BACKGROUND Diabetes is a chronic metabolic syndrome that has become a global public health problem with significant morbidity and mortality. It is a pro-inflammatory and pro-thrombotic condition characterized by increased platelet activation and alterations in platelet indices. However, the use of platelet indices as predictors of poor glucoregulation has not been fully evaluated in this context, and evidence for their role as predictors of poor glycemic status in diabetic patients is limited.AIM To evaluate platelet indices and determine their prognostic significance in relation to inadequate glucoregulation among individuals diagnosed with type 2 diabetes at Bishoftu General Hospital in Ethiopia, from June 15 to August 12,2022.METHODS A comparative cross-sectional study was conducted in 261 participants including 174 individuals with type 2 diabetes mellitus(T2DM)and 87 non-diabetic controls. The systematic random sampling technique was used to select participants.Data were collected using structured questionnaires, physical measurements, checklists, and laboratory tests.Platelet parameters and fasting blood glucose levels were determined from blood samples using Sysmex-XN550 and CobasC311 analyzers, respectively. The hematology analyzer output was checked and participants were also screened for malaria parasites using a prepared blood smear.Collected data were entered into Epi-data version 3.1 and exported to SPSS version 25 for analysis.Theχ^(2) test,Mann-Whitney U test,Kruskal-Wallis test, post hoc test,Spearman correlation, and receiver operating characteristic curve were used for analysis.A P value<0.05 was considered statistically significant.RESULTS The results of our study indicate that diabetic patients have significantly higher levels of platelet distribution width(PDW),mean platelet volume(MPV),platelet large cell ratio(PLCR),and plateletcrit(PCT)compared to healthy individuals(P<0.001).Furthermore,these indices were found to be significantly elevated in individuals with poor glycemic control in T2DM compared to those with good glycemic control and healthy controls. We also observed significant correlations between these indices and various anthropometric and clinical variables. Our findings suggest that PDW,with a cut-off value of 15.75 fL and an area under the curve(AUC)of 0.803,MPV,with a cut-off value of 12.25 fL and an AUC of 0.774,PLCR,with a cut-off value of 36.3%and an AUC of 0.775,and PCT,with a cut-off value of 0.24%and an AUC of 0.761,can serve as predictors of poor glycemic control in patients with diabetes mellitus.CONCLUSION The observed correlation between diabetic patients and a significant increase in platelet indices has highlighted their potential as predictors of poor glycemic control in diabetes. Therefore, regular screening and profiling of platelet indices is recommended as part of the follow-up process for individuals with diabetes mellitus. 展开更多

Objectives This paper aims to investigate the effects of enrollment in the Ethiopian community-based health insurance(CBHI)scheme on household preventive care activities and the timing of treatment-seeking behavior for illness symptoms. There is growing concern about the financial sustainability of CBHI schemes in developing countries. However, few empirical studies have identified potential contributors, including ex-ante and ex-post moral hazards. Methods We implement a household fixed-effect panel data regression model, drawing on three rounds of household survey data collected face to face in districts where CBHI scheme is operational and in districts where it is not operational in Ethiopia. Results The findings show that enrolment in CBHI does not significantly influence household behaviour regarding preventive care activities such as water treatment before drinking and handwashing before meals. However,CBHI significantly increases delay in treatment-seeking behaviour for diseases symptoms.Particularly,on average,we estimate about 4-6 h delay for malaria symptoms,a little above 4 h for tetanus, and 10-11 h for tuberculosis among the insured households. Conclusions While there is evidence that CBHI improve the utilization of outpatient or primary care services, our study suggests that insured members may wait longer before visiting health facilities. This delay could be partly due to moral hazard problems,as insured households, particularly those from rural areas, may consider the opportunity costs associated with visiting health facilities for minor symptoms.Overall,it is essential to identify the primary causes of delays in seeking medical services and implement appropriate interventions to encourage insured individuals to seek early medical attention. 展开更多

BACKGROUND Chronic hepatitis B(CHB)virus infection is a major cause of liver-associated morbidity and mortality, particularly in low-income countries.A better understanding of the epidemiological, clinical, and virological characteristics of CHB will guide appropriate treatment strategies and improve the control and management of CHB in Ethiopia.AIM To investigate the characteristics of CHB in Eastern Ethiopia and assess the efficacy and safety of antiviral treatment.METHODS This cohort study included 193 adults who were human immunodeficiency virus-negative with CHB between June 2016 and December 2019.Baseline assessments included chemistry, serologic, and viral markers.χ^(2) tests,Mann-Whitney U tests, and logistic regression analyses were used to identify the determinants of cirrhosis.Tenofovir disoproxil fumarate(TDF)was initiated using treatment criteria from the Ethiopian CHB pilot program.RESULTS A total of 132 patients(68.4%)were men, with a median age of 30 years[interquartile range(IQR):24-38].At enrollment,60(31.1%)patients had cirrhosis,of whom 35(58.3%)had decompensated cirrhosis.Khat use, hepatitis B envelope antigen positivity, and a high viral load were independently associated with cirrhosis.Additionally,66 patients(33.4%)fulfilled the treatment criteria and 59(30.6%)started TDF.Among 29 patients who completed 24 months of treatment, the median aspartate aminotransferase to platelet ratio index declined from 1.54(IQR:0.66-2.91)to 1.10(IQR:0.75-2.53)(P=0.002),and viral suppression was achieved in 80.9%and 100%of patients after 12 months and 24 months of treatment, respectively.Among the treated patients,12(20.3%)died within the first 6 months of treatment,of whom 8 had decompensated cirrhosis.CONCLUSION This study highlights the high prevalence of cirrhosis, initial mortality, and the efficacy of TDF treatment.Scaling up measures to prevent and control CHB infections in Ethiopia is crucial. 展开更多

Proposed agroforestry options should begin with the species that farmers are most familiar with, which would be the native multipurpose trees that have evolved under smallholder farms and socioeconomic conditions. The African birch(Anogeissus leiocarpa(DC.)Guill.&Perr.)and pink jacaranda(Stereospermum kunthianum Cham.)trees are the dominant species in the agroforestry parkland system in the drylands of Tigray,Ethiopia.Smallholder farmers highly value these trees for their multifunctional uses including timber, firewood, charcoal, medicine, etc. These trees also could improve soil fertility. However, the amount of soil physical and chemical properties enhanced by the two species must be determined to maintain the sustainable conservation of the species in the parklands and to scale up to similar agroecological systems.Hence,we selected twelve isolated trees, six from each species that had similar dendrometric characteristics and were growing in similar environmental conditions. We divided the canopy cover of each tree into three radial distances:mid-canopy,canopy edge, and canopy gap(control). At each distance,we took soil samples from three different depths. We collected 216 soil samples(half disturbed and the other half undisturbed)from each canopy position and soil depth.Bulk density(BD),soil moisture content(SMC),soil organic carbon(SOC),total nitrogen(TN),available phosphorus(AP),available potassium(AK),p H,electrical conductivity(EC),and cation exchange capacity(CEC)were analysed. Results revealed that soil physical and chemical properties significantly improved except for soil texture and EC under both species,CEC under A.leiocarpus, and soil p H under S.kunthianum, all the studied soils were improved under both species canopy as compared with canopy gap.SMC,TN,AP,and AK under canopy of these trees were respectively 24.1%,11.1%,55.0%,and 9.3% higher than those soils under control. The two parkland agroforestry species significantly enhanced soil fertility near the canopy of topsoil through improving soil physical and chemical properties. These two species were recommended in the drylands with similar agro-ecological systems. 展开更多

BACKGROUND Gastroesophageal reflux disease(GERD)is a common disease worldwide with varying clinical presentations and risk factors.Prevalence data for Africa is lacking, but an increasing trend is expected due to demographic and epidemiological transitions.Although endoscopic studies for general gastrointestinal disorders have shown some patients with erosive esophagitis(EE),no studies in Ethiopia have investigated the clinical characteristics, risk factors, and severity of GERD using esophagogastroduodenoscopy(EGD).AIM To assess the clinical features of GERD in Ethiopian patients who underwent EGD and determine the severity and risk factors of EE.METHODS We conducted a multicenter, retrospective cross-sectional study of 221 patients diagnosed with GERD and endoscopic findings of EE at Trauma Associated Severe Hemorrhage and Amniotic Membrane Stem Cell between January 2019 and August 2022.Data were collected from electronic medical records and phone call interviews. We used descriptive statistics and binary logistic regression analysis with SPSS version 26 to identify the association between variables with a statistical significance set at P value<0.05.RESULTS The mean±SD age of the patients was 44.8(±15.9)years,with a male-to-female ratio of 1.6:1.The most commonly reported symptom was epigastric pain(80.5%),followed by heartburn(43%).Los Angeles(LA)-A EE was diagnosed in 71.1%of patients, followed by LA-B(14.9%),LA-C(7.7%),and LA-D(5.9%).Multivariate analysis showed that age 50 or above, presence of bleeding, and endoscopic findings of duodenitis/duodenopathy were significantly associated with severe EE(P<0.05).Stricture and Barrett’s esophagus were observed in 4.5%and 1.36%of patients with EE,respectively.CONCLUSION Most of the patients had milder EE with fewer complications. However, severe EE was more prevalent in older patients and those with duodenitis/duodenopathy. 展开更多

Seasonal rainfall plays a vital role in both environmental dynamics and decision-making for rainfed agriculture in Ethiopia, a country often impacted by extreme climate events such as drought and flooding. Predicting the onset of the rainy season and providing localized rainfall forecasts for Ethiopia is challenging due to the changing spatiotemporal patterns and the country's rugged topography. The Climate Hazards Group Infra Red Precipitation with Station Data(CHIRPS), ERA5-Land total precipitation and temperature data are used from 1981–2022 to predict spatial rainfall by applying an artificial neural network(ANN). The recurrent neural network(RNN) is a nonlinear autoregressive network with exogenous input(NARX), which includes feed-forward connections and multiple network layers, employing the Levenberg Marquart algorithm. This method is applied to downscale data from the European Centre for Medium-range Weather Forecasts fifth-generation seasonal forecast system(ECMWF-SEAS5) and the Euro-Mediterranean Centre for Climate Change(CMCC) to the specific locations of rainfall stations in Ethiopia for the period 1980–2020. Across the stations, the results of NARX exhibit strong associations and reduced errors. The statistical results indicate that, except for the southwestern Ethiopian highlands, the downscaled monthly precipitation data exhibits high skill scores compared to the station records, demonstrating the effectiveness of the NARX approach for predicting local seasonal rainfall in Ethiopia's complex terrain. In addition to this spatial ANN of the summer season precipitation, temperature, as well as the combination of these two variables, show promising results. 展开更多

Mineralogical and geochemical studies have been undertaken on the Triassic to Lower Jurassic Adigrat Sandstone of the Blue Nile Basin of central Ethiopia to infer its source rock type, paleoweathering, and paleoclimatic history. The Adigrat Sandstone occurs at the basal section of the Mesozoic sedimentary formation and unconformably overlays the Neoproterozoic–Paleozoic crystalline rocks,or locally, the Karroo sediments in the northern Blue Nile Basin.A mineralogical study reveals that quartz(Q),feldspars(F),and lithic fragments(L)are the framework grains of the sandstone. On the QFL diagram, the plot of the modal composition of the sandstone mainly falls within the feldspathic arenite and quartzose arenite fields. The geochemical data of the lower section of the sandstone mainly falls within the arkose and subarkose fields, whereas the upper section data falls within the quartzose and sublithic arenite fields.Mineralogical and geochemical weathering indices indicate that the provenances of the Adigrat Sandstone were exposed to pronounced weathering intensity, where the lower part of the sandstone was controlled by arid to semi-arid conditions, whereas the upper section was linked to humid to semi-humid(tropical to subtropical)climatic conditions.Mineralogical and geochemical data also indicate that mafic to intermediate basement rocks were the primary source rocks of the sediment.Perhaps the sediment was assumed to have been reworked by multi-cyclic sedimentary processes. The discriminant function diagram, the REE pattern,La/Th vs.La/Yb,and the Th–Hf–Co plot are consistent.A comparison of provenance studies for the Adigrat Sandstone in the Blue Nile Basin and the Mekele outlier of northern Ethiopia indicates that the sediment of the former is highly sorted, experienced higher weathering intensity, and compositionally derived from mafic to intermediate crystalline rocks. On the other hand, the sediment of the latter is essentially a weathering product of felsic rocks. 展开更多

However, due to human impacts, the forest cover in Ethiopia has been decreasing rapidly. The study was carried out with the purpose of finding out the Regeneration Status, Population Structure and Floristic composition of Woody Plant Species in Sheleko Medihanialem Natural Forest in Gondar, North West Ethiopia, from October 2019 to September 2020. The systematic vegetation sampling method was used to collect data from Fifty plots of 20 m × 20 m (400 m2) along five line transects. In addition, five, 5 m × 5 m subplots were laid within the main plot to sample seedlings and saplings. The floristic composition and population structure of woody individuals of trees and shrubs with a diameter at breast height (DBH) ≥ 2.5 cm and height ≥ 2 m were measured. DBH ≤ 2.5 cm and less than 1 m height were considered as seedlings and DBH ≥ 2.5 cm and height of 1 - 2 m as saplings. Vegetation data of density, frequency, basal area, and importance value index were computed. A total of 65 woody plant species in 54 genera and 34 plant families were recorded. Fabaceae, Moraceae and Euphorbiaceae were the dominant families in terms of species richness. Woody species densities for mature individuals were 2202.5 stems∙ha−1, seedling 2419.2 stems∙ha−1 and sapling 1737.6 stems∙ha−1. The forest was dominated by small-sized/young trees and shrubs, indicating the status of secondary growth and/or regeneration. 展开更多

The study was conducted to determine the applicability of gravity separation method on the Ashashire gold ore deposit Benishangul gumuz region, western Ethiopia. The Ashashire composite was produced to provide sufficient mass for this study and experiment, including sample preparation, mineralogical analysis of gold and associated elements, gravity concentration, and data interpretation and analysis. During the study, a grind optimization was conducted on the composites sample with varying grind size to evaluate the effect of grind size on gold recovery. The ore was moderately ground to the standard grind size of 80%, passing 106 µm, 75 µm, 53 µm and this nominal size was selected for the preliminary assessment for concentration optimization for this deposit. The gravity testing comprised three-stage concentration using Knelson concentrator. High recovery of gold from the gravity concentrates was achieved from the second gravity concentration. Based on the laboratory experimental result analysis, a grind size of P80 75 µm is selected as optimal size for the Ashashire gold deposit. Increasing the grind size from P80 of 75 µm to 106 µm decreases the recovery rate from 75% to 54%, or decreasing the grind size from P80 of 75 µm to 53 µm decreases the gold recovery rate to 37%. The native gold grain in the ores is mostly associated with quartz and fine gold is closely associated with pyrite. According to analysis of the fire assay, chemical, and mineralogical data, only gold and telluride is commercially valuable elements in the ores. Predominantly gold was occurred in the native form of Au-Te. The sample subjected to gravity separation assayed about 2.6 g/t Au. 展开更多

Climate change caused by past, current, and future greenhouse gas emissions has become a major concern for scientists in the field in many countries and regions of the world. This study modelled future precipitation change by downscaling a set of large-scale climate predictor variables(predictors)from the second generation Canadian Earth System Model(CanESM2)under two Representative Concentration Pathway(RCP)emission scenarios(RCP4.5 and RCP8.5)in the semi-arid Borana lowland, southern Ethiopia. The Statistical DownScaling Model(SDSM)4.2.9 was employed to downscale and project future precipitation change in the middle(2036-2065;2050s)and far(2066-2095;2080s)future at the local scale.Historical precipitation observations from eight meteorological stations stretching from 1981 to 1995 and 1996 to 2005 were used for the model calibration and validation, respectively, and the time period of 1981-2018 was considered and used as the baseline period to analyze future precipitation change. The results revealed that the surface-specific humidity and the geopotential height at 500 hPa were the preferred large-scale predictors.Compared to the middle future(2050s),precipitation showed a much greater increase in the far future(2080s)under both RCP4.5 and RCP8.5 scenarios at all meteorological stations(except Teletele and Dillo stations). At Teltele station, the projected annual precipitation will decrease by 26.53%(2050s)and 39.45%(2080s)under RCP4.5 scenario, and 34.99%(2050s)and 60.62%(2080s)under RCP8.5 scenario.Seasonally, the main rainy period would shift from spring(March to May)to autumn(September to November)at Dehas,Dire,Moyale, and Teltele stations, but for Arero and Yabelo stations, spring would consistently receive more precipitation than autumn. It can be concluded that future precipitation in the semi-arid Borana lowland is predicted to differ under the two climate scenarios(RCP4.5 and RCP8.5),showing an increasing trend at most meteorological stations. This information could be helpful for policymakers to design adaptation plans in water resources management, and we suggest that the government should give more attention to improve early warning systems in drought-prone areas by providing dependable climate forecast information as early as possible. 展开更多

Lamb morbidity and mortality cause significant loss in a smallholder production system. A longitudinal prospectivesurvey was conducted on 408 randomly selected farmers located in six purposefully selected kebeles in Gewataworeda from June 2020 to July 2021 to determine the incidence of morbidity and mortality in lambs and to identifyrisk factors. For this purpose, 408 lambs from the Gewata district’s mixed crop-livestock production system wereexamined every day from birth to three months of age. The data was analyzed using the Kaplan-Meier (K-M) method, the log-rank test, and Cox proportional hazards regression. Results showed that the cumulative incidence of allcauseof morbidity and mortality at the end of three month was 12.86% (95% CI: 10.26–16.13%), and 6.86% (95% CI:5.03–9.35%), respectively. Diarrhea was the leading cause of morbidity and mortality, accounting for 49.33 and 50%morbidity and mortality, respectively. Pneumonia was the second most frequent cause of morbidity and death. In theK-M hazard analysis, the greatest risk of lamb morbidity and mortality was observed during the first month of life, andthen the risk decreased significantly as the lamb grew. Of the 17 potential risk factors studied, the multivariable Coxproportional hazards regression model showed that lamb sex, birth weight, umbilical care, time and method of colostrumfeeding were the five predictors that were significantly associated with a higher risk of morbidity, whereas birthweight, lambing difficulty, dams’ parity, method and time of colostrum feeding were the five predictors that weresignificantly associated with a higher risk of mortality. Moreover, a higher risk of morbidity was observed in lambswith methods of colostrum feeding after birth (HR = 3.158;p = 0.000) and with variations in birth weight (HR = 1.418;p = 0.003). Similarly, the mortality risk was 4.926 (p = 0.047), 4.023 (p = 0.012), and 3.206 (p = 0.000) times higher inlambs with lambing difficulties, at the time of colostrum feeding, and by the method of colostrum feeding, respectively.According to this research, lamb morbidity and mortality rates in the study area are significantly high whichhas a great impact on the replacement stock and production. Hence, awareness should be created among farmersconcerning improved lamb management practices. 展开更多

Planting a fast-growing multipurpose Acacia decurrens (AD) tree is one of the climate-smart agricultural practices that have been promoted in Ethiopia, which is widely practiced and an important livelihood strategy in Awi zone. However, the extent of its adoption varies considerably among households in the study area. This study investigated the determinants of intensity of adoption of AD among 385 randomly selected rural households in Awi Zone. Data were gathered using a cross-sectional household survey. Descriptive statistics and Two-limit Tobit model were employed for data analysis. The result of the study shows that on average the intensity of adoption of AD was 0.43 (43% of the total cropland area), though majority of the farmers (48.8%) belong to low level of adoption. The result of the analysis shows that being male, educational level, access to seedling, experience in growing the tree, extension contact has positive and significant relationship with the intensity of adoption. Age of head of household, land holding size, livestock holding size, soil fertility status, disease emergence and road distance have negative and significant influence on intensity of adoption. These suggest that expanding road infrastructure, education, access to seedling, secure land property right, disease management, and provision of extension services related to the AD can also improve smallholder farmers’ intensity of AD adoption. The generated information provides a picture of the study area’s situation to the attention of policy makers, development practitioners and institutional service providers to formulate a better policy intervention to sustain smallholder farmers’ AD plantation. 展开更多

Environmental conditions determine woody plant life such as species diversity, structure and regeneration status. This research aimed to assess the impact of environmental conditions on woody plant species diversity, structure and regeneration in forest patches of Guna Mountain:the case of Este District,South Gondar Zone,north-west Ethiopia.A total of 71 square sample plots(400 m~2 each) were established at 10 transects laid in five forest patches of the study area to collect vegetation data including the abundance of species, height and diameter at breast height(DBH).Environmental data including available potassium, available phosphorus, cation exchange capacity, soil texture, electrical conductivity, soil acidity, total nitrogen, organic matter, organic carbon, sodium adsorption ratio, exchangeable sodium percentage, bulk density, aspect, elevation, slope, latitude and longitude data were collected in the same plots.A correlation analysis between vegetation and environmental data was performed using rcorr(x) function in package Hmisc in R Programming Language. The most pronounced impacts were observed in altitude, grazing and pH,positively and silt, sand, electrical conductivity and cation exchange capacity, negatively. In addition, strong and significant impacts on plant structure were also observed due to the variation in soil texture and p H.The difference in aspect, sand and slope also impacted plant regeneration. The study showed that environmental parameters influence the diversity, structure and regeneration of woody plants. These parameters can be considered in the rehabilitation of the vegetation cover and conservation efforts of the rare woody species.Conservation measures that can minimalize the negative influences of environmental conditions can be applied through collaboration with communities around the forest patches. 展开更多

Studies on mainstreaming climate-smart agriculture(CSA)practices can increase smallholder farmers’capacity and awareness to improve food security and establish sustainable livelihoods through resilient agricultural systems, while achieving adaptation and mitigation benefits.Hence, valuable insights can be obtained from smallholder farmers in responding to present and forthcoming challenges of climate change impacts. However, there is little research work on trade-off and synergy assessments.Taking Geshy watershed in Southwest Ethiopia as a case study area, both quantitative and qualitative data analysis were undertaken in this study. The data were collected from 15 key informant interviews,6 focus group discussions, and 384 households to answer the following questions:(1)what are the top 5 preferred CSA practices for smallholder farmers in Geshy watershed when coping with the impacts of climate change?(2)What is the performance of the preferred CSA practices?And(3)which trade-offs and synergies are experienced upon the implementation of CSA practices? The study came up with the most preferred CSA practices such as the use of improved crop varieties,small-scale irrigation, improved animal husbandry, the use of efficient inorganic fertilizers, and crop rotation with legumes. The selected CSA practices showed that the productivity goal exhibit the best synergy, while the mitigation goal has trade-offs.The study also indicated that the use of improved crop varieties causes high synergies in all 3 goals of CSA practices;small-scale irrigation provides a medium synergy on productivity goal but high synergy for adaptation and mitigation goals;improved animal husbandry shows a high synergy with the adaptation goal,a relatively lower synergy with the productivity goal, and a trade-off with the mitigation goal;the use of efficient inorganic fertilizers shows maximum synergy for the productivity and adaptation goals;and crop rotation with legumes exhibits high synergy with the productivity and mitigation goals but a relatively lower synergy with the adaptation goal. These results can provide evidence to various stakeholder farmers in the value chain that the impacts of climate change can be addressed by the adoption of CSA practices. In general,CSA practices are considered indispensable.Smallholder farmers prefer CSA practices that help to increase crop productivity and household resilience to climate change impacts. The results generate a vital foundation for recommendations to smallholder farming decision-makers.It also sensitizes actions for innovative and sustainable methods that are able to upscale the preferred CSA practices in the agricultural system in Geshy watershed of Southwest Ethiopia and other regions. 展开更多
