Detection of <i>β</i>-Hemoglobin Gene and Sickle Cell Disorder from Umbilical Cord Blood

Sickle cell disease (SCD) is one of the most common hemoglobinopathies, which is caused by the replacement of glutamic acid with valine at the sixth position of the beta-globin amino acid chain which sickling of the entire red blood cells in the homozygous (Hb S/S) condition. There are many analyses and screening procedures were developed to detect sickle cell anemia in the early age of birth, especially from heel prick blood, but in case of developing countries, it would be more acceptable to detect sickle cell disorder using umbilical cord blood just after birth rather than using heel prick blood. In this study, umbilical cord blood (UCB) was used to detect β-hemoglobin gene and sickle cell disorder. Polymerase chain reaction (PCR) based analysis was done using two primers (wild-type and mutant type) to detect this disorder. A total number of 22 samples were enrolled in this experiment for PCR amplification among which nineteen samples were identified by amplification of both 267 bp and 517 bp fragments revealing heterozygous sickle cell trait (Hb A/S), whereas three samples were found to amplify of 517 bp only revealing healthy individuals. The result from PCR analysis was then collaborated with the information of the mothers of each sample to analyze the result more conveniently and found that the mothers of all individuals except the three samples had anemia or mild form of anemia, thus it was expected that the newborn might have anemia trait (Hb A/S) the exception was found in case of sample No. 9 and sample No. 15. Both samples showed the bands on 267 bp and 517 bp thus expressed the sickle cell disease trait although the mothers of these samples were not anemic. However, no samples were recorded having sickle cell anemia (9 Hb S/S). The inherent simplicity and low cost of this PCR based analysis with umbilical cord blood will be considered as an effective tool in future newborn screening in Bangladesh. 展开更多

Clinical Characteristics of COVID-19 Patients with Digestive Symptoms in a Tertiary Level Hospital

Background: The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has greatly challenged public health worldwide. A growing number of studies have reported gastrointestinal (GI) symptoms. The study aimed to estimate the various digestive symptoms frequently reported in Covid-19 patients among the adult population of Bangladesh. Methods: In this descriptive, cross-sectional study, we enrolled confirmed patients with COVID-19 who were admitted to the COVID unit of Shaheed Suhrawardy Medical college hospital, Dhaka from July 2020 to December 2020. All patients were COVID confirmed by real-time polymerase chain reaction (RT-PCR) and were analyzed for clinical characteristics, laboratory findings and imaging study. Results: The study population consisted of 121 COVID-19-positive patients, among them, 57.85% were male and 42.15% female. The majority (43%) of the study population were in the age group of 31 - 40 years. The male to female ratio was 1.4:1. Nearly 94.2% of the sample population were married, among them 92.9% were male and 96.1% were female. Out of 121 Covid-19 patients, 30.65% had a contact history, 17.4% had a history of traveling or residing in an area reporting COVID-19 and 11.6% of family members were affected by Covid-19. Most of the patients had a fever (95%), cough (88.4%) and dyspnoea (43.8%), pneumonia (37.4%) and severe pneumonia (36.4%). In this study, 40% patients reported a digestive symptom including diarrhea 47.9%, vomiting 55.5%, loss of appetite 16.5%, abdominal pain 29.8%, abdominal bloating 24.8%, reflux 0%, jaundice 3.3%. Regarding co-morbidities, the majority had bronchial asthma (50%) followed by hypertension 46%, diabetes 38%, obesity 23%, and CKD 14% and heart disease 3%. Among 121 COVID-19 patients, 98% had recovered from the disease and 2% of patients expired. Conclusion: Gastrointestinal symptoms are common among patients with COVID-19 and this group of patients had a long time of hospital stay from onset to admission, and higher liver enzyme levels. During the management of COVID-19 patients, clinicians need to be alert regarding suspicion of the GI features among COVID-19, so that they can diagnose early and treat effectively and immediately. 展开更多

It is a self-limited and asymptomatic disease in immunocompetent individu-als but remains a frequent cause of opportunistic infection in patients with compromised immune status. Liver in-volvement as a part of disseminated histoplasmosis is well known. However, liver infection as a primary manifestation of histoplasmosis without evidence of primary lung involve-ment is rare. In conclusion, clinicians should be aware of isolated histoplasmosis affecting the hepatobiliary system, and careful evaluation is warranted to confirm the diagno-sis. Given the appropriate clinical context, histoplasmosis should be considered in both immunocompetent and immu-nocompromised patients, regardless of pulmonary symp-toms, in non endemic as well as endemic areas. 展开更多
