collection started after sharing a specific study protocol. Data were recorded through a tailored and protected web-based system. Primary outcomes: harvested lymph nodes, estimated blood loss, hospital stay, complications rate. Among the secondary outcomes, there are: operative time, R0 resections, POD of mobilization, POD of starting liquid diet and soft solid diet. The analysis includes the evaluation of type and grade of postoperative complications. Detailed information of anastomotic leakages is also provided.RESULTS The present analysis was carried out of 1026 gastrectomies. To guarantee homogenous distribution of cases, patients in the RG, LG and OG groups were 1:1:2 matched using a propensity score analysis with a caliper = 0.2. The successful matching resulted in a total sample of 604 patients(RG = 151; LG = 151; OG = 302). The three groups showed no differences in all baseline patients characteristics, type of surgery(P = 0.42) and stage of the disease(P = 0.16). Intraoperative blood loss was significantly lower in the LG(95.93 ± 119.22) and RG(117.91 ± 68.11) groups compared to the OG(127.26 ± 79.50, P = 0.002). The mean number of retrieved lymph nodes was similar between the RG(27.78 ± 11.45), LG(24.58 ± 13.56) and OG(25.82 ± 12.07) approach. A benefit in favor of the minimally invasive approaches was found in the length of hospital stay(P < 0.0001). A similar complications rate was found(P = 0.13). The leakage rate was not different(P = 0.78) between groups.CONCLUSION Laparoscopic and robotic surgery can be safely performed and proposed as possible alternative to open surgery. The main highlighted benefit is a faster postoperative functional recovery.
文摘BACKGROUND Total exposure to ionizing radiation has nearly doubled in the last two decades.This increase is primarily due to increased computed tomography(CT)exposure.Concerns have been raised about the risks associated with patients'exposure to medical imaging radiation,which can increase a person's lifetime risk of developing cancer.Preventing unnecessary examinations becomes critical at this point.To avoid unnecessary examinations,it is necessary to understand the demanding process.AIM To ascertain clinicians'awareness of and reasons for requesting a CT examination.METHODS We developed an online questionnaire that included 20 questions about clinicians'awareness of radiation safety and their reasons for requesting a CT examination,as well as demographic information such as age,gender,and year of medical practice experience.Additionally,we asked participants the number of CT scans requested in a month,the patients'questions and approaches about the imaging method,the effect of the patient's previous imaging history on the current imaging request,whether they believed that they had sufficient information about radiation doses,and whether they requested CT without an indication.We administered the questionnaire to clinicians from a variety of different professions in four different cities.RESULTS A total of 195 clinicians participated.Internal medicine specialists were the most crowded group(38/195,19.5%).Mean age of the population was 33.66±5.92 years.Mean year of experience was 9.01±5.96.Mean number of requested CT scans in a month was 36.88±5.86.Forty-five(23.1%)participants stated that they requested CT scans without clinical indication.The most common reasons for CT scan requests were work load,fear of malpractice,and patient demand/insistence.CONCLUSION CT scan requests are influenced by a variety of factors,both internal and external to the doctors and patients.Raising awareness of radiation safety and reducing fear of malpractice by limiting the number of patients per physician may result in a reduction in unnecessary CT examinations and ionizing radiation exposure.

文摘Spectra of KX-rays emitted from pure 3 dtransition metals and their oxide and sulfate compounds were measured using a single crystal wavelength dispersive X-ray spectroscopy(WDXRF).The measurements were performed using a ZSX-100 esequential spectrometer equipped with an RhX-ray tube operated.An accurate analytical representation of each line was obtained by a fit to a Lorentz function.The spectra were analyzed in order to examine the KX-ray peak parameters such as asymmetry index,peak energy and relative intensity ratios.The energy shifts and relative intensities weredetermined tobetter understand the chemical effect.

文摘Objective:To investigate Blastocystis’etiologic role and association with gastrointestinal symptomatology in acute and chronic urticaria patients and to identify Blastocystis subtypes responsible for urticaria.Methods:The study included urticaria patients and healthy individuals that presented to our polyclinic between June 2015 and May 2017.The participants were assigned into Group栺(137 patients),subdivided into acute(72)and chronic urticaria patients(65),and Group栻(129 control individuals).Blastocystis presence was investigated by native-Lugol examination,trichrome staining,PCR using sequence tagged site primers,and DNA sequencing analysis.The phylogenetic tree was constructed.Results:The native-Lugol and trichrome staining methods revealed that 16 patients(16/133,12.0%)had Blastocystis-positive stool samples,of which seven samples(7/133,5.3%)belonged acute and nine(9/133,6.8%)to chronic urticaria patients.Concerning Blastocystis subtypes,of the acute urticaria patients,three had subtype 1(ST1),one had ST2,and three had ST3.Of the chronic urticaria patients,one had ST1 and eight had ST3.Blastocystis positivity was detected in two control individuals(2/123,1.6%),both being ST3.All subtypes identified by PCR were confirmed by the sequencing analysis.The acute and chronic urticaria groups showed no statistically significant differences for Blastocystis positivity(P=0.60)and subtype distribution(P=0.15).A statistically significant difference was found between the urticaria patients and the controls for Blastocystis positivity(P<0.01),but not for subtype distribution(P=0.67)or for Blastocystis presence and gastrointestinal complaints.Conclusions:This study on Blastocystis subtype distribution among Turkish urticaria patients showed results consistent with the literature.It was concluded that Blastocystis should be kept in mind in patients with urticaria.
文摘Today,in parallel with the use of imaging modalities increases in all fields,the use of imaging methods in pregnant women is increasing.Imaging has become an integral component of routine pregnancy follow-up.Imaging provides parents with an early opportunity to learn about the current situation,including prenatal detection of anomalies or diseases,etiology,prognosis,and the availability of prenatal or postnatal treatments.Various imaging modalities,especially ultrasonography,are frequently used for imaging both maternal and fetal imaging.The goal of this review was to address imaging modalities in terms of usefulness and safety,as well as to provide demonstrative examples for disorders.And this review provides current information on selecting a safe imaging modality to evaluate the pregnant and the fetus,the safety of contrast medium use,and summarizes major pathological situations with demonstrative son-ographic images to assist radiologists and obstetricians in everyday practice.
文摘The present letter to the editor is related to the study titled“Primary orbital monophasic synovial sarcoma with calcification:A case report”.Orbital synovial sarcoma is one of the rare intraorbital masses seen in adult and pediatric populations.Some case reports in the literature revealed that synovial sarcoma may contain calcifications.Therefore,it is important to make differential diagnosis among calcified orbital masses in childhood.
