This study aims to determine the prevalence and characterize ESBLs and carbapenamase producing Enterobactericeae. Method: A cross-sectional study was carried out in Gertrude’s Children’s Hospital, Nairobi. 238 urine samples were collected from patients with urinary symptoms attending the outpatient department within the period 2020-2021. The urine were examined macroscopically and microscopically. Identification and antimicrobial susceptibility testing were done using VITEK® 2 Compact system (BioMérieux). Double disc synergy test and modified hodge tests were done as confirmatory tests for ESBLs and Carbapenamase phenotypes respectively. Polymerase Chain Reaction was used for the detection of blaCTX-M, blaTEM, blaSHV, blaKPC and blaOXA-48 genes. Results: From the 238 children sampled the prevalence of UTI caused by Enterobactericeae was 22.3%. The Enterobacteriaceae species isolated were Escherichia coli (84.9%), Klebsiella pneumoniae (5.66%), Proteus mirabillis (5.66%), Enterobacter aerogenes (1.89%) and Morganella morganii (1.89%). The isolated species were resistant to ampicillin. Meropenem had the highest susceptibility. Only E. coli species had the ESBLs (26.4%) and carbapenamase (1.9%) phenotypes. 100% had BlaCTX-M while 50% had blaTEM resistant gene. There was a significant association (p Conclusion: Ampicillin resistance resulted to use of alternative drugs and Meropenem was the drug of choice where increased resistance to the recommended drugs was noted. Measurement using ionization chamber is a prerequisite for absorbed dose determination for external beam radiotherapy. Calibration coefficient is expressed in terms of air kerma and absorbed dose to water traceable to Secondary Standards Dosimetry Laboratory. The objective of this work was to evaluate the level of accuracy of ionization chamber used for clinical radiotherapy beam determination. Measurement and accuracy determination were carried out according to IAEA TRS 398 protocol. Clinical farmers type ionization chamber measurement and National Reference standard from Secondary Standards Dosimetry Laboratory were both exposed to cobalt-60 beam and measurement results compared under the same environmental conditions. The accuracy level between National Reference Standard and clinical radiotherapy standard was found to be −1.92% and −2.02% for air kerma and absorbed dose to water respectively. To minimize the effect of error and maximize therapeutic dose during treatment in order to achieve required clinical outcome, calibration factor was determined for air kerma (Nk) as 49.7 mGy/nC and absorbed dose to water ND, as 52.9 mGy/nC. The study established that radiotherapy beam measurement chain is prone to errors. Hence there is a need to independently verify the accuracy of radiation dose to ensure precision of dose delivery. The errors must be accounted for during clinical planning by factoring in calibration factor to minimize the systematic errors during treatment, and thereby providing enough room to achieve ±5% dose delivery to tumor target as recommended by ICRU.展开更多

More than 50% of Nyeri County Referral Hospital (NCRH) admissions result from no...Microvascular complications are one of the major causes of morbidity and mortality worldwide among patients with diabetes mellitus (DM). More than 50% of Nyeri County Referral Hospital (NCRH) admissions result from non-communicable diseases (NCDs) and over 55% of hospital deaths are attributable to NCDs. In Kenya, Nyeri County has the highest prevalence of diabetes mellitus compared to other counties. This study therefore sought to assess the prevalence of microvascular complications and the associated risk factors among patients attending Nyeri County Referral Hospital in Kenya. A hospital-based cross-sectional study was conducted on 314 DM patients on follow-up at NCRH from August 2022 to October 2022. Data were analyzed using STATA version 17. Univariate and multivariate logistic regression analyses are used to determine the risk factors associated with Microvascular complications of DM. Among the 314 participants with DM, 58% were females. The overall prevalence of Microvascular complications (MVCs) is 36.62%. Diabetic peripheral neuropathy was the most frequent complication (27.4%). Inadequate physical exercise was a risk factor for all MVCs. Age, marital status, and level of education were risk factors for neuropathy while smoking and alcohol intake were risk factors for nephropathy. Non-smokers were 98% less likely to have nephropathy (OR = 0.024;95% CI 0.003 - 0.145). The odds of those who exercise once weekly getting retinopathic complications reduced by 83% (OR = 0.18, 95% CI 0.049 - 0.398) compared to those who exercise daily. The findings highlight the implication of lifestyle factors in the development of MVCs among DM patients. Therefore, benefits of microvascular complications prevention should thus be factored into the management of patients with diabetes mellitus.展开更多

Natural fiber-reinforced polymeric composites have recently been proposed as a viable alternative to synthetic materials. The current work investigates the suitability of coconut fiber-reinforced polypropylene as a structural material. The coconut fiber-reinforced polypropylene composites were developed. Samples of coconut fiber/polypropylene (PP) composites were prepared using Fused Filament Fabrication (FFF). Tests were then conducted on the mechanical properties of the composites for different proportions of coconut fibers. The results obtained indicate that the composites loaded with 2 wt% exhibited the highest tensile and flexural strength, while the ones loaded with 3 wt% had the highest compression strength. The ultimate tensile and flexural strength at 2 wt% were determined to be 34.13 MPa and 70.47 MPa respectively. The compression strength at 3 wt% was found to be 37.88 MPa. Compared to pure polypropylene, the addition of coconut fibers increased the tensile, flexural, and compression strength of the composite. In the study, an artificial neural network model was proposed to predict the mechanical properties of polymeric composites based on the proportion of fibers. The model was found to predict data with high accuracy.展开更多

WHO recommends at least three doses of sulphadoxine pyrimethamine for Intermittent Preventive Treatment of Malaria in Pregnancy (IPTp) antenatally. This study sought to ascertain the prevalence and individual-level factors influencing the uptake of IPTp-SP3+. Methods: A facility-based cross-sectional study at Busia County Referral Hospital. 384 mothers were consecutively sampled at the maternity unit during delivery. Semi-structured questionnaires were used to collect data. Odds ratio (OR) and adjusted OR were used to determine statistical significance of individual factors influencing uptake of three or more IPTp-SP. Results: 43.0% of participants took IPTp-SP3+. Individual factors that affected the uptake of IPTp-SP3+ included starting ANC visits in the first trimester (adjusted odds ratio (aOR) = 2.1, 95% CI: 1.23 – 3.67, p = 0.046), having more than four ANC visits (aOR = 3.1, 95% CI: 1.49 – 6.50, p = 0.002), having a higher monthly income (aOR = 2.6, 95% CI: 1.24 – 5.36, p = 0.012), being aware of the advantages of IPTp-SP medications (aOR = 3.7, 95% CI: 1.40 – 9.74, p = 0.008), and having a positive attitude toward ANC services (aOR = 3.2, 95% CI: 1.61 – 6.31, p = 0.001). Conclusion: Less than half of the pregnant mothers are complyingIPTp-SP3+. There should be aggressive efforts by the County and National Ministries of Health promoting initiation of ANC attendance early and attendance of all the recommended eight visits together with ensuring availability of the drugs.展开更多

Objective: This study determined the socio-demographic and economic factors associated with the uptake of COVID-19 vaccine among pregnant women utilizing antenatal care services in Pumwani Maternity Hospital, Nairobi County-Kenya. Methods: The study was carried out from 15 June to 23 July 2023. Systematic sampling was used to select 302 women from whom data was collected through face-to-face interviews using a pre-tested semi-structured questionnaire. Data was analyzed using SPSS software in which bivariate and multivariate logistic regression analyses were done at a significance level of p Results: A total of 302 pregnant women participated in the study. Of these, 105 (34.8%) were aged between twenty-six (26) and thirty (30) years. The mean age of the women was 28.60 ± (SD = 5.297). The uptake of the COVID-19 vaccine was 41.1%. The common side effects reported to be associated with the vaccines were fever, headache, joint pain, vomiting and skin rash. Uptake of the COVID-19 vaccine was significantly associated with being married (AOR = 3.65, 95% CI: 0.62 - 1.80, p = 0.001), having a secondary level of education (AOR = 3.78, 95% CI: 0.99 - 2.88, p = 0.001) and being employed (COR = 2.66, 95% CI: 1.31 - 3.06, p = 0.001). Conclusion: COVID-19 vaccination uptake remains low among pregnant women in seeking ANC in Nairobi. The individual factors associated with the uptake of COVID-19 vaccine among pregnant Women at Pumwani maternity hospital in Nairobi County were being married, having secondary level of education and being employed. Integration of the COVID-19 vaccine with other routine vaccinations as per the national immunization program in Kenya and the enhancement of education regarding the safety and efficacy of the COVID-19 vaccine in pregnancy and breastfeeding and economic empowerment of women are recommended.展开更多

Incineration is a common method employed in healthcare waste management to reduce volume, quantity, toxicity as well as elimination of microorganisms. However, some of the substances remain unchanged during incineration and become part of bottom ash, such as heavy metals and persistent organic pollutants. Monitoring of pollution by heavy metals is important since their concentrations in the environment affect public health. The goal of this study was to determine the levels of Copper (Cu), Zinc (Zn) Lead (Pb), Cadmium (Cd) and Nickel (Ni) in the incinerator bottom ash in five selected County hospitals in Kenya. Bottom ash samples were collected over a period of six months. Sample preparation and treatment were done using standard methods. Analysis of the heavy metals were done using atomic absorption spectrophotometer, model AA-6200. One-Way Analysis of Variance (ANOVA) was performed to determine whether there were significant differences on the mean levels of Cu, Zn, Pd, Cd and Ni in incinerator bottom ash from the five sampling locations. A post-hoc Tukey’s Test (HSD) was used to determine if there were significant differences between and within samples. The significant differences were accepted at p ≤ 0.05. To standardize the results, overall mean of each metal from each site was calculated. The metal mean concentration values were compared with existing permissible levels set by the WHO. The concentrations (mg/kg) were in the range of 102.27 - 192.53 for Cu, Zn (131.68 - 2840.85), Pb (41.06 - 303.96), Cd (1.92 - 20.49) whereas Ni was (13.83 - 38.27) with a mean of 150.76 ± 77.88 for Copper, 131.66 ± 1598.95 for Zinc, 234.60 ± 262.76 for Lead, 12.256 ± 10.86 for Cadmium and 29.45 ± 18.24 for Nickel across the five sampling locations. There were significant differences between levels determined by one-way ANOVA of Zn (F (4, 25) = 6.893, p = 0.001, p ≤ 0.05) and Cd (F (4, 25) = 5.641, p = 0.02) and none with Cu (F (4, 25) = 1.405, p = 0.261, p ≤ 0.05), Pb (F (4, 25) = 1.073, p = 0.391, p ≤ 0.05) and Ni (F (4, 25) = 2.492, p = 0.069). Results reveal that metal content in all samples exceed the WHO permissible levels for Cu (100 mg/kg), while those for Ni were below the WHO set standards of 50 mg/kg. Levels of Zn in three hospitals exceeded permissible level of 300 mg/kg while level of Pb exceeded WHO set standards of 100 mg/kg in two hospitals. Samples from four hospitals exceeded permissible level for Cd of 3 mg/kg. This study provides evidence that incinerator bottom ash is contaminated with toxic heavy metals to human health and the environment. This study recommends that hospitals should handle the bottom ash as hazardous wastes and there is need to train and provide appropriate personal protective equipment to healthcare workers, waste handlers, and incinerator operators and enforce compliance to existing regulation and guidelines on healthcare waste management to safeguard the environment and human health.展开更多

Spatial data refers to information that explicitly or indirectly belongs to a particular geographic region or location, also known as geo-spatial data or geographic information. Focusing on spatial heterogeneity, we present a hybrid machine learning model combining two competitive algorithms: the Random Forest Regressor and CNN. The model is fine-tuned using cross validation for hyper-parameter adjustment and performance evaluation, ensuring robustness and generalization. Our approach integrates Global Moran’s I for examining global autocorrelation, and local Moran’s I for assessing local spatial autocorrelation in the residuals. To validate our approach, we implemented the hybrid model on a real-world dataset and compared its performance with that of the traditional machine learning models. Results indicate superior performance with an R-squared of 0.90, outperforming RF 0.84 and CNN 0.74. This study contributed to a detailed understanding of spatial variations in data considering the geographical information (Longitude & Latitude) present in the dataset. Our results, also assessed using the Root Mean Squared Error (RMSE), indicated that the hybrid yielded lower errors, showing a deviation of 53.65% from the RF model and 63.24% from the CNN model. Additionally, the global Moran’s I index was observed to be 0.10. This study underscores that the hybrid was able to predict correctly the house prices both in clusters and in dispersed areas.展开更多

Yet, there is less evidence of empirical research on what factors contribute to this outcome. This study aimed to bridge this gap by examining the effects of contractor-related factors on the performance of bridge construction projects in Kenya through a case study of the Bridge projects Implemented by the Kenya National Highway Agency (KeNHA). The theory of constraints (TOC) was adopted as its theoretical framework. Descriptive research was used, and the target population was 18 bridge construction projects, which were the units of analysis from 2012 to 2022. In each of these projects, 18 respondents, namely clients, consultants, contractors, engineers, environment and social guards, project managers, stakeholders, subcontractors, technical advisors, and inspectors, were included in a target population of 144 respondents. A census was conducted and a structured questionnaire was administered from which a response rate of 68% was achieved. The information was analyzed using descriptive, correlation, and multiple linear regression analysis. The contractor-related factors considered in the study were staff and management factors. The findings indicated that staff and management factors had a positive and significant outcome on performance of bridge construction projects. The study recommends continuous training and a safe learning environment for staff to improve their skills and performance in future projects. The study also recommends that a special category for bridge contractors be created in Kenya’s National Construction Authority rankings to ensure that only qualified contractors implement the Bridge projects.展开更多

These survival methods further assume independent survival times, which is not valid for honey bees, which live in nests. The study introduces a semi-parametric marginal proportional hazards mixture cure (PHMC) model with exchangeable correlation structure, using generalized estimating equations for survival data analysis. The model was tested on clustered right-censored bees survival data with a cured fraction, where two bee species were subjected to different entomopathogens to test the effect of the entomopathogens on the survival of the bee species. The Expectation-Solution algorithm is used to estimate the parameters. The study notes a weak positive association between cure statuses (ρ1=0.0007) and survival times for uncured bees (ρ2=0.0890), emphasizing their importance. The odds of being uncured for A. mellifera is higher than the odds for species M. ferruginea. The bee species, A. mellifera are more susceptible to entomopathogens icipe 7, icipe 20, and icipe 69. The Cox-Snell residuals show that the proposed semiparametric PH model generally fits the data well as compared to model that assume independent correlation structure. Thus, the semi parametric marginal proportional hazards mixture cure is parsimonious model for correlated bees survival data.展开更多

A mathematical model describing the fluid flow in a Fluidized bed dryer was developed using the nonlinear partial differential equations. Due to their non-linearity, the equations were solved numerically by use of the finite difference method. The effects of physical flow parameters on velocity, temperature, concentration and magnetic induction profiles were studied and results were presented graphically. From the mathematical analysis, it was deduced that addition of silver nanoparticles into the fluid flow enhanced velocity and temperature profiles. This led to improved heat transfer in the fluidized bed dryer, hence amplifying the tea drying process. Furthermore, it was noted that induced magnetic field tends to decrease the fluid velocity, which results in uniform distribution of heat leading to efficient heat transfer between the tea particles and the fluid, thus improving the drying process. The research findings provide information to industries on ways to optimize thermal performance of fluidized bed dryers.展开更多

Agricultural and forest wastes are the leading organic waste streams that can be transformed into useful alternative energy resources. Pyrolysis is one of the technologies for converting biomass into more valuable products, such as bio-oil, bio-char, and syngas. This work investigated the production of bio-oil through batch pyrolysis technology. A fixed bed pyrolyzer was designed and fabricated for bio-oil production. The major components of the system include a fixed bed reactor, a condenser, and a bio-oil collector. The reactor was heated using a cylindrical biomass external heater. The pyrolysis process was carried out in a reactor at a pressure of 1atm and a varying operating temperature of 150˚C, 250˚C, 350˚C to 450˚C for 120 minutes. The mass of 1kg of coconut fiber was used with particle sizes between 2.36 mm - 4.75 mm. The results show that the higher the temperature, the more volume of bio-oil produced, with the highest yield being 39.2%, at 450˚C with a heating rate of 10˚C/min. The Fourier transformation Infrared (FTIR) Spectroscopy analysis was used to analyze the bio-oil components. The obtained bio-oil has a pH of 2.4, a density of 1019.385 kg/m<sup>3</sup>, and a calorific value of 17.5 MJ/kg. The analysis also showed the presence of high-oxygenated compounds;carboxylic acids, phenols, alcohols, and branched oxygenated hydrocarbons as the main compounds present in the bio-oil. The results inferred that the liquid product could be bestowed as an alternative resource for polycarbonate material production.展开更多

Occupational radiation monitoring is a significant technique for demonstrating compliance of radiation regulatory limits. The objective of the study was to carry out assessment of occupational radiation exposure among radiotherapy personnel at KNH using thermoluminescence dosimeter, TLD. KNH staff were monitored using dosimeter type TLD-100 made of LiF:Mg,Ti, on monthly basis. The reader system used for analysis was Harshaw 8800. The measurement established the average monthly accumulated occupational personnel dose for KNH to be 0.21 mSv and 0.29 mSv for Hp (10) and Hp (0.07) respectively. The accumulated dose results were within the maximum acceptable dose of 1.67 mSv/month and 41.6 mSv/month for Hp (10) and Hp (0.07) respectively. The investigation results were higher than the acceptable public limit of 0.08 mSv/month. Moreover, incidences were noted where the fetus dose limit 0.42 was also exceeded. Evaluation of statistical dose exposure among doctors, nurses and radiographers’ measurement results were within ±0.02 mSv. The study established the average KNH occupational radiation exposure levels for both Hp (10) and Hp (0.07) were within the ICRU recommendation, validating radiation protection safe practice. Data analysis of healthcare workers did not indicate exposure trend biased to any healthcare profession. Hence radiation risk cut across all professional categories. It is recommended that Radiation Monitoring program be reviewed to include non-clinical staff who access the facility. Radiation reporting should not be limited to one facility, but reflect cases where workers are involved in multiple multiple jobs.展开更多

The paper ...Healthcare waste management (HCWM) is an important aspect of healthcare delivery globally because of its hazardous and infectious components that have potential for adverse health and environmental impacts. The paper introduces a set of indicators for assessing HCWM systems in hospitals. These indicators are: HCWM policies and standard operating procedures, management and oversight, logistics and budget support, training and occupational health and safety, and treatment, disposal and waste treatment equipment housing. By plotting a mark on a continuum which is defined as good and poor on the extremes and is connected with all other marks in a spoke arrangement, it’s possible to describe a baseline for HCWM in any specific hospital. This baseline can be used to improve awareness of the actors and policy-makers, compare the same hospital at a different point in time, to compare observations by different evaluators and to track improvements. Results suggest that in Kenya, the application of such indicators is useful for evaluating which priorities should be addressed to improve outcomes in HCWM systems. Systematic sampling technique was used to identify and collect data by use of observational checklist, interviews, visual verification and review of documents and a HCWM assessment tool. The objective is to suggest an integrated management tool as a method to identify prevailing problems with a HCWM system. The method can be replicated in other contexts worldwide, with a focus on the developing world. The integrated indicators focus on management of HCW and not its potential impact on human health and environment, an area recognized to be critical for future research.展开更多

Cow dung was sourced at JKUAT dairy farm and experiments were conducted at JKUAT Bioenergy laboratory using biogas generated in laboratory scale 1 m<sup>3</sup> bioreactors. Experiments were done in triplicates and repeated under different conditions to get the optimal conditions. The results showed that enhanced cow dung substrate displayed an improved fermentation process with increased biogas yields. Purified biogas optimized methane content from 56% ± 0.18% for raw biogas to 95% ± 0.98% for biomethane which was ideal for liquefaction.展开更多

This study investigated the antimicrobial activities of S. aculeastrum fruit extracts grown in Zambia against Escherichia coli (E. coli) (ATCC 25922), Staphylococcus aureus (S. aureus) (ATCC 25923), and Candida albicans (C. albicans). After being dried in an oven at 50°C for eight days, the fruits of S. aculeastrum were pulverised and subjected to extraction using methanol and distilled water as solvents. The resulting crude extract was then filtered, concentrated using a water bath, and packed into airtight containers separately. The disc diffusion method was employed to evaluate the antimicrobial activity of the extracts. The results showed that both the methanolic and aqueous extracts demonstrated antimicrobial activity against S. aureus and C. albicans, but not against E. coli. The zone of inhibition was greater in S. aureus than in C. albicans. This study highlights the importance of researching plants for their medicinal properties, which can pave the way for discovering natural product-based drugs with potential antimicrobial properties. These findings recommend further studies on plants used in African traditional medicine.展开更多

Attainment of complete combustion is the best strategy for mitigating the release of these emissions. This study sought to experimentally determine the effects of secondary air injection on the emission profiles of NO<sub>x</sub> (NO & NO<sub>2</sub>), CO and C<sub>x</sub>H<sub>x</sub> in a charcoal operated cookstove. Charcoal from Eucalyptus glandis was bought from Kakuzi PLC. Composites from three batches were analyzed for chemical composition and the stoichiometric air equivalent. Proximate analysis data show that the charcoal composed 58.72% ± 3.3% C, 15.95% ± 1.2% Volatile Matter, 4.69% ± 0.55% Moisture, 20.7% ± 0.8% Ash, High heat value (HHV) of 30.5 ± 1.1 and 29.3 ± 1.3 Low heat value (LHV) (MJ/kg) with a chemical formula of C<sub>18</sub>H<sub>2</sub>O and a stoichiometric air requirement of 5.28 ± 0.6 with a fuel flow rate of 1 kg fuel/hr. Emission profiles for CO and C<sub>x</sub>H<sub>x</sub> reduced significantly by 70% and 80% respectively with secondary air injection whereas those of NO<sub>x</sub> increased by between 15% and 20% for NO<sub>2</sub> and NO. The study reveals that secondary air injection has potential to mitigate on emission release, however other measures are required to mitigate NO<sub>x</sub> emissions.展开更多

Flooding is one of the many consequences of a changing climate in Kenya known to cause immense devastation resulting in the loss of lives and property. This paper discusses the risk of flooding in Kenya as one of the many outcomes of climate change in the face of urgency to adapt Kenya’s built environment to flooding which is likely to continue to prevail in the decades as a result of the looming climate change. It also sought to evaluate the physical, traumatic, and psychological effects on communities affected by flood events. This cross-sectional survey, both qualitative and quantitative in nature, executed between 13<sup>th</sup> January 2021 and 14<sup>th</sup> July 2021 with 132 respondents along the western shoreline of Lake Baringo, near Marigat Town focused on the flood levels, structures, their materials, and quantities. Results show that the area covered by Lake Baringo increased by 18% from 236 km<sup>2</sup> to 278 km<sup>2</sup>. The depth of floods ranged from 0.3 m to 1.2 m and exceeded 1.6 m during heavy rainfall up to 3.2 m with homes completely submerged by the lake. Flooding was experienced more by residents living in low areas nearer to the shoreline of the lake as compared to those living on higher grounds. 100% of the structures didn’t have the architectural technology to withstand the impacts of flooding with 59% of housing made of corrugated iron sheets both on wall and roofing, 22% of mud houses roofed with either corrugated iron sheets, 10% being timber with thatch and only 8% stoned walled houses. This predisposed all the residents to the harmful impacts of flooding. Piled sandbags by locals as a mitigating measure proved inadequate to withstand the forces of the rising waters. Flood walls were built around local lodges near the lake but the rising water level quickly breached these defences. The study recommends that county and national governing authorities develop flood adaptation strategies for resilience. These include long-term land-use planning, the establishment of early warning systems, evacuation plans, identification of vulnerable or high-risk populations, measures to ensure water quality, sanitation, and hygiene. Flood-resilient architecture including stilt and floating houses that mechanically rise and fall with respect to the highest water mark are recommended during flood events. Bridges on swollen rivers and resilient construction materials like reinforced concrete are to be used for sustainable development for flood risk adaptation.展开更多

The system of non-linear partial differential equations governing the flow was solved using finite difference numerical approximation method. The resulting numerical schemes were simulated in MATLAB software. Furthermore, the skin-friction coefficient, Sherwood number, and Nusselt number have been presented in tabular form and discussed. The findings demonstrated that increasing Reynolds number increases velocity profiles while increasing permeability parameter, suction parameter and angle of inclination for the applied magnetic field reduces the velocity profiles of the fluid flow. Temperature of the fluid increases as the angle of inclination, magnetic number, Reynolds number and Eckert number increase but decreases as Prandtl number increases. Induced magnetic field profiles decrease as magnetic Prandtl number and suction parameter increase. Concentration profiles decrease as the chemical reaction parameter and Schmidt number increase but increase as the Soret number increases. The study is significant because fluid flow and heat transfer mechanisms with the variable magnetic considerations play an important role in magnetohydrodynamic generator or dynamo and magnetohydrodynamic pumps, nuclear reactors, vehicle thermal control, heat exchangers, cancer therapy, wound treatment and hyperthermia.展开更多

ESBLs are bacteria with enzymes that make them resistant to many antibiotics, po...Background: Urinary tract infection (UTI) is a bacterial infection affecting males and females but is more prevalent in expectant women. ESBLs are bacteria with enzymes that make them resistant to many antibiotics, posing a significant health challenge. This study aims to determine the characteristics of ESBL-producing bacteria causing UTIs in expectant women. Methodology: A self-administered survey was carried out;300 expectant women were recruited using a random sampling method. A questionnaire was used to collect socio-demographic information. Urine samples were collected in sterile universal bottles and processed at the JKUAT Zoology laboratory. Urine samples were analyzed using urinalysis, microscopy, culture, and sensitivity testing. ESBL-producing bacteria were identified phenotypically using the double-disc synergy test (DDST) and genotyped for specific resistant genes using PCR. Results: UTI prevalence was 32.7% (98/300). UTI was significantly associated with the history of previous UTI (OR = 0.84, p = 0.02) and multigravida (OR = 0.14 p = 0.01). UTI was common in women aged between 28-37 years in their second trimester. Bacteria isolated were E. coli 57.1% (56/98), S. aureus 21.4% (21/98) K. pneumonia 11.2% (11/98) and Proteus spp 10.4% (10/98). Bacteria antibiotic resistance patterns were E. coli-tetracycline (91.1%), sulfamethoxazole (55.4%), cefotaxime (53.4%) and augmentin (53.4%). S. aureus-sulfamethozaxole (100%) and augmentin (71.4%), K. pneumoniae-sulfame-thoxazole (72.2%) cefotaxime (63.6%), chloramphenicol and tetracycline (54.5%). Proteus spp: tetracycline (100%), nitrofurantoin (90%), cefotaxime and chloramphenicol (50%). The proportion of ESBLs bacterial producers was 37.6% (29/77) and 44.8% (13/29) possessed ESBLs resistant genes;Bla CTX-M 53.8% (7/13), Bla SHV and Bla TEM 23.1% (3/13) each, Bla OXA (0%) was not detected. Conclusion: The study revealed a high proportion of ESBLs producing bacteria responsible for UTI in expectant women. ESBLs screening, routine culture and sensitivity testing will guide on proper management and empirical treatment of UTI patients thus reducing multi-drug resistance.展开更多
文摘Introduction: Enterobacteriaceae causing urinary tract infections (UTI) have developed resistance to the commonly used antibiotics due to emergence of Extended Spectrum Beta-Lactamases (ESBLs) and Carbapenamase producing Enterobactericeae which are a public health problem worldwide. This study aims to determine the prevalence and characterize ESBLs and carbapenamase producing Enterobactericeae. Method: A cross-sectional study was carried out in Gertrude’s Children’s Hospital, Nairobi. 238 urine samples were collected from patients with urinary symptoms attending the outpatient department within the period 2020-2021. The urine were examined macroscopically and microscopically. Identification and antimicrobial susceptibility testing were done using VITEK® 2 Compact system (BioMérieux). Double disc synergy test and modified hodge tests were done as confirmatory tests for ESBLs and Carbapenamase phenotypes respectively. Polymerase Chain Reaction was used for the detection of blaCTX-M, blaTEM, blaSHV, blaKPC and blaOXA-48 genes. Results: From the 238 children sampled the prevalence of UTI caused by Enterobactericeae was 22.3%. The Enterobacteriaceae species isolated were Escherichia coli (84.9%), Klebsiella pneumoniae (5.66%), Proteus mirabillis (5.66%), Enterobacter aerogenes (1.89%) and Morganella morganii (1.89%). The isolated species were resistant to ampicillin. Meropenem had the highest susceptibility. Only E. coli species had the ESBLs (26.4%) and carbapenamase (1.9%) phenotypes. 100% had BlaCTX-M while 50% had blaTEM resistant gene. There was a significant association (p Conclusion: Ampicillin resistance resulted to use of alternative drugs and Meropenem was the drug of choice where increased resistance to the recommended drugs was noted. Further research on resistant genes is recommended.
文摘Radiotherapy is the most widely applied oncologic treatment modality utilizing ionizing radiation. A high degree of accuracy, reliability and reproducibility is required for a successful treatment outcome. Measurement using ionization chamber is a prerequisite for absorbed dose determination for external beam radiotherapy. Calibration coefficient is expressed in terms of air kerma and absorbed dose to water traceable to Secondary Standards Dosimetry Laboratory. The objective of this work was to evaluate the level of accuracy of ionization chamber used for clinical radiotherapy beam determination. Measurement and accuracy determination were carried out according to IAEA TRS 398 protocol. Clinical farmers type ionization chamber measurement and National Reference standard from Secondary Standards Dosimetry Laboratory were both exposed to cobalt-60 beam and measurement results compared under the same environmental conditions. The accuracy level between National Reference Standard and clinical radiotherapy standard was found to be −1.92% and −2.02% for air kerma and absorbed dose to water respectively. To minimize the effect of error and maximize therapeutic dose during treatment in order to achieve required clinical outcome, calibration factor was determined for air kerma (Nk) as 49.7 mGy/nC and absorbed dose to water ND, as 52.9 mGy/nC. The study established that radiotherapy beam measurement chain is prone to errors. Hence there is a need to independently verify the accuracy of radiation dose to ensure precision of dose delivery. The errors must be accounted for during clinical planning by factoring in calibration factor to minimize the systematic errors during treatment, and thereby providing enough room to achieve ±5% dose delivery to tumor target as recommended by ICRU.
文摘Microvascular complications are one of the major causes of morbidity and mortality worldwide among patients with diabetes mellitus (DM). More than 50% of Nyeri County Referral Hospital (NCRH) admissions result from non-communicable diseases (NCDs) and over 55% of hospital deaths are attributable to NCDs. In Kenya, Nyeri County has the highest prevalence of diabetes mellitus compared to other counties. This study therefore sought to assess the prevalence of microvascular complications and the associated risk factors among patients attending Nyeri County Referral Hospital in Kenya. A hospital-based cross-sectional study was conducted on 314 DM patients on follow-up at NCRH from August 2022 to October 2022. Data were analyzed using STATA version 17. Univariate and multivariate logistic regression analyses are used to determine the risk factors associated with Microvascular complications of DM. Among the 314 participants with DM, 58% were females. The overall prevalence of Microvascular complications (MVCs) is 36.62%. Diabetic peripheral neuropathy was the most frequent complication (27.4%). Inadequate physical exercise was a risk factor for all MVCs. Age, marital status, and level of education were risk factors for neuropathy while smoking and alcohol intake were risk factors for nephropathy. Non-smokers were 98% less likely to have nephropathy (OR = 0.024;95% CI 0.003 - 0.145). The odds of those who exercise once weekly getting retinopathic complications reduced by 83% (OR = 0.18, 95% CI 0.049 - 0.398) compared to those who exercise daily. The findings highlight the implication of lifestyle factors in the development of MVCs among DM patients. Therefore, benefits of microvascular complications prevention should thus be factored into the management of patients with diabetes mellitus.
文摘In the face of the increased global campaign to minimize the emission of greenhouse gases and the need for sustainability in manufacturing, there is a great deal of research focusing on environmentally benign and renewable materials as a substitute for synthetic and petroleum-based products. Natural fiber-reinforced polymeric composites have recently been proposed as a viable alternative to synthetic materials. The current work investigates the suitability of coconut fiber-reinforced polypropylene as a structural material. The coconut fiber-reinforced polypropylene composites were developed. Samples of coconut fiber/polypropylene (PP) composites were prepared using Fused Filament Fabrication (FFF). Tests were then conducted on the mechanical properties of the composites for different proportions of coconut fibers. The results obtained indicate that the composites loaded with 2 wt% exhibited the highest tensile and flexural strength, while the ones loaded with 3 wt% had the highest compression strength. The ultimate tensile and flexural strength at 2 wt% were determined to be 34.13 MPa and 70.47 MPa respectively. The compression strength at 3 wt% was found to be 37.88 MPa. Compared to pure polypropylene, the addition of coconut fibers increased the tensile, flexural, and compression strength of the composite. In the study, an artificial neural network model was proposed to predict the mechanical properties of polymeric composites based on the proportion of fibers. The model was found to predict data with high accuracy.
文摘Background: Malaria in pregnancy causes maternal anemia, low birth weight, intrauterine growth retardation, and preterm deliveries. In malaria-endemic regions in Kenya, percentage of pregnant women hospitalized with malaria reach up to 60%. WHO recommends at least three doses of sulphadoxine pyrimethamine for Intermittent Preventive Treatment of Malaria in Pregnancy (IPTp) antenatally. This study sought to ascertain the prevalence and individual-level factors influencing the uptake of IPTp-SP3+. Methods: A facility-based cross-sectional study at Busia County Referral Hospital. 384 mothers were consecutively sampled at the maternity unit during delivery. Semi-structured questionnaires were used to collect data. Odds ratio (OR) and adjusted OR were used to determine statistical significance of individual factors influencing uptake of three or more IPTp-SP. Results: 43.0% of participants took IPTp-SP3+. Individual factors that affected the uptake of IPTp-SP3+ included starting ANC visits in the first trimester (adjusted odds ratio (aOR) = 2.1, 95% CI: 1.23 – 3.67, p = 0.046), having more than four ANC visits (aOR = 3.1, 95% CI: 1.49 – 6.50, p = 0.002), having a higher monthly income (aOR = 2.6, 95% CI: 1.24 – 5.36, p = 0.012), being aware of the advantages of IPTp-SP medications (aOR = 3.7, 95% CI: 1.40 – 9.74, p = 0.008), and having a positive attitude toward ANC services (aOR = 3.2, 95% CI: 1.61 – 6.31, p = 0.001). Conclusion: Less than half of the pregnant mothers are complyingIPTp-SP3+. There should be aggressive efforts by the County and National Ministries of Health promoting initiation of ANC attendance early and attendance of all the recommended eight visits together with ensuring availability of the drugs.
文摘Introduction: Pregnant women are a highly vulnerable population for COVID-19 with increased risk of hospitalization, intensive-care unit admission, invasive ventilation support, and mortality. Objective: This study determined the socio-demographic and economic factors associated with the uptake of COVID-19 vaccine among pregnant women utilizing antenatal care services in Pumwani Maternity Hospital, Nairobi County-Kenya. Methods: The study was carried out from 15 June to 23 July 2023. Systematic sampling was used to select 302 women from whom data was collected through face-to-face interviews using a pre-tested semi-structured questionnaire. Data was analyzed using SPSS software in which bivariate and multivariate logistic regression analyses were done at a significance level of p Results: A total of 302 pregnant women participated in the study. Of these, 105 (34.8%) were aged between twenty-six (26) and thirty (30) years. The mean age of the women was 28.60 ± (SD = 5.297). The uptake of the COVID-19 vaccine was 41.1%. The common side effects reported to be associated with the vaccines were fever, headache, joint pain, vomiting and skin rash. Uptake of the COVID-19 vaccine was significantly associated with being married (AOR = 3.65, 95% CI: 0.62 - 1.80, p = 0.001), having a secondary level of education (AOR = 3.78, 95% CI: 0.99 - 2.88, p = 0.001) and being employed (COR = 2.66, 95% CI: 1.31 - 3.06, p = 0.001). Conclusion: COVID-19 vaccination uptake remains low among pregnant women in seeking ANC in Nairobi. The individual factors associated with the uptake of COVID-19 vaccine among pregnant Women at Pumwani maternity hospital in Nairobi County were being married, having secondary level of education and being employed. Integration of the COVID-19 vaccine with other routine vaccinations as per the national immunization program in Kenya and the enhancement of education regarding the safety and efficacy of the COVID-19 vaccine in pregnancy and breastfeeding and economic empowerment of women are recommended.
文摘Healthcare wastes contain potentially harmful microorganisms, inorganic and organic compounds that pose a risk to human health and the environment. Incineration is a common method employed in healthcare waste management to reduce volume, quantity, toxicity as well as elimination of microorganisms. However, some of the substances remain unchanged during incineration and become part of bottom ash, such as heavy metals and persistent organic pollutants. Monitoring of pollution by heavy metals is important since their concentrations in the environment affect public health. The goal of this study was to determine the levels of Copper (Cu), Zinc (Zn) Lead (Pb), Cadmium (Cd) and Nickel (Ni) in the incinerator bottom ash in five selected County hospitals in Kenya. Bottom ash samples were collected over a period of six months. Sample preparation and treatment were done using standard methods. Analysis of the heavy metals were done using atomic absorption spectrophotometer, model AA-6200. One-Way Analysis of Variance (ANOVA) was performed to determine whether there were significant differences on the mean levels of Cu, Zn, Pd, Cd and Ni in incinerator bottom ash from the five sampling locations. A post-hoc Tukey’s Test (HSD) was used to determine if there were significant differences between and within samples. The significant differences were accepted at p ≤ 0.05. To standardize the results, overall mean of each metal from each site was calculated. The metal mean concentration values were compared with existing permissible levels set by the WHO. The concentrations (mg/kg) were in the range of 102.27 - 192.53 for Cu, Zn (131.68 - 2840.85), Pb (41.06 - 303.96), Cd (1.92 - 20.49) whereas Ni was (13.83 - 38.27) with a mean of 150.76 ± 77.88 for Copper, 131.66 ± 1598.95 for Zinc, 234.60 ± 262.76 for Lead, 12.256 ± 10.86 for Cadmium and 29.45 ± 18.24 for Nickel across the five sampling locations. There were significant differences between levels determined by one-way ANOVA of Zn (F (4, 25) = 6.893, p = 0.001, p ≤ 0.05) and Cd (F (4, 25) = 5.641, p = 0.02) and none with Cu (F (4, 25) = 1.405, p = 0.261, p ≤ 0.05), Pb (F (4, 25) = 1.073, p = 0.391, p ≤ 0.05) and Ni (F (4, 25) = 2.492, p = 0.069). Results reveal that metal content in all samples exceed the WHO permissible levels for Cu (100 mg/kg), while those for Ni were below the WHO set standards of 50 mg/kg. Levels of Zn in three hospitals exceeded permissible level of 300 mg/kg while level of Pb exceeded WHO set standards of 100 mg/kg in two hospitals. Samples from four hospitals exceeded permissible level for Cd of 3 mg/kg. This study provides evidence that incinerator bottom ash is contaminated with toxic heavy metals to human health and the environment. This study recommends that hospitals should handle the bottom ash as hazardous wastes and there is need to train and provide appropriate personal protective equipment to healthcare workers, waste handlers, and incinerator operators and enforce compliance to existing regulation and guidelines on healthcare waste management to safeguard the environment and human health.
文摘Spatial heterogeneity refers to the variation or differences in characteristics or features across different locations or areas in space. Spatial data refers to information that explicitly or indirectly belongs to a particular geographic region or location, also known as geo-spatial data or geographic information. Focusing on spatial heterogeneity, we present a hybrid machine learning model combining two competitive algorithms: the Random Forest Regressor and CNN. The model is fine-tuned using cross validation for hyper-parameter adjustment and performance evaluation, ensuring robustness and generalization. Our approach integrates Global Moran’s I for examining global autocorrelation, and local Moran’s I for assessing local spatial autocorrelation in the residuals. To validate our approach, we implemented the hybrid model on a real-world dataset and compared its performance with that of the traditional machine learning models. Results indicate superior performance with an R-squared of 0.90, outperforming RF 0.84 and CNN 0.74. This study contributed to a detailed understanding of spatial variations in data considering the geographical information (Longitude & Latitude) present in the dataset. Our results, also assessed using the Root Mean Squared Error (RMSE), indicated that the hybrid yielded lower errors, showing a deviation of 53.65% from the RF model and 63.24% from the CNN model. Additionally, the global Moran’s I index was observed to be 0.10. This study underscores that the hybrid was able to predict correctly the house prices both in clusters and in dispersed areas.
文摘The performance of bridge projects in Kenya is poor in terms of completion by schedule, cost, and quality (scope). Yet, there is less evidence of empirical research on what factors contribute to this outcome. This study aimed to bridge this gap by examining the effects of contractor-related factors on the performance of bridge construction projects in Kenya through a case study of the Bridge projects Implemented by the Kenya National Highway Agency (KeNHA). The theory of constraints (TOC) was adopted as its theoretical framework. Descriptive research was used, and the target population was 18 bridge construction projects, which were the units of analysis from 2012 to 2022. In each of these projects, 18 respondents, namely clients, consultants, contractors, engineers, environment and social guards, project managers, stakeholders, subcontractors, technical advisors, and inspectors, were included in a target population of 144 respondents. A census was conducted and a structured questionnaire was administered from which a response rate of 68% was achieved. The information was analyzed using descriptive, correlation, and multiple linear regression analysis. The contractor-related factors considered in the study were staff and management factors. The findings indicated that staff and management factors had a positive and significant outcome on performance of bridge construction projects. The study recommends continuous training and a safe learning environment for staff to improve their skills and performance in future projects. The study also recommends that a special category for bridge contractors be created in Kenya’s National Construction Authority rankings to ensure that only qualified contractors implement the Bridge projects.
文摘Classical survival analysis assumes all subjects will experience the event of interest, but in some cases, a portion of the population may never encounter the event. These survival methods further assume independent survival times, which is not valid for honey bees, which live in nests. The study introduces a semi-parametric marginal proportional hazards mixture cure (PHMC) model with exchangeable correlation structure, using generalized estimating equations for survival data analysis. The model was tested on clustered right-censored bees survival data with a cured fraction, where two bee species were subjected to different entomopathogens to test the effect of the entomopathogens on the survival of the bee species. The Expectation-Solution algorithm is used to estimate the parameters. The study notes a weak positive association between cure statuses (ρ1=0.0007) and survival times for uncured bees (ρ2=0.0890), emphasizing their importance. The odds of being uncured for A. mellifera is higher than the odds for species M. ferruginea. The bee species, A. mellifera are more susceptible to entomopathogens icipe 7, icipe 20, and icipe 69. The Cox-Snell residuals show that the proposed semiparametric PH model generally fits the data well as compared to model that assume independent correlation structure. Thus, the semi parametric marginal proportional hazards mixture cure is parsimonious model for correlated bees survival data.
文摘This research entails the study of heat and mass transfer of nanofluid flow in a fluidized bed dryer used in tea drying processes in presence of induced magnetic field. A mathematical model describing the fluid flow in a Fluidized bed dryer was developed using the nonlinear partial differential equations. Due to their non-linearity, the equations were solved numerically by use of the finite difference method. The effects of physical flow parameters on velocity, temperature, concentration and magnetic induction profiles were studied and results were presented graphically. From the mathematical analysis, it was deduced that addition of silver nanoparticles into the fluid flow enhanced velocity and temperature profiles. This led to improved heat transfer in the fluidized bed dryer, hence amplifying the tea drying process. Furthermore, it was noted that induced magnetic field tends to decrease the fluid velocity, which results in uniform distribution of heat leading to efficient heat transfer between the tea particles and the fluid, thus improving the drying process. The research findings provide information to industries on ways to optimize thermal performance of fluidized bed dryers.
文摘The current energy crisis could be alleviated by enhancing energy generation using the abundant biomass waste resources. Agricultural and forest wastes are the leading organic waste streams that can be transformed into useful alternative energy resources. Pyrolysis is one of the technologies for converting biomass into more valuable products, such as bio-oil, bio-char, and syngas. This work investigated the production of bio-oil through batch pyrolysis technology. A fixed bed pyrolyzer was designed and fabricated for bio-oil production. The major components of the system include a fixed bed reactor, a condenser, and a bio-oil collector. The reactor was heated using a cylindrical biomass external heater. The pyrolysis process was carried out in a reactor at a pressure of 1atm and a varying operating temperature of 150˚C, 250˚C, 350˚C to 450˚C for 120 minutes. The mass of 1kg of coconut fiber was used with particle sizes between 2.36 mm - 4.75 mm. The results show that the higher the temperature, the more volume of bio-oil produced, with the highest yield being 39.2%, at 450˚C with a heating rate of 10˚C/min. The Fourier transformation Infrared (FTIR) Spectroscopy analysis was used to analyze the bio-oil components. The obtained bio-oil has a pH of 2.4, a density of 1019.385 kg/m<sup>3</sup>, and a calorific value of 17.5 MJ/kg. The analysis also showed the presence of high-oxygenated compounds;carboxylic acids, phenols, alcohols, and branched oxygenated hydrocarbons as the main compounds present in the bio-oil. The results inferred that the liquid product could be bestowed as an alternative resource for polycarbonate material production.
文摘Kenyatta National Hospital (KNH) is the largest public referral hospital with a comprehensive cancer treatment facility in East and Central Africa. Occupational radiation monitoring is a significant technique for demonstrating compliance of radiation regulatory limits. The objective of the study was to carry out assessment of occupational radiation exposure among radiotherapy personnel at KNH using thermoluminescence dosimeter, TLD. KNH staff were monitored using dosimeter type TLD-100 made of LiF:Mg,Ti, on monthly basis. The reader system used for analysis was Harshaw 8800. The measurement established the average monthly accumulated occupational personnel dose for KNH to be 0.21 mSv and 0.29 mSv for Hp (10) and Hp (0.07) respectively. The accumulated dose results were within the maximum acceptable dose of 1.67 mSv/month and 41.6 mSv/month for Hp (10) and Hp (0.07) respectively. The investigation results were higher than the acceptable public limit of 0.08 mSv/month. Moreover, incidences were noted where the fetus dose limit 0.42 was also exceeded. Evaluation of statistical dose exposure among doctors, nurses and radiographers’ measurement results were within ±0.02 mSv. The study established the average KNH occupational radiation exposure levels for both Hp (10) and Hp (0.07) were within the ICRU recommendation, validating radiation protection safe practice. Data analysis of healthcare workers did not indicate exposure trend biased to any healthcare profession. Hence radiation risk cut across all professional categories. It is recommended that Radiation Monitoring program be reviewed to include non-clinical staff who access the facility. Radiation reporting should not be limited to one facility, but reflect cases where workers are involved in multiple multiple jobs.
文摘Healthcare waste management (HCWM) is an important aspect of healthcare delivery globally because of its hazardous and infectious components that have potential for adverse health and environmental impacts. The paper introduces a set of indicators for assessing HCWM systems in hospitals. These indicators are: HCWM policies and standard operating procedures, management and oversight, logistics and budget support, training and occupational health and safety, and treatment, disposal and waste treatment equipment housing. By plotting a mark on a continuum which is defined as good and poor on the extremes and is connected with all other marks in a spoke arrangement, it’s possible to describe a baseline for HCWM in any specific hospital. This baseline can be used to improve awareness of the actors and policy-makers, compare the same hospital at a different point in time, to compare observations by different evaluators and to track improvements. Results suggest that in Kenya, the application of such indicators is useful for evaluating which priorities should be addressed to improve outcomes in HCWM systems. Systematic sampling technique was used to identify and collect data by use of observational checklist, interviews, visual verification and review of documents and a HCWM assessment tool. The objective is to suggest an integrated management tool as a method to identify prevailing problems with a HCWM system. The method can be replicated in other contexts worldwide, with a focus on the developing world. The integrated indicators focus on management of HCW and not its potential impact on human health and environment, an area recognized to be critical for future research.
文摘The present work dealt with the generation, purifying and liquefaction of biomethane to improve energy density using local materials for domestic applications. Cow dung was sourced at JKUAT dairy farm and experiments were conducted at JKUAT Bioenergy laboratory using biogas generated in laboratory scale 1 m<sup>3</sup> bioreactors. Experiments were done in triplicates and repeated under different conditions to get the optimal conditions. The results showed that enhanced cow dung substrate displayed an improved fermentation process with increased biogas yields. Purified biogas optimized methane content from 56% ± 0.18% for raw biogas to 95% ± 0.98% for biomethane which was ideal for liquefaction.
文摘Solanum aculeastrum, a member of the Solanum genus, has a rich history of traditional use in various populations throughout tropical and southern Africa for treating different ailments. This study investigated the antimicrobial activities of S. aculeastrum fruit extracts grown in Zambia against Escherichia coli (E. coli) (ATCC 25922), Staphylococcus aureus (S. aureus) (ATCC 25923), and Candida albicans (C. albicans). After being dried in an oven at 50°C for eight days, the fruits of S. aculeastrum were pulverised and subjected to extraction using methanol and distilled water as solvents. The resulting crude extract was then filtered, concentrated using a water bath, and packed into airtight containers separately. The disc diffusion method was employed to evaluate the antimicrobial activity of the extracts. The results showed that both the methanolic and aqueous extracts demonstrated antimicrobial activity against S. aureus and C. albicans, but not against E. coli. The zone of inhibition was greater in S. aureus than in C. albicans. This study highlights the importance of researching plants for their medicinal properties, which can pave the way for discovering natural product-based drugs with potential antimicrobial properties. These findings recommend further studies on plants used in African traditional medicine.
文摘Use of biomass in domestic cookstoves leads to the release of oxides of nitrogen (NO<sub>x</sub>), nitric oxide (NO), nitrogen dioxide (NO<sub>2</sub>), carbon monoxide (CO) and hydrocarbons C<sub>x</sub>H<sub>x</sub> that can be detrimental to health of the public and the environment. Attainment of complete combustion is the best strategy for mitigating the release of these emissions. This study sought to experimentally determine the effects of secondary air injection on the emission profiles of NO<sub>x</sub> (NO & NO<sub>2</sub>), CO and C<sub>x</sub>H<sub>x</sub> in a charcoal operated cookstove. Charcoal from Eucalyptus glandis was bought from Kakuzi PLC. Composites from three batches were analyzed for chemical composition and the stoichiometric air equivalent. Proximate analysis data show that the charcoal composed 58.72% ± 3.3% C, 15.95% ± 1.2% Volatile Matter, 4.69% ± 0.55% Moisture, 20.7% ± 0.8% Ash, High heat value (HHV) of 30.5 ± 1.1 and 29.3 ± 1.3 Low heat value (LHV) (MJ/kg) with a chemical formula of C<sub>18</sub>H<sub>2</sub>O and a stoichiometric air requirement of 5.28 ± 0.6 with a fuel flow rate of 1 kg fuel/hr. Emission profiles for CO and C<sub>x</sub>H<sub>x</sub> reduced significantly by 70% and 80% respectively with secondary air injection whereas those of NO<sub>x</sub> increased by between 15% and 20% for NO<sub>2</sub> and NO. The study reveals that secondary air injection has potential to mitigate on emission release, however other measures are required to mitigate NO<sub>x</sub> emissions.
文摘Climate change has grown more apparent in recent years with people becoming more aware of its potentially disastrous consequences. Flooding is one of the many consequences of a changing climate in Kenya known to cause immense devastation resulting in the loss of lives and property. This paper discusses the risk of flooding in Kenya as one of the many outcomes of climate change in the face of urgency to adapt Kenya’s built environment to flooding which is likely to continue to prevail in the decades as a result of the looming climate change. It also sought to evaluate the physical, traumatic, and psychological effects on communities affected by flood events. This cross-sectional survey, both qualitative and quantitative in nature, executed between 13<sup>th</sup> January 2021 and 14<sup>th</sup> July 2021 with 132 respondents along the western shoreline of Lake Baringo, near Marigat Town focused on the flood levels, structures, their materials, and quantities. Results show that the area covered by Lake Baringo increased by 18% from 236 km<sup>2</sup> to 278 km<sup>2</sup>. The depth of floods ranged from 0.3 m to 1.2 m and exceeded 1.6 m during heavy rainfall up to 3.2 m with homes completely submerged by the lake. Flooding was experienced more by residents living in low areas nearer to the shoreline of the lake as compared to those living on higher grounds. 100% of the structures didn’t have the architectural technology to withstand the impacts of flooding with 59% of housing made of corrugated iron sheets both on wall and roofing, 22% of mud houses roofed with either corrugated iron sheets, 10% being timber with thatch and only 8% stoned walled houses. This predisposed all the residents to the harmful impacts of flooding. Piled sandbags by locals as a mitigating measure proved inadequate to withstand the forces of the rising waters. Flood walls were built around local lodges near the lake but the rising water level quickly breached these defences. The study recommends that county and national governing authorities develop flood adaptation strategies for resilience. These include long-term land-use planning, the establishment of early warning systems, evacuation plans, identification of vulnerable or high-risk populations, measures to ensure water quality, sanitation, and hygiene. Flood-resilient architecture including stilt and floating houses that mechanically rise and fall with respect to the highest water mark are recommended during flood events. Bridges on swollen rivers and resilient construction materials like reinforced concrete are to be used for sustainable development for flood risk adaptation.
文摘The aim of this study is to examine the unsteady hydromagnetic flow of non-Newtonian nanofluid past a stretching sheet in the presence of variable magnetic field and chemical reaction. The system of non-linear partial differential equations governing the flow was solved using finite difference numerical approximation method. The resulting numerical schemes were simulated in MATLAB software. Furthermore, the skin-friction coefficient, Sherwood number, and Nusselt number have been presented in tabular form and discussed. The findings demonstrated that increasing Reynolds number increases velocity profiles while increasing permeability parameter, suction parameter and angle of inclination for the applied magnetic field reduces the velocity profiles of the fluid flow. Temperature of the fluid increases as the angle of inclination, magnetic number, Reynolds number and Eckert number increase but decreases as Prandtl number increases. Induced magnetic field profiles decrease as magnetic Prandtl number and suction parameter increase. Concentration profiles decrease as the chemical reaction parameter and Schmidt number increase but increase as the Soret number increases. The study is significant because fluid flow and heat transfer mechanisms with the variable magnetic considerations play an important role in magnetohydrodynamic generator or dynamo and magnetohydrodynamic pumps, nuclear reactors, vehicle thermal control, heat exchangers, cancer therapy, wound treatment and hyperthermia.
文摘Background: Urinary tract infection (UTI) is a bacterial infection affecting males and females but is more prevalent in expectant women. ESBLs are bacteria with enzymes that make them resistant to many antibiotics, posing a significant health challenge. This study aims to determine the characteristics of ESBL-producing bacteria causing UTIs in expectant women. Methodology: A self-administered survey was carried out;300 expectant women were recruited using a random sampling method. A questionnaire was used to collect socio-demographic information. Urine samples were collected in sterile universal bottles and processed at the JKUAT Zoology laboratory. Urine samples were analyzed using urinalysis, microscopy, culture, and sensitivity testing. ESBL-producing bacteria were identified phenotypically using the double-disc synergy test (DDST) and genotyped for specific resistant genes using PCR. Results: UTI prevalence was 32.7% (98/300). UTI was significantly associated with the history of previous UTI (OR = 0.84, p = 0.02) and multigravida (OR = 0.14 p = 0.01). UTI was common in women aged between 28-37 years in their second trimester. Bacteria isolated were E. coli 57.1% (56/98), S. aureus 21.4% (21/98) K. pneumonia 11.2% (11/98) and Proteus spp 10.4% (10/98). Bacteria antibiotic resistance patterns were E. coli-tetracycline (91.1%), sulfamethoxazole (55.4%), cefotaxime (53.4%) and augmentin (53.4%). S. aureus-sulfamethozaxole (100%) and augmentin (71.4%), K. pneumoniae-sulfame-thoxazole (72.2%) cefotaxime (63.6%), chloramphenicol and tetracycline (54.5%). Proteus spp: tetracycline (100%), nitrofurantoin (90%), cefotaxime and chloramphenicol (50%). The proportion of ESBLs bacterial producers was 37.6% (29/77) and 44.8% (13/29) possessed ESBLs resistant genes;Bla CTX-M 53.8% (7/13), Bla SHV and Bla TEM 23.1% (3/13) each, Bla OXA (0%) was not detected. Conclusion: The study revealed a high proportion of ESBLs producing bacteria responsible for UTI in expectant women. ESBLs screening, routine culture and sensitivity testing will guide on proper management and empirical treatment of UTI patients thus reducing multi-drug resistance.
