The Lunar Environment heliospheric X-ray Imager(LEXI)and Solar wind Magnetosphere Ionosphere Link Explorer(SMILE)missions will image the Earth’s dayside magneto pause and cusps in soft X-rays after their respective l...The Lunar Environment heliospheric X-ray Imager(LEXI)and Solar wind Magnetosphere Ionosphere Link Explorer(SMILE)missions will image the Earth’s dayside magneto pause and cusps in soft X-rays after their respective launches in the near future,to specify glo bal magnetic reconnection modes for varying solar wind conditions.To suppo rt the success of these scientific missions,it is critical to develop techniques that extract the magnetopause locations from the observed soft X-ray images.In this research,we introduce a new geometric equation that calculates the subsolar magnetopause position(RS)from a satellite position,the look direction of the instrument,and the angle at which the X-ray emission is maximized.Two assumptions are used in this method:(1)The look direction where soft X-ray emissions are maximized lies tangent to the magnetopause,and(2)the magnetopause surface near the subsolar point is almost spherical and thus RSis nea rly equal to the radius of the magneto pause curvature.We create synthetic soft X-ray images by using the Open Geospace General Circulation Model(OpenGGCM)global magnetohydrodynamic model,the galactic background,the instrument point spread function,and Poisson noise.We then apply the fast Fourier transform and Gaussian low-pass filte rs to the synthetic images to re move noise and obtain accurate look angles for the soft X-ray pea ks.From the filte red images,we calculate RS and its accuracy for different LEXI locations,look directions,and solar wind densities by using the OpenGGCM subsolar magnetopause location as ground truth.Our method estimates RS with an accuracy of<0.3 RE when the solar wind density exceeds>10 cm-3.The accuracy improves for greater solar wind densities and during southward interplanetary magnetic fields.The method ca ptures the magnetopause motion during southwa rd interplaneta ry magnetic field turnings.Consequently,the technique will enable quantitative analysis of the magnetopause motion and help reveal the dayside reconnection modes for dynamic solar wind conditions.This technique will suppo rt the LEXI and SMILE missions in achieving their scientific o bjectives.展开更多
BACKGROUND Patients with diabetes mellitus(DM)are predisposed to an increased risk of infection signifying the importance of vaccination to protect against its potentially severe complications.The Centers for Disease ...BACKGROUND Patients with diabetes mellitus(DM)are predisposed to an increased risk of infection signifying the importance of vaccination to protect against its potentially severe complications.The Centers for Disease Control and Prevention/Advisory Committee on Immunization Practices(CDC/ACIP)issued immunization recommendations to protect this patient population.AIM To assess the adherence of patients with DM to the CDC/ACIP immunization recommendations in Saudi Arabia and to identify the factors associated with the vaccine adherence rate.METHODS An observational retrospective study conducted in 2023 was used to collect data on the vaccination records from 13 diabetes care centers in Saudi Arabia with 1000 eligible patients in phase I with data collected through chart review and 709 patients in phase II through online survey.RESULTS Among participants,10.01%(n=71)had never received any vaccine,while 85.89%(n=609)received at least one dose of the coronavirus disease 2019(COVID-19)vaccine,and 34.83%(n=247)had received the annual influenza vaccine.Only 2.96%(n=21),2.11%(n=15),and 1.12%(n=8)received herpes zoster,tetanus,diphtheria,and pertussis(Tdap),and human papillomavirus(HPV)vaccines,respectively.For patients with DM in Saudi Arabia,the rate of vaccination for annual influenza and COVID-19 vaccines was higher compared to other vaccinations such as herpes zoster,Tdap,pneumococcal,and HPV.Factors such as vaccine recommendations provided by family physicians or specialists,site of care,income level,DM-related hospitalization history,residency site,hemoglobin A1c(HbA1c)level,and health sector type can significantly influence the vaccination rate in patients with DM.Among non-vaccinated patients with DM,the most reported barriers were lack of knowledge and fear of side effects.This signifies the need for large-scale research in this area to identify additional factors that might facilitate adherence to CDC/ACIP vaccine recommendations in patients with DM.CONCLUSION In Saudi Arabia,patients with DM showed higher vaccination rates for annual influenza and COVID-19 vaccines compared to other vaccinations such as herpes zoster,Tdap,pneumococcal,and HPV.Factors such as vaccine recommendations provided by family physicians or specialists,the site of care,income level,DM-related hospitalization history,residency site,HbA1c level,and health sector type can significantly influence the vaccination rate in patients with DM.展开更多

Background:Alcohol-related liver disease(ALRD)has emerged as a significant global health concern,primarily attributed to the overconsumption of alcohol.While alcoholism has the potential to impact various organs,it is...Background:Alcohol-related liver disease(ALRD)has emerged as a significant global health concern,primarily attributed to the overconsumption of alcohol.While alcoholism has the potential to impact various organs,it is the liver that is especially vulnerable.Methods:This review comprehensively examines the challenges encountered during the pre-transplant,intra-transplant,and post-transplant phases,a significant number of which are attributable to alcohol misuse.Historically,liver transplant(LT)programmes have excluded patients with alcohol-related liver disease(ARLD)due to mandatory abstinence requirements and apprehensions regarding potential graft shortages for other hepatic diseases.This review counters these concerns by highlighting the minimal usage of grafts for early liver transplantation.It strongly advocates for the incorporation of severe alcoholic hepatitis into the model for end-stage liver disease allocation,devoid of any stigmatization.The selection of ARLD individuals for LT necessitates the critical involvement of a multidisciplinary team,inclusive of addiction specialists.Results:Despite the complexities associated with LT for patients with ARLD,this review underscores its therapeutic advantages,particularly for those anticipated to experience severe adverse effects.This review accentuates the necessity of ensuring equitable access to medical interventions for all patients,irrespective of their lifestyle choices.Conclusion:The examination of genetic and epigenetic variables that play a role in the onset and advancement of ALD.The identification of potential therapy strategies is also an important area of study.The formulation of intricate eligibility rules for LT in patients with a past of alcohol abuse needs essential interactions between medical practitioners and researchers.The use of new technologies such as genomics and epigenomics could boost the accuracy of ALD diagnostic and prognostic approaches.These targeted investigations could potentially lead to major improvements in the management and treatment results of ALD.展开更多

Clinicians should maintain a level of clinical suspicion for TS in patients with primary amenorrhea even without typical physical characteristics. Interestingly, TS has uncommon variant forms with varying degrees of clinical manifestations. Even so, all TS and TS variants maintain a high risk for cardiovascular events. Therefore, early TS diagnosis is of utmost importance. Here, we present a case of a young, African-American woman with primary amenorrhea with few overt clinical signs of TS. With high clinical suspicion, genetic testing is pursued and demonstrates the TS variant. This is important because variant forms have a similar increased risk of premature hypertension, diabetes, and aortic dissection.展开更多
Background:The important roles of liver and kidney in the elimination of injurious chemicals make them highly susceptible to the noxious activities of various toxicants including cobalt chloride(CoCl_(2)).This study w...Background:The important roles of liver and kidney in the elimination of injurious chemicals make them highly susceptible to the noxious activities of various toxicants including cobalt chloride(CoCl_(2)).This study was designed to investigate the role of glycine in the mitigation of hepato-renal toxicities associated with CoCl_(2)exposure.Methods:Forty-two(42)male rats were grouped as Control;(CoCl_(2);300 ppm);CoCl_(2)+Glycine(50 mg/kg);CoCl_(2)+Glycine(100 mg/kg);Glycine(50 mg/kg);and Glycine(100 mg/kg).The markers of hepatic and renal damage,oxidative stress,the antioxidant defense system,histopathology,and immunohistochemical localization of neutrophil gelatinase associated lipocalin(NGAL)and renal podocin were evaluated.Results:Glycine significantly reduced the markers of oxidative stress(malondialdehyde content and H_(2)O_(2) generation),liver function tests(ALT,AST,and ALP),markers of renal function(creatinine and BUN),and decreased the expression of neutrophil gelatinase-associated lipocalin(NGAL)and podocin compared with rats exposed to CoCl_(2)toxicity without glycine treatment.Histopathology lesions including patchy tubular epithelial necrosis,tubular epithelial degeneration and periglomerular inflammation in renal tissues,and severe portal hepatocellular necrosis,inflammation,and duct hyperplasia were observed in hepatic tissues of rats exposed to CoCl_(2)toxicity,but were mild to absent in glycine-treated rats.Conclusion:The results of this study clearly demonstrate protective effects of glycine against CoCl_(2)-induced tissue injuries and derangement of physiological activities of the hepatic and renal systems in rats.The protective effects are mediated via augmentation of total antioxidant capacity and upregulation of NGAL and podocin expression.展开更多
Background Intrauterine growth retardation(IUGR)is one of the major constraints in animal production.Our previ-ous study showed that piglets with IUGR are associated with abnormal bile acid(BA)metabolism.This study ex...Background Intrauterine growth retardation(IUGR)is one of the major constraints in animal production.Our previ-ous study showed that piglets with IUGR are associated with abnormal bile acid(BA)metabolism.This study explored whether dietary BA supplementation could improve growth performance and colonic development,function,micro-biota,and metabolites in the normal birth weight(NBW)and IUGR piglets.A total of 48 weaned piglets(24 IUGR and 24 NBW)were allocated to four groups(12 piglets per group):(i)NBW group,(ii)NBW+BA group,(iii)IUGR group,and(iv)IUGR+BA group.Samples were collected after 28 days of feeding.Results The results showed that dietary BA supplementation increased the length and weight of the colon and colon weight to body weight ratio,while decreased the plasma diamine oxidase(DAO)concentration in the NBW pig-lets(P<0.05).Dietary BA supplementation to IUGR piglets decreased(P<0.05)the plasma concentrations of D-lactate and endotoxin and colonic DAO and endotoxin,suggesting a beneficial effect on epithelial integrity.Moreover,dietary BA supplementation to NBW and IUGR piglets increased Firmicutes abundance and decreased Bacteroidetes abundance(P<0.05),whereas Lactobacillus was the dominant genus in the colon.Metabolome analysis revealed 65 and 51 differential metabolites in the colon of piglets fed a diet with/without BA,respectively,which was associated with the colonic function of IUGR piglets.Furthermore,dietary BA supplementation to IUGR piglets upregulated the expressions of CAT,GPX,SOD,Nrf1,IL-2,and IFN-γin colonic mucosa(P<0.05).Conclusions Collectively,dietary BA supplementation could improve the colonic function of IUGR piglets,which was associated with increasing proportions of potentially beneficial bacteria and metabolites.Furthermore,BA shows a promising application prospect in improving the intestinal ecosystem and health of animals.展开更多
A catalog of M stars has been published from the Large Sky Area Multi-Object Fiber Spectroscopic Telescope data release 7(LAMOST DR7).We cross-matched the LAMOST M catalog with Kepler,Kepler 2(K2)and Transiting Exopla...A catalog of M stars has been published from the Large Sky Area Multi-Object Fiber Spectroscopic Telescope data release 7(LAMOST DR7).We cross-matched the LAMOST M catalog with Kepler,Kepler 2(K2)and Transiting Exoplanet Survey Satellite(TESS)surveys.We obtained the light curves from the Kepler and TESS surveys.We detected 20,047 flare events from 4053 M stars and calculated their durations,amplitudes,and energies.We analyzed the distribution of the flare durations and energies.The distributions of flare duration for Kepler,K2 and TESS peak are at 3–3.5 hr,4–6 hr and 1–1.5 hr,respectively.This may be the result of the different cadences for the three data sets.The highest regions of the flare energies of Kepler,K2 and TESS are 32–32.5,32–32.5 and31–31.5 erg in Log format,respectively.A linear relationship between flare duration and energy emerges from our analysis.The ratio of flare duration to total observational time is approximately 0.2%–0.3%for the Kepler,K2 and TESS surveys.The occurrence rate of a star with a flare event increases from the M0 to M4 subtypes.We also determined the spatial distribution of the flare rate of M stars in the Milky Way.It seems that the flare rate decreases as the vertical height increases.The power index of the flare energies is in the region of 1.53–2.32,which is similar to previous result for solar type star(2.0).Further,we examined the relationship between the flare amplitude and chromospheric intensity.The flare activity increases rapidly with the increase in the HαEW at the lower values(less approximately 2?)and it increases slowly at the higher values.展开更多
BACKGROUND The respiratory infection caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)has evolved into a multi-organ disorder,with longterm effects known as post-acute sequelae of SARS-CoV-2 infect...BACKGROUND The respiratory infection caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)has evolved into a multi-organ disorder,with longterm effects known as post-acute sequelae of SARS-CoV-2 infection or long coronavirus disease(COVID).AIM To examine the current knowledge and outcomes of long-term neurological and gastrointestinal(GI)symptoms in adult cohorts,including United States minority populations.METHODS PubMed and Google Scholar were searched using relevant terms,and data from five studies were analyzed,comprising 27383 patients with persistent neurological and GI sequelae.RESULTS The main symptoms included anxiety,depression,dysphagia,headache,vomiting,nausea,gastroesophageal reflux,fatigue,and abdominal pain.Patients with comorbidities and metabolic syndromes were at higher risk for long COVID.While most patients were European Americans,there was a need for further study on African Americans.CONCLUSION The underlying causes of these symptoms remain unclear,warranting more investigation into the long-term impact of the SARS-CoV-2 on different populations.展开更多

For preventing and defense against these threats and risks, precise risk perception for effective mitigation is the first step. Risk perception is necessary requirement to mitigate risk as it drives the security strategy at the organizational level and human attitude at individual level. Sometime, individuals understand there is a risk that a negative event or incident can occur, but they do not believe there will be a personal impact if the risk comes to realization but instead, they believe that the negative event will impact others. This belief supports the common belief that individuals tend to think of themselves as invulnerable, i.e., optimistically bias about the situation, thus affecting their attitude for taking preventive measures due to inappropriate risk perception or overconfidence. The main motivation of this meta-analysis is to assess that how the cyber optimistic bias or cyber optimism bias affects individual’s cyber security risk perception and how it changes their decisions. Applying a meta-analysis, this study found that optimistic bias has an overall negative impact on the cyber security due to the inappropriate risk perception and considering themselves invulnerable by biasing that the threat will not occur to them. Due to the cyber optimism bias, the individual will sometimes share passwords by considering it will not be maliciously used, lack in adopting of preventive measures, ignore security incidents, wrong perception of cyber threats and overconfidence on themselves in the context of cyber security.展开更多

Epithelial to Mesenchymal Transition (EMT) is the most common pathophysiological change observed in cancer cells of epithelial origin that promotes metastasis, drug resistance and cancer stem cell formation. Since the information regarding differential gene expression in TNBC cells and cell signaling events leading to EMT is limited, this investigation was done by comparing transcriptomic data generated by RNA isolation and sequencing of a EMT model TNBC cell line in comparison to regular TNBC cells. RNA sequencing and Ingenuity Pathway Software Analysis (IPA) of the transcriptomic data revealed several upregulated and downregulated gene expressions along with novel core canonical pathways including Sirtuin signaling, Oxidative Phosphorylation and Mitochondrial dysfunction events involved in EMT changes of the TNBC cells.展开更多
Objective:The study aimed to determine the overall predictive value of alarm features in diagnosing upper Gastrointestinal(GI)malignancies and significant endoscopic findings among patients undergoing elective Esophag...Objective:The study aimed to determine the overall predictive value of alarm features in diagnosing upper Gastrointestinal(GI)malignancies and significant endoscopic findings among patients undergoing elective Esophagogastroduodenoscopy(EGD)at Tikur Anbessa Special Hospital(TASH)and Adera Medical Centre(AMC).Methods:It was an institution-based cross-sectional study conducted on patients undergoing elective endoscopy for an upper GI complaint from July to September 2022.Data was collected from patient charts,and biopsies were taken for histologic confirmation.The study assessed the association of alarm symptoms and signs with significant upper gastrointestinal(UGI)endoscopic findings and malignancies.Results:142 patients were selected,with an average age of 48.35 and 52.1% being male.Epigastric pain was the most common reason for an endoscopy.62% of patients had at least one alarm feature,the most common being unexplained weight loss and UGI bleeding.The study found a strong association between the presence of alarm features,significant endoscopic findings,and UGI malignancies.The pooled sensitivity and specificity of any alarm feature for any significant finding were 79% and 64.9%,respectively,and for malignancy,100% and 39.7%,respectively.The presence of the alarm feature was associated with an increase of 6.801 in the odds of developing SEF and an increase of 4.199 in the odds of developing malignancy.Conclusions:UGI alarm symptoms and signs like an abdominal mass,persistent vomiting,dysphagia,and UGI bleeding are predictive of significant endoscopic findings and malignancies.Hence,EGD should be done and suspicious lesions should be biopsied early,regardless of gender,age,or duration of symptoms.展开更多
Stiff Person Syndrome (SPS) is a rare autoimmune disease related to the lack of inhibition of excitatory neurons in the central nervous system leading to multiple motor dysfunction and symptoms due to uncontrolled mot...Stiff Person Syndrome (SPS) is a rare autoimmune disease related to the lack of inhibition of excitatory neurons in the central nervous system leading to multiple motor dysfunction and symptoms due to uncontrolled motor neuron firing. The pathophysiology of the disease is not completely understood;however, high titers of Glutamic acid decarboxylase antibodies (anti-GAD Ab) have been found in such patients, which leads to its high association with the disease. We present a case of a 52-year-old female with a 20-year history of ongoing gait and balance issues. She is diagnosed with multiple conditions, including stiff person syndrome (GAD+), spinocerebellar ataxia with epilepsy, systemic lupus erythematosus, type 1 diabetes mellitus, IgA deficiency, hypothyroidism, and pernicious anemia. She presented in our institution with a history of a recent fall from a wheelchair. We review the case presentation and association of anti-GAD antibodies with stiff person syndrome and its treatment.展开更多

文摘Background:Alcohol-related liver disease(ALRD)has emerged as a significant global health concern,primarily attributed to the overconsumption of alcohol.While alcoholism has the potential to impact various organs,it is the liver that is especially vulnerable.Methods:This review comprehensively examines the challenges encountered during the pre-transplant,intra-transplant,and post-transplant phases,a significant number of which are attributable to alcohol misuse.Historically,liver transplant(LT)programmes have excluded patients with alcohol-related liver disease(ARLD)due to mandatory abstinence requirements and apprehensions regarding potential graft shortages for other hepatic diseases.This review counters these concerns by highlighting the minimal usage of grafts for early liver transplantation.It strongly advocates for the incorporation of severe alcoholic hepatitis into the model for end-stage liver disease allocation,devoid of any stigmatization.The selection of ARLD individuals for LT necessitates the critical involvement of a multidisciplinary team,inclusive of addiction specialists.Results:Despite the complexities associated with LT for patients with ARLD,this review underscores its therapeutic advantages,particularly for those anticipated to experience severe adverse effects.This review accentuates the necessity of ensuring equitable access to medical interventions for all patients,irrespective of their lifestyle choices.Conclusion:The examination of genetic and epigenetic variables that play a role in the onset and advancement of ALD.The identification of potential therapy strategies is also an important area of study.The formulation of intricate eligibility rules for LT in patients with a past of alcohol abuse needs essential interactions between medical practitioners and researchers.The use of new technologies such as genomics and epigenomics could boost the accuracy of ALD diagnostic and prognostic approaches.These targeted investigations could potentially lead to major improvements in the management and treatment results of ALD.
文摘The extra gonadal consequences of Turner’s Syndrome (TS) also pose risks to patients, namely cardiovascular. Clinicians should maintain a level of clinical suspicion for TS in patients with primary amenorrhea even without typical physical characteristics. Interestingly, TS has uncommon variant forms with varying degrees of clinical manifestations. Even so, all TS and TS variants maintain a high risk for cardiovascular events. Therefore, early TS diagnosis is of utmost importance. Here, we present a case of a young, African-American woman with primary amenorrhea with few overt clinical signs of TS. With high clinical suspicion, genetic testing is pursued and demonstrates the TS variant. This is important because variant forms have a similar increased risk of premature hypertension, diabetes, and aortic dissection.
文摘Background:The important roles of liver and kidney in the elimination of injurious chemicals make them highly susceptible to the noxious activities of various toxicants including cobalt chloride(CoCl_(2)).This study was designed to investigate the role of glycine in the mitigation of hepato-renal toxicities associated with CoCl_(2)exposure.Methods:Forty-two(42)male rats were grouped as Control;(CoCl_(2);300 ppm);CoCl_(2)+Glycine(50 mg/kg);CoCl_(2)+Glycine(100 mg/kg);Glycine(50 mg/kg);and Glycine(100 mg/kg).The markers of hepatic and renal damage,oxidative stress,the antioxidant defense system,histopathology,and immunohistochemical localization of neutrophil gelatinase associated lipocalin(NGAL)and renal podocin were evaluated.Results:Glycine significantly reduced the markers of oxidative stress(malondialdehyde content and H_(2)O_(2) generation),liver function tests(ALT,AST,and ALP),markers of renal function(creatinine and BUN),and decreased the expression of neutrophil gelatinase-associated lipocalin(NGAL)and podocin compared with rats exposed to CoCl_(2)toxicity without glycine treatment.Histopathology lesions including patchy tubular epithelial necrosis,tubular epithelial degeneration and periglomerular inflammation in renal tissues,and severe portal hepatocellular necrosis,inflammation,and duct hyperplasia were observed in hepatic tissues of rats exposed to CoCl_(2)toxicity,but were mild to absent in glycine-treated rats.Conclusion:The results of this study clearly demonstrate protective effects of glycine against CoCl_(2)-induced tissue injuries and derangement of physiological activities of the hepatic and renal systems in rats.The protective effects are mediated via augmentation of total antioxidant capacity and upregulation of NGAL and podocin expression.

文摘Background Intrauterine growth retardation(IUGR)is one of the major constraints in animal production.Our previ-ous study showed that piglets with IUGR are associated with abnormal bile acid(BA)metabolism.This study explored whether dietary BA supplementation could improve growth performance and colonic development,function,micro-biota,and metabolites in the normal birth weight(NBW)and IUGR piglets.A total of 48 weaned piglets(24 IUGR and 24 NBW)were allocated to four groups(12 piglets per group):(i)NBW group,(ii)NBW+BA group,(iii)IUGR group,and(iv)IUGR+BA group.Samples were collected after 28 days of feeding.Results The results showed that dietary BA supplementation increased the length and weight of the colon and colon weight to body weight ratio,while decreased the plasma diamine oxidase(DAO)concentration in the NBW pig-lets(P<0.05).Dietary BA supplementation to IUGR piglets decreased(P<0.05)the plasma concentrations of D-lactate and endotoxin and colonic DAO and endotoxin,suggesting a beneficial effect on epithelial integrity.Moreover,dietary BA supplementation to NBW and IUGR piglets increased Firmicutes abundance and decreased Bacteroidetes abundance(P<0.05),whereas Lactobacillus was the dominant genus in the colon.Metabolome analysis revealed 65 and 51 differential metabolites in the colon of piglets fed a diet with/without BA,respectively,which was associated with the colonic function of IUGR piglets.Furthermore,dietary BA supplementation to IUGR piglets upregulated the expressions of CAT,GPX,SOD,Nrf1,IL-2,and IFN-γin colonic mucosa(P<0.05).Conclusions Collectively,dietary BA supplementation could improve the colonic function of IUGR piglets,which was associated with increasing proportions of potentially beneficial bacteria and metabolites.Furthermore,BA shows a promising application prospect in improving the intestinal ecosystem and health of animals.

文摘A catalog of M stars has been published from the Large Sky Area Multi-Object Fiber Spectroscopic Telescope data release 7(LAMOST DR7).We cross-matched the LAMOST M catalog with Kepler,Kepler 2(K2)and Transiting Exoplanet Survey Satellite(TESS)surveys.We obtained the light curves from the Kepler and TESS surveys.We detected 20,047 flare events from 4053 M stars and calculated their durations,amplitudes,and energies.We analyzed the distribution of the flare durations and energies.The distributions of flare duration for Kepler,K2 and TESS peak are at 3–3.5 hr,4–6 hr and 1–1.5 hr,respectively.This may be the result of the different cadences for the three data sets.The highest regions of the flare energies of Kepler,K2 and TESS are 32–32.5,32–32.5 and31–31.5 erg in Log format,respectively.A linear relationship between flare duration and energy emerges from our analysis.The ratio of flare duration to total observational time is approximately 0.2%–0.3%for the Kepler,K2 and TESS surveys.The occurrence rate of a star with a flare event increases from the M0 to M4 subtypes.We also determined the spatial distribution of the flare rate of M stars in the Milky Way.It seems that the flare rate decreases as the vertical height increases.The power index of the flare energies is in the region of 1.53–2.32,which is similar to previous result for solar type star(2.0).Further,we examined the relationship between the flare amplitude and chromospheric intensity.The flare activity increases rapidly with the increase in the HαEW at the lower values(less approximately 2?)and it increases slowly at the higher values.
文摘BACKGROUND The respiratory infection caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)has evolved into a multi-organ disorder,with longterm effects known as post-acute sequelae of SARS-CoV-2 infection or long coronavirus disease(COVID).AIM To examine the current knowledge and outcomes of long-term neurological and gastrointestinal(GI)symptoms in adult cohorts,including United States minority populations.METHODS PubMed and Google Scholar were searched using relevant terms,and data from five studies were analyzed,comprising 27383 patients with persistent neurological and GI sequelae.RESULTS The main symptoms included anxiety,depression,dysphagia,headache,vomiting,nausea,gastroesophageal reflux,fatigue,and abdominal pain.Patients with comorbidities and metabolic syndromes were at higher risk for long COVID.While most patients were European Americans,there was a need for further study on African Americans.CONCLUSION The underlying causes of these symptoms remain unclear,warranting more investigation into the long-term impact of the SARS-CoV-2 on different populations.
文摘Cyber threats and risks are increasing exponentially with time. For preventing and defense against these threats and risks, precise risk perception for effective mitigation is the first step. Risk perception is necessary requirement to mitigate risk as it drives the security strategy at the organizational level and human attitude at individual level. Sometime, individuals understand there is a risk that a negative event or incident can occur, but they do not believe there will be a personal impact if the risk comes to realization but instead, they believe that the negative event will impact others. This belief supports the common belief that individuals tend to think of themselves as invulnerable, i.e., optimistically bias about the situation, thus affecting their attitude for taking preventive measures due to inappropriate risk perception or overconfidence. The main motivation of this meta-analysis is to assess that how the cyber optimistic bias or cyber optimism bias affects individual’s cyber security risk perception and how it changes their decisions. Applying a meta-analysis, this study found that optimistic bias has an overall negative impact on the cyber security due to the inappropriate risk perception and considering themselves invulnerable by biasing that the threat will not occur to them. Due to the cyber optimism bias, the individual will sometimes share passwords by considering it will not be maliciously used, lack in adopting of preventive measures, ignore security incidents, wrong perception of cyber threats and overconfidence on themselves in the context of cyber security.
文摘Triple Negative Breast Cancer (TNBC) is a malignant form of cancer with very high mortality and morbidity. Epithelial to Mesenchymal Transition (EMT) is the most common pathophysiological change observed in cancer cells of epithelial origin that promotes metastasis, drug resistance and cancer stem cell formation. Since the information regarding differential gene expression in TNBC cells and cell signaling events leading to EMT is limited, this investigation was done by comparing transcriptomic data generated by RNA isolation and sequencing of a EMT model TNBC cell line in comparison to regular TNBC cells. RNA sequencing and Ingenuity Pathway Software Analysis (IPA) of the transcriptomic data revealed several upregulated and downregulated gene expressions along with novel core canonical pathways including Sirtuin signaling, Oxidative Phosphorylation and Mitochondrial dysfunction events involved in EMT changes of the TNBC cells.
文摘Objective:The study aimed to determine the overall predictive value of alarm features in diagnosing upper Gastrointestinal(GI)malignancies and significant endoscopic findings among patients undergoing elective Esophagogastroduodenoscopy(EGD)at Tikur Anbessa Special Hospital(TASH)and Adera Medical Centre(AMC).Methods:It was an institution-based cross-sectional study conducted on patients undergoing elective endoscopy for an upper GI complaint from July to September 2022.Data was collected from patient charts,and biopsies were taken for histologic confirmation.The study assessed the association of alarm symptoms and signs with significant upper gastrointestinal(UGI)endoscopic findings and malignancies.Results:142 patients were selected,with an average age of 48.35 and 52.1% being male.Epigastric pain was the most common reason for an endoscopy.62% of patients had at least one alarm feature,the most common being unexplained weight loss and UGI bleeding.The study found a strong association between the presence of alarm features,significant endoscopic findings,and UGI malignancies.The pooled sensitivity and specificity of any alarm feature for any significant finding were 79% and 64.9%,respectively,and for malignancy,100% and 39.7%,respectively.The presence of the alarm feature was associated with an increase of 6.801 in the odds of developing SEF and an increase of 4.199 in the odds of developing malignancy.Conclusions:UGI alarm symptoms and signs like an abdominal mass,persistent vomiting,dysphagia,and UGI bleeding are predictive of significant endoscopic findings and malignancies.Hence,EGD should be done and suspicious lesions should be biopsied early,regardless of gender,age,or duration of symptoms.
文摘Stiff Person Syndrome (SPS) is a rare autoimmune disease related to the lack of inhibition of excitatory neurons in the central nervous system leading to multiple motor dysfunction and symptoms due to uncontrolled motor neuron firing. The pathophysiology of the disease is not completely understood;however, high titers of Glutamic acid decarboxylase antibodies (anti-GAD Ab) have been found in such patients, which leads to its high association with the disease. We present a case of a 52-year-old female with a 20-year history of ongoing gait and balance issues. She is diagnosed with multiple conditions, including stiff person syndrome (GAD+), spinocerebellar ataxia with epilepsy, systemic lupus erythematosus, type 1 diabetes mellitus, IgA deficiency, hypothyroidism, and pernicious anemia. She presented in our institution with a history of a recent fall from a wheelchair. We review the case presentation and association of anti-GAD antibodies with stiff person syndrome and its treatment.
