Background:The incidence of obesity is increasing each year,and is often accompanied by multi-system metabolic disorders,typically chronic diseases and multi-disease superposition disease states.Metabolic surgery is a...Background:The incidence of obesity is increasing each year,and is often accompanied by multi-system metabolic disorders,typically chronic diseases and multi-disease superposition disease states.Metabolic surgery is an important treatment option for patients with extreme obesity;however,surgical difficulties and perioperative complications are more serious in these patients.Therefore,there is an urgent need for a new comprehensive diagnosis and treatment model to reduce the weight of patients prior to surgery,subsequently reducing the risks of surgery.Case summary:This model combines traditional Chinese medicine(TCM),external TCM treatment,and health management with western technique of metabolic surgery.Obesity is recognized and treated by multidisciplinary,multi-channel,and multi-means,constituting an innovative diagnosis and treatment model of obesity in integrated traditional Chinese and Western medicine.The application of this model in the treatment of a patient with extreme obesity having multiple systemic diseases is described herein.Conclusion:The integrated traditional Chinese and Western medicine obesity diagnosis and treatment model can be used for precise syndrome differentiation,individualized treatment,and follow-up management of patients with extreme obesity,with strong scalability and significant clinical efficacy.展开更多
Objective:To analyze the prescription rules of traditional Chinese medicine for patients with COVID-19 in Wuhan Third Hospital, and summarize the disease characteristics and clinical treatment ideas, in order to learn...Objective:To analyze the prescription rules of traditional Chinese medicine for patients with COVID-19 in Wuhan Third Hospital, and summarize the disease characteristics and clinical treatment ideas, in order to learn from the treatment of this disease by traditional Chinese medicine.Methods: This study established a cloud database based on the ancient and modern medical cases to mine the rules of medication with analyses of frequency of drug used, association analysis, cluster analysis, complex network and other methods.Results:Of 340 COVID-19 patients discharged from our hospital, 143 males and 197 females with male: female = 0.73: 1, and there were 167 patients over 60 years old, and the majority of clinical illnesses were mainly middle-aged and elderly people. Most of them had dry cough, anorexia, fatigue, etc. The traditional Chinese medicine for the treatment of COVID-19 involves a total of 204 flavors. The most frequent use is Fuling of 225 times. The treatment of traditional Chinese medicines is mainly involved with flavors such as warm, spicy and sweet. Mainly detoxification, dampness and phlegm, Qingfei Paidu Decoction is a common clinical prescription, of which the core drugs are Huoxiang, Fabanxia, Chenpi, Baizhu, Cangzhu, Fuling, Chaihu, Huangqin, Xingren, and Gancao.Conclusion: Through this study, the patients with COVID-19 in our hospital are mainly middle-aged and elderly people, and the etiology is characterized by "cold, wet, poisonous, and epidemic". The traditional Chinese medicine (TCM) extract solution was compounded with scaffolds (gelatin and Bletilla hyacinthine gum), and then frozen and dried to form spongy and porous material CYL. According to the standard of biological evaluation of medical devices that was instituted by the ministry of health of China \, the biological evaluation of CYL dressing was conducted. The results showed that all the contents of biological evaluation test consisting of acute toxicity, skin irritation, sensitization and cytotoxicity met the requirement of standards. Objective:To classify the subtypes of metabolic-associated fatty liver disease(MAFLD)and provide new insights into the heterogeneity of MAFLD.Methods:Electronic medical records(EMR)of MAFLD diagnosed in accordance wit...Objective:To classify the subtypes of metabolic-associated fatty liver disease(MAFLD)and provide new insights into the heterogeneity of MAFLD.Methods:Electronic medical records(EMR)of MAFLD diagnosed in accordance with the diagnostic criteria of Hubei Provincial Hospital of Traditional Chinese Medicine from 2016-2020 were included in the study.for physical annotation,and the data on each clinical phenotype was normalized according to corresponding aspirational standards.The MAFLD heterogeneous medical record network(HEMnet)was constructed using sex,age,disease diagnosis,symptoms,and Western medicine prescriptions as nodes and the co-occurrence times between phenotypes as edges.K-means clustering was used for disease classification.Relative risk(RR)was used to assess the specificity of each phenotype.Statistical methods were used to compare differences in laboratory indicators among subtypes.Results:A total of patients(12,626)with a mean age of 55.02(±14.21)years were included in the study.MAFLD can be divided into five subtypes:digestive diseases(C0),mental disorders and gynecological diseases(C1),chronic liver diseases and decompensated complications(C2),diabetes mellitus and its complications(C3),and immune joint system diseases(C4).Conclusions:Patients with MAFLD experience various symptoms and complications.The classification of MAFLD based on the HEMnet method is highly reliable.展开更多

We reported the clinical features and therapeu...Coronavirus disease 2019 (COVID-19) is now pandemic worldwide and has heavily overloaded hospitals in Wuhan City, China during the time between late January and February. We reported the clinical features and therapeutic characteristics of moderate COVID-19 cases in Wuhan that were treated via the integration of traditional Chinese medicine (TCM) and Western medicine. We collected electronic medical record (EMR) data, which included the full clinical profiles of patients, from a designated TCM hospital in Wuhan. The structured data of symptoms and drugs from admission notes were obtained through an information extraction process. Other key clinical entities were also confirmed and normalized to obtain information on the diagnosis, clinical treatments, laboratory tests, and outcomes of the patients. A total of 293 COVID-19 inpatient cases, including 207 moderate and 86 (29.3%) severe cases, were included in our research. Among these cases, 238 were discharged, 31 were transferred, and 24 (all severe cases) died in the hospital. Our COVID-19 cases involved elderly patients with advanced ages (57 years on average) and high comorbidity rates (61%). Our results reconfirmed several well-recognized risk factors, such as age, gender (male), and comorbidities, as well as provided novel laboratory indications (e.g., cholesterol) and TCM-specific phenotype markers (e.g., dull tongue) that were relevant to COVID-19 infections and prognosis. In addition to antiviral/antibiotics and standard supportive therapies, TCM herbal prescriptions incorporating 290 distinct herbs were used in 273 (93%) cases. The cases that received TCM treatment had lower death rates than those that did not receive TCM treatment (17/273= 6.2% vs. 7/20= 35%, P = 0.0004 for all cases;17/77= 22% vs. 7/9= 77.7%, P = 0.002 for severe cases). The TCM herbal prescriptions used for the treatment of COVID-19 infections mainly consisted of Pericarpium Citri Reticulatae, Radix Scutellariae, Rhizoma Pinellia, and their combinations, which reflected the practical TCM principles (e.g., clearing heat and dampening phlegm). Lastly, 59% of the patients received treatment, including antiviral, antibiotics, and Chinese patent medicine, before admission. This situation might have some effects on symptoms, such as fever and dry cough. By using EMR data, we described the clinical features and therapeutic characteristics of 293 COVID-19 cases treated via the integration of TCM herbal prescriptions and Western medicine. Clinical manifestations and treatments before admission and in the hospital were investigated. Our results preliminarily showed the potential effectiveness of TCM herbal prescriptions and their regularities in COVID-19 treatment.展开更多
On the basis of real-world clinical data,the study aimed to explore the effect and mechanisms of the treatment plan of“traditional Chinese medicine(TCM)regulating liver regeneration.”A total of 457 patients with HBV...On the basis of real-world clinical data,the study aimed to explore the effect and mechanisms of the treatment plan of“traditional Chinese medicine(TCM)regulating liver regeneration.”A total of 457 patients with HBV-related liver failure were retrospectively collected.The patients were divided into three groups:the modern medicine control group(MMC group),patients treated with routine medical treatment;the control group combining traditional Chinese and Western medicine(CTW),patients treated with routine medical treatment plus the common TCM formula;and the treatment group of“TCM regulating liver regeneration”(RLR),patients treated with both routine medical treatment and the special TCM formula of RLR.After 8 weeks of treatment,the mortality of patients in the RLR group(12.31%)was significantly lower than those in the MMC(50%)and CTW(29.11%)groups.Total bilirubin level significantly decreased and albumin increased in the RLR group when compared with the MMC and CTW groups(P<0.05).In addition,there were significant differences in the expression of several cytokines related to liver regeneration in the RLR group compared with the MMC group.RLR treatment can decrease jaundice,improve liver function,and significantly reduce the mortality in patients with HBV-related liver failure.The mechanism may be related to the role of RLR treatment in influencing cytokines related to liver regeneration.展开更多
MiRNAs are integral for maintaining immune homeostasis and self-tolerance.In this study,qPCR analyses were performed to determine which miRNAs play an important role in wound healing.Next,an experiment in a model of w...MiRNAs are integral for maintaining immune homeostasis and self-tolerance.In this study,qPCR analyses were performed to determine which miRNAs play an important role in wound healing.Next,an experiment in a model of wound healing was performed,and histology,mRNA expression and T-cell subpopulations in wound tissue were analyzed.The accelerated experiments were performed by local injection of either rIL-17A and/or rlL-9 after wound healing.In vitro,the differentiation of Thl7/Th9 in miR-155^+/+ or miR-155^-/- mice was investigated,and the target genes of miR155 were analyzed.From our findings,miR-155^-/- in mice promoted wound healing and weakened T cell-mediated inflammation,especially in IL-17/IL-9,and less severe skin fibrosis developed in the mice.rIL-17A and/or rIL-9 could exacerbate inflammatory injury and delay wound healing.We also demonstrated that miR-155^-/- led to a defect in the differentiation of Th17/Th9 in vitro,and this effect of IL-17/IL-9 might be related to the expression of C-maf,which is a target gene of miR155.MiR-155 regulated IL-17/IL-9-related inflammation in wound healing and might be a potential therapeutic target to attenuate the inflammatory response in wound tissue and promote the closure of wound injuries.展开更多
Background:To explore the clinical efficacy of Huazhi Rougan granules in treating non-alcoholic fatty liver disease(NAFLD)and its effect on the oral and intestinal flora.Methods:48 patients who met the inclusion crite...Background:To explore the clinical efficacy of Huazhi Rougan granules in treating non-alcoholic fatty liver disease(NAFLD)and its effect on the oral and intestinal flora.Methods:48 patients who met the inclusion criteria were intervened with Huazhi Rougan granules,the liver and spleen CT ratio,Traditional Chinese medicine(TCM)syndrome,body mass index(BMI,WC,SMM,body fat percentage,visceral fat area),liver function,and blood lipid levels were compared before and after treatment,and the curative effect was evaluated;6 cases of NAFLD patients whose clinical efficacy after Huazhi Rougan granule intervention was effective were randomly selected,and the patient’s oral and intestinal samples were taken for further analysis.Results:After treatment,the total effective rate of clinical efficacy was 85.14%.The TCM symptoms of NAFLD patients were significantly alleviated compared with before treatment(P<0.05).There were statistically significant differences in ALT,AST,GGT,TC,and TG before and after treatment(P<0.01).After treatment,BW,BMI,WC,body fat percentage,visceral fat area,liver hardness,and fat attenuation index were significantly decreased(P<0.01).Oral-gut flora analysis showed:After treatment,the Shannon index of the gut microbial community increased(P<0.01).Compared with before treatment,the abundance of Firmicutes decreased,while the abundance of Bacteroidetes,Candidatus-Sa,and Tenericutes increased(P<0.05).At the genus level,the abundance of Bacteroides,Lactobacillus,and Bifidobacterium increased,and Streptococcus,Oribacterium,and Peptoanaerobacter decreased(P<0.05).Before and after treatment,the dominant bacterial phyla in the intestine were the same,including Bacteroidetes,Firmicutes,and Proteobacteria.The abundance of Firmicutes and Roseburia decreased after treatment(P<0.05).Conclusions:Huazhi Rougan granules have a significant effect on the treatment of NAFLD,can improve liver function,reduce blood lipids,can effectively reduce body morphological indicators such as body mass,WC,body fat percentage,visceral fat area,liver stiffness measurement(LSM),and fat content,and has the function of protecting the liver and reducing blood pressure,promoting the role of intrahepatic fat metabolism;and significantly relieve the clinical symptoms.NAFLD patients have the gut and oral flora disturbances.Huazhi Rougan granules can restore the diversity of intestinal microbial communities in NAFLD patients and play a therapeutic role by regulating the abundance and composition of oral and Gut flora.展开更多

Serum was collected to determine the levels of TG and TGAb, and metastatic lesion tissue was collected to determine the expression proliferation-related molecules Bcl-2, Skp-2, caspase-3 and p27 as well as invasion molecules S100A4, MMP2, MMP13, SATB1 and Vimentin.Results: The positive rates of serum TGAb and TG of pulmonary metastasis group and bone metastasis group were significantly higher than those of non-metastasis group;S100A4, MMP2, MMP13, SATB1, Vimentin, Bcl-2 and Skp-2 mRNA contents in metastatic lesions of patients with positive serum TG and TGAb were significantly higher than those of patients with negative serum TG and TGAb while caspase-3 and p27 mRNA contents were significantly lower than those of patients with negative serum TG and TGAb.Conclusion:The increase of serum TG and TGAb contents is associated with the pulmonary metastasis and bone metastasis after thyroid carcinoma surgery, and has promoting effect on the proliferation and invasion of cancer cells in metastatic lesions.展开更多
Background:Liver cirrhosis is a complex and heterogeneous disease,with a mortality rate of up to 57%,resulting in 1.03 million deaths per year.The prevalence of liver cirrhosis is on the rise.Patients with liver cirrh...Background:Liver cirrhosis is a complex and heterogeneous disease,with a mortality rate of up to 57%,resulting in 1.03 million deaths per year.The prevalence of liver cirrhosis is on the rise.Patients with liver cirrhosis have a variety of clinical phenotypes and are prone to various complications related to liver cirrhosis.Therefore,there is an urgent need to improve the early prevention and clinical management of cirrhosis and its complications.Methods:We use a precise medical approach to analyze and characterize the complex manifestations of cirrhotic patient populations,and we propose a Heterogeneous Medical Record Network(HEMnet)that includes electronic medical records,molecular interaction networks,and domain knowledge.We train the network embedding vector on HEMnet to obtain the low-dimensional vector representation of each node.With these vectors,we enriched the original medical record and identified six subtypes of cirrhosis.Results:Subtype 1 is characterized by heart disease,and subtype 2 has the strongest association with metabolic-related diseases.Subtype 3 was characterized by Chronic gastritis diseases.Subtype 4 was characterized by Liver cirrhosis-related complications-serous effusion.Subtype 5 had the strongest association with hepatitis-cirrhosis-related complications diseases and gallbladder disease.Subtype 6 was most strongly associated with Liver cirrhosis-related complications and hepatic carcinoma.By assessing the human disease-gene association of each subtype,the rich phenotype and biological functions of each subtype at the gene level were matched to the disease comorbidities and clinical differences we identified through EHR.Conclusion:Our approach demonstrates the utility of applying a precision medicine paradigm to cirrhosis and the prospect of extending this approach to other complexes,multifactorial diseases.展开更多

To evaluate the effect of triple puncture on primary trigeminal neuralgia (pTN),64 patients with pTN were randomly assigned to two groups:treatment group and control group.The participants in the treatment group recei...To evaluate the effect of triple puncture on primary trigeminal neuralgia (pTN),64 patients with pTN were randomly assigned to two groups:treatment group and control group.The participants in the treatment group received triple puncture treatment of 6 times per week for 4 weeks,and those in control group were given carbamazepine (300-600 mg per day) for at least 1 month.Before and after treatment,the primary outcomes including the total efficiency rate and the VAS pain scores,and the secondary outcomes including the frequency of pain attack and adverse events were observed.Sixty-two participants finished the study (33 in treatment group and 29 in control group individually).After treatment,the symptoms (mainly pain) of the two groups were alleviated.The total efficiency rate in the treatment group and control group was 90.9% and 75.9% respectively.The VAS pain scores and frequency of pain attack were significantly reduced in the treatment group as compared with the control group (P<0.05).The incidence of adverse events in the treatment group and control group was 9.1% and 24.1% respectively.It can be inferred that triple puncture can effectively improve the quality of life of patients with pTN and has less side effects.展开更多
This study aimed to explore the protective effects of the traditional Chinese Medicine formula Shenkang Ⅶ recipe(SK-7)on renal fibrosis and the mechanisms.Renal fibrosis was induced by unilateral ureteral obstruction...This study aimed to explore the protective effects of the traditional Chinese Medicine formula Shenkang Ⅶ recipe(SK-7)on renal fibrosis and the mechanisms.Renal fibrosis was induced by unilateral ureteral obstruction(UUO)in rats.The rats were then divided into 5 groups:control group(Sham operation),UUO model group,UUO model plus low to high doses of SK-7(0.5,1.0,or 2.0 g/kg/day,for 14 days)groups.The animals were sacrificed on the 7th or 14th day.K idney tissues were collected for histopathological exarminations(hematoxylin and cosin and Masson's trichrome staining).Immunohistochemistry was used to detect the expression of collagen type II(Col II),fibronectin(FN),α-smooth muscle actin(a-SMA),TIMP metallopeptidase inhibitor 2(TIMP2),matrix metallopeptidase 2(MMP2),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and monocyte chemotactic protein-1(MCP-1).The TGF-β1/Smad,NF-κB and Sonic hedgehog signaling proteins were detected by Western blotting.Our results showed that SK-7 prevented UUO-induced renal injury and accumulation of collagen fibrils.Renal fbrosis biomarkers Col Ⅲ,FN,α-SMA and TMP2 were increased in the rats after UUO and decreased by SK-7,while MMP2 was upregulated after treatment.SK-7 also suppressed the levels of TNF-α,IL-1βand MCP-1 in UUO rats.In addition,SK-7 inhibited activation of the TGF-B/Smad,NF-κB and sonic hedgehog signaling(SHH)pathways.Taken together,these findings suggest that SK-7 may regulate the synthesis and degradation of extracellular matrix,reduce inflammation and suppress the proliferation of fibroblasts,by blocking the TGF-β1/Smad,NF-κB and SHH signaling pathways to exert its anti-renal fbrosis effect in UUO rats.展开更多
However,the best choice of acupuncture therapy for postoperative nausea and vomiting remains controversy.Methods:Several databases were searched from inception to April 2020.Randomized controlled trials met the criter...However,the best choice of acupuncture therapy for postoperative nausea and vomiting remains controversy.Methods:Several databases were searched from inception to April 2020.Randomized controlled trials met the criterion were included.Risk of bias was implemented with Cochrane risk-of-bias tool.Addis,R,OpenBUGS and STATA were used to conduct meta-analysis.The evidence was assessed by GRADE profiler 3.6.Results:Fifty studies involving 5980 patients were included.The risk of bias of most included studies were acceptable.The results of network meta-analyses indicated,compared with placebo,electroacupuncture was the best choice for postoperative nausea(odds ratio=0.09,95%confidence interval:0.02-0.51)and acupoint plaster for postoperative vomiting(odds ratio=0.07,95%confidence interval:0.01-0.42),acupoint catgut embedding+5HTRA for postoperative nausea and vomiting(odds ratio=0.05,95%confidence interval:0.01-0.15),and transcutaneous electrical nerve stimulation+5-hydroxytryptamine receptor antagonists for postoperative rescue antiemetics(odds ratio=0.14,95%confidence interval:0.08-0.46).Conclusion:It was suggested transcutaneous electrical nerve stimulation+5-hydroxytryptamine receptor antagonists was the best choice.The results provided guidance for the prevention of postoperative nausea and vomiting.展开更多

However, the results of promising medications have been unsatisfactory. Chinese acupuncture has a long history of treating dementia, but lack of evidence from well-designed randomized controlled trials that validate its efficacy and safety, as well as its lack of clear underlying mechanisms, contribute to its limited application in clinical practice. In recent years, brain imaging technologies, such as functional magnetic resonance imaging and positron emission tomography, have been used to assess brain responses to acupuncture in a dynamic, visual, and objective way. These techniques are frequently used to explore neurological mechanisms of responses to acupuncture in AD and provide neuroimaging evidence as well as starting points to elucidate the possible mechanisms. This review summarizes the existing brain imaging evidence that explains the effects of acupuncture for AD and analyzes brain responses to acupuncture at cognitive-related acupoints [Baihui (GV 20), Shenmen (HT 7), Zusanli (ST 36), Neiguan (PC 6), and Taixi (KI 3)] from perspectives of acupoint specificity and acupoint combinations. Key issues and directions to consider in future studies are also put forward. This review should deepen our understanding of how brain imaging studies can be used to explore the underlying mechanisms of acupuncture in AD.展开更多

OBJECTIVE:To evaluate the clinical effectiveness of Shumian capsule(舒眠胶囊)in improving the symptoms of insomnia,anxiety,depression,and other symptoms of convalescent patients of COVID-19.METHODS:Totally 200 patient...OBJECTIVE:To evaluate the clinical effectiveness of Shumian capsule(舒眠胶囊)in improving the symptoms of insomnia,anxiety,depression,and other symptoms of convalescent patients of COVID-19.METHODS:Totally 200 patients were collected and randomly divided into experiment group(n=100)and control group(n=100).The control group was treated with Shumian capsule simulator,and the experiment group was treated with Shumian capsule.The improvement of TCM symptom score,the total effective rate and symptom disappearance rate of TCM symptoms in the two groups before and after treatment were observed,and the clinical effect was evaluated.RESULTS:One week after treatment,the scores of anxiety symptoms in the experiment group were significantly different from those in the control group(P<0.05),but there was no significant difference in the scores of insomnia and depression between the experiment group and the control group(P>0.05).There was no significant difference in the total effective rate and disappearance rate of TCM symptoms of insomnia,anxiety and depression between the experiment group and the control group(P>0.05).After 2 weeks of treatment,the scores of insomnia,anxiety,depression and the total effective rate of TCM symptoms in the experiment group were significantly different from those in the control group(P<0.05).There was no significant difference in the disappearance rate of insomnia,anxiety and depression between the experiment group and the control group(P>0.05).There were no significant differences in heart rate,respiration,systolic blood pressure and diastolic blood pressure between the experiment group and the control group(P>0.05).CONCLUSION:Shumian capsule can significantly improve the symptoms of insomnia,anxiety and depression in COVID-19’s convalescent patients with sleep and mood disorders.展开更多

文摘Background:The incidence of obesity is increasing each year,and is often accompanied by multi-system metabolic disorders,typically chronic diseases and multi-disease superposition disease states.Metabolic surgery is an important treatment option for patients with extreme obesity;however,surgical difficulties and perioperative complications are more serious in these patients.Therefore,there is an urgent need for a new comprehensive diagnosis and treatment model to reduce the weight of patients prior to surgery,subsequently reducing the risks of surgery.Case summary:This model combines traditional Chinese medicine(TCM),external TCM treatment,and health management with western technique of metabolic surgery.Obesity is recognized and treated by multidisciplinary,multi-channel,and multi-means,constituting an innovative diagnosis and treatment model of obesity in integrated traditional Chinese and Western medicine.The application of this model in the treatment of a patient with extreme obesity having multiple systemic diseases is described herein.Conclusion:The integrated traditional Chinese and Western medicine obesity diagnosis and treatment model can be used for precise syndrome differentiation,individualized treatment,and follow-up management of patients with extreme obesity,with strong scalability and significant clinical efficacy.

文摘Objective:To analyze the prescription rules of traditional Chinese medicine for patients with COVID-19 in Wuhan Third Hospital, and summarize the disease characteristics and clinical treatment ideas, in order to learn from the treatment of this disease by traditional Chinese medicine.Methods: This study established a cloud database based on the ancient and modern medical cases to mine the rules of medication with analyses of frequency of drug used, association analysis, cluster analysis, complex network and other methods.Results:Of 340 COVID-19 patients discharged from our hospital, 143 males and 197 females with male: female = 0.73: 1, and there were 167 patients over 60 years old, and the majority of clinical illnesses were mainly middle-aged and elderly people. Most of them had dry cough, anorexia, fatigue, etc. The traditional Chinese medicine for the treatment of COVID-19 involves a total of 204 flavors. The most frequent use is Fuling of 225 times. The treatment of traditional Chinese medicines is mainly involved with flavors such as warm, spicy and sweet. Mainly detoxification, dampness and phlegm, Qingfei Paidu Decoction is a common clinical prescription, of which the core drugs are Huoxiang, Fabanxia, Chenpi, Baizhu, Cangzhu, Fuling, Chaihu, Huangqin, Xingren, and Gancao.Conclusion: Through this study, the patients with COVID-19 in our hospital are mainly middle-aged and elderly people, and the etiology is characterized by "cold, wet, poisonous, and epidemic". Wetness is the most important core of the disease.
文摘The safety of Chuangyuling (CYL) dressing—a multifunctional medicine carrying biomaterial was evaluated in order to provide foundation for the application of CYL as material used in the wound healing. The traditional Chinese medicine (TCM) extract solution was compounded with scaffolds (gelatin and Bletilla hyacinthine gum), and then frozen and dried to form spongy and porous material CYL. According to the standard of biological evaluation of medical devices that was instituted by the ministry of health of China \, the biological evaluation of CYL dressing was conducted. The results showed that all the contents of biological evaluation test consisting of acute toxicity, skin irritation, sensitization and cytotoxicity met the requirement of standards. It was concluded that the biomaterial carrying TCM (CYL dressing) is safe for application of wound healing.

文摘Objective:To classify the subtypes of metabolic-associated fatty liver disease(MAFLD)and provide new insights into the heterogeneity of MAFLD.Methods:Electronic medical records(EMR)of MAFLD diagnosed in accordance with the diagnostic criteria of Hubei Provincial Hospital of Traditional Chinese Medicine from 2016-2020 were included in the study.for physical annotation,and the data on each clinical phenotype was normalized according to corresponding aspirational standards.The MAFLD heterogeneous medical record network(HEMnet)was constructed using sex,age,disease diagnosis,symptoms,and Western medicine prescriptions as nodes and the co-occurrence times between phenotypes as edges.K-means clustering was used for disease classification.Relative risk(RR)was used to assess the specificity of each phenotype.Statistical methods were used to compare differences in laboratory indicators among subtypes.Results:A total of patients(12,626)with a mean age of 55.02(±14.21)years were included in the study.MAFLD can be divided into five subtypes:digestive diseases(C0),mental disorders and gynecological diseases(C1),chronic liver diseases and decompensated complications(C2),diabetes mellitus and its complications(C3),and immune joint system diseases(C4).Conclusions:Patients with MAFLD experience various symptoms and complications.The classification of MAFLD based on the HEMnet method is highly reliable.

文摘Coronavirus disease 2019 (COVID-19) is now pandemic worldwide and has heavily overloaded hospitals in Wuhan City, China during the time between late January and February. We reported the clinical features and therapeutic characteristics of moderate COVID-19 cases in Wuhan that were treated via the integration of traditional Chinese medicine (TCM) and Western medicine. We collected electronic medical record (EMR) data, which included the full clinical profiles of patients, from a designated TCM hospital in Wuhan. The structured data of symptoms and drugs from admission notes were obtained through an information extraction process. Other key clinical entities were also confirmed and normalized to obtain information on the diagnosis, clinical treatments, laboratory tests, and outcomes of the patients. A total of 293 COVID-19 inpatient cases, including 207 moderate and 86 (29.3%) severe cases, were included in our research. Among these cases, 238 were discharged, 31 were transferred, and 24 (all severe cases) died in the hospital. Our COVID-19 cases involved elderly patients with advanced ages (57 years on average) and high comorbidity rates (61%). Our results reconfirmed several well-recognized risk factors, such as age, gender (male), and comorbidities, as well as provided novel laboratory indications (e.g., cholesterol) and TCM-specific phenotype markers (e.g., dull tongue) that were relevant to COVID-19 infections and prognosis. In addition to antiviral/antibiotics and standard supportive therapies, TCM herbal prescriptions incorporating 290 distinct herbs were used in 273 (93%) cases. The cases that received TCM treatment had lower death rates than those that did not receive TCM treatment (17/273= 6.2% vs. 7/20= 35%, P = 0.0004 for all cases;17/77= 22% vs. 7/9= 77.7%, P = 0.002 for severe cases). The TCM herbal prescriptions used for the treatment of COVID-19 infections mainly consisted of Pericarpium Citri Reticulatae, Radix Scutellariae, Rhizoma Pinellia, and their combinations, which reflected the practical TCM principles (e.g., clearing heat and dampening phlegm). Lastly, 59% of the patients received treatment, including antiviral, antibiotics, and Chinese patent medicine, before admission. This situation might have some effects on symptoms, such as fever and dry cough. By using EMR data, we described the clinical features and therapeutic characteristics of 293 COVID-19 cases treated via the integration of TCM herbal prescriptions and Western medicine. Clinical manifestations and treatments before admission and in the hospital were investigated. Our results preliminarily showed the potential effectiveness of TCM herbal prescriptions and their regularities in COVID-19 treatment.

文摘On the basis of real-world clinical data,the study aimed to explore the effect and mechanisms of the treatment plan of“traditional Chinese medicine(TCM)regulating liver regeneration.”A total of 457 patients with HBV-related liver failure were retrospectively collected.The patients were divided into three groups:the modern medicine control group(MMC group),patients treated with routine medical treatment;the control group combining traditional Chinese and Western medicine(CTW),patients treated with routine medical treatment plus the common TCM formula;and the treatment group of“TCM regulating liver regeneration”(RLR),patients treated with both routine medical treatment and the special TCM formula of RLR.After 8 weeks of treatment,the mortality of patients in the RLR group(12.31%)was significantly lower than those in the MMC(50%)and CTW(29.11%)groups.Total bilirubin level significantly decreased and albumin increased in the RLR group when compared with the MMC and CTW groups(P<0.05).In addition,there were significant differences in the expression of several cytokines related to liver regeneration in the RLR group compared with the MMC group.RLR treatment can decrease jaundice,improve liver function,and significantly reduce the mortality in patients with HBV-related liver failure.The mechanism may be related to the role of RLR treatment in influencing cytokines related to liver regeneration.

文摘MiRNAs are integral for maintaining immune homeostasis and self-tolerance.In this study,qPCR analyses were performed to determine which miRNAs play an important role in wound healing.Next,an experiment in a model of wound healing was performed,and histology,mRNA expression and T-cell subpopulations in wound tissue were analyzed.The accelerated experiments were performed by local injection of either rIL-17A and/or rlL-9 after wound healing.In vitro,the differentiation of Thl7/Th9 in miR-155^+/+ or miR-155^-/- mice was investigated,and the target genes of miR155 were analyzed.From our findings,miR-155^-/- in mice promoted wound healing and weakened T cell-mediated inflammation,especially in IL-17/IL-9,and less severe skin fibrosis developed in the mice.rIL-17A and/or rIL-9 could exacerbate inflammatory injury and delay wound healing.We also demonstrated that miR-155^-/- led to a defect in the differentiation of Th17/Th9 in vitro,and this effect of IL-17/IL-9 might be related to the expression of C-maf,which is a target gene of miR155.MiR-155 regulated IL-17/IL-9-related inflammation in wound healing and might be a potential therapeutic target to attenuate the inflammatory response in wound tissue and promote the closure of wound injuries.

文摘Background:To explore the clinical efficacy of Huazhi Rougan granules in treating non-alcoholic fatty liver disease(NAFLD)and its effect on the oral and intestinal flora.Methods:48 patients who met the inclusion criteria were intervened with Huazhi Rougan granules,the liver and spleen CT ratio,Traditional Chinese medicine(TCM)syndrome,body mass index(BMI,WC,SMM,body fat percentage,visceral fat area),liver function,and blood lipid levels were compared before and after treatment,and the curative effect was evaluated;6 cases of NAFLD patients whose clinical efficacy after Huazhi Rougan granule intervention was effective were randomly selected,and the patient’s oral and intestinal samples were taken for further analysis.Results:After treatment,the total effective rate of clinical efficacy was 85.14%.The TCM symptoms of NAFLD patients were significantly alleviated compared with before treatment(P<0.05).There were statistically significant differences in ALT,AST,GGT,TC,and TG before and after treatment(P<0.01).After treatment,BW,BMI,WC,body fat percentage,visceral fat area,liver hardness,and fat attenuation index were significantly decreased(P<0.01).Oral-gut flora analysis showed:After treatment,the Shannon index of the gut microbial community increased(P<0.01).Compared with before treatment,the abundance of Firmicutes decreased,while the abundance of Bacteroidetes,Candidatus-Sa,and Tenericutes increased(P<0.05).At the genus level,the abundance of Bacteroides,Lactobacillus,and Bifidobacterium increased,and Streptococcus,Oribacterium,and Peptoanaerobacter decreased(P<0.05).Before and after treatment,the dominant bacterial phyla in the intestine were the same,including Bacteroidetes,Firmicutes,and Proteobacteria.The abundance of Firmicutes and Roseburia decreased after treatment(P<0.05).Conclusions:Huazhi Rougan granules have a significant effect on the treatment of NAFLD,can improve liver function,reduce blood lipids,can effectively reduce body morphological indicators such as body mass,WC,body fat percentage,visceral fat area,liver stiffness measurement(LSM),and fat content,and has the function of protecting the liver and reducing blood pressure,promoting the role of intrahepatic fat metabolism;and significantly relieve the clinical symptoms.NAFLD patients have the gut and oral flora disturbances.Huazhi Rougan granules can restore the diversity of intestinal microbial communities in NAFLD patients and play a therapeutic role by regulating the abundance and composition of oral and Gut flora.
文摘Objective:To study the correlation of serum thyroglobulin (TG) and anti-thyroglobulin antibody (TGAb) levels with pulmonary metastasis and bone metastasis in patients with thyroid cancer.Methods: Patients with thyroid cancer who underwent surgical resection in our hospital were selected as the research subjects, and the patients without distant metastasis, with pulmonary metastasis and with bone metastasis were screened and enrolled in non-metastasis group, pulmonary metastasis group and bone metastasis group respectively. Serum was collected to determine the levels of TG and TGAb, and metastatic lesion tissue was collected to determine the expression proliferation-related molecules Bcl-2, Skp-2, caspase-3 and p27 as well as invasion molecules S100A4, MMP2, MMP13, SATB1 and Vimentin.Results: The positive rates of serum TGAb and TG of pulmonary metastasis group and bone metastasis group were significantly higher than those of non-metastasis group;S100A4, MMP2, MMP13, SATB1, Vimentin, Bcl-2 and Skp-2 mRNA contents in metastatic lesions of patients with positive serum TG and TGAb were significantly higher than those of patients with negative serum TG and TGAb while caspase-3 and p27 mRNA contents were significantly lower than those of patients with negative serum TG and TGAb.Conclusion:The increase of serum TG and TGAb contents is associated with the pulmonary metastasis and bone metastasis after thyroid carcinoma surgery, and has promoting effect on the proliferation and invasion of cancer cells in metastatic lesions.

文摘Background:Liver cirrhosis is a complex and heterogeneous disease,with a mortality rate of up to 57%,resulting in 1.03 million deaths per year.The prevalence of liver cirrhosis is on the rise.Patients with liver cirrhosis have a variety of clinical phenotypes and are prone to various complications related to liver cirrhosis.Therefore,there is an urgent need to improve the early prevention and clinical management of cirrhosis and its complications.Methods:We use a precise medical approach to analyze and characterize the complex manifestations of cirrhotic patient populations,and we propose a Heterogeneous Medical Record Network(HEMnet)that includes electronic medical records,molecular interaction networks,and domain knowledge.We train the network embedding vector on HEMnet to obtain the low-dimensional vector representation of each node.With these vectors,we enriched the original medical record and identified six subtypes of cirrhosis.Results:Subtype 1 is characterized by heart disease,and subtype 2 has the strongest association with metabolic-related diseases.Subtype 3 was characterized by Chronic gastritis diseases.Subtype 4 was characterized by Liver cirrhosis-related complications-serous effusion.Subtype 5 had the strongest association with hepatitis-cirrhosis-related complications diseases and gallbladder disease.Subtype 6 was most strongly associated with Liver cirrhosis-related complications and hepatic carcinoma.By assessing the human disease-gene association of each subtype,the rich phenotype and biological functions of each subtype at the gene level were matched to the disease comorbidities and clinical differences we identified through EHR.Conclusion:Our approach demonstrates the utility of applying a precision medicine paradigm to cirrhosis and the prospect of extending this approach to other complexes,multifactorial diseases.

文摘Objective:In this study,the effective components and related targets of Shugan Jieyu Capsule(SGJY)in the treatment of hepatitis B were determined to explore the mechanism of SGJY in the treatment of hepatitis B.Methods:In this study,the effective components and targets of SGJY,and the related targets of hepatitis B were searched,and obtained the targets of SGJY in the treatment of hepatitis B according to the principle of Venn diagram.To build a protein-protein interaction network,String database was used,Cytoscape(3.7.2)software was used for topology analysis,R(4.0.5)software was used for go analysis,KEGG pathway enrichment analysis,and study of putative signaling pathways to determine how they could work.Results:SZJY was used to predict a total of 11 Chinese herbal components and 85 associated targets for the treatment of hepatitis B.34 important targets were examined,including AKT1,EGFR,and 10 important pathways were examined,including proteoglycans in cancer and the PI3K/Akt signaling pathway.Conclusion:SGJY in the treatment of hepatitis B mainly inhibits the secretion of HBsAg and HBeAg by affecting PI3K-Akt and proteoglycans in cancer,and inhibits the progression of liver cancer.
文摘To evaluate the effect of triple puncture on primary trigeminal neuralgia (pTN),64 patients with pTN were randomly assigned to two groups:treatment group and control group.The participants in the treatment group received triple puncture treatment of 6 times per week for 4 weeks,and those in control group were given carbamazepine (300-600 mg per day) for at least 1 month.Before and after treatment,the primary outcomes including the total efficiency rate and the VAS pain scores,and the secondary outcomes including the frequency of pain attack and adverse events were observed.Sixty-two participants finished the study (33 in treatment group and 29 in control group individually).After treatment,the symptoms (mainly pain) of the two groups were alleviated.The total efficiency rate in the treatment group and control group was 90.9% and 75.9% respectively.The VAS pain scores and frequency of pain attack were significantly reduced in the treatment group as compared with the control group (P<0.05).The incidence of adverse events in the treatment group and control group was 9.1% and 24.1% respectively.It can be inferred that triple puncture can effectively improve the quality of life of patients with pTN and has less side effects.

文摘This study aimed to explore the protective effects of the traditional Chinese Medicine formula Shenkang Ⅶ recipe(SK-7)on renal fibrosis and the mechanisms.Renal fibrosis was induced by unilateral ureteral obstruction(UUO)in rats.The rats were then divided into 5 groups:control group(Sham operation),UUO model group,UUO model plus low to high doses of SK-7(0.5,1.0,or 2.0 g/kg/day,for 14 days)groups.The animals were sacrificed on the 7th or 14th day.K idney tissues were collected for histopathological exarminations(hematoxylin and cosin and Masson's trichrome staining).Immunohistochemistry was used to detect the expression of collagen type II(Col II),fibronectin(FN),α-smooth muscle actin(a-SMA),TIMP metallopeptidase inhibitor 2(TIMP2),matrix metallopeptidase 2(MMP2),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and monocyte chemotactic protein-1(MCP-1).The TGF-β1/Smad,NF-κB and Sonic hedgehog signaling proteins were detected by Western blotting.Our results showed that SK-7 prevented UUO-induced renal injury and accumulation of collagen fibrils.Renal fbrosis biomarkers Col Ⅲ,FN,α-SMA and TMP2 were increased in the rats after UUO and decreased by SK-7,while MMP2 was upregulated after treatment.SK-7 also suppressed the levels of TNF-α,IL-1βand MCP-1 in UUO rats.In addition,SK-7 inhibited activation of the TGF-B/Smad,NF-κB and sonic hedgehog signaling(SHH)pathways.Taken together,these findings suggest that SK-7 may regulate the synthesis and degradation of extracellular matrix,reduce inflammation and suppress the proliferation of fibroblasts,by blocking the TGF-β1/Smad,NF-κB and SHH signaling pathways to exert its anti-renal fbrosis effect in UUO rats.

文摘However,the best choice of acupuncture therapy for postoperative nausea and vomiting remains controversy.Methods:Several databases were searched from inception to April 2020.Randomized controlled trials met the criterion were included.Risk of bias was implemented with Cochrane risk-of-bias tool.Addis,R,OpenBUGS and STATA were used to conduct meta-analysis.The evidence was assessed by GRADE profiler 3.6.Results:Fifty studies involving 5980 patients were included.The risk of bias of most included studies were acceptable.The results of network meta-analyses indicated,compared with placebo,electroacupuncture was the best choice for postoperative nausea(odds ratio=0.09,95%confidence interval:0.02-0.51)and acupoint plaster for postoperative vomiting(odds ratio=0.07,95%confidence interval:0.01-0.42),acupoint catgut embedding+5HTRA for postoperative nausea and vomiting(odds ratio=0.05,95%confidence interval:0.01-0.15),and transcutaneous electrical nerve stimulation+5-hydroxytryptamine receptor antagonists for postoperative rescue antiemetics(odds ratio=0.14,95%confidence interval:0.08-0.46).Conclusion:It was suggested transcutaneous electrical nerve stimulation+5-hydroxytryptamine receptor antagonists was the best choice.The results provided guidance for the prevention of postoperative nausea and vomiting.

文摘As the worldwide population ages, the prevalence of Alzheimer's disease (AD) increases. However, the results of promising medications have been unsatisfactory. Chinese acupuncture has a long history of treating dementia, but lack of evidence from well-designed randomized controlled trials that validate its efficacy and safety, as well as its lack of clear underlying mechanisms, contribute to its limited application in clinical practice. In recent years, brain imaging technologies, such as functional magnetic resonance imaging and positron emission tomography, have been used to assess brain responses to acupuncture in a dynamic, visual, and objective way. These techniques are frequently used to explore neurological mechanisms of responses to acupuncture in AD and provide neuroimaging evidence as well as starting points to elucidate the possible mechanisms. This review summarizes the existing brain imaging evidence that explains the effects of acupuncture for AD and analyzes brain responses to acupuncture at cognitive-related acupoints [Baihui (GV 20), Shenmen (HT 7), Zusanli (ST 36), Neiguan (PC 6), and Taixi (KI 3)] from perspectives of acupoint specificity and acupoint combinations. Key issues and directions to consider in future studies are also put forward. 文摘Relapse remains the worst life-threatening complications after allogeneic hematopoietic stem cell transplantation(allo-HSCT)in patients with acute myeloid leukemia(AML),whose prognosis has been historically dismal.Given the rapid development of genomics and immunotherapies,the interference strategies for AML recurrence have been changing these years.More and more novel targeting agents that have received the U.S.Food and Drug Administration(FDA)approval for de novo AML treatment have been administrated in the salvage or maintenance therapy of post-HSCT relapse.Targeted strategies that regulate the immune microenvironment of and optimize the graft versus leukemia(GVL)effect of immune cells are gradually improved.Such agents not only have been proven to achieve clinical benefits from a single drug,but if combined with classic therapies,can significantly improve the poor prognosis of AML patients who relapse after allo-HSCT.This review will focus on currently available and promising upcoming agents and also discuss the challenges and limitations of targeted therapies in the allogeneic hematopoietic stem cell transplantation community.

文摘OBJECTIVE:To evaluate the clinical effectiveness of Shumian capsule(舒眠胶囊)in improving the symptoms of insomnia,anxiety,depression,and other symptoms of convalescent patients of COVID-19.METHODS:Totally 200 patients were collected and randomly divided into experiment group(n=100)and control group(n=100).The control group was treated with Shumian capsule simulator,and the experiment group was treated with Shumian capsule.The improvement of TCM symptom score,the total effective rate and symptom disappearance rate of TCM symptoms in the two groups before and after treatment were observed,and the clinical effect was evaluated.RESULTS:One week after treatment,the scores of anxiety symptoms in the experiment group were significantly different from those in the control group(P<0.05),but there was no significant difference in the scores of insomnia and depression between the experiment group and the control group(P>0.05).There was no significant difference in the total effective rate and disappearance rate of TCM symptoms of insomnia,anxiety and depression between the experiment group and the control group(P>0.05).After 2 weeks of treatment,the scores of insomnia,anxiety,depression and the total effective rate of TCM symptoms in the experiment group were significantly different from those in the control group(P<0.05).There was no significant difference in the disappearance rate of insomnia,anxiety and depression between the experiment group and the control group(P>0.05).There were no significant differences in heart rate,respiration,systolic blood pressure and diastolic blood pressure between the experiment group and the control group(P>0.05).CONCLUSION:Shumian capsule can significantly improve the symptoms of insomnia,anxiety and depression in COVID-19’s convalescent patients with sleep and mood disorders.
