BACKGROUND Recent studies on dialysis anticoagulation therapy in patients with renal failure have shown that Nafamostat mesylate,a broad-spectrum potent serine protease inhibitor,has strong anticoagulation and anti-fi...BACKGROUND Recent studies on dialysis anticoagulation therapy in patients with renal failure have shown that Nafamostat mesylate,a broad-spectrum potent serine protease inhibitor,has strong anticoagulation and anti-fiber activity.AIM To evaluate the efficacy and safety of Nafamostat mesylate in patients with end-stage renal failure.METHODS Seventy-five patients with end-stage renal failure who received hemodialysis at our hospital between January 2020 and August 2021 were selected and divided into the observation group(Nafamostat mesylate for injection,n=33)and control group(heparin sodium injection,n=32).General patient data,indicators of clinical efficacy,dialyzer hemocoagulation parameters,coagulation function indices,and hemoglobin concentration and platelet count before and after treatment,and the occurrence of adverse reactions after treatment were compared between the two groups.RESULTS The two groups showed no significant differences in general patient data(P>0.05).The post-treatment effectiveness rate in the control group was lower than that in the observation group(P<0.05).The two groups showed no significant difference in the number of patients in grade I(P>0.05),while the number of patients in grade 0 was lower in the control group,and the number of patients in grades II and III was higher in the control group(P<0.05).The post-treatment prothrombin time,activated partial thromboplastin time,thrombin time,and international normalized ratio values in the control group were higher than those in the observation group,while the fibrinogen level in the control group was lower than that in the observation group(P<0.05).The two groups showed no significant difference in the platelet count and hemoglobin level before and after treatment(P>0.05).The total number of post-treatment adverse reactions in the observation group was lower than that in the control group(P<0.05).CONCLUSION Treatment of patients showing end-stage renal failure with Nafamostat mesylate can significantly improve therapeutic efficacy and has high safety and clinical value.展开更多
BACKGROUND Myocardial infarction is a high-risk condition prevalent among the elderly population,often leading to adverse clinical manifestations such as reduced cardiopulmonary function,anxiety,and depression post-su...BACKGROUND Myocardial infarction is a high-risk condition prevalent among the elderly population,often leading to adverse clinical manifestations such as reduced cardiopulmonary function,anxiety,and depression post-surgery.Consequently,cardiac rehabilitation holds immense importance in mitigating these complications.AIM To evaluate the effect of individualized cardiac rehabilitation on blood pressure variability(BPV)and baroreflex sensitivity(BRS)in elderly patients with myocardial infarction.METHODS A cohort of 74 elderly patients diagnosed with myocardial infarction and admitted to our hospital between January 2021 and January 2022 were subjected to random selection.Subsequently,all patients were divided into two groups,namely the research group(n=37)and the control group(n=37),utilizing the number table method.The control group received conventional drug treatment and nursing guidance intervention,while the study group underwent individualized cardiac rehabilitation in addition to the interventions received by the control group.All patients were continuously intervened for 12 wk,and the BPV of these two groups in the 1st wk(T0),the 4th wk(T1)and the 12th wk(T2)were compared,BRS,changes in cardiopulmonary function measures,and adverse cardiovascular events.RESULTS Of 24 h diastolic BPV,24 h systolic BPV,carbon dioxide ventilation equivalent slope of the research group were lower than those of the control group at T1 and T2,BRS,peak heart rate and systolic blood pressure product,1 min heart rate recovery were higher than those of the control group,and the incidence of adverse events in the research group was lower than that of the control group,the difference was statistically significant(P<0.05).CONCLUSION In this study,we found that after individualized cardiac rehabilitation in elderly patients with myocardial infarction,BPV and BRS can be effectively improved,cardiac function is significantly enhanced,and a better prognosis is obtained.展开更多
BACKGROUND Speech disorders have a substantial impact on communication abilities and quality of life.Traditional treatments such as speech and psychological therapies frequently demonstrate limited effectiveness and p...BACKGROUND Speech disorders have a substantial impact on communication abilities and quality of life.Traditional treatments such as speech and psychological therapies frequently demonstrate limited effectiveness and patient compliance.Transcranial electrical stimulation(TES)has emerged as a promising non-invasive treatment to improve neurological functions.However,its effectiveness in enhancing language functions and serum neurofactor levels in individuals with speech disorders requires further investigation.AIM To investigate the impact of TES in conjunction with standard therapies on serum neurotrophic factor levels and language function in patients with speech disorders.METHODS In a controlled study spanning from March 2019 to November 2021,81 patients with speech disorders were divided into a control group(n=40)receiving standard speech stimulation and psychological intervention,and an observation group(n=41)receiving additional TES.The study assessed serum levels of ciliary neurotrophic factor(CNTF),glial cell-derived neurotrophic factor(GDNF),brainderived neurotrophic factor(BDNF),and nerve growth factor(NGF),as well as evaluations of motor function,language function,and development quotient scores.RESULTS After 3 wk of intervention,the observation group exhibited significantly higher serum levels of CNTF,GDNF,BDNF,and NGF compared to the control group.Moreover,improvements were noted in motor function,cognitive function,language skills,physical abilities,and overall development quotient scores.It is worth mentioning that the observation group also displayed superior perfor CONCLUSION This retrospective study concluded that TES combined with traditional speech and psychotherapy can effectively increase the levels of neurokines in the blood and enhance language function in patients with speech disorders.These results provide a promising avenue for integrating TES into standard treatment methods for speech disorders.展开更多
BACKGROUND Regarding the incidence of malignant tumors in China,the incidence of liver cancer ranks fourth,second only to lung,gastric,and esophageal cancers.The case fatality rate ranks third after lung and cervical ...BACKGROUND Regarding the incidence of malignant tumors in China,the incidence of liver cancer ranks fourth,second only to lung,gastric,and esophageal cancers.The case fatality rate ranks third after lung and cervical cancer.In a previous study,the whole-process management model was applied to patients with breast cancer,which effectively reduced their negative emotions and improved treatment adherence and nursing satisfaction.METHODS In this single-center,randomized,controlled study,60 randomly selected patients with liver cancer who had been admitted to our hospital from January 2021 to January 2022 were randomly divided into an observation group(n=30),who received whole-process case management on the basis of routine nursing mea-sures,and a control group(n=30),who were given routine nursing measures.We compared differences between the two groups in terms of anxiety,depression,the level of hope,self-care ability,symptom distress,sleep quality,and quality of life.RESULTS Post-intervention,Hamilton anxiety scale,Hamilton depression scale,memory symptom assessment scale,and Pittsburgh sleep quality index scores in both groups were lower than those pre-intervention,and the observation group had lower scores than the control group(P<0.05).Herth hope index,self-care ability assessment scale-revision in Chinese,and quality of life measurement scale for patients with liver cancer scores in both groups were higher than those pre-intervention,with higher scores in the observation group compared with the control group(P<0.05).CONCLUSION Whole-process case management can effectively reduce anxiety and depression in patients with liver cancer,alleviate symptoms and problems,and improve the level of hope,self-care ability,sleep quality,and quality of life,as well as provide feasible nursing alternatives for patients with liver cancer.展开更多

We investigated the effect of DDPH on vasodilatation and neuroprotection in a rat model of cerebral ischemiain vivo, and a rabbit model of isolated basilar arteriesin vitro. Our results show that DDPH (10 mg/kg) significantly increased hippocampal blood flowin vivo in cerebral ischemic rats, and exerted dose-dependent relaxation of isolated basilar arteries contracted by histamine or KCl in thein vitro rabbit model. DDPH (3 × 10–5 M) also inhibited histamine-stimulated extracellular calcium inlfux and intracellular calcium release. Our ifndings suggest that DDPH has a vasodilative effect bothin vivo andin vitro, which mediates a neuropro-tective effect on ischemic nerve tissue.展开更多
We here aimed to investigate the impact of gender on the clinical characteristics and laboratory results of patients with coronavirus disease 2019(COVID-19)and provide clues to the pathological mechanisms underlying C...We here aimed to investigate the impact of gender on the clinical characteristics and laboratory results of patients with coronavirus disease 2019(COVID-19)and provide clues to the pathological mechanisms underlying COVID-19.A retrospective study was performed.Clinical characteristics,severity of lung infection,laboratory results,and prognoses of patients of different gender were analyzed.A total of 242 patients were finally included.The median age was 58 years(IQR:40-68),including 54(22.3%)hospital staffs.Ninety-four(38.8%)were male and 148(61.1%)were female.The proportion of patients with diabetes was significantly higher in the male group than in the female group(P=0.034).Male patients had a significantly larger proportion of severe lung infection,higher leukocyte count,neutrophil count,neutrophil-to-lymphocyte ratio,C-reactive protein,and procalcitonin than femalc.Furthermore,male patients had worse liver,cardiac,and coagulation function than their female counterparts.Male patients with COVID-19 showed more severe inflammation reaction and coagulation dysfunction than female patients.In conclusion,gender is associated with host response to SARS-CoV-2 infection.展开更多
BACKGROUND Primary intracranial alveolar soft-part sarcoma(PIASPS)is a rare malignancy.We aimed to investigate the clinical profiles and outcomes for PIASPS.CASE SUMMARY We firstly reported five consecutive cases from...BACKGROUND Primary intracranial alveolar soft-part sarcoma(PIASPS)is a rare malignancy.We aimed to investigate the clinical profiles and outcomes for PIASPS.CASE SUMMARY We firstly reported five consecutive cases from our institute.Then,the cases from previous studies were pooled and analyzed to delineate the characteristics of this disease.Our cohort included two males and three females.The median age was 21-years-old(range:8-54-years-old).All the patients received surgical treatment.Gross total resection(GTR),radiotherapy,and chemotherapy were administered in 3 patients,4 patients,and 1 patient,respectively.After a median follow-up of 36 mo,tumor progression was noticed in 4 patients;and 3 patients died of the disease.Pooled data(n=14)contained 5 males and 9 females with a median age of 19 years.The log-rank tests showed that GTR(P=0.011)could prolong progression-free survival,and radiotherapy(P<0.001)resulted in longer overall survival.CONCLUSION Patients with PIASPS suffer from poor outcomes.Surgical treatment is the first choice,and GTR should be achieved when the tumor is feasible.Patients with PIASPS benefit from radiotherapy,which should be considered as a part of treatment therapies.展开更多
BACKGROUND The global outbreak of human severe acute respiratory syndrome coronavirus(SARS-CoV)-2 infection represents an urgent need for readily available,accurate and rapid diagnostic tests.Nucleic acid testing of r...BACKGROUND The global outbreak of human severe acute respiratory syndrome coronavirus(SARS-CoV)-2 infection represents an urgent need for readily available,accurate and rapid diagnostic tests.Nucleic acid testing of respiratory tract specimens for SARS-CoV-2 is the current gold standard for diagnosis of coronavirus disease 2019(COVID-19).However,the diagnostic accuracy of reverse transcription polymerase chain reaction(RT-PCR)tests for detecting SARS-CoV-2 nucleic acid may be lower than optimal.The detection of SARS-CoV-2-specific antibodies should be used as a serological non-invasive tool for the diagnosis and management of SARS-CoV-2 infection.AIM To investigate the diagnostic value of SARS-CoV-2 IgM/IgG and nucleic acid detection in COVID-19.METHODS We retrospectively analyzed 652 suspected COVID-19 patients,and 206 non-COVID-19 patients in Wuhan Integrated TCM and Western Medicine Hospital.Data on SARS-CoV-2 nucleic acid tests and serum antibody tests were collected to investigate the diagnostic value of nucleic acid RT-PCR test kits and immunoglobulin(Ig)M/IgG antibody test kits.The j2 test was used to compare differences between categorical variables.A 95%confidence interval(CI)was provided by the Wilson score method.All analyses were performed with IBM SPSS Statistics version 22.0(IBM Corp.,Armonk,NY,United States).RESULTS Of the 652 suspected COVID-19 patients,237(36.3%)had positive nucleic acid tests,311(47.7%)were positive for IgM,and 592(90.8%)were positive for IgG.There was a significant difference in the positive detection rate between the IgM and IgG test groups(P<0.001).Using the RT-PCR results as a reference,the specificity,sensitivity,and accuracy of IgM/IgG combined tests for SARS-CoV-2 infection were 98.5%,95.8%,and 97.1%,respectively.Of the 415 suspected COVID-19 patients with negative nucleic acid test results,366 had positive IgM/IgG tests with a positive detection rate of 88.2%.CONCLUSION Our data indicate that serological IgM/IgG antibody combined test had high sensitivity and specificity for the diagnosis of SARS-CoV-2 infection,and can be used in combination with RT-PCR for the diagnosis of SARS-CoV-2 infection.展开更多
BACKGROUNDWith the incidence of pancreatic diseases increasing year by year,pancreatichyperglycemia,as one of the common complications,is gradually gaining attentionfor its impact on the skin health of patients.CASE S...BACKGROUNDWith the incidence of pancreatic diseases increasing year by year,pancreatichyperglycemia,as one of the common complications,is gradually gaining attentionfor its impact on the skin health of patients.CASE SUMMARYThis was the case of an elderly female with clinical manifestations of necrolyticmigratory erythema,“three more and one less,”diabetes mellitus,hypertension,anemia,hypoproteinemia,and other syndromes,which had been misdiagnosedas eczema.Abdominal computed tomography showed a pancreatic caudal spaceoccupyinglesion,and the magnetic resonance scanning of the epigastric regionwith dynamic enhancement and diffusion-weighted imaging suggested a tumorof the pancreatic tail,which was considered to be a neuroendocrine tumor orcystadenoma.The patient was referred to a more equipped hospital for laparoscopicpancreatic tail resection.Post-surgery diagnosis revealed a neuroendocrinetumor in the tail of the pancreas.To date,the patient’s general condition is good,and she is still under close follow-up.CONCLUSIONNecrolytic migratory erythema can be induced by endocrine system tumors orendocrine metabolic abnormalities,with complex clinical manifestations,difficultdiagnosis,and easy misdiagnosis by dermatologists.The initial treatment principlesin dermatology include symptomatic supportive therapy and effectivedrugs to relieve skin lesions.After clarifying the etiology of glucagonoma,comprehensive treatment in collaboration with endocrinologists,generalsurgeons,and oncologists can help provide individualized treatment for patientsand improve their prognosis.展开更多

Summary:This case series aimed to describe the clinical characteristics of severely or critically ill patients with COVID-19 and compare the clinical characteristics of patients who received invasive respiratory suppo...Summary:This case series aimed to describe the clinical characteristics of severely or critically ill patients with COVID-19 and compare the clinical characteristics of patients who received invasive respiratory support with those of patients who received noninvasive respiratory support.We included all confirmed severe or critical illness cases of COVID-19 admitted to the Intensive Care Unit(ICU)of Zhongnan Hospital of Wuhan University,a COVID-19-designated hospital,from January 8 to March 12,2020.Cases were analyzed for cpidemiological,demographic,clinical,APACHE II,SOFA,radiological features and laboratory data.Outcomes of all patients were followed up as of March 12,2020.This newly emerging virus had caused 55 confirmed severe or critical illness cases in ICU of a COVID-19-designated hospital.Most of the infected patients were men;more than half had underlying diseases,including hypertension,coronary artery disease and diabetes.The median age was 63 years old.Common symptoms at onset of ilness were fever,fatigue and dry cough.Five(9.1%)hospitalized patients were presumed to have been infected in the hospital,and 4(7.3%)health care workers were infected in their work.Of the 55 confirmed severe or critical ilncss cases,10(18.2%)patients died during the follow-up period as of March 12 with the median follow-up period of 28 days(interquartile range 1635).Nine patients received VV-ECMO for severe respiratory failure and 4(44.4%)patients died.Moreover,28 patients received invasive respiratory support and 14(50.0%)patients died.In this single-center study,55 severely or critically ill ICU patients were confirmed to have COVID-19 in Wuhan and the overall mortality was 29.1%。Totally 28(50.9%)of severely or critically ill ICU patients received invasive respiratory support and 14(50.0%)died during the follow-up period.展开更多

DDPH (3 × 10–5 M) also inhibited histamine-stimulated extracellular calcium inlfux and intracellular calcium release. Our ifndings suggest that DDPH has a vasodilative effect bothin vivo andin vitro, which mediates a neuropro-tective effect on ischemic nerve tissue.

文摘We here aimed to investigate the impact of gender on the clinical characteristics and laboratory results of patients with coronavirus disease 2019(COVID-19)and provide clues to the pathological mechanisms underlying COVID-19.A retrospective study was performed.Clinical characteristics,severity of lung infection,laboratory results,and prognoses of patients of different gender were analyzed.A total of 242 patients were finally included.The median age was 58 years(IQR:40-68),including 54(22.3%)hospital staffs.Ninety-four(38.8%)were male and 148(61.1%)were female.The proportion of patients with diabetes was significantly higher in the male group than in the female group(P=0.034).Male patients had a significantly larger proportion of severe lung infection,higher leukocyte count,neutrophil count,neutrophil-to-lymphocyte ratio,C-reactive protein,and procalcitonin than femalc.Furthermore,male patients had worse liver,cardiac,and coagulation function than their female counterparts.Male patients with COVID-19 showed more severe inflammation reaction and coagulation dysfunction than female patients.In conclusion,gender is associated with host response to SARS-CoV-2 infection.
文摘BACKGROUND Primary intracranial alveolar soft-part sarcoma(PIASPS)is a rare malignancy.We aimed to investigate the clinical profiles and outcomes for PIASPS.CASE SUMMARY We firstly reported five consecutive cases from our institute.Then,the cases from previous studies were pooled and analyzed to delineate the characteristics of this disease.Our cohort included two males and three females.The median age was 21-years-old(range:8-54-years-old).All the patients received surgical treatment.Gross total resection(GTR),radiotherapy,and chemotherapy were administered in 3 patients,4 patients,and 1 patient,respectively.After a median follow-up of 36 mo,tumor progression was noticed in 4 patients;and 3 patients died of the disease.Pooled data(n=14)contained 5 males and 9 females with a median age of 19 years.The log-rank tests showed that GTR(P=0.011)could prolong progression-free survival,and radiotherapy(P<0.001)resulted in longer overall survival.CONCLUSION Patients with PIASPS suffer from poor outcomes.Surgical treatment is the first choice,and GTR should be achieved when the tumor is feasible.Patients with PIASPS benefit from radiotherapy,which should be considered as a part of treatment therapies.

文摘BACKGROUND The global outbreak of human severe acute respiratory syndrome coronavirus(SARS-CoV)-2 infection represents an urgent need for readily available,accurate and rapid diagnostic tests.Nucleic acid testing of respiratory tract specimens for SARS-CoV-2 is the current gold standard for diagnosis of coronavirus disease 2019(COVID-19).However,the diagnostic accuracy of reverse transcription polymerase chain reaction(RT-PCR)tests for detecting SARS-CoV-2 nucleic acid may be lower than optimal.The detection of SARS-CoV-2-specific antibodies should be used as a serological non-invasive tool for the diagnosis and management of SARS-CoV-2 infection.AIM To investigate the diagnostic value of SARS-CoV-2 IgM/IgG and nucleic acid detection in COVID-19.METHODS We retrospectively analyzed 652 suspected COVID-19 patients,and 206 non-COVID-19 patients in Wuhan Integrated TCM and Western Medicine Hospital.Data on SARS-CoV-2 nucleic acid tests and serum antibody tests were collected to investigate the diagnostic value of nucleic acid RT-PCR test kits and immunoglobulin(Ig)M/IgG antibody test kits.The j2 test was used to compare differences between categorical variables.A 95%confidence interval(CI)was provided by the Wilson score method.All analyses were performed with IBM SPSS Statistics version 22.0(IBM Corp.,Armonk,NY,United States).RESULTS Of the 652 suspected COVID-19 patients,237(36.3%)had positive nucleic acid tests,311(47.7%)were positive for IgM,and 592(90.8%)were positive for IgG.There was a significant difference in the positive detection rate between the IgM and IgG test groups(P<0.001).Using the RT-PCR results as a reference,the specificity,sensitivity,and accuracy of IgM/IgG combined tests for SARS-CoV-2 infection were 98.5%,95.8%,and 97.1%,respectively.Of the 415 suspected COVID-19 patients with negative nucleic acid test results,366 had positive IgM/IgG tests with a positive detection rate of 88.2%.CONCLUSION Our data indicate that serological IgM/IgG antibody combined test had high sensitivity and specificity for the diagnosis of SARS-CoV-2 infection,and can be used in combination with RT-PCR for the diagnosis of SARS-CoV-2 infection.
文摘BACKGROUNDWith the incidence of pancreatic diseases increasing year by year,pancreatichyperglycemia,as one of the common complications,is gradually gaining attentionfor its impact on the skin health of patients.CASE SUMMARYThis was the case of an elderly female with clinical manifestations of necrolyticmigratory erythema,“three more and one less,”diabetes mellitus,hypertension,anemia,hypoproteinemia,and other syndromes,which had been misdiagnosedas eczema.Abdominal computed tomography showed a pancreatic caudal spaceoccupyinglesion,and the magnetic resonance scanning of the epigastric regionwith dynamic enhancement and diffusion-weighted imaging suggested a tumorof the pancreatic tail,which was considered to be a neuroendocrine tumor orcystadenoma.The patient was referred to a more equipped hospital for laparoscopicpancreatic tail resection.Post-surgery diagnosis revealed a neuroendocrinetumor in the tail of the pancreas.To date,the patient’s general condition is good,and she is still under close follow-up.CONCLUSIONNecrolytic migratory erythema can be induced by endocrine system tumors orendocrine metabolic abnormalities,with complex clinical manifestations,difficultdiagnosis,and easy misdiagnosis by dermatologists.The initial treatment principlesin dermatology include symptomatic supportive therapy and effectivedrugs to relieve skin lesions.After clarifying the etiology of glucagonoma,comprehensive treatment in collaboration with endocrinologists,generalsurgeons,and oncologists can help provide individualized treatment for patientsand improve their prognosis.

文摘Eukaryotic initiation factor 5A2(eIF5A2),as one of the two isoforms in the family,is reported to be a novel oncogenic protein that is involved in multiple aspects of many types of human cancer.Overexpression or gene amplification of EIF5A2 has been demonstrated in many cancers.Accumulated evidence shows that eIF5A2 initiates tumor formation,enhances cancer cell growth,increases cancer cell metastasis,and promotes treatment resistance through multiple means,including inducing epithelial–mesenchymal transition,cytoskeletal rearrangement,angiogenesis,and metabolic reprogramming.Expression of eIF5A2 in cancer correlates with poor survival,advanced disease stage,as well as metastasis,suggesting that eIF5A2 function is crucial for tumor development and maintenance but not for normal tissue homeostasis.All these studies suggest that eIF5A2 is a useful biomarker in the prediction of cancer prognosis and serves as an anticancer molecular target.This review focuses on the expression,subcellular localization,post-translational modifications,and regulatory networks of eIF5A2,as well as its biochemical functions and evolving clinical applications in cancer,especially in human digestive system neoplasms.
文摘Summary:This case series aimed to describe the clinical characteristics of severely or critically ill patients with COVID-19 and compare the clinical characteristics of patients who received invasive respiratory support with those of patients who received noninvasive respiratory support.We included all confirmed severe or critical illness cases of COVID-19 admitted to the Intensive Care Unit(ICU)of Zhongnan Hospital of Wuhan University,a COVID-19-designated hospital,from January 8 to March 12,2020.Cases were analyzed for cpidemiological,demographic,clinical,APACHE II,SOFA,radiological features and laboratory data.Outcomes of all patients were followed up as of March 12,2020.This newly emerging virus had caused 55 confirmed severe or critical illness cases in ICU of a COVID-19-designated hospital.Most of the infected patients were men;more than half had underlying diseases,including hypertension,coronary artery disease and diabetes.The median age was 63 years old.Common symptoms at onset of ilness were fever,fatigue and dry cough.Five(9.1%)hospitalized patients were presumed to have been infected in the hospital,and 4(7.3%)health care workers were infected in their work.Of the 55 confirmed severe or critical ilncss cases,10(18.2%)patients died during the follow-up period as of March 12 with the median follow-up period of 28 days(interquartile range 1635).Nine patients received VV-ECMO for severe respiratory failure and 4(44.4%)patients died.Moreover,28 patients received invasive respiratory support and 14(50.0%)patients died.In this single-center study,55 severely or critically ill ICU patients were confirmed to have COVID-19 in Wuhan and the overall mortality was 29.1%。Totally 28(50.9%)of severely or critically ill ICU patients received invasive respiratory support and 14(50.0%)died during the follow-up period.
