Objective:Though Chinese medicine(CM)has showed its clinical efficacy for the treatment of obstructive sleep apnea(OSA)in China,no systematic reviews or meta-analyses provide evidences for its therapeutic effects on OSA and the long-term safety. The aim of the present study is to evaluate the effectiveness and safety of CM on OSA using meta-analysis.Methods:We used search items of"Chinese Medicine"AND"obstructive sleep apnea"to retrieve the randomized control trials(RCTs)of CM treatments for OSA in PubMed,Embase,Cochrane Library,China National Knowledge Infrastructure(CNKI),VIP database, and Wanfang database from their respective inception dates to December 2019.Only RCTs of CM therapy versus no treatment which could be quantitatively synthesized were included.Finally,20 studies representing 1,297 participants were included after extraction. Two investigators independently extracted and analyzed the data using RevMan5.3 software. Results:The treatment group using CM decoctions or CM granules presented significantly ameliorative effects on apnoea-hypopnoea index(AHI)compared with the control group(MD:-2.58,95%CI:-3.59 to-1.56,P<0.00001 and MD:-5.47,95%CI:-6.75 to-4.19,P<0.00001,respectively)in the sensitivity analysis. However, there were non significant differences in the duration of treatment between subgroups, indicating that the duration of treatment has no impacts on the therapeutic effects on AHI.CM granules also showed significantly ameliorative effects on the lowest nocturnal oxygen saturation(LSaO2)(MD:2.76,95%CI:1.85 to 3.68,P<0.00001).CM decoctions exhibited significantly improved Epworth Sleepiness Scale(ESS)scores compared with the control group in a sensitivity analysis(MD:-1.50,95%CI:-2.13 to-0.88,P<0.00001).CM granules showed a better improvement of ESS than the control group(MD:-1.35,95%CI:-1.92 to-0.78,P<0.00001).Mild adverse reactions occurred only in five patients and disappeared without special treatment. Conclusion:This study showed favorable therapeutic efficacy of CM on OSA.However,in consideration of the low methodological quality of the included RCTs,more rigorous designed, large sample size RCTs are recommended for providing more high-quality evidences. 展开更多

BACKGROUND: Delayed neuronal death after total cerebral ischemia may accompany with apoptosis, but acupuncture may play a certain role in protecting nerve through inhibiting ischemic neuronal apoptosis. OBJECTIVE: To observe the effect of acupuncture on neuronal apoptosis in rats after cerebral ischemia and analyze its cerebral protective mechanism. DESIGN: Contrast observation among groups. SETTING: Heilongjiang University of Traditional Chinese Medicine. MATERIALS: A total of 30 male healthy Wistar rats of general grade and weighing (250±20) g were randomly divided into three groups, including sham operation group, cerebral ischemia group and acupuncture group with 10 rats in each group. Apoptosis in situ kit was provided by Baolingman Company, Germany. METHODS: The experiment was carried out in the Laboratory Center, Heilongjiang University of Traditional Chinese Medicine from May to November 2004. ① Rats in the cerebral ischemia group and the acupuncture group were used to establish total cerebral ischemic models with four vessels occlusion; in addition, models in the sham operation group were established with the same method as mentioned above. However, four vessels of rats in the sham operation were exposured and cerebral ischemia did not occur. Rats in the acupuncture group were given acupuncture treatment after operation. Needle of 40 mm in length was used to acupuncture bilateral Zusanli (St 36) and Quchi (LI 11) with the depth of 3 mm, and then bilateral acupoints were connected with KWD-808Ⅱ omnipotenc impulse electro-therapeutic apparatus (frequency: 1 Hz; thin waves; voltage: 2 V) once a day for totally 30 minutes. Meanwhile, needle of 25 mm in length was used to acupuncture Baihui (Du 20) with the depth of 3 mm, and then the needle was twirled once every 5 minutes for 30 minutes in total. The course was 7 days. ② Neuronal injuries in hippocampal CAI area after cerebral ischemia were observed with Nissl body staining method at 7 days after treatment; neuronal apoptosis was observed with TUNEL staining; manifestations of neuronal apoptosis in cerebral cortex and hippocampal CAI area were observed with electron microscope. MAIN OUTCOME MEASURES: Neuronal injuries in hippocampal CAI area after cerebral ischemia; neuronal apoptosis in cerebral cortex and hippocampal CAI area after cerebral ischemia; morphological changes under electron microscope. RESULTS: Among 30 Wistar rats, 24 rats were involved in the final analysis. ① Expression of positive neurons in cerebral cortex and hippocampal CAI area with Nissl body staining: Neuronal defect was obvious in cerebral cortex and hippocampal CAI area in the cerebral ischemia group as compared with that in the sham operation group (P 〈 0.05), and neuronal defect was decreased in hippocampal CAI area in the cerebral ischemia group as compared with that in the acupuncture group (P 〈 0.05). ② Expression of positive neurons in cerebral cortex and hippocampal CAI area with TUNEL staining: Positive neurons with TUNEL staining were not observed in the sham operation group, but positive neurons were increased in the cerebral ischemia group as compared with those in the acupuncture group (P 〈0.05). ③ Observational results of electron microscope: Neuronal apoptosis was not found in the sham operation group; neuronal apoptosis was rarely found in the acupuncture group; neuronal apoptosis was typical in the cerebral ischemia group. CONCLUSION: Delayed neuronal death after total cerebral ischemia may accompany with apoptosis, but acupuncture may play a certain role in protecting nerve through inhibiting ischemic neuronal apoptosis. 展开更多

Gentianine has been shown to have a protective effect on hippocampal CA1 neurons in rats subjected to recurrent febrile convulsion(FC).The present study sought to explore the possible mechanism of gentianine by intraperitoneally injecting gentianine into rats with warm water-induced FC.The results revealed that neuronal organelle injury was slightly ameliorated in the hippocampal CA1 region. The level of glutamate was decreased, but the level of γ-aminobutyric acid was increased,as detected by ninhydrin staining. In addition, glutamate acid decarboxylase expression in hippocampal CA1 was increased,as determined by immunohistochemistry. The results demonstrated that gentianine can ameliorate FC-induced neuronal injury by enhancing glutamate acid decarboxylase activity, decreasing glutamate levels and increasing γ-aminobutyric acid levels. 展开更多

Yongquan (KI 1) is a jing (well) point of the Kidney Meridian of Foot-Shaoyin. As this point is widely used for treatment of acute or chronic diseases, it has drawn attentions from physicians both in the ancient and in the present times. If this point is taken accurately and the differentiation of disease is made properly, both the needling and the external drug application can achieve satisfactory therapeutic results. 展开更多

BACKGROUND: Sedative and hypnotic chemical drugs prolong the total-sleep time (TST) by a decrease in slow-wave sleep 2 (SWS2) and rapid-eye-movement sleep (REMS) and a relative increase in slow-wave sleep 1 (SWS1). OBJECTIVE: To investigate the effect of the Chinese medicine Zhusha Anshen Wan at different doses on each sleeping state in insomnic rats, and to identify its mode of action in improving sleep. DESIGN, TIME AND SETTING: A randomized controlled study in rats. This study was performed in the Department of Pharmacology of Chinese Materia Medica, Heilongjiang University of Traditional Chinese Medicine during the period from January 2005 to July 2006. MATERIALS: Twenty-four male Wistar rats, weighing (220±5) g, were selected. The main components in Zhusha Anshen Wan, Cinnabaris, Rhizoma Coptidis, Radix Glycyrrhixae, Prepared Radix Glycyrhizae Radix Angelicae Sinensis, and Rehmannia Pride Rhizome, were authenticated by Dr Xiaowei Du, Professor of Pharmacology. ND-97 Digital Polysomnography was purchased from the Shanghai Medical Instrument High Technology Company and Footplate Electrical Stimulator from the Harbin Research Institute of Electrical Instruments. METHODS: Rats were deprived of sleep by using the Footplate Electrical Stimulator. Insomnic rats were randomized into high-, mid- and low-dose Zhusha Anshen Wan groups, eight rats in each group. Animals were administrated with different doses of Zhusha Anshen Wan (equal to crude drug 7.2, 3.6, 1.8 g/kg) consecutively for seven days. MAIN OUTCOME MEASURES: 30 minutes after the last administration, rats of each group suffered 8 hours foot-shocks while electroencephalography signals were recorded using Digital Polysomnography. Total time of waking (W), SWS1, SWS2, REMS and TST were calculated for pre- and post-administration, respectively. RESULTS: All 24 rats were included in the statistical analysis of the results without any loss. In the low-dose Zhusha Anshen Wan group, SWS2 was increased significantly compared with pre-administration. In the middle-dose Zhusha Anshen Wan group, W was decreased significantly, but SWS1, SWS2 and TST were increased markedly compared with pre-administration, and there were significant differences between pre- and post-administration (P 〈 0.05-0.01). In the high-dose Zhusha Anshen Wan group, the duration of W was significantly decreased after administration, but SWS1, SWS2, REMS and TST were significantly longer than pre-administration (P 〈 0.05-0.01). CONCLUSION: The effect of Zhusha Anshen Wan on sleeping states is dose-dependent. Zhusha Anshen Wan acts by extending SWS1 and SWS2 to increase the total sleeoing time. 展开更多
