BACKGROUND:Shared decision-making(SDM)has broad application in emergencies.Most published studies have focused on SDM for a certain disease or expert opinions on future research gaps without revealing the full picture...BACKGROUND:Shared decision-making(SDM)has broad application in emergencies.Most published studies have focused on SDM for a certain disease or expert opinions on future research gaps without revealing the full picture or detailed guidance for clinical practice.This study is to investigate the optimal application of SDM to guide life-sustaining treatment(LST)in emergencies.METHODS:This study was a prospective two-round Delphi consensus-seeking survey among multiple stakeholders at the China Consortium of Elite Teaching Hospitals for Residency Education.Participants were identified based on their expertise in medicine,law,administration,medical education,or patient advocacy.All individual items and questions in the questionnaire were scored using a 5-point Likert scale,with responses ranging from"very unimportant"(a score of 1)to"extremely important"(a score of 5).The percentages of the responses that had scores of 4-5on the 5-point Likert scale were calculated.A Kendall’s W coefficient was calculated to evaluate the consensus of experts.RESULTS:A two-level framework consisting of 4 domains and 22 items as well as a ready-touse checklist for the informed consent process for LST was established.An acceptable Kendall’s W coefficient was achieved.CONCLUSION:A consensus-based framework supporting SDM during LST in an emergency department can inform the implementation of guidelines for clinical interventions,research studies,medical education,and policy initiatives.展开更多
Objectives:Social media has changed methods of communication in the medical profession.As part of a"doctor as communicator"strategy,Chinese medical professionals are actively embracing social media,which may...Objectives:Social media has changed methods of communication in the medical profession.As part of a"doctor as communicator"strategy,Chinese medical professionals are actively embracing social media,which may have an impact on tense physician-patient relationships in China.This paper aims to examine a pattern of Internet access,Internet usage,and trust among Chinese medical professionals and further explores the reasons for these relationships on an individual level.Designs:A web-based questionnaire was designed,based on a media literacy model,and was divided into 3 dimensions:public information,general medical information,and specialty information.After a two-round pilot study,1001 physicians were included.Additionally,4 interviewees were chosen to participate in in-depth interviews and content analysis was performed.Data were analyzed using SPSS 25.0.Results:Findings showed that new media has become a major approach for medical professionals in China to retrieve and get access to various information.However,they still trust traditional media(n=1001,P<0.01)and professional journals more(n=1001,P<0.01).Internet access,usage,and trust were positively correlated(r-0.185-0.344,P<0.01).Regarding usage habits,47.66%of the participating physicians would practice science popularization through their new media accounts and 71.23%would forward approved health information within their professional realm.A validated instrument can be provided for further related studies.Conclusions:Science popularization is a specific usage of new media among Chinese medical professionals,through which they have achieved new authority and empowered communication because of self-media.To some extent,physician-patient relationships in China can be improved because this online interaction is conducive to building harmonious and lasting offline physician-patient relationships.展开更多
The COVID-19 pandemic has prompted educators to rethink educational practices,especially with regard to technology.The COVID-19 pandemic is a huge challenge to education systems around the world.This Viewpoint offers ...The COVID-19 pandemic has prompted educators to rethink educational practices,especially with regard to technology.The COVID-19 pandemic is a huge challenge to education systems around the world.This Viewpoint offers guidance to teachers,institutional heads,and officials on addressing the crisis.This study investigated technology use in teaching during the COVID-19 lockdown inMalaysia,focusing on technology-based teaching methods,modifications necessitated by this new teaching style,and challenges teachers faced when using technology.Using purposive sampling,a qualitative study was undertaken with a sample of 10 English language teachers from Arabic schools in Malaysia.The results indicated that a digital leap occurred in education during the COVID-19 lockdown because teachers had to quickly adapt to a more technology-based teaching style.The challenges teachers faced included managing virtual classes,ensuring reliable Internet connections,overcoming a lack of preparedness and low digital competence,and dealing with students’mental health.Such changes in teaching methods have created new roles for teachers while also increasing their acceptance of e-learning and remote learning.The contribution of this research is to provide a holistic picture of remote education activities during the pandemic period to establish a linkage between the online teaching-learning process during the COVID-19 outbreak as to ensure the resumption of teaching-learning education as a normal course of procedure in the education system.Despite the human suffering brought by the pandemic,the new norms of education during COVID-19 generally have some pockets of excellence to drive the education into the Fourth Industrial Revolution.展开更多

It can also result in reduced driver frustrations and ...Seeking shortest travel times through smart algorithms may not only optimize the travel times but also reduce carbon emissions, such as CO2, CO and Hydro-Carbons. It can also result in reduced driver frustrations and can increase passenger expectations of consistent travel times, which in turn points to benefits in overall planning of day schedules. Fuel consumption savings are another benefit from the same. However, attempts to elect the shortest path as an assumption of quick travel times, often work counter to the very objective intended and come with the risk of creating a “Braess Paradox” which is about congestion resulting when several drivers attempt to elect the same shortest route. The situation that arises has been referred to as the price of anarchy! We propose algorithms that find multiple shortest paths between an origin and a destination. It must be appreciated that these will not yield the exact number of Kilometers travelled, but favourable weights in terms of travel times so that a reasonable allowable time difference between the multiple shortest paths is attained when the same Origin and Destinations are considered and favourable responsive routes are determined as variables of traffic levels and time of day. These routes are selected on the paradigm of route balancing, re-routing algorithms and traffic light intelligence all coming together to result in optimized consistent travel times whose benefits are evenly spread to all motorist, unlike the Entropy balanced k shortest paths (EBkSP) method which favours some motorists on the basis of urgency. This paper proposes a Fully Balanced Multiple-Candidate shortest path (FBMkP) by which we model in SUMO to overcome the computational overhead of assigning priority differently to each travelling vehicle using intelligence at intersections and other points on the vehicular network. The FBMkP opens up traffic by fully balancing the whole network so as to benefit every motorist. Whereas the EBkSP reserves some routes for cars on high priority, our algorithm distributes the benefits of smart routing to all vehicles on the network and serves the road side units such as induction loops and detectors from having to remember the urgency of each vehicle. Instead, detectors and induction loops simply have to poll the destination of the vehicle and not any urgency factor. The minimal data being processed significantly reduce computational times and the benefits all vehicles. The multiple-candidate shortest paths selected on the basis of current traffic status on each possible route increase the efficiency. Routes are fewer than vehicles so possessing weights of routes is smarter than processing individual vehicle weights. This is a multi-objective function project where improving one factor such as travel times improves many more cost, social and environmental factors.展开更多

Gas flared often resulted in some environmental degradation, one of such influence is soil pollution and poor ...The threat to human, fauna and flora life posed by pollution due to gas flaring cannot be over-emphasized. Gas flared often resulted in some environmental degradation, one of such influence is soil pollution and poor crop yield. This study examined the effect of gas flaring on soil and cassava productivity in Ebedei, Ukwuani LGA, Delta State. For the purpose of data collection, five (5) experimental sites were systematically selected around the flare site in Ebedei and a control site at Obiaruku. Soil samples were collected at surface 0 - 10 cm and 10 - 20 cm at distance of 50 m, 100 m, 150 m, 200 m and 250 m apart respectively away from the bund wall of the flare. The data generated were analysed using multiple regression and paired t-test analyses. The study revealed that the soils found in Ebedei have high composition of sand and soil temperature and are acidic. The soil electrical conductivity, Phosphorous, Nitrogen, Potassium and Sodium were very low. More so, the yield of cassava increases with a corresponding increase in distances from flare site. The first hypothesis revealed that there is significant variation in soil nutrients as distance increases from gas flare sites. This is evident from F value of 234.99 which is greater than the critical table value of 4.39. Furthermore, as flare distance increases, so also the organic carbon, electrical conductivity and Nitrogen increase. The second hypothesis revealed that there is a significant difference in cassava yield at gas-flared area and the non-flared area which is evident at t (6.032) is greater than the critical table (1.895) at P 0.05. It is therefore recommended that Government and FEPA should enact environmental Policies and revisit and review existing environmental and oil drilling laws in Nigeria with a view of updating them to international and environmental friendly standards.展开更多

In the twentieth century, modern Chinese literature generally adopted four paradigms in pursuit of this objective: the socio-political paradigm that viewed traditional literature in terms of "quintessence-dross" under the guidance of the Marxist doctrine of the critical inheritance of cultural heritage; the "spiritual awakening" paradigm that, being inspired by Western schools of thought such as hermeneutics, viewed traditional literature as "the source of new literature"; the scientific-aesthetic paradigm that viewed traditional literature as combining scientific interpretation and modern aesthetics and focused on interpretation, transformation, innovation and creation from within; and the folk paradigm that viewed literary tradition as a narrative of daily life demonstrating the way in which ancient and modern literature were connected in a cultural whole. 文摘Since China introduced legal education from the West in the late 19th Century,legal education has been separated from Chinese social practice.Socialism with Chinese characteristics has entered in a new era,and the development of legal education has ushered in new opportunities.Legal education should be oriented to the modernization of socialism with chinese characteristics,serve the great rejuvenation of the Chinese nation,and cultivate high-level,high-quality,inter-disciplinary talents with both moral and legal skills and a clear approach to the law.
文摘To enter into and utilize the resources of ancient literature is a necessity for literary modernity. In the twentieth century, modern Chinese literature generally adopted four paradigms in pursuit of this objective: the socio-political paradigm that viewed traditional literature in terms of "quintessence-dross" under the guidance of the Marxist doctrine of the critical inheritance of cultural heritage; the "spiritual awakening" paradigm that, being inspired by Western schools of thought such as hermeneutics, viewed traditional literature as "the source of new literature"; the scientific-aesthetic paradigm that viewed traditional literature as combining scientific interpretation and modern aesthetics and focused on interpretation, transformation, innovation and creation from within; and the folk paradigm that viewed literary tradition as a narrative of daily life demonstrating the way in which ancient and modern literature were connected in a cultural whole. What is important in the complementary and competitive development of these coexisting paradigms is highlighting "problem consciousness" in "interaction" and "original creation" and the reconstruction of the value of real human life through entering into and reactivating the resources of ancient literature.

文摘The paper examines the correlation between the degree of agentivity in Russian causatives and their semantic structure.It supports the view that the distinction between agentivity vs.non-agentivity in causatives is an important feature with various ramifications,as demonstrated in Comrie(1976),Talmy(2000).It also motivates the role of the"control"feature in the event structure of the causatives.
